Subjects were divided into 1) a Study Group of subjects who underwent prompt PPV and 2) a Control Group of subjects in which PPV was deferred in favor of less invasive treatment options or observations. Both study and control subjects had a baseline Snellen VA of â¥ 20/50 and a PDR-associated macula-involving TRD of > 6 months duration with a minimum follow-up of 12-months.Results: There were 58 patients analyzed over an average follow-up period of 27.6 (Â± 7.1) months. The change in VA was similar in the Study Group compared to the Control Group (p=0.94) with both groups losing about three lines of VA during the study period (â 0.30 Â± 0.52 logMAR). Although the rates of maintaining â¥ 20/200 Snellen VA and â¥ 20/50 Snellen VA were similar in the Study Group compared to the Control Group (p=0.55 and p=0.28, respectively), the Study Group had more subjects gaining â¥ 2 lines of VA during the study period (p=0.002).Conclusion: Patients presenting with good VA and a PDR-associated macula-involving TRD of > 6 months were more likely to gain â¥ 2 lines of VA when PPV was performed at baseline compared to PPV deferral until further deterioration occurred.Keywords: chronic tractional retinal detachment, diabetic vitrectomy, good visu

3. Glutathione is required for growth and cadmium tolerance in the amphibian chytrid fungus, Batrachochytrium dendrobatidis

Abstract: Batrachochytrium dendrobatidis (Bd) is a lethal amphibian pathogen, partly due to its ability to evade the immune system of susceptible frog species. In many pathogenic fungi, the antioxidant glutathione is a virulence factor that helps neutralise oxidative stressors generated from host immune cells, as well as other environmental stressors such as heavy metals. The role of glutathione in stress tolerance in Bd has not been investigated. Here, we examine the changes in the glutathione pool after stress exposure and quantify the effect of glutathione depletion on cell growth and stress tolerance. Depletion of glutathione repressed growth and release of zoospores, suggesting that glutathione is essential for life cycle completion in Bd. Supplementation with <2 mM exogenous glutathione accelerated zoospore development, but concentrations >2 mM were strongly inhibitory to Bd cells. While hydrogen peroxide exposure lowered the total cellular glutathione levels by 42 %, glutathione depletion did not increase the sensitivity to hydrogen peroxide. Exposure to cadmium increased total cellular glutathione levels by 93 %. Glutathione-depleted cells were more sensitive to cadmium, and this effect was attenuated by glutathione supplementation, suggesting that glutathione plays an important role in cadmium tolerance. The effects of heat and salt were exacerbated by the addition of exogenous glutathione. The impact of glutathione levels on Bd stress sensitivity may help explain differences in host susceptibility to chytridiomycosis and may provide opportunities for synergistic therapeutics.

Abstract: In statistics, samples are drawn from a population in a data-generating process (DGP). Standard errors measure the uncertainty in estimates of population parameters. In science, evidence is generated to test hypotheses in an evidence-generating process (EGP). We claim that EGP variation across researchers adds uncertainty—nonstandard errors (NSEs). We study NSEs by letting 164 teams test the same hypotheses on the same data. NSEs turn out to be sizable, but smaller for more reproducible or higher rated research. Adding peer-review stages reduces NSEs. We further find that this type of uncertainty is underestimated by participants.

Abstract: OBJECTIVE: Trauma history is associated with SLE onset and worse patient-reported outcomes; perceived stress is associated with greater SLE disease activity. Stress perceptions vary in response to life events and may be influenced by psychosocial factors. In an SLE cohort, we examined whether stressful events associated with perceived stress, whether psychosocial factors affected perceived stress, and whether these relationships varied by prior trauma exposure. METHODS: This is a cross-sectional analysis of data from the California Lupus Epidemiology Study, an adult SLE cohort. Multivariable linear regression analyses controlling for age, gender, educational attainment, income, SLE damage, comorbid conditions, glucocorticoids ≥7.5 mg/day and depression examined associations of recent stressful events (Life Events Inventory) and positive (resilience, self-efficacy, emotional support) and negative (social isolation) psychosocial factors with perceived stress. Analyses were stratified by lifetime trauma history (Brief Trauma Questionnaire (BTQ)) and by adverse childhood experiences (ACEs) in a subset. RESULTS: Among 242 individuals with SLE, a greater number of recent stressful events was associated with greater perceived stress (beta (95% CI)=0.20 (0.07 to 0.33), p=0.003). Positive psychosocial factor score representing resilience, self-efficacy and emotional support was associated with lower perceived stress when accounting for number of stressful events (-0.67 (-0.94 to -0.40), p<0.0001); social isolation was associated with higher stress (0.20 (0.14 to 0.25), p<0.0001). In analyses stratified by BTQ trauma and ACEs, associations of psychosocial factors and perceived stress were similar between groups. However, the number of recent stressful events was significantly associated with perceived stress only for people with BTQ trauma (0.17 (0.05 to 0.29), p=0.0077) and ACEs (0.37 (0.15 to 0.58), p=0.0011). Abstract: The ocean’s methane emission to the atmosphere is dominated by continental shelves where cold seeps are globally common features. Seeps emit methane into the hydrosphere, but temporal variations and controls of seep activity and the efficiency of the microbial methane filter in the water column are scarce. Here we address these knowledge gaps by measuring whole water column methane inventories and methanotrophic activity at a temporal resolution of 2 hours at a North Sea cold seep (Doggerbank) in summer and autumn. We found that bottom water methane inventories were 68% (summer) and 11% (autumn) higher during low tide compared to high tide coinciding with increased methanotrophic activity. The activity of methanotrophs was reduced during autumn when the water column was fully mixed and matched by higher methane emissions to the atmosphere. Our results show that tides are underappreciated controls on seepage and methanotrophic activity and methane sea–atmosphere fluxes.

Abstract: In statistics, samples are drawn from a population in a data-generating process (DGP). Standard errors measure the uncertainty in estimates of population parameters. In science, evidence is generated to test hypotheses in an evidence-generating process (EGP). We claim that EGP variation across researchers adds uncertainty—nonstandard errors (NSEs). We study NSEs by letting 164 teams test the same hypotheses on the same data. NSEs turn out to be sizable, but smaller for more reproducible or higher rated research. Adding peer-review stages reduces NSEs. We further find that this type of uncertainty is underestimated by participants.

Abstract: This article describes the rationale, aims, and methodology of the Accelerating Medicines Partnership® Schizophrenia (AMP® SCZ). This is the largest international collaboration to date that will develop algorithms to predict trajectories and outcomes of individuals at clinical high risk (CHR) for psychosis and to advance the development and use of novel pharmacological interventions for CHR individuals. We present a description of the participating research networks and the data processing analysis and coordination center, their processes for data harmonization across 43 sites from 13 participating countries (recruitment across North America, Australia, Europe, Asia, and South America), data flow and quality assessment processes, data analyses, and the transfer of data to the National Institute of Mental Health (NIMH) Data Archive (NDA) for use by the research community. In an expected sample of approximately 2000 CHR individuals and 640 matched healthy controls, AMP SCZ will collect clinical, environmental, and cognitive data along with multimodal biomarkers, including neuroimaging, electrophysiology, fluid biospecimens, speech and facial expression samples, novel measures derived from digital health technologies including smartphone-based daily surveys, and passive sensing as well as actigraphy. The study will investigate a range of clinical outcomes over a 2-year period, including transition to psychosis, remission or persistence of CHR status, attenuated positive symptoms, persistent negative symptoms, mood and anxiety symptoms, and psychosocial functioning. The global reach of AMP SCZ and its harmonized innovative methods promise to catalyze the development of new treatments to address critical unmet clinical and public health needs in CHR individuals.

Abstract: ABSTRACTIn statistics, samples are drawn from a population in a data‐generating process (DGP). Standard errors measure the uncertainty in estimates of population parameters. In science, evidence is generated to test hypotheses in an evidence‐generating process (EGP). We claim that EGP variation across researchers adds uncertainty—nonstandard errors (NSEs). We study NSEs by letting 164 teams test the same hypotheses on the same data. NSEs turn out to be sizable, but smaller for more reproducible or higher rated research. Adding peer‐review stages reduces NSEs. We further find that this type of uncertainty is underestimated by participants.

12. Thermodynamic Formalism for Iterated Function Systems

Abstract: The purpose of this thesis to present two works in the field of thermodynamic formalism. The first concerns multifractal decompositions based on Birkhoff averages for expanding interval maps with countably many branches which we assume can be coded by topologically mixing countable Markov shifts. This generalises previous work on expanding maps with finitely many branches, and expanding maps with countably many branches where the coding is assumed to be the full shift. In the second, we prove the existence of Gibbs-type measures for some superadditive sequences of matrix-norm potentials defined on subshifts of finite type. This extends the results of the well-understood subadditive setting.

Abstract: This article describes the rationale, aims, and methodology of the Accelerating Medicines Partnership® Schizophrenia (AMP® SCZ). This is the largest international collaboration to date that will develop algorithms to predict trajectories and outcomes of individuals at clinical high risk (CHR) for psychosis and to advance the development and use of novel pharmacological interventions for CHR individuals. We present a description of the participating research networks and the data processing analysis and coordination center, their processes for data harmonization across 43 sites from 13 participating countries (recruitment across North America, Australia, Europe, Asia, and South America), data flow and quality assessment processes, data analyses, and the transfer of data to the National Institute of Mental Health (NIMH) Data Archive (NDA) for use by the research community. In an expected sample of approximately 2000 CHR individuals and 640 matched healthy controls, AMP SCZ will collect clinical, environmental, and cognitive data along with multimodal biomarkers, including neuroimaging, electrophysiology, fluid biospecimens, speech and facial expression samples, novel measures derived from digital health technologies including smartphone-based daily surveys, and passive sensing as well as actigraphy. The study will investigate a range of clinical outcomes over a 2-year period, including transition to psychosis, remission or persistence of CHR status, attenuated positive symptoms, persistent negative symptoms, mood and anxiety symptoms, and psychosocial functioning. An accurate assessment of depressive symptomology is crucial for clinical management and research. This study assessed the convergent validity, reliability, and total scale score interconversion across the 9-item Patient Health Questionnaire (PHQ-9) self-report, the 16-item Quick Inventory of Depressive Symptomatology-clinician report (QIDS-C) (two widely used clinical ratings) and the 5-item Very Brief Quick Inventory of Depressive Symptoms-clinician report (VQIDS-C), which evaluate the core features of MDD.Patients and Methods: This study leveraged electronic health record (EHR)-derived, de-identified data from the NeuroBlu Database (Version 23R1), a longitudinal behavioural health real-world platform. Classical Test Theory (CTT) and Item Response Theory (IRT) analyses were used to evaluate the reliability, validity of, and conversions between the scales. The Test Information Function (TIF) was calculated for each scale, with greater test information reflecting higher precision and reliability in measuring depressive symptomology. IRT was also used to generate conversion tables so that total scores on each scale could be compared to the other.Results: The study sample (n = 2,156) had an average age of 36.4 years (standard deviation [SD] = 13.0) and 59.7%

Abstract: This article describes the rationale, aims, and methodology of the Accelerating Medicines Partnership® Schizophrenia (AMP® SCZ). This is the largest international collaboration to date that will develop algorithms to predict trajectories and outcomes of individuals at clinical high risk (CHR) for psychosis and to advance the development and use of novel pharmacological interventions for CHR individuals. We present a description of the participating research networks and the data processing analysis and coordination center, their processes for data harmonization across 43 sites from 13 participating countries (recruitment across North America, Australia, Europe, Asia, and South America), data flow and quality assessment processes, data analyses, and the transfer of data to the National Institute of Mental Health (NIMH) Data Archive (NDA) for use by the research community. In an expected sample of approximately 2000 CHR individuals and 640 matched healthy controls, AMP SCZ will collect clinical, environmental, and cognitive data along with multimodal biomarkers, including neuroimaging, electrophysiology, fluid biospecimens, speech and facial expression samples, novel measures derived from digital health technologies including smartphone-based daily surveys, and passive sensing as well as actigraphy. The study will investigate a range of clinical outcomes over a 2-year period, including transition to psychosis, remission or persistence of CHR status, attenuated positive symptoms, persistent negative symptoms, mood and anxiety symptoms, and psychosocial functioning. 18. FAX: Scalable and Differentiable Federated Primitives in JAX

Abstract: We present FAX, a JAX-based library designed to support large-scale distributed and federated computations in both data center and cross-device applications. FAX leverages JAX's sharding mechanisms to enable native targeting of TPUs and state-of-the-art JAX runtimes, including Pathways. FAX embeds building blocks for federated computations as primitives in JAX. This enables three key benefits. First, FAX computations can be translated to XLA HLO. Second, FAX provides a full implementation of federated automatic differentiation, greatly simplifying the expression of federated computations. Last, FAX computations can be interpreted out to existing production cross-device federated compute systems. We show that FAX provides an easily programmable, performant, and scalable framework for federated computations in the data center. 19. MambaByte: Token-free Selective State Space Model

Abstract: Token-free language models learn directly from raw bytes and remove the inductive bias of subword tokenization. Operating on bytes, however, results in significantly longer sequences. In this setting, standard autoregressive Transformers scale poorly as the effective memory required grows with sequence length. The recent development of the Mamba state space model (SSM) offers an appealing alternative approach with a fixed-sized memory state and efficient decoding. We propose MambaByte, a token-free adaptation of the Mamba SSM trained autoregressively on byte sequences. In terms of modeling, we show MambaByte to be competitive with, and even to outperform, state-of-the-art subword Transformers on language modeling tasks while maintaining the benefits of token-free language models, such as robustness to noise. In terms of efficiency, we develop an adaptation of speculative decoding with tokenized drafting and byte-level verification. This results in a $2.6\times$ inference speedup to the standard MambaByte implementation, showing similar decoding efficiency as the subword Mamba. These findings establish the viability of SSMs in enabling token-free language modeling.

Abstract: Reducing serving cost and latency is a fundamental concern for the deployment of language models (LMs) in business applications. To address this, cascades of LMs offer an effective solution that conditionally employ smaller models for simpler queries. Cascaded systems are typically built with independently trained models, neglecting the advantages of considering inference-time interactions of the cascaded LMs during training. In this paper, we present cascade-aware training(CAT), an approach to optimizing the overall quality-cost performance tradeoff of a cascade of LMs. We achieve inference-time benefits by training the small LM with awareness of its place in a cascade and downstream capabilities. Abstract: Entity disambiguation (ED), which links the mentions of ambiguous entities to their referent entities in a knowledge base, serves as a core component in entity linking (EL). Existing generative approaches demonstrate improved accuracy compared to classification approaches under the standardized ZELDA benchmark. Nevertheless, generative approaches suffer from the need for large-scale pre-training and inefficient generation. Most importantly, entity descriptions, which could contain crucial information to distinguish similar entities from each other, are often overlooked. We propose an encoder-decoder model to disambiguate entities with more detailed entity descriptions. Given text and candidate entities, the encoder learns interactions between the text and each candidate entity, producing representations for each entity candidate. The decoder then fuses the representations of entity candidates together and selects the correct entity. Our experiments, conducted on various entity disambiguation benchmarks, demonstrate the strong and robust performance of this model, particularly +1.5% in the ZELDA benchmark compared with GENRE. 23. The Majority Rules: The Origins of Voter ID Laws and Their Role in Electoral Strategy Today

Abstract: Some states implement voting laws that make it harder to vote. Today, despite evidence that voter impersonation is rare, 34 states have implemented some form of voter ID. I ask: 1) Where do restrictive voting policies come from? 2) Why do state legislators pass restrictive voting policy? What conditions make legislators more likely to sponsor and support these policies? Few studies examine what factors contribute to state legislator support for restrictive voting requirements. I investigate how demographic threat impacts state legislator support for voter identification legislation. I argue that voter identification laws are elite-driven devices that legislators use to retain political power when changing demographics disfavor them and pose an existential threat to their electoral viability, particularly when it is unfeasible for legislators to successfully win support from racial minority voters. The restrictive voting laws prior to the civil rights movement were explicitly racially discriminatory, but in order to overcome legal challenges restrictive voting laws today are written to be race neutral and to produce a racially discriminatory effect. This study takes a holistic approach, examining legislator intent behind voter identification laws, by using historical context around restrictive voting laws to establish the link between the restrictive voting laws of the past and those of today in the literature. First, I examine archived sources to understand how voter identification laws developed from other discriminatory tests and devices. Findings indicate that early voter identification laws were implemented in places where large nonwhite populations enjoyed strong political organization and power, and modern voter identification laws are adopted similar contexts. The main study outcome was the change in the Global Vision Satisfaction Index from the PROWL questionnaire.Results: Forty-six patients underwent treatment and completed the modified PROWL questionnaire before and 26-weeks after TG-LASIK. The Global Vision Satisfaction Index from the modified PROWL questionnaire improved from 4.07 (3.87â 4.26) to 5.00 (4.81â 5.19) after the TG-LASIK treatment (p < 0.0001). The study populationâs binocular uncorrected distance visual acuity was 20/16, 20/12.5, and 20/10 or better in 100%, 87.0%, and 15.2% at 26 weeks post TG-LASIK, respectively.Conclusion: Patient satisfaction as assessed with the modified PROWL questionnaire is very high after undergoing TG-LASIK using Phorcides analytic software. Subjects were classified into one of two possible study groups according to the timing of their initial postoperative cataract surgery examination: Group A received the initial postoperative cataract surgery examination on the same day as the surgery, whereas Group B received the initial postoperative cataract surgery examination on the day following the operation.Results: There were 80 subjects contacted for the phone survey of which 70 (35 in each study group) completed the survey and therefore were included in the analysis. Group A subjects responded favorably in regards to preference and realizing reduced time and cost savings compared to Group B (p < 0.0001 for both). There were no subjects in Group A who would have preferred next-day initial postoperative care, whereas 31.4% of subjects in Group B would have preferred same-day care if given the opportunity.Conclusion: Patients undergoing cataract surgery both prefer and report time and cost savings with same-day initial postoperative care compared to next-day initial postoperative care. Patient preferences regarding their postoperative care should be one of the many factors that a surgeon ought to take into consideration when providing follow-up care after cataract surgery.Keywords: postoperative cataract

Enrolled patients were prospectively administered a standardized Dry Eye Symptom Index survey (analog score of 1 to 5 with 5 being the worst) prior to treatment and at 6-months after FS-LASIK. The following objective measurements were also recorded: objective scatter index (OSI), tear film osmolarity (TFO), and automated tear break-up times (TBUT).Results: There were 40 enrolled patients who underwent bilateral myopic FS-LASIK and completed the 6-month study period. The Dry Eye Symptom Index score improved from 2.3 (2.0â 2.6, 95% Confidence Intervals) prior to treatment to 1.3 (1.0â 1.5) at 6 months (p < 0.0001). Subset analysis of the subjective dry eye symptoms showed improvement in âgrittinessâ (p = 0.001) but not in âlight sensitivityâ or âsorenessâ (p = 0.13 and p = 0.24, respectively). There were no significant changes in the OSI, TFO, or TBUT measurements at 6 months (p > 0.05 for all), and there were no adverse events or complications during the study period.Conclusion: Patient-reported dry eye symptoms improve after 6 months following myopic FS-LASIK. For both the problems of detecting and recovering community structure, an interesting landscape of statistical and computational phase transitions has emerged. A natural unanswered question is: might it be possible to infer properties of the community structure (for instance, the number and sizes of communities) even in situations where actually finding those communities is believed to be computationally hard? We show the answer is no. In particular, we consider certain hypothesis testing problems between models with different community structures, and we show (in the low-degree polynomial framework) that testing between two options is as hard as finding the communities. In addition, our methods give the first computational lower bounds for testing between two different "planted" distributions, whereas previous results have considered testing between a planted distribution and an i.i.d. "null" distribution.

29. Comparative Regional Hydrologic Changes During the Early Eocene

Abstract: The early Eocene is punctuated by a series of rapid warming events, known as hyperthermals. The first among these, the Paleocene-Eocene Thermal Maximum, or PETM, has been argued to be one of the closest approximations to modern-day, anthropogenic warming in the geologic record. The PETM, and the subsequent hyperthermals of smaller magnitude, such as Eocene Thermal Maximum 2, or ETM2, can provide insight into our future warmer world. In addition to temperature changes, these hyperthermals drove significant changes in regional precipitation patterns. This study seeks to better understand these changes, focusing on the mid-latitude North Atlantic coasts, utilizing a suite of techniques including high-resolution climate models and geochemical, sedimentological, and paleoecological proxies. Our models show a significant increase in the seasonality and extremes of precipitation in response to elevated CO2 levels, with significant differences regionally depending upon orbital configuration. In the western Mid-Atlantic we find a dramatic weathering response to these precipitation changes, with an increase both in chemical alteration and physical transport of sediment. During ETM2, in contrast to studies in other regions, the climatic response was much more muted, with temperatures decreasing, a moderate increase in chemical weathering, and little response in the marine palynology, highlighting the importance climatic forcings apart from CO2 such as orbital forcing and dynamic feedbacks.

30. Landscape and Experience in Late Antique Gaul

Abstract: Landscape descriptions are textual products of subjective experiences in a landscape. In this dissertation I make use of both literary landscape descriptions for the purpose of understanding both what people in late antique Gaul thought about their landscapes and how they experienced and interacted with their landscapes. I propose that the landscape within which an author lived and wrote shapes the author’s experiences by imposing natural limits on the author’s actions and thus the author’s written works. There are three elements in this proposal: 1) landscape; 2) a person, i.e., the author; 3) the written works produced by the author. The physical properties of the landscape define the range of possible human action. The physical properties also impose limitations. Therefore, landscape shapes and constrains the human activity within it. Historical landscapes are accessible to modern historians through the study of modern topography, archeology, and, to a lesser extent, paleoclimate data. This body of archeological and scientific knowledge provides insight into the physical world inhabited by late antique authors, which allows a partial reconstruction of historic landscapes that shaped the lives of late antique authors. In this dissertation, one author and his relationship to his landscape is analyzed in each chapter, including Palladius, Sidonius Apollinaris, Avitus of Vienne, and the anonymous hagiographer of the Life of the Jura Fathers. By identifying the natural limits set by a local landscape on each author and how the author responded to those limits, it is possible to determine how each author mentally organized his landscape. Therefore, for each author analyzed I answer two questions: First, how did an author’s landscape impact the author’s literary works through the author’s experience in it? Second, how did each author mentally organize and interpret his landscape? The answers to these questions tell us how an author thought about and interpreted the land

Subjects were divided into Study and Control groups. Both Study and Control subjects had undergone â¥ 6 IVA treatments during the previous 12 months, â¥ 4 IVA treatments during the previous 6 months, had a central macular thickness (CMT) on optical coherence tomography (OCT) of â¥ 300 microns, and had observable intraretinal and/or subretinal fluid on OCT prior to group assignment. Study subjects were switched from IVA to IVF and received 3 treatments within 4 months. Control subjects remained on IVA during the same time period and received 3 treatments within 4 months.Results: There were a total of 55 subjects analyzed. There were 39.3% (11/28) in the Study Group and 7.4% (2/27) in the Control Group attaining a CMT of less than 300 microns without retinal fluid on OCT at the end of the 4-month study period (p = 0.004). There were 35.7% (10/28) in the Study Group and 7.4% (2/27) in the Control Group gaining 2 or more lines of visual acuity at the end of the 4-month study period (p = 0.008).Conclusion: IVF can improve the visual and anatomic outcomes in a significant minority of treatment-resistant nAMD subjects previously managed with IVA. Study patients were switched to IVF and underwent three injections within 4 months, whereas control patients stayed on IVA during the same period and received three injections within 4 months.Results: There were 51 patients analyzed. There were 37.5% (9/24) in the study group and 3.7% (1/27) in the control group who achieved a CMT of less than 300 Î¼m without retinal fluid on OCT at the end of the 4-month study (p=0.001). There were 41.7% (10/24) in the study group and 11.1% (3/27) in the control group who had gained two or more lines of visual acuity at the end of the 4-month study (p=0.01).Conclusion: For a significant minority, IVF can improve the short-term visual and anatomic outcomes in treatment-resistant DME patients formerly managed with IVA. A standardized phone survey assessing patient perspectives on ISBCS was administered to patients before and after routine delayed sequential bilateral cataract surgery (DSBCS) was performed. Subject responses were analyzed, including a subset analysis on patient responses under a variety of circumstances.Results: There were 61 patients enrolled into the study and 47 completed the surveys before and after routine DSBCS (77.0% completion rate). Thirty-nine (83.0%) of respondents had a favorable outlook of ISBCS preoperatively, whereas 36 (76.6%) had a favorable outlook of ISBCS postoperatively (p> 0.05). On the postoperative questionnaire, twenty-five (53.2%) of respondents were willing to accept additional surgical risk if necessary to receive ISBCS, and this finding was significant between the subgroup of patients with systemic health co-morbidities compared to those without systemic health co-morbidities (p=0.05).Conclusion: Most patients had a favorable outlook of ISBCS before and after undergoing DSBCS. Patients with underlying systemic health co-morbidities were most likely to accept additional surgical risk if necessary to receive ISBCS. Abstract: Introduction.Validly and reliably assessing conceptual change is essential for evaluating the effectiveness of pain science education for physiotherapy students. We aimed to 1) evaluate concept of pain before and after a 14-week pain science education university subject, 2) assess structural validity of the Concept of Pain Inventory for Adults (COPI-Adult) in postgraduate entry-level physiotherapy students, and 3) explore possible relationships between baseline variables and baseline COPI-Adult scores.Review of Literature.As the COPI-Adult is a newly developed questionnaire, there is a lack of data regarding its psychometric properties.Subjects.Of 129 enrolled students at an Australian university, 124 (96%) and 114 (88%) completed the baseline and follow-up questionnaires, respectively.Methods.In this prospective cohort study, students who commenced the degree in 2020 or 2021 completed online questionnaires, including the COPI-Adult, at the start and end of their first semester. This semester included a 14-week pain science education subject and other physiotherapy-related subjects. Higher COPI-Adult scores (range = 0-52) indicate better alignment with contemporary pain science. We 1) compared differences in concept of pain before and after the semester, 2) performed a confirmatory factor analysis on the COPI-Adult, and 3) performed exploratory regression analyses.Results.Concept of Pain Inventory for Adults scores increased from baseline (median [interquartile range]: 39 [36-44]) to follow-up (48 [44-51]). The COPI-Adult retained its 1-factor structure, with acceptable internal consistency (Cronbach's alpha = 0.80). Exploratory analysis showed that previously completing a subject on pain was related to higher COPI-Adult baseline scores. Abstract: The eastern Mediterranean Sea (EMS) sedimentary record is periodically interspersed with organic-rich ‘sapropel’ layers. Sapropels are characteristic of basin-wide anoxic events, triggered by precession-forced insolation maxima. Relatively subdued insolation maxima, however, are not always expressed as distinct sapropel events. The EMS sedimentary record is thus useful to investigate feedbacks between marine anoxia and the nitrogen (N) cycle and offers an analogue for modern deoxygenation and past oceanic anoxic events. To this end, we investigated a ∼68 kyr sedimentary record from the EMS containing the well-established sapropel S1 (deposited in two phases: S1a [∼10.5–8.5 ka BP] and S1b [∼7.8–6.1 ka BP]) and sediments timed to the ambiguous S2 sapropel (∼53 ka BP). We used lipid biomarkers of microorganisms to reconstruct key N-cycle components: (1) anaerobic ammonium oxidation (anammox) using ladderanes and a stereoisomer of bacteriohopanetetrol (BHT-x), (2) dinitrogen gas (N2) fixation using heterocyte glycolipids, and (3) nitrification by Thaumarchaeota using crenarchaeol. Additionally, benthic foraminifera and trace metals (U, Mo, Mn) were used to reconstruct redox conditions. During S1a, abundances of Thaumarchaeota increased, likely promoted by elevated high-nutrient freshwater discharge. At this time, a combination of phosphorus supply and intensified loss of bioavailable N via water column anammox, may have reinforced anoxia by favoring diatom-diazotroph associations. During S1b, anammox is equally intense. Yet, no positive feedback on N2-fixation is observed, likely because diazotrophs were phosphorus limited. Instead, anammox may have provided negative feedback on anoxia by quenching primary production. Ladderanes suggest additional episodes of anammox between ∼69 to 39 cal ka BP, corresponding to brief periods of water column deoxygenation. In this study, we aim to further identify BHPs related to ecological niches and/or specific bacteria by characterizing the distribution of BHPs in suspended particulate matter (SPM) of the water column and in sediments in a range of lakes and coastal lagoons from the Azores Archipelago, as well as in a co-culture enriched for methanotrophs. Sediment samples from Azorean lakes with low-oxygen conditions during the summer months (i.e., Azul, Verde, Funda, and Negra) contain relatively high abundances of BHPs that are typically associated with methane-oxidizing (methanotrophic) bacteria (i.e., aminotetrol, aminopentol, and methylcarbamate-aminopentol), as well as the ethenolamine-BHPs (i.e., ethenolamine-BHpentol and ethenolamine-BHhexol) and the N-formylated aminoBHPs. Both ethenolamine-BHPs and N-formylated aminoBHPs were also detected in a methanotroph–methylotroph co-culture that was enriched from a lake. In the SPM of all water columns, bacteriohopanetetrol (BHT), BHT cyclitol ether, and aminotriol are the dominant BHPs. In SPM from Lake Funda, nucleoside BHPs (i.e., Me-adenosylhopaneHG-diMe (where HG refers to head group), N1-methylinosylhopane, 2Me-N1-inosylhopane, and Me-N1-inosylhopane) are present in low abundance or absent under oxic conditions but increase in concentration near the chemocline, suggesting potential in situ production of these nucleoside BHPs rather than an allochthonous origin. In contrast, sediments from shallow, well-mixed lakes (i.e., Empadadas, São Jorge, and Lomba) contain higher abundances of adenosylhopane and N1-methylinosylhopane, which likely originate from bacteria living in nearby soils. Based on our current results we revised the existing Rsoil index, which was previously used to infer terrestrial inputs to aquatic environments, to exclude any potential nucleosides produced in the lake water column (Rsoil-lake). Abstract: The present study examines change in reports of daily, weekly, and monthly psychological distress over 20 years, and of negative and positive affect over 10 years, using data from the Midlife in the United States study. The study includes three waves of data collection on adults ranging from 22 to 95 years old. Cross-sectional findings reveal that older age is related to lower levels of psychological distress and negative affect and to higher levels of positive affect across each successive age group. Yet, longitudinal findings vary across younger, middle-aged, and older adults. Psychological distress decreases over time among younger adults (although only until age 33 for weekly reports), remains stable in midlife, and is stable (monthly) or slightly increases (daily and weekly) among older adults. For negative affect, levels decrease over time for younger and middle-aged adults, and only increase for the oldest adults for daily and monthly affect. Positive affect is stable over time among younger adults, but decreases in midlife starting in the mid-fifties. In conclusion, overall patterns of findings suggest that being old (assessed cross-sectionally) is related to higher levels of emotional well-being. Growing old (assessed longitudinally) is related to improvements in emotional well-being across younger and early middle adulthood, which mirrors cross-sectional findings. 42. Longitudinal change in daily stress across 20 years of adulthood: Results from the national study of daily experiences.

Abstract: This study examined age-related patterns in exposure and affective reactivity to daily stressors across a 20-year time span among adults who were between 22 and 77 years old at their baseline interview. Longitudinal data from the National Study of Daily Experiences (NSDE) consisted of three bursts of eight consecutive nightly interviews of stress and affect. Analyses made use of all available data from a U.S. National sample of respondents who participated in any of the three NSDE bursts (N = 2,845; number of daily assessments = 33,688). Findings revealed increasing age-related benefits. Younger adults (< 30 years) reported the highest levels of stressor exposure and reactivity, but their stress profile improved with age. Over time, adults averaged an 11% reduction in the occurrence of stressor days, and the younger adults exhibited an even steeper decline (a 47% reduction) in their levels of stressor reactivity. For people in midlife and old age, stressor occurrence continued to decrease over time, yet among adults aged 54 years or older at baseline, stress reactivity remained stable across time. Abstract: 01 juli 2023, Item does not contain fulltext, Jansen-de Vries syndrome (JdVS) is a neurodevelopmental condition attributed to pathogenic variants in Exons 5 and 6 of PPM1D. As the full phenotypic spectrum and natural history remain to be defined, we describe a large cohort of children and adults with JdVS. This is a retrospective cohort study of 37 individuals from 34 families with disease-causing variants in PPM1D leading to JdVS. Clinical data were provided by treating physicians and/or families. Of the 37 individuals, 27 were male and 10 female, with median age 8.75 years (range 8 months to 62 years). Four families document autosomal dominant transmission, and 32/34 probands were diagnosed via exome sequencing. The facial gestalt, including a broad forehead and broad mouth with a thin and tented upper lip, was most recognizable between 18 and 48 months of age. Common manifestations included global developmental delay (35/36, 97%), hypotonia (25/34, 74%), short stature (14/33, 42%), constipation (22/31, 71%), and cyclic vomiting (6/35, 17%). Distinctive personality traits include a hypersocial affect (21/31, 68%) and moderate-to-severe anxiety (18/28, 64%). In conclusion, JdVS is a clinically recognizable neurodevelopmental syndrome with a characteristic personality and distinctive facial features. The association of pathogenic variants in PPM1D with cyclic vomiting bears not only medical attention but also further pathogenic and mechanistic evaluation.

Standard errors measure the uncertainty in sample estimates of population parameters. In science, evidence is generated to test hypotheses in an evidence-generating process (EGP). We claim that EGP variation across researchers adds uncertainty: non-standard errors. To study them, we let 164 teams test six hypotheses on the same sample. We find that non-standard errors are sizeable, on par with standard errors. Their size (i) co-varies only weakly with team merits, reproducibility, or peer rating, (ii) declines significantly after peer-feedback, and (iii) is underestimated by participants.

Abstract: Due to socio-economic and climatic changes around the world, large outdoor fires in the built environment have become one of the global issues that threaten billions of people. The devastating effects of them are indicative of weaknesses in existing building codes and standard testing methodologies. This is due in part to our limited understanding of large outdoor fire exposures, including the ones from wildland to communities and within communities. To address this problem, the Ignition Resistance Committee (IRC) of the International Association of the Fire Safety Science working group ‘Large Outdoor Fires and the Built Environment’ was established. This manuscript is the result of one of the IRC's initiatives to review current knowledge on exposures associated with large outdoor fires, identify existing knowledge gaps, and provide recommendations for future research. The article consists of two sections: the wildland fire exposure to the built environment and the settlement fire exposure to structures. Each section presents a comprehensive review of experimental and numerical studies of exposure mechanisms (flame contact and convection, radiation, and firebrands). Abstract: Due to socio-economic and climatic changes around the world, large outdoor fires in the built environment have become one of the global issues that threaten billions of people. The devastating effects of them are indicative of weaknesses in existing building codes and standard testing methodologies. This is due in part to our limited understanding of large outdoor fire exposures, including the ones from wildland to communities and within communities. To address this problem, the Ignition Resistance Committee (IRC) of the International Association of the Fire Safety Science working group ‘Large Outdoor Fires and the Built Environment’ was established. This manuscript is the result of one of the IRC's initiatives to review current knowledge on exposures associated with large outdoor fires, identify existing knowledge gaps, and provide recommendations for future research. The article consists of two sections: the wildland fire exposure to the built environment and the settlement fire exposure to structures. Each section presents a comprehensive review of experimental and numerical studies of exposure mechanisms (flame contact and convection, radiation, and firebrands). The review concludes with a discussion on data consistency and existing knowledge gaps to highlight future directions for each of the three fire exposure mechanisms.

48. Marine nitrogen cycling dynamics under altering redox conditions: Insights from deposition of sapropels S1 and the ambiguous S2 in the Eastern Mediterranean Sea

Abstract: The eastern Mediterranean Sea (EMS) sedimentary record is periodically interspersed with organic-rich ‘sapropel’ layers. Sapropels are characteristic of basin-wide anoxic events, triggered by precession-forced insolation maxima. Relatively subdued insolation maxima, however, are not always expressed as distinct sapropel events. The EMS sedimentary record is thus useful to investigate feedbacks between marine anoxia and the nitrogen (N) cycle and offers an analogue for modern deoxygenation and past oceanic anoxic events. To this end, we investigated a ∼68 kyr sedimentary record from the EMS containing the well-established sapropel S1 (deposited in two phases: S1a [∼10.5–8.5 ka BP] and S1b [∼7.8–6.1 ka BP]) and sediments timed to the ambiguous S2 sapropel (∼53 ka BP). We used lipid biomarkers of microorganisms to reconstruct key N-cycle components: (1) anaerobic ammonium oxidation (anammox) using ladderanes and a stereoisomer of bacteriohopanetetrol (BHT-x), (2) dinitrogen gas (N2) fixation using heterocyte glycolipids, and (3) nitrification by Thaumarchaeota using crenarchaeol. Additionally, benthic foraminifera and trace metals (U, Mo, Mn) were used to reconstruct redox conditions. During S1a, abundances of Thaumarchaeota increased, likely promoted by elevated high-nutrient freshwater discharge. At this time, a combination of phosphorus supply and intensified loss of bioavailable N via water column anammox, may have reinforced anoxia by favoring diatom-diazotroph associations. During S1b, anammox is equally intense. Yet, no positive feedback on N2-fixation is observed, likely because diazotrophs were phosphorus limited. Instead, anammox may have provided negative feedback on anoxia by quenching primary production. Ladderanes suggest additional episodes of anammox between ∼69 to 39 cal ka BP, corresponding to brief periods of water column deoxygenation. Abstract: ContextHypophysitis is a known immune-related adverse event (irAE) of immune checkpoint inhibitors (CPIs), commonly associated with CTLA-4 inhibitors and less often with PD-1/PD-L1 inhibitors.ObjectiveWe aimed to determine clinical, imaging, and HLA characteristics of CPI-induced hypophysitis (CPI-hypophysitis).MethodsWe examined the clinical and biochemical characteristics, magnetic resonance imaging (MRI) of the pituitary, and association with HLA type in patients with CPI-hypophysitis.ResultsForty-nine patients were identified. Mean age was 61.3 years, 61.2% were men, 81.6% were Caucasian, 38.8% had melanoma, and 44.5% received PD-1/PD-L1 inhibitor monotherapy while the remainder received CTLA-4 inhibitor monotherapy or CTLA-4/PD-1 inhibitor combination therapy. A comparison of CTLA-4 inhibitor exposure vs PD-1/PD-L1 inhibitor monotherapy revealed faster time to CPI-hypophysitis (median 84 vs 185 days, P < .01) and abnormal pituitary appearance on MRI (odds ratio 7.00, P = .03). We observed effect modification by sex in the association between CPI type and time to CPI-hypophysitis. In particular, anti-CTLA-4 exposed men had a shorter time to onset than women. MRI changes of the pituitary were most common at the time of hypophysitis diagnosis (55.6% enlarged, 37.0% normal, 7.4% empty or partially empty) but persisted in follow-up (23.8% enlarged, 57.1% normal, 19.1% empty or partially empty). HLA typing was done on 55 subjects; HLA type DQ0602 was over-represented in CPI-hypophysitis relative to the Caucasian American population (39.4% vs 21.5%, P = 0.01) and CPI population.ConclusionThe association of CPI-hypophysitis with HLA DQ0602 suggests a genetic risk for its development. The clinical phenotype of hypophysitis appears heterogenous, with differences in timing of onset, changes in thyroid function tests, MRI changes, and possibly sex related to CPI type. These factors may play an important role in our mechanistic understanding of CPI-hypophysitis.

50. Paclitaxel-induced dorsal hand-foot syndrome

Abstract: Hand-foot syndrome (HFS), also known as palmoplantar erythrodysesthesia or acral erythema, is a known adverse effect of chemotherapeutic agents that most commonly presents as palmoplantar dysesthesia and erythematous plaques localized to the palms and soles. Paclitaxel is an uncommon cause of HFS and is notable for its unique presentation on the dorsal hands and feet. We present an unusual case of paclitaxel-induced HFS localized to the dorsal hands of a 66-year-old man with metastatic angiosarcoma. Early identification and management of HFS is critical to allow for continuation of chemotherapy while improving patient quality of life.
