Abstract

Background: This meta-analysis investigated the effects of enhanced recovery after surgery (ERAS) protocols compared to conventional care on postoperative outcomes in patients aged 70 years or older undergoing pancreatoduodenectomy (PD). Methods: Five databases were systematically searched. Comparative studies with available individual patient data (IPD) were included. The main outcomes were postoperative morbidity, length of stay, readmission and postoperative functional recovery elements. To assess an age-dependent effect, the group was divided in septuagenarians (70–79 years) and older patients (≥80 years). Results: IPD were obtained from 15 of 31 eligible studies comprising 1109 patients. The overall complication and major complication rates were comparable in both groups (OR 0.92 [95% CI: 0.65–1.29], p =.596 and OR 1.22 [95% CI: 0.61–2.46], p =.508). Length of hospital stay tended to be shorter in the ERAS group compared to the conventional care group (−0.14 days [95% CI: −0.29 to 0.01], p =.071) while readmission rates were comparable and the total length of stay including days in hospital after readmission tended to be shorter in the ERAS group (−0.28 days [95% CI: −0.62 to 0.05], p =.069). In the subgroups, the length of stay was shorter in octogenarians treated with ERAS (−0.36 days [95% CI: −0.71 to −0.004], p =.048). The readmission rate increased slightly but not significantly while the total length of stay was not longer in the ERAS group. Conclusion: ERAS in the elderly is safe and its benefits are preserved in the care of even in patients older than 80 years. Standardized care protocol should be encouraged in all pancreatic centers.

Abstract

Background: This individual-patient data meta-Analysis investigated the effects of enhanced recovery after surgery (ERAS) protocols compared with conventional care on postoperative outcomes in patients undergoing pancreatoduodenectomy. Methods: The Cochrane Library, MEDLINE, Embase, Scopus, and Web of Science were searched systematically for articles reporting outcomes of ERAS after pancreatoduodenectomy published up to August 2020. Comparative studies were included. Main outcomes were postoperative functional recovery elements, postoperative morbidity, duration of hospital stay, and readmission. Results: Individual-patient data were obtained from 17 of 31 eligible studies comprising 3108 patients. Time to liquid (mean difference (MD)-3.23 (95 per cent c.i.-4.62 to-1.85) days; P < 0.001) and solid (-3.84 (-5.09 to-2.60) days; P < 0.001) intake, time to passage of first stool (MD-1.38 (-1.82 to-0.94) days; P < 0.001) and time to removal of the nasogastric tube (3.03 (-4.87 to-1.18) days; P = 0.001) were reduced with ERAS. ERAS was associated with lower overall morbidity (risk difference (RD)-0.04, 95 per cent c.i.-0.08 to-0.01; P = 0.015), less delayed gastric emptying (RD-0.11,-0.22 to-0.01; P = 0.039) and a shorter duration of hospital stay (MD-2.33 (-2.98 to-1.69) days; P < 0.001) without a higher readmission rate. Conclusion: ERAS improved postoperative outcome after pancreatoduodenectomy. Implementation should be encouraged.

Abstract

Hard X-/soft Gamma-ray astronomy is a key field for the study of important astrophysical phenomena such as the electromagnetic counterparts of gravitational waves, gamma-ray bursts, black holes physics and many more. However, the spatial localization, imaging capabilities and sensitivity of the measurements are strongly limited for the energy range $>$70 keV due to the lack of focusing instruments operating in this energy band. A new generation of instruments suitable to focus hard X-/ soft Gamma-rays is necessary to shed light on the nature of astrophysical phenomena which are still unclear due to the limitations of current direct-viewing telescopes. Laue lenses can be the answer to those needs. A Laue lens is an optical device consisting of a large number of properly oriented crystals which are capable, through Laue diffraction, of concentrating the radiation into the common Laue lens focus. In contrast with the grazing incidence telescopes commonly used for softer X-rays, the transmission configuration of the Laue lenses allows us to obtain a significant sensitive area even at energies of hundreds of keV. At the University of Ferrara we are actively working on the modelization and construction of a broad-band Laue lens. Abstract

Gamma-ray astronomy is a branch whose potential has not yet been fully exploited. The observations of elemental and isotopic abundances in supernova (SN) explosions are key probes not only of the stellar structure and evolution but also for understanding the physics that makes Type-Ia SNe as standard candles for the study of the Universe expansion properties. In spite of its crucial role, nuclear astrophysics remains a poorly explored field mainly for the typical emission lines intensity which are vanishing small and requires very high sensitivities of the telescopes. Furthermore, in spite that the Galactic bulge-dominated intensity of positron annihilation line at 511 keV has been measured, its origin is still a mystery due to the poor angular resolution and insufficient sensitivity of the commonly employed instrumentation in the sub-MeV energy domain. To answer these scientific issues a jump in sensitivity and angular resolution with respect to the present instrumentation is required. Conceived within the EU project AHEAD, a new high energy mission, capable of tackling the previously mentioned topics, has been proposed. This concept of mission named ASTENA (Advanced Surveyor of Transient Events and Nuclear Astrophysics), includes two instruments: a Wide Field Monitor with Imaging and Spectroscopic (WFM-IS, 2 keV - 20 MeV) capabilities and a Narrow Field Telescope (NFT, 50 - 700 keV). Thanks to the combination of angular resolution, sensitivity and large FoV, ASTENA will be a breakthrough in the hard X and soft gamma--ray energy band, also enabling polarimetry in this energy band. Abstract

We present measurements of the azimuthal dependence of charged jet production in central and semi-central root s(NN) = 2.76 TeV Pb-Pb collisions with respect to the second harmonic event plane, quantified as nu(ch)(2) (jet). Jet finding is performed employing the anti-k(T) algorithm with a resolution parameter R = 0.2 using charged tracks from the ALICE tracking system. The contribution of the azimuthal anisotropy of the underlying event is taken into account event-by-event. The remaining (statistical) region-to-region fluctuations are removed on an ensemble basis by unfolding the jet spectra for different event plane orientations independently. Abstract

At present, Europe lacks innovative companies, which grow quickly and end up big. This book examines SME growth, innovation and success, to suggest that fast-growing firms could offer a major contribution to the recovery of a European economy. The contributors examine eleven case studies from Italian firms, breaking the book up into three parts: context, actors and strategy. The topics discussed include entrepreneurship and technological clusters, innovative start-ups and growth factors, and family firms as the incubators of new ventures. Abstract

Breast cancer (BC) recovery has increased in recent years thanks to efforts of Omics-based research in this field. However, despite the important results obtained, BC remains a complex multifactorial pathology that is difficult to treat appropriately. Caveolin-1 (CAV1), the basic constituent protein of specialized plasma membrane invaginations called caveolae, is emerging as a potential therapeutic biomarker in BC. This factor may modulate BC response to chemotherapy and radiation therapy. In addition, recent reports describe the key role of CAV1 during cell response to oxidative stress. The aim of the present review was to describe the biological roles of CAV1 in BC considering its contrasting dual functions as an oncogene and as a tumor suppressor. In addition, we report on how CAV1 may contribute to tumor cell response to ionizing radiation treatment. Finally, new roles of CAV1 in BC both on epithelium and stroma may be useful as prognostic indicators for patient treatment and help clinicians in the selection of the best personalized therapy.
