Although recent research using implicit measures has shown promise in detecting deviant sexual associations, the discriminatory and predictive quality of implicit tasks has not yet surpassed traditional assessment methods such as questionnaires and phallometry. The current research extended previous findings by examining whether a combination of two implicit tasks, the Implicit Association Task (IAT) and the Picture Association Task (PAT), was capable of differentiating pedophiles from non-pedophiles, and whether the PAT, which allows separate analysis for male, female, boy and girl stimulus categories, was more sensitive to specific sexual associations in pedophiles than the IAT. A total of 20 male self-reported pedophiles (10 offender and 10 non-offenders) and 20 male self-reported heterosexual controls completed the two implicit measures. Results indicated that the combination of both tasks produced the strongest results to date in detecting implicit pedophilic preferences (AUC = .97). Additionally, the PAT showed promise in decomposing the sexual associations in pedophiles. Interestingly, as there was an equal distribution of offenders and non-offenders in the pedophile group, it was possible to test for implicit association differences between these groups. This comparison showed no clear link between having these implicit sexual associations and actual offending.

Phallometry is considered gold standard in objectifying sexual orientation, yet this measurement has been criticized because of its intrusiveness and limited reliability. AIM: To evaluate whether the spatial response pattern to sexual stimuli as revealed by a change in blood oxygen level-dependent (BOLD) signal can be used for individual classification of sexual orientation. METHODS: We used a preexisting functional MRI (fMRI) data set that had been acquired in a nonclinical sample of 12 heterosexual men and 14 homosexual men. During fMRI, participants were briefly exposed to pictures of same-sex and opposite-sex genitals. Data analysis involved four steps: (i) differences in the BOLD response to female and male sexual stimuli were calculated for each subject; (ii) these contrast images were entered into a group analysis to calculate whole-brain difference maps between homosexual and heterosexual participants; (iii) a single expression value was computed for each subject expressing its correspondence to the group result; and (iv) based on these expression values, Fisher's linear discriminant analysis and the kappa-nearest neighbor classification method were used to predict the sexual orientation of each subject. MEAN OUTCOME MEASURE: Sensitivity and specificity of the two classification methods in predicting individual sexual orientation. RESULTS: Both classification methods performed well in predicting individual sexual orientation with a mean accuracy of >85% (Fisher's linear discriminant analysis: 92% sensitivity, 85% specificity; kappa-nearest neighbor classification: 88% sensitivity, 92% specificity). Investigating sex interest, including deviant preferences, can be troublesome, mainly because of the methods chosen for assessment. Self-report and objective measures are invaluable to this research, however self-report is subject to socially desirable responding and phallometry is prone to faking and can be invasive (Kalmus & Beech, 2005). Consequently, more reliable and less invasive methods, commonly termed indirect measures have been developed (Snowden et al., 2011). However, most of these measures are still in their infancy, difficult to implement in practice, and only provide a single outcome variable (e.g., response-latency). Thus, in order to amalgamate the positives of the aforementioned methods, and lessen their limitations, a new tablet PC application (M-TASI; Multimodal Tablet for Assessing Sexual Interest) has been developed for use in an academic capacity, by drawing upon previous research (Hofmann et al., 2009; Kraus & Hofmann, 2014). Participants must ‘swipe’ sexually relevant stimuli towards or away from them in an approach or avoidance procedure, respectively. Approach-avoidance procedures have been deemed viable in sexuality research (Hofmann et al., 2009), yet there is no evidence of using a tablet-based approach-avoidance paradigm to investigate human sexuality, thus the M-TASI could contribute to filing this dearth in the literature. A pilot study has been conducted to test the viability of the method, focusing on variables such as ‘swipe speed’, response time and location of first touch. These results will be discussed. This research will, hopefully, give credence to using this methodology in future, and could be applied to deviant sex interest.

A deviant interest greatly increases the risk of committing and recommitting a sexual offence (Hanson & Morton-Bourgon, 2005; Marshall & Barbaree, 1990). Thus, it is imperative that sexual deviancy is measured reliably. However, self-report is subject to socially desirable responding and phallometry is prone to faking and can be invasive (Kalmus & Beech, 2005). As a result, many researchers have set out to develop more reliable and less invasive methods, commonly termed indirect measures (Snowden et al., 2011). On the M-TASI, individuals are shown a series of pictures (e.g., children and adults), which they either swipe downwards (towards them) or upwards (away from them) to indicate liking or disliking the picture, respectively. The M-TASI records the speed at which the images are swiped, along with reaction times and ‘touch’ data (i.e., where one touches the image). This single tool, therefore, provides converging information to determine an individual’s sexual preference. The theoretical underpinning is based on the well-supported notion that pulling movements are associated with ‘approaching’ a desired object, while pushing movements are associated with ‘avoiding’ undesired objects (Markman & Brendl, 2005). The M-TASI will be a beneficial new tool for clinicians as it adopts an indirect approach; is easy to implement; and provides multiple outcomes variables. Laws (2009) stated that the future of sexual preference assessment belongs to new technologies and we propose that the M-TASI epitomises this sentiment.

While phallometric assessment of sexual offenders logically follows from sexual deviance theories of offending, we conclude that the evidence for this assessment process is inconsistent. A wide variety of methodologies, poorly specified and poorly chosen, or the absence of control groups has resulted in a fragmented literature in this area. Nonetheless, recent meta-analyses continue to portray sexual deviance (usually phallometrically determined) as the best predictor of future sexual violence. Recent studies have suggested that proxy measures of such deviance may provide sufficient evidence of deviance. We suggest that only by the use of research protocols with very rigorous methodology will the validity of phallometry be determined.

The penile plethysmograph is a device which measures male sexual arousal by means of a transducer around the subjects’ penis while various stimuli are presented. Although this seems to be a simple enough premise, half a century of research has failed to demonstrate that the assessment is either reliable or valid. It does, however, seem to be a significant predictor of risk. This chapter reviews the literature around the reliability and validity of phallometric assessment, the limitations to its use, its contribution to risk assessment, and alternatives to its use, and concludes with remarks on the future of the phallometry.

Phallometric assessment, the direct measurement of male sexual arousal in response to stimuli presented in a controlled setting, has been widely used in the assessment of sexual offenders to detect deviant sexual interests, determine treatment needs and inform risk assessments, but they have also been criticised due to a weak theoretical basis, wide variations in methodology and serious concerns around reliability and validity. From 1999 to 2007, phallometric assessments were conducted at two treatment units for incarcerated child sex offenders in New Zealand using a standardised Monarch 3.1 phallometric system which provided a database of 583 cases for analysis. This project, the only large scale analysis of phallometric data known to have been conducted using New Zealand data, was designed to explore a large number of research questions in three areas. The first area explored the factor structure of the assessments and the relationships between arousal profiles and a variety of co-existing demographic and offence related variables including age, social desirability, victim gender and victim age. The second area investigated the ability of a large number of possible phallometric indices to predict future both any sexual reconvictions and those involving children, with a particular focus on the role played by stimuli depicting teenagers. The third area investigated the prevalence and effects of the deliberate suppression of arousal, and analysed the ability of physiological measures to detect such suppression. The results of these investigations indicated that phallometric data factored according to the gender of the stimuli and could be further divided into age preferences resembling pedophila and teleiophilia. Phallometric indices consistently related to known victim gender but not victim age, suggesting that these offenders tended to target victims based on gender preferences but not age preferences to the same degree, posing questions about the relevance of diag
