INTRODUCTION Although high-quality postnatal care provides information and recognizes women's personal and cultural contexts, foreign-born women are more exposed to poor health and adverse birth outcomes. The aim of this study was to compare the length and model of postnatal care, along with the content of care, between foreign-born and native-born women living in Sweden. Another aim was to explore factors associated with being very satisfied with various aspects of postnatal care. METHODS This was a descriptive cross-sectional study of 483 postnatal women in two Swedish hospitals in 2017. Women completed a questionnaire comprising background data, pregnancy and birth related variables and the Early Postnatal Questionnaire. Data were analyzed using descriptive statistics, analysis of variance and multivariate logistic regression analyses. RESULTS Foreign-born women were more likely to have a shorter (<24 h) or longer (>48 h) length of postnatal stay than women born in Sweden. No differences in birth outcomes emerged between the two groups. Foreign-born women rated the medical (OR=1.77; 95% CI: 1.04–3.03) and emotional (OR=2.0; 95% CI: 1.17–3.40) aspects of postnatal care as being more important than Swedish-born women did. The most important aspect of overall satisfaction was the content of care, and the subscale Caring Relationship (AOR=8.15; 95% CI: 4.87–14.62) outscored all other aspects. CONCLUSIONS Important factors of satisfactory experiences with postnatal care in a Swedish context were receiving information, professional care, and a hospital environment that facilitates recovery after labor and birth. Culturally sensitive and individualized postnatal care with continuity should therefore be prioritized.

Background: The Dawan tribe is a tribe on the mainland of Timor Island that owns a tradition of caring for postpartum mothers called Tatobi. Tatobi is a tradition done by compressing the vagina, perineum, abdomen, and mother's breast using a traditional cloth dipped in hot water. Heat stimulation can cause vasodilation, possibly affecting the process of returning the uterus to its original state, known as uterine involution. Objectives: This study aims to determine differences of uterine involution in postpartum mothers who did and did not practice tatobi. Methods: This quantitative study uses a comparative descriptive study design involving 15 exposed groups and 15 control group people. Determination of respondents using the quota in Binaus Health Center, Kapan Health Center, and Fatumnasi Health Center. Technique data collection using direct lochia observation, measurement of uterine fundus height, and interviews to find out the practice of Tatobi. The data were then analyzed using the paired t-test. Results: The study showed no significant difference in FH of exposed and control groups on the first and third days of postpartum, which showed a p-value of 0.325 and 0.261, respectively. In contrast, there was a significant difference in FH on the seventh day, with a p-value of 0.000 at a 95% confidence level. The results also showed no significant difference in lochia’s volume on the 1st and the third day of postpartum (p-values 0.764 and 0.426, respectively). However, the lochia’s volume differed in the exposed and control groups with a p-value of 0.048). The results of this study could be due to the lax practice of tatobi, which is supported by factors of early mobilization, fulfilment of nutria ents, age, and parity. Conclusion: This research concludes that there is a difference of the process of uterine involution in the exposed group and the control group on the seventh day. &nbsp

The aim was to describe mothers’ overall experience receiving postnatal care in a Swedish home-based midwifery care model. Methods: A descriptive qualitative study was conducted. Mothers meeting the inclusion criteria for a new home-based postnatal care model at a hospital in Stockholm, Sweden, were included. In total, 24 healthy mothers participated in a semi-structured telephone interview, averaging 58 minutes. Data were analyzed using thematic analysis, according to Braun and Clarke. Results: The main theme explored, ‘The home-based postnatal care model facilitated a smooth succession into motherhood’, is explained by the themes: 1) Mothers felt ‘not left adrift’ when cared for by the home-based postnatal midwives; 2) Professional midwives with authority guided the way into motherhood; and 3) The home, a safe and secure space for new mothers. Conclusions: Mothers valued the well-structured home-based postnatal midwifery care. Important for mothers was to receive health checks, adequate information, and that midwives have a kind and individual approach to the families. Det finns många fördelar med att amma varför amning är förstahandsvalet. Antalet kvinnor som helammar sitt nyfödda barn minskar trots att majoriteten av svenska kvinnor planerar att helamma. WHO rekommenderar att barnet helammas till sex månaders ålder. Trots rekommendationen helammas 44 procent av barnen globalt vid sex månaders ålder. Amning är en del av barnmorskans kompetensområde vilket innebär att de ska främja amning. Barnmorskor har en plikt att genomföra amningsrådgivning till samtliga kvinnor. Många kvinnor upplever initialt amningssvårigheter och för att förhindra tidigt amningsstopp är barnmorskans amningsstöd grundläggande. För att utveckla barnmorskans amningsstöd är det därför viktigt att ta reda på hur kvinnor upplever att det är att ta emot amningsrådgivning. Syftet med studien var att undersöka kvinnors upplevelse av amningsstöd och amningsinformation från barnets födelse till sex månaders ålder. Metoden som valdes var en litteraturöversikt med systematisk metod. Studier med kvalitativ och mixad metod inkluderades och endast kvalitativa data användes i resultatet. Databassökning genomfördes i Pubmed och Cinahl. Därefter kvalitetsgranskades artiklarna och endast de som uppfyllde kvalitetskraven inkluderades i resultatet. Vald data analyserades sedan genom integrerad analys. I resultatet inkluderades femton artiklar. Likheter och skillnader i hur kvinnor upplevde amningsstödet identifierades och utformade subkategorierna: kontinuitet inom vårdkedjan, tid för amningsstöd, praktiskt amningsstöd, betydelsen av bekräftelse och uppmuntran samt information och rådgivning. Utifrån subkategorierna identifierades sedan kategorierna kontinuitet och tillgänglighet samt vårdpersonalens bemötande. Slutsatsen indikerade att kvinnorna hade både positiva och negativa upplevelser av amningsrådgivning. I resultatet framkom faktorer som barnmorskor kan påverka kvinnans amningsupplevelse på egen hand genom sitt bemötande. Many women experience initial difficulties with breastfeeding, and to prevent early disruption of breastfeeding, the midwife's support is essential. Understanding women's perception of breastfeeding advice is key to improving midwives' support. The aim of the study was to examine women's experiences of breastfeeding support and breastfeeding information from birth to six months of age. The method was a literature review using a systematic approach. Studies with qualitative and mixed methods were included, and only qualitative data were used in the results. A database search was conducted in PubMed and Cinahl. The articles were quality-reviewed, and only those meeting the quality criteria were included in the results. The selected data was then analyzed through integrated analysis. The results included fifteen articles. Similarities and differences in how women experienced breastfeeding support were identified and constituted the subcategories: continuity within the healthcare chain, time for breastfeeding support, practical breastfeeding support, the significance of affirmation and encouragement and information and guidance. Based on the subcategories, these categories where formulated: continuity and accessibility, as well as healthcare professionals' approach. The conclusion indicated that women had both positive and negative experiences of breastfeeding advice. The results revealed factors that midwives can independently influence women’s breastfeeding experience with their approach. Since 2015, the Swedish government has invested more money in the post-partum care to make it more accessible. The number of Swedish qualitative studies, which highlight women's experiences of sexological support after childbirth since 2015, is limited. This means that we do not know how adequate the government's investments have been in terms of patients' access to sexological support after childbirth. The purpose of the study is to gain an increased understanding of what sexological support, from healthcare, people feel they need after childbirth. The author has conducted ten qualitative semi-structured interviews with respondents who had a vaginal birth in the last two years. Analyzed through Merleau-Ponty's (1945/2006) and Ahmed's (2014) phenomenological theories the results show that the respondents' sexual health was affected by the interaction of the changes in their physical body and life situation. These aspects were also influenced by their partner and the post-partum care. During the follow-up, several respondents felt that their midwife focused too much on their physical body instead of changes related to their sexual health. Participants expressed a need for more information and confirmation regarding the physical, psychological, and social changes that affected their sexual health. The participants also wished to have more sexological support for their partner. This result indicates that the government's investment in the post-partum care has not been sufficient in terms of sexual health after childbirth, which means that parts of this care remain unavailable. Introduction: Despite the decrease in maternal mortality ratio, many women in Bangladesh are still at high-risk of death due to pregnancy-related morbidities. Increasing the rate of skilled maternal healthcare service utilization is effective to reduce maternal mortality rate. This paper examines the intervention effect of an integrated community-based maternal healthcare project implemented by a non-government organization, Friendship, aiming to provide maternal health services to women living in the remote riverine regions of Bangladesh. Methods: We examined the skilled maternal healthcare service utilization before and after project implementation of the mothers with birth experience of 0-6 months from the intervention (N =1,304) and comparison areas (N = 1,304). A difference-in-differences logistic model measured the effect of the intervention. Results: After the intervention, mothers were three times more likely to receive & GE; 4 ANC visits from skilled providers (AOR: 2.9; 95 % CI: 2.1-4.2), 1.5 times more likely to have skilled birth attendants during deliveries (AOR: 1.5; 95 % CI: 1.1-2.1) and 1.5 times more likely to seek at least one PNC within 42 days after delivery (AOR: 1.5; 95 % CI: 1.1-2.2) as compared to the comparison group. Conclusion: The intervention showed positive effect on improving the ANC coverage, skilled delivery, and PNC among the mothers residing the remote riverine areas. Therefore, it opens up the opportunity for adaptation of such integrated community and facility-based interventions by other LMICs.

Furthermore, the low breastfeeding proportion at six months is characterised by a marked social inequality. A previous intervention tested in a hospital setting succeeded in increasing the proportion of mothers breastfeeding exclusively at six months. However, most breastfeeding support is provided within the Danish municipality-based health visiting programme. Therefore, the intervention was adapted to fit the health visiting programme and implemented in 21 Danish municipalities. This article reports the study protocol, which will be used to evaluate the adapted intervention. Methods The intervention is tested in a cluster-randomised trial at the municipal level. A comprehensive evaluation approach is taken. The effectiveness of the intervention will be evaluated using survey and register data. Primary outcomes are the proportion of women who breastfeed exclusively at four months postpartum and duration of exclusive breastfeeding measured as a continuous outcome. A process evaluation will be completed to evaluate the implementation of the intervention; a realist evaluation will provide an understanding of the mechanisms of change characterising the intervention. Finally, a health economic evaluation will assess the cost-effectiveness and cost-utility of this complex intervention. OBJECTIVE: Continuity of care as provided by midwives promoting the health and well-being of new mothers during the postnatal period is critical; thus, access to midwifery services needs to be facilitated. The aim of this study was to describe new mothers' and midwives' experiences and perceptions of a new coordinated postnatal care intervention in a midwifery clinic. DESIGN: New mothers responded to open-ended questions in a survey, and midwives were interviewed individually about the intervention. A deductive content analysis research related to continuity of care concepts was used. SETTING AND PARTICIPANTS: The study was carried out at a midwifery clinic in a larger city in Sweden, for an eight-month period in 2019 and 2020. Two hundred and sixteen answers from new mothers and nine interviews with midwives were analysed. INTERVENTION: All registered pregnant women at the midwifery clinic received enhanced postnatal support based on a new coordinated postnatal care model. The focus was on continuity of care, from pregnancy to the postnatal period and included planning for the first weeks after childbirth at the end of pregnancy, early postnatal contact and several visits to the midwifery clinic. FINDINGS: New mothers describing the coordinated postnatal care model highlighted continuity and accessibility as empowering factors that made them feel assured and confident. Midwives emphasised the pregnancy to postnatal continuity as crucial to providing care based on individual needs. KEY CONCLUSIONS/IMPLICATIONS FOR PRACTICE: Using a structured and coordinated care model as a midwife that includes planning for the postnatal period together with the pregnant woman at the end of pregnancy may be a good and relatively easy way to create continuity and thus ensure satisfaction and confidence in expectant and new mothers.

Många kvinnor upplever initialt amningssvårigheter och för att förhindra tidigt amningsstopp är barnmorskans amningsstöd grundläggande. För att utveckla barnmorskans amningsstöd är det därför viktigt att ta reda på hur kvinnor upplever att det är att ta emot amningsrådgivning. Syftet med studien var att undersöka kvinnors upplevelse av amningsstöd och amningsinformation från barnets födelse till sex månaders ålder. Metoden som valdes var en litteraturöversikt med systematisk metod. Studier med kvalitativ och mixad metod inkluderades och endast kvalitativa data användes i resultatet. Databassökning genomfördes i Pubmed och Cinahl. Därefter kvalitetsgranskades artiklarna och endast de som uppfyllde kvalitetskraven inkluderades i resultatet. Vald data analyserades sedan genom integrerad analys. I resultatet inkluderades femton artiklar. Likheter och skillnader i hur kvinnor upplevde amningsstödet identifierades och utformade subkategorierna: kontinuitet inom vårdkedjan, tid för amningsstöd, praktiskt amningsstöd, betydelsen av bekräftelse och uppmuntran samt information och rådgivning. Utifrån subkategorierna identifierades sedan kategorierna kontinuitet och tillgänglighet samt vårdpersonalens bemötande. Slutsatsen indikerade att kvinnorna hade både positiva och negativa upplevelser av amningsrådgivning. I resultatet framkom faktorer som barnmorskor kan påverka kvinnans amningsupplevelse på egen hand genom sitt bemötande. Despite the majority of the Swedish women intending to exclusively breastfeed, the numbers are decreasing. WHO recommends exclusive breastfeeding until six months, however only 44 percent globally follow through. Midwives are experts in the field of breastfeeding, they should promote and provide efficient support. Many women experience initial difficulties with breastfeeding, and to prevent early disruption of breastfeeding, the midwife's support is essential. Understanding women's perception of breastfeeding advice is key to improving midwives' support. The aim of the study was to examine women's experiences of breastfeeding support and breastfeeding information from birth to six months of age. The method was a literature review using a systematic approach. Studies with qualitative and mixed methods were included, and only qualitative data were used in the results. A database search was conducted in PubMed and Cinahl. The articles were quality-reviewed, and only those meeting the quality criteria were included in the results. The selected data was then analyzed through integrated analysis. The results included fifteen articles. Similarities and differences in how women experienced breastfeeding support were identified and constituted the subcategories: continuity within the healthcare chain, time for breastfeeding support, practical breastfeeding support, the significance of affirmation and encouragement and information and guidance. Based on the subcategories, these categories where formulated: continuity and accessibility, as well as healthcare professionals' approach. The conclusion indicated that women had both positive and negative experiences of breastfeeding advice. The results revealed factors that midwives can independently influence women’s breastfeeding experience with their approach. Since 2015, the Swedish government has invested more money in the post-partum care to make it more accessible. The number of Swedish qualitative studies, which highlight women's experiences of sexological support after childbirth since 2015, is limited. This means that we do not know how adequate the government's investments have been in terms of patients' access to sexological support after childbirth. The purpose of the study is to gain an increased understanding of what sexological support, from healthcare, people feel they need after childbirth. The author has conducted ten qualitative semi-structured interviews with respondents who had a vaginal birth in the last two years. Analyzed through Merleau-Ponty's (1945/2006) and Ahmed's (2014) phenomenological theories the results show that the respondents' sexual health was affected by the interaction of the changes in their physical body and life situation. These aspects were also influenced by their partner and the post-partum care. During the follow-up, several respondents felt that their midwife focused too much on their physical body instead of changes related to their sexual health. Participants expressed a need for more information and confirmation regarding the physical, psychological, and social changes that affected their sexual health. The participants also wished to have more sexological support for their partner. This result indicates that the government's investment in the post-partum care has not been sufficient in terms of sexual health after childbirth, which means that parts of this care remain unavailable. Background: Evidence suggests that Aboriginal babies in Western Australia are not receiving adequate primary health care in their first 3 months of life, leading to questions about enablers and constraints to delivering such care. This paper presents findings from a qualitative research project investigating health providers’ perceptions and experiences of best and current practice in discharge planning, postnatal care and health education for Aboriginal mothers and their newborn babies. Methods: Constructivist grounded theory guided this research involving 58 semi-structured interviews conducted with health providers who deliver care to Aboriginal mothers and infants. Participants were recruited from hospital-based and primary health sites in metropolitan Perth, and regional and remote locations in Western Australia. Results: Structural factors enabling best practice in discharge planning, postnatal care, and health education for mothers included health providers following best practice guidelines and adequate staffing levels. Organisational enablers included continuity of care throughout pregnancy, birth and postnatally. In particular, good communication between services around discharge planning, birth notifications, and training in culturally respectful care. Structural and organisational constraints to delivering best practice and compromising continuity of care were identified as beyond individual control. These included poor communication between different health and social services, insufficient hospital staffing levels leading to early discharge, inadequate cultural training, delayed receipt of birth notifications and discharge summaries received by Aboriginal primary health services. Conclusion: Findings highlight the importance of examining current policies and practices to promote best practice in postnatal care to improve health outcomes for mothers and their Aboriginal babies.

ObjectivesPrenatal and postpartum care for women with substance use is important due to increased risk of poor health outcomes. The influence of substance use on perinatal care utilization is not well characterized, especially postpartum care. The objective of this study was to examine the effect of substance use during pregnancy on prenatal and postpartum care utilization in a nationally representative sample and to identify maternal characteristics associated with inadequate prenatal and postpartum care among women with substance use.MethodsPregnancy Risk Assessment Monitoring System data (2016-2018) from 8 states were used for this study. Logistic regression models adjusted for complex survey weights and confounder variables were used to estimate the odds of not receiving adequate prenatal care and postpartum care. Weighted Rao-Scott chi-square tests were used to examine maternal characteristics associated with care utilization among women who reported substance use during pregnancy.ResultsThe study included 15,131 women, with 5.3% who reported illicit substance use during pregnancy. In multivariable models, substance use was associated with an increase in the odds of not receiving adequate prenatal care (OR 1.69, CI 1.32, 2.17) and not receiving postpartum care (OR: 1.47, CI 1.10, 1.95). Among women who reported substance use, depression and smoking status were associated with not receiving adequate prenatal or postpartum care.ConclusionsSubstance use during pregnancy is independently associated with disparities in prenatal and postpartum care access. Future studies are needed to identify how barriers lead to care inequalities and importantly, to identify strategies to improve care utilization.

The aim was to explore women's experience of the decision-making process about early return from hospital with home-based postnatal midwifery care. METHODS: This is a descriptive qualitative study. In total, 24 women participated in a semi-structured telephone interview, averaging 58 minutes. Data were analyzed using thematic analysis according to Braun and Clarke. RESULTS: The main theme explored was 'The supremacy of giving new mothers autonomy to decide on the postnatal care model they would prefer'. Important aspects of the women's decision-making process were the time-point for receiving information about the home-based midwifery model of care, to receive sufficient time for consideration about the model, to have a rationale for choosing home-based care, and to comprehend the concept. CONCLUSIONS: Women must be given sufficient time for consideration and necessary information about postnatal care models, which is essential for making an informed decision. Parents' readiness for discharge must be identified by midwives who need to facilitate shared decision-making by introducing early postnatal care model choices, describe these options, and support women to explore their preferences. Midwives must ensure parents' participation in decision-making for the time of discharge from hospital.

Traumatic experiences can have significant health effects, particularly when they are experienced during childhood. Structural determinants of health including environmental disasters and limited access to mental health services and affordable housing can contribute additional stress for parents with a personal history of childhood adversity. These factors can directly affect their children, contributing to intergenerational trauma. Pregnant people and families with young children are often referred to public health nursing maternal and child home visiting (HV) programs when there are concerns about historical or evolving childhood trauma. The strict eligibility and participation requirements of existing evidence-based maternal and child HV programs can exclude families that have experienced or are experiencing childhood trauma and its effects and can limit innovation by public health nurses, a hallmark of the field. Therefore, we advocate and describe the implementation of the Trauma Informed Approach in Public Health Nursing (TIA PHN) model, which incorporates a trauma-informed approach into a traditional maternal and child HV program in 3 California counties. TIA PHN, which began enrollment in March 2021, involves public health nurses and community health workers and integrates program evaluations in pursuit of evidence-based status. Especially parental mentalization, i.e. the caregiver’s ability to notice and interpret the child’s behavior in terms of mental states, is important in infancy where the caregiver-infant communication is based solely on the infant’s behavioral cues. Methods: This study evaluates the efficacy of the intervention Understanding Your Baby (UYB) compared to Care As Usual (CAU) in 10 Danish municipalities. UYB aims at promoting parental competence in new parents by supporting them in noticing their infants’ behavioral cues and interpreting them in terms of mental states. Participants will be approximately 1,130 singletons and their parents. Inclusion criteria are first-time parents, minimum 18 years old, living in one of the 10 municipalities, and registered in the Danish Civil Registration Register (CPR). Around 230 health visitors deliver the UYB as part of their routine observation of infant social withdrawal in the Danish home visiting program. During an interaction between the health visitor and the infant, the health visitor articulates specific infant behaviors and helps the caregivers interpret these behaviors to mental states. The study is a controlled parallel group study with data obtained at four time points in two phases: First in the control group receiving the publicly available postnatal care (CAU), secondly in the intervention group after UYB implementation into the existing postnatal services. The primary outcome is maternal competence. Secondary measures include paternal competence, parental stress, parental mentalizing, and infant socioemotional development. BackgroundAs mobile phone uptake in India continues to grow, there is also continued interest in mobile platform-based interventions for health education. There is a significant gender gap in mobile phone access-women's access to mobile phones is constrained by economic and social barriers. Pregnancy and postpartum care is one of many targets for mobile health (mHealth) interventions that particularly rely upon women's access to and facility with mobile phone use.ObjectiveWe aimed to describe the dynamics and patterns of married pregnant and postpartum women's mobile phone access and use (among both phone owners and nonowners) who participated in an mHealth postpartum care intervention and to identify potential barriers to their participation in mobile platform-based interventions.MethodsA secondary analysis was performed on mixed methods data obtained for a pilot mHealth intervention for postpartum care of mothers in rural Punjab from July 2020 to February 2021. Two formative sources included exploratory in-depth interviews among postpartum women (n=20; 1-3 months postpartum) and quantitative maternal health survey among women who were pregnant or who had recently given birth (n=102). We also utilized mixed methods intervention assessment data from early postpartum women who participated in the pilot intervention (n=29), including intervention moderator perspectives. Qualitative and quantitative analyses were performed, and pertinent findings were grouped thematically.ResultsThe majority of women owned a phone (maternal health survey: 75/102, 74%; demographic survey: 17/29, 59%), though approximately half (53/102, 52%) still reported sharing phones with other family members. Objetivo: Analizar la relación del retraso de la lactogénesis II con la percepción materna de leche insuficiente. Método: Se realizó un estudio observacional longitudinal prospectivo, multicéntrico. Se obtuvieron datos al alta y entre 1 y 5 meses posparto sobre la percepción de leche insuficiente y las variables relacionadas mediante un cuestionario autoadministrado y el posterior seguimiento postal y online. Para el desarrollo del modelo explicativo, se realizó un análisis de regresión logística. Resultados: Participaron un total de 260 puérperas. El 31,9% de ellas percibieron leche insuficiente y el 23,6% presentaron retraso de la lactogénesis II. Durante el ingreso posparto, se relacionó con la percepción materna de leche insuficiente el retraso de la lactogénesis II (OR = 2,26; IC 95% = 1,07-4,79), la dificultad para amamantar (OR = 1,02; IC 95% = 1,00-1,03) y la ayuda de los profesionales en la lactancia (OR = 0,70; IC 95% = 0,50-0,97). Conclusiones: La aparición de dificultades en la lactancia durante el ingreso posparto y al alta, especialmente cuando existe un retraso de la lactogénesis II, deben considerarse indicadores de riesgo, que sugieren la necesidad de un apoyo adicional a los cuidados estandarizados. Data were obtained at discharge and between 1 and 5 months postpartum on the perception of insufficient milk and related variables, by means of a self-administered questionnaire, and subsequent postal and online follow-up. Logistic regression analysis was used to develop the explanatory model. Results: A total of 260 puerperal mothers participated. Of these, 31.9% had insufficient milk and 23.6% had delayed lactogenesis II. During postpartum admission, delayed lactogenesis II (OR = 2.26; 95% CI = 1.07-4.79), difficulty in breastfeeding (OR = 1.02; 95% CI = 1.00-1.03), and professional help in breastfeeding (OR = 0.70; 95% CI = 0.50-0.97) were associated with maternal perception of insufficient milk. Conclusions: The occurrence of breastfeeding difficulties during postpartum admission and at discharge, especially when there is delayed lactogenesis II, should be considered risk indicators, suggesting the need for additional support to standardized care. The perception of insufficient milk is a suitable indicator to assess the quality of professional breastfeeding support in improvement interventions.

Durante el ingreso posparto, se relacionó con la percepción materna de leche insuficiente el retraso de la lactogénesis II (OR = 2,26; IC95% = 1,07–4,79), la dificultad para amamantar (OR = 1,02; IC95% = 1,00–1,03), y la ayuda de los profesionales en la lactancia (OR = 0,70; IC95% = 0,50–0,97). Conclusiones: La aparición de dificultades en la lactancia durante el ingreso posparto y al alta, especialmente cuando existe un retraso de la lactogénesis II, deben considerarse indicadores de riesgo, que sugieren la necesidad de un apoyo adicional a los cuidados estandarizados. La PIM es un indicador adecuado para evaluar la calidad del apoyo profesional a la lactancia en intervenciones de mejora.

BackgroundAs mobile phone uptake in India continues to grow, there is also continued interest in mobile platform-based interventions for health education. There is a significant gender gap in mobile phone access-women's access to mobile phones is constrained by economic and social barriers. Pregnancy and postpartum care is one of many targets for mobile health (mHealth) interventions that particularly rely upon women's access to and facility with mobile phone use.ObjectiveWe aimed to describe the dynamics and patterns of married pregnant and postpartum women's mobile phone access and use (among both phone owners and nonowners) who participated in an mHealth postpartum care intervention and to identify potential barriers to their participation in mobile platform-based interventions.MethodsA secondary analysis was performed on mixed methods data obtained for a pilot mHealth intervention for postpartum care of mothers in rural Punjab from July 2020 to February 2021. Two formative sources included exploratory in-depth interviews among postpartum women (n=20; 1-3 months postpartum) and quantitative maternal health survey among women who were pregnant or who had recently given birth (n=102). We also utilized mixed methods intervention assessment data from early postpartum women who participated in the pilot intervention (n=29), including intervention moderator perspectives. Qualitative and quantitative analyses were performed, and pertinent findings were grouped thematically.ResultsThe majority of women owned a phone (maternal health survey: 75/102, 74%; demographic survey: 17/29, 59%), though approximately half (53/102, 52%) still reported sharing phones with other family members. Sharing a phone with female family members typically allowed for better access than sharing with male family members. Indigenous practices are performances that occur naturally in a region or a growing living environment. Most women believe in indigenous practices because of their cultures and social structure. In South Africa regardless of the availability and accessibility of maternal and child health services, 50% of women were found that they still consult traditional birth attendants as their first choice during pregnancy, labour, delivery, and postnatal care. The purpose of the study was to determine the indigenous practices of women during pregnancy, labour and puerperium amongst cultural groupings at selected hospitals in Limpopo Province, South Africa. A Convergent parallel mixed method design was used in the study to collect both qualitative and quantitative data at the same time. Non-probability purposive sampling was used to select 15 participants and Probability simple random sampling was used to select 125 women who were pregnant, in labour and puerperium using slovin’s formula. Data were collected through a semi-structured interview with a guide for qualitative strand and a self-administered structured questionnaire for quantitative srtand. Data were analysed qualitatively using tech’s open coding method and quantitatively using Statistical Package for the Social Sciences (SPSS) Version 25 with the assistance of the University of Limpopo’s Bio-statistician. The results of the study showed that most women use indigeneous practices for protection against witchcraft, fear of giving birth in caesarian section and many other reasons.THPs and church leaders are regarded as the most principled people in their community. BACKGROUND The quality Kangaroo Mother Care (KMC) continues to be sub-standard as it is not managed properly, despite the maternal and neonatal services provided by the midwives during postpartum and neonatal period, as this can lead to increased neonatal mortality rate and maternal depression at selected hospitals of Limpopo Province, therefore the researcher is interested in developing strategies to enhance quality KMC at selected hospitals of Limpopo Province. KMC has been found to have physiological, behavioural, psychosocial and cognitive developmental benefits, and it enhances motherinfant bonding. The aim of the study was to develop strategies to enhance quality Kangaroo Mother Care in selected hospitals of Limpopo Province. RESEARCH METHOD A quantitative descriptive cross-sectional research method was used to collect numerical data with regard to the factors that hinders the quality Kangaroo Mother Care in selected hospitals of Limpopo Province. Population size was 77, Simple Random Probability Sampling was used in this study with the sample size of 65 midwives. Data were collected using self-administered questionnaires and analysed using Statistical Package for Social Sciences (SPSS) version 24. Quantitative analysis is the numerical representation and manipulation of observations for the purpose of describing and explaining the phenomenon reflected on observations (Babbie & Roberts, 2018). RESULTS The study revealed that the factors that hinders quality of Kangaroo Mother Care are lack of education and training to midwives regarding Kangaroo Mother Care, and other sources suggested that all categories of nurses should also be trained. The midwives are knowledgeable with regard to KMC, however, the problem remains the work overload when they have to monitor both the mother and neonate during feeding. The Utah Department of Health Office of Primary Care and Rural Health (OPCRH) exists to increase access to these services in rural and under-served communities in Utah. The Primary Care Needs Assessment analyzes various indicators of health access, health status, and social determinants of health to understand needs and increase access to healthcare. The uneven distribution of healthcare workforce throughout Utah has contributed to lower access to care resulting in Health Professional Shortage Areas. Health Professional Shortage Areas (HPSAs) are defined as an area with too few healthcare providers to meet the needs of the population. Maternal and neonatal mortality in Bihar, India was far higher than the aspirational levels set out by the Sustainable Development Goals. Provider training programs have been implemented in many low-resource settings to improve obstetric and neonatal outcomes. This longitudinal investigation assessed diagnoses and management of postpartum hemorrhage (PPH), hypertensive disorders of pregnancy, birth asphyxia (BA), and low birth weight (LBW), as part of the CARE's AMANAT program in 22 District Hospitals in Bihar, between 2015 and 2017. Physicians and nurse mentors conducted clinical instruction, simulations and teamwork and communication activities, infrastructure and management support, and data collection for 6 consecutive months. Analysis of diagnosis included 11,259 non-referred and management included 11,800 total (non-referred and referred) admissions that were observed. Data were analyzed using the chi-square test for trend. PPH was diagnosed in 3.7% with no significant trend but diagnosis of hypertensive disorders increased from 1.0% to 1.7%, (ptrend = 0.04), over the 6 months. BA was diagnosed in 5.8% with no significant trend but LBW diagnoses increased from 11% to 16% (ptrend<0.01). Among PPH patients, 96% received fluids, 85% received uterotonics and 11% received Tranexamic Acid (TXA). There was a significant positive trend in the number of patients receiving TXA for PPH (6% to 13.8%, ptrend = 0.03). Of all neonates with BA, there were statistically significant increases in the proportion who were initially warmed, dried, and stimulated (78% to 94%, ptrend = 0.02), received airway suction (80% to 93%, ptrend = 0.03), and supplemental oxygen without positive pressure ventilation (73% to 86%, ptrend = 0.05). Diagnoses of hypertensive disorders and LBW as well as initial management of BA increased during the AMANAT program. However, underdiagnoses of PPH and hypertensive disorders relative to population levels remain critical barriers to improving maternal

Food insecurity (FI) is an understudied risk factor for depression among perinatal women in sub-Saharan Africa. We therefore explored the longitudinal relationship between FI and depressive symptoms among a cohort of perinatal women of mixed HIV status (n = 371) in Kenya (NCT02974972, NCT02979418). Using longitudinal linear and logistic regressions with random effects, we assessed bivariate and adjusted associations between maternal FI and depressive symptoms. HIV status was also assessed as a potential effect modifier. At baseline, 58% of pregnant women had probable depression (CES-D score > 16) and 84% were severely food insecure. In adjusted analyses, severely food-insecure women had 5.90 greater odds (95% CI 2.32, 15.02, p < 0.001) of having probable depression and scored 4.58 points higher on the CES-D scale (SE: 1.04, p < 0.001) relative to food-secure women. HIV status did not modify the association between FI and depressive symptoms. Interventions to reduce FI may reduce perinatal depression, benefiting mothers and their infants.

Digitalization of health information can assist patient information management and improve health services even in low middle-income countries. We have implemented a mother and child health registration system in the study areas of Kenya and Lao PDR to evaluate barriers to digitalization. We conducted in-depth interviews with 20 healthcare workers (HCWs) who used the system and analyzed it qualitatively with thematic framework analysis. Quantitatively, we analyzed the quality of recorded data according to missing information by the logistic regression analysis. The qualitative analysis identified six themes related to digitalization: satisfaction with the system, mothers’ resistance, need for training, double work, working environment, and other resources. The quantitative analysis showed that data entry errors improved around 10% to 80% based on odds ratios in subsequent quarters compared to first quarter periods. The number of registration numbers was not significantly related to the data quality, but the motivation, including financial incentives among HCWs, was related to the registration behavior. Considering both analysis results, workload and motivation to maintain high performance were significant obstacles to implementing a digital health system. DESIGN: A descriptive cross-sectional qualitative study was conducted as a part of a larger study. SETTING: Families who qualified to be discharged early were offered to participate in a postnatal home-based model of midwifery care by a hospital in Stockholm, Sweden. PARTICIPANTS AND MEASUREMENTS: In total, 16 fathers participated in a semi-structured telephone interview, averaging 43 min. Data were analyzed using systematic text condensation. FINDINGS: Three major themes emerged: To decide on home- or hospital-based postnatal care - a matter of safety, To be offered professional midwifery postnatal support at home, and To be at home helped fathers to navigate parenthood. Fathers appreciated the home-based postnatal care and felt safe because of the received professional support from midwives. KEY CONCLUSIONS: Home-based postnatal care was valued by fathers whose partner had a non-complicated vaginal birth because they felt safe in their home environment and supported by midwives. The home environment aided fathers in supporting their partners and developing a father-infant bond. IMPLICATIONS FOR PRACTICE: Home-based postnatal care was valued by fathers and should be considered an option for new families. To offer home-based postnatal care may result in less overcrowded postnatal wards. Background: Routine maternity care visits (MCVs) such as antenatal care (ANC), institutional delivery, and postnatal care (PNC) visits are crucial to utilisation of maternal and newborn health (MNH) interventions during pregnancy-postnatal period. In Nepal, however, not all women complete these routine MCVs. Therefore, this study examined the levels and correlates of (dis)continuity of MCVs across the antenatal-postnatal period. Methods: We conducted further analysis of the 2016 Nepal Demographic and Health Survey. A total of 1,978 women aged 15–49 years, who had live birth two years preceding the survey, were included in the analysis. The outcome variable was (dis)continuity of routine MCVs (at least four ANC visits, institutional delivery, and PNC visit) across the pathway of antennal through to postnatal period. Independent variables included several social determinants of health under structural, intermediary, and health system domains. Multinomial logistic regression was conducted to identify the correlates of routine MCVs. Relative risk ratios (RR) were reported with 95% confidence intervals at a significance level of p<0.05. Results: Approximately 41% of women completed all routine MCVs with a high proportion of discontinuation around childbirth. Women of disadvantaged ethnicities, from low wealth status, who were illiterate had higher RR of discontinuation of MCVs (compared to completion of all MCVs). Similarly, women who speak Bhojpuri, from remote provinces (Karnali and Sudurpaschim), who had a high birth order (≥4), who were involved in the agricultural sector, and who had unwanted last birth had a higher RR of discontinuation of MCVs. Women discontinued routine MCVs if they had poor awareness of health mother-groups and perceived the problem of not having female providers. Transposition of the great arteries is a congenital cardiac anomaly, in which the position of the pulmonary artery and the aorta is reversed. This paper presents the clinical case of a neonate with this pathology who underwent surgery with the arterial switch technique, which is the most frequently used in these cases, because it is associated with lower mortality and complication rates. Upon arrival of the patient on the hospital ward, she presented a series of medical problems related to her cardiac pathology and surgical intervention, such as diaphragmatic paresis and chylothorax. Also, the needs of feeding, elimination, mobilization, sleep-rest and hygiene were compromised. The parents of the new-born participated in the care provided by nurses. In this sense, counselling on breastfeeding and performing massage as therapy for colic were key to their resolution. Clinical follow-up of these patients is essential to avoid short- and long-term sequelae. Additionally adequate nursing education regarding this type of heart disease is essential in order to meet the needs of this population., La transposición de las grandes arterias es una anomalía cardiaca congénita, en el que la arteria pulmonar y la aorta están intercambiadas posicionalmente. Se presenta el caso clínico de un neonato con esta patología que es intervenido quirúrgicamente con la técnica de switch arterial, que se caracteriza por ser la cirugía más frecuentemente utilizada en estos casos por disminuir la tasa de mortalidad y por su escasa tasa de complicaciones. A su llegada a la planta de hospitalización presenta una serie de problemas médicos relacionados con su patología cardiaca e intervención quirúrgica como la paresia diafragmática y quilotórax. Se vieron comprometidas a su vez las necesidades alimentación, eliminación, movilización, sueño-reposo e higiene, incluyendo en los cuidados proporcionados a los progenitores de la recién nacida. En este sentido, el asesoramiento en lactancia materna y la realización del masaje como terapia para los cólicos fueron clave para su resolución. Es fundamental el seguimiento clínico de estos pacientes para evitar secuelas a corto y largo plazo, así como el conocimiento de este tipo de cardiopatías por parte de enfermería para poder cubrir las necesidades de esta población.

Bakgrund: Teamarbete anses nödvändigt i hälso- och sjukvården. Barnmorskan och undersköterskan ska kunna samarbeta i team tillsammans med andra barnmorskor, undersköterskor samt med andra professioner. Syfte: Syftet med studien var att beskriva barnmorskors och undersköterskors upplevelser av teamarbete på ett BB. Metod: En kvalitativ metod används i genomförandet av studien. Tio telefonintervjuer, med fem barnmorskor och fem undersköterskor, utfördes. Datamaterialet analyserades med hjälp av Graneheim och Lundmans innehållsanalys. Resultat: Resultatet sammanställdes i sex kategorier: Att arbeta mot samma mål, Att kommunicera, Att stödja varandra i arbetet, Att ha förståelse för varandra inom teamen, Att känna trygghet i teamet samt Att hantera stress. Två av kategorierna genererade i underkategorier. Slutsats: Det är grundläggande att ha ett effektivt teamarbete för att skapa en god vård. För att skapa ett effektivt teamarbete krävs det att teammedlemmarna stödjer varandra och arbetar mot samma mål. Det är även viktigt att ha en god kommunikation med varandra och ha förståelse för varandras professioner., Background: Teamwork is considered necessary in health care. The midwife and the assistant nurse must be able to work in teams together with other midwives, assistant nurses and other professions. Purpose: To describe midwives and assistant nurses experience of teamwork in postnatal care. Method: A qualitative method was used. The data collection was conducted using semi structured interviews. Interview material was analyzed using a qualitative content analysis by Graneheim och Lundman. Result: The results were compiled in six categories: To work towards the same goal, To communicate, To support each other in the work, To have an understanding of each other in the teams, To feel secure in the team and To manage stress. Two of the categories generated in subcategories. Conclusion: It is fundamental to have effective teamwork to create good care. To create effective teamwork, team members need to support each other and work towards the same goal. It is also important to have good communication with each other and have an understanding of each other's professions.

DESIGN: A descriptive cross-sectional qualitative study was conducted as a part of a larger study. SETTING: Families who qualified to be discharged early were offered to participate in a postnatal home-based model of midwifery care by a hospital in Stockholm, Sweden. PARTICIPANTS AND MEASUREMENTS: In total, 16 fathers participated in a semi-structured telephone interview, averaging 43 min. Data were analyzed using systematic text condensation. FINDINGS: Three major themes emerged: To decide on home- or hospital-based postnatal care - a matter of safety, To be offered professional midwifery postnatal support at home, and To be at home helped fathers to navigate parenthood. Fathers appreciated the home-based postnatal care and felt safe because of the received professional support from midwives. KEY CONCLUSIONS: Home-based postnatal care was valued by fathers whose partner had a non-complicated vaginal birth because they felt safe in their home environment and supported by midwives. The home environment aided fathers in supporting their partners and developing a father-infant bond. IMPLICATIONS FOR PRACTICE: Home-based postnatal care was valued by fathers and should be considered an option for new families. To offer home-based postnatal care may result in less overcrowded postnatal wards. Midwives need to enable fathers' participation and support their parental role regardless of where the care takes place.
