1. Optimization of Banana Flour Cookie Formulations Based on Physicochemical Properties

Abstract: The optimisation of banana flour cookie formulations based on physicochemical properties is a multifaceted process that involves the analysis of various ingredients and their concentrations to develop a high-quality and nourishing snack. This study aimed to optimize banana flour and shortening for the best cookie formulations employing response surface methodology (RSM) in conjunction with central control rotatable design (CCRD). Thirteen different cookie formulations based on CCRD were developed. The cookies' physicochemical properties (spread ratio, density, moisture content, water activity, L value, total color difference and hardness) were determined. Regression models, response surface models, numerical optimization based on desirability function samples to determine an optimum formulation and graphical optimization by superimposing all contour plots of response surface models to visualize an optimum region were determined using a Design Expert software. The regression models could predict the physicochemical properties of cookies as a function of factors with an accuracy result depending on the responses. 60% banana flour with 58% shortening provided the desired quality of banana flour cookies. The proximate analysis result shows its quality characteristics. The optimum formulation of cookies could be used for the commercial production of banana flour cookies to develop value-added cookies with improved nutrition.

2. Risk Management Practices in Nigeria Construction Sector and Impact on Project Performance

Abstract: The construction industry in every nation is essential to the growth of the economy and its development. The industry is characterized by complex projects, unpredictable variables, and is dependent on many different stakeholders, which subject projects in this sector to a variety of risks that can have a significant impact on project delivery. Risks in construction projects in Nigeria can lead to cost overruns, schedules not being met, and low quality of the project. The concept of risk management involves the management of identified risks which reduces the likelihood of their occurrence and the consequences of adverse risks. A project may be delivered or deemed successful, but not meet the project performance objectives. The Nigerian construction industry has undergone incredible growth and progress; however, there have been numerous challenges that have prevented project performance, such as a lack of expertise, knowledge gaps, and a drought of experienced personnel. The study examines the various risk management practices and their impact on construction project performance. Project performance implies that the project objectives are met in terms of cost, schedule, and quality. A survey research method was adopted using a questionnaire as the primary instrument for data collection on a five-point Likert scale. The questionnaire was designed to collect data relating to risk management’s impact on project performance. The questionnaires were also designed to collect data on risk identification as a risk management practice and its impact on project performance. A total of 85 questionnaires were administered and analyzed. The results from the study indicate a strong relationship between risk management practices and project performance.

Abstract: The incidence of endometrial cancer is rising. Measures to identify women at risk and to detect endometrial cancer earlier are required to reduce the morbidity triggered by the aggressive treatment required for advanced endometrial cancer. We developed the WID-EC (Women's cancer risk IDentification-Endometrial Cancer) test, which is based on DNA methylation at 500 CpG sites, in a discovery set of cervical liquid-based cytology samples from 1086 women with and without an endometrial cancer (217 cancer cases and 869 healthy controls) with a worse prognosis (grade 3 or ≥stage IB). We validated the WID-EC test in an independent external validation set of 64 endometrial cancer cases and 225 controls. We further validated the test in 150 healthy women (prospective set) who provided a cervical sample as part of the routine Swedish cervical screening programme, 54 of whom developed endometrial cancer within 3 years of sample collection. The WID-EC test identified women with endometrial cancer with a receiver operator characteristic area under the curve (AUC) of 0.92 (95% CI: 0.88-0.97) in the external set and of 0.82 (95% CI: 0.74-0.89) in the prospective validation set. Using an optimal cutoff, cancer cases were detected with a sensitivity of 86% and a specificity of 90% in the external validation set, and a sensitivity and specificity of 52% and 98% respectively in the prospective validation set. The WID-EC test can identify women with or at risk of endometrial cancer.

7. Comparative Antimicrobial Properties of a Consortium of Nauclea latifolia Sm. and Ocimum gratissium L. Extracts with Their CuO

Abstract: Over the years, several microorganisms have developed antibiotic resistance, rendering them impotent and otherwise useless. This research work is intended at exemplifying the consortium of crude extracts of Nauclea latifolia and Ocimum gratissium for antimicrobial potentials. Nanoparticles made from extracts were equated to the activities. Plant nanoparticles were created by combining powdered plant materials with copper oxide and suspending them in ethanol and distilled water, which were then screened to achieve crude extracts (CuO). The presence of bioactive machinery tartan in crude extracts. Agar well diffusion was betrothed to test antibacterial activity while the poisoned plate method was utilized to define antifungal potentials. Flavonoids, alkaloids, steroids, saponins, terpenoids, and tannins were discovered in plant extracts. At a dosage of 40 mg/mL, the aqueous crude extract outperformed the ethanolic extract, with the best inhibition zone (30) against Bacillus megaterium. The antimicrobial properties of the aqueous nanoparticle extract were stronger than those of the crude equivalent, antibacterial and antifungal inhibition zones were 36 mm at 40 mg/mL and 32 mm at 60 mg/mL respectively. According to the findings, aqueous extracts have stronger antibacterial activity than ethanolic extracts and nanoparticle created utilizing a syndicate of two plants has the potential to improve antimicrobial properties.

Abstract: The study investigated the impact of class size on teaching and learning of social studies in Bwari Area Council, FCT-Abuja. The study adopted descript study adopted descriptive design of the survey type. The sample size was 200 teachers drawn from 10 wards in Bwari Area Council using multi-stage sampling procedure. One validated instrument was used for data collection. Self-design 25 items questionnaire was used titled “Teachers’ Class Size Questionnaire” (TCSQ). The questionnaire has two sections; section A contained respondents’ bio-data while B contained items on class size. All items are on 4-point scale namely; strongly Agree (SA) = 4, Agree (3), Disagree (D), Strongly Disagree (SD). Both reliability and validity of the instrument was ensured. One general question and research hypotheses were formulated and answered. Data were analyzed using simple percentage and chi-square. The result showed that there was a significant difference in the teaching and learning of pupils in small and large classes. It was recommended that policy makers and government should ensure that more classrooms are built and number of pupils in a class should not be more than 30. The Parent-Teacher Association (PTA), philanthropists and other charitable organizations are also implored to compliment the effort of the government to boost the teaching and learning of social studies by building more classrooms and buildings. 11. Optimization and Isothermal Studies of Antibiotics Mixture Biosorption From Wastewater Using Palm Kernel, Chrysophyllum albidum, and Coconut Shells Biocomposite

Abstract: The presence of persistent pharmaceutical products in water bodies is a significant problem that obstructs wastewater reuse. This study investigated the adsorption process for removing the recalcitrant antibiotics, including tetracycline (TC), ampicillin (AMP), and amoxicillin (AMOX) from an aqueous solution using a composite biosorbent made from a mixture of palm kernel shell (PKS), Chrysophyllum albidum (CAS), and coconut shell (CS). Simplex centroid design in the Design of Expert (12.0.1.0) was applied to optimize the percentage composition (20-55%) of the composite biosorbent precursor and to remove TC-AMP-AMOX mixtures from the aqueous solution in a batch study. The equilibrium data were fitted to 12 isotherm models and analyzed statistically. The maximum adsorption capacity of 9.12 mg/g, 8.66 mg/g, and 7.11 mg/g was achieved for TC, AMP, and AMOX, respectively, using the biocomposite biosorbent with an optimal mixture of 55% PKS, 20% CAS, and 25% CS. The adsorption behavior of TC, AMP, and AMOX was well-described by the Langmuir/Elovich isotherm (R2=1.000), Hill-DeBoer (R2=0.9953), and Freundlich/ Halsey (R2=0.9898) models, respectively. The obtained results showed that the biocomposite PKS-CAS-CS leverages the individual adsorptive capacity of each constituent to enhance the adsorption process. Moreover, the composite biosorbent demonstrated excellent potential for removing recalcitrant pharmaceuticals from wastewater effectively.

Abstract: Background: Patients with acute respiratory failure caused by cardiogenic pulmonary edema (CPE) may require mechanical ventilation that can cause further lung damage. Our aim was to determine the impact of ventilatory settings on CPE mortality. Methods: Patients from the LUNG SAFE cohort, a multicenter prospective cohort study of patients undergoing mechanical ventilation, were studied. Relationships between ventilatory parameters and outcomes (ICU discharge/hospital mortality) were assessed using latent mixture analysis and a marginal structural model. Results: From 4499 patients, 391 meeting CPE criteria (median age 70 [interquartile range 59–78], 40% female) were included. ICU and hospital mortality were 34% and 40%, respectively. ICU survivors were younger (67 [57–77] vs 74 [64–80] years, p < 0.001) and had lower driving (12 [8–16] vs 15 [11–17] cmH2O, p < 0.001), plateau (20 [15–23] vs 22 [19–26] cmH2O, p < 0.001) and peak (21 [17–27] vs 26 [20–32] cmH2O, p < 0.001) pressures. Latent mixture analysis of patients receiving invasive mechanical ventilation on ICU day 1 revealed a subgroup ventilated with high pressures with lower probability of being discharged alive from the ICU (hazard ratio [HR] 0.79 [95% confidence interval 0.60–1.05], p = 0.103) and increased hospital mortality (HR 1.65 [1.16–2.36], p = 0.005). In a marginal structural model, driving pressures in the first week (HR 1.12 [1.06–1.18], p < 0.001) and tidal volume after day 7 (HR 0.69 [0.52–0.93], p = 0.015) were related to survival. Conclusions: Higher airway pressures in invasively ventilated patients with CPE are related to mortality. These patients may be exposed to an increased risk of ventilator-induced lung injury. Abstract: Background Human genetics research lacks diversity; over 80% of genome-wide association studies have been conducted on individuals of European ancestry. In addition to limiting insights regarding disease mechanisms, disproportionate representation can create disparities preventing equitable implementation of personalized medicine. Objective This systematic review provides an overview of research involving Parkinson's disease (PD) genetics in underrepresented populations (URP) and sets a baseline to measure the future impact of current efforts in those populations. Methods We searched PubMed and EMBASE until October 2021 using search strings for "PD," "genetics," the main "URP," and and the countries in Latin America, Caribbean, Africa, Asia, and Oceania (excluding Australia and New Zealand). Inclusion criteria were original studies, written in English, reporting genetic results on PD from non-European populations. Two levels of independent reviewers identified and extracted information. Results We observed imbalances in PD genetic studies among URPs. Asian participants from Greater China were described in the majority of the articles published (57%), but other populations were less well studied; for example, Blacks were represented in just 4.0% of the publications. Also, although idiopathic PD was more studied than monogenic forms of the disease, most studies analyzed a limited number of genetic variants. We identified just nine studies using a genome-wide approach published up to 2021, including URPs. Conclusion This review provides insight into the significant lack of population diversity in PD research highlighting the immediate need for better representation. The Global Parkinson's Genetics Program (GP2) and similar initiatives aim to impact research in URPs, and the early metrics presented here can be used to measure progress in the field of PD genetics in the future. Abstract: The orthodox conceptualisation of development and its reforms which are rooted in Western schools of thought recognizes the existence of three categories of polities, viz: developed, developing and underdeveloped states. But the major questions that are of concern are: on what grounds do we submit that a country is developed, and the other is either developing or underdeveloped? Using a historical method of research and utilising mainly secondary sources of data, the paper attempts to answer these salient issues by critiquing the orthodox western development theories and reforms, citing a wide range of instances of faulty development reforms imposed on the global south by the West. The study found that the western driven ̳development reforms or policies‘ for the governments of the global North are incompatible with the dynamics and interests of the South. It also found that the western driven ̳development reforms or policies‘ were never intended to sustain the economies of the global south or worsen the North-South dichotomy. The paper concludes that every society has its own sense of development and that the orthodox conception of development, and its reforms are not sacrosanct given the peculiarities of the global South. The paper also posits that western driven ̳development reforms or policies‘ are ̳neo-colonial schemes‘ targeted at strengthening the core-periphery arrangements between the global North and the global South.

16. Spatial-temporal Variation and Local Source Identification of Air Pollutants in a Semi-urban Settlement in Nigeria Using Low-cost Sensors

Abstract: Low-cost sensors were deployed at five locations in a growing, semi-urban settlement in southwest Nigeria between June 8 and July 31, 2018 to measure particulate matter (PM2.5 and PM10), gaseous pollutants (CO, NO, NO2, O3 and CO2), and meteorological variables (air temperature, relative humidity, wind speed and wind-direction). The spatial and temporal variations of measured pollutants were determined, and the probable sources of pollutants were inferred using conditional bivariate probability function (CBPF). Hourly PM2.5 and PM10 concentrations ranged from 20.7 ± 0.7 to 36.3 ± 1.6 µg m–3 and 47.5 ± 1.5 to 102.9 ± 5.6 µg m–3, respectively. Hourly gaseous pollutant concentrations ranged from 348 ± 132 to 542 ± 200 ppb CO, 21.5 ± 7.2 ppb NO2 and 57.5 ± 11.3 to 64.4 ± 14.0 ppb O3. Kruskal-Wallis ANOVA on ranks determined statistically significant spatial differences in the hourly-average pollutant concentrations. Diel variation analyses indicated that CO2, PM2.5, and PM10 peaked in the early hours of most days, O3 at noon while NO, NO2, and CO peaked in the evening. Most pollutants were of anthropogenic origins and exhibited the highest contributions from the southwest at most sampling locations. There were strong similarities between pollutants source contribution at two of the monitoring sites that were in residential areas with a frequently used paved road. Mitigation strategies need to be established to avoid further deterioration of ambient air quality that negatively affect public health.

Abstract: The Food and Drug Administration (FDA) has been regulating human islets for allotransplantation as a biologic drug in the US. Consequently, the requirement of a biological license application (BLA) approval before clinical use of islet transplantation as a standard of care procedure has stalled the development of the field for the last 20 years. Herein, we provide our commentary to the multiple FDA's position papers and guidance for industry arguing that BLA requirement has been inappropriately applied to allogeneic islets, which was delivered to the FDA Cellular, Tissue and Gene Therapies Advisory Committee on 15 April 2021. We provided evidence that BLA requirement and drug related regulations are inadequate in reassuring islet product quality and potency as well as patient safety and clinical outcomes. As leaders in the field of transplantation and endocrinology under the "Islets for US Collaborative" designation, we examined the current regulatory status of islet transplantation in the US and identified several anticipated negative consequences of the BLA approval. In our commentary we also offer an alternative pathway for islet transplantation under the regulatory framework for organ transplantation, which would address deficiencies of in current system.

19. Novel Dry Particle Layering for Drug Delivery to the Lungs

Abstract: Formulation of dry powder based delivery system using existing technologies such as high shear blending and TURBULA® mixing lacks predictability, scalability and involves extensive trial and improvement to achieve uniform powder blends. This challenge is compounded when dealing with drugs that are heat and moisture sensitive, fracture during blending, which requires extensive modifications such as temperature jackets and product quarantine respectively. Following the successful development of a novel aerosolized and isothermal dry particle coating device (iDPC) for the formation of composite particles by a previous work in this research group, this work aimed to develop a mechanistic understanding of the iDPC and explore its use in the formulation of dry powders for inhalation. There are three main sub-divisions in this work; the mechanistic understanding of the novel iDPC device, to develop a scientific rationale for its processes, determination of changes to carrier surface properties by particle surface characterisation techniques, and a Quality by design approach based on the mechanistic understanding obtained to develop dry powders for inhalation for pulmonary drug delivery. The capability of the novel iDPC device to produce composite particles were directly linked to its centrifugal force and micro fluidization technique, which provide the blending energy input. The stages of dry particle coating as already established in literature i.e. deagglomeration, dispersion and redistribution were identified in the novel iDPC device along with the periods at which they commence. The use of the micro fluidization technique was found to hasten these processes, although they still occurred in its absence. Abstract: The orthodox conceptualisation of development and its reforms which are rooted in Western schools of thought recognizes the existence of three categories of polities, viz: developed, developing and underdeveloped states. But the major questions that are of concern are: on what grounds do we submit that a country is developed, and the other is either developing or underdeveloped? Using a historical method of research and utilising mainly secondary sources of data, the paper attempts to answer these salient issues by critiquing the orthodox western development theories and reforms, citing a wide range of instances of faulty development reforms imposed on the global south by the West. The study found that the western driven ̳development reforms or policies‘ for the governments of the global North are incompatible with the dynamics and interests of the South. It also found that the western driven ̳development reforms or policies‘ were never intended to sustain the economies of the global south or worsen the North-South dichotomy. The paper concludes that every society has its own sense of development and that the orthodox conception of development, and its reforms are not sacrosanct given the peculiarities of the global South. Abstract: BACKGROUND Treatment for patients with recurrent ovarian cancer has been mainly based on systemic therapy. The role of secondary cytoreductive surgery is unclear. METHODS We randomly assigned patients with recurrent ovarian cancer who had a first relapse after a platinum-free interval (an interval during which no platinum-based chemotherapy was used) of 6 months or more to undergo secondary cytoreductive surgery and then receive platinum-based chemotherapy or to receive platinumbased chemotherapy alone. Patients were eligible if they presented with a positive Arbeitsgemeinschaft Gynäkologische Onkologie (AGO) score, defined as an Eastern Cooperative Oncology Group performance-status score of 0 (on a 5-point scale, with higher scores indicating greater disability), ascites of less than 500 ml, and complete resection at initial surgery. A positive AGO score is used to identify patients in whom a complete resection might be achieved. The primary end point was overall survival. We also assessed quality of life and prognostic factors for survival. RESULTS A total of 407 patients underwent randomization: 206 were assigned to cytoreductive surgery and chemotherapy, and 201 to chemotherapy alone. A complete resection was achieved in 75.5% of the patients in the surgery group who underwent the procedure. The median overall survival was 53.7 months in the surgery group and 46.0 months in the no-surgery group (hazard ratio for death, 0.75; 95% confidence interval, 0.59 to 0.96; P = 0.02). Patients with a complete resection had the most favorable outcome, with a median overall survival of 61.9 months. A benefit from surgery was seen in all analyses in subgroups according to prognostic factors. Quality-of-life measures through 1 year of follow-up did not differ between the two groups, and we observed no perioperative mortality within 30 days after surgery. One antidiabetic therapeutic approach is to reduce gastrointestinal glucose production and absorption through the inhibition of carbohydrate digesting enzymes such as alpha-amylase as well as through the inhibition of hemoglobin glycosylation. Objective: This study sets out to evaluate the in vitro antidiabetic activity of Ziziphus mucronata extracts for their effect on alpha-amylase and glycosylation of hemoglobin. Methods: Successive gradient maceration of Z. mucronata leaves were carried out using Hexane, Acetone, Methanol and separately with water to obtain four (4) extracts labelled HE, AE, ME, and WE respectively. These were subjected to in vitro studies for their inhibitory effect on alpha-amylase and hemoglobin glycosylation, Standard laboratory methods were used to screen for phytochemicals of the most potent extract. Results: The result showed that AE, ME and WE extract exhibited a dose-dependent increase in percentage inhibition of both alpha-amylase and hemoglobin glycosylation. However, on a stricking note, the AE showed a more potent data result with percentage (%) potency of 71.02 at 1mg/ml, the lowest glucose concentration (of 25mg/ml) at 0.242nm as well as the highest hemoglobin glycosylation inhibitory mean concentration of 3.663nm after 72 hours. The AE of Z. mucronata (the most potent) revealed the presence of alkaloids, anthraquinone, glycosides, flavonoids, phenols, saponin, tanins and terpenoids. Conclusion: Thus, the Acetone extract is more likely to give a lead antidiabetic drug molecule of drug when further explored; which somewhat justify the folkloric claims of Z. mucronata leave as an antidiabetic. Abstract: The study investigates the challenges on the use of electronic and print resources by students in selected tertiary institutions in Kwara State, Nigeria. In order to achieve the set objectives, the researcher adopted survey research design using questionnaire as the major instrument for data gathering and complimented by observation. The total population for the study was 7, 954 from which 150 students were sampled. The study finds out that undergraduate students use information resources for academic related purposes; print information resources were more available for students use in the academic libraries; the findings further indicated that most libraries do not provide electronic information resources; the challenges facing include lack of awareness of available resources, obsolete books and lack and lack of adequate relevant books. Some of the recommendations include: institutions should improve more on the information resources provided in their various libraries; intensive training in print and electronic information resources; the libraries should provide current and adequate print information resources for the programmes in their various institutions among others

Abstract: Purpose: The study was carried out to investigate the effect of mobile apps on students’ achievement in petrol engine maintenance work in technical colleges in Enugu State, Nigeria. Approach/Methodology/Design: The study adopted quasi-experimental design and was carried out in Enugu State, Nigeria. The population for the study was 170 NTC II students. The sample size for the study was 40 motor vehicle mechanics work NTC II students who have any mobile technologies (Smart phones) participated in the experimental group and were selected using purposive sampling technique. The instrument for data collection was motor vehicle mechanic work achievement test (MWMWAT). The data collected were analyzed using mean, to answer the research questions while ANCOVA was used to test the hypothesis. Three research questions were answered, while one null hypothesis formulated were tested at 0.05 level of significance. Findings: The study revealed that students taught petrol vehicle maintenance works with mobile apps had a higher mean achievement score than those students taught using conventional teaching aids in the achievement test. There was a significant difference between the mean achievement scores of students taught with mobile apps and those taught with conventional teaching aids in motor vehicle mechanics work trade. Practical Implications: The study will contribute positively to the understanding of using mobile apps to increase students’ motivation and academic achievement. Originality/value: This study innovates in examining the use of mobile apps by motor vehicle mechanic work teachers in improving the motivation and academic achievement of students.

To contribute to the body of knowledge that addresses this gap, this paper examines literature on disciplinary types and team performance. Using an analytic framework identified in literature, this paper studies the organizational and social aspects of building design practice in order to shed light on the ways in which the multiple disciplines involved building design work together. Findings presented in this paper suggest that building design teams combine and integrate knowledge, skills and capabilities in a multidisciplinary manner. In addition, this paper discusses four social and organizational characteristics of multidisciplinary building design teams – the project delivery approach, disciplinary roles, preexisting social and professional relationships, and location and geographic proximity – and documents their impacts on team functioning. &nbsp

Abstract: IntroductionPalliative care is a clinically and cost-effective component of cancer services in sub-Saharan Africa (SSA). Despite the significant need for palliative cancer care in SSA, coverage remains inadequate. The exploration of digital health approaches could support increases in the quality and reach of palliative cancer care services in SSA. However, there is currently a lack of any theoretical underpinning or data to understand stakeholder drivers for digital health components in this context. This project addresses this gap through engaging with key stakeholders to determine data and information needs that could be supported through digital health interventions.Methods and analysisThis is a multicountry, cross-sectional, qualitative study conducted in Nigeria, Uganda and Zimbabwe. In-depth interviews will be conducted in patients with advanced cancer (n=20), caregivers (n=15), health professionals (n=20) and policy-makers (n=10) in each of the three participating countries. Data from a total of 195 interviews will transcribed verbatim and translated into English before being imported into NVivo software for deductive framework analysis. The analysis will seek to understand the acceptability and define mechanisms of patient-level data capture and usage via digital technologies.Ethics and disseminationEthics approvals have been obtained from the Institutional Review Boards of University of Leeds (Ref: MREC 18-032), Research Council of Zimbabwe (Ref: 03507), Medical Research Council of Zimbabwe (Ref: MRCZ/A/2421), Uganda Cancer Institute (Ref: 19-2018), Uganda National Council of Science and Technology (Ref: HS325ES) and College of Medicine University of Lagos (Ref: HREC/15/04/2015). Abstract: Background: Pregnant women in malaria-endemic areas are at high risk of Plasmodium falciparum infection and its complications. This study investigated the impact of asymptomatic P. falciparum on haematological parameters of pregnant women at first antenatal visit in south-western Nigeria. Methods: Hospital-based cross-sectional evaluation of 130 asymptomatic pregnant women was conducted. Plasmodium infection was diagnosed using Giemsa-stained blood smear microscopy and rapid diagnostic test, while haemoglobin levels (Hb), Packed Cell Volume (PCV), white blood cells (WBC) count, red blood cell (RBC), mean corpuscular volume (MCV), mean corpuscular haemoglobin (MCH) and mean corpuscular haemoglobin concentration (MCHC) were determined using semi-automated haematology analyser. Results: Multigravid pregnant women presented more during their second trimester. Overall malaria prevalence in the sampled population was 14.6%. Prevalence of malaria parasitaemia was highest (68.4%) in the second trimester. P. falciparum infected pregnant women had significantly lower mean values of packed cell volume (PCV), haematocrit (Hb), and total white blood cell (WBC) compared to non-infected individuals (t = -4.07, p= 0.001; t = 3.30, p = <0.001; t = -2.77, p <0.001). Conclusion: There is asymptomatic P. falciparum infection in the study area and this may form a reservoir for transmission. Pregnant women infected with malaria parasites exhibited important changes in haematocrit level, haemoglobin concentration, and total white blood cells. Use of anti-malaria therapy and insecticide-treated bed net would result in greater haematological benefits.

Abstract: Purpose: To evaluate patients with resolved versus confirmed ARDS, identify subgroups with substantial mortality risk, and to determine the utility of day 2 ARDS reclassification. Methods: Our primary objective, in this secondary LUNG SAFE analysis, was to compare outcome in patients with resolved versus confirmed ARDS after 24 h. Secondary objectives included identifying factors associated with ARDS persistence and mortality, and the utility of day 2 ARDS reclassification. Results: Of 2377 patients fulfilling the ARDS definition on the first day of ARDS (day 1) and receiving invasive mechanical ventilation, 503 (24%) no longer fulfilled the ARDS definition the next day, 52% of whom initially had moderate or severe ARDS. Higher tidal volume on day 1 of ARDS was associated with confirmed ARDS [OR 1.07 (CI 1.01–1.13), P = 0.035]. Hospital mortality was 38% overall, ranging from 31% in resolved ARDS to 41% in confirmed ARDS, and 57% in confirmed severe ARDS at day 2. In both resolved and confirmed ARDS, age, non-respiratory SOFA score, lower PEEP and P/F ratio, higher peak pressure and respiratory rate were each associated with mortality. In confirmed ARDS, pH and the presence of immunosuppression or neoplasm were also associated with mortality. The increase in area under the receiver operating curve for ARDS reclassification on day 2 was marginal. Conclusions: ARDS, whether resolved or confirmed at day 2, has a high mortality rate. ARDS reclassification at day 2 has limited predictive value for mortality. The substantial mortality risk in severe confirmed ARDS suggests that complex interventions might best be tested in this population. Trial Registration: ClinicalTrials.gov NCT02010073

Abstract: We propose a nonparallel data-driven emotional speech conversion method. It enables the transfer of emotion-related characteristics of a speech signal while preserving the speaker's identity and linguistic content. Most existing approaches require parallel data and time alignment, which is not available in most real applications. We achieve nonparallel training based on an unsupervised style transfer technique, which learns a translation model between two distributions instead of a deterministic one-to-one mapping between paired examples. The conversion model consists of an encoder and a decoder for each emotion domain. We assume that the speech signal can be decomposed into an emotion-invariant content code and an emotion-related style code in latent space. Emotion conversion is performed by extracting and recombining the content code of the source speech and the style code of the target emotion. We tested our method on a nonparallel corpora with four emotions. 38. Effects of Gender and School Location on the Ekiti State Secondary Schools Students’ Achievement in Reading Comprehension in English Language

Abstract: The purpose of this study was to find out the effects of gender and school location on the Ekiti State secondary school students achievement in reading comprehension in English language. The study adopted pre-test, post-test and control quasi-experimental research using two experimental groups and one control group. The sample for the study comprised 270 Senior Secondary class two students (SSSII) selected from nine public senior secondary schools across the three senatorial district of the State using multi-stage sampling procedure that incorporated stratified random sampling and purposive sampling (with sex and location of school as stratification variables). The selected schools were assigned into three major groups, with each group made up of three schools of 30 students per school (two experimental groups and one control group). The instruments adapted for the study were: Morphological Production Task on Students’ Academic Ability (r = 0.74), Morphological Analysis Task of Students’ Academic Ability (r = 0.83) and Achievement Test in Comprehension (0.75). Experimental group 1 and 2 were taught with (Problem-Solving and Eclectic methods) with Morphological Instructional Packages respectively; while Conventional Teaching Method Guide was used by the research assistants to teach the control group. Data collected were analysed using Mean, Standard Deviation, Analysis of Covariance, (ANCOVA), Scheffe post-hoc and Multiple Classification Analysis (MCA). The study showed that there was no significant difference in the achievement of males and females between the experimental and control group. The study found that students in urban schools performed better than the rural schools in reading comprehension, inferring word meanings between the experimental and control groups. 40. Effects of Gender and School Location on the Ekiti State Secondary Schools Students’ Achievement in Reading Comprehension in English Language

Abstract: The purpose of this study was to find out the effects of gender and school location on the Ekiti State secondary school students achievement in reading comprehension in English language. The study adopted pre-test, post-test and control quasi-experimental research using two experimental groups and one control group. The sample for the study comprised 270 Senior Secondary class two students (SSSII) selected from nine public senior secondary schools across the three senatorial district of the State using multi-stage sampling procedure that incorporated stratified random sampling and purposive sampling (with sex and location of school as stratification variables). The selected schools were assigned into three major groups, with each group made up of three schools of 30 students per school (two experimental groups and one control group). The instruments adapted for the study were: Morphological Production Task on Students’ Academic Ability (r = 0.74), Morphological Analysis Task of Students’ Academic Ability (r = 0.83) and Achievement Test in Comprehension (0.75). Experimental group 1 and 2 were taught with (Problem-Solving and Eclectic methods) with Morphological Instructional Packages respectively; while Conventional Teaching Method Guide was used by the research assistants to teach the control group. Data collected were analysed using Mean, Standard Deviation, Analysis of Covariance, (ANCOVA), Scheffe post-hoc and Multiple Classification Analysis (MCA). The study showed that there was no significant difference in the achievement of males and females between the experimental and control group. The study found that students in urban schools performed better than the rural schools in reading comprehension, inferring word meanings between the experimental and control groups. 41. Outcome of patients with advanced ovarian cancer who do not undergo debulking surgery: A single institution retrospective review

Abstract: Objective To assess the outcome of patients with advanced ovarian cancer (OC) who were treated without surgery, having received upfront chemotherapy and no interval debulking surgery (IDS). Methods Retrospective analysis of medical and chemotherapy records of consecutive patients with OC between 2005 and 2013 at UCL Hospitals London, UK who received neoadjuvant chemotherapy (NACT) was then found to be unsuitable for IDS following review by the multidisciplinary team. Results Eighty-three patients (18%) out of 467 receiving NACT did not undergo IDS. Median age was 70 years (range 33–88); out of these 83 patients, 43 (51.8%) presented with stage IV disease. Forty-three of these 83 patients received carboplatin and paclitaxel (CP) (51.8%) and 37 received carboplatin alone (C) (44.6%); 3 patients (3.6%) received other platinum-based combinations. Reasons for not proceeding to surgery were: poor response to chemotherapy after 3–4 cycles of NACT (61/83, 73.5%); comorbidities (12/83, 14.5%); patient decision (4/83, 4.8%). Six patients (7.2%) received < 3 cycles of NACT due to a worsening clinical condition. The median overall survival (OS) for patients not undergoing IDS was 18 months (95% CI 10–20 months). Forty-four of 83 patients (53%) received > 2 lines of chemotherapy. In a univariate analysis CP, age < 70 years, and absence of comorbidities were factors influencing OS. In a multivariate analysis only having received CP remained independently associated with OS (HR 0.49, 95% CI 0.29–0.84). Conclusions Chemotherapy alone can provide reasonable disease control in patients unsuitable for IDS and CP should be used if possible.

The appropriateness and clinical outcomes of these referrals were evaluated and compared with cancers diagnosed from other sources in the same hospital over the same time period. This was a retrospective review of RAC gynecological cancer referrals to University College London Hospital in 2012. Clinical and demographic details were collected from the Clinical Data Repository. Statistics were calculated with MeasuringU software. Among 335 women referred to the RAC, 14 (4.2%) had cancer. Most women had benign pathology (80.6%). This same year, 13 cancers were diagnosed elsewhere, including in the emergency department. A total of 172 referrals did not fulfill the guidelines. The apparent positive predictive value (PPV) for the referrals was 0.042. If appropriately followed, the PPV for guidelines would be 0.091. The apparent PPV of the current referral system for urgent gynecological cancers is low. Greater discretion in primary care and proper use of referral guidelines are required. Avoiding inappropriate referrals can double the PPV, leading to cost savings for the National Health Service. 43. Performance Model of Reliable Hybrid Multicast Protocol (RHMP)

Abstract: The Pragmatic General multicast (PGM) and Elastic Reliable Multicast (ERM) are reliable multicast protocols. The difference between reliable and unreliable multicast protocol is that they make sure that the multicast data packets gets to its destinations. Both the PGM and ERM sends flood messages to the Rendezvous Point source (RPS) from the source node towards the stub nodes which then forward it to leaf nodes, leaf nodes that are not interested sends a prune message while any leaf node that misses a packet sends a message to the RPS through the stub node requesting for the multicast packet. A repair multicast packet is then forwarded to all leaf nodes that requested for it. In the reliable hybrid multicast protocol (RHMP) being proposed the stub nodes originates the flood message to the leaf and uninterested leaf sends prune message, any stub that has one or more interested leaf sends a join message to the RPS. If a leaf node in the multicast distribution misses a multicast packet it requests a repair packet from its stub node and the stub node sends the repair data. A simulation model was developed to mimic the behaviour of PGM, ERM and RHMP in different network size using hierarchical network and the control bandwidth overhead (CBO) for each of the multicast protocols was calculated, the CBO was use as the cost metric. The result shows that the RHMP uses less CBO than PGM and ERM in a sparsely and densely populated network. For state storage it was discovered that the RHMP uses more resources at the stub nodes than at the source / RPS or leaf node when compared with PGM and ERM, but since the stub nodes are present in a distributed way it does not necessarily affect the multicast process.

44. Emergence of colistin-resistant bacteria in humans without colistin usage: a new worry and cause for vigilance

Abstract: Colistin is currently regarded as one of the 'last-resort' antibiotics used for the treatment of critical infections caused by multidrug-resistant Gram-negative pathogens. There have been numerous reports of the emergence of colistin resistance in patients, most of whom had previously received colistin therapy or with acquisition via nosocomial transmission. However, there are also ample reports of colistin resistance in humans who have not received the drug previously or without nosocomial transmission. We have also observed a similar occurrence in our study involving colistin resistance from several countries along with a similar phenomenon being reported by researchers. The observation of colistin resistance in humans without prior colistin exposure is of particularly great clinical importance and concern because of the current importance of colistin in clinical medicine. Colistin use and colistin-resistant bacteria in animals have been recently reported, suggesting that animals could also be a source of transmission of colistin- resistant bacteria to humans. This is a real worry and calls for clinicians to be aware and vigilant of this phenomenon and of the possibility of independent resistance to colistin in some patients. (Résumé d'auteur)

45. The Informal Sector and Tax Revenue Drive: A Nigerian Case Study

Abstract: There has been a recent need to boost internally generated revenue in Nigeria due to the fall in the price of oil, upon which Nigeria has been dependent for revenue. The Nigerian government's attention has been drawn to the massive size of the informal sector from which little or no taxes are being collected. To resolve this discrepancy, it plans to introduce a presumptive tax regime aimed at taxing the firms in the informal sector. In this paper, I analyze the effect of different presumptive taxation policies on the size of the informal sector, tax revenue, output and welfare. Results show that an assessment in the form of proportional presumptive tax seems to be more efficient than a uniform lump-sum presumptive tax. However, I also show that in certain cases, the optimal policy for a government that cares about inequality would be a proportional presumptive tax with size-based "tax holiday" for informal firms who operate with less than a prescribed capital threshold.

47. Association between self-esteem and smoking among religous school students in Petaling District, Selangor

Abstract: Introduction: According to the Youth Behaviour Risk Factor Surveillance (YBRFSS, 2011) in Malaysia, the prevalence of smokers nationally among Form 1, 2 and 4 students government secondary school students was 9.1% (ever smokers) and 8.7% (current smokers). However, there is limited information on the prevalence of smoking among religious secondary school students and self-esteem as risk factor. Thus, this study was conducted to determine the prevalence and factors associated with smoking among the religious secondary school students in Petaling district, Selangor. Methods: A cross-sectional study was conducted among 899 students. The schools were selected using cluster sampling and self-administered questionnaire was used. Data was analyzed using chi-square test and multivariate logistic regression to identify the independent factors for smoking. Results: The response rate was 94%. The prevalence of ever smoking was 10.1%; 3.9% among female and 18.6% among male. The median age (interquartile range) of smoking initiation was 12.00 (4.01) years. The results showed that independent factors for smoking were: males (OR= 5.47); age group 14-15years (OR=2.82) and 16-17 years (OR=3.63) and having low self -esteem (OR=6.24). Conclusion: prevalence of ever smokers is higher when compared to YBRFSS, (2011) and it was revealed that most of the smokers started smoking even before secondary school. The results also revealed the importance of promoting self-esteem. Efforts in smoking prevention should therefore be focused on curbing the initiation of the acts and improving students’ self-worth.

48. A framework for creating production and inventory control strategies

Abstract: In multiproduct manufacturing systems, it is difficult to assure that an optimised setting of a pull production control strategy will be able to maintain its service level and inventory control performances. This is because the competition for resources among products is liable to make them affect the service levels of one another. By comparing different pull strategies, this research has observed that tightly coupled strategies are able to maintain lower amount of inventory than decoupled strategies, but they do so at the detriment of service level robustness. As a result, tightly coupled strategies are better suited to manufacturing environments with low variability, while decoupled strategies are more robust in high variability environments. Here, robustness is a measure of how well a strategy is able to minimise the drop below its original optimised service level when the initial system conditions change. Furthermore, the Kanban allocation policy applied under a strategy plays a major role in its ability to manage the performances of multiple products. Experimental results show that the Shared Kanban Allocation Policy (SKAP) keeps a lower amount of inventory than the Dedicated Kanban Allocation Policy (DKAP), but it is more susceptible to the variability in the demand or processing times of one product impacting the service level of another. Therefore, a Hybrid Kanban allocation policy (HKAP) that combines both the DKAP and the SKAP has been implemented. This approach considers products’ demand and processing time attributes before categorising them into the same Kanban sharing group. The results of the implementation of the HKAP show that it can keep as low inventory as the SKAP and avoid products impacting the service levels of one another. Additionally, it offers a better approach to managing large multiproduct systems, as the performances of product groups can be differentially managed through the combination of Kanban sharing and dedication policies.
