Introduction: Effects of the birthing position on perineal trauma, which depend on local and regional midwifery/obstetric practices, such as application of manual perineal protection technique, episiotomy rates, operative delivery rates etc., have not yet been sufficiently researched. The aim of this study was to examine a potential association between birthing positions and the risk of perineal trauma in a Slovenian hospital.Methods: This retrospective cohort study included 625 women with singleton fetuses who delivered at the Jesenice General Hospital in 2021. We excluded premature deliveries, deliveries with a fetus in the pelvic or occipito-posterior insertion, vacuum extractions of the fetus, and episiotomies. Multiple logistic regression analysis was used to investigate whether the birthing position (supine (n=66), lateral (n=404), or other position (n=155)) was independently associated with perineal trauma.Results: In the majority of women included in the study (n=363, or 58%), no perineal trauma occurred. First-degree perineal tear was present in 192 women (73%), second-degree in 59 women (23%) and third- or fourth-degree in 11 women (4%). The birthing position was not significantly associated with perineal trauma (odds ratio (OR) 0.691; 95% confidence interval (CI) 0.401–1.191 for the lateral position, and OR 0.710; 95% CI 0.386–1.306 for other positions compared with the supine position).Discussion and conclusion: In a Slovenian maternity unit with a high percentage of women delivering in "flexible sacrum positions", the birthing position at delivery was not found to be an independent risk factor for perineal trauma., Uvod: Vpliv porodnega položaja na tveganje za poškodbo presredka, ki je odvisen od lokalnih in regionalnih babiških praks, kot so ročna tehnika zaščite presredka, razne stopnje epiziotomije in operativnih porodov itd., še ni zadostno raziskan. Namen raziskave je bil v eni od slovenskih porodnišnic preučiti morebitno povezavo med porodnim položajem ob iztisu ploda in pojavnostjo poškodbe presredka.Metode: V retrospektivno kohortno raziskavo smo vključili 625 porodnic, ki so leta 2021 rodile v Splošni bolnišnici Jesenice. Izključili smo večplodne nosečnosti, prezgodnje porode, porode s plodom v medenični ali okcipito-posteriorni vstavi, vakuumske ekstrakcije ploda in epiziotomije. Z multiplo logistično regresijo smo preučili, ali je bil položaj porodnice ob iztisu ploda (leže na hrbtu (n = 66), leže na boku (n = 404) ali vsi ostali položaji (n = 155)) neodvisno povezan s pojavnostjo poškodbe presredka.Rezultati:Pri 363 porodnicah (58 %) ni prišlo do poškodbe presredka. Pri 192 (73 %) je prišlo do poškodbe prve stopnje, pri 59 (23 %) do poškodbe druge stopnje in pri 11 (4 %) do poškodbe presredka tretje ali četrte stopnje. Porodni položaj ob iztisu ploda ob upoštevanju drugih potencialnih dejavnikov tveganja ni bil statistično pomembno povezan s tveganjem za poškodbo presredka: razmerje obetov (RO) 0,691, 95% interval zaupanja (IZ) – 0,401–1,191 za položaj na boku; RO 0,710, 95% IZ – 0,386–1,306 za ostale položaje v primerjavi s porajanjem leže na hrbtu.Diskusija in zaključek: V slovenski porodnišnici z visokim odstotkom porodnic, ki ne rodijo leže na hrbtu, porodni položaj ob iztisu ploda ni neodvisni dejavnik tveganja za nastanek poškodbe presredka

First births with vacuum extraction carry an increased risk of obstetric anal sphincter injuries (OASIS). Lateral or mediolateral episiotomy at vacuum extraction may decrease the risk of OASIS. Our aim was to assess whether lateral or mediolateral episiotomy, or OASIS, at vacuum extraction in nulliparous women is associated with prelabor cesarean delivery in the subsequent childbirth. Material and methods: This is a nationwide observational study using data from the Swedish Medical Birth Register, including women having a first birth with vacuum extraction and a second birth in 2000–2014. Both births were live, single, cephalic, ≥34 gestational weeks without malformations. The association between episiotomy or OASIS in the first birth and prelabor cesarean delivery in the second birth was examined using univariate and multivariate logistic regression with inverse probability of treatment weighting, and interaction analysis. Main outcome measure was prelabor cesarean delivery in the second birth. Results: In total, 44 656 women with vacuum extraction at their first birth were included. The rate of prelabor cesarean delivery in the second birth was 5.9% (824 of 13 950) in women with episiotomy, compared with 6.0% (1830 of 30 706) in women without episiotomy. Thus, women with episiotomy did not have an increased risk of prelabor cesarean delivery (adjusted odds ratio [aOR] 1.00, 95% confidence interval [95% CI] 0.83–1.20) compared with women without episiotomy. For comparison, the rate of prelabor cesarean delivery in the second birth was 20.6% (1275 of 6176) in women with OASIS, compared with 3.6% (1379 of 38 480) in women without OASIS (aOR 6.57, 95% CI 5.97–7.23). There was no interaction between episiotomy and OASIS. Conclusions: Lateral or mediolateral episiotomy at vacuum extraction in nulliparous women did not increase the risk of prelabor cesarean

The negative predictive value was 96.7% and the positive predictive value was only 9.9%. Comments were left on 175 examinations, of which 74% referred to the management of the examination. Conclusions: Using 3D-EAUS in a maternity ward is demanding because staff generally have little experience in endoanal ultrasound, which contributes to difficulties in obtaining good image quality. Introduction: Anal incontinence (AI) is a distressing and common condition for women. Women who sustain obstetric anal sphincter injuries (OASI) have been reported to have a higher prevalence of long-term AI than those who do not. However, the data is wide ranging, inconsistent and seldom comparative, with rates quoted varying between 13.1% and 74%. Methods: 139 women who sustained OASI at their first delivery and 44 women who did not (controls) were contacted at 6 weeks and then a mean of 8 years following their first delivery. They were asked to complete a questionnaire including the St Mark's Incontinence Score (SMIS) to assess the prevalence and severity of anal incontinence. Results: There was a high prevalence of anal incontinence (SMIS ≥ 1) in both women with OASI (62%) and controls (59%) (NS). The prevalence of severe AI (SMIS ≥ 6) was 27% in the OASI group and 18% in controls (NS). Higher grade tears and operative delivery were associated with higher rates of severe AI. In women with OASI, the mode of subsequent delivery did not impact the prevalence of AI. Conclusion: Women with OASI have a similar prevalence of long-term AI compared with women who do not sustain OASI. Some degree of AI is common 8 years post-partum, with our study demonstrating a high prevalence of 59% in controls. Brief summary: Women with and without OASI were contacted 5–15 years post-partum, and there was no difference in the rates of anal incontinence between the two groups.

We calculated the proportion of supposedly protective episiotomies (fulfilling criteria of a lateral or mediolateral episiotomy and a length >= 30 mm). We compared the results between obstetricians, gynecologists, and residents using Chi-square and Kruskal-Wallis tests for differences between groups, and logistic regression to estimate the odds ratio (OR) of drawing a protective episiotomy. Results: We received 432 responses. Doctors without a vacuum delivery in the past year were excluded, leaving 384 respondents for further analyses. In all, 222 (57.8%) doctors reported use of episiotomy in<50% of vacuum extractions. We obtained 308 illustrated episiotomies with a median angle of 53 degrees, incision point distance from the midline of 21 mm, and length of 36 mm, corresponding to a lateral episiotomy. Few doctors combined these parameters correctly resulting in 167 (54.2%) incorrectly drawn episiotomies. Residents drew shorter episiotomies than obstetricians and gynecologists. Doctors ranked episiotomy the least important intervention to prevent obstetric anal sphincter injuries in vacuum extraction. OBJECTIVE: Few studies have investigated women's experiences of daily life after childbirth complicated by obstetric anal sphincter injury (OASI). The aim of the present study was to explore experiences related to recovery, sexual function, relationships and coping strategies among women affected by OASI. METHODS: In-depth interviews were conducted using a purposive sample of 11 women affected by OASI. Women were interviewed 1-2 years after their first childbirth. Inductive qualitative content analysis was applied. RESULTS: The theme "From hell to healed" illustrates women's experiences of recovery, relationships and sexual function after OASI. Three categories addressing women's perceptions emerged: "Challenged to the core", "At the mercy of the care provider" and "For better or for worse". Support from partners and family and comprehensive care were important elements for the experiences of coping and healing from OASI. Elements that negatively influenced women's experiences were the pain and physical symptoms of pelvic floor dysfunction, normalization of symptoms by heath care providers, and unrealistic expectations about how this period in life should be experienced. CONCLUSION: OASI greatly affects women's experiences of their first years with their newborn child, relationships, social context and sexuality. For some women, OASI negatively affects everyday life for a long period after childbirth. However, others heal and cope quite quickly. Health care professionals need to identify and pay attention to women with persisting problems after OASI so that they can be directed to the right level of care.

Background: There is increased focus on obstetric anal sphincter injury (OASI) and there are several well-established risk factors such as birthweight, instrumental delivery and median episiotomy. Some studies have found increased risk of OASI in women delivering vaginally after a previous caesarean section (VBAC). Objective: To evaluate whether there is increased prevalence of OASI in VBAC compared with primiparous women. Search strategy: Literature search using PubMed, Embase and Cochrane databases. Selection criteria: All studies with data on both primiparous women and women undergoing VBAC were included. All included studies were evaluated using the “SIGN – methodology checklist” to verify if the quality was acceptable. Data collection and analysis: This systematic review included 23 articles conducted in 11 countries over 19 years. Included studies were analysed using RevMan version 5.4. Main results: We found increased prevalence of OASI in the VBAC group; 8.18% (95% confidence interval [CI] 8.07–8.29) compared with 6.59% (95% CI 6.56–6.62) in primiparous women. Correspondingly, the meta-analysis revealed increased prevalence for OASI in the VBAC group (odds ratio 1.27, 95% CI 1.10–1.47). We found a high level of heterogeneity (I2 = 98%). Conclusion: Women undergoing VBAC had a higher prevalence of OASI compared with primiparous women.

Background: There is increased focus on obstetric anal sphincter injury (OASI) and there are several well-established risk factors such as birthweight, instrumental delivery and median episiotomy. Some studies have found increased risk of OASI in women delivering vaginally after a previous caesarean section (VBAC). Objective: To evaluate whether there is increased prevalence of OASI in VBAC compared with primiparous women. Search strategy: Literature search using PubMed, Embase and Cochrane databases. Selection criteria: All studies with data on both primiparous women and women undergoing VBAC were included. All included studies were evaluated using the “SIGN – methodology checklist” to verify if the quality was acceptable. Data collection and analysis: This systematic review included 23 articles conducted in 11 countries over 19 years. Included studies were analysed using RevMan version 5.4. Main results: We found increased prevalence of OASI in the VBAC group; 8.18% (95% confidence interval [CI] 8.07–8.29) compared with 6.59% (95% CI 6.56–6.62) in primiparous women. Objectives: Correct episiotomy use and technique may prevent obstetric anal sphincter injuries. We aimed to explore the attitudes, use, and technique regarding episiotomy among doctors in Sweden, and their willingness to contribute to a randomized controlled trial of lateral episiotomy or no episiotomy in vacuum extraction in nulliparous women. Study design: A web-based survey was sent to members of the Swedish Society of Obstetrics and Gynecology (n = 2140). The survey included 31 questions addressing personal characteristics, use of episiotomy, a two-dimensional picture on which the respondents drew an episiotomy, and questions regarding attitudes towards episiotomy and participation in a randomized controlled trial. We calculated the proportion of supposedly protective episiotomies (fulfilling criteria of a lateral or mediolateral episiotomy and a length >= 30 mm). We compared the results between obstetricians, gynecologists, and residents using Chi-square and Kruskal-Wallis tests for differences between groups, and logistic regression to estimate the odds ratio (OR) of drawing a protective episiotomy. Results: We received 432 responses. Doctors without a vacuum delivery in the past year were excluded, leaving 384 respondents for further analyses. In all, 222 (57.8%) doctors reported use of episiotomy in<50% of vacuum extractions. We obtained 308 illustrated episiotomies with a median angle of 53 degrees, incision point distance from the midline of 21 mm, and length of 36 mm, corresponding to a lateral episiotomy. Few doctors combined these parameters correctly resulting in 167 (54.2%) incorrectly drawn episiotomies. Residents drew shorter episiotomies than obstetricians and gynecologists. Doctors ranked episiotomy the least important intervention to prevent obstetric anal sphincter injuries in vacuum extraction. The aim of the present study was to explore experiences related to recovery, sexual function, relationships and coping strategies among women affected by OASI. METHODS: In-depth interviews were conducted using a purposive sample of 11 women affected by OASI. Women were interviewed 1-2 years after their first childbirth. Inductive qualitative content analysis was applied. RESULTS: The theme "From hell to healed" illustrates women's experiences of recovery, relationships and sexual function after OASI. Three categories addressing women's perceptions emerged: "Challenged to the core", "At the mercy of the care provider" and "For better or for worse". Support from partners and family and comprehensive care were important elements for the experiences of coping and healing from OASI. Elements that negatively influenced women's experiences were the pain and physical symptoms of pelvic floor dysfunction, normalization of symptoms by heath care providers, and unrealistic expectations about how this period in life should be experienced. CONCLUSION: OASI greatly affects women's experiences of their first years with their newborn child, relationships, social context and sexuality. For some women, OASI negatively affects everyday life for a long period after childbirth. However, others heal and cope quite quickly. Health care professionals need to identify and pay attention to women with persisting problems after OASI so that they can be directed to the right level of care.

We reported the association between ultrasound diagnosed OASIS and anal incontinence symptoms using relative risk (RR) with 95 % CI. Result(s): We included 103 studies involving 16,110 women. Of all women who delivered vaginally, OASIS were diagnosed on ultrasound in 26 % (95 %CI, 21-30, I2 = 91 %), and 19 % experienced anal incontinence (95 %CI, 14-25, I2 = 92 %). In women without clinical suspicion of OASIS (n = 3688), sphincter defects were observed in 13 % (10-17, I2 = 89 %) and anal incontinence experienced by 14 % (95 % CI: 6-24, I2 = 95 %). Following primary repair of OASIS, 55 % (46-63, I2 = 98 %) of 7549 women had persistent sphincter defect with 38 % experiencing anal incontinence (33-43, I2 = 92 %). There was a significant association between ultrasound diagnosed OASIS and anal incontinence (RR 3.74, 2.17-6.45, I2 = 98 %). Interpretation(s): Women and clinicians should be aware of the high risk for sphincter defects following vaginal delivery even when clinically unsuspected. This underlines the need of careful and systematic perineal assessment after birth to mitigate the risk of missing OASIS. Introduction and hypothesis: Postpartum anal incontinence is common and distressing for women. We sought to look at the prevalence of anal incontinence in women who sustained obstetric anal sphincter injuries (OASI) compared with women who did not, and factors leading to these symptoms during the postpartum period. Methods: A total of 129 primiparous women sustaining OASI were compared with 131 women who did not (controls). They were contacted at approximately 6–10 weeks postpartum to obtain information on their symptoms of anal incontinence (AI). The data underwent univariate and multivariate analysis. Results: There was no difference in the prevalence of AI symptoms, occurring in 30% of women with OASI, and 23% of women without at 6–10 weeks postpartum; however, in women with high-grade tears the prevalence was 59%. Severe OASI (grade 3c and 4) was associated with an increased prevalence of both AI and severe AI, whereas forceps delivery and increasing maternal age were associated with an increased prevalence of severe AI only. Conclusion: Women with less severe (grade 3a and 3b) OASI do not experience a higher prevalence of AI than women without OASI in the postpartum period. Higher grade (3c and 4) tears, forceps delivery and increasing maternal age are associated with higher rates of AI. These factors should be avoided where possible to reduce postpartum AI. All women should be warned of the 23–30% chance of experiencing some mild AI in this period. Whether these symptoms are transient or long-lasting requires further investigation.

Introduction The number of women postponing childbirth until an advanced age is increasing. Our aim was to study the outcome of labor in nulliparous women >= 40 years, compared with women 25-29 years, after both spontaneous onset and induction of labor. Material and methods The nationwide population-based Swedish Medical Birth Register was used to study the perinatal outcome in nulliparous women with a singleton, term (gestational weeks 37-44), live fetus in cephalic presentation and a planned vaginal delivery from 1992 to 2011. We included 7796 nulliparous women >= 40 years and 264 262 nulliparous women 25-29 years. Prevalence and risk of intrapartum cesarean section, operative vaginal delivery, obstetric anal sphincter injury and a 5-minute Apgar score <7 were calculated for women >= 40 years stratified for spontaneous onset and induction of labor, using women 25-29 years as the reference in both strata. Crude and adjusted odds ratios (aOR) were calculated by unconditional logistic regression and presented with 95% confidence intervals (CI). Results Overall, 79% of women >= 40 years with a trial of labor reached a vaginal delivery. After spontaneous onset, intrapartum cesarean section was performed in 15.4% of women >= 40 years compared with 5.4% of women 25-29 years (aOR 3.07, 95% CI 2.81-3.35). Operative vaginal delivery was performed in 22.3% of women >= 40 years compared with 14.2% of women 25-29 years (aOR 1.71, 95% CI 1.59-1.85). After induction of labor, an intrapartum cesarean section was performed in 37.2% women >= 40 years compared with 20.2% women 25-29 years (aOR 2.51, 95% CI 2.24-2.81). Operative vaginal delivery was performed in 22.6% of women >= 40 years compared with 18.4% women 25-29 years (aOR 1.45, 95% CI 1.28-1.65).
