Introduction: The aim of this analysis was to investigate to what extent median cerebral perfusion pressure (CPP) differs between severe traumatic brain injury (TBI) patients and between centres, and whether the 2007 change in CPP threshold in the Brain Trauma Foundation guidelines is reflected in patient data collected at several centres over different time periods. Methods: Data were collected from the Brain-IT database, a multi-centre project between 2003 and 2005, and from a recent project in four centres between 2009 and 2013. For patients nursed with their head up at 30° and with the blood pressure transducer at atrium level, CPP was corrected by 10 mmHg. Median CPP, interquartile ranges and total CPP ranges over the monitoring time were calculated per patient and per centre. Results: Per-centre medians pre-2007 were situated between 50 and 70 mmHg in 6 out of 16 centres, while 10 centres had medians above 70 mmHg and 4 above 80 mmHg. Post-2007, three out of four centres had medians between 60 and 70 mmHg and one above 80 mmHg. One out of two centres with data pre- and post-2007 shifted from a median CPP of 76 mmHg to 60 mmHg, while the other remained at 68–67 mmHg. Conclusions: CPP data are characterised by a high inter-individual variability, but the data also suggest differences in CPP policies between centres. The 2007 guideline change may have affected policies towards lower CPP in some centres. Deviations from the guidelines occur in the direction of CPP > 70 mmHg.

The aim of this study is to assess visually the impact of duration and intensity of cerebrovascular autoregulation insults on 6-month neurological outcome in severe traumatic brain injury.

The definition of cerebral perfusion pressure (CPP) secondary insults in severe traumatic brain injury remains unclear. The purpose of the present study is to visualize the association of intensity and duration of episodes below or above CPP thresholds and outcome. The analysis was based on prospectively collected minute-by-minute intracranial pressure (ICP) and blood pressure data and outcome from 259 adult patients. The relationship of episodes of CPP below or above a certain threshold for certain duration with the 6-month Glasgow Outcome Score was visualized separately for episodes of active or deficient autoregulation (AR). In adults ≤65 years, an almost exponential transition curve separates the episodes of CPP associated with better outcomes from the episodes of low CPP associated with worse outcomes, indicating that lower CPP could only be tolerated for a brief time. Analysis of episodes of high CPP again showed a time-intensity dependent association with outcome. When combining the two plots, a safe CPP zone between 60 and 70 mm Hg could be delineated - however, only for AR active insults. The AR status predominantly affected the transition curve for insults of low CPP. Episodes with ICP >25 mm Hg were associated with poor outcome regardless of CPP. In the present study, the CPP pressure-time burden associated with poor outcome was visualized. A safe zone between 60 and 70 mm Hg could be identified for adults ≤65 years, provided AR was active and ICP was ≤25 mm Hg. Deficient AR reduces the tolerability for low CPP.

Objectives: RESCUEicp studied decompressive craniectomy (DC) applied as third-tier option in severe traumatic brain injury (TBI) patients in a randomized controlled setting and demonstrated a decrease in mortality with similar rates of favorable outcome in the DC group compared to the medical management group. In many centers, DC is being used in combination with other second/third-tier therapies. The aim of the present study is to investigate outcomes from DC in a prospective non-RCT context. Methods: This is a prospective observational study of 2 patient cohorts: one from the University Hospitals Leuven (2008–2016) and one from the Brain-IT study, a European multicenter database (2003–2005). In thirty-seven patients with refractory elevated intracranial pressure who underwent DC as a second/third-tier intervention, patient, injury and management variables including physiological monitoring data and administration of thiopental were analysed, as well as Extended Glasgow Outcome score (GOSE) at 6 months. Results: In the current cohorts, patients were older than in the surgical RESCUEicp cohort (mean 39.6 vs. 32.3; p < 0.001), had higher Glasgow Motor Score on admission (GMS < 3 in 24.3% vs. 53.0%; p = 0.003) and 37.8% received thiopental (vs. 9.4%; p < 0.001). Other variables were not significantly different. GOSE distribution was: death 24.3%; vegetative 2.7%; lower severe disability 10.8%; upper severe disability 13.5%; lower moderate disability 5.4%; upper moderate disability 2.7%, lower good recovery 35.1%; and upper good recovery 5.4%. The outcome was unfavorable in 51.4% and favorable in 48.6%, as opposed to 72.6% and 27.4% respectively in RESCUEicp (p = 0.02). Conclusion: Outcomes in DC patients from two prospective cohorts reflecting everyday practice were better than in RESCUEicp surgical patients. Purpose: To assess the impact of the duration and intensity of episodes of increased intracranial pressure on 6-month neurological outcome in adult and paediatric traumatic brain injury. Methods: Analysis of prospectively collected minute-by-minute intracranial pressure and mean arterial blood pressure data of 261 adult and 99 paediatric traumatic brain injury patients from multiple European centres. The relationship of episodes of elevated intracranial pressure (defined as a pressure above a certain threshold during a certain time) with 6-month Glasgow Outcome Scale was visualized in a colour-coded plot. Results: The colour-coded plot illustrates the intuitive concept that episodes of higher intracranial pressure can only be tolerated for shorter durations: the curve that delineates the duration and intensity of those intracranial pressure episodes associated with worse outcome is an approximately exponential decay curve. In children, the curve resembles that of adults, but the delineation between episodes associated with worse outcome occurs at lower intracranial pressure thresholds. Intracranial pressures above 20 mmHg lasting longer than 37 min in adults, and longer than 8 min in children, are associated with worse outcomes. In a multivariate model, together with known baseline risk factors for outcome in severe traumatic brain injury, the cumulative intracranial pressure–time burden is independently associated with mortality. When cerebrovascular autoregulation, assessed with the low-frequency autoregulation index, is impaired, the ability to tolerate elevated intracranial pressures is reduced. When the cerebral perfusion pressure is below 50 mmHg, all intracranial pressure insults, regardless of duration, are associated with worse outcome. Conclusions: The intracranial pressure–time burden associated with worse outcome is visualised in a colour-coded plot. The BaBar detector operated successfully at the PEP-II asymmetric e +e- collider at the SLAC National Accelerator Laboratory from 1999 to 2008. This report covers upgrades, operation, and performance of the collider and the detector systems, as well as the trigger, online and offline computing, and aspects of event reconstruction since the beginning of data taking.

Prostate cancer is a highly heritable disease with large disparities in incidence rates across ancestry populations. We conducted a multiancestry meta-analysis of prostate cancer genome-wide association studies (107,247 cases and 127,006 controls) and identified 86 new genetic risk variants independently associated with prostate cancer risk, bringing the total to 269 known risk variants. The top genetic risk score (GRS) decile was associated with odds ratios that ranged from 5.06 (95% confidence interval (CI), 4.84–5.29) for men of European ancestry to 3.74 (95% CI, 3.36–4.17) for men of African ancestry. Men of African ancestry were estimated to have a mean GRS that was 2.18-times higher (95% CI, 2.14–2.22), and men of East Asian ancestry 0.73-times lower (95% CI, 0.71–0.76), than men of European ancestry. These findings support the role of germline variation contributing to population differences in prostate cancer risk, with the GRS offering an approach for personalized risk prediction.

In July 2016 we described the first known case of canine ocular thelaziosis in the UK in a dog recently imported from Romania. Here we confirm our initial diagnosis using PCR followed by sequence analysis, and we report a further two clinical cases in dogs with recent history of travel to Italy and France. In view of the presence in the UK of the vector for Thelazia callipaeda, namely Phortica spp, we discuss the significance of these three cases in the context of the UK government's pet travel scheme, disease control and both animal and public health in the UK.

Introducción: El tratamiento con antagonistas del factor de necrosis tumoral alfa (anti TNF) ha impactado el pronóstico y la calidad de vida de los pacientes con artritis reumatoide (AR) positivamente, sin embargo, se interroga un incremento en el riesgo de desarrollar melanoma. Objetivo: Conocer la asociación entre el uso de anti TNF y el desarrollo de melanoma maligno en pacientes con AR. Metodología: Se realizó una búsqueda sistemática en MEDLINE, EMBASE, COCHRANE LIBRARY y LILACS para ensayos clínicos, estudios observacionales, revisiones y meta-análisis en pacientes adultos con diagnóstico de AR y manejo con anti TNF (Certolizumab pegol, Adalimumab, Etanercept, Infliximab y Golimumab). Resultados: 37 estudios clínicos cumplieron los criterios de inclusión para el meta-análisis, con una población de 16567 pacientes. El análisis de heterogeneidad no fue significativo (p=1), no se encontró diferencia en el riesgo entre los grupos comparados DR -0.00 (IC 95% -0.001; -0.001). Un análisis adicional de los estudios en los que se reportó al menos 1 caso de melanoma (4222 pacientes) tampoco mostró diferencia en el riesgo DR -0.00 (IC 95% -0.004 ; -0.003). Conclusión: En la evidencia disponible a la fecha no encontramos asociación significativa entre el tratamiento con anti TNF en pacientes con diagnóstico de AR y el desarrollo de melanoma cutáneo., Introduction: Treatment with antagonists of the tumor necrosis factor alpha (anti- TNF) has impacted the prognosis and quality of life of patients with rheumatoid arthritis (RA) positively, however, there may be an incrased risk for cutaneous melanoma with this therapy. Objective: To determine the association between the use of anti- TNF and the development of malignant melanoma in patients with RA. Methods: A systematic search of MEDLINE, EMBASE, Cochrane Library and LILACS for clinical trials, observational studies, reviews and meta-analyzes in adult patients with RA diagnosis and management with anti TNF (certolizumab pegol, adalimumab, etanercept, infliximab and Golimumab) was performed. Results: 37 trials met the inclusion criteria for the meta-analysis, with a population of 16567 patients. The analysis of heterogeneity was not significant (p = 1), no difference was found in risk between the compared groups risk difference (RD) -0.00 (95% -0.001, -0.001). Further analysis of studies in which at least 1 case of melanoma (4222 patients) reported no difference in risk showed RD -0.00 (95% -0.004, -0.003). Conclusion: On the available evidence to date ther is no significant association between treatment with anti TNF and the development of cutaneous melanoma in patients with RA diagnosis.

N sub2 H sub4 derivatives: The f.p. of N sub2 H sub3 Me (I) wa s calculated as -56.08 deg. C; the eutectic for the system I-N sub2 H sub4 occured at 85 wt.-% I and -64 deg. C. Neutralizing I.H sub2 SO sub4 with KOH yielded 95. 6% I. The formation of trimethylazonium bromide in the reaction of N sub2 H sub4 and MeBr was not prevented by using 0.5 M Me Br to 1.0 M N sub2 H sub4 or by lowering the reaction temperature to 0 deg. C. The method adopted for seperating N sub2 H sub4, unsym-dimethylhydrazine, and I utilized the formation of complexes with BzH. Thermal decomposition of MeNO sub2: Analyzing the contents of heated ampoules with a mass spectrometer appeared a promising method for studying the decomposition mechanism. The apparatus for spectrometer determinations was completed. Engine spectrometer determination was completed. Engine test program: In fourteen 100-lb.-thrust tests, the 13% solution of LiBH sub4 in N sub2 H sub4 and O had a maximum I sub sp value of 256 lb.-sec./lb., which was 94% of the theoretical maximum. All runs were characterized by copious deposits of gray or white oxides, which almost covered the chamber, nozzle walls, turbulator, and injector face.

The nomenclature, properties, and preparation of mono-, di-, tri-, and tetra-substituted hydrazine compounds are discussed. Condensation products of hydrazine and substituted hydrazines with aliphatic carbonyl compounds are described as well as qualitative analysis of hydrazine compounds.

The effect of the location of the longitudinal centers of buoyancy and floatation on resistance in a fully developed Pierson-Moskowitz sea is predicted from regular wave model tests. La adopción internacional es una medida de protección a los menores mediante la cual, un menor considerado como adoptable por una autoridad extranjera competente, y con residencia habitual en el extranjero, es o será desplazado a España por adoptantes con residencia habitual en España. Entre los objetivos del presente trabajo de investigación destacan: describir los niveles de desarrollo, socioemocionales y de aprendizaje de la población infantil adoptada internacionalmente, detectar posibles dificultades u ofrecer apoyo y asesoramiento a las familias. Concretamente, de los menores con edades comprendidas entre el nacimiento y los seis años de edad, lo que nos sitúa dentro del campo de la Atención Temprana. Entendemos por Atención Temprana el conjunto de actuaciones dirigidas a la población infantil (0-6 años), su familia y entorno, que dan respuesta lo más pronto posible a las necesidades transitorias o permanentes que presentan los niños con trastornos en su desarrollo o que tienen el riesgo de padecerlos. La literatura científica ha demostrado a través de numerosas investigaciones los siguientes datos: por cada tres meses en un orfanato se produce un mes de retraso en el desarrollo físico, el desarrollo emocional corresponde a la mitad de la edad cronológica o que el abandono físico tiene consecuencias negativas para el desarrollo de la flexibilidad cognitiva. Por tanto, nuestra investigación responde a la siguiente pregunta: ¿qué alteraciones del desarrollo, socioemocionales y de aprendizaje pueden presentarse entre los niños procedentes de los programas de adopción internacional? Para resolver esta cuestión, hemos optado por un tipo de investigación no experimental, de corte descriptivo., International adoption is a child protection measure by which, a minor considered as adoptable by a competent abroad authority, and with habitual residence abroad, is or will be displaced to Spain by adopters who has a habitual residence in Spain. The objectives of the present research work include: describe levels of emotional and learning development of children form international adoption, detect possible dificulties and provide support or advice to families. Specifically, minors aged between birth and six years, therefore we researched about the field of early intervention. Early intervention is the set of actions aimed at childhood (0-6 years), their families and enviroment, that responds, as soon as possible, to transitory or permanent needs who have children with disorders in their development or have the risk. The scientific literature has demonstrated through a lot of researchs the following datas: for every three months living in an orphanage there is a month of delay in physical development, the emotional development imply half of the chronological age or the physical neglect has negative consequences for the development or cognitive flexibility. Therefore, our research answers the following question: what developmental, socio-emotional and learning alterations can occur among children from international adoption programs? To answer this question, we have opted for a type of non-experimental research, of a descriptive nature.

We report significant failure rates (28%, 95% confidence interval 15%-45%) after administering 1 g azithromycin to men with Mycoplasma genitalium-positive nongonococcal urethritis. In vitro evidence supported reduced susceptibility of M. genitalium to macrolides. Moxifloxacin administration resulted in rapid symptom resolution and eradication of infection in all cases. These findings have implications for management of urethritis.

BACKGROUND: We wished to determine recurrences of bacterial vaginosis (BV) after treatment over the course of 12 months and to establish factors associated with recurrence. METHODS: Women with symptomatic BV (a Nugent score [NS] of 7-10 or of 4-6 with >or=3 Amsel criteria) were enrolled. BV was treated with 400 mg of oral metronidazole twice a day for 7 days. Participants completed a questionnaire and vaginal swabs were collected at 1, 3, 6, and 12 months; the study end point was an NS of 7-10. RESULTS: A total of 121 (87%) women with an NS of 7-10 and 18 (13%) with an NS of 4-6 and >or=3 Amsel criteria were enrolled; 130 (94%) returned >or=1 vaginal samples. Sixty-eight women (58% [95% confidence interval {CI}, 49%-66%]) had a recurrence of BV (NS 7-10), and 84 (69% [95% CI, 61%-77%]) had a recurrence of abnormal vaginal flora (NS 4-10) by 12 months. A past history of BV, a regular sex partner throughout the study, and female sex partners were significantly associated with recurrence of BV and abnormal vaginal flora by multivariate analysis; the use of hormonal contraception had a negative association with recurrence. CONCLUSION: Current recommended treatment is not preventing the recurrence of BV or abnormal vaginal flora in the majority of women; factors associated with recurrence support a possible role for sexual transmission in the pathogenesis of recurrent BV.
