Abstract

Background: The multimorbid burden and use of systemic immunosuppressants in people with psoriasis may confer greater risk of adverse outcomes of coronavirus disease 2019 (COVID-19), but the data are limited. Objective: Our aim was to characterize the course of COVID-19 in patients with psoriasis and identify factors associated with hospitalization. Methods: Clinicians reported patients with psoriasis with confirmed/suspected COVID-19 via an international registry, Psoriasis Patient Registry for Outcomes, Therapy and Epidemiology of COVID-19 Infection. Multiple logistic regression was used to assess the association between clinical and/or demographic characteristics and hospitalization. A separate patient-facing registry characterized risk-mitigating behaviors. Results: Of 374 clinician-reported patients from 25 countries, 71% were receiving a biologic, 18% were receiving a nonbiologic, and 10% were not receiving any systemic treatment for psoriasis. In all, 348 patients (93%) were fully recovered from COVID-19, 77 (21%) were hospitalized, and 9 (2%) died. Increased hospitalization risk was associated with older age (multivariable-adjusted odds ratio [OR] = 1.59 per 10 years; 95% CI = 1.19-2.13), male sex (OR = 2.51; 95% CI = 1.23-5.12), nonwhite ethnicity (OR = 3.15; 95% CI = 1.24-8.03), and comorbid chronic lung disease (OR = 3.87; 95% CI = 1.52-9.83). Hospitalization was more frequent in patients using nonbiologic systemic therapy than in those using biologics (OR = 2.84; 95% CI = 1.31-6.18). No significant differences were found between classes of biologics. Independent patient-reported data (n = 1626 across 48 countries) suggested lower levels of social isolation in individuals receiving nonbiologic systemic therapy than in those receiving biologics (OR = 0.68; 95% CI = 0.50-0.94). Abstract

Background Social risk factors influence rehospitalization rates yet are challenging to incorporate into prediction models. Integration of social risk factors using natural language processing (NLP) and machine learning could improve risk prediction of 30-day readmission following an acute myocardial infarction. Methods and Results Patients were enrolled into derivation and validation cohorts. The derivation cohort included inpatient discharges from Vanderbilt University Medical Center between January 1, 2007, and December 31, 2016, with a primary diagnosis of acute myocardial infarction, who were discharged alive, and not transferred from another facility. The validation cohort included patients from Dartmouth-Hitchcock Health Center between April 2, 2011, and December 31, 2016, meeting the same eligibility criteria described above. Data from both sites were linked to Centers for Medicare & Medicaid Services administrative data to supplement 30-day hospital readmissions. Clinical notes from each cohort were extracted, and an NLP model was deployed, counting mentions of 7 social risk factors. Five machine learning models were run using clinical and NLP-derived variables. Model discrimination and calibration were assessed, and receiver operating characteristic comparison analyses were performed. The 30-day rehospitalization rates among the derivation (n=6165) and validation (n=4024) cohorts were 15.1% (n=934) and 10.2% (n=412), respectively. The derivation models demonstrated no statistical improvement in model performance with the addition of the selected NLP-derived social risk factors. Conclusions Social risk factors extracted using NLP did not significantly improve 30-day readmission prediction among hospitalized patients with acute myocardial infarction. Alternative methods are needed to capture social risk factors.

Abstract

High-quality randomised clinical trials testing moderately fractionated breast radiotherapy have clearly shown that local control and survival is at least as effective as with 2 Gy daily fractions with similar or reduced normal tissue toxicity. Fewer treatment visits are welcomed by patients and their families, and reduced fractions produce substantial savings for health-care systems. Implementation of hypofractionation, however, has moved at a slow pace. The oncology community have now reached an inflection point created by new evidence from the FAST-Forward five- fraction randomised trial and catalysed by the need for the global radiation oncology community to unite during the COVID-19 pandemic and rapidly rethink hypofractionation implementation. The aim of this paper is to support equity of access for all patients to receive evidence-based breast external beam radiotherapy and to facilitate the translation of new evidence into routine daily practice. The results from this European Society for Radiotherapy and Oncology Advisory Committee in Radiation Oncology Practice consensus state that moderately hypofractionated radiotherapy can be offered to any patient for whole breast, chest wall (with or without reconstruction), and nodal volumes. Ultrafractionation (five fractions) can also be offered for non-nodal breast or chest wall (without reconstruction) radiotherapy either as standard of care or within a randomised trial or prospective cohort. The consensus is timely; not only is it a pragmatic framework for radiation oncologists, but it provides a measured proposal for the path forward to influence policy makers and empower patients to ensure equity of access to evidence- based radiotherapy.

6. Adaptive and Neural Network Based Control of Pitch of Unmanned Aerial Vehicles

Abstract

The objective is to develop a new control strategy for quadrotor types of Unmanned Aerial Systems (UAVs) that assist in closing the existing research gaps between the undesired uncertainties and current control systems. This research investigates a system modeling and the effectiveness of error elimination through adaptation in the change of the plant/system’s output. The UAV is controlled via Wi-Fi using MATLAB. Proportional-Integral-Derivative (PID) controller, the two degrees of freedom Proportional-Integral-Derivative (2DOF-PID) controller and the model reference Neural Network (NN) controller are used for the aircraft pitch control. External disturbances and modeling errors can lead to the instability and unpredictable behaviors of a system. Even though the disturbances cannot be avoided the effects of such disturbances can be reduced significantly by neural network-based adaptive controllers for high-performance tracking in the presence of the disturbances. The robust control systems with the adaptive control will improve navigation performance and operation for military, reconnaissance and surveillance applications. The application of a robust controller determines the performance by observing the system’s behavior to identify and reduce the unpredictable effects of disturbances and achieve a better behavior of the whole system. By observing the system, the neural network is suitably used to learn from a set of training patterns. System identification uses a neural network to capture the behavior of system dynamics, which assists the neural network to train itself to act as a controller. Moreover, the system’s performance of the controllers was tested using simulations to demonstrate the effectiveness in improving the speed and stability for the dynamic system. The presented work will produce a flexible, robust and effective control system model that provides additional stability and reduce the effects of disturbances.

Abstract

We present a catalogue of nearly 3,000 submillimetre sources detected at 850um over ~5 square degrees surveyed as part of the James Clerk Maxwell Telescope (JCMT) SCUBA-2 Cosmology Legacy Survey (S2CLS). This is the largest survey of its kind at 850um, probing a meaningful cosmic volume at the peak of star formation activity and increasing the sample size of submillimetre galaxies selected at 850um by an order of magnitude. We describe the wide 850um survey component of S2CLS, which covers the key extragalactic survey fields: UKIDSS-UDS, COSMOS, Akari-NEP, Extended Groth Strip, Lockman Hole North, SSA22 and GOODS-North. The average 1-sigma depth of S2CLS is 1.2 mJy/beam, approaching the SCUBA-2 850um confusion limit, which we determine to be ~0.8 mJy/beam. We measure the single dish 850um number counts to unprecedented accuracy, reducing the Poisson errors on the differential counts to approximately 4% at S_850~3mJy. With several independent fields, we investigate field-to-field variance, finding that the number counts on 0.5-1 degree scales are generally within 50% of the S2CLS mean for S_850>3mJy, with scatter consistent with the Poisson and estimated cosmic variance uncertainties, although there is a marginal (2-sigma) density enhancement in the GOODS-North field. The observed number counts are in reasonable agreement with recent phenomenological and semi-analytic models. Finally, the large solid angle of S2CLS allows us to measure the bright-end counts: at S_850>10mJy there are approximately ten sources per square degree, and we detect the distinctive up-turn in the number counts indicative of the detection of local sources of 850um emission and strongly lensed high-redshift galaxies. Abstract

[Abridged] We use the Planck all-sky submm and mm maps to search for rare sources distinguished by extreme brightness, a few hundreds of mJy, and their potential for being situated at high redshift. These "cold" Planck sources, selected using the High Frequency Instrument (HFI) directly from the maps and from the Planck Catalogue of Compact Sources (PCCS), all satisfy the criterion of having their rest-frame far-infrared peak redshifted to the frequency range 353 and 857 GHz. This colour-selection favours galaxies in the redshift range z=2-4, which we consider as cold peaks in the cosmic infrared background (CIB). We perform a dedicated Herschel-SPIRE follow-up of 234 such Planck targets, finding a significant excess of red 350 and 500um sources, in comparison to reference SPIRE fields. About 94% of the SPIRE sources in the Planck fields are consistent with being overdensities of galaxies peaking at 350um. About 3% are candidate lensed systems, all 12 of which have secure spectroscopic confirmations, placing them at redshifts z>2.2. The galaxy overdensities are detected with high significance, half of the sample showing statistical significance above 10sigma. The SPIRE photometric redshifts of galaxies in overdensities suggest a peak at z~2. Under the Td=35K assumption, we derive an infrared (IR) luminosity for each SPIRE source of about 4x10^12 Lsun, yielding star formation rates of typically 700 Msun.yr^-1. If the observed overdensities are actual gravitationally-bound structures, the total total star formation rates reaches 7x10^3 Msun.yr^-1. Taken together, these sources show the signatures of high-z (z>$) protoclusters of intensively star-forming galaxies. Abstract

We have used the Planck all-sky submillimetre and millimetre maps to search for rare sources distinguished by extreme brightness, a few hundred millijanskies, and their potential for being situated at high redshift. These "cold" Planck sources, selected using the High Frequency Instrument (HFI) directly from the maps and from the Planck Catalogue of Compact Sources (PCCS), all satisfy the criterion of having their rest-frame far-infrared peak redshifted to the frequency range 353-857 GHz. This colour-selection favours galaxies in the redshift range z = 2-4, which we consider as cold peaks in the cosmic infrared background. With a 4.'5 beam at the four highest frequencies, our sample is expected to include overdensities of galaxies in groups or clusters, lensed galaxies, and chance line-of-sight projections. We perform a dedicated Herschel-SPIRE follow-up of 234 such Planck targets, finding a significant excess of red 350 and 500 mu m sources, in comparison to reference SPIRE fields. About 94% of the SPIRE sources in the Planck fields are consistent with being overdensities of galaxies peaking at 350 mu m, with 3% peaking at 500 mu m, and none peaking at 250 mu m. About 3% are candidate lensed systems, all 12 of which have secure spectroscopic confirmations, placing them at redshifts z > 2.2. Only four targets are Galactic cirrus, yielding a success rate in our search strategy for identifying extragalactic sources within the Planck beam of better than 98%. The galaxy overdensities are detected with high significance, half of the sample showing statistical significance above 10 sigma. The SPIRE photometric redshifts of galaxies in overdensities suggest a peak at z similar or equal to 2, assuming a single common dust temperature for the sources of T-d = 35 K. Under this assumption, we derive an infrared (IR) luminosity for each SPIRE source of about 4x10(12) L-circle dot, yielding star formation rates of typically 700 M-circle dot yr(-1). Under the Td=35K assumption, we derive an infrared (IR) luminosity for each SPIRE source of about 4x10^12 Lsun, yielding star formation rates of typically 700 Msun.yr^-1. If the observed overdensities are actual gravitationally-bound structures, the total total star formation rates reaches 7x10^3 Msun.yr^-1. Taken together, these sources show the signatures of high-z (z>$) protoclusters of intensively star-forming galaxies. Abstract

We investigate the multi-wavelength properties of a sample of 450-\mu m selected sources from the SCUBA-2 Cosmology Legacy Survey (S2CLS). A total of 69 sources were identified above 4\sigma\ in deep SCUBA-2 450-\mu m observations overlapping the UDS and COSMOS fields and covering 210 sq. arcmin to a typical depth of \sigma 450=1.5 mJy. Reliable cross identification are found for 58 sources (84 per cent) in Spitzer and Hubble Space Telescope WFC3/IR data. The photometric redshift distribution (dN/dz) of 450\mu m-selected sources is presented, showing a broad peak in the redshift range 1=10^12 +/- 0.8 L_sol, =42 +/- 11 K and <\beta_D>=1.6 +/- 0.5, respectively. The relationship between these SED parameters and the physical properties of galaxies is investigated, revealing correlations between T_D and LIR and between \beta_D and both stellar mass and effective radius. The connection between star formation rate and stellar mass is explored, with 24 per cent of 450 \mu m sources found to be ``star-bursts'', i.e. displaying anomalously high specific SFRs. However, both the number density and observed properties of these ``star-burst'' galaxies are found consistent with the population of normal star-forming galaxies., Comment: 20 pages, 12 figures, accepted for publication in MNRAS

Abstract

OBJECTIVE: High doses of opiate substitution pharmacotherapy are associated with greater treatment retention and lower illicit drug consumption, although the neurobiological bases of these benefits are poorly understood. Dysfunction of the anterior cingulate cortex (ACC) is associated with greater addiction severity and mood dysregulation in opiate users, such that the beneficial effects of substitution pharmacotherapy may relate to normalisation of ACC function. This study aimed to investigate the differential impact of methadone compared with buprenorphine on dorsal ACC biochemistry. A secondary aim was to explore the differential effects of methadone and buprenorphine on dorsal ACC biochemistry in relation to depressive symptoms. METHODS: Twenty-four heroin-dependent individuals stabilised on methadone (n=10) or buprenorphine (n=14) and 24 healthy controls were scanned using proton Magnetic Resonance Spectroscopy and compared for metabolite concentrations of N-acetylaspartate, glutamate/glutamine, and myo-inositol. RESULTS: (1) Methadone was associated with normalisation of dorsal ACC biochemistry (increased N-acetylaspartate and glutamate/glutamine levels, and decreased myo-inositol levels) in a dose-dependent manner; (2) buprenorphine-treated individuals had higher myo-inositol and glutamate/glutamine levels than methadone-treated patients in the right dorsal ACC; and (3) myo-inositol levels were positively correlated with depressive symptoms in participants stabilised on buprenorphine. CONCLUSIONS: These findings point to a beneficial role of high-dose methadone on dorsal ACC biochemistry, and suggest a link between elevated myo-inositol levels and depressive symptoms in the context of buprenorphine treatment.

Abstract

The first deep blank-field 450um map (1-sigma~1.3mJy) from the SCUBA-2 Cosmology Legacy Survey (S2CLS), conducted with the James Clerk Maxwell Telescope (JCMT) is presented. Our map covers 140 arcmin^2 of the COSMOS field, in the footprint of the HST CANDELS area. Using 60 submillimetre galaxies (SMGs) detected at >3.75-sigma, we evaluate the number counts of 450um-selected galaxies with flux densities S_450>5mJy. The 8-arcsec JCMT beam and high sensitivity of SCUBA-2 now make it possible to directly resolve a larger fraction of the cosmic infrared background (CIB, peaking at ~200um) into the individual galaxies responsible for its emission than has previously been possible at this wavelength. At S_450>5mJy we resolve (7.4[+/-]0.7)x10^-2 MJy/sr of the CIB at 450um (equivalent to 16[+/-]7% of the absolute brightness measured by COBE at this wavelength) into point sources. A further ~40% of the CIB can be recovered through a statistical stack of 24um emitters in this field, indicating that the majority (~60%) of the CIB at 450um is emitted by galaxies with S_450>2mJy. The average redshift of 450um emitters identified with an optical/near-infrared counterpart is estimated to be =1.3, implying that the galaxies in the sample are in the ultraluminous class (L_IR~1.1x10^12 L_sun). If the galaxies contributing to the statistical stack lie at similar redshifts, then the majority of the CIB at 450um is emitted by galaxies in the LIRG class with L_IR>3.6x10^11 L_sun., Comment: 9 pages, 5 figures, submitted to MNRAS

Abstract

We present a catalogue of ∼3000 submillimetre sources detected (≥3.5σ) at 850 μm over ∼5 deg2 surveyed as part of the James Clerk Maxwell Telescope (JCMT) SCUBA-2 Cosmology Legacy Survey (S2CLS). This is the largest survey of its kind at 850 μm, increasing the sample size of 850 μm selected submillimetre galaxies by an order of magnitude. The wide 850 μm survey component of S2CLS covers the extragalactic fields: UKIDSS-UDS, COSMOS, Akari-NEP, Extended Groth Strip, Lockman Hole North, SSA22 and GOODS-North. The average 1σ depth of S2CLS is 1.2 mJy beam-1, approaching the SCUBA-2 850 μm confusion limit, which we determine to be σc ≈ 0.8 mJy beam-1. We measure the 850 μm number counts, reducing the Poisson errors on the differential counts to approximately 4 per cent at S850 ≈ 3 mJy. With several independent fields, we investigate field-to-field variance, finding that the number counts on 0.5°–1° scales are generally within 50 per cent of the S2CLS mean for S850 > 3 mJy, with scatter consistent with the Poisson and estimated cosmic variance uncertainties, although there is a marginal (2σ) density enhancement in GOODS-North. The observed counts are in reasonable agreement with recent phenomenological and semi-analytic models, although determining the shape of the faint-end slope (S850 < 3 mJy) remains a key test. The large solid angle of S2CLS allows us to measure the bright-end counts: at S850 > 10 mJy there are approximately 10 sources per square degree, and we detect the distinctive up-turn in the number counts indicative of the detection of local sources of 850 μm emission, and strongly lensed high-redshift galaxies. All calibrated maps and the catalogue are made publicly available.
