Abstract

Background and aimsProblem gambling and tobacco use are highly comorbid among adults. However, there are few treatment frameworks that target both gambling and tobacco use simultaneously (i.e., an integrated approach), while also being accessible and evidence-based. The aim of this two-arm open label RCT was to examine the efficacy of an integrated online treatment for problem gambling and tobacco use.MethodsA sample of 209 participants (M$_{age}$ = 37.66, SD = 13.81; 62.2% female) from North America were randomized into one of two treatment conditions (integrated [n = 91] or gambling only [n = 118]) that lasted for eight weeks and consisted of seven online modules. Participants completed assessments at baseline, after treatment completion, and at 24-week follow-up.ResultsWhile a priori planned generalized linear mixed models showed no condition differences on primary (gambling days, money spent, time spent) and secondary outcomes, both conditions did appear to significantly reduce problem gambling and smoking behaviours over time. Post hoc analyses showed that reductions in smoking and gambling craving were correlated with reductions in days spent gambling, as well as with gambling disorder symptoms. Relatively high (versus low) nicotine replacement therapy use was associated with greater reductions in gambling behaviours in the integrated treatment condition.Discussion and conclusionsWhile our open label RCT does not support a clear benefit of integrated treatment, findings suggest that changes in smoking and gambling were correlated over time, regardless of treatment condition, suggesting that more research on mechanisms of smoking outcomes in the context of gambling treatment may be relevant.

Abstract

Background: Foot surgery is common in patients with RA but research into surgical outcomes is limited and conceptually flawed as current outcome measures lack face validity: to date no one has asked patients what is important to them. This study aimed to determine which factors are important to patients when evaluating the success of foot surgery in RA Methods: Semi structured interviews of RA patients who had undergone foot surgery were conducted and transcribed verbatim. Thematic analysis of interviews was conducted to explore issues that were important to patients. Results: 11 RA patients (9 ♂, mean age 59, dis dur = 22yrs, mean of 3 yrs post op) with mixed experiences of foot surgery were interviewed. Patients interpreted outcome in respect to a multitude of factors, frequently positive change in one aspect contrasted with negative opinions about another. Overall, four major themes emerged. Function: Functional ability & participation in valued activities were very important to patients. Walking ability was a key concern but patients interpreted levels of activity in light of other aspects of their disease, reflecting on change in functional ability more than overall level. Positive feelings of improved mobility were often moderated by negative self perception ("I mean, I still walk like a waddling duck”). Appearance: Appearance was important to almost all patients but perhaps the most complex theme of all. Physical appearance, foot shape, and footwear were closely interlinked, yet patients saw these as distinct separate concepts. Patients need to legitimize these feelings was clear and they frequently entered into a defensive repertoire ("it's not cosmetic surgery; it's something that's more important than that, you know?”). 5. Reading in mirrors: Using Street Literature to facilitate practitioner inquiry with urban public service librarians

Abstract

This dissertation is about a group of African American inner-city public librarians who, over the course of 16 months, read the literary genre Street Lit in an inquiry-based book club for the purpose of professional development. I explore their literary responses to Street Literature to learn what it means to be an educated, African American literacy professional in inner-city settings. This study was designed to understand what happens when librarians read what their patrons read as a form of practitioner inquiry to inform their professional practices as Black librarians. The Librarians' Book Club comprised of a group of 8 African American librarians who met once a month, for a three-hour long book club meeting at a private home to discuss one or two titles that represented Street Literature across a historical literary continuum. This means that the group read canonical works with Street Lit themes as well as contemporary iterations of the genre. Using Practitioner Inquiry as a methodological framework, I collected data from various sources including book club discourse, field notes, reaction sheets, librarian journals, and interviews. The data revealed that the librarians' primary response to reading Street Literature was that they personally related the stories and characters to their own lived lives. This relating factor was the foundation upon which other reading responses emerged including whether they liked or disliked a novel or character, or if they found the text to be quality literature or not. Their reading responses revealed ways in which the librarians unpacked, questioned and transformed their perceptions towards their library patrons as well as how they began to modify their approaches to professional practice in the library such that they were more invested and involved in community life. TV audiences are fed up with how the developing world is portrayed on the small screen, according to a new VSO report published today. Reflecting the real world? How British TV portrayed developing countries in 2005 reveals that television viewers have an overwhelmingly negative view of the developing world and that they hold TV responsible. The report shows that viewers have a real appetite for richer representations of the world outside the UK and calls on broadcasters to invest more money, creativity and talent in bringing the world to UK audiences. The report is based on interviews with TV viewers and leading broadcasters. It shows that despite the high level of developing world coverage on TV over the last year, there has been no sign of a positive shift in public attitude. TV viewers associated the developing world with famine, disaster and corruption and people's initial image was very often of starving babies with flies around their eyes. The research highlights that news coverage and charity campaigns have also contributed to a feeling that the developing world is a hopeless cause. News reporting of the Asian Tsunami and the Pakistan earthquake brought people's attention to poorer countries but reinforced a doom-laden view of them. Even the Make Poverty History campaign and the Live 8 concerts, which enthused millions of people, appear to have inadvertently contrived to confirm a stereotype of Africa as a continent on its knees and added to a sense that nothing has changed over the last 20 years. The research uncovered a very strong sentiment that TV coverage of developing countries is too negative. Viewers expressed a desire to see the positive side of life in the developing world and hear about any progress being made. Abstract

Background: Foot surgery is common in patients with RA but research into surgical outcomes is limited and conceptually flawed as current outcome measures lack face validity: to date no one has asked patients what is important to them. This study aimed to determine which factors are important to patients when evaluating the success of foot surgery in RA Methods: Semi structured interviews of RA patients who had undergone foot surgery were conducted and transcribed verbatim. Thematic analysis of interviews was conducted to explore issues that were important to patients. Results: 11 RA patients (9 ♂, mean age 59, dis dur = 22yrs, mean of 3 yrs post op) with mixed experiences of foot surgery were interviewed. Patients interpreted outcome in respect to a multitude of factors, frequently positive change in one aspect contrasted with negative opinions about another. Overall, four major themes emerged. Function: Functional ability & participation in valued activities were very important to patients. Walking ability was a key concern but patients interpreted levels of activity in light of other aspects of their disease, reflecting on change in functional ability more than overall level. Positive feelings of improved mobility were often moderated by negative self perception ("I mean, I still walk like a waddling duck”). Appearance: Appearance was important to almost all patients but perhaps the most complex theme of all. Physical appearance, foot shape, and footwear were closely interlinked, yet patients saw these as distinct separate concepts. Patients need to legitimize these feelings was clear and they frequently entered into a defensive repertoire ("it's not cosmetic surgery; it's something that's more important than that, you know?”). Clinician opinion: Surgeons' post operative evaluation of the procedure was very influential.
