Abstract: 176 p., En la presente Tesis Doctoral se ha realizado un análisis de las narrativas de hombres inmigrantes procedentes de América Latina, los cuales depositan su fuerza de trabajo en lo concerniente a los cuidados de personas mayores y/o dependientes. Para esto fue necesario direccionar la idea inicial del género como el encargado de distribuir los roles y las tareas, entre ellas las de cuidado que por lo general han sido relacionadas con la esfera de lo doméstico y de lo femenino. Desde ese escenario se introducen la migraciones internacionales, las cuales se encargan de suplir los vacios de cuidado que no han sido cubiertos por un Estado de bienestar ni la idea de la familia tradicional. La estrategia seguida en el presente trabajo de investigación fue la de identificar una serie de cinco categorías de orden deductivo: Identidad, cuidados, experiencias de racismo y de discriminación, el género en los espacios de trabajo, las masculinidades y su reconfiguración. Para finalmente presentar una serie tres posicionamientos-tipo, de orden inductivo en los cuales se presenta hasta qué punto existe en estos hombres una autoidentificación como cuidadores, así como la creación o no de un vinculo con la persona a la que cuidan. Así se ha logrado identificar, gracias al uso de la interseccionalidiad como herramienta de análisis, que un mayor nivel de estudios del sujeto migrante reporta una menor autoidentificación como cuidador.

2. Quality of locally designed surveys in a quality improvement collaborative : review of survey validity and identification of common errors

Abstract: Objective: Surveys are a commonly used tool in quality improvement (QI) projects, but little is known about the standards to which they are designed and applied. We aimed to investigate the quality of surveys used within a QI collaborative, and to characterise the common errors made in survey design. Methods: Five reviewers (two research methodology and QI, three clinical and QI experts) independently assessed 20 surveys, comprising 250 survey items, that were developed in a North American cystic fibrosis lung transplant transition collaborative. Content Validity Index (CVI) scores were calculated for each survey. Reviewer consensus discussions decided an overall quality assessment for each survey and survey item (analysed using descriptive statistics) and explored the rationale for scoring (using qualitative thematic analysis). Results: 3/20 surveys scored as high quality (CVI >80%). 19% (n=47) of survey items were recommended by the reviewers, with 35% (n=87) requiring improvements, and 46% (n=116) not recommended. Quality assessment criteria were agreed upon. Types of common errors identified included the ethics and appropriateness of questions and survey format; usefulness of survey items to inform learning or lead to action, and methodological issues with survey questions, survey response options; and overall survey design. Conclusion: Survey development is a task that requires careful consideration, time and expertise. QI teams should consider whether a survey is the most appropriate form for capturing information during the improvement process. There is a need to educate and support QI teams to adhere to good practice and avoid common errors, thereby increasing the value of surveys for evaluation and QI. The methodology, quality assessment criteria and common errors described in this paper can provide a useful resource for this purpose. © Author(s) (or their employer(s)) 2024. Abstract: Noncoding DNA is central to our understanding of human gene regulation and complex diseases1,2, and measuring the evolutionary sequence constraint can establish the functional relevance of putative regulatory elements in the human genome3,4,5,6,7,8,9. Identifying the genomic elements that have become constrained specifically in primates has been hampered by the faster evolution of noncoding DNA compared to protein-coding DNA10, the relatively short timescales separating primate species11, and the previously limited availability of whole-genome sequences12. Here we construct a whole-genome alignment of 239 species, representing nearly half of all extant species in the primate order. Using this resource, we identified human regulatory elements that are under selective constraint across primates and other mammals at a 5% false discovery rate. We detected 111,318 DNase I hypersensitivity sites and 267,410 transcription factor binding sites that are constrained specifically in primates but not across other placental mammals and validate their cis-regulatory effects on gene expression. These regulatory elements are enriched for human genetic variants that affect gene expression and complex traits and diseases. Our results highlight the important role of recent evolution in regulatory sequence elements differentiating primates, including humans, from other placental mammals.

Abstract: Research question Are serum progesterone concentrations on the day of modified natural cycle (mNC) frozen blastocyst transfer (FET) without luteal phase support (LPS) associated with clinical pregnancy rate (CPR)? Design Data were collected between January 2019 and October 2022 as a sub-study of an ongoing randomized controlled trial assessing pregnancy outcomes in mNC-FET. The sub-study included all women (n = 209) randomized to mNC-FET without LPS at the time of data extraction. Participants were aged 18–41 years, had regular menstrual cycles and underwent mNC-FET treatment with single-blastocyst transfer. Associations between the serum progesterone concentration on the day of blastocyst transfer and CPR, pregnancy rate and pregnancy loss rate (PLR) were examined between groups with low and higher progesterone concentrations using the 25th and 10th percentiles as cut-offs. Multivariate logistic regression analyses were performed to adjust for potential confounding factors. Results Progesterone concentrations on the day of blastocyst transfer in mNC-FET without LPS ranged from 4.9 to 91.8 nmol/l, with the 25th and 10th percentiles at 29.0 nmol/l and 22.5 nmol/l, respectively. Serum progesterone concentrations did not differ between women with or without a clinical pregnancy (mean [SD] 38.5 [14.0] versus 36.8 [12.4] nmol/l; P = 0.350). Furthermore, the CPR, pregancy rate and PLR were similar in women with low or high progesterone concentrations when using the 25th or the 10th progesterone percentile as cut-off. Multivariate regression analyses showed no association between progesterone concentrations and CPR. Abstract: INTRODUCTION: Sodium-glucose cotransporter 2 (SGLT2) inhibitors have previously demonstrated cardioprotective properties in patients with type 2 diabetes, suggesting a preventive effect on heart failure (HF). The Empire Prevent trial program investigates the therapeutic potential for HF prevention by evaluating the cardiac, metabolic, and renal effects of the SGLT2 inhibitor empagliflozin in patients with increased risk of developing HF, but without diabetes or established HF.METHODS: The Empire Prevent trial program is an investigator-initiated, double-blind, randomized clinical trial program including elderly and obese patients (60-84 years, body mass index >28 kg/m2) with at least one manifestation of hypertension, cardiovascular or chronic kidney disease, but no history of diabetes or HF. The aims are to investigate the effects of empagliflozin on 1) physical capacity and left ventricular and atrial structural changes with peak oxygen consumption and left ventricular mass as primary endpoints (Empire Prevent Cardiac), and 2) cardiac-adipose tissue interaction and volume homeostasis with primary endpoints of changes in epicardial adipose tissue and estimated extracellular volume (Empire Prevent Metabolic). At present, 138 of 204 patients have been randomized in the Empire Prevent trial program. Patients are randomized 1:1 to 180 days treatment with empagliflozin 10 mg daily or placebo, while undergoing a comprehensive examination program at baseline and follow-up.DISCUSSION: The Empire Prevent trial program will mark the first step towards elucidating the potential of SGLT2 inhibition for HF prevention in an outpatient setting in elderly and obese patients with increased risk of developing HF, but with no history of diabetes or established HF. Furthermore, the Empire Prevent trial program will supplement the larger event-driven trials by providing mechanistic insights to the beneficial effects of SGLT2 inhibition.TRIAL REGISTRATION: Bot

Abstract: Pathogenic variants in multiple genes on the X chromosome have been implicated in syndromic and non-syndromic intellectual disability disorders. ZFX on Xp22.11 encodes a transcription factor that has been linked to diverse processes including oncogenesis and development, but germline variants have not been characterized in association with disease. Here, we present clinical and molecular characterization of 18 individuals with germline ZFX variants. Exome or genome sequencing revealed 11 variants in 18 subjects (14 males and 4 females) from 16 unrelated families. Four missense variants were identified in 11 subjects, with seven truncation variants in the remaining individuals. Clinical findings included developmental delay/intellectual disability, behavioral abnormalities, hypotonia, and congenital anomalies. Overlapping and recurrent facial features were identified in all subjects, including thickening and medial broadening of eyebrows, variations in the shape of the face, external eye abnormalities, smooth and/or long philtrum, and ear abnormalities. Hyperparathyroidism was found in four families with missense variants, and enrichment of different tumor types was observed. In molecular studies, DNA-binding domain variants elicited differential expression of a small set of target genes relative to wild-type ZFX in cultured cells, suggesting a gain or loss of transcriptional activity. Additionally, a zebrafish model of ZFX loss displayed an altered behavioral phenotype, providing additional evidence for the functional significance of ZFX. Our clinical and experimental data support that variants in ZFX are associated with an X-linked intellectual disability syndrome characterized by a recurrent facial gestalt, neurocognitive and behavioral abnormalities, and an increased risk for congenital anomalies and hyperparathyroidism., Pathogenic variants in multiple genes on the X chromosome have been implicated in syndromic and non-syndromic intellectual disability disorders. ZFX on Xp22.11 encodes a transcription factor that has been linked to diverse processes including oncogenesis and development, but germline variants have not been characterized in association with disease. Here, we present clinical and molecular characterization of 18 individuals with germline ZFX variants. Exome or genome sequencing revealed 11 variants in 18 subjects (14 males and 4 females) from 16 unrelated families. Four missense variants were identified in 11 subjects, with seven truncation variants in the remaining individuals. Clinical findings included developmental delay/intellectual disability, behavioral abnormalities, hypotonia, and congenital anomalies. Overlapping and recurrent facial features were identified in all subjects, including thickening and medial broadening of eyebrows, variations in the shape of the face, external eye abnormalities, smooth and/or long philtrum, and ear abnormalities. Hyperparathyroidism was found in four families with missense variants, and enrichment of different tumor types was observed. In molecular studies, DNA-binding domain variants elicited differential expression of a small set of target genes relative to wild-type ZFX in cultured cells, suggesting a gain or loss of transcriptional activity. Additionally, a zebrafish model of ZFX loss displayed an altered behavioral phenotype, providing additional evidence for the functional significance of ZFX. Abstract: Introduction Immature granulocyte percentage (IG%) is an important biomarker for infection control. We observed spurious cases where the IG% was dramatically underestimated on the automated Sysmex XN-series hehmatology analyzer compared with manual differential. These cases were associated with high values of “Neutrophil Reactivity Intensity” (NEUT-RI), which should reflect the metabolic activity of the neutrophils. Methods We conducted a three-stage study to evaluate whether NEUT-RI could be utilized to screen for misclassified IG% results defined as the manual differential estimating a 10 percentage points higher IG% compared with the automated Sysmex differential. First, 124 patient samples were selected for 800-cell manual smear analysis based on their NEUT-RI values and compared with the automatic Sysmex IG% results. Next, 11 098 routine 110-cell manual smear analyses were compared with the corresponding Sysmex IG% results. Finally, during a 19-day period 160 additional patient samples underwent smear based on NEUT-RI values ≥56 fluorescence intensity (FI) to screen for misclassified results beyond our current smear practice. Results NEUT-RI ≥56 predicted IG% misclassification with 91% sensitivity and 88% specificity, but primarily when the internal Sysmex flag “Abnormal WBC Scattergram” was present. 90.1% of misclassified results were identified by this flag. Beyond our existing smear rules including this flag, NEUT-RI ≥56 FI had a positive predictive value below 1%. Conclusion Both NEUT-RI and the internal Sysmex flag “Abnormal WBC Scattergram” work well to identify cases of IG% misclassification. Abstract: Noncoding DNA is central to our understanding of human gene regulation and complex diseases1,2, and measuring the evolutionary sequence constraint can establish the functional relevance of putative regulatory elements in the human genome3,4,5,6,7,8,9. Identifying the genomic elements that have become constrained specifically in primates has been hampered by the faster evolution of noncoding DNA compared to protein-coding DNA10, the relatively short timescales separating primate species11, and the previously limited availability of whole-genome sequences12. Here we construct a whole-genome alignment of 239 species, representing nearly half of all extant species in the primate order. Using this resource, we identified human regulatory elements that are under selective constraint across primates and other mammals at a 5% false discovery rate. We detected 111,318 DNase I hypersensitivity sites and 267,410 transcription factor binding sites that are constrained specifically in primates but not across other placental mammals and validate their cis-regulatory effects on gene expression. These regulatory elements are enriched for human genetic variants that affect gene expression and complex traits and diseases. Abstract: Objectives: The traditional view on psychiatric disorders as categorical and distinct is being challenged by perspectives emphasizing the relevance of dimensional and transdiagnostic assessment. However, most diagnostic instruments are based on a categorical view with a threshold-approach to disease classification.Methods: We here describe algorithms for dimensionalizing the psychopathological ratings of the widely used diagnostic interview for children and adolescents, the Kiddie-Schedule for Affective Disorders and Schizophrenia – Present and Lifetime Version (K-SADS-PL). We further evaluate the criterion-related construct validity of the dimensionalized attention-deficit/hyperactivity disorder (ADHD) scales using Rasch models in a sample of 590 children (mean age 10.29 (.36), 49% girls).Results: The algorithms generate scores of current symptom load, i.e., the sum of clinician-rated symptoms within each disorder assessed with the interview. We found support for counting symptoms of inattention and hyperactivity/impulsivity, respectively, but not for a single combined ADHD scale.Conclusions: The algorithms constitute an initial step in creating a framework for clinician-rated dimensional analyses of symptoms derived from the K-SADS-PL, but future studies are needed to further evaluate the construct validity of the remaining scales and the reliability and clinical utility of the method. We believe that our proposed algorithms offer a novel method of dimensional psychopathological assessment, which can be applied in multiple branches of child and adolescent psychiatry.

Abstract: Research question Are serum progesterone concentrations on the day of modified natural cycle (mNC) frozen blastocyst transfer (FET) without luteal phase support (LPS) associated with clinical pregnancy rate (CPR)? Design Data were collected between January 2019 and October 2022 as a sub-study of an ongoing randomized controlled trial assessing pregnancy outcomes in mNC-FET. The sub-study included all women (n = 209) randomized to mNC-FET without LPS at the time of data extraction. Participants were aged 18–41 years, had regular menstrual cycles and underwent mNC-FET treatment with single-blastocyst transfer. Associations between the serum progesterone concentration on the day of blastocyst transfer and CPR, pregnancy rate and pregnancy loss rate (PLR) were examined between groups with low and higher progesterone concentrations using the 25th and 10th percentiles as cut-offs. Multivariate logistic regression analyses were performed to adjust for potential confounding factors. Results Progesterone concentrations on the day of blastocyst transfer in mNC-FET without LPS ranged from 4.9 to 91.8 nmol/l, with the 25th and 10th percentiles at 29.0 nmol/l and 22.5 nmol/l, respectively. Serum progesterone concentrations did not differ between women with or without a clinical pregnancy (mean [SD] 38.5 [14.0] versus 36.8 [12.4] nmol/l; P = 0.350). Furthermore, the CPR, pregancy rate and PLR were similar in women with low or high progesterone concentrations when using the 25th or the 10th progesterone percentile as cut-off. Multivariate regression analyses showed no association between progesterone concentrations and CPR. Abstract: INTRODUCTION: Sodium-glucose cotransporter 2 (SGLT2) inhibitors have previously demonstrated cardioprotective properties in patients with type 2 diabetes, suggesting a preventive effect on heart failure (HF). The Empire Prevent trial program investigates the therapeutic potential for HF prevention by evaluating the cardiac, metabolic, and renal effects of the SGLT2 inhibitor empagliflozin in patients with increased risk of developing HF, but without diabetes or established HF.METHODS: The Empire Prevent trial program is an investigator-initiated, double-blind, randomized clinical trial program including elderly and obese patients (60-84 years, body mass index >28 kg/m2) with at least one manifestation of hypertension, cardiovascular or chronic kidney disease, but no history of diabetes or HF. The aims are to investigate the effects of empagliflozin on 1) physical capacity and left ventricular and atrial structural changes with peak oxygen consumption and left ventricular mass as primary endpoints (Empire Prevent Cardiac), and 2) cardiac-adipose tissue interaction and volume homeostasis with primary endpoints of changes in epicardial adipose tissue and estimated extracellular volume (Empire Prevent Metabolic). At present, 138 of 204 patients have been randomized in the Empire Prevent trial program. Patients are randomized 1:1 to 180 days treatment with empagliflozin 10 mg daily or placebo, while undergoing a comprehensive examination program at baseline and follow-up.DISCUSSION: The Empire Prevent trial program will mark the first step towards elucidating the potential of SGLT2 inhibition for HF prevention in an outpatient setting in elderly and obese patients with increased risk of developing HF, but with no history of diabetes or established HF. Abstract: Noncoding DNA is central to our understanding of human gene regulation and complex diseases1,2, and measuring the evolutionary sequence constraint can establish the functional relevance of putative regulatory elements in the human genome3,4,5,6,7,8,9. Identifying the genomic elements that have become constrained specifically in primates has been hampered by the faster evolution of noncoding DNA compared to protein-coding DNA10, the relatively short timescales separating primate species11, and the previously limited availability of whole-genome sequences12. Here we construct a whole-genome alignment of 239 species, representing nearly half of all extant species in the primate order. Using this resource, we identified human regulatory elements that are under selective constraint across primates and other mammals at a 5% false discovery rate. We detected 111,318 DNase I hypersensitivity sites and 267,410 transcription factor binding sites that are constrained specifically in primates but not across other placental mammals and validate their cis-regulatory effects on gene expression. These regulatory elements are enriched for human genetic variants that affect gene expression and complex traits and diseases. Our results highlight the important role of recent evolution in regulatory sequence elements differentiating primates, including humans, from other placental mammals.

Abstract: The Chiral Magnetic Wave (CMW) phenomenon is essential to provide insights into the strong interaction in QCD, the properties of the quark-gluon plasma, and the topological characteristics of the early universe, offering a deeper understanding of fundamental physics in high-energy collisions. Measurements of the charge-dependent anisotropic flow coefficients are studied in Pb-Pb collisions at center-of-mass energy per nucleon-nucleon collision sNN−−−√= 5.02 TeV to probe the CMW. In particular, the slope of the normalized difference in elliptic (v2) and triangular (v3) flow coefficients of positively and negatively charged particles as a function of their event-wise normalized number difference, is reported for inclusive and identified particles. The slope rNorm3 is found to be larger than zero and to have a magnitude similar to rNorm2, thus pointing to a large background contribution for these measurements. Furthermore, rNorm2 can be described by a blast wave model calculation that incorporates local charge conservation. In addition, using the event shape engineering technique yields a fraction of CMW (fCMW) contribution to this measurement which is compatible with zero. This measurement provides the very first upper limit for fCMW, and in the 10-60% centrality interval it is found to be 26% (38%) at 95% (99.7%) confidence level.

Abstract: The growing interest in meta-research reflects an increasing sector appetite to understand the methodological, philosophical and societal implications of research, and provides vital insights into fundamental issues such as integrity, bias and reproducibility. However, the research sector is far from static, and routinely faces changes to funding and assessment expectations within pressured, changeable and competitive political climates. Whilst research on research provides vital insights, the processes of managing research within both established and fluid research environments is often omitted from discourse. A focus on research, without sufficient consideration of the research environment or institutional infrastructure, inaccurately characterises research as a dyadic (researcher-research), disconnected and unsupported activity. Research Management and Administration (RMA) is an increasingly core function within universities, with roles including funding management, post-award support, financial compliance, outreach and impact amongst others. RMA often provides a formal, subject agnostic brokerage role between researchers, the institution and the wider sector, intermediating between the requirements of each. In recent years the role and professionalisation of these colleagues – as distinct from the process itself – has grown substantially, and in tandem with the growth in Professional RMA Societies. This chapter will use the example of impact to explore the significance of RMA to the research environment, the challenges and institutional responses to sector change, and the vital governance role to optimise ‘space’ for academics to deliver robust and original research.

Most practitioners use only an anteroposterior (AP) or oblique view. Although injection into the periarticular space may yield adequate pain relief, intraarticular needle placement is imperative to identify SIJ pathology and plan future management. This study highlights the importance of obtaining an additional lateral view during fluoroscopy to better evaluate SIJ disease.Methods: A retrospective review of 38 patients who underwent fluoroscopy guided SIJ injection was conducted, for which IRB approval was granted by the MetroWest Medical Center Institutional Review Board. Patient demographics (age, sex, BMI) and pre- and post-operative numerical rating scale (NRS) scores were collected, and initial needle location was reviewed. Patients were placed into groups according to initial needle location. Statistical analysis was conducted using a MannâWhitney U-test with significance defined as p < 0.05.Results: The 21 females and 17 males had a mean age and BMI of 70.5 years and 27.8 kg/m2, respectively. Thirty-one patients had initial intraarticular needle placement confirmed with lateral arthrogram, and 7 patients had initial periarticular needle placement, requiring needle readjustment in lateral confirmatory

20. Simulation of Flood-Induced Human Migration at the Municipal Scale: A Stochastic Agent-Based Model of Relocation Response to Coastal Flooding

Abstract: Human migration triggered by flooding will create sociodemographic, economic, and cultural challenges in coastal communities, and adaptation to these challenges will primarily occur at the municipal level. However, existing migration models at larger spatial scales do not necessarily capture relevant social responses to flooding at the local and municipal levels. Furthermore, projecting migration dynamics into the future becomes difficult due to uncertainties in human–environment interactions, particularly when historic observations are used for model calibration. This study proposes a stochastic agent-based model (ABM) designed for the long-term projection of municipal-scale migration due to repeated flood events. A baseline model is demonstrated initially, capable of using stochastic bottom-up decision rules to replicate county-level population. This approach is then combined with physical flood-exposure data to simulate how population projections diverge under different flooding assumptions. The methodology is applied to a study area comprising 16 counties in coastal Virginia and Maryland, U.S., and include rural areas which are often overlooked in adaptation research. The results show that incorporating flood impacts results in divergent population growth patterns in both urban and rural locations, demonstrating potential municipal-level migration response to coastal flooding.

Abstract: The objective of the research is determining the influence of the Academic Reinforcement Workshop on improving the learning of integral calculus in students of the second cycle of Systems Engineering and Computer Science of the Faculty of Sciences, semester 2022-II- UNASAM -Huaraz, 2023. The research is applied type, quantitative method and pre-experimental design; Two instruments were applied: Pre test and post test validated by two experts, applied to a sample of 26 students. The variables analyzed Workshop on academic reinforcement and learning of integral calculus, in whose research an affirmative hypothesis and a null hypothesis were formulated. After analyzing the results, it is stated that there is a significant influence of the academic reinforcement workshop on the learning of integral calculus, so that the affirmative hypothesis is accepted. The verification of the hypothesis was done by applying Student's T. It is concluded that there is a significant influence of the academic reinforcement workshop on the learning of integral calculus in students of the professional career of Systems Engineering and Computer Science, semester 2022-II, UNASAM -Huaraz, 2023., El objetivo de la investigación es determinar la influencia del Taller de reforzamiento académico en la mejora del aprendizaje del cálculo integral en estudiantes del segundo ciclo de Ingeniería de Sistemas e Informática de la Facultad de Ciencias, semestre 2022-II- UNASAM -Huaraz, 2023. La investigación es tipo aplicada, método cuantitativo y diseño pre experimental; se aplicaron dos instrumentos: Pre test y post test validados por dos expertos, aplicados a una muestra de 26 estudiantes. Las variables analizadas Taller de reforzamiento académico y aprendizaje del cálculo integral, en cuya investigación se formuló una hipótesis afirmativa y una hipótesis nula. Abstract: This paper examines empathetic behavior in the United States, a strongly individualistic country, as contrasted with Hungary and Ethiopia, which are moderately individualistic and strongly collectivistic respectively. It suggests that empathy may have a wider than originally perceived application in diverse settings to combat factors of ethnic bias and discrimination that adversely impact health. Models that distinctly focus on the development of healthcare provider empathic care are needed to enable the needs of resource scarce regions of the world to be met, including pockets of the U.S. More investigation is warranted on how empathic behavior can positively impact health outcomes and disparities.

Abstract: Preventing pressure ulcers is fundamental in managing and caring for the patient following a fragility fracture. Patients who have sustained a hip fracture and have had associated surgery are at high risk as ageing skin, frailty, immobility, and multiple comorbidities are significant factors in skin injury. The causes of pressure ulcers are complex, and their prevention is the responsibility of the entire multidisciplinary team. Accessing evidence and education for practitioners is vital to making a difference in all aspects of care.

29. Polyphony of Voices and the Silent-Others: A Reflection of Anthropocentrism at the Gabriela Mistral School in Popayán, Colombia

Abstract: This paper presents part of the findings from the research project Sense of Respect for Life: Between the Silent-Other and the School, conducted as part of a master’s degree in education at the University of Cauca, Colombia. We explore the way in which humans have positioned themselves as 'superior beings' in their relationship with animals, referred to as the Silent Others by San Martín (2016). The objective was to understand the sense of respect for life attributed to the Silent-Other by students and teachers in the field of Natural Sciences in the eighth grade at the Gabriela Mistral School in the city of Popayán. We followed the phenomenological method based on Lester Embree's (2003) Reflective Analysis proposal, which allowed us to achieve the three phenomenological reductions: natural, eidetic, and transcendental. The study revealed an anthropocentric perspective of the Silent-Others, perceiving them as beings with human-like feelings and upon whom the selfhood of humans is projected., [Introducción]: Se presenta una parte de los hallazgos de la investigación Sentido de Respeto a la Vida: Entre el Otro-Silencioso y la Escuela, desarrollada en el marco de la Maestría en Educación de la Universidad del Cauca, Colombia. Se comprende el modo en que el ser humano se ha situado como “ser superior” en su relación con los animales, denominados por San Martín (2016) los Otros Silenciosos. El objetivo fue comprender el sentido de respeto a la vida por el Otro-Silencioso que otorgan los estudiantes y maestros del área de ciencias naturales de octavo grado de la Institución Educativa Gabriela Mistral de la ciudad de Popayán. Se siguió el método fenomenológico Análisis Reflexivo de Lester Embree (2003); el diseño permitió alcanzar las tres reducciones fenomenológicas: natural, eidética y trascendental. Abstract: Introduction Harm arising from National Health Service (NHS) healthcare results in significant human cost for the patient, those who care for them, and the medical staff involved. Furthermore, patient harm results in substantial financial costs to the public purse. Improving how NHS providers in England respond to patient harm could reduce the number of claims for clinical negligence brought against NHS. Doing so will ensure those affected receive the justice they deserve and protect the public purse. Law and policy are key to supporting providers of NHS healthcare to respond to patient harm but are not necessarily understood and therefore can be challenging to apply to practice. Research exploring how law and policy supports this understanding is limited. The purpose of this scoping review is to address this knowledge gap and improve understanding by critically evaluating how law and policy supports providers of NHS healthcare in England to respond to patient harm. Methods and analysis The review will use the methodological framework proposed by Arskey and O’Malley, Levac et al and the Joanna Briggs Institute. Search strategies will be developed using selected key words and index terms. MEDLINE, CINAHL, and Westlaw and reference lists of relevant publications will be searched to identify relevant grey literature. Two reviewers will independently assess the extracted data against the eligibility criteria. All studies identified will be charted and the results presented as a narrative synthesis.

31. Dysphagia management in community/home settings: a scoping review investigating practices in Africa

Abstract: Purpose Little is known about how people living with dysphagia in rural, socio-economically impoverished contexts in Africa are supported and manage their disability. This scoping review sought to map and synthesise evidence relating to the management of dysphagia in adults in community/home settings in Africa as a “starting point” for a broader study on this topic. Method A multifaceted search strategy involved searches of electronic databases and grey literature, hand searches, ancestry searches, and consultation with expert advisors. Records were screened by two blinded reviewers. Characteristics of included studies were summarised, and their findings synthesised using the Framework approach. Result Six studies were included, relating to services for people with dysphagia secondary to various aetiologies. No grey literature was identified, that provided service delivery descriptions or practice guidance. This limited evidence suggests little professional support is available to people living with dysphagia in the community. Individuals and carers use a range of strategies, including choosing different food and drink items and modifying how food is chewed and swallowed. Conclusion Further research is required to understand current practice in managing dysphagia in the community in Africa, and the needs and priorities of community members who experience dysphagia and their carers.

32. Personal values, subjective wellbeing, and the effects of perceived social support in childhood: A pre-registered study

Abstract: Personal values are broad motivational goals that have been found to have systematic relations with subjective wellbeing in adults. Values that promote higher subjective wellbeing are considered healthy while those that hamper it are considered unhealthy (Schwartz & Sortheix, 2018). However, little is known about these relations in children. This pre-registered study examined (1) whether the values of children (6 to 12 years of age) relate to their subjective wellbeing and (2) whether these relations are moderated or mediated by perceived social support from parents, teachers, classmates, and close friends. These research questions were examined with a sample of 738 primary school students (50% female). Our results show that healthy growth values were positively related to subjective wellbeing overall, and for the subgroups of girls and children 9 to 12 years but not boys and children 6 to 8 years; however, unhealthy anxiety values were only negatively associated with subjective wellbeing for girls. While perceived social support partially mediated relations between growth values and subjective wellbeing, the direct values-wellbeing relations accounted for over half the variance. Interestingly, this study also found that growth values positively, and anxiety values negatively, influenced perceived social support from all referents. While perceived social support did not moderate values-wellbeing relations in the overall sample, differences were found in the way perceived social support moderated these relations in some age and gender subgroups. Taken together, these findings suggest that healthy growth values positively influence subjective wellbeing in middle childhood, even after accounting for perceived social support.

Abstract: Over the last decades, theoretical perspectives in the interdisciplinary field of the affective sciences have proliferated rather than converged due to differing assumptions about what human affective phenomena are and how they work. These metaphysical and mechanistic assumptions, shaped by academic context and values, have dictated affective constructs and operationalizations. However, an assumption about the purpose of affective phenomena can guide us to a common set of metaphysical and mechanistic assumptions. In this capstone paper, we home in on a nested teleological principle for human affective phenomena in order to synthesize metaphysical and mechanistic assumptions. Under this framework, human affective phenomena can collectively be considered algorithms that either adjust based on the human comfort zone (affective concerns) or monitor those adaptive processes (affective features). This teleologically-grounded framework offers a principled agenda and launchpad for both organizing existing perspectives and generating new ones. Ultimately, we hope the Human Affectome brings us a step closer to not only an integrated understanding of human affective phenomena, but an integrated field for affective research.

Abstract: Additive manufacturing (AM), known as 3D printing, enables rapid fabrication of geometrically complex copper (Cu) components for electrical conduction and heat management applications. However, pure Cu or Cu alloys produced by 3D printing often suffer from either low strength or low conductivity at room and elevated temperatures. Here, we demonstrate a design strategy for 3D printing of high strength, high conductivity Cu by uniformly dispersing a minor portion of lanthanum hexaboride (LaB6) nanoparticles in pure Cu through laser powder bed fusion (L-PBF). We show that trace additions of LaB6 to pure Cu results in an improved L-PBF processability, an enhanced strength, an improved thermal stability, all whilst maintaining a high conductivity. The presented strategy could expand the applicability of 3D printed Cu components to more demanding conditions where high strength, high conductivity and thermal stability are required.

37. Fire-Induced Cracking of Modern Window Glazing: An Experimental Study

Abstract: The knowledge of the behaviour of windows and, thus, of the actual ventilation in a fire room is paramount for predicting the fire development and the thermal response of structural elements. In this study, 75 tests were conducted on different glazing units made of annealed, toughened, and laminated glass, to investigate how modern insulating glazing units (IGUs) break and contribute to the ventilation factor in fires. The glass was tested as single, double, and triple pane glazing. The tests considered variations in glass pane thickness (3-8 mm) and sizes (side lengths 200-500 mm). A constant radiant heat flux of ca. 20 kW/m2 was applied to simulate post-flashover conditions. No cracking and no fallout were observed in toughened and laminated glass panes, respectively. In annealed glass pane units, the onset of cracking of subsequent panes was delayed depending on the amount of fallout of the first pane, indicating that such unit could provide no ventilation at flashover.

Abstract: Hybrid-electric aircraft represent a promising solution for the urgent need to decarbonize short-haul flights and bolster aviation sustainability. Nevertheless, the realization of hybrid-electric aircraft demands rigorous environmental impact analysis, given the substantial investments, time, and research required for technology development. This study offers a comprehensive life cycle inventory spanning the years 2030, 2040, and 2050 for both conventional and hybrid-electric aircraft configurations. Our inventory datasets are meticulously constructed through a systematic approach, ensuring data harmonization by drawing upon scientific literature, industry expertise, and primary data sources. This extensive dataset encompasses all pertinent systems necessary to model the environmental footprint of flights covering distances ranging from 200 to 600 nautical miles, utilizing a 50-passenger aircraft with the ATR42 as a reference model. Additionally, we furnish supplemental data for end-of-life considerations and uncertainty analysis. The systems under examination include the airframe, powertrain, power electronics and drives, batteries, fuel cells, hydrogen onboard storage, airport infrastructure, and battery charging stations. Notably, the carbon footprint of conventional aircraft aligns with data from the ecoinvent v3.8 database; however, our provided datasets are more than tenfold more detailed and incorporate a forward-looking perspective. These meticulously curated life cycle inventories can be amalgamated to simulate the potential environmental ramifications of conventional aircraft powered by kerosene or alternative aviation fuels, hybrid-electric aircraft utilizing battery technology, and hybrid-electric aircraft employing hydrogen as a fuel in conjunction with batteries. In this context, our findings play a pivotal role in nurturing the development of technology roadmaps that prioritize environmental sustainability within the realm of regional aviation.

Abstract: Atlantic salmon (Salmo salar) might encounter toxic hydrogen sulphide (H2S) gas during aquaculture production. Exposure to this gas can be acute or chronic, with heightened levels often linked to significant mortality rates. Despite its recognised toxicity, our understanding of the physiological implications of H2S on salmon remains limited. This report details the mucosal and systemic physiological consequences in post-smolt salmon reared in brackish water at 12 ppt after prolonged exposure to elevated H2S levels over 4 weeks. The fish were subjected to two concentrations of H2S: 1 µg/L (low group) and 5 µg/L (high group). An unexposed group at 0 µg/L served as the control. Both groups exposed to H2S exhibited incremental mortality, with cumulative mortality rates of 4.7 % and 16 % for the low and high groups, respectively. Production performance, including weight and condition factors, were reduced in the H2S-exposed groups, particularly in the high group. Mucosal response of the olfactory organ revealed higher tissue damage scores in the H2S-exposed groups, albeit only at week 4. The high group displayed pronounced features such as increased mucus cell density and oedema-like vacuoles. Transcriptome analysis of the olfactory organ unveiled that the effects of H2S were more prominent at week 4, with the high group experiencing a greater magnitude of change than the low group. Genes associated with the extracellular matrix were predominantly downregulated, while the upregulated genes primarily pertained to immune response. H2S-induced alterations in the metabolome were more substantial in plasma than skin mucus. Furthermore, the number of differentially affected circulating metabolites was higher in the low group compared to the high group. 40. Comparing biogenic blue mussel (Mytilus edulis) reef definitions in Northern Europe: implications for management and conservation

Abstract: To combat biodiversity loss, the European Union (EU) established a network of protected areas to be implemented by 2000, termed Natura 2000 (N2000). However, several N2000 sites remain unmapped, and various habitats remain undefined. Delayed implementation allows continued habitat degradation and biodiversity loss. Targeting biogenic reefs by blue mussels (Mytilus edulis and Mytilus trossulus), this study 1) compared blue mussel reef definitions across EU member states surrounding the North Sea and Baltic Sea, 2) scrutinized biological mechanisms underpinning the habitat definitions, and 3) provided suggestions for harmonized habitat definitions, respecting spatial and biological variation. By comparing official definitions of blue mussel reefs applied in Denmark, Sweden, The United Kingdom, and Germany, this study revealed A) decadal delay in implementation, B) reef definitions varying significantly regarding number of parameters used for identification (e.g., mussel seabed area (m2), and proportion seabed mussel coverage (%)) and the associated parameter limits (e.g., 2.500 m2, and 30%, respectively). Specifically, parameter limits for identification of reefs range between 1 and 10,000 m2 for mussel area and between 5% and 30% seabed mussel coverage. The study failed to identify biological mechanisms justifying this variation. Variable habitat definitions, unjustified by biological and spatial variation, may result in uneven protection levels across borders, potentially compromising connectivity of the N2000 network. To avoid this scenario, our study highlights the need for compatible biogenic reef definitions reflecting spatial and biological properties and suggests moving from protection of individual reefs to protection of areas where habitat forming blue mussels are regularly occurring.

Abstract: The great pond snail Lymnaea stagnalis has served as a model organism for over a century in diverse disciplines such as neurophysiology, evolution, ecotoxicology and developmental biology. To support both established uses and newly emerging research interests we have performed whole genome sequencing (~ 176 x depth), assembly and annotation of a single individual derived from an inbred line. These efforts resulted in a final assembly of 943 Mb (L50 = 257; N50 = 957,215) with a total of 22,499 predicted gene models. As a first step towards understanding the hermaphroditic reproductive biology of L. stagnalis, we identified molecular receptors, specifically nuclear receptors (including newly discovered 2xDNA binding domain-NRs), G protein-coupled receptors, and receptor tyrosine kinases, that may be involved in the cellular specification and maintenance of simultaneously active male and female reproductive systems. A phylogenetic analysis of one particular family of GPCRs (Rhodopsin neuropeptide FMRFamide-receptor-like genes) shows a remarkable expansion that coincides with the occurrence of simultaneous hermaphroditism in the Euthyneura gastropods. As some GPCRs and NRs also showed qualitative differences in expression in female (albumen gland) and male (prostate gland) organs, it is possible that separate regulation of male and female reproductive processes may in part have been enabled by an increased abundance of receptors in the transition from a separate-sexed state to a hermaphroditic condition. These findings will support efforts to pair receptors with their activating ligands, and more generally stimulate deeper insight into the mechanisms that underlie the modes of action of compounds involved in neuroendocrine regulation of reproduction, induced toxicity, and development in L. stagnalis, and molluscs in general.

Abstract: Disorders of consciousness are neurological conditions characterized by impaired arousal and awareness of self and environment. Behavioural responses are absent or are present but fluctuate. Disorders of consciousness are commonly encountered as a consequence of both acute and chronic brain injuries, yet reliable epidemiological estimates would require inclusive, operational definitions of the concept, as well as wider knowledge dissemination among involved professionals. Whereas several manifestations have been described, including coma, vegetative state/unresponsive wakefulness syndrome and minimally conscious state, a comprehensive neurobiological definition for disorders of consciousness is still lacking. The scientific literature is primarily observational, and studies-specific aetiologies lead to disorders of consciousness. Despite advances in these disease-related forms, there remains uncertainty about whether disorders of consciousness are a disease-agnostic unitary entity with a common mechanism, prognosis or treatment response paradigm. Our knowledge of disorders of consciousness has also been hampered by heterogeneity of study designs, variables, and outcomes, leading to results that are not comparable for evidence synthesis. The different backgrounds of professionals caring for patients with disorders of consciousness and the different goals at different stages of care could partly explain this variability. The Prospective Studies working group of the Neurocritical Care Society Curing Coma Campaign was established to create a platform for observational studies and future clinical trials on disorders of consciousness and coma across the continuum of care. Abstract: A quarter of ischaemic strokes are lacunar subtype, typically neurologically mild, usually resulting from intrinsic cerebral small vessel pathology, with risk factor profiles and outcome rates differing from other stroke subtypes. This European Stroke Organisation (ESO) guideline provides evidence-based recommendations to assist with clinical decisions about management of lacunar ischaemic stroke to prevent adverse clinical outcomes. The guideline was developed according to ESO standard operating procedures and Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) methodology. We addressed acute treatment (including progressive lacunar stroke) and secondary prevention in lacunar ischaemic stroke, and prioritised the interventions of thrombolysis, antiplatelet drugs, blood pressure lowering, lipid lowering, lifestyle, and other interventions and their potential effects on the clinical outcomes recurrent stroke, dependency, major adverse cardiovascular events, death, cognitive decline, mobility, gait, or mood disorders. We systematically reviewed the literature, assessed the evidence and where feasible formulated evidence-based recommendations, and expert concensus statements. We found little direct evidence, mostly of low quality. We recommend that patients with suspected acute lacunar ischaemic stroke receive intravenous alteplase, antiplatelet drugs and avoid blood pressure lowering according to current acute ischaemic stroke guidelines. For secondary prevention, we recommend single antiplatelet treatment long-term, blood pressure control, and lipid lowering according to current guidelines. We recommend smoking cessation, regular exercise, other healthy lifestyle modifications, and avoid obesity for general health benefits. 44. Domestic gestures of breakage and repair: Transformation of material culture in contemporary sculpture

Abstract: This research is borne out of an object-based, visual art practice centred on sculpture and domestic material culture. In this exegesis, I examine the mutability of objects in the home environment, with particular emphasis on what I will call ‘peripheral objects:’ personal items that are not thrown way, but remain in the domestic periphery, in garages, under beds, or along the sides of houses. I argue that these objects, of ambiguous purpose and value, are infused with human experiences and therefore offer unique opportunities for investigating human subjectivity and psychology. This is particularly true when they are used as materials for sculpture. This exegesis considers the capacity of peripheral objects to convey the emotional value of daily routines and practices in the home. Of special interest to me are the repetitive, caredriven, material actions or gestures that are frequently put to use in the running of a family household. I use the term ‘domestic gesture’ to refer to actions including, but not limited to, wrapping, ripping, folding, smashing, stacking, arranging, stitching, and hanging. I assert that while these gestures may be motivated in the first instance by mundane practical necessity, they can also contain an implicit psychological complexity. Domestic gestures can constitute acts of intimacy and care for the family and its possessions. They can also be personally and psychologically challenging, acting as a material centre point for interpersonal obligation, sadness, grief, and regret. 45. Do people with dementia and carers get what they need? : Barriers in social care and carers needs assessments

Abstract: Background People with dementia and unpaid carers need to go through a social care or carers needs assessment to access and receive subsidised or fully-funded social care. With no previous evidence, this qualitative study aimed to provide insights into the access to, experiences of receiving and conducting social care or carers needs assessments, and access to social care. Methods Unpaid carers of people with dementia and professionals conducting social care or carers needs assessment living or working in England were interviewed remotely about their experiences between April and August 2023. Topic guides were co-produced with two unpaid carers, and both were supported to code anonymised transcripts. Thematic analysis was used to analyse the data. Findings Twenty-seven unpaid carers (n = 21) and professionals (n = 6) participated. Four themes were generated: (1) Issues with accessing needs assessments, not the process; (2) Knowledge of needs assessments and the health and social care system; (3) Expectations of unpaid carers; and (4) Post-assessment unmet needs. The most prominent barriers unpaid carers and their relatives with dementia encountered were awareness of and access to needs assessment. Unpaid carers were mostly unaware of the existence and entitlement to a needs assessment, and sometimes realised they had participated in one without their knowledge. Professionals described the pressures on their time and the lack of financial resources within services. Conclusions To facilitate improved access to dementia care and support for carers, the pathway to accessing needs assessments needs to be clearer, with better integration and communication between health and social care.

46. Clinical outcomes and adverse events of bariatric surgery in adults with severe obesity in Scotland : the SCOTS observational cohort study

Abstract: Background Bariatric surgery is a common procedure worldwide for the treatment of severe obesity and associated comorbid conditions but there is a lack of evidence as to medium-term safety and effectiveness outcomes in a United Kingdom setting. Objective To establish the clinical outcomes and adverse events of different bariatric surgical procedures, their impact on quality of life and the effect on comorbidities. Design Prospective observational cohort study. Setting National Health Service secondary care and private practice in Scotland, United Kingdom. Participants Adults (age >16 years) undergoing their first bariatric surgery procedure. Main outcome measures Change in weight, hospital length of stay, readmission and reoperation rate, mortality, diabetes outcomes (HbA1c, medications), quality of life, anxiety, depression. Data sources Patient-reported outcome measures, hospital records, national electronic health records (Scottish Morbidity Record 01, Scottish Care Information Diabetes, National Records Scotland, Prescription Information System). Results Between December 2013 and February 2017, 548 eligible patients were approached and 445 participants were enrolled in the study. Of those, 335 had bariatric surgery and 1 withdrew from the study. Mean age was 46.0 (9.2) years, 74.7% were female and the median body mass index was 46.4 (42.4; 52.0) kg/m2. Weight was available for 128 participants at 3 years: mean change was −19.0% (±14.1) from the operation and −24.2% (±12.8) from the start of the preoperative weight-management programme. One hundred and thirty-nine (41.4%) participants were readmitted to hospital in the same or subsequent 35 months post surgery, 18 (5.4% of the operated cohort) had a reoperation or procedure considered to be related to bariatric surgery gastrointestinal complications or revisions. Abstract: Background: The European Association for Palliative Care (EAPC) acknowledges palliative sedation as an important, broadly accepted intervention for patients with life-limiting disease experiencing refractory symptoms. The EAPC therefore developed 2009 a framework on palliative sedation. A revision was needed due to new evidence from literature, ongoing debate and criticism of methodology, terminology and applicability. Aim: To provide evidence- and consensus-based guidance on palliative sedation for healthcare professionals involved in end-of-life care, for medical associations and health policy decision-makers. Design: Revision between June 2020 and September 2022 of the 2009 framework using a literature update and a Delphi procedure. Setting: European. Participants: International experts on palliative sedation (identified through literature search and nomination by national palliative care associations) and a European patient organisation. Results: A framework with 42 statements for which high or very high level of consensus was reached. Terminology is defined more precisely with the terms suffering used to encompass distressing physical and psychological symptoms as well as existential suffering and refractory to describe the untreatable (healthcare professionals) and intolerable (patient) nature of the suffering. The principle of proportionality is introduced in the definition of palliative sedation. No specific period of remaining life expectancy is defined, based on the principles of refractoriness of suffering, proportionality and independent decision-making for hydration. Patient autonomy is emphasised. A stepwise pharmacological approach and a guidance on hydration decision-making are provided. Conclusions: This is the first framework on palliative sedation using a strict consensus methodology. It should serve as comprehensive and soundly developed information for healthcare professionals.

49. Immersive Storymaking : An Emerging Art Form for Virtual Reality Learning Environments

Abstract: Virtual Reality (VR) technologies are moving stories from non-participatory to participatory formats through the effects of immersion, interaction, presence and agency. The study’s thesis therefore is that the shifting storytelling paradigm from teller-listener to participant-builder requires a new art form that includes a grammar of making. The study presents the concept of storymaking as this emerging art form. The first objective was to develop a conceptual framework for a storymaking practice, and secondly to implement this framework to determine if it satisfied the need for a new paradigm. To do this, the works of key narrative theorists such as Aristotle (2016) and interactive narrative practitioners such as Louchart and Aylett (2005) were referenced in defining a story and making grammar that resulted in the Storymaking Framework. This Framework was then applied to the creation of a 360° VR experience on traditional Trinidad Carnival characters. The method of autoethnography captured important lessons from this practical exercise through researcher-as-instrument. However, to determine the relevance of storymaking, a small group of nine participants (6 students pursuing Caribbean Studies, 2 administrators and 1 teacher) evaluated the experience. The results from pre- and post-experience surveys revealed that the experience did in fact contribute to significant levels of immersion, interaction and presence in participants. Eight participants felt immersed; all nine participants felt sure they were moving through the experience on their own and all participants indicated that they had learned something new from being a part of the narrative. This study therefore pinpointed that the key factor that justified the need for the new storymaking art form was the embodiment of the participant in the story. Abstract: We describe the preparation, characterization, and imaging studies of rhenium carbonyl complexes with a pyta (4-(2-pyridyl)-1,2,3-triazole) or tapy (1-(2-pyridyl)-1,2,3-triazole)-based heteroaromatic N∧N ligand and thiolate or selenoate X ligand. The stability and photophysical properties of the selenolate complexes are compared with parent chloride complexes and previously described analogues with benzenethiolate ligands. Two complexes were imaged in A549 cells upon excitation at 405 nm. Colocalization studies suggest a lysosomal accumulation, while one parent chloride complex was described to localize at the Golgi apparatus. Preliminary fluorescence lifetime measurements and imaging demonstrate potential for application in time-resolved microscopy techniques due to the long and variable lifetimes observed in cellular environments, including an increase in lifetime between the solution and solid state many times larger than previously reported.
