We studied the association of immunological cell subsets with antitumor response and pulmonary toxicity in breast cancer patients treated with everolimus plus exemestane. METHODS: In this exploratory analysis, peripheral blood mononuclear cells (PBMCs) were collected at baseline and 14, 35, 60, and 90 days after start of treatment, and at the moment of pulmonary toxicity. The percentage and absolute number of T-cells, B-cells, NK-cells, monocytes and numerous subtypes were measured in peripheral blood using flow cytometric analysis and were compared using a (paired) t-test. RESULTS: From 20 patients, a total of 89 samples were collected. At baseline, responders versus non-responders had 0.86% versus 0.32% CD4+ effector cells (CD45RA+CD27-) (p = 0.1266) and non-response could be predicted with 0.71 sensitivity and 0.82 specificity. Patients who developed pulmonary toxicity compared to patients without pulmonary toxicity had relatively more NKT-cells at baseline (6.0% versus 1.3%, p = 0.0068, 59 k versus 12 k * 10(9)/l, p = 0.0081) and at the moment of toxicity (5.2% versus 1.2%, p = 0.0106 and 47 k versus 16 k * 10(9)/l, p = 0.0466). Baseline percentage NKT cells predicted pulmonary toxicity with 0.78 sensitivity and 1.0 specificity. CONCLUSIONS: Our results suggest that baseline CD4+ effector cells may be predictive of antitumor responses and baseline NKT cells may be predictive of pulmonary toxicity. These results warrant further validation.

Item does not contain fulltext, BACKGROUND: Observational data suggest that some donors might benefit from donating while others may be harmed. The aim of this study was to investigate the prevalence and potential, routinely measured, determinants of pre- and post-donation symptoms. MATERIALS AND METHODS: In Donor InSight, questionnaire data from 23,064 whole blood donors (53% female) were linked to routinely measured data on donors' physical characteristics (haemoglobin, blood pressure, body mass index and estimated blood volume) from the Dutch donor database. Absolute and relative associations between donors' physical donor and the presence of pre- and post-donation symptoms were studied using multivariable logistic regression. RESULTS: Pre-donation symptoms (lack of energy, headaches) were reported by 3% of men and 3% of women. Five percent of men and 4% of women reported positive post-donation symptoms (feeling fit, fewer headaches). Negative symptoms (fatigue, dizziness) were more common, occurring in 8% of men and 19% of women. All the studied donors' physical characteristics were positively associated with pre- and positive post-donation symptoms and negatively associated with negative symptoms. Body mass index was most consistently and independently associated with symptoms. DISCUSSION: Donors' physical characteristics, in particular body mass index, were consistently associated with pre- and post-donation symptoms. This indicates that subgroups of donors more and less tolerant to donation might be identifiable using routinely measured data. Further research is warranted to study underlying mechanisms and potential strategies to predict and prevent donor reactions.

The aim of this study was to develop a composite measure of multiple outcome parameters defined as 'textbook outcome', to assess quality of care for patients undergoing oesophagogastric cancer surgery. METHODS: Patients with oesophagogastric cancer, operated on with the intent of curative resection between 2011 and 2014, were identified from a national database (Dutch Upper Gastrointestinal Cancer Audit). Textbook outcome was defined as the percentage of patients who underwent a complete tumour resection with at least 15 lymph nodes in the resected specimen and an uneventful postoperative course, without hospital readmission. Hospital variation in textbook outcome was analysed after adjustment for case-mix factors. RESULTS: In total, 2748 patients with oesophageal cancer and 1772 with gastric cancer were included in this study. A textbook outcome was achieved in 29.7 per cent of patients with oesophageal cancer and 32.1 per cent of those with gastric cancer. Adjusted textbook outcome rates varied from 8.5 to 52.4 per cent between hospitals. The outcome parameter 'at least 15 lymph nodes examined' had the greatest negative impact on a textbook outcome both for patients with oesophageal cancer and for those with gastric cancer. CONCLUSION: Most patients did not achieve a textbook outcome and there was wide variation between hospitals.

Het geslacht Allium is met ruim 700 of misschien wel 800 soorten een zeer groot geslacht. Het is dan ook niet zo verwonderlijk dat de kennis van de soorten van dit geslacht nog veel hiaten vertoont. Het geslacht bestaat uit bolgewassen, waarbij bij diverse soorten de bollen op een wortelstok staan. Deze worden gezien als vaste planten. Hieronder zijn er enige die als tuinplant meer aandacht verdienen.

Het initiatief van de KVBC in 1961 om met de uitgave van Dendroflora te beginnen heeft een schitterend gevolg gekregen. Bij het verschijnen van Dendroflora 50 is het een goed moment om terug te kijken. Dendroflora heeft tot nu toe slechts drie hoofdredacteuren gekend. Ze hebben alledrie tijdens hun hoofdredacteurschap onmiskenbaar hun stempel gedrukt op de uitgaven.

Deze bijdrage heeft als basis de in 2012 gehouden Dendrologendag in Doorn. De belangrijkste lezing werd verzorgd door de Zweedse Eva Wallander, die in 2001 een interessant proefschrift heeft geschreven over de evolutie en de bloei van de es. Cees van der Linden besprak het gekweekte sortiment. Het geslacht staat op dit moment extra in de belangstelling vanwege de Essentaksterfte.

In aansluiting op Carpinus in Dendroflora 45 volgt nu het geslacht Ostrya (Hopbeuk). Van dit geslacht is op dit moment geen enkele soort belangrijk in cultuur. Er zijn ook geen cultivars bekend. Toch heeft dit vrij kleine geslacht enkele soorten die veel meer toepassing verdienen als laanbeplanting en in parken en plantsoenen. Het zijn gezonde, groeikrachtige bomen, die in de nazomer getooid zijn met fraaie hopbellen.

Op het moment wordt er veel gesproken over de chytridiomycose-schimmel in de hobby. Liefhebbers zijn bang dat hun kikkers de schimmel hebben of krijgen door de aanschaf van een nieuwe kikker voor de hobby. Stoppen met het kopen van kikkers is geen optie, de collectie zal aangevuld moeten worden bij het doodgaan van een fokman of vrouw. Maar hoe voorkom je dan dat je kikkers ziek worden door de aanschaf van een nieuwe kikker? Het antwoord daarop is quarantaine. Je zou nieuwe kikkers voor 60 dagen in quarantaine moeten houden om mogelijke besmettingen met ziektes of parasieten te herkennen en te behandelen.

Aan het geslacht Carpinus, de Haagbeuk, is tot nu toe in Dendroflora slechts eenmaal aandacht besteed. In nummer twee (1965) werd het sortiment besproken. Een klein sortiment van vier soorten, waarvan de bekenste, Carpinus betulus, als enige een viertal cultivars had. In nummer 24 (1987) werd C. laxiflora 'Pendula' vermeld als gouden medaille winnar op de tentoonstelling Flora Nova '87 en in nummer 43 (2006) werd melding gemaakt van een Bronzen Medaille op Groot Groen voor C. betulus 'Lucas'. De technische Keuringscommissie van de Naktuinbouw heeft studie gemaakt van het in Nederland gekweekte sortiment van C. betulus. Resultaten daarvan zijn in deze publicatie opgenomen.

Hoe komt het dat de ene boom zijn blad grauw verdord laat vallen en de andere afscheid neemt met een kleurrijk palet van goudgeel tot scharlakenrood en diep purper? Waarom verkleuren in sommige streken hele landschappen? Waarom toont een geïmporteerde boom, die op zijn natuurlijke groeiplaats zo prachtig verkleurde bij ons die kleuren vaak in het geheel niet? Allemaal vragen die aan de orde kwamen op de in november 2007 gehouden Dendrologendag in Wageningen. Het Nederlandse zeeklimaat geeft beperkingen aan de intensiteit van herftskleuren. Interessant daarbij is dat de natuurlijke processen die ten grondslag liggen aan herfstverkleuring door mutaties kunnen worden versterkt. Dit heeft al geresulteerd in diverse selecties die ook in ons land een goede herfstkleur geven. Dit aantal kan in de toekomst nog verder worden vergroot.
