Objective Gout treatment is largely suboptimal in clinical practice. We aimed to assess the predictors of disease-activity at 12 months in a real-life setting. Methods Consecutive patients referred to Rheumatology Units for suspected acute crystal-induced arthritis were enrolled in a multicentre-cohort study. Only patients with clinical diagnosis of gout were eligible. Disease-activity was evaluated by the Patient Acceptable Symptom State (PASS) on a visual analogue scale (VAS, 0=unsatisfactory, 100=satisfactory) at 0 (T0) and 12 months (T12), and the composite score called Gout Activity Score (GAS) calculated on the number of arthritic attacks (flare count), serum uric acid (sUA), cumulative number of tophi, VAS (T12), PtGA (T12). Multivariate linear regression model was performed to assess predictors of gout disease-activity at T12 with PASS and GAS as outcomes. Results: 201 patients had gout (diagnosis on synovial fluid in 45%, tophi in 26%, mean sUA 7.4±1.9 mg/L, 85% with urate-lowering therapy (ULT) in progress/initiated at T0); mean age 63±13 years, 88% men, median (interquartile range) disease duration 2.9 years (0.7-9.4). Follow-up visits were performed in 113 (56%) patients at T12. Mean PASS observed at T0 and at T12 were 38±27 and 74±23, respectively, whereas GAS at T12 was 10±8. A significant association was observed between the presence of tophi and PASS at T12 (-15.3, 95% CI -25.5, -5.2; p=0.003) and GAS at T12 (+4.0, 95% CI 0.6,7.4; p=0.02), adjusted for age, sex, disease duration, sUA <6 mg/dL, tender joint count, PASS at T0, ULT). CONCLUSIONS:The baseline presence of tophi may predict high disease-activity at T12, thus worsening GAS and patients’ pain perception.

Objective We aimed to assess the performance of the 2015 American College of Rheumatology/European League Against Rheumatism (ACR/EULAR) gout classification criteria in an Italian cohort of patients with crystal-induced arthritis stratified by disease duration and gender in a real-life setting. Methods Consecutive patients referred to Rheumatology Units for suspected acute crystal-induced arthritis were enrolled in a multicentre cohort study by the Italian Society of Rheumatology which was designed to improve the management of crystal-induced arthritis (ATTACk). To test the performance of the criteria (sensitivity and specificity), the presence of monosodium urate (MSU) crystals in synovial fluid (SF) was used as gold standard. Subgroup analyses by gender and disease duration were performed. Results Two hundred and seventy-seven patients were enrolled. SF analysis was available in 137 (49%) patients. Complete SF analysis and ACR/EULAR scores were obtained in 44% of patients. MSU crystals were found in 66% of patients. The sensitivity and the specificity of all criteria sets were 78% (95%CI, 67–86) and 98% (95%CI, 87–100), respectively; only clinical criteria yielded 70% (95%CI, 59–80) sensitivity and 93% (95%CI, 80-98) specificity, respectively. In early-stage disease (<2 years), the sensitivity dropped to 58% (95%CI, 39-75), while the specificity was 100% (95%CI, 85–100). Conclusion The ACR/EULAR criteria showed good performance in patients presenting with acute arthritis; changes were observed when a subset of criteria were used, especially in early-stage disease.

Objective: In this study, we aim to assess the current situation of postgraduate medical education in obstetrics and gynaecology in Germany, Austria and Switzerland. In addition, we aim to determine transferable advantages amongst the countries. Study design: We performed a survey through a digital questionnaire with a total of 40 questions. The survey was advertised via communication channels of the German, Austrian and Swiss gynaecological societies; the participants were enrolled anonymously. Results: A total of 422 trainees took part in the survey. Differences within the three countries where found regarding the workload and the training of sub-specialties. Generally, the participants described to spend the majority of their daily working hours on documentation. Concerning assessment of current training regulations, more than half of trainees stated that they were actually faced with notable difficulties to fulfil the required obligatory numbers of self-performed interventions being documented. When asked for their intrinsic feeling of safety, around two-third of trainees felt "confident to very confident" during standard interventions. These numbers were up to 12% higher in the group of trainees who experienced simulation training during their education. Conclusion: With the help of this survey, weak points can be identified such as workload and implementation of current training regulations. Projects and ideas as EBCOG PACT, EPAs, the reduction of bureaucracy through digitization and deepening skills through simulation make a valuable contribution to compensate for these deficits and to adapt to future requirements., Ziel: Diese Studie stellt die aktuelle Situation der medizinischen Weiterbildung zum Facharzt für Gynäkologie und Geburtshilfe in Deutschland, Österreich und der Schweiz dar. Zusätzlich werden adaptierbare Stärken der jeweiligen Länder identifiziert.Studiendesign: Wir führten eine Online-Umfrage mit 30 Fragen durch. Die Umfrage wurde durch verschiedenste Kommunikationskanäle der Deutschen, Österreichischen und Schweizer Gesellschaft für Gynäkologie und Geburtshilfe beworben. Die Teilnahme war freiwillig und anonym.Resultat: 422 Assistenzärzt*innen und junge Fachärzt*innen nahmen an der Umfrage teil. Ein beunruhigendes Ergebnis ist die Tatsache, dass mehr als die Hälfte der Teilnehmer*innen angaben, Schwierigkeiten mit der Erfüllung der geforderten Fallzahlen für selbstdurchgeführte Eingriffe zu haben. Auf die Frage, wie sicher sie sich bei Standardeingriffen fühlen, gaben 2/3 an, "sich sicher bis sehr sicher" zu fühlen. Diese Zahl war 12% höher bei Assistenzärzt*innen, welche während ihrer Weiterbildung Simulationstrainings besucht haben.Zusammenfassung: Mit Hilfe dieser Umfrage konnten Schwachstellen bei der Arbeitsverteilung und der Umsetzung aktueller Weiterbildungsinhalte identifiziert werden. Projekte und Ideen wie EBCOG PACT und EPA, die Reduktion von Bürokratie, und die Vertiefung von Fähigkeiten durch Simulationstraining helfen individuelle Schwachstellen zu kompensieren und für die Zukunft vorbereitet zu sein.

Every measure for each Outcome Measures in Rheumatology core domain was considered. A 3-step approach (factor analysis, linear discriminant analysis, and linear regression) was applied to derive the Gout Activity Score (GAS). Decision to change treatment or 6-month flare count were used as the surrogate criteria of high disease activity. Baseline and 12-month followup data of 446 patients included in the Kick-Off of the Italian Network for Gout cohort were used. Construct- and criterion-related validity were tested. External validation on an independent sample is reported. Results: Factor analysis identified 5 factors: patient-reported outcomes, joint examination, flares, tophi, and serum uric acid (sUA). Discriminant function analysis resulted in a correct classification of 79%. Linear regression analysis identified a first candidate GAS including 12-month flare count, sUA, visual analog scale (VAS) of pain, VAS global activity assessment, swollen and tender joint counts, and a cumulative measure of tophi. Alternative scores were also developed. The developed GAS demonstrated a good correlation with functional disability (criterion validity) and discrimination between patient- and physician-reported measures of active disease (construct validity). The results were reproduced in the external sample. Conclusion: This study developed and validated a composite measure of disease activity in gout. Further testing is required to confirm its generalizability, responsiveness, and usefulness in assisting with clinical decisions.

Diet and lifestyles modification are core aspects of the non-pharmacological management of gout, but a poor consistency with suggested guidelines is reported. This study aimed to investigate dietary and lifestyle habits of patients with gout followed in rheumatology settings. Data were retrieved from the baseline dataset of the KING study, a multicentre cohort study of patients with gout followed in rheumatology settings. Dietary habits were assessed with the Italian National Institute of Statistics (ISTAT) food-frequency questionnaire and compared with reported data about general population. The relative increase of exposure was estimated by standardized prevalence ratios adjusted for gender, age and geographical distribution. The study population included 446 patients, with a mean age of 63.9 years and a M/F ratio of 9:1. Compared to the Italian population, gouty patients showed a higher prevalence of obesity [1.82 (1.52-2.18)] and a higher consumption of wine [1.85 (1.48-2.32)] and beer [2.21 (1.68-2.90)], but a lower prevalence of smoking and a lower intake of liquor. They showed a lower intake of red meat [0.80 (0.71-0.91)], but a similar intake of other tested dietary factors. Gouty patients’ lifestyle is still partially different from the recommended.

The Italian Society of Rheumatology (SIR) aimed to update, adapt to national contest and disseminate the 2006 EULAR recommendations for the management of gout. Methods: The multidisciplinary group of experts included rheumatologists, general practitioners, internists, geriatricians, nephrologists, cardiologists and evidence-based medicine experts. To maintain consistency with EULAR recommendations, a similar methodology was utilized by the Italian group. The original propositions were translated in Italian and priority research queries were identified through a Delphi consensus approach. A systematic search was conducted for selected queries. Efficacy and safety data on drugs reported in RCTs were combined in a meta-analysis where feasible. The strength of recommendation was measured by utilising the EULAR ordinal and visual analogue scales. Results: The original 12 propositions were translated and adapted to Italian context. Further evidences were collected about the role of diet in the non-pharmacological treatment of gout and the efficacy of oral corticosteroids and low-dose colchicine in the management of acute attacks. Statements concerning uricosuric treatments were withdrawn and replaced with a proposition focused on a new urate lowering agent, febuxostat. A research agenda was developed to identify topics still not adequately investigated concerning the management of gout. Conclusions: The SIR has developed updated recommendations for the management of gout adapted to the Italian healthcare system. Their implementation in clinical practice is expected to improve the management of patients with gout.

The multidisciplinary group of experts included specialists involved in the management of patients with hand OA. In order to maintain consistency with EULAR recommendations, a similar methodology was utilized by the Italian group. The original propositions were reformulated in terms of a search query and for every recommendation a systematic search was conducted updating EULAR recommendations' review. The propositions were translated in Italian and reformulated basing on collected evidences and expert opinion. The strength of recommendation was measured for each proposition with the EULAR ordinal and visual analogue scales. The original 11 propositions of EULAR recommendations were translated and adapted to Italian context. Further evidences were collected about non-pharmacological therapies, local treatments, intra-articular injection with SYSADOA and corticosteroids, and surgery. The SIR has developed updated recommendations for the management of hand OA adapted to the Italian healthcare system. Their implementation in clinical practice is expected to improve the management of patients with hand OA.

Seabirds are amongst the most globally-threatened of all groups of birds, and conservation issues specific to albatrosses (Diomedeidae) and large petrels (Procellaria spp. and giant petrels Macronectes spp.) led to drafting of the multi-lateral Agreement on the Conservation of Albatrosses and Petrels (ACAP). Here we review the taxonomy, breeding and foraging distributions, population status and trends, threats and priorities for the 29 species covered by ACAP. Nineteen (66%) are listed as threatened by IUCN, and 11 (38%) are declining. Most have extensive at-sea distributions, and the greatest threat is incidental mortality (bycatch) in industrial pelagic or demersal longline, trawl or artisanal fisheries, often in both national and international waters. Mitigation measures are available that reduce bycatch in most types of fisheries, but some management bodies are yet to make these mandatory, levels of implementation and monitoring of compliance are often inadequate, and there are insufficient observer programmes collecting robust data on bycatch rates. Intentional take, pollution (including plastic ingestion), and threats at colonies affect fewer species than bycatch; however, the impacts of disease (mainly avian cholera) and of predation by introduced species, including feral cats (Felis catus), rats (Rattus spp.) and house mice (Mus musculus), are severe for some breeding populations. Although major progress has been made in recent years in reducing bycatch rates and in controlling or eradicating pests at breeding sites, unless conservation efforts are intensified, the future prospects of many species of albatrosses and large petrels will remain bleak.

Most have extensive at-sea distributions, and the greatest threat is incidental mortality (bycatch) in industrial pelagic or demersal longline, trawl or artisanal fisheries, often in both national and international waters. Mitigation measures are available that reduce bycatch in most types of fisheries, but some management bodies are yet to make these mandatory, levels of implementation and monitoring of compliance are often inadequate, and there are insufficient observer programmes collecting robust data on bycatch rates. Intentional take, pollution (including plastic ingestion), and threats at colonies affect fewer species than bycatch; however, the impacts of disease (mainly avian cholera) and of predation by introduced species, including feral cats (Felis catus), rats (Rattus spp.) and house mice (Mus musculus), are severe for some breeding populations. Although major progress has been made in recent years in reducing bycatch rates and in controlling or eradicating pests at breeding sites, unless conservation efforts are intensified, the future prospects of many species of albatrosses and large petrels will remain bleak.

While sustainable forestry in Europe is characterized by the provision of a multitude of forest ecosystem services, there exists no comprehensive study that scrutinizes their sensitivity to forest management on a pan-European scale, so far. We compile scenario runs from regionally tailored forest growth models and Decision Support Systems (DSS) from 20 case studies throughout Europe and analyze whether the ecosystem service provision depends on management intensity and other co-variables, comprising regional affiliation, social environment, and tree species composition. The simulation runs provide information about the case-specifically most important ecosystem services in terms of appropriate indicators. We found a strong positive correlation between management intensity and wood production, but only weak correlation with protective and socioeconomic forest functions. Interestingly, depending on the forest region, we found that biodiversity can react in both ways, positively and negatively, to increased management intensity. Thus, it may be in tradeoff or in synergy with wood production and forest resource maintenance. The covariables species composition and social environment are of punctual interest only, while the affiliation to a certain region often makes an important difference in terms of an ecosystem service’s treatment sensitivity.

Polynucleotide phosphorylase (PNPase), an enzyme conserved in bacteria and eukaryotic organelles, processively catalyzes the phosphorolysis of RNA, releasing nucleotide diphosphates, and the reverse polymerization reaction. In Escherichia coli, both reactions are implicated in RNA decay, as addition of either poly(A) or heteropolymeric tails targets RNA to degradation. PNPase may also be associated with the RNA degradosome, a heteromultimeric protein machine that can degrade highly structured RNA. Here, we report that ATP binds to PNPase and allosterically inhibits both its phosphorolytic and polymerization activities. Our data suggest that PNPase-dependent RNA tailing and degradation occur mainly at low ATP concentrations, whereas other enzymes may play a more significant role at high energy charge. These findings connect RNA turnover with the energy charge of the cell and highlight unforeseen metabolic roles of PNPase.

Polynucleotide phosphorylase (PNPase) is a phosphate-dependent 3' to 5' exonuclease widely diffused among bacteria and eukaryotes. The enzyme, a homotrimer, can also be found associated with the endonuclease RNase E and other proteins in a heteromultimeric complex, the RNA degradosome. PNPase negatively controls its own gene (pnp) expression by destabilizing pnp mRNA. A current model of autoregulation maintains that PNPase and a short duplex at the 5'-end of pnp mRNA are the only determinants of mRNA stability. During the cold acclimation phase autoregulation is transiently relieved and cellular pnp mRNA abundance increases significantly. Although PNPase has been extensively studied and widely employed in molecular biology for about 50 years, several aspects of structure-function relationships of such a complex protein are still elusive. In this work, we performed a systematic PCR mutagenesis of discrete pnp regions and screened the mutants for diverse phenotypic traits affected by PNPase. Overall our results support previous proposals that both first and second core domains are involved in the catalysis of the phosphorolytic reaction, and that both phosphorolytic activity and RNA binding are required for autogenous regulation and growth in the cold, and give new insights on PNPase structure-function relationships by implicating the alpha-helical domain in PNPase enzymatic activity. The RNA degradosome is a bacterial protein machine devoted to RNA degradation and processing. In Escherichia coli it is typically composed of the endoribonuclease RNase E, which also serves as a scaffold for the other components, the exoribonuclease PNPase, the RNA helicase RhlB, and enolase. Several other proteins have been found associated to the core complex. However, it remains unclear in most cases whether such proteins are occasional contaminants or specific components, and which is their function. To facilitate the analysis of the RNA degradosome composition under different physiological and genetic conditions we set up a simplified preparation procedure based on the affinity purification of FLAG epitope-tagged RNase E coupled to Multidimensional Protein Identification Technology (MudPIT) for the rapid and quantitative identification of the different components. By this proteomic approach, we show that the chaperone protein DnaK, previously identified as a "minor component" of the degradosome, associates with abnormal complexes under stressful conditions such as overexpression of RNase E, low temperature, and in the absence of PNPase; however, DnaK does not seem to be essential for RNA degradosome structure nor for its assembly. In addition, we show that normalized score values obtain by MudPIT analysis may be taken as quantitative estimates of the relative protein abundance in different degradosome preparations.
