3. IoT-Based Technology Solution for Monitoring Small Breed Cardiac Dogs with Heart Disease

Abstract

Caring for geriatric small-breed dogs with heart disease from home is a challenge due to the lack of technological tools available in the market, specifically in South America. This lack limits the quality of life of these canine companions and hinders early detection of cardiac problems. Heart disease is common in geriatric dogs of small and medium breeds and the lack of accurate information about their health prevents proper care. In this study, a technological solution based on an IoT device is proposed for monitoring geriatric cardiac dogs in Peru. The device allows measuring temperature and heart rate and recording electrocardiograms in real time for early detection of cardiac problems in pets. For the development of the technological solution, a methodology based on four phases was applied: (1) Analysis of IoT devices, (2) Construction of IoT devices, (3) System construction and (4) System validation. Two experiments were conducted in a private veterinary medical center with two small-breed geriatric dogs with heart diseases, in which each dog had its Temperature (T) and Heart Rate (HR) measured in real-time. Comparing the results of both experiments, the temperature parameter had a better accuracy with an error rate (er) of less than 1%, compared to the average error rate of 7.95% for heart rate. In addition, a survey of veterinarians' experience with the resulting system showed high scores for usability and monitoring, confirming the potential of IoT systems for monitoring geriatric small breed dogs with cardiac problems with the novelty of being a low-cost, usable and reliable device

5. Health information sources and health-seeking behaviours of Filipinos living in medically underserved communities: Empirical quantitative research.

Abstract

AIMS: To describe sources of health information and health-seeking behaviours of adults (aged ≥18) living in medically underserved communities in the Philippines. DESIGN: This is a secondary, quantitative analysis from a cross-sectional parent study. Participants completed a 10-item, self-report survey on their sources of health information, healthcare providers sought for health and wellness and health-seeking behaviours when ill. Responses were evaluated across two age groups (<60 vs. ≥60 years) and genders using generalized linear mixed models. RESULTS: Surveys were completed by 1202 participants in rural settings (64.6% female, mean age 49.5 ± 17.6). Friends and/or family were their key source of health information (59.6%), followed by traditional media (37%) and healthcare professionals (12.2%). For health promotion, participants went to healthcare professionals (60.9%), informal healthcare providers (17.2%) or others (7.2%). When ill, they visited a healthcare professional 69.1% of the time, self-medicated (43.9%), prayed (39.5%) or sought treatment from a rural health clinic (31.5%). We also found differences in health-seeking behaviours based on age and gender. CONCLUSIONS: Our findings highlight the need to organize programs that explicitly deliver accurate health information and adequate care for wellness and illness. Study findings emphasize the importance of integrating family, friends, media and healthcare professionals, including public health nurses, to deliver evidence-based health information, health promotion and sufficient treatment to medically underserved Filipinos. IMPLICATIONS: New knowledge provides valuable information to healthcare providers, including public health nurses, in addressing health disparities among medically underserved Filipinos. 6. Adapting and Evaluating a Theory-Driven, Non-Pharmacological Intervention to Self-Manage Pain.

Abstract

BACKGROUND: The existing literature has limited detail on theory-driven interventions, particularly in pain studies. We adapted Banduras self-efficacy framework toward a theory-driven, non-pharmacological intervention using auricular point acupressure (APA) and evaluated participants perceptions of this intervention on their pain self-management. APA is a non-invasive modality based on auricular acupuncture principles. METHODS: We mapped our study intervention components according to Banduras key sources of self-efficacy (performance accomplishments, vicarious experience, verbal persuasion, and emotional arousal) to facilitate the self-management of pain. Through a qualitative study design, we conducted virtual interviews at one and three months after a 4-week APA intervention among 23 participants using purposive sampling to describe their experiences in managing their pain based on our theory-driven APA intervention. RESULTS: Using thematic analyses, we found four themes: the enhanced self-management of pain, improved pain outcomes, the feasibility of technology, and the sustainability of APA. CONCLUSIONS: Describing how interventions are mapped according to the elements of theoretical frameworks can help to guide intervention development, advance science and knowledge development, and promote the implementation of interventions. As such, using Banduras self-efficacy theory as a foundation for the APA intervention, APA was found to be feasible and sustainable, improving self-management, pain intensity, and pain-related outcomes. Participants provided recommendations for the further improvement of this theory-driven intervention.

7. The Impact of Recurrence or Presence of a New Malignancy on Tracheoesophageal Puncture Device Failure

Abstract

In the United States there are approximately 67,000 cases of Head and Neck Cancer annually and 15,400 deaths. The NIH estimates that in 2023 approximately 12, 380 new cases of laryngeal cancer, were diagnosed which has a 61.6% 5-year relative survival rate.The total laryngectomy is a surgical procedure that removes all of the laryngeal structures and sections of the upper trachea. Tracheoesophageal speech is considered the gold standard for voice rehabilitation following total laryngectomy. Tracheoesophageal speech utilizes a voice prosthesis (VP) that is inserted through a puncture in the common wall separating the trachea from the esophagus. The current literature has established that many factors that can impact a VP lifespan. In patients with advanced laryngeal cancer (T4a), up to 10% of patients can develop a recurrence, while less severe cases can result in up to 25% of recurrence. However, there is limited information on how the development of a secondary malignancy or recurrence can impact VP device failures or their lifespan. Therefore, understanding the factors that may influence TEP device failures particularly in the context of cancer recurrence or new malignancies is essential in the management of patients.

9. Partially cooperative RL for hybrid action CRNs with imperfect CSI

Abstract

Cognitive radio networks (CRNs) mitigate spectrum scarcity by leveraging the holes in the licensed spectrum to enable Internet of Things (IoT) devices to opportunistically access the spectrum. However, IoT devices need to sense the spectrum before they can access it, which is an energy-intensive process and hinders the practical implementation of opportunistic spectrum access for energy-constrained IoT devices. In this context, reinforcement learning-based algorithms that encourage cooperation among IoT devices to eliminate the need for constant sensing are promising candidates for practical CRN implementation. As exciting as the application of reinforcement learning to CRNs is, benchmarking the performance of different algorithms is a huge challenge due to a lack of standardized comparison metrics, especially for hybrid action spaces that comprise both discrete and continuous actions. We propose a hybrid discrete-continuous space deep reinforcement learning algorithm that maximizes the energy efficiency of CRNs by optimizing sensing, cooperation, and transmission by IoT devices. We also analyze the algorithm’s performance by setting the theoretical upper bound for throughput and find that it reaches 99.4% of the theoretical upper bound, while its discrete action-space version reaches 96% and other baseline algorithms range between 70% and 86%.

Abstract

Backgrounds : Obesity occurs due to an imbalance between incoming and outgoing energy intake, so excess energy is stored as fat tissue. The prevalence of overweight and obese children in the world in 2010 was 6.7%, increasing to 9.1% in 2020. Frequent consumption of ready-to-eat foods can increase the incidence of obesity. Fast food usually contains large amounts of sodium, saturated fat, and cholesterol. Aims : This systematic review aims to determine the effect of fast food on the incidence of obesity in school children. Method : This study used a systematic review approach. The research data source used scientific journals that met the inclusion and exclusion criteria. Result : Based on nine scientific articles analyzed, seven journals showed that the habitual pattern of fast food consumption can increase the incidence of obesity in children, and two journals showed no effect of fast food consumption on obesity. Conclusion : Fast food consumption patterns in school children occur due to technological developments and cultural changes. Therefore, it is necessary to change behaviour and eating patterns in school children.

Abstract

The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. The importance of the CTFB is evidenced by the elevated number of citations in the scientific literature in diverse areas of biology, law, anthropology, education, forensic science, and veterinary science, among others.

Abstract

Autophagy-related genes have been closely associated with intestinal homeostasis. BECLIN1 is a component of Class III phosphatidylinositol 3-kinase complexes that orchestrate autophagy initiation and endocytic trafficking. Here we show intestinal epithelium-specific BECLIN1 deletion in adult mice leads to rapid fatal enteritis with compromised gut barrier integrity, highlighting its intrinsic critical role in gut maintenance. BECLIN1-deficient intestinal epithelial cells exhibit extensive apoptosis, impaired autophagy, and stressed endoplasmic reticulum and mitochondria. Remaining absorptive enterocytes and secretory cells display morphological abnormalities. Deletion of the autophagy regulator, ATG7, fails to elicit similar effects, suggesting additional novel autophagy-independent functions of BECLIN1 distinct from ATG7. Indeed, organoids derived from BECLIN1 KO mice show E-CADHERIN mislocalisation associated with abnormalities in the endocytic trafficking pathway. This provides a mechanism linking endocytic trafficking mediated by BECLIN1 and loss of intestinal barrier integrity. Our findings establish an indispensable role of BECLIN1 in maintaining mammalian intestinal homeostasis and uncover its involvement in endocytic trafficking in this process. Hence, this study has important implications for our understanding of intestinal pathophysiology.

16. Gamma-Ray Burst Detection with Poisson-FOCuS and Other Trigger Algorithms

Abstract

We describe how a novel online change-point detection algorithm, called Poisson-FOCuS, can be used to optimally detect gamma-ray bursts within the computational constraints imposed by miniaturized satellites such as the upcoming HERMES-Pathfinder constellation. Poisson-FOCuS enables testing for gamma-ray burst onset at all intervals in a count time series, across all timescales and offsets, in real time and at a fraction of the computational cost of conventional strategies. We validate an implementation with automatic background assessment through exponential smoothing, using archival data from Fermi-GBM. Through simulations of lightcurves modeled after real short and long gamma-ray bursts, we demonstrate that the same implementation has higher detection power than algorithms designed to emulate the logic of Fermi-GBM and Compton-BATSE, reaching the performance of a brute-force benchmark with oracle information on the true background rate, when not hindered by automatic background assessment. Finally, using simulated data with different lengths and means, we show that Poisson-FOCuS can analyze data twice as fast as a similarly implemented benchmark emulator for the historic Fermi-GBM on-board trigger algorithms.

Abstract

Food waste occurs when edible goods are thrown at any point along the food chain. Foodwaste occurs everywhere, including households, restaurants, fast food chains, and otherareas. Every year, around 1.3 billion tons of food are wasted, which is comparable to morethan half of the world's entire cereal production. Microbial fuel cells (MFCs) are a form ofbioelectrochemical system (BES) that uses bacteria to generate power by producing electronswithin their cells and transferring them across their cell. It is also useful in situations wherewaste, such as food and water waste, can be used to generate power through the use ofmicroorganisms. The results of the study in terms of construction materials, procedures, effectiveness, andpresentation, as observed by the two groups of respondents, chemical engineers, and foodtechnologists, are accepted. It indicates that there is no significant difference between the twogroups of respondents on the level of acceptability assessment of Microbial Fuel Cells usingFood Waste in terms of construction materials, procedures, effectiveness, and presentation. For further enhancement and improvement of the project, the ensuing discussions and therecommendations proposed are as follows: collect and store the food waste as soon aspossible to be able to produce microbes and be less time-consuming. The respondents alsorecommend using materials that can resist corrosion due to the food residue and looking intoother possible container materials that may reduce plastic waste. Moreover, they recommendconducting this experiment in a laboratory to avoid contamination and ensure health protocolsas well. Lastly, do the experiment for 3 or 2 months because it will only be able to light up asmall LED after two months.

20. Changes over time in patient-reported outcomes in patients with heart failure

Abstract

AIM: This paper describes the trajectory during 1 year of four patient-reported outcomes (PROs), namely, sleep, depressive symptoms, health-related quality of life (HrQoL), and well-being, in patients with heart failure (HF), their relationship and the patient characteristics associated with changes in these PROs. METHODS AND RESULTS: Data analyses of PROs from 603 patients (mean age 67 years; 29% female, 60% NYHA II) enrolled in the HF-Wii study. On short term, between baseline and 3 months, 16% of the patients experienced continuing poor sleep, 11% had sustained depressive symptoms, 13% had consistent poor HrQoL, and 13% consistent poor well-being. Across the entire 1-year period only 21% of the patients had good PRO scores at all timepoints (baseline, 3, 6, and 12 months). All others had at least one low score in any of the PROs at some timepoint during the study. Over the 12 months, 17% had consistently poor sleep, 17% had sustained symptoms of depression, 15% consistently rated a poor HrQoL, and 13% poor well-being. Different patient characteristics per PRO were associated with a poor outcomes across the 12 months. Age, education, New York Heart Association, and length of disease were related to two PRO domains and submaximal exercise capacity (6 min test), co-morbidity, and poor physical activity to one. CONCLUSION: In total, 79% of the patients with HF encountered problems related to sleep, depressive symptoms, HrQoL, and well-being at least once during a 1-year period. This underscores the need for continuous monitoring and follow-up of patients with HF and the need for dynamic adjustments in treatment and care regularly throughout the HF trajectory.

On short term, between baseline and 3 months, 16% of the patients experienced continuing poor sleep, 11% had sustained depressive symptoms, 13% had consistent poor HrQoL, and 13% consistent poor well-being. Across the entire 1-year period only 21% of the patients had good PRO scores at all timepoints (baseline, 3, 6, and 12 months). All others had at least one low score in any of the PROs at some timepoint during the study. Over the 12 months, 17% had consistently poor sleep, 17% had sustained symptoms of depression, 15% consistently rated a poor HrQoL, and 13% poor well-being. Different patient characteristics per PRO were associated with a poor outcomes across the 12 months. Age, education, New York Heart Association, and length of disease were related to two PRO domains and submaximal exercise capacity (6 min test), co-morbidity, and poor physical activity to one. CONCLUSION: In total, 79% of the patients with HF encountered problems related to sleep, depressive symptoms, HrQoL, and well-being at least once during a 1-year period. This underscores the need for continuous monitoring and follow-up of patients with HF and the need for dynamic adjustments in treatment and care regularly throughout the HF trajectory.

Abstract

Theatre or theatre is a collaborative form of fine arts that uses live performance, typically actors, to present the experience of a real or imagined event before a live audience in a specific place, often a stage. The Performers may communicate this experience to the audience through gestures, speech, song, music and dance. This study focuses on the Challenges encountered in making theatre productions of Performing Arts students of Bestlink College pf the Philippines. This study used a survey questionnaire conducted by the researchers to gather data from the 15 respondents. The respondents were into two groups, mainly the Male and the Female. As of the findings, 4 of the respondents who answered the survey questionnaire with a percentage of 26.7% were male respondents, whereas 73.3% percent or 11 students were female respondents. The subsequent recommendation about the study was to adapt to the challenges they experienced in making theatre productions, such as performing on the stage, rehearsing, and looking in the mirror when practising to boost their self-confidence. When challenges or problems are encountered, always see it as a lesson or new learning to pursue your dream.

Hospitality management is a broad field that oversees the day-to-day operational and commercial activities of businesses in the hospitality industry. This core helps hospitality and tourism management students improve their skills, knowledge, and behaviour as individuals. It also teaches them about the things they may encounter and experience in the future, especially in real life, such as how to treat customers or clients correctly, being a satisfying leader to manage everything, or being maintenance and housekeeping, among others. This study shows that hospitality and tourism management students should be hospitable, especially in actual hospitality and tourism industry services. People in the hospitality sector strive to improve their leadership abilities as they advance in their professions. The question that naturally follows is what constitutes leadership and how it applies to the hospitality sector. Abstract

We used the Imaging X-ray Polarimetry Explorer (IXPE) satellite to measure, for the first time, the 2-8 keV polarization of NGC 1068. We pointed IXPE for a net exposure time of 1.15 Ms on the target, in addition to two ~ 10 ks each Chandra snapshots in order to account for the potential impact of several ultraluminous X-ray source (ULXs) within IXPE's field-of-view. We measured a 2 - 8 keV polarization degree of 12.4% +/- 3.6% and an electric vector polarization angle of 101{\deg} +/- 8{\deg} at 68% confidence level. If we exclude the spectral region containing the bright Fe K lines and other soft X-ray lines where depolarization occurs, the polarization fraction rises up to 21.3% +/- 6.7% in the 3.5 - 6.0 keV band, with a similar polarization angle. The observed polarization angle is found to be perpendicular to the parsec scale radio jet. Using a combined Chandra and IXPE analysis plus multi-wavelength constraints, we estimated that the circumnuclear "torus" may sustain a half-opening angle of 50{\deg} - 55{\deg} (from the vertical axis of the system). Thanks to IXPE, we have measured the X-ray polarization of NGC 1068 and found comparable results, both in terms of polarization angle orientation with respect to the radio-jet and torus half-opening angle, to the X-ray polarimetric measurement achieved for the other archetypal Compton-thick AGN : the Circinus galaxy. Abstract

We report on X-ray polarization measurements of the extra-galactic Crab-like PSR B0540-69 and its Pulsar Wind Nebula (PWN) in the Large Magellanic Cloud (LMC), using a ~850 ks Imaging X-ray Polarimetry Explorer (IXPE) exposure. The PWN is unresolved by IXPE. No statistically significant polarization is detected for the image-averaged data, giving a 99% confidence polarization upper limit (MDP99) of 5.3% in 2-8 keV energy range. However, a phase-resolved analysis detects polarization for both the nebula and pulsar in the 4-6 keV energy range. For the PWN defined as the off-pulse phases, the polarization degree (PD) of (24.5 ${\pm}$ 5.3)% and polarization angle (PA) of (78.1 ${\pm}$ 6.2){\deg} is detected at 4.6${\sigma}$ significance level, consistent with the PA observed in the optical band. In a single on-pulse window, a hint of polarization is measured at 3.8${\sigma}$ with polarization degree of (50.0 ${\pm}$ 13.1)% and polarization angle of (6.2 ${\pm}$ 7.4){\deg}. A 'simultaneous' PSR/PWN analysis finds two bins at the edges of the pulse exceeding 3${\sigma}$ PD significance, with PD of (68 ${\pm}$ 20)% and (62 ${\pm}$ 20)%; intervening bins at 2-3${\sigma}$ significance have lower PD, hinting at additional polarization structure., Comment: 10 pages, 7 figures, 4 tables, author's version of the paper accepted for publication in ApJ

Abstract

The HERMES Technologic and Scientific Pathfinder project is a constellation of six CubeSats aiming to observe transient high-energy events such as the Gamma Ray Bursts (GRBs). HERMES will be the first space telescope to include a siswich detector, able to perform spectroscopy in the 2 keV to 2 MeV energy band. The particular siswich architecture, which combines a solid-state Silicon Drift Detector and a scintillator crystal, requires specific calibration procedures that have not been yet standardized in a pipeline. We present in this paper the HERMES calibration pipeline, mescal, intended for raw HERMES data energy calibration and formatting. The software is designed to deal with the particularities of the siswich architecture and to minimize user interaction, including also an automated calibration line identification procedure, and an independent calibration of each detector pixel, in its two different operating modes. The mescal pipeline can set the basis for similar applications in future siswich telescopes.

Abstract

Weakly magnetized neutron stars in X-ray binaries show complex phenomenology with several spectral components that can be associated with the accretion disk, boundary and/or spreading layer, a corona, and a wind. Spectroscopic information alone is, however, not enough to disentangle these components. Additional information about the nature of the spectral components and in particular the geometry of the emission region can be provided by X-ray polarimetry. One of the objects of the class, a bright, persistent, and rather peculiar galactic Type I X-ray burster was observed with the Imaging X-ray Polarimetry Explorer (IXPE) and the X-ray Multi-Mirror Mission Newton (XMM-Newton). Using the XMM-Newton data we estimated the current state of the source as well as detected strong absorption lines associated with the accretion disk wind. IXPE data showed the source to be significantly polarized in the 2-8 keV energy band with the overall polarization degree (PD) of 1.4% at a polarization angle (PA) of -2 degrees (errors at 68% confidence level). During the two-day long observation, we detected rotation of the PA by about 70 degrees with the corresponding changes in the PD from 2% to non-detectable and then up to 5%. These variations in polarization properties are not accompanied by visible changes in spectroscopic characteristics. The energy-resolved polarimetric analysis showed a significant change in polarization, from being strongly dependent on energy at the beginning of the observation to being almost constant with energy in the later parts of the observation. As a possible interpretation, we suggest the presence of a constant component of polarization, strong wind scattering, or different polarization of the two main spectral components with individually peculiar behavior. Abstract

Observations of linear polarization in the 2-8 keV energy range with the Imaging X-ray Polarimetry Explorer (IXPE) explore the magnetic field geometry and dynamics of the regions generating non-thermal radiation in relativistic jets of blazars. These jets, particularly in blazars whose spectral energy distribution peaks at X-ray energies, emit X-rays via synchrotron radiation from high-energy particles within the jet. IXPE observations of the X-ray selected BL Lac-type blazar 1ES 1959+650 in 2022 May 3-4 showed a significant linear polarization degree of $\Pi_\mathrm{x} = 8.0\% \pm 2.3\%$ at an electric-vector position angle $\psi_\mathrm{x} = 123^\circ \pm 8^\circ$. However, in 2022 June 9-12, only an upper limit of $\Pi_\mathrm{x} \leq 5.1\%$ could be derived (at the 99% confidence level). The degree of optical polarization at that time $\Pi_\mathrm{O} \sim 5\%$ is comparable to the X-ray measurement. We investigate possible scenarios for these findings, including temporal and geometrical depolarization effects. Unlike some other X-ray selected BL Lac objects, there is no significant chromatic dependence of the measured polarization in 1ES 1959+650, and its low X-ray polarization may be attributed to turbulence in the jet flow with dynamical timescales shorter than 1 day., Comment: 16 pages, 4 figures, accepted for publication in The Astrophysical Journal

Abstract

This study aimed to identify determinant factors of attack efficacy considering the effects of reception, attack tempo, attack performance, setting type and attack type. Firstly, 19,454 offensive actions of complex I of the Brazilian Volleyball Men’s Super League were considered and analyzed separately from the middle, wing, and opposite attacker standpoints. The middle attack was the most effective, so the 5,259 offensive actions were analyzed considering the setting type and the attack direction. The independent variables were the quality of reception, type and direction of attack, whereas attack performance was the dependent variable. We found that the middle attack efficacy was related to excellent receptions, right tempo and left tempo (near the setter), and power attack. The results can afford coaches with the key information for better training plan designs and preparation for competition.

Abstract

Background & Aims: Human genetic variation is thought to guide the outcome of HCV infection, but model systems within which to dissect these host genetic mechanisms are limited. Norway rat hepacivirus, closely related to HCV, causes chronic liver infection in rats but causes acute self-limiting hepatitis in typical strains of laboratory mice, which resolves in 2 weeks. The Collaborative Cross (CC) is a robust mouse genetics resource comprised of a panel of recombinant inbred strains, which model the complexity of the human genome and provide a system within which to understand diseases driven by complex allelic variation. Approach & Results: We infected a panel of CC strains with Norway rat hepacivirus and identified several that failed to clear the virus after 4 weeks. Strains displayed an array of virologic phenotypes ranging from delayed clearance (CC046) to chronicity (CC071, CC080) with viremia for at least 10 months. Body weight loss, hepatocyte infection frequency, viral evolution, T-cell recruitment to the liver, liver inflammation, and the capacity to develop liver fibrosis varied among infected CC strains. Conclusions: These models recapitulate many aspects of HCV infection in humans and demonstrate that host genetic variation affects a multitude of viruses and host phenotypes. These models can be used to better understand the molecular mechanisms that drive hepacivirus clearance and chronicity, the virus and host interactions that promote chronic disease manifestations like liver fibrosis, therapeutic and vaccine performance, and how these factors are affected by host genetic variation.

Abstract

Background: The EURO-NMD Registry collects data from all neuromuscular patients seen at EURO-NMD's expert centres. In-kind contributions from three patient organisations have ensured that the registry is patient-centred, meaningful, and impactful. The consenting process covers other uses, such as research, cohort finding and trial readiness. Results: The registry has three-layered datasets, with European Commission-mandated data elements (EU-CDEs), a set of cross-neuromuscular data elements (NMD-CDEs) and a dataset of disease-specific data elements that function modularly (DS-DEs). The registry captures clinical, neuromuscular imaging, neuromuscular histopathology, biological and genetic data and patient-reported outcomes in a computer-interpretable format using selected ontologies and classifications. The EURO-NMD registry is connected to the EURO-NMD Registry Hub through an interoperability layer. The Hub provides an entry point to other neuromuscular registries that follow the FAIR data stewardship principles and enable GDPR-compliant information exchange. Four national or disease-specific patient registries are interoperable with the EURO-NMD Registry, allowing for federated analysis across these different resources. Conclusions: Collectively, the Registry Hub brings together data that are currently siloed and fragmented to improve healthcare and advance research for neuromuscular diseases.

33. CombiNeRF: A Combination of Regularization Techniques for Few-Shot Neural Radiance Field View Synthesis

Abstract

Neural Radiance Fields (NeRFs) have shown impressive results for novel view synthesis when a sufficiently large amount of views are available. When dealing with few-shot settings, i.e. with a small set of input views, the training could overfit those views, leading to artifacts and geometric and chromatic inconsistencies in the resulting rendering. Regularization is a valid solution that helps NeRF generalization. On the other hand, each of the most recent NeRF regularization techniques aim to mitigate a specific rendering problem. Starting from this observation, in this paper we propose CombiNeRF, a framework that synergically combines several regularization techniques, some of them novel, in order to unify the benefits of each. In particular, we regularize single and neighboring rays distributions and we add a smoothness term to regularize near geometries. After these geometric approaches, we propose to exploit Lipschitz regularization to both NeRF density and color networks and to use encoding masks for input features regularization. We show that CombiNeRF outperforms the state-of-the-art methods with few-shot settings in several publicly available datasets. We also present an ablation study on the LLFF and NeRF-Synthetic datasets that support the choices made. 34. Nonlinear identification algorithm for online and offline study of pulmonary mechanical ventilation

Abstract

This work presents an algorithm for determining the parameters of a nonlinear dynamic model of the respiratory system in patients undergoing assisted ventilation. Using the pressure and flow signals measured at the mouth, the model's quadratic pressure-volume characteristic is fit to this data in each respiratory cycle by appropriate estimates of the model parameters. Parameter changes during ventilation can thus also be detected. The algorithm is first refined and assessed using data derived from simulated patients represented through a sigmoidal pressure-volume characteristic with hysteresis. As satisfactory results are achieved with the simulated data, the algorithm is evaluated with real data obtained from actual patients undergoing assisted ventilation. The proposed nonlinear dynamic model and associated parameter estimation algorithm yield closer fits than the static linear models computed by respiratory machines, with only a minor increase in computation. They also provide more information to the physician, such as the pressure-volume (P-V) curvature and the condition of the lung (whether normal, under-inflated, or over-inflated). This information can be used to provide safer ventilation for patients, for instance by ventilating them in the linear region of the respiratory system.

Abstract

Objetivo: Describir los factores asociados al sobrepeso/obesidad en España según datos de la última Encuesta Nacional de Salud (2011). Métodos: Estudio descriptivo transversal en población infantil de 2 a 15 años. Participaron 3752 individuos (niños n=2007 y niñas n=1745). Muestreo aleatorio polietápico estratificado. Principales variables: sobrepeso; obesidad; sexo; nivel de estudios del progenitor; ingresos mensuales; horas de sueño; ejercicio físico; horas de uso de televisión y/u ordenador; desayuno diario; percepción ponderal de los progenitores. Los criterios diagnósticos fueron los estándares de OMS, definiéndose sobrepeso con +1SD y obesidad, +2SD. Resultados: Niños más obesos que niñas. Niños/as cuyos progenitores presentaban un nivel de estudios bajo, niños/as que no hicieron ningún ejercicio y cuyos progenitores ganaron menos que 900€ mensuales presentaron un mayor sobrepeso/obesidad. Abstract

We present a lattice determination of the inclusive decay rate of the process $\tau\mapsto X_{us} \nu_\tau$ in which the $\tau$ lepton decays into a generic hadronic state $X_{us}$ with $\bar u s$ flavour quantum numbers. Our results have been obtained in $n_f=2+1+1$ iso-symmetric QCD with full non-perturbative accuracy, without any OPE approximation and, except for the presently missing long-distance isospin-breaking corrections, include a solid estimate of all sources of theoretical uncertainties. This has been possible by using the Hansen-Lupo-Tantalo method [1] that we have already successfully applied in [2] to compute the inclusive decay rate of the process $\tau\mapsto X_{ud} \nu_\tau$ in the $\bar u d$ flavour channel. By combining our first-principles theoretical results with the presently-available experimental data we extract the CKM matrix element $\vert V_{us}\vert$, the Cabibbo angle, with a $0.9$\% accuracy, dominated by the experimental error., Comment: 9 pages, 5 figures, 1 table. Abstract

This paper focuses on the development of a space-variant regularization model for solving an under-determined linear inverse problem. The case study is a medical image reconstruction from few-view tomographic noisy data. The primary objective of the proposed optimization model is to achieve a good balance between denoising and the preservation of fine details and edges, overcoming the performance of the popular and largely used Total Variation (TV) regularization through the application of appropriate pixel-dependent weights. The proposed strategy leverages the role of gradient approximations for the computation of the space-variant TV weights. For this reason, a convolutional neural network is designed, to approximate both the ground truth image and its gradient using an elastic loss function in its training. Additionally, the paper provides a theoretical analysis of the proposed model, showing the uniqueness of its solution, and illustrates a Chambolle-Pock algorithm tailored to address the specific problem at hand. This comprehensive framework integrates innovative regularization techniques with advanced neural network capabilities, demonstrating promising results in achieving high-quality reconstructions from low-sampled tomographic data.

Abstract

BACKGROUND: Birth defects are functional and structural abnormalities that impact about 1 in 33 births in the United States. They have been attributed to genetic and other factors such as drugs, cosmetics, food, and environmental pollutants during pregnancy, but for most birth defects there are no known causes. METHODS: To further characterize associations between small molecule compounds and their potential to induce specific birth abnormalities, we gathered knowledge from multiple sources to construct a reproductive toxicity Knowledge Graph (ReproTox-KG) with a focus on associations between birth defects, drugs, and genes. Specifically, we gathered data from drug/birth-defect associations from co-mentions in published abstracts, gene/birth-defect associations from genetic studies, drug- and preclinical-compound-induced gene expression changes in cell lines, known drug targets, genetic burden scores for human genes, and placental crossing scores for small molecules. RESULTS: Using ReproTox-KG and semi-supervised learning (SSL), we scored >30,000 preclinical small molecules for their potential to cross the placenta and induce birth defects, and identified >500 birth-defect/gene/drug cliques that can be used to explain molecular mechanisms for drug-induced birth defects. The ReproTox-KG can be accessed via a web-based user interface available at https://maayanlab.cloud/reprotox-kg . This site enables users to explore the associations between birth defects, approved and preclinical drugs, and all human genes. CONCLUSIONS: ReproTox-KG provides a resource for exploring knowledge about the molecular mechanisms of birth defects with the potential of predicting the likelihood of genes and preclinical small molecules to induce birth defects.

Abstract

Background: The current management of lung cancer patients has reached a high level of complexity. Indeed, besides the traditional clinical variables (e.g., age, sex, TNM stage), new omics data have recently been introduced in clinical practice, thereby making more complex the decision-making process. With the advent of Artificial intelligence (AI) techniques, various omics datasets may be used to create more accurate predictive models paving the way for a better care in lung cancer patients. Methods: The LANTERN study is a multi-center observational clinical trial involving a multidisciplinary consortium of five institutions from different European countries. The aim of this trial is to develop accurate several predictive models for lung cancer patients, through the creation of Digital Human Avatars (DHA), defined as digital representations of patients using various omics-based variables and integrating well-established clinical factors with genomic data, quantitative imaging data etc. A total of 600 lung cancer patients will be prospectively enrolled by the recruiting centers and multi-omics data will be collected. Data will then be modelled and parameterized in an experimental context of cutting-edge big data analysis. All data variables will be recorded according to a shared common ontology based on variable-specific domains in order to enhance their direct actionability. An exploratory analysis will then initiate the biomarker identification process. The second phase of the project will focus on creating multiple multivariate models trained though advanced machine learning (ML) and AI techniques for the specific areas of interest. Finally, the developed models will be validated in order to test their robustness, transferability and generalizability, leading to the development of the DHA. Abstract

Simple Summary Thymic epithelial tumours are rare and insidious malignancies. Histologically, they can be divided into different WHO subtypes and relapse risk classes. Pre-treatment biopsy is not always feasible or accurate in distinguishing WHO classes. (18)FDG PET/CT scan has been reported to play a remarkable role in the prediction of histology in these tumours (the so-called "non-invasive biopsy"). The present narrative review would like to summarise current evidence on this topic and discuss potential applications. Background: The usefulness of (18)FDG PET/CT scan in the evaluation of thymic epithelial tumours (TETs) has been reported by several authors, but data are still limited and its application in clinical practice is far from being defined. Methods: We performed a narrative review of pertinent literature in order to clarify the role of (18)FDG PET/CT in the prediction of TET histology and to discuss clinical implications and future perspectives. Results: There is only little evidence that (18)FDG PET/CT scan may distinguish thymic hyperplasia from thymic epithelial tumours. On the other hand, it seems to discriminate well thymomas from carcinomas and, even more, to predict the grade of malignancy (WHO classes). To this end, SUVmax and other PET variables (i.e., the ratio between SUVmax and tumour dimensions) have been adopted, with good results. Finally, however promising, the future of PET/CT and theranostics in TETs is far from being defined; more robust analysis of imaging texture on thymic neoplasms, as well as new exploratory studies with "stromal PET tracers," are ongoing. Conclusions: PET may play a role in predicting histology in TETs and help physicians in the management of these insidious malignancies.

Abstract

In the last decade, the emergence of effective systemic therapies (ESTs) in the form of both targeted and immuno-based therapies has revolutionized the treatment of patients with advanced stage III and stage IV melanoma. Even though lungs represent the most frequent site of melanoma metastases, only limited data are available on the role of surgery in isolated pulmonary metastases from malignant melanoma (PmMM) in the era of ESTs. The aim of this study is to describe the outcomes of patients who underwent metastasectomy of PmMM in the era of ESTs, in order to identify prognostic factors affecting survival and to provide a framework for more informed patient selection of treatmeant with lung surgery in the future. Clinical data of 183 patients who underwent metastasectomy of PmMM between June 2008 and June 2021 were collected among four Italian Thoracic Centers. The main clinical, surgical and oncological variables reviewed were: sex, comorbidities, previous oncological history, melanoma histotypes and primary site, date of primary cancer surgical treatment, melanoma growth phase, Breslow thickness, mutation pattern disease, stage at diagnosis, metastatic sites, DFI (Disease Free Interval), characteristics of lung metastases (number, side, dimension, type of resection), adjuvant therapy after lung metastasectomy, site of recurrence, disease-free survival (DFS) and cancer-specific survival (CSS; defined as the time interval between the first melanoma resection or lung metastasectomy and death from cancer). All patients underwent surgical resection of the primary melanoma before lung metastasectomy. Twenty-six (14.2%) patients already had a synchronous lung metastasis at the time of primary melanoma diagnosis. A wedge resection was performed in 95.6% of cases to radically remove the pulmonary localizations, while an anatomical resection was necessary in the remaining cases. Abstract

Alveolo-pleural fistula remains a serious post-operative complication in lung cancer patients after surgery, which is associated with prolonged hospital stay and higher healthcare costs. The aim of this study is to evaluate the efficacy of a polyglycol acid (PGA)-sheet known as Neoveil in preventing post-operative air-leak in cases of detected intra-operative air-leak after lung resection. Between 11/2021 and 7/2022, a total of 329 non-small cell lung cancer (NSCLC) patients were surgically treated in two institutions. Major lung resections were performed in 251 cases. Among them, 44 patients with significant intra-operative air-leak at surgery were treated by reinforcing staple lines with Neoveil (study group). On the other hand, a historical group (selected by propensity score matched analysis) consisting of 44 lung cancer patients with significant intra-operative air leak treated by methods other than the application of sealant patches were considered as the control group. The presence of prolonged air-leak (primary endpoint), pleural drainage duration, hospital stay, and post-operative complication rates were evaluated. The results showed that prolonged air-leak (>5 days after surgery) was not observed in study group, while this event occurred in four patients (9.1%) in the control group. Additionally, a substantial reduction (despite not statistically significant) in the chest tube removal was noted in the study group with respect to the control group (3.5 vs. 4.5, p = 0.189). In addition, a significant decrease in hospital stay (4 vs. 6 days, p = 0.045) and a reduction in post-operative complications (2 vs. 10, p = 0.015) were observed in the study group when compared with the control group. Therefore, in cases associated with intra-operative air-leak after major lung resection, Neoveil was considered a safer and more effective aerostatic tool and represents a viable option during surgical procedures.

Abstract

The cause of structural valve deterioration (SVD) is unclear. Therefore, we investigated oxidative stress markers in sera from patients with bioprosthetic heart valves (BHVs) and their association with SVD. Blood samples were taken from SVD (Phase A) and BHV patients during the first 24 (Phase B1) and >48 months (Phase B2) after BHV implantation to assess total antioxidant capacity (TAC), malondialdehyde (MDA), and nitrotyrosine (NT). The results show that MDA levels increased significantly 1 month after surgery in all groups but were higher at 6 months only in incipient SVD patients. NT levels increased gradually for the first 24 months after implantation in the BHV group. Patients with transcatheter aortic valve implantation (TAVI) showed even higher levels of stress markers. After >48 months, MDA and NT continued to increase in BHV patients with a further elevation after 60-72 months; however, these levels were significantly lower in the incipient and established SVD groups. In conclusion, oxidative stress may play a significant role in SVD, increasing early after BHV implantation, especially in TAVI cases, and also after 48 months' follow-up, but decreasing when SVD develops. Oxidative stress potentially represents a target of therapeutic intervention and a biomarker of BHV dysfunction.

Abstract

Background: The extent to which health care systems have adapted to the COVID-19 pandemic to provide necessary cardiac diagnostic services is unknown. Objectives: The aim of this study was to determine the impact of the pandemic on cardiac testing practices, volumes and types of diagnostic services, and perceived psychological stress to health care providers worldwide. Methods: The International Atomic Energy Agency conducted a worldwide survey assessing alterations from baseline in cardiovascular diagnostic care at the pandemic's onset and 1 year later. Multivariable regression was used to determine factors associated with procedure volume recovery. Results: Surveys were submitted from 669 centers in 107 countries. Worldwide reduction in cardiac procedure volumes of 64% from March 2019 to April 2020 recovered by April 2021 in high- and upper middle-income countries (recovery rates of 108% and 99%) but remained depressed in lower middle- and low-income countries (46% and 30% recovery). Although stress testing was used 12% less frequently in 2021 than in 2019, coronary computed tomographic angiography was used 14% more, a trend also seen for other advanced cardiac imaging modalities (positron emission tomography and magnetic resonance; 22%-25% increases). Pandemic-related psychological stress was estimated to have affected nearly 40% of staff, impacting patient care at 78% of sites. In multivariable regression, only lower-income status and physicians’ psychological stress were significant in predicting recovery of cardiac testing. Conclusions: Cardiac diagnostic testing has yet to recover to prepandemic levels in lower-income countries. Worldwide, the decrease in standard stress testing is offset by greater use of advanced cardiac imaging modalities. Pandemic-related psychological stress among providers is widespread and associated with poor recovery of cardiac testing.

Abstract

Chronic pain affects almost 20% of the European adult population and it significantly reduces patients’ quality of life. Chronic pain is considered a multidimensional experience determined by the interaction of several genetic and environmental factors. The effect of specific genetic contributions is often unclear, and the interpretation of the results from studies focused on genetic influences on pain has been complicated by the existence of multiple pain phenotypes. A step forward from genetics could be given by the application of metabolomics and microbiomics tools. Metabolomics is a powerful approach for hypothesis generation in biology, and it aims to analyze low molecular weight compounds, either metabolic intermediates or metabolic end-products, resulting from human or microbial metabolism. Microbiomics is a fast-growing field in which all the microbes are examined together, and as a result, its perturbation may indicate the development of chronic diseases. By applying these methodologies for the study of chronic pain, several differences have been identified. The alteration of the choline-PAF pathway is an intriguing finding recognized by several groups. In our opinion, metabolomics and microbiomics techniques will allow significant progress into the medical field. Patients may benefit from the possibility of being stratified and classified based on their metabolic and microbial profile, which, in the next future, may lead to personalized therapy.

Abstract

Colorectal cancer (CRC) is one of the most common causes of death from cancer. Lung seeding occurs in approximately 10% of patients surgically treated for primary CRC with radical intent: the lung is the most common site of metastases after the liver. While surgical treatment of liver metastases is widely accepted to affect long-term outcomes, more controversial and not standardized is the therapy for CRC patients developing lung metastases. Experience suggests the potential curative role of pulmonary metastasectomy, especially in oligometastatic disease. However, the optimal strategy of care and the definition of prognostic factors after treatment still need to be defined. This review focused on the uncommon scenario of single pulmonary metastases from CRC. We explored pertinent literature and provide an overview of the epidemiology, clinical characteristics and imaging of single pulmonary metastases from CRC. Additionally, we identified the best available evidence for overall management. In particular, we analyzed the role and results of locoregional approaches (surgery, radiotherapy or ablative procedures) and their integration with systemic therapy.

Abstract

Antequera in southern Spain is widely recognised as an outstanding example of the European megalithic phenomenon. One of its most remarkable features is the evident relationship between conspicuous natural formations and human-built monuments. Here, the authors report the results of their investigation of a tomb newly discovered at the site of Piedras Blancas at the foot of La Peña de los Enamorados, a limestone massif that dominates the Antequera plain. Excavation and multidisciplinary study, including geological, architectural and archaeoastronomical investigations, have revealed a complex funerary monument that is part natural, part built, part hypogeum, part megalith. The results emphasise the centrality of La Peña in the Neolithicworldviewand encourage wider investigation of prehistoric place-making.

My question is: At what point does Salette Tavares’ poetics betray this tradition and subvert this identity?

The study aimed to determine which objective dynamic swallow study (DSS) parameters predict improved voice outcomes in patients who have TEP placement.

In order to determine this, four samples were extracted from the leachate pond of the Santa Rosa de Shapajilla transitory cell and taken to the water quality laboratory of the Universidad Nacional Agria de la Selva (UNAS), where the physical parameters such as odor, color, chemical parameters such as pH, electrical conductivity, total dissolved solids, dissolved oxygen (OD1, OD5, BOD5, Copper, Zinc, Lead, Cadmium) were measured; Chemical parameters such as pH, electrical conductivity, total dissolved solids, dissolved oxygen (OD1, OD5), BOD5, Copper, Zinc, Lead, Cadmium; Microbiological parameters such as number of viable aerobic microorganisms, Salmonella, total coliforms, thermotolerant coliforms, Staphylococcus aereus, Vibrio cholerae, and international and national standards were used for comparison, As a result, Lead was the only element that exceeded the limits established according to the Mexican standard NOM-052-SEMARNAT-2005 and within the microbiological parameters, the presence of Salmonella, Staphylococcus Aereus, Vibrio Cholerae was determined., La presente investigación tuvo como finalidad determinar y evaluar los parámetros físicos, químicos y biológicos de los lixiviados generados en las Celdas Transitorias de Santa Rosa de Shapajilla de igual manera identificar el problema ambiental generado por estos lixiviados, y para poder determinarlo se extrajeron cuatro muestras de la poza de lixiviados de la celda transitoria de Santa Rosa de Shapajilla, se trasladó al laboratorio de calidad de agua de la Universidad Nacional Agria de la Selva (UNAS), se midieron los parámetros de físicos como olor, color; parámetros químicos como pH, conductividad eléctrica, sólidos disueltos totales, Oxígeno disuelto (OD1, OD5), DBO5, Cobre, Zinc, Plomo, Cadmio; parámetros microbiológicos como Numeración de Microorganismos Aerobios Viables, Salmonella, Coliformes Totales, Coliformes Termo tolerantes, Staphylococcus Aereus, Vibrio Cholerae, se utilizaron normas internacionales y nacionales para su comparación, se obtuvo como resultado que el Plomo fue el único elemento que sobrepasó los limites establecido según la norma mexicana NOM-052-SEMARNAT-2005 y dentro de los parámetros microbiológico se determinó la presencia de Salmonella, Staphylococcus Aereus, Vibrio Cholerae.
