We investigate the role of physician agency in determining health care sup- ply and patient outcomes. We show that an increase in health care supply due to a change in private physician incentives has a theoretically ambiguous impact on patient welfare. The increase can reflect either induced demand for ineffective care or a reduction in prior rationing of effective care. Furthermore, physician market structure matters in determining the welfare effects of changes in private physician incentives. We then analyze a change to Medicare fees that caused physicians to increase their provision of chemotherapy. We find that this increase in treatment improved patient survival, extending median life expectancy for lung cancer patients by about 18%. Consistent with the model, we find that while the treatment response was larger in less concentrated markets, survival improvements were larger in more concentrated markets.

Neighborhood-level socioeconomic deprivation can increase risk for higher blood pressure or hypertension, while greater neighborhood safety and walkability may protect against hypertension. Large-scale prospective research, particularly among Hispanics/Latinos, is lacking. We examined cross-sectional and prospective associations between neighborhood environments and blood pressure and hypertension among 3851 Hispanic/Latinos enrolled in the Hispanic Community Health Study/Study of Latinos San Diego, CA cohort. Addresses from Visit 1 (2008-2011) were geocoded and neighborhood characteristics were determined as part of the SOL CASAS ancillary study. Home addresses were geocoded and home areas created using 800 m circular radial buffers. Neighborhood indices socioeconomic deprivation, residential stability, and social disorder were created using Census and other publicly available data. Walkability was computed as density of intersections, retail spaces, and residences. Greenness was measured via satellite imagery using the Normalized Difference Vegetation Index. Visit 1 and Visit 2 (2014-2017) clinical outcomes included systolic (SBP) and diastolic (DBP) blood pressure, as well as prevalent and 6-year incident hypertension, defined as SBP/DBP ≥140/90 mmHg or antihypertensive medication use. Complex survey regression models adjusted for covariates revealed cross-sectional associations between greater walkability and lower SBP (B = -0.05; 95% CI: -0.09, -0.003). In prospective analyses, greater neighborhood social disorder was related to increasing SBP (B = 0.05; 95% CI: 0.01, 0.09) and DBP (B = 0.07; 95% CI: 0.02, 0.12) over time. Greater socioeconomic deprivation (OR = 1.47; 95% CI: 1.06, 2.04) and greater social disorder (OR = 1.25; 95% CI: 1.02, 1.54) were associated with higher odds of incident hypertension. BACKGROUND: Across Ontario, since the year 2006 various knowledge translation (KT) interventions designed to improve the quality of rectal cancer surgery have been implemented by the provincial cancer agency or by individual researchers. Ontario is divided administratively into 14 health regions. We piloted a method to audit and score for each region of the province the KT interventions implemented to improve the quality of rectal cancer surgery. METHODS: We interviewed stakeholders to audit KT interventions used in respective regions over years 2006 to 2014. Results were summarized into narrative and visual forms. Using a modified Delphi approach, KT experts reviewed these data and then, for each region, scored implementation of KT interventions using a 20-item KT Signature Assessment Tool. Scores could range from 20 to 100 with higher scores commensurate with greater KT intervention implementation. RESULTS: There were thirty interviews. KT experts produced scores for each region that were bimodally distributed, with an average score for 2 regions of 78 (range 73-83) and for 12 regions of 30.5 (range 22-38). CONCLUSION: Our methods efficiently identified two groups with similar KT Signature scores. Two regions had relatively high scores reflecting numerous KT interventions and the use of sustained iterative approaches in addition to those encouraged by the provincial cancer agency, while 12 regions had relatively low scores reflecting minimal activities outside of those encouraged by the provincial cancer agency. These groupings will be used for future comparative quantitative analyses to help determine if higher KT signature scores correlate with improved measures for quality of rectal cancer surgery.

INTRODUCTION: The International Cancer Benchmarking Partnership (ICBP) identified significant international differences in lung cancer survival. Differing levels of comorbid disease across ICBP countries has been suggested as a potential explanation of this variation but, to date, no studies have quantified its impact. This study investigated whether comparable, robust comorbidity scores can be derived from the different routine population-based cancer data sets available in the ICBP jurisdictions and, if so, use them to quantify international variation in comorbidity and determine its influence on outcome. METHODS: Linked population-based lung cancer registry and hospital discharge data sets were acquired from nine ICBP jurisdictions in Australia, Canada, Norway and the UK providing a study population of 233 981 individuals. For each person in this cohort Charlson, Elixhauser and inpatient bed day Comorbidity Scores were derived relating to the 4-36 months prior to their lung cancer diagnosis. The scores were then compared to assess their validity and feasibility of use in international survival comparisons. RESULTS: It was feasible to generate the three comorbidity scores for each jurisdiction, which were found to have good content, face and concurrent validity. Predictive validity was limited and there was evidence that the reliability was questionable. CONCLUSION: The results presented here indicate that interjurisdictional comparability of recorded comorbidity was limited due to probable differences in coding and hospital admission practices in each area. Before the contribution of comorbidity on international differences in cancer survival can be investigated an internationally harmonised comorbidity index is required.

Background: Sidedness of primary is an established prognostic factor in mCRC and recently has been shown to be an important predictive factor for patients (pts) treated with EGFR-Ab (always in combination with chemotherapy) in 1st and 2nd line settings. Limited data is available in 3rd line and beyond, where EGFR-Ab are used either as monotherapy or with chemotherapy. In Ontario, Canada, public funding of EGFR-Ab is restricted to chemorefractory disease. This study examines the impact of sidedness on overall survival (OS) in chemorefractory mCRC treated with EGFR-Ab monotherapy and combination. Method(s): This population-based retrospective cohort study used linked data from the Institute for Clinical Evaluative Sciences to evaluate mCRC pts treated in Ontario with EGFR-Ab from Jan 2006-Dec 2014. Over 99% of cases are captured via the Ontario Cancer Registry. The primary outcome was OS. Monotherapy v combination was compared by panitumumab (pani; funded only as monotherapy) v cetuximab (cet; funded only with chemotherapy) outcomes. Sidedness was determined by ICD-10 code as right (R; including transverse colon) or left (L). Result(s): Of 67117 CRC pts,1553 received EGFR-Ab for refractory mCRC (429 R, 1080 L, 44 unknown). 71% received pani. R were more commonly female, with significantly shorter time (months, m) from diagnosis to EGFR-Ab therapy (mean+/-SD: 28.7+/-18.3 v 32.8+/-19.2; p<0.001). Median OS for R with any EGFR-Ab therapy was significantly worse than L: 30.5 v 39.3m(HR 1.22; 95% CI 1.07-1.38; p=0.0026); this was true for monotherapy (HR 1.22; 95% CI 1.05-1.42, p=0.0089) with a nearsignificant trend for combination (HR 1.30; 95% CI 0.99-1.7, p=0.055). For L, OS was identical between monotherapy and combination; for R there was a near-significant trend to longer survival with combination (HR 0.76; 95% CI 0.58-1.01, p=0.058). In particular, microRNA-21 (miR-21) appears to have a critical role in tumor cell survival, chemoresistance and HNSCC progression. In this study, we investigated matrix hyaluronan (HA)-induced CD44 (a primary HA receptor) interaction with the stem cell markers, Nanog and Stat-3, in HNSCC cells (HSC-3 cells). Our results indicate that HA binding to CD44 promotes Nanog-Stat-3 (also tyrosine phosphorylated Stat-3) complex formation, nuclear translocation and transcriptional activation. Further analyses reveal that miR-21 is controlled by an upstream promoter containing Stat-3 binding site(s), while chromatin immunoprecipitation assays demonstrate that stimulation of miR-21 expression by HA/CD44 signaling is Nanog/Stat-3-dependent in HNSCC cells. This process results in a decrease of a tumor suppressor protein (PDCD4), and an upregulation of i nhibitors of the apoptosis family of proteins (IAPs) as well as chemoresistance in HSC-3 cells. Treatment of HSC-3 cells with Nanog- and/or Stat-3-specific small interfering RNAs effectively blocks HA-mediated Nanog-Stat-3 signaling events, abrogates miR-21 production and increases PDCD4 expression. Subsequently, this Nanog-Stat-3 signaling inhibition causes downregulation of survival protein (IAP) expression and enhancement of chemosensitivity. To further evaluate the role of miR-21 in tumor cell-specific functions, HSC-3 cells were also transfected with a specific anti-miR-21 inhibitor in order to silence miR-21 expression and block its target functions. Our results demonstrate that anti-miR-21 inhibitor not only upregulates PDCD4 expression but also decreases IAP expression and enhances chemosensitivity in HA-treated HNSCC cells. Recent legislative changes have altered the structure of the industry by allowing banks, insurance companies, and securities dealers to consolidate into large financial holding companies that cut across industry sectors. US financial services firms are also positioning to compete here and abroad with international firms. Advances in information technology continue to provide opportunities for firms to increase business and customer service, but also create an industry risk in the form of cyber crime. The industry will continue to be competitive, but profitable, and will remain well positioned to support continued US leadership in the global economy.

Insurgent warfare occurs on the lower end of the spectrum of conflict. The Soviets continue to use this type of conflict as a very feasible weapon to advance their strategy of denial. Denial strategy success represents a serious threat to the national security of the United States. The United States must pursue, therefore, a successful counterinsurgency warfighting strategy. This study examines types of insurgent warfare, Foreign Internal Defense strategy, foreign assistance programs and nation building. The examination leads to consideration of how the United States Army Corps of Engineers can best contribute in this particular conflict environment.

The Bohemian Grove is owned by The Bohemian Club in San Francisco, a private men's club founded in 1872, by a group of newspapermen and artists.
