Aim: Slc26a9 is a member of the Slc26 multifunctional anion transporter family. Polymorphisms in Slc26a9 are associated with an increased incidence of meconium ileus and diabetes in cystic fibrosis patients. We investigated the expression of Slc26a9 in the murine pancreatic ducts, islets and parenchyma, and elucidated its role in pancreatic ductal electrolyte and fluid secretion and endocrine function. Methods: Pancreatic Slc26a9 and CFTR mRNA expression, fluid and bicarbonate secretion were assessed in slc26a9−/− mice and their age- and sex-matched wild-type (wt) littermates. Glucose and insulin tolerance tests were performed. Results: Compared with stomach, the mRNA expression of Slc26a9 was low in pancreatic parenchyma, 20-fold higher in microdissected pancreatic ducts than parenchyma, and very low in islets. CFTR mRNA was ~10 fold higher than Slc26a9 mRNA expression in each pancreatic cell type. Significantly reduced pancreatic fluid secretory rates and impaired glucose tolerance were observed in female slc26a9−/− mice, whereas alterations in male mice did not reach statistical significance. No significant difference was observed in peripheral insulin resistance in slc26a9−/− compared to sex- and aged-matched wt controls. In contrast, isolated slc26a9−/− islets in short term culture displayed no difference in insulin content, but a significantly reduced glucose-stimulated insulin secretion compared to age- and sex-matched wt islets, suggesting that the impaired glucose tolerance in the absence of Slc26a9 expression these is a pancreatic defect. Conclusions: Deletion of Slc26a9 is associated with a reduction in pancreatic fluid secretion and impaired glucose tolerance in female mice. The results underline the importance of Slc26a9 in pancreatic physiology.

In this neurophysiological study in healthy humans, we assessed how central sensitization induced by either high-frequency stimulation (HFS) or topical capsaicin application modulates features of the RIII reflex response. The ability of these stimuli to engage the endogenous pain modulatory system was also tested. In 26 healthy participants we elicited an RIII reflex using suprathreshold stimulation of the sural nerve. Subsequently HFS or capsaicin were applied to the foot and the RIII reflex repeated after 15 minutes. Contact heating of the volar forearm served as the heterotopic test stimulus to probe activation of the endogenous pain modulatory system. HFS significantly reduced the pain threshold by 29% and the RIII reflex threshold by 20%. Capsaicin significantly reduced the pain threshold by 17% and the RIII reflex threshold by 18%. Both HFS and capsaicin left RIII reflex size unaffected. Numerical Rating Scale (NRS) pain scores elicited by the heterotopic noxious heat stimulus were unaffected by capsaicin and slightly increased by HFS. HFS and capsaicin similarly modulated the pain threshold and RIII reflex threshold, without a concomitant inhibitory effect of the endogenous pain modulatory system. Our neurophysiological study supports the use of the RIII reflex in investigating central sensitization in humans.

The N13 component of somatosensory evoked potential (N13 SEP) represents the segmental response of dorsal horn neurons. In this neurophysiological study, we aimed to verify whether N13 SEP might reflect excitability changes of dorsal horn neurons during central sensitization. In 22 healthy participants, we investigated how central sensitization induced by application of topical capsaicin to the ulnar nerve territory of the hand dorsum modulated N13 SEP elicited by ulnar nerve stimulation. Using a double-blind placebo-controlled crossover design, we also tested whether pregabalin, an analgesic drug with proven efficacy on the dorsal horn, influenced capsaicin-induced N13 SEP modulation. Topical application of capsaicin produced an area of secondary mechanical hyperalgesia, a sign of central sensitization, and increased the N13 SEP amplitude but not the peripheral N9 nor the cortical N20-P25 amplitude. This increase in N13 SEP amplitude paralleled the mechanical hyperalgesia and persisted for 120 min. Pregabalin prevented the N13 SEP modulation associated with capsaicin-induced central sensitization, whereas capsaicin application still increased N13 SEP amplitude in the placebo treatment session. Our neurophysiological study showed that capsaicin application specifically modulates N13 SEP and that this modulation is prevented by pregabalin, thus suggesting that N13 SEP may reflect changes in dorsal horn excitability and represent a useful biomarker of central sensitization in human studies.

AIMS: The ORal anticoagulants In fraGile patients with percutAneous endoscopic gastrostoMy and atrIal fibrillation (ORIGAMI) study investigates the safety and efficacy of Edoxaban administered via PEG in patients with atrial fibrillation and a clinical indication for a long-term anticoagulation. DESIGN: In this prospective, single-centre observational study, 12 PEG-treated patients with indication to anticoagulation will receive edoxaban via PEG and will be followed up to 6 months. Plasma antifactor Xa activity and edoxaban concentrations will be assessed. Thromboembolic (ischaemic stroke, systemic embolism, venous thromboembolism) and bleeding events (Bleeding Academic Research Consortium and Thrombolysis in Myocardial Infarction) will be recorded at 1 and 6 months. PRELIMINARY RESULTS: A retrospective analysis of five atrial fibrillation cases undergoing PEG implantation at our Institution who received edoxaban via PEG showed plasma anti-FXa levels at a steady state of 146 ± 15 ng/ml, without major adverse event at a mean follow-up of 6 months. CONCLUSION: ORIGAMI prospectively investigates PEG-administration of edoxaban in PEG-treated patients requiring long-term anticoagulation. Background and purpose: Trigeminal neuralgia (TN) is an extremely painful condition which can be difficult to diagnose and treat. In Europe, TN patients are managed by many different specialities. Therefore, there is a great need for comprehensive European guidelines for the management of TN. The European Academy of Neurology asked an expert panel to develop recommendations for a series of questions that are essential for daily clinical management of patients with TN. Methods: A systematic review of the literature was performed and recommendations was developed based on GRADE, where feasible; if not, a good practice statement was given. Results: The use of the most recent classification system is recommended, which diagnoses TN as primary TN, either classical or idiopathic depending on the degree of neurovascular contact, or as secondary TN caused by pathology other than neurovascular contact. Magnetic resonance imaging (MRI), using a combination of three high-resolution sequences, should be performed as part of the work-up in TN patients, because no clinical characteristics can exclude secondary TN. If MRI is not possible, trigeminal reflexes can be used. Neurovascular contact plays an important role in primary TN, but demonstration of a neurovascular contact should not be used to confirm the diagnosis of TN. Rather, it may help to decide if and when a patient should be referred for microvascular decompression. In acute exacerbations of pain, intravenous infusion of fosphenytoin or lidocaine can be used. For long-term treatment, carbamazepine or oxcarbazepine are recommended as drugs of first choice. Lamotrigine, gabapentin, botulinum toxin type A, pregabalin, baclofen and phenytoin may be used either alone or as add-on therapy. Abstract Extreme and inaccessible environments are a new frontier that unmanned and remotely operated vehicles can today safely access and monitor. The Lusi mud eruption (NE Java Island, Indonesia) represents one of these harsh environments that are totally unreachable with traditional techniques. Here boiling mud is constantly spewed tens of meters in height and tall gas clouds surround the 100 m wide active crater. The crater is surrounded by a ∼600 m diameter circular zone of hot mud that prevents any approach to investigate and sample the eruption site. In order to access this active crater we designed and assembled a multipurpose drone. The Lusi drone is equipped with numerous airborne devices suitable for use on board of other multicopters. During the missions, three cameras can complete 1) video survey, 2) high resolution photogrammetry of desired and preselected polygons, and 3) thermal photogrammetry surveys with infra-red camera to locate hot fluids seepage areas or faulted zones. Crater sampling and monitoring operations can be pre-planned with a flight software, and the pilot is required only for take-off and landing. A winch allows the deployment of gas, mud and water samplers and contact thermometers to be operated with no risk for the aircraft. During the winch operations (that can be performed automatically), the aircraft hovers at a safety height until the tasks controlled by the winch-embedded processor are completed. Abstract The use of low-cost hand-held infrared (IR) thermal cameras based on uncooled micro-bolometer detector arrays became more widespread during the recent years. Thermal cameras have the ability to estimate temperature values without contact and therefore can be used in conditions where targets are difficult or dangerous to reach such as volcanic eruptions. Since May 2006 the Indonesian Lusi mud eruption continues to spew boiling mud, water, aqueous vapour, CO2, CH4 and covers a surface of nearly 7 km2. Here we performed surveys above and around the erupting crater using a specifically equipped remote-controlled aerial vehicle (drone). Despite the harsh logistics and the continuously varying gas concentrations, we managed to collect IR images composing mosaics to estimate the crater zone spatial and temporal thermal variations as well as that in the surrounding regions. In this manuscript we provide a) a description of the main processes that affect and control the acquisition of IR images; b) an overview of still non disclosed physical model used by the thermal camera employed during our survey (i.e. Flir I7 IR); c) a method for capturing high resolution infrared images over an erupting clastic system; d) analysis and interpretation of the acquired data and scientific results. The results show that it is possible to obtain good quality mosaics also in inaccessible areas such as erupting craters where fixed reference points are not constant, and where the presence of IR attenuation factors introduce errors in terms of temperature estimates. BACKGROUND AND AIM: L-Acetyl-carnitine (LAC) exerts an energetic effect on nerves and muscles. Recently, preclinical experiments have demonstrated a central anti-nociceptive action. OBJECTIVE: Our objective was to assess the effects of LAC on neuroprotection, pain, and function in carpal tunnel syndrome (CTS), a very frequent chronic compressive neuropathy. METHODS: In a multicentre, examiner-blinded, clinical and neurophysiological 4-month study, we enrolled 82 patients and examined 120 hands with CTS of mild to moderate severity. Patients were assessed at baseline and 10, 60 and 120 days after treatment with LAC 500 mg twice daily (BID). All patients underwent a conduction study of the median nerve, the Boston Carpal Tunnel Questionnaire (BCTQ) and the Neuropathic Pain Symptom Inventory (NPSI). The primary endpoint was the sensory conduction velocity (SCV) of the median nerve. RESULTS: The primary endpoint was met, with significant improvement of the SCV (P < 0.0001). All sensory neurophysiological measures also significantly improved. BCTQ score changed significantly (P < 0.0001), with a greater improvement in the symptom component. Nine of the NPSI types of pain, particularly squeezing and pressure pain and pain evoked by pressure, showed a significant reduction (P < 0.0001). CONCLUSIONS: Our clinical and neurophysiological study indicated that 4 months of treatment with LAC exerted a neuroprotective effect. LAC reduced pain in patients with mild and moderate CTS, a result that is possibly due to both its neuroprotective action and its central anti-nociceptive properties. Boomerang is a balloon-borne telescope designed for long duration (LDB) flights around Antarctica. The second LDB flight of Boomerang took place in January 2003. The primary goal of this flight was to measure the polarization of the CMB. The receiver uses polarization sensitive bolometers at 145 GHz. Polarizing grids provide polarization sensitivity at 245 and 345 GHz. We describe the Boomerang telescope noting changes made for 2003 LDB flight, and discuss some of the issues involved in the measurement of polarization with bolometers. Lastly, we report on the 2003 flight and provide an estimate of the expected results.

OLIMPO is a balloon-borne telescope designed to observe the sky in the mm and sub-mm bands, with an unprecedented combination of angular resolution, frequency coverage, and sensitivity. The experiment uses a 2.6 m reflective telescope with four diffraction-limited bolometric detector arrays; it is equipped with a custom attitude control system, and is designed for long-duration polar flights. OLIMPO features original spectroscopic capabilities, including a plug-in differential Fourier transform spectrometer (DFTS). This is an imaging spectrometer with very high throughput, wide spectral coverage, medium to high spectral resolution and excellent rejection of common-mode signals. The main objective of the experiment is to measure the Sunyaev-Zel'dovich (SZ) effect on selected clusters of galaxies. For this reason we explore four frequency bands centered at 145, 210, 345 and 480 GHz, matching the negative, zero and positive regions of the SZ spectrum. Simulations show that measuring the spectrum of the SZ within the four bands, in addition to making wide-band photometry in the same bands degeneracy in cluster parameters, can be avoided. The experiment is being prepared for a launch in 2014, devoted to the observation of ∼50 clusters per flight, and the measurement of spatial-spectral Cosmic Microwave Background (CMB) anisotropy in a deep blind survey area of ∼100 square degrees.

To gain information on the topographical distribution of warmth, burning and itch sensations in healthy humans, we delivered laser stimuli to elicit sensations of warmth, applied capsaicin cream for burning, and pricked histamine for itch on the skin of the face, shoulder, hand, thigh and foot in 12 healthy subjects. We found that whereas warm and burning sensations progressively increased from foot to face, itch sensation increased from face to foot (P<0.0001). Hence our findings indicate that unlike thermal and pain receptors, itch receptors are denser at distal than at proximal body sites. Our psychophysical study provides new information supporting the idea that specific unmyelinated neuronal pathways mediate sensations of warmth, burning and itch.

The physical properties of galactic cirrus emission are not well characterized. BOOMERanG is a balloon-borne experiment designed to study the cosmic microwave background at high angular resolution in the millimeter range. The BOOMERanG 245 and 345GHz channels are sensitive to interstellar signals, in a spectral range intermediate between FIR and microwave frequencies. We look for physical characteristics of cirrus structures in a region at high galactic latitudes (b~-40{\deg}) where BOOMERanG performed its deepest integration, combining the BOOMERanG data with other available datasets at different wavelengths. We have detected eight emission patches in the 345 GHz map, consistent with cirrus dust in the Infrared Astronomical Satellite maps. The analysis technique we have developed allows to identify the location and the shape of cirrus clouds, and to extract the flux from observations with different instruments at different wavelengths and angular resolutions. We study the integrated flux emitted from these cirrus clouds using data from Infrared Astronomical Satellite (IRAS), DIRBE, BOOMERanG and Wilkinson Microwave Anisotropy Probe (WMAP) in the frequency range 23-3000 GHz (13 mm 100 micron wavelength). We fit the measured spectral energy distributions with a combination of a grey body and a power-law spectra considering two models for the thermal emission. The temperature of the thermal dust component varies in the 7 - 20 K range and its emissivity spectral index is in the 1 - 5 range. We identified a physical relation between temperature and spectral index as had been proposed in previous works. We use Minkowski Functionals (MF) to constrain a primordial non-Gaussian contribution to the CMB intensity field as observed in the 150 GHz and 145 GHz BOOMERanG maps from the 1998 and 2003 flights, respectively, performing for the first time a joint analysis of the two datasets. A perturbative expansion of the MF formulae in the limit of a weakly non-Gaussian field yields analytical formulae, derived by Hikage et al. (2006), which can be used to constrain the coupling parameter f_NL without the need for non-Gaussian simulations. We find -1020

The Sunyaev-Zel'dovich (SZ) effect is the inverse Compton-scattering of cosmic microwave background (CMB) photons by hot electrons in the intervening gas throughout the universe. The effect has a distinct spectral signature that allows its separation from other signals in multifrequency CMB datasets. Using CMB anisotropies measured at three frequencies by the BOOMERanG 2003 flight we constrain SZ fluctuations in the 10 arcmin to 1 deg angular range. Propagating errors and potential systematic effects through simulations, we obtain an overall upper limit of 15.3 uK (2 sigma) for rms SZ fluctuations in a broad bin between multipoles of of 250 and 1200 at the Rayleigh-Jeans (RJ) end of the spectrum. When combined with other CMB anisotropy and SZ measurements, we find that the local universe normalization of the density perturbations is sigma-8(SZ) < 0.96 at the 95% confidence level, consistent with sigma-8 determined from primordial perturbations., Comment: accepted for publication in ApJ. We have detected eight emission patches in the 345 GHz map, consistent with cirrus dust in the Infrared Astronomical Satellite maps. The analysis technique we have developed allows to identify the location and the shape of cirrus clouds, and to extract the flux from observations with different instruments at different wavelengths and angular resolutions. We study the integrated flux emitted from these cirrus clouds using data from Infrared Astronomical Satellite (IRAS), DIRBE, BOOMERanG and Wilkinson Microwave Anisotropy Probe (WMAP) in the frequency range 23-3000 GHz (13 mm 100 micron wavelength). We fit the measured spectral energy distributions with a combination of a grey body and a power-law spectra considering two models for the thermal emission. The temperature of the thermal dust component varies in the 7 - 20 K range and its emissivity spectral index is in the 1 - 5 range. We identified a physical relation between temperature and spectral index as had been proposed in previous works. We use Minkowski Functionals (MF) to constrain a primordial non-Gaussian contribution to the CMB intensity field as observed in the 150 GHz and 145 GHz BOOMERanG maps from the 1998 and 2003 flights, respectively, performing for the first time a joint analysis of the two datasets. A perturbative expansion of the MF formulae in the limit of a weakly non-Gaussian field yields analytical formulae, derived by Hikage et al. (2006), which can be used to constrain the coupling parameter f_NL without the need for non-Gaussian simulations. We find -1020

The Sunyaev-Zel'dovich (SZ) effect is the inverse Compton-scattering of cosmic microwave background (CMB) photons by hot electrons in the intervening gas throughout the universe. The effect has a distinct spectral signature that allows its separation from other signals in multifrequency CMB datasets. Using CMB anisotropies measured at three frequencies by the BOOMERanG 2003 flight we constrain SZ fluctuations in the 10 arcmin to 1 deg angular range. Propagating errors and potential systematic effects through simulations, we obtain an overall upper limit of 15.3 uK (2 sigma) for rms SZ fluctuations in a broad bin between multipoles of of 250 and 1200 at the Rayleigh-Jeans (RJ) end of the spectrum. When combined with other CMB anisotropy and SZ measurements, we find that the local universe normalization of the density perturbations is sigma-8(SZ) < 0.96 at the 95% confidence level, consistent with sigma-8 determined from primordial perturbations., Comment: accepted for publication in ApJ. We present a measurement of the temperature-polarization angular cross power spectrum, , of the Cosmic Microwave Background. The result is based on $\sim 200$ hours of data from 8 polarization sensitive bolometers operating at 145 GHz during the 2003 flight of BOOMERANG. We detect a significant correlation in the $\ell$-range between 50 and 950 with a statistical significance > 3.5 sigma. Contamination by polarized foreground emission and systematic effects are negligible in comparison with statistical uncertainty. The spectrum is consistent with previous detections and with the "concordance model" that assumes adiabatic initial conditions. We report measurements of the CMB polarization power spectra from the January 2003 Antarctic flight of BOOMERANG. The primary results come from six days of observation of a patch covering 0.22% of the sky centered near R.A. = 82.5 deg., Dec= -45 deg. The observations were made using four pairs of polarization sensitive bolometers operating in bands centered at 145 GHz. Using two independent analysis pipelines, we measure a non-zero signal in the range 100< l <1000 with a significance 4.8-sigma, a 2-sigma upper limit of 8.6 uK^2 for any contribution, and a 2-sigma upper limit of 7.0 uK^2 for the spectrum. Estimates of foreground intensity fluctuations and the non-detection of and signals rule out any significant contribution from galactic foregrounds. The results are consistent with a Lambda-CDM cosmology seeded by adiabatic perturbations. We present the BOOMERanG-03 experiment and maps of the Stokes parameters I, Q, U of the microwave sky obtained during a 14 day balloon flight in 2003. Three regions of the southern sky were surveyed: a deep survey (~ 90 square degrees) and a shallow survey (~ 750 square degrees) at high Galactic latitudes (both centered at RA ~ 5.5 h, dec ~ -45 deg) and a survey of ~ 300 square degrees across the Galactic plane at RA ~ 9.1 h, dec ~ -47 deg. All three surveys were carried out in three wide frequency bands centered at 145, 245 and 345 GHz, with an angular resolution of ~ 10'. The 145 GHz maps of Stokes I are dominated by Cosmic Microwave Background (CMB) temperature anisotropy, which is mapped with high signal to noise ratio. The measured anisotropy pattern is consistent with the pattern measured in the same region by BOOMERanG-98 and by WMAP. The 145 GHz maps of Stokes Q and U provide a robust statistical detection of polarization of the CMB when subjected to a power spectrum analysis. This amplitude of the polarization is consistent with that of the CMB in the $\Lambda$CDM cosmological scenario. At 145 GHz, in the CMB surveys, the intensity and polarization of the astrophysical foregrounds are found to be negligible with respect to the cosmological signal. At 245 and 345 GHz we detect ISD emission correlated to the 3000 GHz IRAS/DIRBE maps, and give upper limits for any other non-CMB component. We also present intensity maps of the surveyed section of the Galactic plane. These results are derived from 195 hours of observation with four 145 GHz Polarization Sensitive Bolometer (PSB) pairs, identical in design to the four 143 GHz Planck HFI polarized pixels. The data include 75 hours of observations distributed over 1.84% of the sky with an additional 120 hours concentrated on the central portion of the field, itself representing 0.22% of the full sky. From these data we derive an estimate of the angular power spectrum of temperature fluctuations of the CMB in 24 bands over the multipole range (50 < l < 1500). A series of features, consistent with those expected from acoustic oscillations in the primordial photon-baryon fluid, are clearly evident in the power spectrum, as is the exponential damping of power on scales smaller than the photon mean free path at the epoch of last scattering (l > 900). Cell-mediated immunodeficiency, with Total and T lymphocytes count decrease, is well established in cancer patients and it predicts a poor prognosis and poor survival rates. Furthermore, major surgery induces a transient immunodeficiency, too. Nevertheless, cell-mediated immunity in pancreatic cancer, which has a very poor prognosis, has not been completely outlined. Aim of this study is to evaluate the cell-mediated IL-2 dependent immune status in operable pancreatic cancer patients and to compare it with other gastrointestinal tumors. One hundred and twenty-one cancer patients (22 pancreatic, 48 gastric and 51 colorectal), with a median age of 66 years (range 42-83), 55 males and 66 females, were enrolled. Total lymphocyte count and lymphocytes subset (T helper count - CD4+) were assessed preoperatively and on the 14th and 50th postoperative day. Results obtained were compared between the groups and related to nodal involvement (NO versus N+). Colorectal and gastric cancer patients showed quantitative lymphocyte deficiency at baseline in 29% and 41% of cases, respectively. Fourteen days after surgery values below normal range were found in 44% and 54% (Total) and 53% and 67% (T helper), respectively. Recovery of postoperative surgery-related lymphocytopenia occurred late only in patients with normal count at baseline. According to regional nodal involvement (pN0/N+) T helper deficiency was significantly more frequent in patients with nodal involvement than in patients without. In pancreatic cancer, percentage of immunodepressed patients at baseline was higher compared to the other two groups (71%). Lymphocyte count was significantly different between pancreatic and gastric/colorectal cancer, reaching a statistical significance at baseline and on the 14th and 50th postoperative day.
