1. Agency, participation in decision making and wellbeing among care leavers in care system: A quantitative mediation study

Abstract

Care leavers are young people who have grown up in the care system, either in foster care or in residential care homes in transition from care to adulthood. They are a vulnerable group, with a high risk of negative outcomes such as poor mental health. The development of agency and the promotion of well-being are two of the most important aspects of the psychological and educational intervention with care leavers. The purpose of this quantitative study is to investigate the relationship between agency and well-being in a group of care leavers (N = 48) recruited from the alternative care services offered by the cities of Florence and Prato and involved in the Caring project. The study employed the World Health Organization's Well-Being Index (WHO-5), Participation in Decision Making Scale and Schneider's Agency Scale as quantitative sources of data. The data were analyzed using structural equation modelling controlled for demographic data. The findings show that participation in decision making fully mediated the positive association between dispositional agency and well-being. The conclusions are discussed in terms of integrated practical recommendations for professionals concerned with care leaver protection and the development of psychological and educational interventions.

3. Regional Integrated Systems for Sustainable Human Development in local level

Abstract

Nowadays, development processes are also crucially embedded in diverse local and country contexts, with high diversity and heterogeneity of values, interests, policy ideas and discourses within the socio-economic infrastructure and policy networks of each society. Moreover, as the world cogitates about the post-2015 Sustainable Development Goals (SDG) Agenda, the importance of dealing with economic competitiveness, social inclusion, human rights and environmental protection were essential in the territorial level (UNDP, 2014). In these circumstances, the synergies between local development (LD) and Sustainable Human Development (SHD) will become more remarkable than ever before. The aim of this paper is to deepen the importance of the integrated territorial systems for SHD at the local level, based on the recognition that the SHD cannot be accomplished without the LD but neither without a coherent MLG setting., Hoy en día, los procesos de desarrollo están además insertados en distintos contextos locales y nacionales, caracterizados por una gran heterogeneidad de valores, intereses, ideas políticas y debates entre las infraestructuras socioeconómicas y redes políticas de cada sociedad. Además, a partir de las reflexiones en la Agenda de los Objetivos de Desarrollo Sostenible (ODS) post-2015, la importancia de enfrentar temáticas como la competitividad económica, la inclusión social, los derechos humanos y la protección ambiental fueron esenciales a nivel territorial (PNUD, 2014). En estas circunstancias, las sinergias entre el desarrollo local (DL) y el desarrollo humano sostenible (DHS) están haciéndose más notables. El objetivo de este artículo es profundizar en la importancia de los sistemas territoriales integrados para el DHS a nivel local, basado en la conciencia de que el DHS no puede lograrse sin el DL, pero tampoco sin un coherente ajuste de la gobernanza multinivel., Aujourd’hui, les processus de développement sont également insérés dans différents contextes locaux et nationaux, caractérisés par une grande variété de valeurs, d’intérêts, d’opinions politiques et de débats entre les infrastructures socio-économiques et les réseaux politiques de chaque société. Également, à partir des réflexions sur l’Agenda des Objectifs de Développement Durable (ODD) post-2015, l’importance d’aborder des questions comme la compétitivité économique, l’inclusion sociale, les droits humains et la protection environnementale furent essentiels dans le niveau territorial (PNUD, 2014). Dans ces circonstances, les synergies entre le Développement local (DL) et le Développement Humain Durable (DHD) sont devenus plus remarquables. Abstract

Objective To discuss the results of the KETASER01 trial and the reasons for its failure, particularly in view of future studies. Methods KETASER01 is a multicenter, randomized, controlled, open-label, sequentially designed, non-profit Italian study that aimed to assess the efficacy of ketamine compared with conventional anesthetics in the treatment of refractory convulsive status epilepticus (RCSE) in children. Results During the 5-year recruitment phase, a total of 76 RCSEs treated with third-line therapy were observed in five of the 10 participating Centers; only 10 individuals (five for each study arm; five females, mean age 6.5 +/- 6.3 years) were enrolled in the KETASER01 study. Two of the five patients (40%) in the experimental arm were successfully treated with ketamine and two of the five (40%) children in the control arm, where successfully treated with thiopental. In the remaining six (60%) enrolled patients, RCSE was not controlled by the randomized anesthetic(s). Significance The KETASER01 study was prematurely halted due to low eligibility of patients and no successful recruitment. No conclusions can be drawn regarding the objectives of the study. Here, we discuss the KETASER01 results and critically analyze the reasons for its failure in view of future trials.

6. Role of meteorological factors on SARS-CoV-2 infection incidence in Italy and Spain before the vaccination campaign. A multi-city time series study

Abstract

Numerous studies have been conducted worldwide to investigate if an association exists between meteorological factors and the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) infection incidence. Although research studies provide conflicting results, which can be partially explained by different methods used, some clear trends emerge on the role of weather conditions and SARS-CoV-2 infection, especially for temperature and humidity. This study sheds more light on the relationship between meteorological factors and SARS-CoV-2 infection incidence in 23 Italian and 52 Spanish cities. For the purposes of this study, daily air temperature, absolute and relative humidity, wind speed, ultraviolet radiation, and rainfall are considered exposure variables. We conducted a two-stage meta-regression. In the first stage, we estimated the exposure-response association through time series regression analysis at the municipal level. In the second stage, we pooled the association parameters using a meta-analytic model. The study demonstrates an association between meteorological factors and SARS-CoV-2 infection incidence. Specifically, low levels of ambient temperatures and absolute humidity were associated with an increased relative risk. On the other hand, low and high levels of relative humidity and ultraviolet radiation were associated with a decreased relative risk. Concerning wind speed and rainfall, higher values contributed to the reduction of the risk of infection. Overall, our results contribute to a better understanding of how the meteorological factors influence the spread of the SARS-CoV-2 and should be considered in a wider context of existing robust literature that highlight the importance of measures such as social distancing, improved hygiene, face masks and vaccination campaign.

Abstract

This chapter will explore the role of stakeholder and institutional networks in shaping the development of social enterprise ecosystems across Europe. A Weberian view of power is adopted in order to show that dominant stakeholders can distort reality, obscure truth, and create paradigmatic narratives while pursuing their own interests. This chapter will also draw upon biological evolutionary theory, social network theory, and network pluralism to identify a typology of social enterprise ecosystems. This typology was developed based upon qualitative data (semi-structured interviews and focus groups) collected across 11 European countries from 258 key stakeholders, while also exploring the historical, political, social, legal, and economic conditions in each country. The data was used to create stakeholder network maps for each country, which were then analyzed to identify trends and linkages. The results identify the emergence of four social enterprise ecosystem types (Statist-macro, Statist-micro, Private-macro, and Private-micro) that help explain the differences found in the social enterprise ecosystems of all eleven countries. The emergence of an “ideal-type” ecosystem is posited in relation to these four types, which can be used by stakeholders across Europe to understand how best to support their local social economies.

Abstract

This thesis sheds light on the multidimensional development experienced by China in the last decades, investigating to which extent different regions and segments of the society have benefitted from this process. The thesis comprises three independent chapters, each of which addresses this issue from a different angle and with different methodologies. The purpose of the first chapter is to document and analyze the trend and distribution of income, education and nutrition of Chinese individuals between 1989 and 2011. These three dimensions are analyzed separately (dashboard approach) through similar techniques, to preserve the possibility to compare the results of different dimensions. The inequalities observed in the three variables are measured with the Gini index, and are associated to the policies of the government and other explanatory variables. Moreover, the first chapter investigates how each type of inequality is related to the inequality of opportunities, and how the different types of discrimination evolved over time. The discrimination of individuals based on their circumstances (inequality of opportunities) is measured over time with a regression-based decomposition of inequalities. Income, education and nutrition inequalities show different trends, leading to dimension-specific policy implications. The purpose of the second chapter is to measure the Chinese poverty reduction in the multidimensional space and to individuate an informative index of multidimensional deprivation. Two alternative methodologies, the Multidimensional Poverty Index (MPI) and the Multidimensional Synthesis Indicator (MSI), are adopted and compared, highlighting the strengths and weaknesses of both indexes. Moreover, an original version of the MSI index is introduced, consistently with the idea of income as a mean, not an end, to achieve well-being. Abstract

Today, Mediterranean marginal territories are facing tremendous challenges but at the same time they have relevant endogenous resources, which are often underutilized and unexploited and that could be pivotal for the strategic recovery and economic and social development of the whole European territory. In the last decades, they have been characterised by a progressive abandonment in favour to urban areas, with consequent high social costs such as the hydrogeological instability, degradation and soil erosion. This research investigates the potential active role of Mediterranean "marginal territories" with respect to the re-formulation, adaptation, interpretation and implementation of the European development policies. The paper aims to verify the idea that Mediterranean marginal territories, in the sense of weak, mountainous and inland, could take part at the construction of their own development trajectories and actively contribute to the harmonious development of Europe, creating new jobs opportunities and stable development patterns. Moreover, the paper aims to formulate policy implications and strategies for the studied areas and for Mediterranean marginal territories more in general. The structure of this paper starts from general theoretical arguments and a short description of European policies for development; it follows with the diagnostic analysis of three local territorial contexts - i.e. Casentino (Italy), Algarve (Portugal) and Corse (France) - and then it comes back on the general European issues proposing implications and lessons learnt in the analysis of the development processes at the local level.

27. Multidimensional Development and Inequality in China. The effects of the reforms after Deng Xiaoping

Abstract

This thesis sheds light on the multidimensional development experienced by China in the last decades, investigating to which extent different regions and segments of the society have benefitted from this process. The thesis comprises three independent chapters, each of which addresses this issue from a different angle and with different methodologies. The purpose of the first chapter is to document and analyze the trend and distribution of income, education and nutrition of Chinese individuals between 1989 and 2011. These three dimensions are analyzed separately (dashboard approach) through similar techniques, to preserve the possibility to compare the results of different dimensions. The inequalities observed in the three variables are measured with the Gini index, and are associated to the policies of the government and other explanatory variables. Moreover, the first chapter investigates how each type of inequality is related to the inequality of opportunities, and how the different types of discrimination evolved over time. The discrimination of individuals based on their circumstances (inequality of opportunities) is measured over time with a regression-based decomposition of inequalities. Income, education and nutrition inequalities show different trends, leading to dimension-specific policy implications. The purpose of the second chapter is to measure the Chinese poverty reduction in the multidimensional space and to individuate an informative index of multidimensional deprivation. Two alternative methodologies, the Multidimensional Poverty Index (MPI) and the Multidimensional Synthesis Indicator (MSI), are adopted and compared, highlighting the strengths and weaknesses of both indexes. Moreover, an original version of the MSI index is introduced, consistently with the idea of income as a mean, not an end, to achieve well-being. Abstract

The different and seemingly unrelated practices of Information and Communication Technologies (ICT) used to collect and share personal and scientific data within networked communities, and the organized storage of human genetic samples and information-namely biobanking-have merged with another recent epistemic and social phenomenon, namely scientists and citizens collaborating as "peers" in creating knowledge (or peer-production of knowledge). These different dimensions can be found in joint initiatives where scientists-and-citizens use genetic information and ICT as powerful ways to gain more control over their health and the environment. While this kind of initiative usually takes place only after rights have been infringed (or are put at risk)-as the two cases presented in the paper show-collaborative scientists-and-citizens' knowledge should be institutionally allowed to complement and corroborate official knowledge-supporting policies.

Abstract

Introduction: Status epilepticus (SE) is a lifethreatening neurological emergency. SE lasting longer than 120 min and not responding to first-line and second-line antiepileptic drugs is defined as 'refractory' (RCSE) and requires intensive care unit treatment. There is currently neither evidence nor consensus to guide either the optimal choice of therapy or treatment goals for RCSE, which is generally treated with coma induction using conventional anaesthetics (high dose midazolam, thiopental and/or propofol). Increasing evidence indicates that ketamine (KE), a strong N-methyl-D-aspartate glutamate receptor antagonist, may be effective in treating RCSE. We hypothesised that intravenous KE is more efficacious and safer than conventional anaesthetics in treating RCSE. Methods and analysis: A multicentre, randomised, controlled, open-label, non-profit, sequentially designed study will be conducted to assess the efficacy of KE compared with conventional anaesthetics in the treatment of RCSE in children. 10 Italian centres/ hospitals are involved in enrolling 57 patients aged 1 month to 18 years with RCSE. Primary outcome is the resolution of SE up to 24 hours after withdrawal of therapy and is updated for each patient treated according to the sequential method. Ethics and dissemination: The study received ethical approval from the Tuscan Paediatric Ethics Committee (12/2015). The results of this study will be published in peer-reviewed journals and presented at international conferences.

Abstract

Link and Phelan have proposed to explain the persistence of health inequalities from the fact that socioeconomic status is a "fundamental cause" which embodies an array of resources that can be used to avoid disease risks no matter what mechanisms are relevant at any given time. To test this theory we compared the magnitude of inequalities in mortality between more and less preventable causes of death in 19 European populations, and assessed whether inequalities in mortality from preventable causes are larger in countries with larger resource inequalities.We collected and harmonized mortality data by educational level on 19 national and regional populations from 16 European countries in the first decade of the 21st century. We calculated age-adjusted Relative Risks of mortality among men and women aged 30-79 for 24 causes of death, which were classified into four groups: amenable to behavior change, amenable to medical intervention, amenable to injury prevention, and non-preventable.Although an overwhelming majority of Relative Risks indicate higher mortality risks among the lower educated, the strength of the education-mortality relation is highly variable between causes of death and populations. Inequalities in mortality are generally larger for causes amenable to behavior change, medical intervention and injury prevention than for non-preventable causes. The contrast between preventable and non-preventable causes is large for causes amenable to behavior change, but absent for causes amenable to injury prevention among women. The contrast between preventable and non-preventable causes is larger in Central & Eastern Europe, where resource inequalities are substantial, than in the Nordic countries and continental Europe, where resource inequalities are relatively small, but they are absent or small in Southern Europe, where resource inequalities are also large.In conclusion, our results provide some further support for the theory of "fundamental causes". Abstract

The different and seemingly unrelated practices of ICT devices used to collect and share personal and scientific data within networked communities, and the organized storage of human genetic samples and information—namely biobanking—have merged with another recent epistemic and social phenomenon, namely scientists and citizens collaborating as "peers" in creating knowledge (or peer-production of knowledge). These different dimensions can be found in joint initiatives where scientists-andcitizens use genetic information and ICT as powerful ways to gain more control over their health and the environment. While this kind of initiative usually takes place only after rights have been infringed (or are put at risk)—as the two cases presented in the paper show—, collaborative scientists-and-citizens' knowledge should be institutionally allowed to complement and corroborate official knowledge supporting policies.

Abstract

Fasciola hepatica infection challenges health, welfare and productivity of small ruminants throughout the world. The distribution of F. hepatica in sheep in Europe is usually scattered and studies are generally concerned with a single area making it difficult to compare results from different environments, climates and management regimes. In order to elucidate the current scenario in terms of prevalence and intensity of F. hepatica infection in sheep farms across Europe, a standardized cross-sectional survey was conducted in three pilot areas in Ireland, Switzerland and Italy, all part of the EU funded GLOWORM project. Two consecutive field surveys (in 2012 and 2013) were conducted in the three countries in the same period (August-October) in 361 sheep farms in total. Harmonized procedures (from farm to laboratory) based on pooled samples and the highly sensitive and accurate, diagnostic FLOTAC technique were used. The georeferenced parasitological results were modelled (at the pilot area level) following a Bayesian geostatistical approach with correction for preferential sampling and accounting for climatic and environmental covariates. The observed F. hepatica prevalence rates did not differ between the two study years in any of the three pilot areas, but they did vary between the countries showing high values in Ireland (61.6%) compared to Italy (7.9%) and Switzerland (4.0%). Spatial patterns of F. hepatica distribution were detected by the Bayesian geostatistical approach in Ireland with a high risk of infection in the south-western part of the pilot area there. The latent factor analysis highlighted the importance of year-to-year variation of mean temperature, rainfall and seasonality within a country, while long-term trends of temperature and rainfall dominated between countries with respect to prevalence of infection.
