Abstract

Phase angle (PhA) is a recently proposed marker of nutritional status in many clinical conditions. Its use in patients with obesity presents different critical concerns due to the higher variability of the two measured parameters (resistance, R, and reactance, Xc) that contribute to the determination of PhA. Controversial is the relation between PhA and BMI that might vary with graded levels of obesity due to the variation in fat and free fat mass. Obesity is frequently associated with metabolic, hepatic, cardiovascular and kidney diseases that introduce variations in PhA values, in relation to multimorbidity and severity degree of these diseases. It is reported that the improvement of clinical condition is associated with a positive change in PhA. Also, the treatment of obesity with weight loss might confirm this effect, but with different responses in relation to the type and duration of the intervention applied. In fact, the effect appears not only related to the percentage of weight loss but also the possible loss of free fat mass and the nutritional, metabolic and structural modifications that might follow each therapeutic approach to decrease body weight. We can conclude that the PhA could be used as marker of health status in patients with obesity supporting an appropriate weight loss intervention to monitor efficacy and fat free mass preservation.

3. Should I care for my mum or for my kid? Sandwich generation and depression burden in Italy

Abstract

Setting and objective: In Italy, over the last decades, elderly care has been mostly provided by family members, especially adult offspring, and in particular daughters. This paper investigates the relationship between informal caregiving and mental distress among Italians aged 35–59, with a focus on gender effect and parenthood responsibilities. Data: The dataset is the European Health Interview Survey (EHIS), second wave, year 2015. As far as it is known, the Italian EHIS has not been used for studies on ageing and caregiving. Methods: Using selected subsamples, a Propensity Score Matching between caregivers and non-caregivers aged 35–59 is implemented, with the aim of measuring the difference in level of depression, if any, between the two groups. Results: Findings show that women providing their frail relatives with informal care are less likely to suffer from mental distress compared to non-carers. However, results change radically if they have children aged less than 15 at home, and a higher probability of being depressed is detected for women overwhelmed by the double responsibility of assisting both dependent relatives and their own children. Results are not significant for men.

4. Detection of HIV-1-specific gastrointestinal tissue resident CD8+ T-cells in chronic infection.

Abstract

Tissue-resident memory (TRM) CD8+ T-cells are non-recirculating, long-lived cells housed in tissues that can confer protection against mucosal pathogens. Human immunodeficiency virus-1 (HIV-1) is a mucosal pathogen and the gastrointestinal tract is an important site of viral pathogenesis and transmission. Thus, CD8+ TRM cells may be an important effector subset for controlling HIV-1 in mucosal tissues. This study sought to determine the abundance, phenotype, and functionality of CD8+ TRM cells in the context of chronic HIV-1 infection. We found that the majority of rectosigmoid CD8+ T-cells were CD69+CD103+S1PR1- and T-betLowEomesoderminNeg, indicative of a tissue-residency phenotype similar to that described in murine models. HIV-1-specific CD8+ TRM responses appeared strongest in individuals naturally controlling HIV-1 infection. Two CD8+ TRM subsets, distinguished by CD103 expression intensity, were identified. CD103Low CD8+ TRM primarily displayed a transitional memory phenotype and contained HIV-1-specific cells and cells expressing high levels of Eomesodermin, whereas CD103High CD8+ TRM primarily displayed an effector memory phenotype and were EomesoderminNeg. These findings suggest a large fraction of CD8+ T-cells housed in the human rectosigmoid mucosa are tissue-resident and that TRM contribute to the anti-HIV-1 immune response. Further exploration of CD8+ TRM will inform development of anti-HIV-1 immune-based therapies and vaccines targeted to the mucosa.

6. Reproductive and gynecological complication risks among thyroid cancer survivors.

Abstract

PURPOSE:Thyroid cancer is the most rapidly increasing cancer in the USA, affects a young, mostly female population, and has high survival. The aim of this study was to determine if there is an increased risk of reproductive system adverse events or pregnancy complications among women diagnosed with thyroid cancer under the age of 50. METHODS:Up to five female cancer-free individuals were matched to each female thyroid cancer survivor diagnosed before the age of 50 based on birth year, birth state, and follow-up time, within the Utah Population Database. Medical records were used to identify disease diagnoses stratified over three time periods: 0-1, > 1-5, and > 5-10 years after cancer diagnosis. Cox proportional hazards models were used to estimate hazard ratios (HR) with adjustment on matching factors, race, BMI, and Charlson Comorbidity Index. RESULTS:There were 1832 thyroid cancer survivors and 7921 matched individuals. Thyroid cancer survivors had higher rates of having multiple health conditions associated with the gynecological system (15.4% vs. 9.4%) and pregnancy (14.3% vs 9.5%) > 1-5 years after cancer diagnosis. Increased risks persisted > 5-10 years after cancer diagnosis for menopausal disorders (HR = 1.78, 99% CI = 1.37, 2.33) and complications related to pregnancy (HR = 2.13, 99% CI = 1.14, 3.98). Stratified analyses showed these risks remained increased across different treatment types. CONCLUSIONS:There were significant risk increases in reproductive system and pregnancy complications among female thyroid cancer survivors within this study. IMPLICATIONS FOR CANCER SURVIVORS:Although radiation has been linked to reproductive risks in previous studies, we found risks were increased in patients regardless of treatment.

7. Differential Expression of CD8+ T Cell Cytotoxic Effector Molecules in Blood and Gastrointestinal Mucosa in HIV-1 Infection.

Abstract

We previously reported that CD8+ T cells in human gastrointestinal mucosa exhibit reduced perforin expression and weak or impaired cytotoxic capacity compared with their counterparts in blood. Nevertheless, these cells degranulate and express cytokines and chemokines in response to cognate Ag. In addition to weak expression of perforin, earlier studies suggested differential regulation of perforin and granzymes (Gzms), with GzmA and B expressed by significantly higher percentages of mucosal CD8+ T cells than perforin. However, this topic has not been fully explored. The goal of this study was to elucidate the expression and coexpression patterns of GzmA, B, and K in conjunction with perforin in rectosigmoid CD8+ T cells during HIV-1 infection. We found that expression of both perforin and GzmB, but not GzmA or GzmK, was reduced in mucosa compared with blood. A large fraction of rectosigmoid CD8+ T cells either did not express Gzms or were single-positive for GzmA. Rectosigmoid CD8+ T cells appeared skewed toward cytokine production rather than cytotoxic responses, with cells expressing multiple cytokines and chemokines generally lacking in perforin and Gzm expression. These data support the interpretation that perforin and Gzms are differentially regulated, and display distinct expression patterns in blood and rectosigmoid T cells. These studies may help inform the development of strategies to combat HIV-1 and other mucosal pathogens.

Purpose of review:This review summarizes recent literature defining tissue-resident memory T cells (TRM) and discusses implications for HIV pathogenesis, vaccines, and eradication efforts.Recent findings:Investigations using animal models and human tissues have identified a TRM transcriptional profile and elucidated signals within the tissue microenvironment leading to TRM development and maintenance. TRM are major contributors to host response in infectious diseases and cancer; in addition, TRM contribute to pathogenic inflammation in a variety of settings. Although TRM are daunting to study in HIV infection, recent work has helped define their molecular signatures and effector functions and tested strategies for their mobilization. Exclusive reliance on blood sampling to gain an understanding of host immunity overlooks the contribution of TRM, which differ in significant ways from their counterparts in circulation. It is hoped that greater understanding of these cells will lead to novel approaches to prevent and/or eradicate HIV infection.

10. Endocrine and Metabolic Diseases Among Colorectal Cancer Survivors in a Population-Based Cohort.

Abstract

BACKGROUND:There are an estimated 1.4 million colorectal cancer (CRC) survivors in the United States. Research on endocrine and metabolic diseases over the long term in CRC survivors is limited. Obesity is a risk factor for CRC; thus it is of interest to investigate diseases that may share this risk factor, such as diabetes, for long-term health outcomes among CRC survivors. METHODS:A total of 7114 CRC patients were identified from the Utah Population Database and matched to a general population cohort of 25 979 individuals on birth year, sex, and birth state. Disease diagnoses (assessed over three time periods of 1-5 years, 5-10 years, and >10 years) were identified using electronic medical records and statewide ambulatory and inpatient discharge data. Cox proportional hazard models were used to estimate the risk of endocrine and metabolic disease. RESULTS:Across all three time periods, risks for endocrine and metabolic diseases were statistically significantly greater for CRC survivors compared with the general population cohort. At 1-5 years postdiagnosis, CRC survivors' risk for diabetes mellitus with complications was statistically significantly elevated (hazard ratio [HR] = 1.36, 99% confidence interval [CI] = 1.09 to 1.70). CRC survivors also experienced a 40% increased risk of obesity at 1-5 years postcancer diagnosis (HR= 1.40, 99% CI= 1.66 to 2.18) and a 50% increased risk at 5-10 years postdiagnosis (HR = 1.50, 99% CI= 1.16 to 1.95). CONCLUSIONS:Endocrine and metabolic diseases were statistically significantly higher in CRC survivors throughout the follow-up periods of 1-5 years, 5-10 years, and more than 10 years postdiagnosis. As the number of CRC survivors increases, understanding the long-term trajectory is critical for improved survivorship care.

11. The implementation of health technology assessment principles in public decisions concerning orphan drugs

Abstract

Purpose: Over the last few years, the share of public spending for orphan drugs (ODs) has increased in several western countries, raising concern on the exemptions granted to this sector with respect to the implementation of health technology assessment (HTA) principles. The aim of this paper is to shed light on both the HTA criteria adopted and the international agreements implemented in the OD regulation, given the new challenges imposed on western countries by a growing number of therapies for rare diseases. Methods: We carried out a literature review to analyse the development of the international debate on the adaptability of HTA criteria for the OD assessment and regulation. The time span lies between January 1990 and May 2018, and the policies considered relate to both market authorization and reimbursement decisions within western countries. We focus specifically on HTA criteria in some of the dimensions included in the Core Model of the European net for HTA (EUnetHTA). Results: OD high prices, the absence of clarity on the possible high revenues realized by the distribution of a new OD outside the national borders, the risk that – once marketed – a new OD can be used to treat common diseases, are all issues that raise concern on OD regulation and have to be carefully monitored by policymakers in the next future. Conclusions: Across western countries, the preferential track granted to ODs in the implementation of HTA principles is not homogeneous, but fragmented and differentiated. The need for common rules at an international level is underlined, with a view to assessing the sustainability of a sector which, due to this regulatory void, can lend itself to producers’ strategic and opportunistic behaviours.

12. The Development of a Consistent Europe-Wide Approach to Investigating the Economic Impact of Myalgic Encephalomyelitis (ME/CFS)

Abstract

We have developed a Europe-wide approach to investigating the economic impact of Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS), facilitating acquisition of information on the economic burden of ME/CFS, and international comparisons of economic costs between countries. The economic burden of ME/CFS in Europe appears large, with productivity losses most significant, giving scope for substantial savings through effective prevention and treatment. However, economic studies of ME/CFS, including cost-of-illness analyses and economic evaluations of interventions, are problematic due to different, arbitrary case definitions, and unwillingness of doctors to diagnose it. We therefore lack accurate incidence and prevalence data, with no obvious way to estimate costs incurred by undiagnosed patients. Other problems include, as for other conditions, difficulties estimating direct and indirect costs incurred by healthcare systems, patients and families, and heterogeneous healthcare systems and patterns of economic development across countries. We have made recommendations, including use of the Fukuda (CDC-1994) case definition and Canadian Consensus Criteria (CCC), a pan-European common symptom checklist, and implementation of prevalence-based cost-of-illness studies in different countries using an agreed data list. We recommend using purchasing power parities (PPP) to facilitate international comparisons, and EuroQol-5D as a generic measure of health status and multi-attribute utility instrument to inform future economic evaluations in ME/CFS.

Abstract

Background: Thyroid cancer is the most rapidly increasing cancer in the United States, affects a young population, has high survival, and is one of the most common cancers in people under age 40. The aim of this study was to examine the risks of aging-related diseases in a statewide sample of thyroid cancer survivors who were diagnosed <40 years compared with those diagnosed ≥40 and a cancer-free sample.Methods: Thyroid cancer survivors diagnosed 1997 to 2012 were matched to up to 5 cancer-free individuals on birth year, sex, birth state, using the statewide Utah Population Database. Medical records were used to identify disease diagnoses stratified over three time periods: 1 to 5, >5 to 10, and 10+ years after cancer diagnosis. Cox proportional hazards models were used to estimate hazard ratios with adjustment on matching factors, race, body mass index, and Charlson Comorbidity Index.Results: There were 3,706 thyroid cancer survivors and 15,587 matched cancer-free individuals (1,365 cases diagnosed <40 years old). Both age groups had increased risks for multiple circulatory health conditions 1 to 5 years after cancer diagnosis compared with cancer-free individuals. Survivors <40 had a higher risk of hypertension, cardiomyopathy, and nutritional deficiencies.Conclusions: Increased risks for diseases associated with aging were observed for both age groups, with younger thyroid cancer survivors having higher risks for select diseases.Impact: As thyroid cancer survivors in this study were found to have increased risks for aging-related diseases, future studies are needed to assess what can be done to reduce the increased risks of these long-term health effects. Mucosal tissues of the genitourinary and gastrointestinal tracts serve as the primary sites of HIV-1 transmission, and provide "front line" barrier defenses against HIV-1 and other pathogens. In addition, the gastrointestinal tract remains a significant viral reservoir throughout the chronic phase of infection. Tissue-based immune responses may be critical in fighting infection, and understanding these defenses may lead to improved vaccines and immunotherapeutic strategies.

Mucosal tissues of the genitourinary and gastrointestinal tracts serve as the primary sites of HIV-1 transmission, and provide "front line" barrier defenses against HIV-1 and other pathogens. In addition, the gastrointestinal tract remains a significant viral reservoir throughout the chronic phase of infection. Tissue-based immune responses may be critical in fighting infection, and understanding these defenses may lead to improved vaccines and immunotherapeutic strategies.

Abstract

Durante gli ultimi decenni, il calo delle nascite e il contestuale allungamento della vita media hanno mutato l’equilibrio demografico dell’Europa e di buona parte dei Paesi industrializzati. Il nuovo assetto comporta una riduzione nella consistenza delle coorti di giovani generazioni, più che compensata da un incremento nel numero di anziani. L’invecchiamento è associato ad un progressivo deterioramento dello stato di salute delle persone e conseguentemente ad un accresciuto bisogno di cure. Queste possono essere fornite per via formale, cioè da personale qualificato e remunerato, oppure per via informale, da parenti, amici o vicini di casa che dedicano il proprio tempo per assistere la persona non autosufficiente. Il termine utilizzato per designare chi offre cure informali è caregiver, ovvero “donatore di cure”. All’interno del nucleo familiare sono prevalentemente le donne a farsi carico della cura dei soggetti più fragili. In particolare, per quanto riguarda la cura degli anziani, la categoria più rappresentata è quella delle figlie femmine, spesso schiacciate fra le responsabilità verso i figli non ancora autonomi e l’assistenza ai genitori anziani e per questo definite, insieme ai loro coetanei, “generazione sandwich. Il capitolo prende spunto da queste considerazioni per analizzare, tramite il contributo della letteratura, le scelte allocative individuali e famigliari nel fornire assistenza a genitori o parenti anziani in Europa e nei Paesi occidentali. In particolare si studiano gli effetti sul piano lavorativo e le possibili ricadute sulla salute relative alla categoria maggiormente rappresentata di caregiver, quella delle figlie femmine.

In this study, we sought to determine whether two beekeeper-applied pesticide products, tau-fluvalinate and Fumagilin-B®, and one agrochemical, chlorothalonil, impact the nutrient levels in honey bee workers in a natural colony environment. Treatments were performed in-hive and at three different periods (fall, spring, and summer) over the course of one year. Bees were sampled both at pre-treatment and two and four weeks post-treatment, weighed, and their protein and carbohydrate levels were determined using BCA and anthrone based biochemical assays, respectively. We report that, based on the pesticide concentrations tested, no significant negative impact of the pesticide products was observed on wet weight, protein levels, or carbohydrate levels of bees from treated colonies compared with bees from untreated control colonies.

Abstract

The Silicon Vertex Tracker of the BaBar experiment is a five-layer, double-sided AC-coupled silicon microstrip detector operating on the PEP-II storage ring at the Stanford Linear Accelerator Center. After more than four years of running, the silicon sensors and the front-end electronics in the inner layer have absorbed radiation doses up to 2 Mrad. In this paper we present results from radiation hardness tests and discuss the implications of the absorbed radiation dose on the Silicon Vertex Tracker lifetime.

Abstract

The Silicon Vertex Tracker (SVT) of the BABAR experiment at the PEP-II asymmetric B factory is a crucial element of the BABAR physics program. The SVT is a five layer device with double-sided silicon strip detectors and a custom readout chip based on a time-over-threshold circuit. The SVT is fully operational since the installation in the experiment in 1999. We briefly review the detector performance after more than 3 years of data taking in factory mode. The high-luminosity running of PEP-II through the end of the decade will increase the radiation level in the SVT above its design limit. Radiation issues will be reviewed and results from radiation tests of the various detector components will be presented. We finally discuss the outlook for the long-term running of the SVT.

Abstract

The Silicon Vertex Tracker (SVT) is an essential component of the BABAR experiment at the PEP-II asymmetric B factory. The SVT has performed very well and several radiation studies have been carried out to determine that it will continue do so up to integrated radiation doses of 5 Mrad. The SVT radiation monitoring and protection system consists of silicon PIN-diodes, which are suffering from radiation damage, reducing their precision. Diamond sensors are highly resistant to radiation damage and the possibility of replacing the PIN-diodes with diamonds is currently under investigation. Two polycrystalline diamonds have been installed near the BABAR interaction region and show promising results when compared to the PIN-diodes.

Abstract

The BaBar Silicon Vertex Tracker is a five layers, double sided AC-coupled silicon microstrip detector operating on the PEP-II storage ring at the Stanford Linear Accelerator Center. The performance of the SVT after 4 years of running is described. Results from radiation hardness tests are presented and the implications of the absorbed radiation dose on the SVT lifetime are discussed.

Abstract

The Silicon Vertex Tracker (SVT) of the BABAR experiment at the PEP-II asymmetric B factory is a five layer double sided, AC coupled silicon microstrip detector. It represents the crucial element to precisely measure the decay position of B mesons. The SVT architecture is shown and its performances, like hit resolutions and efficiencies, are described. Radiation issues are discussed and the alignment procedure for the SVT is described. As an example for the physics performance the B lifetime measurement, including B vertex reconstruction, is shown.

Abstract

A brief summary of the design goals, description, and performance of the BABAR Silicon Vertex Tracker is given. Results from radiation hardness tests are discussed, which indicate satisfactory operation up to 5 Mrad of accumulated radiation. The local alignment procedure has made significant improvements recently, and four readout sections were recovered during the BABAR shutdown in 2002.

Abstract

The silicon-vertex tracker (SVT) for the BaBar Experiment at the PEP-II Asymmetric B Factory at Stanford Linear Accelerator Center (SLAC), is a five-layer double-sided ac-coupled silicon-microstrip detector. It is a crucial element for the precise measurement of the decay position of B mesons, satisfying the severe constraints imposed by the accelerator design, in terms of geometry of the interaction region and conditions of operation. In the following paper, the SVT performance is described. Radiation issues are discussed and the results of different tests aimed to study the decrease in the SVT performance due to radiation damage are presented. The alignment procedure is described, and a few BaBar physics results are presented as examples of the SVT capabilities to reconstruct decay vertices with good resolution and efficiency.

Abstract

The BaBar Silicon Vertex Tracker (SVT) is a five layer device, made from double-sided silicon strip detectors and read out via a custom time-over-threshold circuit, the AToM chip. The SVT is an essential part of the physics program of BaBar, and is able to reconstruct B meson decay vertices with a precision sufficient to measure time-dependent CP violating asymmetries at the PEP-II asymmetric e+e- collider. This report will give an overview of the SVT, with particular focus on the performance of the SVT, which has been taking colliding beam data since May 1999.

Abstract

The BaBar Silicon Vertex Tracker (SVT) is designed to provide the high-precision vertexing necessary for making measurements of CP violation at the SLAC B-Factory PEP-II. The instrument consists of five layers of double-sided silicon strip detectors and has been installed in the BaBar experiment and taking colliding beam data since May 1999. An overview of the design as well as performance and experience from the initial running will be presented.

Abstract

The Silicon Vertex Tracker (SVT) of the BaBar experiment at the PEP-II asymmetric B factory consists of five layers of double-sided, AC-coupled silicon strip detectors. The detectors are readout with a custom IC, capable of simultaneous acquisition, digitization and transmission of data. The SVT geometry is shown and the construction phases of its modules are described in detail, with emphasis on the bending procedures needed for the arch-modules of the outer layers.

Abstract

The Silicon Vertex Tracker (SVT) for the BaBar experiment at the PEP-II asymmetric B factory is a 5-layer device based on double-sided, AC-coupled silicon strip detectors. It is read out by a custom IC, the AToM chip, that can simultaneously acquire, digitize and transmit data. The main purpose of the SVT is to accurately measure the decay position of the B mesons that are produced, which is essential for extracting CP asymmetries. Here, we report on the SVT design as well as progress on its fabrication and assembly.

Since 2006, beekeepers have reported increased losses of Apis mellifera colonies, and one factor that has been potentially implicated in these losses is the microsporidian Nosema ceranae. Since N. ceranae is a fairly recently discovered parasite, there is little knowledge of the variation in infection levels among individual workers within a colony. In this study we examined the levels of infection in individual bees from five colonies over three seasons using both spore counting and quantitative real-time PCR. The results show considerable intra-colony variation in infection intensity among individual workers with a higher percentage of low-level infections detected by PCR than by spore counting. Colonies generally had the highest percentage of infected bees in early summer (June) and the lowest levels in the fall (September). Nosema apis was detected in only 16/705 bees (2.3%) and always as a low-level co-infection with N. ceranae. The results also indicate that intra-colony variation in infection levels could influence the accuracy of Nosema diagnosis.

38. Exogenously-introduced Homing Endonucleases Catalyze Double-stranded DNA Breaks in Aedes aegypti

Abstract

Aedes aegypti transmits the viruses which cause yellow fever, dengue fever, and dengue hemorrhagic fever. Homing endonucleases are selfish genetic elements which introduce double-stranded DNA (dsDNA) breaks in a sequence-specific manner. In this study, we aimed to validate a somatic assay to detect recombinant homing endonuclease (rHE)-induced dsDNA breaks in both cultured cells and adult female Ae. aegypti. While the cell culture-based two plasmid assay used to test rHE ability to induce dsDNA breaks was inconclusive, assays used to test rHEs in Ae. aegypti were successful. Recognition sequences for various rHEs were introduced into Ae. aegypti through germline transformation, and imperfect repair at each of these exogenous sites was evaluated. In mosquitoes containing a single exogenous HE site, imperfect gap repair was detected in 40% and 21% of clones sequenced from mosquitoes exposed to I-PpoI and Iâ SceI, respectively. In mosquitoes containing two exogenous HE sites flanking a marker gene (EGFP), 100% of clones sequenced from mosquitoes exposed to I-PpoI, I-CreI, and I-AniI demonstrated excision of EGFP. No evidence of EGFP excision or imperfect repair at any HE recognition site was detected in mosquitoes not exposed to a rHE. In summary, a somatic genomic footprint assay was developed and validated to detect rHE or other meganuclease-induced site-specific dsDNA breaks in chromosomal DNA in Ae. aegypti.

Abstract

This study aimed to develop and optimize a method for the extraction and analysis of moenomycin antibiotics (a.k.a. flavomycin) in corn-based feed premix and in chicken litter. Moenomycin-A was isolated from chicken litter using pressurized liquid extraction followed by a solid-phase extraction (SPE) clean-up step. The highly lipophilic nature of moenomycin necessitated the use of the less hydrophobic sorbent, C4-based SPE cartridge, and a higher temperature elution solvent, methanol at 50ºC, in order to obtain satisfactory percent recoveries. After clean-up, the sample was analyzed by liquid chromatography–electrospray ionization–mass spectrometry (LC–ESI–MS). Various reversed-phase columns were examined, including C18, CN, perfluorinated C6, and a porous graphitic carbon. The set of conditions that gave the highest separation efficiency while still maintaining symmetrical peak shape was the C18 column using the H2O + 0.3% HCOOH and acetonitrile mobile phase.
