Abstract: We aimed to identify persistent asthma phenotypes among adolescents and to evaluate longitudinally asthma-related outcomes across phenotypes. Adolescents (13-17 years) from the prospective, observational, and multicenter INSPIRERS studies, conducted in Portugal and Spain, were included (n = 162). Latent class analysis was applied to demographic, environmental, and clinical variables, collected at a baseline medical visit. Longitudinal differences in clinical variables were assessed at a 4-month follow-up telephone contact (n = 128). Three classes/phenotypes of persistent asthma were identified. Adolescents in class 1 (n = 87) were highly symptomatic at baseline and presented the highest number of unscheduled healthcare visits per month and exacerbations per month, both at baseline and follow-up. Class 2 (n = 32) was characterized by female predominance, more frequent obesity, and uncontrolled upper/lower airways symptoms at baseline. At follow-up, there was a significant increase in the proportion of controlled lower airway symptoms (p < 0.001). Class 3 (n = 43) included mostly males with controlled lower airways symptoms; at follow-up, while keeping symptom control, there was a significant increase in exacerbations/month (p = 0.015). We have identified distinct phenotypes of persistent asthma in adolescents with different patterns in longitudinal asthma-related outcomes, supporting the importance of profiling asthma phenotypes in predicting disease outcomes that might inform targeted interventions and reduce future risk.

2. Markets, Infrastructrures and Infrastructuring Markets

Abstract: Despite a growing understanding of market infrastructures - the rules and socio-material arrangements that enable agreements on the properties of goods, and the calculation of value, equivalence and exchange - we know little of what lies beneath the arrangements that underpin and are implicated in exchange. The socio-material lens has done much to explain how specific assemblages circulate information and goods, but has done little to explain how different infrastructures configure relations between dispersed market practices. Using the history of the development of the market for market research we show how knowledge-based infrastructures constitute markets as knowledge objects: new expertise emerged through alliances between academia, government, and private actors form a new occupation embodied in specialist agencies that set themselves up in an infrastructural relation to marketing practices. Our conceptualization of markets as knowledge objects extends extant understandings of markets by showing how: 1) extant knowledge-based infrastructures are drawn on to construct new markets; 2), infrastructural relations emerge between different markets to constitute multiple systems of provision and demand, leading to an increasingly valuable knowledge infrastructure; and 3) organized practices in one market are often heavily reliant on connections to other markets, including knowledge-based infrastructures such as market research services.

3. Implementing Marketization in Public Healthcare Systems:Performing Reform in the English National Health Service

Abstract: To implement marketization in public healthcare systems, policymakers need to situate abstract models of prescriptive practice in complex settings. Using a performativity lens we show how policy processes bring about the changes they presume. Investigating the implementation of the Health and Social Care Act 2012, and the development of a policy instruments and Clinical Commissioning Groups, we explicate the performance of a marketization programme. This longitudinal perspective on the interactions amongst the Act’s aims, the multiple constituencies the Act attempted to enrol and the existing socio-technical arrangements the Act aimed to change, generates three core contributions. We (1) characterise the performativity of policy instruments as a process of bricolage that incorporates the principled attitude of making do on both sides of the divide – those who design the policy and those who are charged to implement it; (2) identify the mechanisms through which the performativity of an envisioned model of marketization operates at multiple scales within a complex and highly distributed system of provision; and (3) document and explicate why specific performances result in misfires and unintended outcomes. Thus, we conceptualise policy performativity as a non-linear, dynamic process where theories and their effects are constantly being assessed and reconfigured.

Abstract: Background: Poor medication adherence is a major challenge in asthma, and objective assessment of inhaler adherence is needed. The InspirerMundi app aims to monitor adherence while providing a positive experience through gamification and social support. Objective: This study aimed to evaluate the feasibility and acceptability of the InspirerMundi app to monitor medication adherence in adolescents and adults with persistent asthma (treated with daily inhaled medication). Methods: A 1-month mixed method multicenter observational study was conducted in 26 secondary care centers from Portugal and Spain. During an initial face-to-face visit, physicians reported patients' asthma therapeutic plan in a structured questionnaire. During the visits, patients were invited to use the app daily to register their asthma medication intakes. A scheduled intake was considered taken when patients registered the intake (inhaler, blister, or other drug formulation) by using the image-based medication detection tool. At 1 month, patients were interviewed by phone, and app satisfaction was assessed on a 1 (low) to 5 (high) scale. Patients were also asked to point out the most and least preferred app features and make suggestions for future app improvements. Results: A total of 107 patients (median 27 [P25-P75 14-40] years) were invited, 92.5% (99/107) installed the app, and 73.8% (79/107) completed the 1-month interview. Patients interacted with the app a median of 9 (P25-P75 1-24) days. At least one medication was registered in the app by 78% (77/99) of patients. A total of 53% (52/99) of participants registered all prescribed inhalers, and 34% (34/99) registered the complete asthma therapeutic plan. Median medication adherence was 75% (P25-P75 25%-90%) for inhalers and 82% (P25-P75 50%-94%) for other drug formulations. Abstract: Organic dyes have been studied for applications in large-area, flexible, cheap, and efficient organic electronic devices. Among them, diketopyrrolopyrrole (DPP) has gained attention thanks to its planar structure, photochemical and thermal stability, and easy processability. Also, the electron-withdrawing nature of DPP makes its application attractive in the synthesis of donor-acceptor (D-A) copolymers, with appealing features such as the tunable energy levels and photophysical and electrochemical properties. Inspired by these exciting characteristics, a copolymer was developed based on DPP, thiophene, and fluorene (PFDPP2T). Photophysical and electrochemical studies using both experimental and theoretical approaches were performed aiming to understand the properties of this material, such as, for instance, the D-A characteristic and the outstanding electrochemical stability upon oxidation that enables more than 400 cycles of p-doping. The outcomes unveil fundamental aspects of this class of copolymers, reinforcing their suitability for photo-electrochemical and optoelectronic applications.

8. A new CBD-CC-E spectral similarity scale for optimizing computer-simulated UV?vis spectra

Abstract: A new CBD-CC-E spectral similarity scale is proposed to optimize computer-simulated UV?vis spectra. The scale was tested using the S1?S0 spectrum of the dithienyl-diketopyrrolopyrrole molecule (DPP2T), an important building block for manufacturing materials for optoelectronic applications. Our results indicate that the spectrum calculated at M06/6-311++G(d,p) level was the one that best reproduced the intensity and shape features of the experimental spectrum, while CAM-B3LYP/6-311++G(d,p) was the one that best reproduced the energy. The CBD-CC-E scale makes the comparison between computer-simulated and experimental spectra statistically based, allowing a systematic and automated choice of the theory level whose calculated spectrum best reproduces the shape, intensity or energy of the experimental UV?vis spectrum.

Abstract: Background: Poor medication adherence is a major challenge in asthma, and objective assessment of inhaler adherence is needed. The InspirerMundi app aims to monitor adherence while providing a positive experience through gamification and social support. Objective: This study aimed to evaluate the feasibility and acceptability of the InspirerMundi app to monitor medication adherence in adolescents and adults with persistent asthma (treated with daily inhaled medication). Methods: A 1-month mixed method multicenter observational study was conducted in 26 secondary care centers from Portugal and Spain. During an initial face-to-face visit, physicians reported patients' asthma therapeutic plan in a structured questionnaire. During the visits, patients were invited to use the app daily to register their asthma medication intakes. A scheduled intake was considered taken when patients registered the intake (inhaler, blister, or other drug formulation) by using the image-based medication detection tool. At 1 month, patients were interviewed by phone, and app satisfaction was assessed on a 1 (low) to 5 (high) scale. Patients were also asked to point out the most and least preferred app features and make suggestions for future app improvements. Results: A total of 107 patients (median 27 [P25-P75 14-40] years) were invited, 92.5% (99/107) installed the app, and 73.8% (79/107) completed the 1-month interview. Patients interacted with the app a median of 9 (P25-P75 1-24) days. At least one medication was registered in the app by 78% (77/99) of patients. A total of 53% (52/99) of participants registered all prescribed inhalers, and 34% (34/99) registered the complete asthma therapeutic plan. Median medication adherence was 75% (P25-P75 25%-90%) for inhalers and 82% (P25-P75 50%-94%) for other drug formulations. Abstract: We aimed to identify persistent asthma phenotypes among adolescents and to evaluate longitudinally asthma-related outcomes across phenotypes. Adolescents (13-17 years) from the prospective, observational, and multicenter INSPIRERS studies, conducted in Portugal and Spain, were included (n = 162). Latent class analysis was applied to demographic, environmental, and clinical variables, collected at a baseline medical visit. Longitudinal differences in clinical variables were assessed at a 4-month follow-up telephone contact (n = 128). Three classes/phenotypes of persistent asthma were identified. Adolescents in class 1 (n = 87) were highly symptomatic at baseline and presented the highest number of unscheduled healthcare visits per month and exacerbations per month, both at baseline and follow-up. Class 2 (n = 32) was characterized by female predominance, more frequent obesity, and uncontrolled upper/lower airways symptoms at baseline. At follow-up, there was a significant increase in the proportion of controlled lower airway symptoms (p < 0.001). Class 3 (n = 43) included mostly males with controlled lower airways symptoms; at follow-up, while keeping symptom control, there was a significant increase in exacerbations/month (p = 0.015). We have identified distinct phenotypes of persistent asthma in adolescents with different patterns in longitudinal asthma-related outcomes, supporting the importance of profiling asthma phenotypes in predicting disease outcomes that might inform targeted interventions and reduce future risk.

Methods: Two real-world multicenter observational studies, with one initial face-to-face visit and a 4-month telephone interview, were conducted in 29 secondary care centers from Portugal. During an initial face-to-face visit, patients were invited to use the app daily to register their asthma medication intakes. A scheduled intake was considered taken when patients took a photo of the medication (inhaler, blister, or others) using the image-based medication detection tool. Medication adherence was calculated as the number of doses taken as a percentage of the number scheduled. Interacting with the app =30 days was used as the cut-off for higher app use. Results: A total of 114 patients {median 20 [percentile 25 to percentile 75 (P25-P75) 16-36] years, 62% adults} were invited, 107 (94%) installed the app and 83 (73%) completed the 4-month interview. Patients interacted with the app for a median of 18 [3-45] days, translated on a median use rate of 15 [3-38]%. Median inhaler adherence assessed through the app was 34 [4-73]% when considering all scheduled inhalations for the study period. Inhaler adherence assessed was not significantly correlated with self-reported estimates. Median adherence for oral and other medication was 41 [6-83]% and 43 [3-73]%, respectively. 16. Markets, Infrastructrures and Infrastructuring Markets

Abstract: Despite a growing understanding of market infrastructures - the rules and socio-material arrangements that enable agreements on the properties of goods, and the calculation of value, equivalence and exchange - we know little of what lies beneath the arrangements that underpin and are implicated in exchange. The socio-material lens has done much to explain how specific assemblages circulate information and goods, but has done little to explain how different infrastructures configure relations between dispersed market practices. Using the history of the development of the market for market research we show how knowledge-based infrastructures constitute markets as knowledge objects: new expertise emerged through alliances between academia, government, and private actors form a new occupation embodied in specialist agencies that set themselves up in an infrastructural relation to marketing practices. Our conceptualization of markets as knowledge objects extends extant understandings of markets by showing how: 1) extant knowledge-based infrastructures are drawn on to construct new markets; 2), infrastructural relations emerge between different markets to constitute multiple systems of provision and demand, leading to an increasingly valuable knowledge infrastructure; and 3) organized practices in one market are often heavily reliant on connections to other markets, including knowledge-based infrastructures such as market research services.

17. Implementing Marketization in Public Healthcare Systems : Performing Reform in the English National Health Service

Abstract: To implement marketization in public healthcare systems, policymakers need to situate abstract models of prescriptive practice in complex settings. Using a performativity lens we show how policy processes bring about the changes they presume. Investigating the implementation of the Health and Social Care Act 2012, and the development of a policy instruments and Clinical Commissioning Groups, we explicate the performance of a marketization programme. This longitudinal perspective on the interactions amongst the Act’s aims, the multiple constituencies the Act attempted to enrol and the existing socio-technical arrangements the Act aimed to change, generates three core contributions. We (1) characterise the performativity of policy instruments as a process of bricolage that incorporates the principled attitude of making do on both sides of the divide – those who design the policy and those who are charged to implement it; (2) identify the mechanisms through which the performativity of an envisioned model of marketization operates at multiple scales within a complex and highly distributed system of provision; and (3) document and explicate why specific performances result in misfires and unintended outcomes. Thus, we conceptualise policy performativity as a non-linear, dynamic process where theories and their effects are constantly being assessed and reconfigured.

InspirerMundi app aims to monitor inhaler adherence while turning it into a positive experience through gamification and social support. Abstract: Organic dyes have been studied for applications in large-area, flexible, cheap, and efficient organic electronic devices. Among them, diketopyrrolopyrrole (DPP) has gained attention thanks to its planar structure, photochemical and thermal stability, and easy processability. Also, the electron-withdrawing nature of DPP makes its application attractive in the synthesis of donor-acceptor (D-A) copolymers, with appealing features such as the tunable energy levels and photophysical and electrochemical properties. Inspired by these exciting characteristics, a copolymer was developed based on DPP, thiophene, and fluorene (PFDPP2T). Photophysical and electrochemical studies using both experimental and theoretical approaches were performed aiming to understand the properties of this material, such as, for instance, the D-A characteristic and the outstanding electrochemical stability upon oxidation that enables more than 400 cycles of p-doping. The outcomes unveil fundamental aspects of this class of copolymers, reinforcing their suitability for photo-electrochemical and optoelectronic applications.

Objective: This study aimed to evaluate the feasibility and acceptability of the InspirerMundi app to monitor medication adherence in adolescents and adults with persistent asthma (treated with daily inhaled medication). Methods: A 1-month mixed method multicenter observational study was conducted in 26 secondary care centers from Portugal and Spain. During an initial face-to-face visit, physicians reported patients' asthma therapeutic plan in a structured questionnaire. During the visits, patients were invited to use the app daily to register their asthma medication intakes. A scheduled intake was considered taken when patients registered the intake (inhaler, blister, or other drug formulation) by using the image-based medication detection tool. At 1 month, patients were interviewed by phone, and app satisfaction was assessed on a 1 (low) to 5 (high) scale. Patients were also asked to point out the most and least preferred app features and make suggestions for future app improvements. Results: A total of 107 patients (median 27 [P25-P75 14-40] years) were invited, 92.5% (99/107) installed the app, and 73.8% (79/107) completed the 1-month interview. Patients interacted with the app a median of 9 (P25-P75 1-24) days. At least one medication was registered in the app by 78% (77/99) of patients. A total of 53% (52/99) of participants registered all prescribed inhalers, and 34% (34/99) registered the complete asthma therapeutic plan. Median medication adherence was 75% (P25-P75 25%-90%) for inhalers and 82% (P25-P75 50%-94%) for other drug formulations. Abstract: We aimed to identify persistent asthma phenotypes among adolescents and to evaluate longitudinally asthma-related outcomes across phenotypes. Adolescents (13-17 years) from the prospective, observational, and multicenter INSPIRERS studies, conducted in Portugal and Spain, were included (n = 162). Latent class analysis was applied to demographic, environmental, and clinical variables, collected at a baseline medical visit. Longitudinal differences in clinical variables were assessed at a 4-month follow-up telephone contact (n = 128). Three classes/phenotypes of persistent asthma were identified. Adolescents in class 1 (n = 87) were highly symptomatic at baseline and presented the highest number of unscheduled healthcare visits per month and exacerbations per month, both at baseline and follow-up. Class 2 (n = 32) was characterized by female predominance, more frequent obesity, and uncontrolled upper/lower airways symptoms at baseline. At follow-up, there was a significant increase in the proportion of controlled lower airway symptoms (p < 0.001). Class 3 (n = 43) included mostly males with controlled lower airways symptoms; at follow-up, while keeping symptom control, there was a significant increase in exacerbations/month (p = 0.015). We have identified distinct phenotypes of persistent asthma in adolescents with different patterns in longitudinal asthma-related outcomes, supporting the importance of profiling asthma phenotypes in predicting disease outcomes that might inform targeted interventions and reduce future risk.

Abstract: [Resumen] El artista plástico estadounidense Alexander Calder es conocido principalmente por sus esculturas móviles y sus monumentales estables. A lo largo de su trayectoria, realizó varias colaboraciones con arquitectos, logrando que su aportación artística formase parte del proyecto desde los inicios. En estos casos, la pieza se vuelve parte indisoluble del proyecto, modificando el espacio y generando nuevas relaciones. Este trabajo propone un estudio de la relación entre pieza y edificio a partir del análisis de tres casos de estudio: "La fuente de mercurio" en el pabellón de España para la expo de Paris del 37, "Las nubes colgantes" en el Aula Magna de la Universidad Central de Venezuela y el Móvil incorporado al edificio este de la Galería Nacional de Arte de Washington D.C. Para ello, en primer lugar, se realiza un repaso por los condicionantes del encargo y el contexto de la pieza; en segundo lugar, se analiza la obra de manera aislada; y finalmente se estudia la relación entre la pieza y el edificio y la pieza y el usuario, y se compara con los resultados obtenidos con el estudio del mismo espacio sin la pieza artística con el fin de comprobar el efecto que la obra de Caldera genera en la configuración del espacio arquitectónico., [Abstract] The American artist Alexander Calder is known primarily for his mobile sculptures and monumental stables. Throughout his career, he made several collaborations with architects, making his artistic contribution to be part of the project from the beginning. In these cases, the piece becomes an indissoluble part of the project, modifying the space and generating new relationships. This work proposes a study of the relationship between piece and building based on the analysis of three case studies: "The mercury Fountain" in the Spanish pavilion for the Paris expo of 37, "Floating clouds" in the Aula Magna from the Central University ofVenezuela and the "Untitled" Mobile built into the east building ofthe National Gallery of Art in Washington D.C. To do this, first of all, a review is made of the conditions of the order and the context of the pie ce; second, the work is analyzed in isolation; Finally, the relationship between the piece and the building and the piece and the user is studied, and compared with the results obtained with the study of the same space without the artistic pie ce in order to verify the effect that Caldera' s work generates on the configuration of the architectural space.

Abstract: Objectives To report the baseline results of a longitudinal psychosocial study that forms part of the IMPACT study, a multi-national investigation of targeted prostate cancer (PCa) screening among men with a known pathogenic germline mutation in the BRCA1 or BRCA2 genes. Particpants and Methods Men enrolled in the IMPACT study were invited to complete a questionnaire at collaborating sites prior to each annual screening visit. The questionnaire included sociodemographic characteristics and the following measures: the Hospital Anxiety and Depression Scale (HADS), Impact of Event Scale (IES), 36-item short-form health survey (SF-36), Memorial Anxiety Scale for Prostate Cancer, Cancer Worry Scale-Revised, risk perception and knowledge. The results of the baseline questionnaire are presented. Results A total of 432 men completed questionnaires: 98 and 160 had mutations in BRCA1 and BRCA2 genes, respectively, and 174 were controls (familial mutation negative). Participants' perception of PCa risk was influenced by genetic status. Knowledge levels were high and unrelated to genetic status. Mean scores for the HADS and SF-36 were within reported general population norms and mean IES scores were within normal range. IES mean intrusion and avoidance scores were significantly higher in BRCA1/BRCA2 carriers than in controls and were higher in men with increased PCa risk perception. At the multivariate level, risk perception contributed more significantly to variance in IES scores than genetic status. Conclusion This is the first study to report the psychosocial profile of men with BRCA1/BRCA2 mutations undergoing PCa screening. No clinically concerning levels of general or cancer-specific distress or poor quality of life were detected in the cohort as a whole. Abstract: Objectives To report the baseline results of a longitudinal psychosocial study that forms part of the IMPACT study, a multi-national investigation of targeted prostate cancer (PCa) screening among men with a known pathogenic germline mutation in the BRCA1 or BRCA2 genes. Particpants and Methods Men enrolled in the IMPACT study were invited to complete a questionnaire at collaborating sites prior to each annual screening visit. The questionnaire included sociodemographic characteristics and the following measures: the Hospital Anxiety and Depression Scale (HADS), Impact of Event Scale (IES), 36-item short-form health survey (SF-36), Memorial Anxiety Scale for Prostate Cancer, Cancer Worry Scale-Revised, risk perception and knowledge. The results of the baseline questionnaire are presented. Results A total of 432 men completed questionnaires: 98 and 160 had mutations in BRCA1 and BRCA2 genes, respectively, and 174 were controls (familial mutation negative). Participants' perception of PCa risk was influenced by genetic status. Knowledge levels were high and unrelated to genetic status. Mean scores for the HADS and SF-36 were within reported general population norms and mean IES scores were within normal range. IES mean intrusion and avoidance scores were significantly higher in BRCA1/BRCA2 carriers than in controls and were higher in men with increased PCa risk perception. At the multivariate level, risk perception contributed more significantly to variance in IES scores than genetic status. Conclusion This is the first study to report the psychosocial profile of men with BRCA1/BRCA2 mutations undergoing PCa screening. No clinically concerning levels of general or cancer-specific distress or poor quality of life were detected in the cohort as a whole. 25. Dimensional constructs and methods for their treatment in psychiatry research

Abstract: Psychiatry has lagged behind other fields within the cognitive sciences regarding the analysis of dimensional constructs. This has surely improved recently by diagnostic classifications’ organisation of communication around mental disorders, however it has not kept up with developments in psychology in recent decades. Statistical methods developed for the study of intelligence could have been incorporated in psychiatry, as they may improve the field by permitting objective evaluation of dimensional constructs. For several reasons, this path has not been taken. This project aims to ameliorate this issue, by trying to do just that. It first applies a familiar method in psychology called structural equation modelling (SEM) to specify latent variables in data sets with information from subjects suffering from mental disorders. Genetic and neuroimaging (fractional anisotropy and volumetric segmentation) data is used and incorporated into structural equation models. Secondly, the project analyses what this approach can offer psychiatry, and what limitations of these techniques one should expect. It was found that these methods may help in specifying relations between psychometric scores and neurological features that are not reductive, or at least are not radically so. These methods are more parsimonious when comparing data from neuroimaging and data from cognitive abilities. When variables representing cognitive scores are involved in multiple comparisons, they may overestimate significant correlations, because of the shared variability that these types of cognitive variables present. SEM helps to improve that. Furthermore, factor analysis may be a good method of defining latent constructs that should fit appropriately with new proposals for classification like the recent Research Domain Criteria. Finally, psychiatry may benefit from a picture of science that accepts modelling as a viable approach for describing and understanding psychiatric conditions.

Abstract: This paper investigates practice dynamics in kitchens situated at the boundary between markets and consumption. The kitchen is conceptualized as a market-consumption junction, a space where multiple concerned actors in markets and consumption come to shape, and get shaped by, the practices in the kitchen. Drawing upon archival research of the Swedish household magazine Husmodern (1938–1958), this study traces two matters of concern in and around the kitchen: the scarcity of resources in food markets and the lack of time to prepare food for consumption. Findings reveal how thrifty and convenient practices became enacted, and their transformative implications for consumption, demand, and market action. The mechanisms involved in disrupting and reconnecting the dynamic elements of practices (meaning, competence, and objects) are explained through the notions of concerning, agencing, and overflows, which recursively work to redraw the boundaries between markets and consumption to establish novel practices

Instead, it uses the notion of the product biography to argue that products are chronically unstable, both physically and institutionally, and focusses on the managerial and institutional effort required to temporarily stabilise and qualify products for exchange or service value-creation. The context of the circular economy, which presents particularly acute challenges of qualification, is used to stimulate insights into how the product biography approach can inform the servitization debate. In particular, the circular economy perspective emphasises the need to see products as qualified by and constitutive of a distributed network, rather than defined once and for all by their producer, and points to entrepreneurial opportunity in the moments of transition between singularised, unique specimens and general, commodified, manageable objects – and vice versa. The wider and multiple product biographies occasioned by the circular economy also lead to reconfiguration of networks, as new potential valuations give rise to new entrepreneurial spaces.

Abstract: This paper investigates the relationships between states and markets, through an explication of the role of public policy in introducing quasi-markets in the health sector in England. In an extended qualitative study, we followed the development and implementation of the Health and Social Care Act 2012. The paper focuses on the controversial distinction between providers and purchasers through the introduction of a new type of actor, clinical commissioning groups. First, our analysis revealed how the State performs a strong interventionist role by regulating and de-regulating economic activity, promoting the expansion of markets into novel domains and changing competition rules of health services provision. In contrast with other scholars, we argue that states and markets are not mutually exclusive alternatives. Rather they are entangled through the duality of the state’s involvement in marketization processes and practice, and as regulator. Second, our analysis suggests that transformational quasi-market reforms are failing because of how a programme of intervention was conceived; the policy instruments they deploy were used and reinterpreted in practice, and because of a more nuanced form of performativity of ‘quasi-markets’. Our findings show that whereas the notion of quasi-markets sees no apparent clash between classic economic notions such as choice and competition, and public service ideals such as universal accessibility and uniform quality of care, others disagree. In practice, turning GPs into hybrid subjects equipped to balance objectives embodying a set of disparate and often conflicting values (e.g. value for money, improving clinical outcomes) has proved difficult. Similarly GPs have struggled in defining and qualifying what is to be exchanged. The reforms that envisaged commissioning as market-like exchanges, more closely resemble high-trust networks than markets.

31. Purchasing as market-shaping:the case of component-based software engineering

Abstract: The efforts to address markets as socio-technical orders have hitherto focused on the role of marketing in shaping demand. However, in many markets the role of purchasing is just as important. This paper uses a case study to examine how a single buying company can attempt to shape an emerging market through its purchasing practices. As a result, the study identifies five types of market-shaping actions. Within each action type, the market-shaping behavior of a buyer in an emerging market can be very diverse and include internal actions as well as actions aimed at influencing other market actors. While agency on the purchasing side is often associated with large size organizations, namely government and public sector agencies, our case study shows that agencies on the purchasing sides can be deployed in a variety of ways rather than merely through raw bargaining power. The findings indicate that buying firms do not simply adjust their own purchasing processes according to existing offerings, but actively attempt to drive market evolution in particular directions.

It is based on an ethnography within the account planning department of a London advertising agency as it worked closely with a client and a market research agency to develop a new energy/health drink for launch in the United Kingdom. The case provides detailed support for the idea that marketing (and other) theories help perform or bring into existence that which they purport to describe. It also shows how such theories are hybridised and streamlined in the iterative battleground of a creative process, framed by the need to create plausible consumption and competitive relations. The main contribution of this article is to show how the performative power of marketing knowledge is ultimately determined not by its ?truth value?, but by its ability to produce compelling stories that stabilise the reflexive relationships between product attributes, consumer profiles and competitive relations.

Abstract: Background: Men with germline breast cancer 1, early onset (BRCA1) or breast cancer 2, early onset (BRCA2) gene mutations have a higher risk of developing prostate cancer (PCa) than noncarriers. IMPACT (Identification of Men with a genetic predisposition to ProstAte Cancer: Targeted screening in BRCA1/2 mutation carriers and controls) is an international consortium of 62 centres in 20 countries evaluating the use of targeted PCa screening in men with BRCA1/2 mutations. Objective: To report the first year's screening results for all men at enrolment in the study. Design, setting and participants: We recruited men aged 40-69 yr with germline BRCA1/2 mutations and a control group of men who have tested negative for a pathogenic BRCA1 or BRCA2 mutation known to be present in their families. All men underwent prostate-specific antigen (PSA) testing at enrolment, and those men with PSA > 3 ng/ml were offered prostate biopsy. Outcome measurements and statistical analysis: PSA levels, PCa incidence, and tumour characteristics were evaluated. The Fisher exact test was used to compare the number of PCa cases among groups and the differences among disease types. Results and limitations: We recruited 2481 men (791 BRCA1 carriers, 531 BRCA1 controls; 731 BRCA2 carriers, 428 BRCA2 controls). A total of 199 men (8%) presented with PSA > 3.0 ng/ml, 162 biopsies were performed, and 59 PCas were diagnosed (18 BRCA1 carriers, 10 BRCA1 controls; 24 BRCA2 carriers, 7 BRCA2 controls); 66% of the tumours were classified as intermediate-or high-risk disease. The positive predictive value (PPV) for biopsy using a PSA threshold of 3.0 ng/ml in BRCA2 mutation carriers was 48%-double the PPV reported in population screening studies. A significant difference in detecting intermediate-or high-risk disease was observed in BRCA2 carriers. Abstract: Background: Men with germline breast cancer 1, early onset (BRCA1) or breast cancer 2, early onset (BRCA2) gene mutations have a higher risk of developing prostate cancer (PCa) than noncarriers. IMPACT (Identification of Men with a genetic predisposition to ProstAte Cancer: Targeted screening in BRCA1/2 mutation carriers and controls) is an international consortium of 62 centres in 20 countries evaluating the use of targeted PCa screening in men with BRCA1/2 mutations. Objective: To report the first year's screening results for all men at enrolment in the study. Design, setting and participants: We recruited men aged 40-69 yr with germline BRCA1/2 mutations and a control group of men who have tested negative for a pathogenic BRCA1 or BRCA2 mutation known to be present in their families. All men underwent prostate-specific antigen (PSA) testing at enrolment, and those men with PSA > 3 ng/ml were offered prostate biopsy. Outcome measurements and statistical analysis: PSA levels, PCa incidence, and tumour characteristics were evaluated. The Fisher exact test was used to compare the number of PCa cases among groups and the differences among disease types. Results and limitations: We recruited 2481 men (791 BRCA1 carriers, 531 BRCA1 controls; 731 BRCA2 carriers, 428 BRCA2 controls). A total of 199 men (8%) presented with PSA > 3.0 ng/ml, 162 biopsies were performed, and 59 PCas were diagnosed (18 BRCA1 carriers, 10 BRCA1 controls; 24 BRCA2 carriers, 7 BRCA2 controls); 66% of the tumours were classified as intermediate-or high-risk disease. The positive predictive value (PPV) for biopsy using a PSA threshold of 3.0 ng/ml in BRCA2 mutation carriers was 48%-double the PPV reported in population screening studies. A significant difference in detecting intermediate-or high-risk disease was observed in BRCA2 carriers. Abstract: Purpose – The paper argues that indirect capabilities – the ability to access other organizations’ capabilities – are an important and neglected part of firm strategy in PCP (Procuring Complex Performance) settings, and that this is especially so if these settings are treated as genuinely complex, rather than merely complicated. Elements of indirect capabilities are identified. Design/methodology – This is a theoretical paper, drawing on complexity notions and Penrose’s analysis of endogenous innovation to drive a disequilibrium-oriented discussion of the capabilities required by firms in a PCP setting. Findings – Six inter-related elements of indirect capabilities are proposed and discussed: IT infrastructure; boundary management practices; contracting; interface artefacts; valuing others’ capabilities and relating direct to indirect capabilities. These are important in PCP settings and in other operations and supply settings characterised by complexity. Originality/value – This paper reconsiders the way complexity has been treated in the PCP literature, and develops an extended discussion of the notion of indirect capabilities. It potentially provides the basis for an operations and supply strategy more attuned to the demands of shifting inter-organizational networks.

Abstract: Background: Men with germline breast cancer 1, early onset (BRCA1) or breast cancer 2, early onset (BRCA2) gene mutations have a higher risk of developing prostate cancer (PCa) than noncarriers. IMPACT (Identification of Men with a genetic predisposition to ProstAte Cancer: Targeted screening in BRCA1/2 mutation carriers and controls) is an international consortium of 62 centres in 20 countries evaluating the use of targeted PCa screening in men with BRCA1/2 mutations. Objective: To report the first year's screening results for all men at enrolment in the study. Design, setting and participants: We recruited men aged 40-69 yr with germline BRCA1/2 mutations and a control group of men who have tested negative for a pathogenic BRCA1 or BRCA2 mutation known to be present in their families. All men underwent prostate-specific antigen (PSA) testing at enrolment, and those men with PSA > 3 ng/ml were offered prostate biopsy. Outcome measurements and statistical analysis: PSA levels, PCa incidence, and tumour characteristics were evaluated. The Fisher exact test was used to compare the number of PCa cases among groups and the differences among disease types. Results and limitations: We recruited 2481 men (791 BRCA1 carriers, 531 BRCA1 controls; 731 BRCA2 carriers, 428 BRCA2 controls). A total of 199 men (8%) presented with PSA > 3.0 ng/ml, 162 biopsies were performed, and 59 PCas were diagnosed (18 BRCA1 carriers, 10 BRCA1 controls; 24 BRCA2 carriers, 7 BRCA2 controls); 66% of the tumours were classified as intermediate-or high-risk disease. The positive predictive value (PPV) for biopsy using a PSA threshold of 3.0 ng/ml in BRCA2 mutation carriers was 48%-double the PPV reported in population screening studies. A significant difference in detecting intermediate-or high-risk disease was observed in BRCA2 carriers. 48. Provider involvement in business service definition:a typology

Abstract: This paper proposes a typology for provider roles in defining business services. The starting point of the study is the underlying rationale of much of the service purchasing literature – buyers have or can easily access the necessary know-how to procure business services. If this does not hold, the implication is that buying firms would shy away from buying complex services. An alternative perspective recognizes that purchasing business services requires its own set of sourcing capabilities and that such capabilities may be lacking. Indeed, buying firms may have limited know-how in terms of defining and articulating their requirements or not be fully aware of them in the first place. However, the buyer’s lack of sourcing capabilities need not be an injunction to internalize the service. In these circumstances, service providers often step in, help buying firms specify their needs and requirements and play a key role in defining what is procured and how. We build on this interactive view of service definition to undertake a comparative case analysis of four business service contracting situations arrayed along two dimensions – buyer perceived uncertainty and provider’s buyer-specific experience. We conclude that service providers play different roles in each case. These are classified as translating, re-engineering, developing and fine-tuning roles.

Abstract: This comment addresses the context within which markets have turned into purported panaceas to alleviate poverty and promote development. Expanding formal markets and rescuing the poor from the traps of informality is often seen as recipe to unlock the untapped energies at the base of the pyramid (BoP), turning individuals into budding entrepreneurs and aspiring consumers. But, so far we have failed to appreciate how the informal sector plays a key role in supporting the formal economy and providing a route for subsistence at the BoP.

50. Beyond the service factory: service innovation in manufacturing supply networks

Abstract: This paper is concerned with the nature of the connection between services and manufacturing in manufacturing-oriented supply networks. The existing literature on manufacturing shifting into service is reviewed and, although such moves are seen as a way to increased revenue and profit, there are concerns that firms do not understand how the capabilities that underpin manufacturing may be extended to enable effective service delivery. Inspired by Chase’s concept of the ‘service factory’, which sees the factory as a repository of knowledge and a platform for services, the paper applies Edith Penrose’s conception of services as emanating from firms’ resources to examine an advanced component manufacturing firm in the course of a number of service-oriented projects. This leads to a model of service development in manufacturing firms, consisting of a network trigger, an opportunity to change the ‘productive opportunity’, the ‘revelation’ of resources and Penrose-services, a reconfiguration of the network, leading to an expanded productive opportunity and hence a platform for marketing new service capabilities. In this sense a network is seen as an inter-connected set of productive opportunities. It also draws attention to the importance in theory and practice of understanding both the institutional and the ontological connection between service offerings and the products, factories, firms and networks with which they are associated.
