The underlying cause of anemia can be found starting with the establishment of the anemia etiology. In this thesis, we investigated the cost-effectiveness of an extensive laboratory panel to find the anemia etiology in general practice.

Background Many cities in developing countries experience an unplanned and rapid growth. Several studies have shown that the irregular urbanization and equipment of cities produce different health risks and uneven exposure to specific diseases. Consequently, health surveys within cities should be carried out at the micro-local scale and sampling methods should try to capture this urban diversity. Methods This article describes the methodology used to develop a multi-stage sampling protocol to select a population for a demographic survey that investigates health disparities in the medium-sized city of Bobo-Dioulasso, Burkina Faso. It is based on the characterization of Bobo-Dioulasso city typology by taking into account the city heterogeneity, as determined by analysis of the built environment and of the distribution of urban infrastructures, such as healthcare structures or even water fountains, by photo-interpretation of aerial photographs and satellite images. Principal component analysis and hierarchical ascendant classification were then used to generate the city typology. Results Five groups of spaces with specific profiles were identified according to a set of variables which could be considered as proxy indicators of health status. Within these five groups, four sub-spaces were randomly selected for the study. We were then able to survey 1045 households in all the selected sub-spaces. The pertinence of this approach is discussed regarding to classical sampling as random walk method for example. Conclusion This urban space typology allowed to select a population living in areas representative of the uneven urbanization process, and to characterize its health status in regards to several indicators (nutritional status, communicable and non-communicable diseases, and anaemia). Although this method should be validated and compared with more established methods, it appears as an alternative in developing countries where geographic and population data are scarce.

Scientifically, this study aimed to validate the potential of ragworm to alleviate anaemia in common sole and to identify the dietary requirements of common sole to alleviate this anaemia. At the same time it was aimed to explain part of the difference in growth between sole fed a commercial pellet and sole fed ragworm. Practically, this study aimed to develop a diet that achieves similar growth rates in sole as when fed ragworm. Sole fed commercial pellets developed nutritional anaemia. Feeding ragworm or mussel alleviates this nutritional anaemia. It is suggested that the ability of mussel or ragworm (meal) to alleviate anaemia and improve growth in sole can be explained by heme iron and high B12 levels. Yet, iron absorption in sole is high and independent of iron source. Still, heme increases the absorption of copper. The high absorption of iron and copper in sole fed heme does not affect the haematocrit (Hct) and haemoglobin (Hb) levels, which indicates the anaemia in sole is not an iron or a copper deficiency anaemia. The Hct and Hb levels in sole are affected by dietary B12. Yet, the applied levels are unable to alleviate the anaemia in sole induced by feeding commercial pellets. More options to alleviate the nutritional anaemia in common sole are discussed. Nutrients as vitamin C, B1, B2, B5 and a possible role of dietary EPO are discussed. It is suggested that the slow growth of pellet-fed sole might be due to the low Hct and Hb levels, which hampers the uptake of oxygen, and thus also the overall metabolic capacity, including the scope for growth. Discussed is a 7°C difference in the “optimal” temperature between sole fed ragworm and the 2nd generation pellet and that the “worm effect” is dependent on temperature. However, the growth rate of sole fed the 3rd generation pellet at 18.4°C was comparable to the growth rate of sole fed ragworm, which could not have been the consequence of increasing Hct and Hb levels as these were comparable to levels found in s

Naar schatting 90% van de zeugenhouders geeft pasgeboren biggen een standaardtoediening van ijzer om bloedarmoede te voorkomen. IJzersupplementatie behoort zodanig tot de routine dat ze zelden ter discussie wordt gesteld. Maar voldoet die standaardtoediening wel aan de behoefte van de big? Geven we niet te veel? Of net te weinig? Praktijkonderzoek leverde interessante resultaten op.

Anemie is een belangrijke co-morbiditeit bij patienten met cardiovasculaire ziekte. In het eerste deel van het huidige proefschrift onderzoeken we oorzakelijke factoren bij het onstaan van anemie met hart en vaatziekten. We laten zien dat een verhoogde centraal veneuze druk geassocieerd is met het optreden van anemie en dat dit gepaard gaat met een verhoogde mortaliteit. We laten zien dat ontstekings cytokines een oorzaak kunnen zijn van anemie bij patienten met hartfalen en na cardiothoracale chirurgie. Ook laten we zien dat ACE remmers post operatief zorgen voor vertraagd herstel van anemie. Erythropoetine is een hormoon dat zorgt voor bloedaanmaak, maar het heeft ook andere gunstige effecten zoals vaatnieuwvorming en verminderd celverval, waardoor het bescherming kan bieden aan het hart. In zijn proefschrift brengt Snijders de afweerreactie, die optreedt bij een buiten het ziekenhuis verworven longontsteking, nauwgezet in kaart. Hij zoomt verder in op de activiteit van het stollingsysteem en op de gewijzigde samenstelling van het bloed, zogenaamde hematologische veranderingen. In een gerandomiseerd onderzoek onder 213 patiënten met een buiten het ziekenhuis verworven longontsteking geeft de behandeling met corticosteroïden naast antibiotica geen beter klinisch herstel, zo stelt Snijders vast. Wel kan toediening van corticosteroïden extra medicamenteuze behandeling noodzakelijk maken. Ook onderzocht Snijders hematologische veranderingen bij patiënten met een buiten het ziekenhuis verworven longontsteking. D-dimeren, afbraakproducten van stollingsactivatie, blijken niet geschikt als prognostische markers. Noch als zelfstandige marker, noch in combinatie met het vaak gebruikte CURB-65 risicomodel leiden zij tot een betere risico-inschatting. Snijders stelt vast dat hematologische veranderingen tijdens een longontsteking berusten op een verminderde inbouw van ijzer in rode bloedcellen. Dit lijkt in belangrijke mate gereguleerd te worden door het eiwit hepcidine. Een verminderd zuurstofgehalte in de hersenen van te vroeg geboren baby’s kan hersenschade tot gevolg hebben. UMCG-promovendus Elise Verhagen onderzocht het zuurstofgehalte in de hersenen van te vroeg geboren baby’s en de precieze gevolgen ervan. De gebruikte onderzoeksmethode, near-infrared spectroscopy (NIRS), lijkt een bruikbare methode te zijn om een verstoorde zuurstofvoorziening in de hersenen van te vroeg geboren baby’s vast te stellen. Verhagen vond nieuwe aanwijzingen dat roken tijdens de zwangerschap slecht is voor de zuurstofvoorziening van de hersenen van de pasgeboren baby. Daarmee biedt het onderzoek een nieuwe, extra reden om vrouwen dringend te adviseren te stoppen met roken voor zij zwanger raken, of in het geval direct te stoppen met roken zodra ze weten dat ze zwanger zijn. Roken tijdens de zwangerschap leidt tot een hogere zuurstofextractie in de hersenen bij te vroeg geboren baby’s in de eerste dagen na de geboorte, zo stelt Verhagen vast. Een hogere zuurstofextractie kan een aanwijzing zijn van een te lage bloeddoorstroming van de hersenen. De promovenda concludeert tevens dat het zuurstofgehalte in de hersenen tijdens de eerste dagen na de geboorte bij te vroeg geboren baby’s is geassocieerd met de neurologische ontwikkeling op 2- tot 3-jarige leeftijd. Relatief hoge en lage waarden zijn gerelateerd aan een slechtere cognitie. Een hoge zuurstofextractie is gerelateerd aan een slechtere motorische uitkomst.

Background Adding iron rich foods such as amaranth grain flour or micronutrient powders (MNP) containing low doses of highly bioavailable iron (e.g. NaFeEDTA) could be options to control iron deficiency (ID) in pre-school children. However, data evaluating the impact of such food-to-food or in-home fortification strategies is limited. The aim of this thesis was to investigate the effect of consumption of amaranth grain flour on anaemia and iron status of Kenyan pre-school children. Methods First a nutrition situation was assessed through a food consumption and nutrition study. We then simulated the potential effect of fortifying maize porridge with amaranth grain flour or MNP on prevalence of inadequate iron intake and ID. Next, an acceptability study was conducted. Factors that would predict the intention of consumption of amaranth porridge among children were identified and the sensory acceptability of amaranth porridge evaluated. Finally, a controlled, randomized 16-weeks intervention trial was conducted (n=279; aged 12-59 months) to determine the efficacy of maize porridge enriched with amaranth grain flour or MNP in form of NaFeEDTA. Primary outcome was anaemia and iron status with treatment effects estimated relative to control group receiving maize porridge only. Results The nutrition situation assessment showed that there were deficits in dietary energy, iron and zinc with early introduction of complementary foods. The simulation estimates resulted in a significant increase (P<0.005) in ferritin concentration in amaranth (1.82µg/L: CI 1.42-2.34) and MNP (1.80µg/L: CI 1.40-2.31) group compared to maize porridge group decreasing the prevalence ID to 27% in both groups. In the acceptability study, knowledge and health value significantly predicted health behaviour identity. None of the models significantly predicted intention. Práce se zabývala měřením hematologického parametru hemoglobinu v retikulocytech (CHr) na analyzátoru ADVIA 120 firmy Siemens. Předmětem práce bylo ověřit referenční interval CHr uvedený v literatuře, preciznost měření CHr jako základní charakteristiky pro verifikaci metody a odhadnout diagnostické charakteristiky (senzitivitu, specificitu, pozitivní a negativní prediktivní hodnotu) včetně diagnostické správnosti ve skupině pacientů na základě srovnání s klasickými diagnostickými kritérii anemie z nedostatku železa. Námi zjištěné výsledky potvrzují vhodnost výběru parametru CHr pro diagnózu různých typů anemií z nedostatku železa. Zvyšující se dostupnost stanovení CHr v běžné laboratoři sehraje pozitivní roli jak při diagnostice, tak i při léčbě anemií z nedostatku železa., The thesis deals with measuring the haematology parameter of the reticulocyte hemoglobin content (CHr) using ADVIA 120 analyzer made by Siemens. The aim of the thesis was to verify the CHr reference interval given in the literature and accuracy of measuring CHr as principal characteristics for verification of the method and to estimate diagnostic characteristics (sensitivity, specificity, positive and negative predictive value) including diagnostic accuracy in a group of patients based on comparison with classical diagnostic criteria of anaemia caused by iron deficiency. Results found here confirm the fact that CHr parameter for diagnosing various types of anaemia caused by iron deficiency was chosen appropriately. This thesis aimed at investigating the role of genetic and nutritional factors that affect the immune response to malaria in Tanzanian children. The introductory chapter (Chapter 1) reviews the importance of nutritional deficiencies, particularly of zinc, and presents the hypothesis that such deficiencies lead to impaired immunity and contribute to the burden of malaria. The chapter also describes current efforts to prevent malaria through intermittent preventive treatment, both in infants (IPTi) and pregnant women (IPTp). Sulfadoxinepyrimethamine is still used for first-line treatment of uncomplicated malaria, or, in many countries, to prevent malaria and anaemia in pregnancy. In malaria endemic areas, development of resistance to previously valuable antimalarial drugs has been continuously reported for decades. Thus our initial longitudinal study aimed at measuring the prevalence of resistance-associated mutations on dihydrofolate reductase (DHFR) and dihydropteroate synthase (DHPS) genes (dhfr and dhps) that confer parasite resistance to sulphadoxinepyrimethamine (SP) that was used as an interim antimalarial drug after chloroquine resistance. Although SP resistance-associated point mutations were highly prevalent, we observed an adequate parasite response to SP (Chapter 2). We speculated that the impact of the dhfr and dhps mutations on SP resistance may be dependent at least in part on the protective immunity that has developed in response to frequent exposure to infection and may be weighed down by host immunity in endemic areas and thus impacts in the continued use of the drug for treatment of malaria. The impact of other drugs with similar mechanisms of action used as antibiotics in selecting mutations responsible for SP resistance needs therefore to be studied for their modulating activity of the immune response. Cet article rapporte deux observations cliniques récentes chez des patients âgés, hospitalisés dans un contexte d’altération de l’état général et neurologique. Le diagnostic de macroadénome hypophysaire non fonctionnel a pu être posé sur base de dosages hormonaux orientés et d’une IRM centrée sur la région sellaire. Nous avons voulu mettre en évidence la présentation assez atypique de cette pathologie chez la personne âgée, ainsi que l’amélioration clinique rapide sous traitement hormonal substitutif.

The present thesis aimed to determine the efficacy of a school-based food fortification program to improve hemoglobin concentrations and iron stores of intestinal parasites-prone school children. Furthermore this thesis also compares the effect of iron fortification and iron supplementation on the changes in hemoglobin and iron status.

In developing countries, adolescent girls often have anemia and micronutrient deficiencies. The objective of this thesis was to investigate the role of riboflavin or vitamin A as determinant of anemia and iron deficiency in Indonesian adolescent girls.

Fanconi's anemia is a rare autosomal recessive disease characterized by congenital abnormalities, a progressive pancytopenia and a predisposition to cancer. The diagnosis is based on an abnormal increase of spontaneous chromosome breakage, more specifically on a clear-cut increase of chromosome breakage in the presence of bifunctional alkylating agents. Eight complementation groups (A to H) have been defined, and the genes corresponding to four of these groups have been cloned (FANCA, FANCC, FANCF and FANCG). The function of the proteins encoded by the genes of Fanconi's anemia remains unknown. Numerous studies indicate that different cellular processes are probably involved, including DNA repair pathways, apoptosis, cell cycle regulation and oxygen metabolism. Nevertheless, the exact cellular and molecular mechanisms implicated in Fanconi's anemia remain a challenge for fundamental research. The treatment of Fanconi's anemia is also the subject of intense research, bearing principally upon bone marrow transplantation, which is successful in the case of HLA-identical sibling donors, and gene therapy, which is still at a preliminary stage on the clinical level.

Approximately three quarters of east African children <5 y of age suffer from anaemia, which is due, at least in part, to malaria and iron deficiency. In children in areas of seasonal malaria, the benefits of iron supplementation may not outweigh possible inherent risks of adverse effects caused by malaria. Intermittent administration of sulfadoxine-pyrimethamine (SP) might improve haemoglobin concentrations while allowing children to develop protective immunity against severe disease and subsequent death caused by malaria. With a view to contribute to the development of programmes for anaemia control in preschool children in Africa, the immediate objectives of this thesis were as follows: 1) to measure the efficacy in improving haemoglobin concentrations in children aged 2-36 mo of intermittent iron supplementation and intermittent administration of SP, either alone or when given in combination; 2) to develop and evaluate survey methods for rapid assessment at community level of the burden of anaemia and its risk factors; 3) to contribute to improved methods for diagnosis of anaemia, iron deficiency, and malaria; 4) to evaluate the role of impaired erythropoiesis in the pathogenesis of malarial anaemia.In randomised controlled trial (n=328) in anaemic, asymptomatic children aged 2-36 mo, the effect on change in haemoglobin concentration in the group receiving iron plus sulfadoxine-pyrimethamine relative to the placebo group, adjusted for prognostic factors at baseline, was 12.5 g/L (95% CI: 8.5 to 16.4 g/L). In the former group, the estimated prevalence of anaemia reduced from 100% at baseline to 36% at 12 w, and prevalence of iron deficiency reduced correspondingly from 66% to 8%. Administration of SP in addition to iron supplementation gave no haemoglobin response. Survival analysis indicated no evidence of increased risk of malaria following iron supplementation. We report the case of a 70-year old woman with primary biliary cirrhosis, CREST syndrome and vascular gastric lesions corresponding to watermelon stomach. Oestrogen-progesterone treatment successfully controlled recurrent blood loss.

Nutritional anaemia affects 50-70% of pregnant women in the developing world where vitamin A deficiency is also a problem. Since previous studies have indicated that vitamin A deficiency can be involved in the aetiology of nutritional anaemia, the role of vitamin A deficiency in nutritional anaemia in pregnant women has been investigated in West Java, Indonesia. Of the 318 normal pregnant women examined in a cross-sectional study, 49% were anaemic and, according to multiple criteria, 43% had iron-deficiency anaemia, 22% had iron-deficient erythropoiesis, and 7% were iron depleted. Based on serum retinol values, 2.5% of the women were vitamin A deficient and 31 % had marginal vitamin A status. After adjustment for gestational stage, parity and subdistrict, serum retinol concentrations were significantly positively associated ( p <0.01) with haemoglobin concentrations, haematocrit, and serum iron concentrations. In order to test whether the anaemia observed would respond to improvement in vitamin A supply, a randomised, double-masked, placebo-controlled field trial was carried out in which anaemic pregnant women in the same area were supplemented with vitamin A and iron. The women (n=251), aged 17-35 years, parity 0-4, gestation 16-24 weeks, and haemoglobin between 80 and 109 g/L were randomly allocated to four groups: vitamin A (2.4 mg retinol) and placebo iron tablets; iron (60 mg elemental iron) and placebo vitamin A; vitamin A and iron; or both placebos, all daily for 8 weeks. Maximum response in haemoglobin was achieved with both vitamin A and iron supplementation (12.78 g/L, 95% CI 10.86 to 14.70), with one-third of the response attributable to vitamin A (3.68 g/L, 2.03 to 5.33) and two-thirds to iron (7.71 g/L, 5.97 to 9.45). Women of the reproductive age in China play a very important role in the labour force. Information on anaemia prevalence in this group is hardly available, notwithstanding the fact that iron deficiency anaemia is considered to be a major public health problem in China. Iron deficiency may cause adverse effects on physical performance. However, data available are fragmentary, inconsistent and the results are inconclusive. Furthermore, only a few studies so far have been carried out under free living conditions. The present study was done to resolve some of the uncertainties in this area.The general objectives of the present study were: 1) to investigate the prevalence of anaemia, its type and the contributing factors to iron deficiency in Chinese female workers; 2) to quantify the functional consequences of iron deficiency in both field and laboratory studies, with emphasis on mean heart rate at work, total energy expenditure at work, and production efficiency defined as the ratio of productivity to energy expended; 3) to identify the effect of iron deficiency per se on physical performance.In order to study these objectives, both a cross-sectional and an intervention study were designed. The analyses revealed the following results:Prevalence of iron deficiency anaemia was high in Chinese female menstruating cotton mill workers (34%).Oral iron supplementation was effective in the women with poor iron nutritional status.Physical performance, both on the job and in the laboratory tests, was improved with the improvement of iron status. The total energy expenditure at work was reduced and production efficiency significantly increased by 18% after iron supplementation.Marginal iron deficiency may not limit maximal physical work capacity of exercise, but it may cause impairment of prolonged submaximal physical performance.

Anemie door ijzergebrek is de meest voorkomende bloedziekte en in sommige gebieden een volkskwaal (de Vries, 1963). Kilpatriek (1961) vermeldt dat wel 25°/o van een groep ogenschijnlijk gezonde vrouwen tussen 15 en 34 jaar een anemie door ijzergebrek heeft. Bij vrouwen is de hoeveelheid ijzer die geresorbeerd wordt uit het voedsel, vaak niet voldoende om het ijzerverlies met de menstruatie, graviditeit en lactatie te compenseren. De mens krijgt normaliter het benodigde ijzer uithetvoedsel dat dagelijks ongeveer 15 mg ijzer behoort te bevatten. Hieruit wordt 1 tot 2 mg per dag geresorbeerd {Hayhoe, 1960). Dagelijks gaat 0,5 tot 1,0 mg ijzer verloren via faeces, urine en zweet (Bothwell en Finch, 1962). Is de voeding insufficient dan kan ook bij de man op de lange duur een anemie door ijzergebrek ontstaan (Rosenbaum en leonard, 1964). De normale mens bezit 4 gram ijzer, waarvan 60 à 70°/o in de hemoglobine, 20 à 35% in de hemosiderine of ferritine, 4% in de myoglobine en slechts 0,1 Ofo gebonden aan transferrine of in de cytochromen en het kata!ase. Aanvullende ijzertherapie dateert reeds uit de oudheid. De Grieken schreven ijzer voor om toestanden van zwakte te genezen en meenden aan de patient de kracht van het ijzer toe te dienen. Patienten met een zeer bleek uiterlijk kregen water te drinken, waarin men oude zwaarden had laten roesten. Celsus adviseerde een grote milt te behandelen met water waarin wit gloeiend ijzer was gedompeld. In pig farming the main reason for anaemia is iron deficiency. From the composition of sow's milk and the feed supplements eaten by the piglet it was deduced that deficiency of copper and vitamins of the B group does not occur under conditions in the Netherlands. Giving iron to the sow during pregnancy does not increase the piglet's liver storage of iron or its blood haemoglobin content at birth. Iron must therefore be given directly to the piglet either orally or parenterally. As injections can cause muscle damage the best method of iron intake for the piglet's physiology is orally. With the oral method the absorption rate of iron from the intestines is important. In experiments many iron compounds, both organic and inorganic, were given to piglets but the published results showed no agreement for blood values and weight gain.In our experiments we examined the influence of fumaric and succinic acid on the uptake of iron from the intestines and on the use of iron in the haemoproteides. These two organic acids were chosen because Brise & Hallberg (1962) found that more iron was recovered in the erythrocytes with iron succinate than with iron sulphate which is used in man; iron fumarate is widely used for pigs. Famaric and succinic acids are both intermediates in the Krebs cycle and components for certain iron-protein compounds. Our experiments were done with piglets weaned two days after birth and with rats weaned as soon as possible. Rats and piglets were then made anaemic with feeds low in iron (table 7 and 8).After the depletion period the effects of iron and the organic acids of piglets were studied in two 22 factorial experiments. Different functions have been attributed to natural regulatory CD4+CD25+FOXP+ (Treg) cells during malaria infection. Herein, we assessed the role for Treg cells during infections with lethal (DS) and non-lethal (DK) Plasmodium chabaudi adami parasites, comparing the levels of parasitemia, inflammation and anaemia. Independent of parasite virulence, the population of splenic Treg cells expanded during infection, and the absolute numbers of activated CD69+ Treg cells were higher in DS-infected mice. In vivo depletion of CD25+ T cells, which eliminated 80% of CD4+FOXP3+CD25+ T cells and 60–70% of CD4+FOXP3+ T cells, significantly decreased the number of CD69+ Treg cells in mice with lethal malaria. As a result, higher parasite burden and morbidity were measured in the latter, whereas the kinetics of infection with non-lethal parasites remained unaffected. In the absence of Treg cells, parasite-specific IFN-γ responses by CD4+ T cells increased significantly, both in mice with lethal and non-lethal infections, whereas IL-2 production was only stimulated in mice with non-lethal malaria. Following the depletion of CD25+ T cells, the production of IL-10 by CD90− cells was also enhanced in infected mice. Interestingly, a potent induction of TNF- and IFN-γ production by CD4+ and CD90− lymphocytes was measured in DS-infected mice, which also suffered severe anaemia earlier than non-depleted infected controls. Taken together, our data suggest that the expansion and activation of natural Treg cells represent a counter-regulatory response to the overwhelming inflammation associated with lethal P.c. adami. This response to infection involves TH1 lymphocytes as well as cells from the innate immune system.
