INTRODUCTION: First-Episode Psychosis (FEP) is a devastating mental health condition that commonly emerges during early adulthood, and is characterised by a disconnect in perceptions of reality. Current evidence suggests that inflammation and perturbed immune responses are involved in the pathology of FEP and may be associated specifically with negative symptoms. Exercise training is a potent anti-inflammatory stimulus that can reduce persistent inflammation, and can improve mood profiles in general populations. Therefore, exercise may represent a novel adjunct therapy for FEP. The aim of this study was to assess the effect of exercise on biomarkers of inflammation, negative symptoms of psychosis, and physiological health markers in FEP. METHODS: Seventeen young males (26.67 ± 6.64 years) were recruited from Birmingham Early Intervention in Psychosis Services and randomised to a 6-week exercise programme consisting of two-to-three sessions per week that targeted 60-70 % heart-rate max (HRMax), or a treatment as usual (TAU) condition. Immune T-helper (Th-) cell phenotypes and cytokines, symptom severity, functional wellbeing, and cognition were assessed before and after 6-weeks of regular exercise. RESULTS: Participants in the exercise group (n = 10) achieved 81.11 % attendance to the intervention, with an average exercise intensity of 67.54 % ± 7.75 % HRMax. This led to favourable changes in immune cell phenotypes, and a significant reduction in the Th1:Th2 ratio (-3.86 %) compared to the TAU group (p = 0.014). After the exercise intervention, there was also a significant reduction in plasma IL-6 concentration (-22.17 %) when compared to the TAU group (p = 0.006). IL-8, and IL-10 did not show statistically significant differences between the groups after exercise. AIM: Oxidative stress is strongly implicated in many psychiatric disorders, which has resulted in the development of new interventions to attempt to perturb this pathology. A great deal of attention has been paid to glutathione, which is the brain's dominant antioxidant and plays a fundamental role in removing free radicals and other reactive oxygen species. Measurement of glutathione concentration in the brain in vivo can provide information on redox status and potential for oxidative stress to develop. Glutathione might also represent a marker to assess treatment response. METHODS: This paper systematically reviews studies that assess glutathione concentration (measured using magnetic resonance spectroscopy) in various mental health conditions. RESULTS: There is limited evidence showing altered brain glutathione concentration in mental disorders; the best evidence suggests glutathione is decreased in depression, but is not altered in bipolar disorder. The review then outlines the various methodological options for acquiring glutathione data using spectroscopy. CONCLUSIONS: Analysis of the minimum effect size measurable in existing studies indicates that increased number of participants is required to measure subtle but possibly important differences and move the field forward.

This study investigated immunophenotypic differences in intracellular thiol redox state of peripheral blood mononuclear cells (PBMCs) isolated from trained (TR, n=9, mean {plus minus} SD: age 28 {plus minus} 5 years; BMI 23.2 {plus minus} 2.6 kg·m2; VO2max 56.9 {plus minus} 6.1 ml·kg-1·min-1) and recreationally active (RA, n=11, mean {plus minus} SD: age 27 {plus minus} 6 years; BMI 24.2 {plus minus} 3.7 kg·m2; VO2max 45.1 {plus minus} 6.4 ml·kg-1·min-1) participants before and after a maximal aerobic exercise tolerance test. Blood samples were taken before (PRE), during (sample acquired at 70% HRmax), immediately (POST+0) and 15 minutes post-exercise (POST+15). PBMCs were isolated and reduced thiol analysis (fluorescein-5 maleimide (F5M)) by immunophenotype (CD3+, CD4+ and CD8+) was performed using flow cytometry. A significant increase in cellular F5M fluorescence was observed in CD3+ T-cells at POST+0, with changes driven to a greater extent by CD8+ T-cells (fold change in both groups CD4: +2.3, CD8: +2.8; p<0.05). Further analysis revealed a population of highly reduced CD8+ T-cells (CD8+T-reduced+) that significantly increased from PRE to POST+0 in RA participants only (RA: +272 cell/µL, p<0.05). To further understand these results, CD8+T-reduced+ and CD8+T-reduced- cells were analysed for immunophenotype in response to the same exercise protocol (n=6, mean {plus minus} SD: age 24 {plus minus} 5 years; BMI 25.7 {plus minus} 4.1 kg·m-2; VO2max 41.33 {plus minus} 7.63 ml·kg-1·min-1). CD8+T-reduced+ had significantly less lymphoid homing potential (CCR7) POST+0 compared to PRE. This study is the first to demonstrate that lymphocyte populations become more reductive in response to acute exercise.

This paper presents the findings of a technological adoption assessment of health monitoring devices for a dementia study. The work was motivated by the need to monitor physical activity interventions in a study cohort of dementia patients living with caregiver support in the community. The system requirements were for a discrete and unobtrusive solution with activity level (energy expenditure) and heart rate monitoring. In addition to fulfilling system requirements, successful technology adoption requires careful consideration of practical challenges in deployment. The paper addresses these challenges - in particular, aspects relating to the servicing and maintenance of units over the study period and the access and synchronisation of data. Test data visualisations and data mining results for sustained, long-term data capture are provided.

HUVECs are shown to export Hcy at a rate determined by the flux through the methionine/Hcy pathway. Additional methionine increases intracellular methionine, decreases intracellular folate, and increases Hcy export, whereas additional folate inhibits export. An inverse relationship exists between intracellular folate and Hcy export. Hcy export may be regulated by intracellular S-adenosyl methionine rather than by Hcy. Human LDLs exposed to HUVECs exporting Hcy undergo time-related lipid oxidation, a process inhibited by the thiol trap dithionitrobenzoate. This is likely to be related to the generation of hydroxyl radicals, which we show are associated with Hcy export. Although Hcy is the major oxidant, cysteine also contributes, as shown by the effect of glutamate. Finally, the LDL oxidized in this system showed a time-dependent increase in uptake by human macrophages, implying an upregulation of the scavenger receptor. These results suggest that continuous export of Hcy from endothelial cells contributes to the generation of extracellular hydroxyl radicals, with associated oxidative modification of LDL and incorporation into macrophages, a key step in atherosclerosis.
