Abstract: Dendritic cells (DCs) play a pivotal role in immune surveillance, acting as sentinels that coordinate immune responses within tissues. Although differences in the identity and functional states of DC subpopulations have been identified through multiparametric flow cytometry and single-cell RNA sequencing, these methods do not provide information about the spatial context in which the cells are located. This knowledge is crucial for understanding tissue organisation and cellular cross-talk. Recent developments in multiplex imaging techniques can now offer insights into this complex spatial and functional landscape. This review provides a concise overview of these imaging methodologies, emphasising their application in identifying DCs to delineate their tissue-specific functions and aiding newcomers in navigating this field.

Abstract: Morgana is a ubiquitous HSP90 co-chaperone protein coded by the CHORDC1 gene. Morgana heterozygous mice develop with age a myeloid malignancy resembling human atypical myeloid leukemia (aCML), now renamed MDS/MPN with neutrophilia. Patients affected by this pathology exhibit low Morgana levels in the bone marrow (BM), suggesting that Morgana downregulation plays a causative role in the human malignancy. A decrease in Morgana expression levels is also evident in the BM of a subgroup of Philadelphia-positive (Ph+) chronic myeloid leukemia (CML) patients showing resistance or an incomplete response to imatinib. Despite the relevance of these data, the mechanism through which Morgana expression is downregulated in patients’ bone marrow remains unclear. In this study, we investigated the possibility that Morgana expression is regulated by miRNAs and we demonstrated that Morgana is under the control of four miRNAs (miR-15a/b and miR-26a/b) and that miR-15a may account for Morgana downregulation in CML patients.

Abstract: Veillonella parvula, prototypical member of the oral and gut microbiota, is at times commensal yet also potentially pathogenic. The definition of the molecular basis tailoring this contrasting behavior is key for broadening our understanding of the microbiota-driven pathogenic and/or tolerogenic mechanisms that take place within our body. In this study, we focused on the chemistry of the main constituent of the outer membrane of V. parvula, the lipopolysaccharide (LPS). LPS molecules indeed elicit pro-inflammatory and immunomodulatory responses depending on their chemical structures. Herein we report the structural elucidation of the LPS from two strains of V. parvula and show important and unprecedented differences in both the lipid and carbohydrate moieties, including the identification of a novel galactofuranose and mannitol-containing O-antigen repeating unit for one of the two strains. Furthermore, by harnessing computational studies, in vitro human cell models, as well as lectin binding solid-phase assays, we discovered that the two chemically diverse LPS immunologically behave differently and have attempted to identify the molecular determinant(s) governing this phenomenon. Whereas pro-inflammatory potential has been evidenced for the lipid A moiety, by contrast a plausible "immune modulating" action has been proposed for the peculiar O-antigen portion.

Abstract: This article describes how the transcriptional alterations of the innate immune system divide dysplasias into aggressive forms that, despite the treatment, relapse quickly and more easily, and others where the progression is slow and more treatable. It elaborates on how the immune system can change the extracellular matrix, favoring neoplastic progression, and how infections can enhance disease progression by increasing epithelial damage due to the loss of surface immunoglobulin and amplifying the inflammatory response. We investigated whether these dysregulated genes were linked to disease progression, delay, or recovery. These transcriptional alterations were observed using the RNA-based next-generation sequencing (NGS) panel Oncomine Immune Response Research Assay (OIRRA) to measure the expression of genes associated with lymphocyte regulation, cytokine signaling, lymphocyte markers, and checkpoint pathways. During the analysis, it became apparent that certain alterations divide dysplasia into two categories: progressive or not. In the future, these biological alterations are the first step to provide new treatment modalities with different classes of drugs currently in use in a systemic or local approach, including classical chemotherapy drugs such as cisplatin and fluorouracile, older drugs like fenretinide, and new checkpoint inhibitor drugs such as nivolumab and pembrolizumab, as well as newer options like T cell therapy (CAR-T). Following these observed alterations, it is possible to differentiate which dysplasias progress or not or relapse quickly. This information could, in the future, be the basis for determining a close follow-up, minimizing surgical interventions, planning a correct and personalized treatment protocol for each patient and, after specific clinical trials, tailoring new drug treatments.

Abstract: Systemic vasculitides comprise a collection of rare and heterogeneous disorders capable of impacting any organ and system, posing a considerable burden of mortality and comorbidity. As with previous annual reviews of this series, this review will offer a critical overview of the latest literature on pathogenesis, biomarkers, and treatment options in both small- and large-vessel vasculitis.

Abstract: The workload of general surgeons is multifaceted, encompassing not only surgical procedures but also a myriad of other responsibilities. From April to May 2023, we conducted a CHERRIES-compliant internet-based survey analyzing clinical practice, academic engagement, and post-on-call rest. The questionnaire featured six sections with 35 questions. Statistical analysis used Chi-square tests, ANOVA, and logistic regression (SPSS (R) v. 28). The survey received a total of 1.046 responses (65.4%). Over 78.0% of responders came from Europe, 65.1% came from a general surgery unit; 92.8% of European and 87.5% of North American respondents were involved in research, compared to 71.7% in Africa. Europe led in publishing research studies (6.6 +/- 8.6 yearly). Teaching involvement was high in North America (100%) and Africa (91.7%). Surgeons reported an average of 6.7 +/- 4.9 on-call shifts per month, with European and North American surgeons experiencing 6.5 +/- 4.9 and 7.8 +/- 4.1 on-calls monthly, respectively. African surgeons had the highest on-call frequency (8.7 +/- 6.1). Post-on-call, only 35.1% of respondents received a day off. Europeans were most likely (40%) to have a day off, while African surgeons were least likely (6.7%). On the adjusted multivariable analysis HDI (Human Development Index) (aOR 1.993) hospital capacity > 400 beds (aOR 2.423), working in a specialty surgery unit (aOR 2.087), and making the on-call in-house (aOR 5.446), significantly predicted the likelihood of having a day off after an on-call shift. Our study revealed critical insights into the disparities in workload, access to research, and professional opportunities for surgeons across different continents, underscored by the HDI.

7. Transcranial magnetic stimulation on the right dorsal attention network modulates the center-surround profile of the attentional focus

Abstract: Psychophysical observations indicate that the spatial profile of visuospatial attention includes a central enhancement around the attentional focus, encircled by a narrow zone of reduced excitability in the immediate surround. This inhibitory ring optimally amplifies relevant target information, likely stemming from top-down frontoparietal recurrent activity modulating early visual cortex activations. However, the mechanisms through which neural suppression gives rise to the surrounding attenuation and any potential hemispheric specialization remain unclear. We used transcranial magnetic stimulation to evaluate the role of two regions of the dorsal attention network in the center-surround profile: the frontal eye field and the intraparietal sulcus. Participants performed a psychophysical task that mapped the entire spatial attentional profile, while transcranial magnetic stimulation was delivered either to intraparietal sulcus or frontal eye field on the right (Experiment 1) and left (Experiment 2) hemisphere. Results showed that stimulation of right frontal eye field and right intraparietal sulcus significantly changed the center-surround profile, by widening the inhibitory ring around the attentional focus. The stimulation on the left frontal eye field, but not left intraparietal sulcus, induced a general decrease in performance but did not alter the center-surround profile. Results point to a pivotal role of the right dorsal attention network in orchestrating inhibitory spatial mechanisms required to limit interference by surrounding distractors.

8. Using computer-generated faces in experimental psychology: The role of realism and exposure

Abstract: In psychology, researchers often rely on face photographs to study face perception. However, finding suitable face stimuli for experiments is often challenging. Computer-generated (CG) faces have emerged as a potential solution due to their flexibility and controllability. However, it has been suggested that these stimuli are evaluated and processed differently from real faces, and their suitability as alternatives in experimental settings remains unclear. To address this, two studies were conducted to examine the impact of CG faces' realism and observers’ self-reported exposure to CG faces on faces appraisals (Study 1) and processing (Study 2). In Study 1 (n = 97), we assessed perceptions of both real and CG faces. Findings indicated that participants generally viewed CG faces less favourably, especially when these faces lacked realism. This trend was particularly pronounced among individuals less exposed to digital characters. In Study 2 (n = 33), we examined the recognition accuracy of these faces in a memory task. The data revealed that CG faces, especially those less realistic, were less accurately recognized. However, this discrepancy was primarily observed among individuals with limited exposure to digital characters, while those more familiar with such characters showed no significant difference in recognition. Overall, this work confirmed that, to date, CG faces are not an adequate alternative to real faces and that researchers should be cautious when using these stimuli in experiments involving face processing. However, as digital graphics improve and as digital characters become more commonplace in daily life, the perceptual gap between CG and real faces may diminish.

Abstract: Background: Major depressive disorder (MDD) is prevalent and disabling among older adults. Standing on its tolerability profile, vortioxetine might be a promising alternative to selective serotonin reuptake inhibitors (SSRIs) in such a vulnerable population. Methods: We conducted a randomised, assessor- and statistician-blinded, superiority trial including older adults with MDD. The study was conducted between 02/02/2019 and 02/22/2023 in 11 Italian Psychiatric Services. Participants were randomised to vortioxetine or one of the SSRIs, selected according to common practice. Treatment discontinuation due to adverse events after six months was the primary outcome, for which we aimed to detect a 12% difference in favour of vortioxetine. The study was registered in the online repository clinicaltrials.gov (NCT03779789). Findings: The intention-to-treat population included 179 individuals randomised to vortioxetine and 178 to SSRIs. Mean age was 73.7 years (standard deviation 6.1), and 264 participants (69%) were female. Of those on vortioxetine, 78 (44%) discontinued the treatment due to adverse events at six months, compared to 59 (33%) of those on SSRIs (odds ratio 1.56; 95% confidence interval 1.01–2.39). Adjusted and per-protocol analyses confirmed point estimates in favour of SSRIs, but without a significant difference. With the exception of the unadjusted survival analysis showing SSRIs to outperform vortioxetine, secondary outcomes provided results consistent with a lack of substantial safety and tolerability differences between the two arms. Overall, no significant differences emerged in terms of response rates, depressive symptoms and quality of life, while SSRIs outperformed vortioxetine in terms of cognitive performance. Interpretation: As opposed to what was previously hypothesised, vortioxetine did not show a better tolerability profile compared to SSRIs in older adults with MDD in this study. Abstract: Veillonella parvula, prototypical member of the oral and gut microbiota, is at times commensal yet also potentially pathogenic. The definition of the molecular basis tailoring this contrasting behavior is key for broadening our understanding of the microbiota driven pathogenic and/or tolerogenic mechanisms that take place within our body. In this study, we focused on the chemistry of the main constituent of the outer membrane of V. parvula, the lipopolysaccharide (LPS). LPS molecules indeed elicit pro-inflammatory and immunomodulatory responses depending on their chemical structures. Here we report about the structural elucidation of the LPS from two strains of V. parvula and show important and unprecedented differences in both the lipid and carbohydrate moieties, including the identification of a novel galactofuranose and mannitol-containing O-antigen repeating unit for one of the two strains. Furthermore, by harnessing computational studies, in vitro human cell models, as well as lectin binding solid phase assays, we discovered that the two chemically diverse LPS immunologically behave differently and have attempted to identify the molecular determinant(s) governing this phenomenon. Whether a pro-inflammatory potential has been evidenced for the lipid A moiety, by contrast a plausible “immune modulating” action has been proposed for the peculiar O-antigen portion.

Abstract: Syn-rift extensional faults play a significant role during the early stage of rifting. Constraining the age of faulting, and how and when deformation shifts from the proximal to the distal margin areas, is crucial for the reconstruction of the rifting process. Previous assessments of the structural and temporal evolution of rift-related faults of the Adria proximal margin have primarily relied on indirect biostratigraphic evidence. Additionally, the majority of rift-related faults underwent tectonic inversion during the Alpine orogeny. In this study, in-situ U-Pb geochronology was applied on syn-kinematic calcites to unravel the activity of the Amora Fault, a remarkable example of a Jurassic growth fault unaffected by the Alpine orogeny. The obtained ages, spanning from Hettangian to Callovian, extend the AF activity beyond the previously established Early Jurassic time. This chrono-structural model has significant implications on the role of major extensional faults in focusing deformation throughout the rift system's evolution.

Abstract: Background: Malignant pheochromocytomas (PCCs) and paragangliomas (PGLs) are rare tumors and available systemic therapies are limited. Aim: To explore the role of peptide receptor radionuclide therapy (PRRT) with Yttrium-90 (90Y) and Lutetium-177 (177Lu) peptides in pheochromocytomas (PCCs) and paragangliomas (PGLs). Methods: We retrospectively analyzed more than 1500 patients with histologically proven neuroendocrine tumors treated with 177Lu- or 90Y-DOTA-TATE or –TOC between 1999 to 2017 at our Institute. Overall, 30 patients with confirmed malignant PCCs and PGLs matched inclusion/exclusion criteria and were considered eligible for this analysis. Results: Thirty (n = 30) patients were treated: 22 with PGLs and 8 with PCCs (12 M and 18 F, median age 47 [IQR: 35–60 years]). Eighteen patients (n = 18) had head and neck PGLs, 3 patients thoracic PGLs and 1 patient abdominal PGL. Sixteen patients (53%) had locally advanced and fourteen (47%) had metastatic disease. Twenty-seven (90%) patients had disease progression at baseline. Four (13%) patients were treated with 90Y, sixteen (53%) with 177Lu and ten (33%) with 90Y + 177Lu respectively. The median total cumulative activity from treatment with 90Y- alone was 9.45 GBq (range 5.11–14.02 GBq), from 177Lu- alone was 21.9 GBq (7.55–32.12 GBq) and from the combination treatment was 4.94 GBq from 90Y- and 6.83 GBq from 177Lu- (ranges 1.04–10.1 and 2.66–20.13 GBq, respectively). Seven out of 30 (23%) patients had partial response and 19 (63%) stable disease. Median follow up was 8.9 years (IQR: 2.9–12). The 5-y and 10-y PFS was 68% (95% CI: 48–82) and 53% (95% CI: 33–69), respectively, whereas 5-y and 10-y OS was 75% (95% CI: 54–87) and 59% (95% CI: 38–75), respectively. Grade 3 or 4 acute hematological toxicity occurred in three patients, two with leucopenia and one with thrombocytopenia, respectively. Conclusion: PRRT with 177Lu- or 90Y-DOTA-TATE or –TOC is feasible and well tolerated in advanced PGLs and PCCs.

Abstract: Direct oral anticoagulants (DOACs) have largely replaced vitamin K antagonists, mostly warfarin, for the main indications for oral anticoagulation, prevention and treatment of venous thromboembolism, and prevention of embolic stroke in atrial fibrillation. WhileDOACs offer practical, fixed-dose anticoagulation in many patients, specific restrictions or contraindications may apply. DOACs are not sufficiently effective in high-thrombotic risk conditions such as antiphospholipid syndrome and mechanical heart valves. Patients with cancer-associated thrombosis may benefit from DOACs, but the bleeding risk, particularly in those with gastrointestinal or urogenital tumors, must be carefully weighed. In patients with frailty, excess body weight, and/or moderate-to-severe chronic kidney disease,DOACs must be cautiously administered and may require laboratory monitoring. Reversal agents have been developed and approved for life-threatening bleeding. In addition, the clinical testing of potentially safer anticoagulants such as factor XI(a) inhibitors is important to further optimize anticoagulant therapy in an increasingly elderly and frail population worldwide.

Abstract: Background and aims: Thromboxane (TX) A2, released by activated platelets, plays an important role in atherothrombosis. Urinary 11-dehydro-TXB2 (U-TXM), a stable metabolite reflecting the whole-body TXA2 biosynthesis, is reduced by ∼70% by daily low-dose aspirin. The U-TXM represents a non-invasive biomarker of in vivo platelet activation and is enhanced in patients with diabetes. This study assessed whether U-TXM is associated with the risk of future serious vascular events or revascularizations (SVE-R), major bleeding, or cancer in patients with diabetes. Methods: The U-TXM was measured pre-randomization to aspirin or placebo in 5948 people with type 1 or 2 diabetes and no cardiovascular disease, in the ASCEND trial. Associations between log U-TXM and SVE-R (n = 618), major bleed (n = 206), and cancer (n = 700) during 6.6 years of follow-up were investigated by Cox regression; comparisons of these associations with the effects of randomization to aspirin were made. Results: Higher U-TXM was associated with older age, female sex, current smoking, type 2 diabetes, higher body size, urinary albumin/creatinine ratio of ≥3 mg/mmol, and higher estimated glomerular filtration rate. After adjustment for these, U-TXM was marginally statistically significantly associated with SVE-R and major bleed but not cancer [hazard ratios per 1 SD higher log U-TXM (95% confidence interval): 1.09 (1.00-1.18), 1.16 (1.01-1.34), and 1.06 (0.98-1.14)]. The hazard ratio was similar to that implied by the clinical effects of randomization to aspirin for SVE-R but not for major bleed. Conclusions: The U-TXM was log-linearly independently associated with SVE-R in diabetes. This is consistent with the involvement of platelet TXA2 in diabetic atherothrombosis.

Methods: A scoping review of empirical studies evaluating non-pharmacologic/non-surgical interventions promoting health and well-being among adult women with traumatic experiences linked to armed, torture, and/or forced migration, identified through systematic searches in February 2022 within five databases (AMED, CINAHL, Cochrane Library, PsycINFO, and PubMed). Methodological characteristics and results were extracted and analyzed with narrative analysis using tabulations, descriptive statistics, text-based summaries, and thematization. Results: Assessment of 16 748 records resulted in 13 included reports. The methodological approaches were quantitative (n = 9), qualitative (n = 2), and mixed methods (n = 2), with most reports being feasibility/pilot studies (n = 5) and/or randomized controlled trials (n = 4). The most common recruitment strategy was non-probability sampling (n = 8). Most interventions were conducted in North America (n = 4), Asia (n = 3) or Middle East (n = 3). Thirteen intra-intervention techniques and five categories of components utilized within the interventions were identified, the most common being skill building (n = 12). Ten developed the interventions through theoretical frameworks or manuals/therapy, while five developed interventions through public or stakeholder involvement. Abstract: The Hodrick-Prescott filter is a popular macroeconomics tool to decompose time series into a smooth trend and a business cycle components. The last few years have witnessed global events, such as the Global Financial Crisis, the COVID-19 pandemic, and the war in Ukraine, that have had abrupt structural impacts on many economic time series. At the same time, governments in Western countries have been arranging structural reforms to adapt their labor markets to socio-economic changes. Among the countries at the center of these processes stands Italy. Accordingly, it is reasonable to assume that the combination of structural reforms and socioeconomic shocks have generated structural changes in the number of employed people within the country. We then employ a robust version of the Hodrick-Prescott filter to identify structural breaks in the quarterly time series of employed people in Italy for different age groups from 1993 to 2023.

The purpose of this qualitative study is to identify key conceptual factors connected to the Chinese cultural context that luxury brands should consider in order to effectively advertise through social media in China. These factors are identified throughout the observation of selected cross-cultural theories. The aim is to understand the role of the cross-cultural theory in the effort to develop effective social media advertising strategies for luxury brands in China.

Abstract: Mystical tourism in Cusco is the subject of this research, aiming to understand its management and contribution to the diversification of the tourist offer. Under a quantitative approach, a non-experimental descriptive methodology was employed. The population included tourists who visited Cusco to participate in mystical tourism activities. A random sample of visitors was selected, encompassing various ages, genders, and nationalities. The technique used was structured surveys, administered both digitally and in print. The main instrument was a questionnaire with closed and open questions about the tourists' experiences, their satisfaction with the services received, and their perceptions of mystical tourism in the region. Data were collected over a decade (2013-2023). The results show sustained growth in mystical tourism in Cusco, despite the COVID-19 pandemic. High tourist satisfaction with the services received is highlighted, suggesting significant potential for sustainable sector development. Additionally, the research underscores the importance of improving the management and promotion of mystical tourism to meet the growing demand. This study provides significant data to better understand the phenomenon of mystical tourism in Cusco and offers a solid basis for future research and tourism development policies. In conclusion, mystical tourism in Cusco has not only shown resilience in the face of global challenges but has also demonstrated growth and satisfaction that can drive sustainable development and the diversification of the tourist offer in the region., El turismo místico en Cusco es objeto de esta investigación, con el objetivo de comprender su gestión y contribución a la diversificación de la oferta turística. Bajo un enfoque cuantitativo, se empleó una metodología descriptiva no experimental. La población incluyó a turistas que visitaron Cusco con el propósito de participar en actividades de turismo místico. Se seleccionó una muestra aleatoria de visitantes, abarcando diversas edades, géneros y nacionalidades. La técnica utilizada fue la encuesta estructurada, administrada tanto en formato digital como impreso. El instrumento principal fue un cuestionario con preguntas cerradas y abiertas sobre las experiencias de los turistas, su satisfacción con los servicios recibidos y sus percepciones sobre el turismo místico en la región. Los datos fueron recolectados a lo largo de una década (2013-2023). Los resultados muestran un crecimiento sostenido del turismo místico en Cusco, a pesar de la pandemia de COVID-19. Se destaca la alta satisfacción de los turistas con los servicios recibidos, lo que sugiere un potencial significativo para el desarrollo sostenible del sector. Además, la investigación resalta la importancia de mejorar la gestión y promoción del turismo místico para satisfacer la creciente demanda. Esta investigación aporta datos significativos para comprender mejor el fenómeno del turismo místico en Cusco y proporciona una base sólida para futuras investigaciones y políticas de desarrollo turístico. En conclusión, el turismo místico en Cusco no solo ha mostrado resiliencia ante desafíos globales, sino que también ha evidenciado un crecimiento y satisfacción que pueden impulsar el desarrollo sostenible y la diversificación de la oferta turística en la región.

Abstract: Introduction: The severity of COVID-19 infection in kidney transplant patients has been well-documented. Objectives: This study aims to determine the epidemiological and clinical data and identify predictors of poor prognosis during the epidemic’s early stages. Material and methods: This is a national semi-prospective, multicenter study of subjects with functioning grafts who were infected during the first wave of the pandemic in Chile between March 1 and September 31, 2020. Results: during this period, we recorded the hospitalization of 97 adult patients throughout the entire national territory. The average age was 52.5 years, 62% men, 45% hypertensive, 11% coronary, 10% diabetic, and 5% with chronic obstructive pulmonary disease, with an average post-transplant follow-up of 7.2 years and an average previous renal function of 47.7ml/min/1.7m2 (CKD-EPI formula). Between the onset of symptoms and diagnosis, there was an average period of 4.8 days, with a predominance of cough (44%), dyspnea (42%), and fever (42%). 34% developed acute kidney injury, and 36% of them required dialysis support. The lethality was 30%, prevailing in those with multiple organ failure (80%) and those who required invasive mechanical ventilation (52%). In the multivariate analysis, the best predictors of mortality were older age (OR: 2.92) and living in a low-income commune (OR: 2.35). Conclusions: This national project of the Chilean Society of Nephrology provided valuable information for monitoring the epidemiological evolution of the pandemic. It also helped to propose priority vaccination strategies, adjust immunosuppressive therapy, and design logistical aspects to reduce the risks for transplant patients., Introducción: La evolución de la infección por COVID-19 se ha demostrado grave en los pacientes trasplantados de riñón. Objetivos: Conocer datos epidemiológicos, clínicos y determinar predictores de mal pronóstico en los albores de la epidemia. Material y Métodos: estudio nacional semiprospectivo, multicéntrico, en sujetos con injerto funcionante, infectados durante la primera ola de la pandemia, entre el 1 de marzo y el 31 de septiembre de 2020 en Chile. Resultados: registramos la hospitalización de 97 adultos con injerto funcionante a través de todo el territorio nacional con edad promedio 52.5 años, 62% hombres, 45% hipertensos, 11% coronarios, 10% diabéticos, y 5% con enfermedad pulmonar obstructiva crónica, con un seguimiento post-trasplante promedio de 7.2 años y función renal previa promedio de 47.7 ml/min/1.7m2 (fórmula CKD-EPI). Entre el inicio de los síntomas y el diagnóstico hubo un periodo promedio de 4.8 días, predominando la tos (44%), disnea (42%) y fiebre (42%). Un 34% desarrolló injuria renal aguda y de ellos un 36% requirió soporte dialítico. La letalidad fue de un 30%, predominando en aquellos con falla orgánica múltiple 80% y quienes requirieron ventilación mecánica invasiva (52%). En el análisis multivariado, los mejores predictores de mortalidad fueron la mayor edad (OR: 2,92) y vivir en una comuna de bajos ingresos (OR: 2,35). Conclusiones: la información generada por este proyecto de la Sociedad Chilena de Nefrología permitió monitorizar la evolución inicial de la pandemia y proponer estrategias de vacunación prioritarias, ajustes de inmunosupresión y diseñar aspectos logísticos para reducir los riesgos de nuestros pacientes trasplantados.

Abstract: This is the abstract of the Italian-language review of the volume Un confronto mancato: Walter Benjamin e Antonio Gramsci. The book (Macerata, Quodilbet, 2023) publishes the proceedings of a conference in Rome on the two Marxists held in Autumn 2022 which continues explicitly the earlier Vienna conference, the contributions to which are collected together in “International Gramsci Journal” 3(4), 2020. The proceedings fall into four sections. The first one deals with the philosophy of history and historical materialism, as elaborated by Gramsci in his “philosophy of praxis”; despite different starting points and apparently different assessments of historicism, there turns out in the end to be a convergence based on an anti-determinism. The second part focuses on revolution, counter-revolution and passive revolution, taking in the questions of the political subject and contemporary situations. Subjectivity is then the theme of the third section, as forms of life appropriate to the capitalist mode of production and as regards subjects attempting to emancipate themselves from this mode. The fourth section includes the two thinker-revolutionaries’ approaches to the question of the various types of intellectual, including their conceptions of the artistic vanguards, folklore and kitsch, and the translation of experience from one country to another.

Abstract: Aims: Heart failure (HF), a global pandemic affecting millions of individuals, calls for adequate predictive guidance for improved therapy. Congestion, a key factor in HF-related hospitalizations, further underscores the need for timely interventions. Proactive monitoring of intracardiac pressures, guided by pulmonary artery (PA) pressure, offers opportunities for efficient early-stage intervention, since haemodynamic congestion precedes clinical symptoms. Methods: The BioMEMS study, a substudy of the MONITOR-HF trial, proposes a multifaceted approach integrating blood biobank data with traditional and novel HF parameters. Two additional blood samples from 340 active participants in the MONITOR-HF trial were collected at baseline, 3-, 6-, and 12-month visits and stored for the BioMEMS biobank. The main aims are to identify the relationship between temporal biomarker patterns and PA pressures derived from the CardioMEMS-HF system, and to identify the biomarker profile(s) associated with the risk of HF events and cardiovascular death. Conclusion: Since the prognostic value of single baseline measurements of biomarkers like N-terminal pro-B-type natriuretic peptide is limited, with the BioMEMS study we advocate a dynamic, serial approach to better capture HF progression. We will substantiate this by relating repeated biomarker measurements to PA pressures. This design rationale presents a comprehensive review on cardiac biomarkers in HF, and aims to contribute valuable insights into personalized HF therapy and patient risk assessment, advancing our ability to address the evolving nature of HF effectively.

Abstract: Aims: Despite clear guideline recommendations for initiating four drug classes in all patients with heart failure (HF) with reduced ejection fraction (HFrEF) and the availability of rapid titration schemes, information on real-world implementation lags behind. Closely following the 2021 ESC HF guidelines and 2023 focused update, the TITRATE-HF study started to prospectively investigate the use, sequencing, and titration of guideline-directed medical therapy (GDMT) in HF patients, including the identification of implementation barriers. Methods and results: TITRATE-HF is an ongoing long-term HF registry conducted in the Netherlands. Overall, 4288 patients from 48 hospitals were included. Among these patients, 1732 presented with de novo, 2240 with chronic, and 316 with worsening HF. The median age was 71 years (interquartile range [IQR] 63–78), 29% were female, and median ejection fraction was 35% (IQR 25–40). In total, 44% of chronic and worsening HFrEF patients were prescribed quadruple therapy. However, only 1% of HFrEF patients achieved target dose for all drug classes. In addition, quadruple therapy was more often prescribed to patients treated in a dedicated HF outpatient clinic as compared to a general cardiology outpatient clinic. In each GDMT drug class, 19% to 36% of non-use in HFrEF patients was related to side-effects, intolerances, or contraindications. In the de novo HF cohort, 49% of patients already used one or more GDMT drug classes for other indications than HF. Conclusion: This first analysis of the TITRATE-HF study reports relatively high use of GDMT in a contemporary HF cohort, while still showing room for improvement regarding quadruple therapy. Importantly, the use and dose of GDMT were suboptimal, with the reasons often remaining unclear. This underscores the urgency for further optimization of GDMT and implementation strategies within HF management.

Abstract: Cardiac amyloidosis (CA)—mostly transthyretin-related (ATTR-CA)—has recently gained interest in cardiology. Bone scintigraphy (BS) is one of the main screening tools for ATTR-CA but also used for various other reasons. The objective was to evaluate whether all CA cases are detected and what happens during follow-up. All routine BS performed at the Maastricht University Medical Center (May 2012–August 2020) were screened for the presence of CA. Scans performed for suspected CA were excluded. A Perugini stage ≥1 was classified as positive necessitating further examination. The electronic medical record system was evaluated for any contact with cardiology or other specialists until 2021. Of the 2738 BS evaluated, 40 scans (1.46%; median age 73.5 [IQR: 65.8–79.5], 82.5% male) were positive (Perugini grade 1: 31/77.5%, grade 2: 6/15%, grade 3: 3/7.5%); the potential diagnosis ATTR-CA was not seen in 38 patients (95%) by the nuclear medicine specialist. During follow-up, 19 out of those 40 patients (47.5%) underwent cardiac evaluation without diagnosing CA. Available echocardiograms of patients with a positive BS showed left ventricular hypertrophy, a preserved ejection fraction, and diastolic dysfunction ≥2 in 9/47%, 10/53%, and 4/21% of patients, respectively. Additionally, 20 (50%) patients presented to at least one specialty with symptoms indicative of cardiac amyloidosis. The prevalence of a positive BS indicating potential CA in an unselected population is low but substantial. Abstract: Microalgae are a promising renewable feedstock that can be produced on non-arable land using seawater. Their biomass contains proteins, lipids, carbohydrates, and pigments, and can be used for various biobased products, such as food, feed, biochemicals, and biofuels. For such applications, the production costs need to be reduced, for example, by improving biomass productivity in photobioreactors. In this study, Picochlorum sp. (BPE23) was cultivated in a prototype of a novel outdoor V-shaped photobioreactor on Bonaire (12°N, 68°W). The novel photobioreactor design was previously proposed for the capture and dilution of sunlight at low-latitude locations. During several months, the biomass productivity of the local thermotolerant microalgae was determined at different dilution rates in continuous dilution and batch dilution experiments, without any form of temperature control. Reactor temperatures increased to 35°C–45°C at midday. In the continuous dilution experiments, high average biomass productivities of 28–31 g m−2 d−1 and photosynthetic efficiencies of 3.5%–4.3% were achieved. In the batch dilution experiments, biomass productivities were lower (17–23 g m−2 d−1), as microalgal cells likely experienced sudden light and temperature stress after daily reactor dilution. Nonetheless, dense cultures were characterized by high maximum photosynthetic rates, illustrating the potential of Picochlorum sp. for fast growth under outdoor conditions.

39. Time and Labour Markets in the Regulation of Temporary Labour Migration in the European Union

Abstract: The importance of the phenomenon of temporary labour migration is growing both from a quantitative and from a political point of view. Its legal regulation is grounded on concepts, such as “temporariness” and “labour market”, which are either not defined by the law or in potential conflict with their socio-economic reality. We propose two approaches to better identify the meaning of these terms in the context of European Union (EU) law, by focusing on the three categories of labour migration which are explicitly defined as temporary. These are posting of workers, seasonal work, and intra-corporate transfers. We analyse the concept of temporariness through a necessary and sufficient conditions approach, whereas to identify the meaning of “labour market” law we propose an original systematic document analysis using a corpus of more than 1800 mentions of the concept in EU “hard law”. Our article highlights the polysemic nature of these concepts and proposes operationalised criteria for further research into the legal regulation of temporary labour migration in the EU.

41. Individual consent in cluster randomised trials for non-pharmaceutical interventions: going beyond the Ottawa statement

Abstract: This paper discusses the issue of overriding the right of individual consent to participation in cluster randomised trials (CRTs). We focus on CRTs testing the efficacy of non-pharmaceutical interventions. As an example, we consider school closures during the COVID-19 pandemic. In Norway, a CRT was promoted as necessary for providing the best evidence to inform pandemic management policy. However, the proposal was rejected by the Norwegian Research Ethics Committee since it would violate the requirement for individual informed consent. This sparked debate about whether ethics stand in the way of evidence-based health policy, since the Norwegian Research Ethics law’s strict requirements for individual consent make it practically impossible to carry out CRTs of public health interventions. We argue that, in the case of the school closure trial, the suggested CRT would not have eliminated an epistemic gap and thus would not have justified the violation of consent rights. First, we focus on the methodological challenges to estimating quantifiable effects of school closures in the specific case of an airborne infectious disease. Second, in line with Evidential Pluralism, we highlight the value of alternative lines of evidence for informing school closure policy in a pandemic. In general, we propose that a trial requiring the waiver of participants’ consent rights must be highly likely to eliminate an epistemic gap. We elaborate on the practical aspects of this criterion and discuss the potential advantages of adding it to the Ottawa Statement on the Ethical Design and Conduct of Cluster Randomized Trials.

Abstract: Background: Women exposed to armed conflicts and forced migration face significant health-related risks and consequences. Consequently, there is a need to identify and develop effective interventions that provide tailored support for them. The aim of this scoping review was to examine research evaluating support interventions promoting the health and well-being among women with traumatic experiences linked to armed conflict and/or forced migration. Methods: A scoping review of empirical studies evaluating non-pharmacologic/non-surgical interventions promoting health and well-being among adult women with traumatic experiences linked to armed, torture, and/or forced migration, identified through systematic searches in February 2022 within five databases (AMED, CINAHL, Cochrane Library, PsycINFO, and PubMed). Methodological characteristics and results were extracted and analyzed with narrative analysis using tabulations, descriptive statistics, text-based summaries, and thematization. Results: Assessment of 16 748 records resulted in 13 included reports. The methodological approaches were quantitative (n = 9), qualitative (n = 2), and mixed methods (n = 2), with most reports being feasibility/pilot studies (n = 5) and/or randomized controlled trials (n = 4). The most common recruitment strategy was non-probability sampling (n = 8). Most interventions were conducted in North America (n = 4), Asia (n = 3) or Middle East (n = 3). Thirteen intra-intervention techniques and five categories of components utilized within the interventions were identified, the most common being skill building (n = 12). Ten developed the interventions through theoretical frameworks or manuals/therapy, while five developed interventions through public or stakeholder involvement. Abstract: Including production considerations in the early design stages of aircraft structures is challenging. Production information is mostly known by experts and rarely formally documented such that it can be effectively used during the design process. Producibility is mostly considered after completing the design, resulting in increased cost and development time due to the late discovery of production issues. This paper presents a new model, called the Manufacturing Information Model (MIM), which supports the automatic inclusion of production considerations into the design process. The MIM provides a single source of truth and a generic structure to capture and organize production-related information in a product system. Furthermore, it provides compatibility analyses to automatically warn for or exclude infeasible designs. Analysis tools use the information stored within the MIM to calculate the mass, costs, and production rate of the product. To show the functionalities of the MIM, it has been applied to the conceptual design of a wing box at a Tier 1 company. This use case shows how the MIM supports trade-off decisions, as it allows for the identification of trends and the ranking of different manufacturing concepts. Abstract: During the early design stages of airframe components, many possible design architectures and production methods need to be traded to find the best configuration. Evaluating different production methods can be challenging as different production methods put different requirements on the product to be designed. This paper presents a new methodology that enables the inclusion of manufacturing and assembly in the design process. By extending the architectural design space model with components of the production system, the design choices regarding production are made explicit. Through the modeling of product and production requirements and assigning them a verification method, a dynamic MDAO workflow is formulated. Within a dynamic workflow, the design variables, analysis tools, and constraints change depending on the current design vector. The methodology has been applied to the design and manufacturing of a wing rib in which two manufacturing options were traded: metal machining and composite stamp forming. The dynamic MDAO workflow successfully found the Pareto front for both manufacturing methods. The main benefit is that only one workflow needed to be formulated and executed, whereas previously a separate MDAO workflow needed to be created for each combination of product design and production method. Abstract: Aims: Current heart failure (HF) guidelines recommend to prescribe four drug classes in patients with HF with reduced ejection fraction (HFrEF). A clear challenge exists to adequately implement guideline-directed medical therapy (GDMT) regarding the sequencing of drugs and timely reaching target dose. It is largely unknown how the paradigm shift from a serial and sequential approach for drug therapy to early parallel application of the four drug classes will be executed in daily clinical practice, as well as the reason clinicians may not adhere to new guidelines. We present the design and rationale for the real-world TITRATE-HF study, which aims to assess sequencing strategies for GDMT initiation, dose titration patterns (order and speed), intolerance for GDMT, barriers for implementation, and long-term outcomes in patients with de novo, chronic, and worsening HF. Methods and results: A total of 4000 patients with HFrEF, HF with mildly reduced ejection fraction, and HF with improved ejection fraction will be enrolled in >40 Dutch centres with a follow-up of at least 3 years. Data collection will include demographics, physical examination and vital parameters, electrocardiogram, laboratory measurements, echocardiogram, medication, and quality of life. Detailed information on titration steps will be collected for the four GDMT drug classes. Information will include date, primary reason for change, and potential intolerances. The primary clinical endpoints are HF-related hospitalizations, HF-related urgent visits with a need for intravenous diuretics, all-cause mortality, and cardiovascular mortality. Conclusions: TITRATE-HF is a real-world multicentre longitudinal registry that will provide unique information on contemporary GDMT implementation, sequencing strategies (order and speed), and prognosis in de novo, worsening, and chronic HF patients.
