(1) Objective: This study investigated the relationship between long-term particulate matter (PM 2.5 ) exposure and optic disc parameters-vertical cup-to-disc ratio (vCDR), vertical optic disc diameter (vDD), and vertical optic cup diameter (vCD)-in patients with type 2 diabetes mellitus (T2DM). (2) Methods: A cross-sectional analysis was conducted using data from 65,750 T2DM patients in the 2017-2018 Shanghai Cohort Study of Diabetic Eye Disease (SCODE). Optic disc parameters were extracted from fundus images, and PM 2.5 exposure was estimated using a random forest model incorporating satellite and meteorological data. Multivariate linear regression models were applied, adjusting for confounders including age, gender, body mass index, blood pressure, glucose, time of T2DM duration, smoking, drinking, and physical exercise. (3) Results: A 10 μg/m 3 increase in PM 2.5 exposure was associated with significant reductions in vCDR (-0.008), vDD (-42.547 μm), and vCD (-30.517 μm) (all p -values < 0.001). These associations persisted after sensitivity analyses and adjustments for other pollutants like O 3 and NO 2 . (4) Conclusions: Long-term PM 2.5 exposure is associated with detrimental changes in optic disc parameters in patients with T2DM, suggesting possible optic nerve atrophy. Considering the close relationship between the optic nerve and the central nervous system, these findings may also reflect broader neurodegenerative processes.

Background: To investigate the relationship between changes in the optic disc size and color, cup-to-disc (C/D) ratio, and axial elongation in schoolchildren., Methods: A prospective cohort study was performed in 75 right eyes of elementary school students for six years (from 8.5 to 14.5 years old). In the first and last year, all participants underwent optical axial length measurement and color fundus photography. The optic disc color was calculated by dividing the intensity of red by the sum of the intensity of red, green, and blue. The optic disc area was calculated by modifying the number of pixels according to Bennett's formula. The C/D ratio was calculated by dividing the vertical cup diameter by vertical optic disc diameter. Wilcoxon signed rank test was used to compare these optic disc parameters and axial length in the first and last year., Results: Mean axial length in the last year (24.82 mm) was significantly longer than that in the first year (23.34 mm). Likewise, the mean optic disc size was significantly smaller in the last year (41,946 pixels) than that in the first year (46,144 pixels). The mean optic disc color in the last year (0.49) was significantly more reddish than that in the first year (0.46), while the mean C/D ratio in last year (0.50) was significantly smaller than that in first year (0.52)., Conclusions: During the period from 8.5 years to 14.5 years of age, both the optic disc size and C/D ratio became smaller, while the color became more red., (© 2024. Background/aims: Primary open-angle glaucoma (POAG) disproportionately affects individuals of African ancestry. In these patients' eyes, a large cup-to-disc ratio (LCDR > 0.90) suggests greater retinal ganglion cell loss, though these patients often display varied visual ability. This study investigated the prevalence and risk factors associated with LCDR in African ancestry individuals with POAG and explored the differences between blind (>20/200) and not blind (≤20/200) LCDR eyes., Methods: A case-control methodology was used to investigate the demographic, optic disc, and genetic risk factors of subjects in the Primary Open-Angle African American Glaucoma Genetics Study. Risk factors were analyzed using univariable and multivariable logistic regression models with inter-eye correlation adjusted using generalized estimating equations., Results: Out of 5605 eyes with POAG, 1440 eyes (25.7%) had LCDR. In the multivariable analysis, LCDR was associated with previous glaucoma surgery (OR = 1.72), increased intraocular pressure (OR = 1.04), decreased mean deviation (OR = 1.08), increased pattern standard deviation (OR = 1.06), thinner retinal nerve fiber layer (OR = 1.05), nasalization of vessels (OR = 2.67), bayonetting of vessels (OR = 1.98), visible pores in the lamina cribrosa (OR = 1.68), and a bean-shaped cup (OR = 2.11). Of LCDR eyes, 30.1% were classified as blind (≤20/200). In the multivariable analysis, the statistically significant risk factors of blindness in LCDR eyes were previous glaucoma surgery (OR = 1.72), increased intraocular pressure (OR = 1.05), decreased mean deviation (OR = 1.04), and decreased pattern standard deviation (OR = 0.90)., Conclusions: These findings underscore the importance of close monitoring of intraocular pressure and visual function in African ancestry POAG patients, particularly those with LCDR, to preserve visual function.

Purpose: To determine the diagnostic value of optic nerve head (ONH) topographic parameters measured by swept-source optical coherence tomography (SS-OCT) in differentiating between early glaucoma cases, glaucoma suspects (GSs), and healthy eyes., Methods: The files of GSs, those with an early primary open angle glaucoma (POAG) diagnosis, and healthy eyes were retrospectively screened. Demographic characteristics, retinal nerve fibre measurements, visual field examinations, and ONH topographic parameters, including rim area, disc area, vertical and horizontal cup-to-disc ratios (CDRs), and cup volume, were compared between the groups. A receiver operating characteristic (ROC) curve analysis was performed on the ONH parameters., Results: A total of 170 eyes from 85 GSs, 114 eyes from 57 patients with a diagnosis of early POAG, and 70 healthy eyes from 48 subjects were included in the study. The median age was 52.1 ± 0.9 years for the POAG group, 60.2 ± 1.1 years for the GS group, and 60.5 ± 1.6 for the controls. In the comparison of the ONH parameters between the groups, only rim area statistically significantly differed between the POAG and GS groups. However, all ONH parameters statistically significantly differed between the GS and control groups (p< 0.05). The mean rim areas of the GS, POAG, and control groups were 1.278 ± 0.055 mm 2 , 1.073 ± 0.065 mm 2 , and 1.446 ± 0.055 mm 2 , respectively, being statistically significantly higher in the controls and lower in the POAG group (p < 0.001). The highest area under the curve (AUC) value belonged to vertical CDR for the discrimination of the eyes with POAG from healthy eyes [0.806 (0.728-0.869, p < 0.001] and rim area for the discrimination of POAG cases and GSs [0.728 (0.650-0.797, p < 0.001]., Conclusion: There are significant differences in all ONH parameters between the eyes of GSs and healthy eyes. For the discrimination of early POAG and GS cases, topographic disc parameters may have value. Proper estimation of the cup-to-disc ratio (C/D ratio) plays a significant role in ophthalmic examinations, and it is urgent to improve the efficiency of C/D ratio automatic measurement. Therefore, we propose a new method for measuring the C/D ratio of OCTs in normal subjects. Firstly, the end-to-end deep convolution network is used to segment and detect the inner limiting membrane (ILM) and the two Bruch's membrane opening (BMO) terminations. Then, we introduce an ellipse fitting technique to post-process the edge of the optic disc. Finally, the proposed method is evaluated on 41 normal subjects using the optic-disc-area scanning mode of three machines: BV1000, Topcon 3D OCT-1, and Nidek ARK-1. In addition, pairwise correlation analyses are carried out to compare the C/D ratio measurement method of BV1000 to existing commercial OCT machines as well as other state-of-the-art methods. The correlation coefficient between the C/D ratio calculated by BV1000 and the C/D ratio calculated by manual annotation is 0.84, which indicates that the proposed method has a strong correlation with the results of manual annotation by ophthalmologists. Moreover, in comparison between BV1000, Topcon and Nidek in practical screening among normal subjects, the proportion of the C/D ratio less than 0.6 calculated by BV1000 accounts for 96.34%, which is the closest to the clinical statistics among the three OCT machines. Background: Digital optic disc photographs are integral to remote telehealth ophthalmology, yet no quality control standards exist for the brightness setting of the images. This study evaluated the relationship between brightness setting and cup/disc ratio (c/d) grading among glaucoma specialists., Methods: Optic disc photographs obtained during routine examinations under anesthesia were collected to construct an image library. For each optic disc, photographs were obtained at 3 light intensity settings: dark, medium, and bright. From the image library, photograph triads (dark, medium and bright) of 50 eyes (50 patients) were used to construct the study set. Nine glaucoma specialists evaluated the c/d of the study set photographs in randomized order. The relationships between the brightness levels and the c/d grading as well as graders' years in practice and variability were evaluated., Results: The c/d were graded as significantly larger in bright photographs when compared to photographs taken at the medium light intensity (0.53 vs 0.48, P < 0.001) as well as those taken at the dark setting (0.47, P < 0.001). In addition, no relationship was found between ophthalmologists' years in practice and the variability of their c/d grading (P = 0.76)., Conclusion: Image brightness affects c/d grading of nonstereoscopic disc photographs. (1) Background: Vertical cup-to-disc ratio (CDR) is an important measure for evaluating damage to the optic nerve head (ONH) in glaucoma patients. However, this measure often does not fully capture the irregular cupping observed in glaucomatous nerves. We developed and evaluated a method to measure cup-to-disc ratio (CDR) at all 360 degrees of the ONH. (2) Methods: Non-physician graders from the Scheie Reading Center outlined the cup and disc on digital stereo color disc images from African American patients enrolled in the Primary Open-Angle African American Glaucoma Genetics (POAAGG) study. After converting the resultant coordinates into polar representation, the CDR at each 360-degree location of the ONH was obtained. We compared grader VCDR values with clinical VCDR values, using Spearman correlation analysis, and validated significant genetic associations with clinical VCDR, using grader VCDR values. (3) Results: Graders delineated outlines of the cup contour and disc boundaries twice in each of 1815 stereo disc images. For both cases and controls, the mean CDR was highest at the horizontal bisector, particularly in the temporal region, as compared to other degree locations. There was a good correlation between grader CDR at the vertical bisector and clinical VCDR (Spearman Correlation OD: r = 0.78 [95% CI: 0.76-0.79]). An SNP in the MPDZ gene, associated with clinical VCDR in a prior genome-wide association study, showed a significant association with grader VCDR ( p = 0.01) and grader CDR area ratio ( p = 0.02). (4) Conclusions: The CDR of both glaucomatous and non-glaucomatous eyes varies by degree location, with the highest measurements in the temporal region of the eye. This method can be useful for capturing innate eccentric ONH morphology, tracking disease progression, and identifying genetic associations.

Background: Surgical procedures are used as 1 of the main treatment modalities for pediatric glaucoma, even though progression may occur. In this study, we aimed to investigate the risk factors affecting the progression of pediatric glaucoma., Methods: In this retrospective cohort study, we reviewed the medical records of patients diagnosed with pediatric glaucoma between April 2009 and March 2017. Pediatric glaucoma patients who underwent regular follow-up for at least 1 year were included. Demographics, intraocular pressure (IOP), central corneal thickness (CCT), axial length (AL), cup-to-disc ratio (C/D ratio), corneal diameter, type of glaucoma, age at time of diagnosis, and age at surgery were recorded. Progression was defined as an increase in AL > 2 mm, C/D ratio > 0.2, or corneal diameter > 1 mm during 1 year of follow-up., Results: Eighty-three eyes from 46 patients were included: 37 eyes (45%) with primary congenital glaucoma (PCG), 46 eyes (55%) with secondary glaucoma, and 27 of these 83 eyes (32.5%) showed progression. Progression was comparable between eyes with PCG and secondary glaucoma (PCG, 22%; secondary glaucoma, 41%; P = 0.152). Age at the time of diagnosis and age at the time of the first surgery were significantly lower in the eyes with progression ( P = 0.046 and 0.012, respectively). The mean (standard deviation) of surgeries in progressed versus non-progressed eyes was 1.88 (1.1) versus 1 (0.8) ( P = 0.015). The frequency of comorbid systemic disease was significantly higher in patients with glaucoma progression ( P = 0.043). The progressed and non-progressed eyes were comparable in terms of other demographic characteristics and ocular parameters (all P > 0.05)., Conclusions: Pediatric glaucoma patients who were younger at the time of diagnosis and the first glaucoma surgery and those with comorbid systemic disease are at higher risk of glaucoma progression. These findings are useful for clinicians when counseling parents of children with pediatric glaucoma about disease outcomes. Background: Enlarged optic disc cupping and interocular cup-to-disc ratio (CDR) asymmetry are often important indicators of glaucoma. Clinically, we occasionally encounter children with large CDR and interocular CDR asymmetry during vision screening. This study aimed to report longitudinal change of ocular parameters in children with large cup-to-disc ratio (CDR) and interocular CDR asymmetry., Methods: This was a retrospective, observational case series of 160 eyes of 160 children with large CDR who visited a tertiary eye center from January 2010 to June 2016. Average CDR ≥ 0.6 were considered large CDR values, and CDR asymmetry was defined as an interocular difference ratio value greater than 0.2. All included patients showed interocular pressure (IOP) < 21 mmHg at least three ophthalmic examinations conducted at total intervals of at least 30 months., Results: The mean age of children included in the study was 7.14 ± 2.42 years, with a follow-up period of 54.46 ± 19.82 months. Changes in refractive error and axial length were significantly different between initial and final examination (p < 0.001). However, optic nerve head (ONH) analysis and retinal nerve fiber layer (RNFL) and macular ganglion cell-inner plexiform layer (mGCIPL) thicknesses were not significantly different between initial and final examination. In interocular comparisons of patients with CDR asymmetry, changes of refractive error, axial length, ONH analysis, and RNFL and mGCIPL thickness were not significantly different between the two eyes., Conclusions: There were no significant differences in the changes of ONH analysis, and RNFL and mGCIPL thicknesses in children with large CDR, or those with interocular CDR asymmetry over the study period. The disc damage likelihood scale (DDLS) is a tool for classifying glaucomatous structural changes to the optic disc based on the radial width of the neuroretinal rim at its thinnest location, or if no rim is present, the extent of absence of the rim. Unlike cup disc ratio (CDR), the DDLS also considers disc size. Twenty years after its first description, the aim of this review was to critically appraise evidence for the DDLS and evaluate its role in current practice. A literature search by two independent authors identified 33 relevant articles for inclusion. Five studies evaluated reproducibility, 5 diagnostic performance, and 2 studies examined ability to detect progression. Eleven studies evaluated correlation between DDLS and other markers of glaucoma. Despite the widespread availability of imaging devices such as optical coherence tomography (OCT), clinical examination of the optic disc remains an essential component of glaucoma diagnosis and monitoring. We investigated the association of the single nucleotide polymorphism (SNP) rs112369934 near the TRIM66 gene with qualitative and quantitative phenotypes of primary open-angle glaucoma (POAG) in African Americans (AA). AA subjects over 35 years old were recruited for the Primary Open-Angle African American Glaucoma Genetics (POAAGG) study in Philadelphia, PA. Glaucoma cases were evaluated for phenotypes associated with POAG pathogenesis, and the associations between rs112369934 and phenotypes were investigated by logistic regression analysis and in gender-stratified case cohorts: The SNP rs112369934 was found to have a suggestive association with retinal nerve fiber layer (RNFL) thickness and cup-to-disc ratio (CDR) in 1087 male AA POAG cases, individuals with the TC genotype having thinner RNFL (95% CI 0.85 to 6.61, p = 0.01) and larger CDR (95% CI -0.07 to -0.01, p = 0.02) than those with wildtype TT. No other significant associations were found. In conclusion SNP rs112369934 may play a role in POAG pathogenesis in male AA individuals. However, this SNP has been implicated in higher POAG risk in both male and female AA POAG cases.

Glaucoma is one of the world's leading causes of irreversible blindness. A complex, multifactorial disease, the underlying pathogenesis and reasons for disease progression are not fully understood. The most common form of glaucoma, primary open-angle glaucoma (POAG), was traditionally understood to be the result of elevated intraocular pressure (IOP), leading to optic nerve damage and functional vision loss. Recently, researchers have suggested that POAG may have an underlying genetic component. In fact, studies of genetic association and heritability have yielded encouraging results showing that glaucoma may be influenced by genetic factors, and estimates for the heritability of POAG and disease-related endophenotypes show encouraging results. However, the vast majority of the underlying genetic variants and their molecular mechanisms have not been elucidated. Several genes have been suggested to have molecular mechanisms contributing to alterations in key endophenotypes such as IOP ( LMX1B , MADD , NR1H3 , and SEPT9) , and VCDR ( ABCA1 , ELN , ASAP1 , and ATOH7 ). Still, genetic studies about glaucoma and its molecular mechanisms are limited by the multifactorial nature of the disease and the large number of genes that have been identified to have an association with glaucoma. Therefore, further study into the molecular mechanisms of the disease itself are required for the future development of therapies targeted at genes leading to POAG endophenotypes and, therefore, increased risk of disease.

Purpose: To study the incidence and risk factors for onset or progression of posterior vitreous detachment (PVD) at the vitreomacular interface (VMI) after cataract surgery., Design: Retrospective case series., Participants: Patients with a history of phacoemulsification from April 2018 through April 2019 at the Keye Eye Center, Seoul, Korea, and postoperative monitoring for more than 2 months., Methods: Cox proportional hazard ratios for the onset or development of PVD after cataract surgery in the presence of selected risk factors and demographic data were calculated. To evaluate the ocular risk factors, various ocular metrics, including spherical equivalent (SE), axial length (AL), anterior chamber depth, lens thickness, central subfield thickness, PVD status at macula and optic nerve head (ONH), peripapillary retinal nerve fiber layer thickness, and ONH parameters from OCT scans, were used for the analysis., Main Outcome Measures: Onset or development of PVD at the VMI., Results: Among 988 eyes without PVD at baseline, 174 eyes (17.6%) showed changes in the VMI. Univariate analysis showed that age, SE, AL, PVD status at macula and ONH, and average and vertical cup-to-disc ratios (CDRs) were associated significantly with PVD onset or development (P = 0.046, P = 0.004, P = 0.040, P < 0.001, P < 0.001, P = 0.008, and P = 0.042, respectively). In a multivariate analysis, PVD status at the macula and ONH and smaller CDR were associated with PVD onset or progression after cataract surgery after adjustment for age, SE, and AL (P < 0.001, P < 0.001, and P = 0.005, respectively)., Conclusions: The risk of PVD onset or progression was dependent on PVD status and the CDR detected on OCT scans, not on age or AL, in a large patient cohort. Purpose: To evaluate the clinical and socio-demographic profile of patients living with glaucoma and receiving care in a tertiary eye center in Zimbabwe., Method: A hospital-based retrospective study of clinical records of glaucoma patients from January 2014 to December 2018. The study involved collating demographic information of patients, visual acuities, (VA) intraocular pressure, (IOP), cup-to-disc ratios, (CDRs), average retinal nerve fibers thickness, (RNFL), cup volume, cup-to-disc area, vertical cup-to-disc ratio, (VCDR), rim area, disc area, glaucoma hemifield test, visual field indices and the management of glaucoma., Results: Nine thousand one hundred and eighty-five (9,185) folders were retrieved. Out of these, 432 (4.7%) qualified for the study and were analyzed. There were 267 (61.8%) males and 165 (38.2%) females. The mean age (± Standard deviation, SD) of the patients was 62.66 ± 15.94 years, (range 10 - 110 years). The means visual acuity (VA): OD =1.30 ± 1.06 Logarithm of the Minimum Angle of Resolution, (logMAR), OS = 1.33 ± 1.06 logMAR; IOP: OD = 29.51 ± 12.89 mmHg, OS: 29.17 ± 12.59 mmHg; CDRs: OD = 0.91 ± 0.14 D, OS = 0.92 ± 0.14 D; and the average RNFL thickness was 72.76 ± 18.26 µm and 71.24 ± 23.17 µm in the right and left eye respectively. The mainstay of treatment was medication only., Conclusion: There were more males than females receiving glaucoma care at the tertiary level. Glaucoma cases included juveniles but the mean age was mostly the elderly. This paper proposes a deep image analysis-based model for glaucoma diagnosis that uses several features to detect the formation of glaucoma in retinal fundus. These features are combined with most extracted parameters like inferior, superior, nasal, and temporal region area, and cup-to-disc ratio that overall forms a deep image analysis. This proposed model is exercised to investigate the various aspects related to the prediction of glaucoma in retinal fundus images that help the ophthalmologist in making better decisions for the human eye. The proposed model is presented with the combination of four machine learning algorithms that provide the classification accuracy of 98.60% while other existing models like support vector machine (SVM), K-nearest neighbors (KNN), and Naïve Bayes provide individually with accuracies of 97.61%, 90.47%, and 95.23% respectively. These results clearly demonstrate that this proposed model offers the best methodology to an early diagnosis of glaucoma in retinal fundus.

Importance: Evidence-based guidelines are essential for glaucoma screening to work effectively., Background: To derive a vertical cup-to-disc ratio (VCDR) cut-off for glaucoma screening in a multi-ethnic Asian population., Design: The Singapore Epidemiology of Eye Diseases (SEED) study is a population-based study conducted from 2004 to 2011 in a single tertiary care research institute., Participants: SEED comprised of 10 033 Chinese, Malay and Indian adults aged ≥40 (response rate 75.6%). After excluding participants with a history of glaucoma medication or surgery, 9673 participants were included for analysis., Methods: A systematic eye examination, which included applanation tonometry, visual field testing, gonioscopy and dilated fundus examination was conducted., Main Outcome Measure: Diagnosis of glaucoma., Results: The distribution of VCDR and VCDR asymmetry were relatively homogenous in this multi-ethnic Asian population, with a 97.5th percentile value of 0.67 and 0.17, respectively. In the absence of more definite signs of glaucoma, VCDR ≥0.60 and VCDR asymmetry ≥0.20 provided the best balance between sensitivity (95.1%) and specificity (90.9%) in detecting glaucoma. For larger optic disc (≥2.0 mm), VCDR ≥0.65 with VCDR asymmetry ≥0.20 provided the best balance between sensitivity (84.8%) and specificity (93.2%)., Conclusion and Relevance: Overall, VCDR ≥0.60 with VCDR ≥0.20 asymmetry provides a good balance between sensitivity and specificity in detecting glaucoma. For larger optic disc, VCDR ≥0.65 should be considered instead to mitigate against false-referrals due to larger physiological disc cupping. Purpose: Recent guidelines recommend disc damage likelihood scale (DDLS) is recorded for all referrals of suspected glaucoma from community optometrists to hospital eye services (HES) in Scotland. This study aimed to determine whether lower DDLS grades were associated with higher rates of discharge at the first visit to HES., Methods: A retrospective analysis of 618 consecutive new referrals from community optometrists to a university hospital glaucoma service. 65 (10.5%) included DDLS graded by the community optometrist. A comprehensive eye examination and optical coherence tomography (OCT) was performed in the hospital glaucoma clinic and first visit discharge rate (FVDR) for different grades of DDLS compared. The relationship between DDLS and retinal nerve fibre layer (RNFL) thickness on OCT was also examined., Results: The FVDR for patients with DDLS recorded in the referral was 27.7% (18 of 65) compared to 25% (138 of 553) in those without DDLS (p = 0.631). The FVDR was 50% for those with a DDLS of 3 in the worse eye, decreasing to 32% and 21% for DDLS grades of 4 and 5 respectively. No patient with a DDLS ≥ 6 was discharged at the first visit and none with a DDLS < 4 (the cut off for consideration of referral in Scottish guidelines) were found to have glaucoma. There was a significant but weak inverse relationship between DDLS and RNFL thickness. The strongest relationship was with average RNFL thickness (r = -0.378, p < 0.01) followed by superotemporal (r = -0.359, p < 0.01) and inferotemporal (r = -0.353, p < 0.01) RNFL thickness., Conclusions: In patients referred to HES with DDLS information included, lower DDLS grading was associated with higher odds of being discharged at the first visit. Early detection of glaucoma is important to slow down progression of the disease and to prevent total vision loss. Retinal fundus photography is frequently obtained for various eye disease diagnosis and record and is a suitable screening exam for its simplicity and low cost. However, the number of ophthalmologists who are specialized in glaucoma diagnosis is limited. We have been studying automated schemes for detection of nerve fiber layer defects and analysis of optic disc deformation, two major signs of glaucoma, in assisting ophthalmologists' accurate and efficient diagnosis. In this chapter, our recent progress in computerized methods is discussed.

Background and Objective: Visual impairment affects a significant part of the population worldwide. Glaucoma is one of these main causes, a chronic eye disease leading to progressive vision loss. Early glaucoma screening is an important task, allowing a slowing down of the pathology spreading and avoidance of irreversible vision damages. When manual assessment by experts suffers from disadvantages, exploiting the relevant Cup-to-Disc Ratio (CDR) feature as a structural indicator to assess the damage to the optic nerve head (ONH) is an efficient way for early glaucoma screening and diagnosis., Methods: In this paper, we propose a new fully automated methodology for glaucoma screening and diagnosis from retinal fundus images. In order to allow eye examination in remote locations with limited access to clinical facilities, we focus in this work on the development of a computationally-efficient algorithm for further implementation on mobile devices. First, the method provides a robust optic disc (OD) detection method, combining a brightness criterion and a template matching technique, to effectively detect the optic disc (OD) even in the presence of bright lesions associated to pathological cases. Second, an efficient optic cup (OC) and optic disc (OD) segmentation is performed, using a texture-based and model-based approach. Finally, Cup-to-Disc Ratio (CDR) computation leads to glaucoma screening with a classification between healthy and glaucomatous patients., Results: The proposed approach for glaucoma screening and diagnosis have been tested on the publicly available DRISHTI-GS1 dataset. Fifty retinal images are provided and labeled healthy or glaucomatous by trained specialists. The method achieves 98% of accuracy on final glaucoma screening and diagnosis, and excellent performance rates on evaluation metrics, outperforming the state-of-the-art CDR feature-based approaches., Conclusions: We proposed a fully automated method for glaucoma screening and diagnosis from retinal images. Excellent performance was obtained on final screening, classifying healthy and glaucomatous subjects. As it effectively detects the presence of glaucoma, in a low-computational manner, the approach can be part of a mobile help-diagnosis system, to improve the final diagnosis by the specialist and develop widespread visual health programs., (Copyright © 2019 Elsevier Ltd. Glaucoma is a sight-threading disease which can lead to irreversible blindness. Currently, extracting the vertical cup-to-disc ratio (CDR) from 2D retinal fundus images is promising for automatic glaucoma diagnosis. In this paper, we present a novel sparse coding approach for glaucoma diagnosis called adaptive weighted locality-constrained sparse coding (AWLCSC). Different from the existing reconstruction-based glaucoma diagnosis approaches, the weighted matrix in AWLCSC is constructed by adaptively fusing multiple distance measurement information between the reference images and the testing image, making our approach more robust and effective to glaucoma diagnosis. In our approach, the disc image is firstly extracted and reconstructed according to the proposed AWLCSC technique. Then, with the usage of the obtained reconstruction coefficients and a series of reference disc images with known CDRs, the CDR of the testing disc image can be automated estimation for glaucoma diagnosis. The performance of the proposed AWLCSC is evaluated on two publicly available DRISHTI-GS1 and RIM-ONE r2 databases. Purpose: To evaluate the characteristics of patients with previously undiagnosed primary open-angle glaucoma (POAG) in Korea., Methods: This study examined data from 391 subjects obtained from the 2008-2009 Korean National Health and Nutrition Examination Survey (KNHANES). The KNHANES is a population-based, cross-sectional epidemiological survey. Participants aged 19 years or older completed standardized interviews and dilated ocular examinations, including measurement of intraocular pressure, visual fields with frequency doubling perimetry, and fundus photography. Data from the 361 patients with previously undiagnosed POAG were analyzed and compared with data from the 30 patients with previously diagnosed glaucoma., Results: A total of 92.3% of POAG cases were undiagnosed before this study. Adjusted for age and sex, the strongest risk factor for undiagnosed glaucoma was longer elapsed time since last eye doctor visit. Glaucoma patients who had not visited an eye specialist in the last 3 years were 22 times (95% confidence interval, CI, 4.49-105.64, p < 0.001) more likely to have undiagnosed disease compared with patients who had visited an eye specialist in the last month. Another significant factor for previously undiagnosed glaucoma was smaller cup-to-disc ratio (odds ratio, OR, 0.60/0.1 units, 95% CI 0.43-0.85/0.1 units, p = 0.004). The higher vertical cup-to-disc ratio of a subject's two eyes was significantly different between those with previously undiagnosed (0.69) and diagnosed (0.78) POAG (p = 0.001)., Conclusions: The undiagnosed POAG group had a longer interval from last eye doctor visit and smaller vertical cup-to-disc ratio compared to the diagnosed group.

Purpose: To assess the relationships between optic cup-to-disc ratio (CDR) and age, sex, and other demographic and health characteristics in the healthy Korean population., Methods: The study design was retrospective and population-based. A total of 28,377 subjects who participated in the Korea National Health and Nutrition Examination Survey between 2008 and 2011 were enrolled in this study. Participants underwent structured interviews as well as systemic and ophthalmic examinations. Patients with glaucoma who were diagnosed using the International Society of Geographical and Epidemiological Ophthalmology classification were excluded. Changes in vertical CDR were examined by age in relation to systemic variables on multiple regression analysis., Results: The mean vertical CDR was 0.34 ± 0.12. The vertical CDR increased with age from subjects in their 20s to those in their 80s (p < 0.001). The mean CDR in males was significantly higher than that of females (p < 0.001). On multiple regression analysis, the vertical CDR was positively associated with age (p < 0.001), male sex (p < 0.001), diastolic blood pressure (p = 0.009), and intraocular pressure (p < 0.001) but was negatively associated with body mass index (p < 0.001)., Conclusions: Greater vertical CDR was related to age, male sex, higher diastolic blood pressure, higher intraocular pressure, and lower body mass index in healthy Koreans.

This study seeks to characterise potential differences in the cup-to-disc ratio (CDR) of patients with idiopathic intracranial hypertension (IIH) with papilloedema (IIHWP) compared with IIH without papilloedema (IIHWOP). The medical charts, optical coherence tomography (OCT), and digital optic disc photos of 30 patients (59 eyes) with IIHWP and 4 patients (8 eyes) with IIHWOP were reviewed retrospectively. The CDR values of the two groups were analysed using the Wilcoxon-Mann-Whitney test. Results showed a statistically significant smaller CDR in patients with IIHWP as compared with IIHWOP. These findings suggest that structural factors of the optic disc may play a role in the development of or lack thereof of papilloedema in patients with IIH.
