Background: Large-scale and long-term studies are not sufficient to determine the efficiency that IT solutions can bring to transfusion safety., Objective: This quality-improvement report describes our continuous efforts to implement and upgrade a bar code-based transfusion management (BCTM) system since 2011 and examines its effectiveness and sustainability in reducing blood transfusion errors, in a 3000-bed tertiary hospital, where more than 60,000 prescriptions of blood transfusion are covered by 2500 nurses each year., Methods: The BCTM system uses barcodes for patient identification, onsite labeling, and blood product verification, through wireless connection to the hospital information systems. Plan-Do-Study-Act (PDSA) cycles were used to improve the process. Process maps before and after implementation of the BCTM system in 2011 were drawn to highlight the changes. The numbers of incorrect labeling or wrong blood in tube incidents that occurred quarterly were plotted on a run chart to monitor the quality changes of each intervention introduced. The annual occurrences of error events from 2011 to 2017 were compared with the mean occurrence of 2008-2010 to determine whether implementation of the BCTM system could effectively reduce the number of errors in 2016 and whether this reduction could persist in 2017., Results: The error rate decreased from 0.03% in 2008-2010 to 0.002% in 2016 (P<.001) and 0.001% in 2017 (P<.001) after implementation of the BTCM system. Only one incorrect labeling incident was noted among the 68,324 samples for blood typing, and no incorrect transfusions occurred among 67,423 transfusion orders in 2017., Conclusions: This report demonstrates that continuous efforts to upgrade the existing process is critical to reduce errors in transfusion therapy, with support from information technology., (©Shin-Shang Chou, Ying-Ju Chen, Yu-Te Shen, Hsiu-Fang Yen, Shu-Chen Kuo. Ethanol was used to induce acute hepatotoxicity in mice. Effects of cinnamic acid (CA) and syringic acid (SA) post-intake for hepatic recovery from alcoholic injury was investigated. Ethanol treated mice were supplied by CA or SA at 40 or 80 mg/kg BW/day for 5 days. Results showed that ethanol stimulated protein expression of CYP2E1, p47 phox , gp91 phox , cyclooxygenase-2 and nuclear factor kappa B in liver. CA or SA post-intake restricted hepatic expression of these molecules. Ethanol suppressed nuclear factor erythroid 2-related factor (Nrf2) expression, and CA or SA enhanced Nrf2 expression in cytosolic and nuclear fractions. Ethanol increased the release of reactive oxygen species, oxidized glutathione, interleukin-6, tumor necrosis factor-alpha, nitric acid and prostaglandin E 2 . CA or SA lowered hepatic production of these oxidative and inflammatory factors. Histological data revealed that ethanol administration caused obvious foci of inflammatory cell infiltration, and CA or SA post-intake improved hepatic inflammatory infiltration. These findings support that cinnamic acid and syringic acid are potent nutraceutical agents for acute alcoholic liver disease therapy. However, potential additive or synergistic benefits of cinnamic and syringic acids against ethanol-induced hepatotoxicity need to be investigated., (Copyright © 2016 Elsevier Ltd. Twenty-nine commercial essential oil (EO) products that were purchased from the Taiwan market, including three different company-made Melissa officinalis essential oils, were assayed on their glucose consumption activity and lipid accumulation activity on 3T3-L1 adipocytes. The EOs of M. officinalis were significantly active in both model assays. By contrast, EOs of peppermint, lavender, bergamot, cypress, niaouli nerolidol, geranium-rose, and revensara did not increase glucose consumption activity from media, but displayed inhibited lipid accumulation activity (65-90% of lipid accumulation vs. the control 100%). Because of the promising activity of M. officinalis EOs, three different products were collected and compared for their gas chromatography chemical profiles and bioactivity. The Western blot data suggest that the key factors of the adenosine monophosphate-activated protein kinase/acetyl-CoA carboxylase pathway can be mediated by M. officinalis EOs. Together with biodata, gas chromatography-mass spectrometry profiles suggested mixtures of citrals and minor compounds of M. officinalis EOs may play an important role on effect of antidiabetes., (Copyright © 2014. The purpose of this study was to investigate the physiological maturation-inducing steroid (MIS) in the marine protandrous yellowfin porgy (Acanthopagrus latus). Female fish were injected with 2 doses of LHRH analog (10 and 40 μg per kg). Ovarian tissue was obtained at 6 h intervals for in vitro analysis of oocyte maturation. The most effective steroids for inducing in vitro maturation (germinal vesicle breakdown and GVBD) in cultured oocytes were 17,20β-dihydroxy-4-pregnen-3-one (17,20βP) and 17,20β,21-trihydroxy-4-pregnen-3-one (20β-S). 17,20βP was less potent than 20βS in inducing oocyte maturation. At higher concentrations, 11-deoxycortisol, 17α-hydroxy-progesterone, and 20β-21-dihydroxy-4-pregnen-3-one also significantly induced oocyte maturation. A tritiated precursor [(3)H]-pregnenolone, was cultured in vitro together with the maturing ovarian tissue. The tritiated metabolites were purified and identified by solvent extraction, HPLC, TLC, acetylation reaction and recrystallization. HPLC, TLC and recrystallization analysis showed that significant levels of tritiated 11-deoxycortisol (a precursor of 20β-S) and 20β-S, but not 17,20βP, were biosynthesized from [(3)H]-pregnenolone. Similar TLC profiles were obtained from the tritiated products that were isolated from the HPLC/TLC 20β-S fraction and standard 20β-S after the acetylation reaction. Constant specific radioactivity of tritiated 11-deoxycortisol and 20β-S but not 17,20βP by recrystallization was obtained in the tritiated metabolites isolated from HPLC and TLC fractions. The expression of 20β-hydroxysteroid dehydrogenase (20β-HSD) mRNA (a key enzyme that converts 11-deoxycortisol to 20β-S) was significantly increased in maturing ovarian tissue. Background: Female breast cancer has been ranked as the leading cause of cancer death in Taiwan. Women who undergo a mastectomy are at risk of suffering emotional disturbance because of deteriorating body image. A nursing intervention designed to restore body image perceptions and to reduce anxiety and emotional distress in this vulnerable group should be developed and administered., Purpose: This study was developed to evaluate the effectiveness of informational and emotional consultation on body image, anxiety, and emotional distress in women with breast cancer who underwent modified radical mastectomy., Methods: A quasi-experimental design with convenience sampling was used. Two hospitals within the same hospital system in southern Taiwan provided the research setting. Sixty-three women diagnosed with breast cancer completed the interviews (experimental group = 32, control group = 31). The experimental group participants received a two-session informational and emotional consultation, whereas those in the control group received routine nursing care only. All participants completed a series of three face-to-face interviews, including one pretest (Time 1, before surgery), a first posttest (Time 2, the day of discharge), and a second (follow-up) test (Time 3, 2 months after surgery)., Results: The intervention had an immediate positive effect (i.e., on the day of hospital discharge) on anxiety and a delayed positive effect (i.e., 2 months after surgery) on body image, anxiety, and emotional distress. The experimental group participants indicated a high level of satisfaction with the intervention., Conclusions/implications for Practice: Our findings suggest the ability of this nursing informational and emotional consultation intervention to reduce patient anxiety at both short- and long-term stages. However, improved perception of body image and emotional distress only over the longer term indicates that these dimensions take time to be internalized and improved. This nursing informational and emotional consultation intervention may provide a guide for hospitals and nurses in caring for women recovering from modified radical mastectomy surgery.

The effects of n-acetyl cysteine (NAC), s-allyl cysteine (SAC), s-ethyl cysteine, s-methyl cysteine and s-propyl cysteine (SPC) activity on Balb/cA mice against diabetic complications were examined. These complications included hyperglycemia, hyperlipidemia, oxidation stress, blood coagulation, and cytokine imbalance. To induce diabetes, mice were treated with streptozotocin i.p. for 5 consecutive days. Five cysteine-containing compounds at 1 g/L were added to the drinking water. After intake of the 5 cysteine-containing agents for 4 wk, body weight loss, plasma concentrations of glucose and insulin, and fibronectin levels were improved (P < 0.05) in diabetic mice. The administration of these agents restored the glutathione level (P < 0.05), reduced the loss of catalase and glutathione peroxidase activities in kidney and liver (P < 0.05), and decreased glucose-induced lipid oxidation, as assessed by malondialdehyde formation (P < 0.05). In all diabetic mice, the intake of these agents reduced triglyceride levels in plasma and liver (P < 0.05); however, only NAC, SAC and SPC treatments reduced cholesterol level in liver (P < 0.05). These cysteine-containing agents elevated the activity of 2 fibrinolytic factors, protein C and antithrombin III (P < 0.05). The overexpression of interleukin-6 and tumor necrosis factor-alpha in diabetic mice was suppressed by the intake of the 5 cysteine-containing agents (P < 0.05). Via their antioxidant activities, the 5 compounds effectively improved glycemic control, delayed oxidation damage, downregulated inflammatory cytokines, and enhanced anticoagulant activity in diabetic mice. These data support the multiple roles of these agents as potential protective agents for delaying diabetic deterioration.
