The primary objective is to find out the epidemiology, characteristics, preparedness, and survival of elderly patients initiated on dialysis from a tertiary center in Sabah. All newly confirmed patients aged 65 and above who were initiated on renal replacement therapy (RRT) from January 1, 2014, to December 31, 2017, were included. There were two groups of patients: Group 1 - optimally prepared for RRT; Group 2 - suboptimally prepared for RRT. Survival outcome was recorded at the 3 rd -, 6 th -, and 12 th -month interval. Diabetes mellitus accounted for majority of the primary cause of end-stage renal disease, 67.3%. The median duration of follow-up was 550 days (interquartile range = 787). There were 81 patients (30.5%) in Group 1. 182 patients (69.2%) in Group 2 were considered as suboptimally prepared for RRT. The survival of this elderly group was 85.6% at 3 months, 74.5% at six months, and 65% at 1 year. Group 2 patients had much poorer survival outcome compared to Group 1 (56% vs. 85.2% at one year, P <0.001). Age and RRT preparation appeared to be associated with the survival outcome. Decision in initiating dialysis in this group of patients is not as straightforward as in the younger age groups. A more realistic view on the actual survival, as well as consideration regarding the quality of life after the initiation of dialysis, needs to be taken into account. If dialysis therapy is decided, early preparation is important.

Objective: This study aims to evaluate the magnetic resonance imaging (MRI) brain patterns among hypoxic-ischemic encephalopathy (HIE) babies who underwent post-cooling MRI brain as well as to correlate the post-cooling brain scoring with patient's neurodevelopmental outcome at 2 years., Subjects and Methods: It was a retrospective cross sectional study carried out at a tertiary university hospital. Record of patients diagnosed with neonatal HIE from 2007 until 2016 who completed 72 h of cooling therapy and had MRI brain within 2 weeks of life were included in this study. A new scoring system by Trivedi et al . that emphasizes on subcortical deep gray matter and posterior limb internal capsule injury were utilized upon MRI assessment, using TW, T2W, and diffusion-weighted imaging (DWI) sequences. Cumulative MRI brain score was obtained and graded as none, mild, moderate, and severe brain injury. The MRI brain scoring was then correlated with patient's 2 years neurodevelopmental outcome using Fisher's Exact Test., Results: A total of 23 patients were eligible of which 19 term neonates were included. 13% of these neonates ( n = 3) had mild MRI brain injury grading with 52.2% ( n = 12) moderate and 34.8% ( n = 8) severe. There was no significant correlation seen between MRI brain grading and developmental outcome at 2 years old ( P > 0.05)., Conclusion: There was no significant correlation between neonatal MRI brain injury grading and 2 years neurodevelopmental outcome. Activity dependent setting of synaptic tags is critical for the establishment and maintenance of long-term plasticity and its associative properties such as synaptic tagging and capture (STC), a widely studied cellular model of associative memory. Although the known mechanisms of STC such as setting of synaptic tags or distribution of plasticity related proteins (PRPs) are the processes mainly happening within the neuronal compartments, the role of non-neuronal components is still elusive. Here, we report that microglia has a specific role in setting the synaptic tags and thus promotes long-term plasticity and STC. Treatment of hippocampal slices with clodronate, a specific inhibitor of microglia, resulted in an activated morphology of microglia but not of the hippocampal pyramidal neurons, oligodendrocytes or astrocytes. Activation of microglia before or 60 min after the induction of long-term plasticity prevented its maintenance and thus the expression of STC. Interestingly, activation of microglia 2 h after the induction of long-term plasticity neither prevented its maintenance nor its associative interaction with activated nearby synaptic populations. Given the half-life of synaptic tags is until about 60-90 min, activation of microglia beyond this time point while the maintenance phase is still unperturbed, suggests a lack of microglial interference in the synthesis or trigger of plasticity related products. Dissipative Kerr solitons in resonant frequency combs offer a promising route for ultrafast mode-locking, precision spectroscopy and time-frequency standards. The dynamics for the dissipative soliton generation, however, are intrinsically intertwined with thermal nonlinearities, limiting the soliton generation parameter map and statistical success probabilities of the solitary state. Here, via use of an auxiliary laser heating approach to suppress thermal dragging dynamics in dissipative soliton comb formation, we demonstrate stable Kerr soliton singlet formation and soliton bursts. First, we access a new soliton existence range with an inverse-sloped Kerr soliton evolution-diminishing soliton energy with increasing pump detuning. Second, we achieve deterministic transitions from Turing-like comb patterns directly into the dissipative Kerr soliton singlet pulse bypassing the chaotic states. This is achieved by avoiding subcomb overlaps at lower pump power, with near-identical singlet soliton comb generation over twenty instances. Third, with the red-detuned pump entrance route enabled, we uncover unique spontaneous soliton bursts in the direct formation of low-noise optical frequency combs from continuum background noise. The burst dynamics are due to the rapid entry and mutual attraction of the pump laser into the cavity mode, aided by the auxiliary laser and matching well with our numerical simulations. Enabled by the auxiliary-assisted frequency comb dynamics, we demonstrate an application of automatic soliton comb recovery and long-term stabilization against strong external perturbations. Chaos has revolutionized the field of nonlinear science and stimulated foundational studies from neural networks, extreme event statistics, to physics of electron transport. Recent studies in cavity optomechanics provide a new platform to uncover quintessential architectures of chaos generation and the underlying physics. Here, we report the generation of dynamical chaos in silicon-based monolithic optomechanical oscillators, enabled by the strong and coupled nonlinearities of two-photon absorption induced Drude electron-hole plasma. Deterministic chaotic oscillation is achieved, and statistical and entropic characterization quantifies the chaos complexity at 60 fJ intracavity energies. The correlation dimension D 2 is determined at 1.67 for the chaotic attractor, along with a maximal Lyapunov exponent rate of about 2.94 times the fundamental optomechanical oscillation for fast adjacent trajectory divergence. Nonlinear dynamical maps demonstrate the subharmonics, bifurcations and stable regimes, along with distinct transitional routes into chaos. This provides a CMOS-compatible and scalable architecture for understanding complex dynamics on the mesoscopic scale.

Comprehensive feeding practices questionnaire (CFPQ) is an instrument specifically developed to evaluate parental feeding practices. It has been confirmed among children in America and applied to populations in France, Norway, and New Zealand. In order to extend the application of CFPQ, we conducted a factor structure validation of the translated version of CFPQ (CFPQ-M) using confirmatory factor analysis among mothers of primary school children (N = 397) in Malaysia. Several items were modified for cultural adaptation. Of 49 items, 39 items with loading factors >0.40 were retained in the final model. The confirmatory factor analysis revealed that the final model (twelve-factor model with 39 items and 2 error covariances) displayed the best fit for our sample (Chi-square = 1147; df = 634; P < 0.05; CFI = 0.900; RMSEA = 0.045; SRMR = 0.0058). The instrument with some modifications was confirmed among mothers of school children in Malaysia. The present study extends the usability of the CFPQ and enables researchers and parents to better understand the relationships between parental feeding practices and related problems such as childhood obesity.

Selenium in the form of sodium selenite (SSE) is an essential micronutrient which known to possess antioxidant and anticancer properties. This study emphasizes the role of selenium on oxidative stress in experimental rats with N-diethylnitrosamine (DEN) initiated and 2-acetylaminofluorene (2-AAF) promoted multistage hepatocellular carcinogenesis (HCC). Rats were divided randomly into six groups: negative control, positive control (DEN+2-AAF), preventive group (pre-SEE 4 weeks+DEN), preventive control (respective control for preventive group), therapeutic group (DEN+post-SSE 12 weeks) and therapeutic control (respective control for therapeutic group). SSE (4 mg L(-1)) was given to animals before initiation and during promotion phase of HCC. The levels of total protein (TP), conjugated diens (CD), malondialdehyde (MDA), fluorescent pigment (FP), antioxidant activity (AOA) and DNA damage were measured. Supplementation of SSE before the initiation phase of carcinogenicity significantly increased TP and AOA level (p < 0.05) while it decreased the levels of CD, MDA, DNA damage and FP (p < 0.05). Supplementation of SSE during the promotion phase of carcinogenicity significantly decreased the DNA damage and FP level (p < 0.05) and there were negative correlation between the level of AOA and with the level of FP and CD. Thus, supplementation of SSE reduced the adverse changes which occur in liver cancer. However, the chemoprevention effect of SSE was more pronounced when it was supplemented before initiation phase of cancer when compared to promotion phase.
