Objective: To assess the implementation of the bedside medication preparation process in an Intensive Care Unit, following a quality improvement cycle., Method: A quasi-experimental study with non-paired samples, pre- and post-implementation, conducted in an Intensive Care Unit of a public hospital in southern Brazil, from September 2022 to April 2023, following the guidelines of the Standards for Quality Improvement Reporting Excellence 2.0. Adherence to bedside medication preparation, interruptions during preparation, adequate storage, identification and validity of multidose medications, and recording of storage refrigerator temperature were evaluated. Shapiro-Wilk and Mann-Whitney U tests were used for data analysis, and Carter's Positivity Index was used to determine compliance with observed practices., Results: Forty-five audits were conducted pre-intervention and 122 audits three months after the implementation of the improvement cycle. All variables showed significant improvements. Overall compliance increased from 46% to 80% in the pre- and post-implementation periods, respectively, indicating a transition from "undesirable" to "safe" care stratum., Conclusion: The study revealed a positive relationship between the implementation of a quality improvement cycle focused on medication preparation and improvements in patient safety.

Objective: To verify the characteristics of safety incident reports resulting in moderate and severe harm to pediatric patients in two hospitals during the COVID-19 pandemic., Method: Cross-sectional study conducted in two hospitals in southern Brazil. The sample consisted of 137 notifications from March 2020 to August 2021. The data were collected through the electronic records of the institutions' notification systems and analyzed using descriptive and inferential statistics., Results: The most prevalent incidents were related to clinical processes or procedures (41.6%), affecting slightly more females (49.6%) and infants (39.4%). The majority of incidents (48.2%) occurred in inpatient units. The event sector (p=0.001) and the shift (p=0.011) showed statistically significant associations in both hospitals., Conclusion: The characteristics of the notifications are similar between the institutions surveyed, with a low number of moderate and severe incidents.

Objective: To identify factors associated with umbilical cord clamping in term newborns and to compare the recording of clamping time in the medical record with what was observed., Method: Cross-sectional study, with 300 mothers-infants, in a university hospital. Clamping time and medical records were observed, and a structured questionnaire was applied to postpartum women for sociodemographic variables. Bivariate analysis, multivariate Poisson Regression model, and Kappa concordance test were performed., Results: The percentage of late/optimal clamping observed was 53.7%. The associated factors were skin-to-skin contact in the delivery room (PR = 0.76; 0.61-0.95; p = 0.014), position of the newborn below the vaginal canal (PR = 2.6; CI95%: 1.66-4.07; p < 0.001), position of the newborn at the vaginal level (PR = 2.03; CI95%: 1.5-2.75; p < 0.001), and need for newborn resuscitation in the delivery room (PR = 1.42; CI95%; 1.16-1.73; p = 0.001). Kappa concordance level of the professionals, records compared to the observation was: nurse 0.47, obstetrician 0.59, and pediatrician 0.86., Conclusion: the identification of associated factors and the comparison between recording and observing the clamping time can help in the planning and implementation of improvements for adherence to good practices at birth.

Objective: To evaluate the environment of the professional practice of the nursing staff from inpatient units and to compare it according to professional and work characteristics., Method: A cross-sectional analytical study conducted in southern Brazil. Data was collected through the Brazilian version of the Nursing Work Index-Revised with a sample of (n=142) professionals, them being nurses (n=56; 34.9%) and nursing technicians/assistants (n=86; 60.6%). The analysis was performed by means of descriptive and analytical statistics., Results: The professional practice environment was considered favorable in all the subscales. The subscale of relationships between physicians/nursing staff showed a significant difference among the professional categories, with better results for nurses. The afternoon shift stood out in the favorable assessment. Longer working time at the institution and unit tended to have worse assessments for autonomy and control, respectively., Conclusion: The assessment of the practice environment was favorable; however, the study signals opportunities for improvements for the nursing team.

Objective: To know the main safety incidents reported by relatives of patients hospitalized in pediatric units., Methods: This is a qualitative research with exploratory-descriptive design. Data were collected between April 2016 and December 2017, in three hospitals in Porto Alegre/RS. A total of 91 semi-structured interviews were conducted with relatives of pediatric patients hospitalized in emergencies, infirmaries, and intensive care centers. The statements were transcribed in full and submitted to a thematic content analysis., Results: The category Identification of safety incidents by relatives of hospitalized pediatric patients was formulated. Eight subtopics emerged: drug therapy, hand and environment hygiene, diet therapy, risk of falls, communication, patient identification, care processes/procedures and access to the institution., Conclusions: The family identified safety incidents in various stages of care for hospitalized children. It is necessary to think about strategies that integrate the family in the care of children, to minimize risks and complications resulting from care.

Objective: To describe the safety culture of the patient from the perspective of nurses and physicians working in the maternal-child area., Method: A cross-sectional study conducted from January to September 2018 with 41 professionals of the Obstetrics Center and obstetric hospitalization of a university hospital in the south of the country. The Hospital Survey on Patient Safety Culture was used, with 12 dimensions of the safety culture, measured by means of a general score (0 to 10) and of positive answer percentages to assess strengths and weaknesses., Results: The action of supervisors/bosses can be considered a strength of patient safety, with 78.2% of positive answers; already regarding communication, it was considered a fragility, punctuating 13.24%. The general safety grade of the patient assigned to the work's unit was very good, in a confidence interval of 95%., Conclusion: With the identification of the strengths and weaknesses of patient safety, it is possible to plan improvement actions. We emphasize that the non-punitive approach is essential.

Objective: to analyze patient safety incidents identified by caregivers of hospitalized children., Method: a qualitative, exploratory-descriptive study was carried out with 40 caregivers of children hospitalized in three hospital institutions in the city of Porto Alegre, Rio Grande do Sul State, Brazil, from April to December 2016. Semi-structured, recorded and transcribed interviews were carried out in their entirety, submitted to a thematic analysis using the NVivo 11.0 software., Results: reports related to falls, infant feeding, patient/caregiver identification, medication process, communication, hand hygiene and hygiene of the hospital environment, spread of diseases, relations between caregivers and professionals and care processes/procedures were all cited., Final Considerations: communication and the relations among caregivers and professionals are the main contributory factors for patient safety incidents, interfering with the quality of care. The participation of caregivers and engagement in child care may be strategies to be developed to promote a safety culture.

Objective: To analyze the perception of health professionals and companions/family about the development of communication for patient safety in pediatric hospitalizations., Method: It is an exploratory-descriptive study with a qualitative approach, performed in pediatric clinical-surgical hospitalization units of three hospitals in Porto Alegre, RS, Brazil. 44 health professionals and 94 companions of hospitalized children participated in the study, to a total of 138 participants. Data collection took place between 2016 and 2017, through semi-structured interviews. A thematic content analysis was used., Results: Two categories emerged: "Barriers to an effective communication", addressing the failures and difficulties in the communication process and "Tools to improve communication", that present recommendations for improvements, especially instrumentalization of the companion/family member., Conclusions: The barriers to an effective communication involve several factors, and effective communication strategies can assist in the development of improvements for pediatric patient safety.

Objective: Describe the implementation of care transfer flow chart in postoperative, based on a risk classification model., Method: Experience report on the implementation of a pilot project between the post-anesthetic recovery room and the surgical hospitalization unit, developed between December/2016 and March/2017, aimed at transferring s the care of patients with low risk of postoperative mortality, in a university hospital in the South of Brazil., Results: The project made it possible to expedite the discharge of the patient from the Post-Anesthetic Recovery Room to the surgical hospitalization unit, to qualify the records regarding nursing care and to optimize the time of nurses in both units for care activities., Conclusion: The implementation of a care transfer flow chart from the risk classification for postoperative patients contributed to a more effective communication, culminating in improvements in patient safety.

Objectives: To describe evidence of international literature on the safe care of the hospitalised child after the World Alliance for Patient Safety and list contributions of the general theoretical framework of patient safety for paediatric nursing., Method: An integrative literature review between 2004 and 2015 using the databases PubMed, Cumulative Index of Nursing and Allied Health Literature (CINAHL), Scopus, Web of Science and Wiley Online Library, and the descriptors Safety or Patient safety, Hospitalised child, Paediatric nursing, and Nursing care., Results: Thirty-two articles were analysed, most of which were from North American, with a descriptive approach. The quality of the recorded information in the medical records, the use of checklists, and the training of health workers contribute to safe care in paediatric nursing and improve the medication process and partnerships with parents., Conclusion: General information available on patient safety should be incorporated in paediatric nursing care.

This case study with a qualitative approach presents an analysis of the perceptions of family caregivers/companions and health workers concerning adverse events in care contexts. A total of 15 companions and 23 health workers from pediatric hospitalization units of a university hospital in Porto Alegre, Brazil participated in this study from August to December 2010. Data were collected through semi-structured interviews and analyzed through thematic content analysis using QSR's Nvivo version 7.0. Seven thematic categories emerged from the analysis. The results highlight reviewing and checking workers' interventions and implementing effective communication among workers, caregivers and children. The main recommendations concern changing the organizational culture of health facilities, emphasizing safe care and a systemic view of the evaluation of adverse events.

The possibility of having a companion is a constitutional right guaranteed to children, elderly and parturients. Independently of their stage in the life cycle, having a companion is a situation socially and culturally determined in Brazil. This is a qualitative, descriptive-exploratory and interventionist study that aims to describe and discuss the perceptions of female caregivers of children with cancer while accompanying them at the hospital. Data were collected through a focal group with nine women, between March and May, 2007 at a teaching hospital in Porto Alegre, Rio Grande do Sul, Brazil. The thematic analyses shows the participants' passive and kind behavior in the face of daily adversities, reflection of the power and authority conditions of the institutional objectives, so common in health scenarios in the Brazilian reality. Emancipation could be reached through educative strategies, characterized by information and dissemination of users' rights as well as critical and demanding attitudes from the user when confronted.

This is a qualitative, descriptive, exploratory, and interventionist research carried out with nine female lay caretakers of children undergoing oncologic treatment. The objectives were to present the multiple social roles performed by female lay caretakers of children with cancer and to discuss the caretaker's role in society. The study was carried out between March and April, 2007, at Hospital de Clinicas de Porto Alegre, Rio Grande do Sul, Brazil. The data collection was performed with the focal group technique. The results were examined through the analysis of thematic content, which evidenced the multiple social roles performed by women, the main group of caretakers in society, regardless of the context. Final considerations enhance a discussion in the educational, political and social levels about the redistribution of social roles, particularly family responsibility between men and women. The participation of the nursing staff can take place through the comprehension of those roles imposed by the context and facilitate family inclusion in the care relationship.

This narrative-descriptive review is about the safety/protection of hospitalized children who, due to their fragility, vulnerability and peculiar growth and development conditions need special attention from health professionals. This study aimed to identify knowledge production on safety, protection and violence to hospitalized children between 1997 and 2007. In total, 15 national and international articles were analyzed, using the key words: hospitalized child, safety, violence and nursing. This qualitative approach enabled the development of four categories: adverse occurrences; medication errors; notification of adverse occurrences; and safety of pediatric patients. Results indicate the need to develop strategies to reduce the probability of these events occurring during children's hospitalization, so that they do not suffer any problem neither violation of their fundamental rights.

This study aims to present and discuss part of the findings of a research carried out at a teaching hospital. It is a qualitative descriptive-exploratory and interventionist study with nine lay caregiver women accompanying hospitalized children with cancer. Data were collected through the focal group technique, organized in the QSR Nvivo software and analyzed through thematic analysis. Results originated during the discussion on participants' health conceptions indicate the need to (re)think the rights of patients' companions and provide instruments to several social, political and institutional stakeholders in order to (re)plan health actions that can be developed during professionals' education and qualification in the context of health care, which can be the focus of discussion within diverse contexts of society.

This article aimed to reflect about the importance of the role nurses perform in strategies of health education to lay caregivers, who are important actors in the health/disease process. It is a thematic reflection, built through bibliographic review, which discusses the role of lay caregivers in different stages of the life cycle. Considering the participation of lay caregivers in the Brazilian reality, strategies have been sought to include them in the health care, given the little attention Public Policies have given to them. Therefore, the nurses as health educators, must offer support to the lay caregivers helping them to choose among available alternatives, while delivering care, to not harm their own health. Concluding, health education can be an alternative to offer attention to the lay caregivers, while the nurse has important role in the development of alternatives.

This article partially describes a qualitative phenomenological study on teaching and practice of human care. The study was carried out in the Nursing Course of the Federal University of Rio Grande do Sul from 2004 to 2006. It is theoretically and philosophically based on Martin Heidegger's work. Data was collected through semi-structured interviews, which guiding question was: "How does the professor of the Nursing School insert the theoretical content on human care in his/her discipline"? The phenomenological analysis revealed that professors consider that the theoretical content is developed along with practical teaching, with the interaction with the students, and it is based on the humanization of care.

The object of this study is teaching and practicing human care. It comprises the' third stage of the research on the theme: the challenge of teaching how to care from the point of view of the academic students, nurses and, in this one, the professors of EEUFRGS. It is a qualitative, phenomenological study with Martin Heidegger's theoretical and philosophical support. The purpose of this stage is knowing/understanding how the theoretical and practical teaching of human care occurs, considering that it is the curricular guidance of the graduation course. The phenomenological analysis evidences that the professors initiate the students'foundations regarding care from their daily experiences and from the scientific foundation; holistic care characterizes the human care.
