Diagnosis by rapid antigen tests (RATs) is useful for early initiation of antiviral treatment. Because RATs are easy to use, they can be adapted for self-testing. Several kinds of RATs approved for such use by the Japanese regulatory authority are available from drug stores and websites. Most RATs for COVID-19 are based on antibody detection of the SARS-CoV-2 N protein. Since Omicron and its subvariants have accumulated several amino acid substitutions in the N protein, such amino acid changes might affect the sensitivity of RATs. Here, we investigated the sensitivity of seven RATs available in Japan, six of which are approved for public use and one of which is approved for clinical use, for the detection of BA.5, BA.2.75, BF.7, XBB.1, and BQ.1.1, as well as the delta variant (B.1.627.2). All tested RATs detected the delta variant with a detection level between 7500 and 75 000 pfu per test, and all tested RATs showed similar sensitivity to the Omicron variant and its subvariants (BA.5, BA.2.75, BF.7, XBB.1, and BQ.1.1). Human saliva did not reduce the sensitivity of the RATs tested. Espline SARS-CoV-2 N showed the highest sensitivity followed by Inspecter KOWA SARS-CoV-2 and V Trust SARS-CoV-2 Ag. Since the RATs failed to detect low levels of infectious virus, individuals whose specimens contained less infectious virus than the detection limit would be considered negative. Background: The COVID-19 pandemic has caused many adults with intellectual/developmental disabilities (IDD) to lose their daily routines and social support, and as a result, many adults with IDD are increasingly reliant on their family caregivers. Siblings often play a crucial support role for their brothers and sisters with IDD. As such, this study aimed to describe the experiences of adult siblings of people with IDD during the COVID-19 pandemic., Methods: The Sibling Collaborative worked with researchers to codesign an online survey, completed by 91 people, exploring sibling supports and concerns during the COVID-19 pandemic. The survey also aimed to identify helpful resources for siblings during this time., Results: The results showed that the majority of siblings are supporting their brother or sister with IDD during the COVID-19 pandemic and are concerned about the health and well-being of their brother/sister. The most common concern related to disruption of their brother's or sister's routine and activities. Although responses of older and younger siblings did not differ from each other, siblings whose brother or sister with IDD lived with family had some unique concerns relative to those whose siblings no longer lived with family. Siblings described how their own self-care and relationships with others, as well as support for their brother/sister, were particularly helpful during the COVID-19 pandemic., Conclusions: Siblings are providing key support to their brother or sister with IDD during the COVID-19 pandemic, and they too must be supported. Siblings should be included in efforts to disseminate resources targeting people with IDD and their feedback and input must be obtained. It is also important to include sibling mental wellness as caregiver supports are created and implemented. The systematic review aimed to systematize the empirical literature on the psychological impact of disability on the siblings of persons with disabilities, to inform research and provide clinical recommendations. Two research questions addressed the review: (1) What are the main psychological constructs investigated in siblings of persons with disability? (2) What is the main role of each psychological construct in siblings of persons with disability experience? The electronic search was conducted in 7 databases and the PRISMA diagram was used. The inclusion criteria were: Papers published in English and in peer-reviewed journals; papers published between January 2014 and June 2024; qualitative, quantitative, and mixed studies; and papers on the psychological impact of disabilities and/or chronic illnesses on the experience of siblings of persons with disabilities. The standardized Mixed Method Appraisal Tool protocol was used to appraise the methodological quality of the studies. To summarize the findings, a narrative approach was adopted. A total of 60 studies have been reviewed. According to the methodological quality appraisal of studies, most of them reported a high (n = 45) and medium (n = 15) quality. They involved 10,146 participants. Findings revealed that sibling relationships, sibling-focused parentification, and emotional/behavioral adjustment are the main psychological constructs investigated by existing literature. Few studies focused on siblings' well-being. Studies exploring more than a psychological construct were included as a hybrid. Only one study examined the siblings' psychological experience during the COVID-19 pandemic. The quality of the sibling relationship ranged from good to poor; the caregiver is the main role played by siblings of persons with disabilities; anxiety, depression, and aggressive behaviors are the main emotional/behavioral symptoms revealed. Most studies (n = 39) recruited participants with brothers or sisters with mental disorders. Background: Adult siblings are important in the lives of individuals with profound intellectual disabilities, especially as parents age. However, little is known about the roles they assume., Method: We examined these roles among 58 participants from the Netherlands, who completed an online questionnaire., Results: Most participants (89.7%) assumed multiple roles, and the majority were content with their roles. Between 7% and 58% indicated that they were the only individuals providing a certain role. Shared roles primarily involved healthcare professionals, other siblings, and parents. Almost half the participants (48.2%) assigned higher scores for the amount of support provided than for the experienced burden., Conclusions: Adult siblings often assume multiple roles. We propose a method for the global optimization of stochastic functions. During the course of the optimization, a probability distribution is built up for the location and the value of the global optimum. The concept of information entropy is used to make the optimization as efficient as possible. The entropy measures the information content of a probability distribution, and thus gives a criterion for decisions: From several possibilities we choose the one which yields the most information concerning location and value of the global maximum sought.

Recycling of light allows the luminance (radiance) emitted by a light source to be increased at the cost of reducing the total luminous flux (radiant power). Recycling of light means returning part of the emitted light to the source, where part of it will escape absorption. An optical design that is suitable for multiple and controlled recycling is described. Carambola optics is named for its resemblance to star fruit. Several pairs of mirrors or prisms redirect light repeatedly onto the source, thus achieving multiple transits of the light through the source. This recycled light exits the carambola in the same phase space as light directly emitted and not recycled.

Light guides are employed to homogenize an illumination distribution, to translate cross-sectional beam shapes, and to provide concentration. Microstructures enhance the performance of light guides based on the selective mixing of direction vector components in the two-dimensional phase space (etendue) that is perpendicular to the system's optical axis. For linear concentrators we find that the microstructured light guide beats the two-dimensional concentration limit and roughly allows for a three-dimensional concentration ratio. This result is verified in simulation and experiment.

Our purpose was to investigate the utility of the DNA amplification by ligase chain reaction (LCR) for the direct detection of Mycobacterium tuberculosis complex (MTC) in pleural fluid specimens from patients suspected for tuberculous pleural effusion. We have used the LCx M. tuberculosis kit (Abbott) which uses the amplification of the gene that encodes for antigen b. We have examined 81 pleural fluid specimens by isolation (on Löwenstein-Jensen medium and MB/BacT system) and by LCR. Out of 10 positive specimens in culture, 4 were also positive by LCR; out of 71 negative specimens in culture, 8 were positive by LCR. We have re-evaluated the LCR results according to the clinical diagnosis, sustained by the successful therapy, and to the pathological diagnosis on the pleural biopsy. The sensitivity and specificity of LCR in the diagnosis of tuberculous pleural effusion were 31.5% and 100%. This commercial LCR kit is a rapid, specific, but less sensitive test for the routine diagnosis of the tuberculous pleural effusion.

The changes in the public health system in Germany have led to an increasing number of in-house patients in clinics for psychotherapy, whose attitudes towards their disorders can be described as medically orientated and who are less motivated towards psychotherapy. To integrate these patients with low motivation or little experience and knowledge of behavior therapy, therapists of the Fachklinik Bad Dürkheim developed a special program for group therapy ("Psychosomatic Group Therapy", PSG). The goals of this program, which consists of 10 sessions, are: 1. Teaching the basics of psychosomatic medicine and behavioral therapy, 2. imparting a bio-psycho-social model of psychosomatic disorders, and 3. initiating and supporting basic capabilities of self management, thereby increasing the motivation for behavior therapy and reducing the symptoms of the patients. This empirical study follows a one-group pre-post design. The results show several positive outcomes of the program: In the eyes of the patients, the sessions themselves and the exercises between the sessions ("homework") are didactically excellent and personally relevant. The patients' knowledge about and their attitudes towards psychosomatics change in a positive direction. Furthermore, the motivation for psychotherapy increases and general improvements in physical and psychological symptoms can be shown. However, these changes in motivation do not correlate with changes in knowledge of and attitudes towards psychotherapy. The reasons for these results are critically discussed.

Freeform optical surfaces embedded in three-dimensional space, without any symmetry, are tailored so as to solve the archetypal problem of illumination design: redistribute the radiation of a given small light source onto a given reference surface, thus achieving a desired irradiance distribution on that surface. The shape of the optical surface is found by solving a set of partial nonlinear differential equations. For most cases, a few topologically distinct solutions exist, given suitable boundary conditions.

An optical measurement method for nonimaging radiation concentrators is proposed. A Lambertian light source is placed in the exit aperture of the concentrator. Looking into the concentrator's entrance aperture from a remote position, one can photograph the transmission patterns. The patterns show the transmission of radiation through the concentrator with the full resolution of the four-dimensional phase space of geometric optics. By matching ray-tracing simulations to the measurement, one can achieve detailed and accurate information about the geometry of the concentrator. This is a remote, noncontact measurement and can be performed in situ for installed concentrators. Additional information regarding small-scale reflector waviness and surface reflectivity can also be obtained from the same measurement with additional analysis.

For technical reasons, large three-dimensional compound parabolic concentrators (CPC's) are often built from facets with either no or only one-dimensional curvature. We analyze CPC approximations made with various numbers of axial and circumferential subdivisions. Incident radiation within half-angles of 10 degrees and 30 degrees is considered. The reflectivity of the mirrors is assumed to be 90% or 95%. The performance of faceted concentrators can be significantly improved by optimization as compared with heuristic CPC approaches. The highest increase in transmission that we observed was 19% greater as compared with that of a heuristic CPC approximation. The shapes of the optimized concentrators differ from that of a classic CPC, and most of the optimized concentrators are longer than a classic CPC. For practical concentrators with a small number of facets, the optimized geometry provides better performance than a heuristic approximation of the CPC shape.

Designs for flexible, high-power-density, remote irradiation systems are presented. Applications include industrial infrared heating such as in semiconductor processing, alternatives to laser light for certain medical procedures, and general remote high-brightness lighting. The high power densities in herent to the small active radiating regions of conventional metal-halide, halogen, xenon, microwave-sulfur, and related lamps can be restored with nonimaging concentrators with little loss of power. These high fluxlevels can then be transported at high transmissivity with light channels such as optical fibers or lightpipes, and reshaped into luminaires that can deliver prescribed angular and spatial flux distributions onto desired targets. Details for nominally two- and three-dimensional systems are developed, along with estimates ofoptical performance.

The maximum concentration of radiation is proportional to the square of the refractive index of the medium in which it propagates. A medium with a high refractive index can also serve as a lightguide for concentrated radiation. However, if concentrated radiation is extracted from one medium, with a high refractive index, to another, whose index is lower (e.g., from fused silica into air), part of the radiation may be lost because of the total internal reflection at the interface. We present polygonal shapes suitable for efficient extraction of the concentrated radiation in a controllable way, without increasing the cross-section area (or diameter) of the lightguide. It is shown analytically and experimentally that the use of a secondary concentrator, followed by such a light extractor, both having a high refractive index, can provide considerably more power to a solar receiver with a specific aperture.

For parabolic trough solar collectors with tubular absorbers, we design new tailored secondary concentrators. The design is applicable for any rim angle of a parabolic reflector. With the secondary, the concentration can be increased by a factor of more than 2 with a compact secondary reflector consisting of a single piece, even for the important case of a rim angle of 90 deg. The parabolic reflector can be used without changes; the reduced absorber is still tubular but smaller than the original absorber and slightly displaced toward the primary.

For both linear and point-focus Fresnel reflectors, we present a new type of ideal nonimaging secondary concentrator, the tailored edge-ray concentrator, that can closely approach the thermodynamic limit of concentration. For large rim-angle heliostat fields, practical-sized secondaries with shapes that should be relatively easy to fabricate can achieve concentrations substantially above those of compound parabolic concentrators. This superiority stems from designing so as to accommodate the particular flux from the heliostat field. The edge-ray principle used for generating the new secondary dictates a heliostat tracking strategy that is different from the conventional one but is equally easy to implement.

The emergence of COVID-19 in 2020 significantly enhanced the application of wastewater monitoring for detecting SARS-CoV-2 circulation within communities. From October 2021 to October 2022, we collected 406 wastewater samples weekly from the Córdoba Central Pipeline Network (BG-WWTP) and six specific sewer manholes from sanitary zones (SZs). Following WHO guidelines, we processed samples and detected SARS-CoV-2 RNA and variants using real-time PCR. Monitoring at the SZ level allowed for the development of a viral activity flow map, pinpointing key areas of SARS-CoV-2 circulation and tracking its temporal spread and variant evolution. Our findings demonstrate that wastewater-based surveillance acts as a sensitive indicator of viral activity, detecting imminent increases in COVID-19 cases before they become evident in clinical data. This study highlights the effectiveness of targeted wastewater monitoring at both municipal and SZ levels in identifying viral hotspots and assessing community-wide circulation. Polyphasic approach has become a generally accepted method for the classification of cyanobacteria. In this study, we present a detailed characterisation of two strains, KUCC C1 and KUCC C2, isolated from the Curonian Lagoon and classified to the Aphanizomenon genus. Despite phylogenetic similarity, the strains differ with respect to morphology, ultrastructure characteristics, and the metabolite profile. In the KUCC C1 extract, three unknown peptides and eight anabaenopeptins were detected, while KUCC C2 produced one unknown peptide and one aeruginosin. In both strains, a total of eleven pigments were detected. The production of myxoxantophyll, chlorophyll-a, chlorophylide-a, and zeaxanthin was higher in KUCC C2 than in KUCC C1. Extracts from both strains of Aphanizomenon also had different effects in antibacterial, anticancer and enzyme inhibition assays. Comprehensive analyses of Aphanizomenon strains performed in this study showed significant diversity between the isolates from the same bloom sample. Chaperones safeguard protein homeostasis by promoting folding and preventing aggregation. HSP110 is a cytosolic chaperone that functions as a nucleotide exchange factor for the HSP70 cycle. Together with HSP70 and a J-domain protein (JDP), HSP110 maintains protein folding and resolubilizes aggregates. Interestingly, HSP110 is vital for the HSP70/110/JDP-mediated disaggregation of amyloidogenic proteins implicated in neurodegenerative diseases (i.e., α-synuclein, HTT, and tau). However, despite its abundance, HSP110 remains still an enigmatic chaperone, and its functional spectrum is not very well understood. Of note, the disaggregation activity of neurodegenerative disease-associated amyloid fibrils showed both beneficial and detrimental outcomes in vivo. To gain a more comprehensive understanding of the chaperone HSP110 in vivo, we analyzed its role in neuronal proteostasis and neurodegeneration in C. elegans. Specifically, we investigated the role of HSP110 in the regulation of amyloid beta peptide (Aβ) aggregation using an established Aβ-C. elegans model that mimics Alzheimer's disease pathology. We generated a novel C. elegans model that over-expresses hsp-110 pan-neuronally, and we also depleted hsp-110 by RNAi-mediated knockdown. We assessed Aβ aggregation in vivo and in situ by fluorescence lifetime imaging. We found that hsp-110 over-expression exacerbated Aβ aggregation and appeared to reduce the conformational variability of the Aβ aggregates, whereas hsp-110 depletion reduced aggregation more significantly in the IL2 neurons, which marked the onset of Aβ aggregation. HSP-110 also plays a central role in growth and fertility as its over-expression compromises nematode physiology. In addition, we found that HSP-110 modulation affects the autophagy pathway. While hsp-110 over-expression impairs the autophagic flux, a depletion enhances it. Context: In the renewable industry, pressure-dependent CsPbBr 3 perovskite has a lot of potential due to its exceptional properties. Present work revealed the mechanical stability of CsPbBr 3 between 0 to 50 GPa. The bandgap of unstressed CsPbBr 3 is 2.90 eV, indicating a direct bandgap. Band gap values decrease by increasing external pressure. CsPbBr 3 structure showed a direct band gap from 0 to 35 GPa and in-direct from 40 to 50 GPa. The unit cell volume and lattice constants are substantially decreased. Mechanical parameters, i.e., Young's modulus, bulk modulus, anisotropy factor, shear modulus, and poison's ratio are obtained. Under ambient conditions, the mechanical properties of CsPbBr 3 showed ductile behavior and with induced pressure, their ductility has significantly improved. By applying stresses ranging from 0 to 50 GPa, the considerable fluctuation in values of dielectric function (imaginary and real), absorption, reflectivity, loss function, refractive index (imaginary and real), and conductivity (imaginary and real), was also identified. When pressure rises, the optical parameters increase and drag in the direction of high energies. Response functions are used to predict the density of states and the phonon lattice dispersion to study the phonon properties. By using the quasi-harmonic Debye model, the thermal effect on the free energy, entropy, enthalpy, and heat capacity were predicted and compared. These results would be useful for theoretical research and indicate how external pressure significantly affects the physical characteristics of CsPbBr 3 perovskites, which may open up new possibilities for use in optoelectronic, photonic, and solar cell applications., Methods: The structural, electrical, mechanical, optical, and thermal properties of cesium lead bromide (CsPbBr 3 ) are investigated by applying external pressure from 0 to 50 GPa, using generalized gradient approximations (GGA) and Perdew-Burke-Ernzerhof (PBE) with CASTEP code built-in material studio by density functional theory (DFT)., (© 2024. Young boys and girls when opening their mouth show an involuntary movement of one or both hands, i.e. slight flexion with maximal extension of fingers. The highest incidence of this "hand-stretching reflex" (HSR) is found in the age group of 4 to 5. Below this age more girls than boys are found with a positive HSR, whereas at the age of eleven girls are outnumbered by boys. Beyond this age the reflex disappears. Lefthandedness and leftsided reflex seem to correspond and vice versa. Lack of this reflex in the age group of 4 to 5 as well as persistence over the age of eleven seems to indicate retarded cerebral maturation.

Background: During the transition to adulthood, a common challenge that youth with a neurodisability may experience is learning how to navigate services in the adult care system. During this transition youth may rely on their families, including siblings, for support. However, delineation of sibling roles and responsibilities during this transition period are unclear. This study aims to identify the roles and responsibilities that siblings perceive to have with their sibling with a neurodisability during the transition to adulthood, and describe the decision-making process of how siblings chose these roles., Methods: In this descriptive qualitative case study, siblings were eligible to participate if they were between 14 to 40 years old, had a sibling between 14 to 21 years with a childhood-onset neurodisability and spoke English. Semi-structured interviews augmented by techniques of photo elicitation and relational maps were conducted. Reflexive thematic analysis was applied to identify sibling roles, as well as the emotional and decision-making process associated with these roles. Our team partnered with siblings with lived experience in all study phases., Results: Nineteen participants (median age = 19 years, range = 14 to 33 years) from 16 unique families were interviewed. Six unique roles were described: friend, role model/mentor, protector, advocate, supporter, or caregiver. The emotions that siblings experienced with each role, also known as emotional responsibility, were categorized into levels of low, medium or high. Siblings also described a four-phase decision-making process for their roles: (1) acquiring knowledge; (2) preparing plans; (3) making adjustments; and (4) seeking support. Intrapersonal characteristics, including personal identity, values and experiences, influenced roles assumed by siblings., Conclusions: Siblings identified needing support as they process their decisions and emotional responsibility in their roles when their sibling with a neurodisability is transitioning to adulthood. Natural products with the 3,6-diene-2,5-diketopiperazine core are widely distributed in nature; however, the biosynthetic mechanism of 3,6-diene-2,5-diketopiperazine in fungi remains to be further elucidated. Through heterologous expression and biochemical investigation of an Fe II /2-oxoglutarate-dependent oxidase (AspE) and a heme-dependent P450 enzyme (AspF), we report that AspE, AspF and subsequent dehydration account for the formation of the 3,6-diene-2,5-diketopiperazine substructure of brevianamide K from Aspergillus sp. SK-28, a symbiotic fungus of mangrove plant Kandelia candel. More interestingly, in-depth investigation of the enzymatic mechanism showed that AspE promotes hydroxylation of brevianamide Q with unprecedented stereoinversion through hydrogen atom abstraction and water nucleophilic attack from the opposite face of the resultant iminium cation intermediate., (© 2023 Wiley-VCH GmbH.)

Background: The COVID-19 pandemic poses risks to the quality of life (QoL) of relatives of individuals with intellectual disabilities. This paper investigates relatives' QoL and associated risk and buffering factors before and during the pandemic., Method: Surveys were administered to three samples of relatives in the Netherlands in 2019 (N = 105) and during the first waves of COVID-19 in June (N = 207) and October 2020 (N = 332). Associations between QoL and care burden, care competence, social support, and resilience, and changes over time were analysed using (logistic) regressions., Results: No significant changes in overall QoL nor its domains were found. Care burden was negatively associated with QoL and increased during the pandemic. Care competence was lower than in 2019. Competence, social support, and resilience were positively associated with QoL., Conclusions: Although relatives' QoL remained stable, the pandemic poses non-negligible risks to their wellbeing. Teenage siblings of children with autism spectrum disorder (ASD) are at risk of worse mental health outcomes than their peers, yet there have been few interventions focused on improving their psychosocial wellbeing. This study explored the acceptability of an 8-session virtual group mind-body resiliency intervention for teen siblings of children with ASD. We used mixed methods to assess quantitative and qualitative survey results. Participants reported that the intervention had the right amount of sessions (88%), structure (74%), and duration (89%). Most participants felt comfortable during sessions (74%), found it helpful to learn mind-body exercises (74%), and that the intervention helped in coping with stress (71%). Though participants were satisfied with the opportunity to meet peers, they desired more social connection., (© 2022. Natural and artificial autonomous molecular machines operate by constantly dissipating energy coming from an external source to maintain a non-equilibrium state. Quantitative thermodynamic characterization of these dissipative states is highly challenging as they exist only as long as energy is provided. Here we report on the detailed physicochemical characterization of the dissipative operation of a supramolecular pump. The pump transduces light energy into chemical energy by bringing self-assembly reactions to non-equilibrium steady states. The composition of the system under light irradiation was followed in real time by 1 H NMR for four different irradiation intensities. The experimental composition and photon flow were then fed into a theoretical model describing the non-equilibrium dissipation and the energy storage at the steady state. We quantitatively probed the relationship between the light energy input and the deviation of the dissipative state from thermodynamic equilibrium in this artificial system.
