Introduction: The prevalence of chronic conditions is increasing worldwide. The objective of this study was to describe the trajectory of the occurrence of multiple chronic conditions during 6 years of follow-up and investigate their association with demographic, socioeconomic, and behavioral health characteristics of older adults in Southern Brazil., Methods: We used data from a longitudinal study (the Como Vai? study) of noninstitutionalized adults aged 60 or older living in the urban area of Pelotas, Rio Grande do Sul. We assessed the number of chronic conditions based on a list of 24 conditions in 3 interviews, conducted in 2014, 2016-2017, and 2019-2020. We used group-based semiparametric modeling to identify groups of participants based on the number of chronic conditions. For associations with participant characteristics, we performed multinomial logistic regression and considered a low, moderate, and high burden of chronic conditions., Results: Of the 1,451 older adults in the cohort, 1,098 (75.7%) were included in analysis. Almost one-third (30.9%) had a low burden (2.3 conditions), more than half (52.0%) had a moderate burden (5.6 conditions), and 17.1% had a high burden (9.7 conditions). Men (relative risk [RR] = 6.10; 95% CI, 3.64-10.22), those aged 80 years or older (RR = 2.33; 95% CI, 1.15-4.72), those with no education (RR= 4.78; 95% CI, 2.19-10.45), and former smokers (RR = 1.53; 95% CI, 0.96-2.44) had a higher risk of being classified in the high-burden group than in the low-burden group., Conclusion: Most older adults belonged to the group with a moderate number of chronic conditions. Several sociodemographic characteristics were associated with belonging to the trajectory with a greater number of conditions.

Brazil is characterized by an unfinished agenda of health inequalities, which impact health problems in the childhood. This study aimed to evaluate the socioeconomic inequalities of health problems in the early childhood. This is a prospective study, using data from the birth cohort carried out in the city of Pelotas (Rio Grande do Sul State, Brazil) in 2015. The outcomes were health problems presented at 12 and 24 months: cough, breathing difficulty, diarrhea, ear pain, pneumonia, urinary infection, hospitalization, and other health problems. Socioeconomic inequalities were measured applying the slope index of inequality (SII) and the concentration index (CIX), with wealth index and maternal schooling being the socioeconomic variables. The inequalities in the number of health problems were evaluated by Poisson regression. The perinatal sample comprised 4,275 children. At 12 months approximately 74% of the children presented 1 or more health problems, while at 24 months, approximately 44% presented 2 or more health problems. For all period, the mean number of health problems was 2.9 (standard deviation = 2.0). Higher frequencies were observed for children belonging to the poorest income quintile and with lower maternal education, except for 1 or more health problems at 24 months. The greatest absolute and relative inequality was observed for 2 or more health problems at 12 months (SII: -0.23, 95%CI: -0.29; -0.18 and CIX: -0.19, 95%CI: -0.25; -0.14). There is an opposite dose-response relation for the risk of accumulation of health problems according to maternal schooling (1.07, 95%CI: 1.04; 1.09) and wealth categories (1.03, 95%CI: 1.01; 1.04), in the full adjusted models. The study confirms inequalities due to health problems in Brazilian children, especially in the first year of life.

Aim: Machine learning may be a tool with the potential for obesity prediction. This study aims to review the literature on the performance of machine learning models in predicting obesity and to quantify the pooled results through a meta-analysis., Data Synthesis: A systematic review and meta-analysis were conducted, including studies that used machine learning to predict obesity. Searches were conducted in October 2023 across databases including LILACS, Web of Science, Scopus, Embase, and CINAHL. We included studies that utilized classification models and reported results in the Area Under the ROC Curve (AUC) (PROSPERO registration: CRD42022306940), without imposing restrictions on the year of publication. The risk of bias was assessed using an adapted version of the Transparent Reporting of a multivariable prediction model for individual Prognosis or Diagnosis (TRIPOD). Meta-analysis was conducted using MedCalc software. A total of 14 studies were included, with the majority demonstrating satisfactory performance for obesity prediction, with AUCs exceeding 0.70. The random forest algorithm emerged as the top performer in obesity prediction, achieving an AUC of 0.86 (95%CI: 0.76-0.96; I 2 : 99.8%), closely followed by logistic regression with an AUC of 0.85 (95%CI: 0.75-0.95; I 2 : 99.6%). The least effective model was gradient boosting, with an AUC of 0.77 (95%CI: 0.71-0.82; I 2 : 98.1%)., Conclusion: Machine learning models demonstrated satisfactory predictive performance for obesity. Background: Current clustering of multimorbidity based on the frequency of common disease combinations is inadequate. We estimated the causal relationships among prevalent diseases and mapped out the clusters of multimorbidity progression among them., Methods: In this cohort study, we examined the progression of multimorbidity among 190 diseases among over 500,000 UK Biobank participants over 12.7 years of follow-up. Using a machine learning method for causal inference, we analyzed patterns of how diseases influenced and were influenced by others in females and males. We used clustering analysis and visualization algorithms to identify multimorbidity progress constellations., Results: We show the top influential and influenced diseases largely overlap between sexes in chronic diseases, with sex-specific ones tending to be acute diseases. Patterns of diseases that influence and are influenced by other diseases also emerged (clustering significance P au  > 0.87), with the top influential diseases affecting many clusters and the top influenced diseases concentrating on a few, suggesting that complex mechanisms are at play for the diseases that increase the development of other diseases while share underlying causes exist among the diseases whose development are increased by others. Bi-directional multimorbidity progress presents substantial clustering tendencies both within and across International Classification Disease chapters, compared to uni-directional ones, which can inform future studies for developing cross-specialty strategies for multimorbidity. Finally, we identify 10 multimorbidity progress constellations for females and 9 for males (clustering stability, adjusted Rand index >0.75), showing interesting differences between sexes., Conclusion: Our findings could inform the future development of targeted interventions and provide an essential foundation for future studies seeking to improve the prevention and management of multimorbidity., (© 2024. Background: In ageing populations, multimorbidity is a complex challenge to health systems, especially when the individuals have both mental and physical morbidities. Although a regular source of primary care (RSPC) is associated with better health outcomes, its relation with health service utilisation in elderly patients with mental-physical multimorbidity (MP-MM) is scarce., Objective: This study explored the relations among health service utilisation, presence of RSPC and MP-MM among elderly Brazilians., Methods: A national cross-sectional study performed with data from national representative samples from the Brazilian National Health Research (PNS, in Portuguese; Pesquisa Nacional de Saúde) carried out in 2013 with 11,177 elderly Brazilian people. MP-MM was defined as the presence of two or more morbidities, including at least one mental morbidity, and was evaluated using a list of 16 physical and mental morbidities. The RSPC was analysed by the presence of regular font of care in primary care and health service utilisation according to the demand for health services ≤ 15 days, medical consultation ≤ 12 months, and hospitalisation ≤ 1 year. Frequency description of variables and bivariate association were performed using Stata v.15.2 software., Results: The majority of individuals was female (56.4%), and their mean age was 69.8 years. The observed prevalence of MP-MM was 12.2%. Individuals with MP-MM had higher utilisation of health services when compared to those without MP-MM. RSPC was present at 36.5% and was higher in women (37.8% vs. 34.9%). There was a lower occurrence of hospitalisation ≤ 1 year among MP-MM individuals with RSPC and without a private plan of health., Conclusion: Our findings demonstrate that RSPC can be an important component of care in elderly individuals with MP-MM because it was associated with lower occurrence of hospitalisation, mainly in those that have not a private plan of health. Objective: To assess association between multimorbidity and use of health services in a population diagnosed with COVID-19, in southern Brazil., Methods: This was a cross-sectional study with data from a longitudinal study carried out in the city of Rio Grande, Rio Grande do Sul, Brazil, in 2021 with all adult individuals diagnosed with COVID-19; descriptive analyses were performed and presented as proportions with 95% confidence intervals (95%CI); Poisson regression was performed and reported as prevalence ratios (PR) in order to assess association between multimorbidity (3 or more diseases) and healthcare service use., Results: In total, 2,919 participants were included, of which 40.4% had multimorbidity (≥ 2 diseases); the adjusted results showed that individuals with multimorbidity were more likely to use most of the services assessed, PR = 3.21 (95%CI 1.40;7.37), for Emergency Rooms., Conclusion: Multimorbidity was associated with using different types of health services.

This study aimed to describe the prevalence of demand for health services among Brazilian adolescents and to investigate its association with contextual characteristics of the territory. Study with data from the Brazilian National Health Survey, conducted in 2019, including 43,774 individuals aged from 10 to 19 years. Adolescent's information was obtained through a proxy resident of 18 years or more who answered for all the residents of the household. Poisson regression was used to assess the demand for health services according to geopolitical region, economic status, and type of municipality. The interaction of the health insurance was also evaluated in these associations. Of the total, 11.7% (95%CI: 11.1; 12.3) of the adolescents sought health services in the two weeks prior to the survey. Greater figures of demand were observed in the Southeast (PR = 1.32; 95%CI: 1.15; 1.52) and South regions (PR = 1.31; 95%CI: 1.13; 1.52) compared to the Northern Region of Brazil. Having a health insurance increased the demand for services by adolescents living in rural areas and in capitals and the municipalities of the Metropolitan Areas and/or Integrated Development Regions. The study showed a low prevalence of demand for health services among adolescents and contextual inequalities for the geopolitical region. Having a health insurance was an important marker to understand the disparities in the economic status and in the type of municipality.

To investigate the prevalence of multimorbidity and complex multimorbidity and their association with sociodemographic and health variables in individuals with severe obesity. This is a baseline data analysis of 150 individuals with severe obesity (body mass index ≥ 35.0 kg/m 2 ) aged 18-65 years. The outcomes were multimorbidity and complex multimorbidity. Sociodemographic, lifestyle, anthropometric and self-perceived health data were collected. Poisson multiple regression was conducted to identify multimorbidity risk factors. The frequency of two or more morbidities was 90.7%, three or more morbidities was 76.7%, and complex multimorbidity was 72.0%. Living with four or more household residents was associated with ≥ 3 morbidities and complex multimorbidity. Fair and very poor self-perceived health was associated with ≥ 2 morbidities, ≥ 3 morbidities and complex multimorbidity. A higher BMI range (45.0-65.0 kg/m 2 ) was associated with ≥ 2 morbidities and ≥ 3 morbidities. Anxiety (82.7%), varicose veins of lower limbs (58.7%), hypertension (56.0%) were the most frequent morbidities, as well as the pairs and triads including them. The prevalence of multimorbidity and complex multimorbidity in individuals with severe obesity was higher and the risk for multimorbidity and complex multimorbidity increased in individuals living in households of four or more residents, with fair or poor/very poor self-perceived health and with a higher BMI., (© 2023. Background: Although the association between multimorbidity (MM) and hospitalisation is known, the different effects of MM patterns by age and sex in this outcome needs to be elucidated. Our study aimed to analyse the association of hospitalisations' variables (occurrence, readmission, length of stay) and patterns of multimorbidity (MM) according to sex and age., Methods: Data from 8.807 participants aged ≥ 50 years sourced from the baseline of the Brazilian Longitudinal Study of Ageing (ELSI-Brazil) were analysed. Multimorbidity was defined as ≥ 2 (MM2) and ≥ 3 (MM3) chronic conditions. Poisson regression was used to verify the association between the independent variables and hospitalisation according to sex and age group. Multiple linear regression models were constructed for the outcomes of readmission and length of stay. Ising models were used to estimate the networks of diseases and MM patterns., Results: Regarding the risk of hospitalisation among those with MM2, we observed a positive association with male sex, age ≥ 75 years and women aged ≥ 75 years. For MM3, there was a positive association with hospitalisation among males. For the outcomes hospital readmission and length of stay, we observed a positive association with male sex and women aged ≥ 75 years. Network analysis identified two groups that are more strongly associated with occurrence of hospitalisation: the cardiovascular-cancer-glaucoma-cataract group stratified by sex and the neurodegenerative diseases-renal failure-haemorrhagic stroke group stratified by age group., Conclusion: We conclude that the association between hospitalisation, readmission, length of stay, and MM changes when sex and age group are considered. This study aimed to review and quantify the association between overweight and obesity in the risk of multimorbidity among the general population. We conducted a systematic review and meta-analysis in the databases of Pubmed, Lilacs, Web of Science, Scopus, and Embase. We included cohort studies that assessed the association between overweight and/or obesity with the risk of multimorbidity. The Newcastle-Ottawa assessed the studies' individual quality. A random-effect model meta-analysis was performed to evaluate the association between overweight and obesity with the relative risk (RR) of multimorbidity; the I 2 test evaluated heterogeneity. After excluding duplicates, we found 1.655 manuscripts, of which eight met the inclusion criteria. Of these, seven (87.5%) evidenced an increased risk of multimorbidity among subjects with overweight and/or obesity. Overall, we observed an increased risk of multimorbidity among subjects with overweight (RR: 1.26; CI95%: 1.12; 1.40, I 2  = 98%) and obesity (RR: 1.99; CI95%: 1.45;2.72, I 2  = 99%) compared to normal weight. According to the I 2 test, the heterogeneities of the meta-analyses were high. The Newcastle-Ottawa scale showed that all studies were classified as high quality. Objective  To analyze the association of sleep duration and use of sleeping medication with multimorbidity. Materials and Methods  We conducted a cross-sectional study using data from the Prospective Study about Mental and Physical Health (PAMPA) cohort. Multimorbidity was defined as the presence of two or more conditions from a list of twelve health problems. Descriptive analyses were performed considering proportion and its 95% confidence interval (95%CI). We performed logistic regression (to obtain odds ratios, ORs) to estimate the associations, including models adjusted for confounding factors. Results  In total, 2,936 participants were included, 79,1% of them women, 54.2% aged between 18 and 39 years, and 88.9% with white skin color. Compared with regular sleep (seven to eight hours a day), five hours or less of sleep increased the odds of multimorbidity by 145% (95%CI: 1.90-3.14), and 9 hours or more of sleep increased the odds by 49% (95%CI: 1.14-1.95) for the crude model; the results remained significant even in the adjusted models. Discussion  Consumption of sleeping medication was associated with multimorbidity. Short and prolonged sleep duration increased the odds of multimorbidity, regardless of the sociodemographic and behavior characteristics. The regular use of sleeping medication was also associated with multimorbidity. Objetivo: To describe the initial baseline results of a population-based study, as well as a protocol in order to evaluate the performance of different machine learning algorithms with the objective of predicting the demand for urgent and emergency services in a representative sample of adults from the urban area of Pelotas, Southern Brazil., Methods: The study is entitled "Emergency department use and Artificial Intelligence in PELOTAS (RS) (EAI PELOTAS)" (https://wp.ufpel.edu.br/eaipelotas/). Between September and December 2021, a baseline was carried out with participants. A follow-up was planned to be conducted after 12 months in order to assess the use of urgent and emergency services in the last year. Afterwards, machine learning algorithms will be tested to predict the use of urgent and emergency services over one year., Results: In total, 5,722 participants answered the survey, mostly females (66.8%), with an average age of 50.3 years. The mean number of household people was 2.6. Most of the sample has white skin color and incomplete elementary school or less. Around 30% of the sample has obesity, 14% diabetes, and 39% hypertension., Conclusion: The present paper presented a protocol describing the steps that were and will be taken to produce a model capable of predicting the demand for urgent and emergency services in one year among residents of Pelotas, in Rio Grande do Sul state.

The aim of this review article was to evaluate the association between the intake of ultra-processed foods and sleep-related outcomes through a systematic review and meta-analysis. Pubmed, LILACS, Scielo, Scopus, Embase, and Web of Science were searched on December 31, 2021, for studies that evaluated the association between ultra-processed foods and sleep-related outcomes (self-reported sleep duration and quality). Pooled odds ratios were assessed through a random-effects model; heterogeneity was evaluated using the I 2 statistic. Fifteen cross-sectional studies were included; 14 showed that the high intake of ultra-processed foods was statistically significantly associated with sleep-related outcomes (sleep duration and quality). In the crude analysis, compared with low intake, high intake of ultra-processed foods increased the odds of sleep-related outcomes, with increased odds among children and/or adolescents, and null results among adults. When adjusted for cofounders, we found statistically significant results for all ages. The high intake of ultra-processed foods was associated with sleep-related outcomes, with moderate credibility of the evidence. Objective: This study evaluated the relationship between physical activity (PA) and multimorbidity in community-dwelling older adults., Data Source: A systematic review and meta-analysis in the following databases: Pubmed, Lilacs, Scielo, Web of Science, Scopus, and AgeLine., Study Inclusion and Exclusion Criteria: It included observational studies investigating the association between physical activity and multimorbidity, with older adults, published until May 2021. Studies with institutionalized individuals or that assessed specific diseases were excluded., Data Extraction: Two reviewers independently extracted the studies based on previous inclusion and exclusion criteria, started by selecting titles, followed by abstracts and full-text reading., Data Synthesis: Meta-analysis results were reported as Odds Ratio (OR) with a 95% confidence interval using R language. The Newcastle Ottawa scale was used to assess the quality of the studies., Results: Fifteen studies were included in the systematic review, from which 12 reported an inverse association between physical activity and multimorbidity. In the meta-analysis, from over 77 000 older adults, there was an inverse association between physical activity and multimorbidity [OR: .81; 95% CI: .73-.89]. We found significant results only for men in the analysis by sex., Conclusions: Low levels of physical activity were associated with a higher risk of multimorbidity in older adults. It is expected that public policies will be conducted aimed at the practice of physical activity among older adults.

Objectives: This study aimed to evaluate the existence of socioeconomic inequalities related to the prevalence of multimorbidity in the Brazilian population aged 60 and older., Methods: This was a cross-sectional study with data from the last Brazilian National Health Survey (PNS) collected in 2019. Multimorbidity was the dependent variable and was defined as the presence of two or more chronic diseases. All the diseases were assessed based on a self-reported previous medical diagnosis. Education and per capita family income were the measures of socioeconomic position. Socioeconomic inequalities related to multimorbidity were assessed using two complex measures of inequality; the Slope Index of Inequality (SII) and the Concentration Index (CI)., Results: The prevalence of multimorbidity in Brazil was 56.5% 95% CI (55.4; 57.6) and varied from 46.9% (44.3; 49.6) in the North region to 59.3% (57.0; 61.5) in the South region. In general, individuals with higher socioeconomic positions had a lower prevalence of multimorbidity. Significant absolute and relative income inequalities were observed in the South region [SII -9.0; CI -0.054], Southeast [SII -9.8; CI -0.06], and Middle-east [SII -10.4; CI -0.063]. Absolute and relative education inequalities were significant for the country and two of its regions (Southeast [SII -12.7; CI -0.079] and South [SII -19.0; CI -0.109])., Conclusions: The prevalence of multimorbidity is high in Brazil and all of its macro-regions. Background: The consumption of some food groups is associated with the risk of diabetes. However, there is no evidence from meta-analysis which evaluates the consumption of ultra-processed products in the risk of diabetes. This study aimed to review the literature assessing longitudinally the association between consumption of ultra-processed food and the risk of type 2 diabetes and to quantify this risk through a meta-analysis., Methods: We conducted a systematic review and meta-analysis with records from PubMed, Latin American and Caribbean Literature in Health Sciences (LILACS), Scielo, Scopus, Embase, and Web of Science. We included longitudinal studies assessing ultra-processed foods and the risk of type 2 diabetes. The review process was conducted independently by two reviewers. The Newcastle Ottawa scale assessed the quality of the studies. A meta-analysis was conducted to assess the effect of moderate and high consumption of ultra-processed food on the risk of diabetes., Results: In total 2272 records were screened, of which 18 studies, including almost 1.1 million individuals, were included in this review and 72% showed a positive association between ultra-processed foods and the risk of diabetes. According to the studies included in the meta-analysis, compared with non-consumption, moderate intake of ultra-processed food increased the risk of diabetes by 12% [relative risk (RR): 1.12; 95% confidence interval (CI): 1.06-1.17, I2 = 24%], whereas high intake increased risk by 31% (RR: 1.31; 95% CI: 1.21-1.42, I2 = 60%)., Conclusions: The consumption of ultra-processed foods increased the risk for type 2 diabetes as dose-response effect, with moderate to high credibility of evidence., (© The Author(s) 2021; all rights reserved. In aging populations, multimorbidity (MM) is a significant challenge for health systems, however there are scarce evidence available in Low- and Middle-Income Countries, particularly in Brazil. A national cross-sectional study was conducted with 11,177 Brazilian older adults to evaluate the occurrence of MM and related clusters in Brazilians aged ≥ 60 years old. MM was assessed by a list of 16 physical and mental morbidities and it was defined considering ≥ 2 morbidities. The frequencies of MM and its associated factors were analyzed. After this initial approach, a network analysis was performed to verify the occurrence of clusters of MM and the network of interactions between coexisting morbidities. The occurrence of MM was 58.6% (95% confidence interval [CI]: 57.0-60.2). Hypertension (50.6%) was the most frequent morbidity and it was present all combinations of morbidities. Network analysis has demonstrated 4 MM clusters: 1) cardiometabolic; 2) respiratory + cancer; 3) musculoskeletal; and 4) a mixed mental illness + other diseases. Depression was the most central morbidity in the model according to nodes' centrality measures (strength, closeness, and betweenness) followed by heart disease, and low back pain. Similarity in male and female networks was observed with a conformation of four clusters of MM and cancer as an isolated morbidity. The prevalence of MM in the older Brazilians was high, especially in female sex and persons living in the South region of Brazil. Use of network analysis could be an important tool for identifying MM clusters and address the appropriate health care, research, and medical education for older adults in Brazil., Competing Interests: The authors have declared that no competing interests exist.

Multimorbidity is highly prevalent in older adults and can lead to hospitalisation. We investigate the prevalence, associated factors, and multimorbidity pattern associated to hospitalisation, readmission, and length of stay in the population aged 50 years and older. We analysed baseline data (2015-2016) from the ELSI-Brazil cohort, a representative sample of non-institutionalised Brazilians aged ≥ 50 years. In total, 8807 individuals aged ≥ 50 years were included. Poisson regression with robust variance adjusted for confounders was used to verify the associations with hospitalisation. Multiple linear regression was used to analyse the associations with readmission and length of stay. Network analysis was conducted using 19 morbidities and the outcome variables. In 8807 participants, the prevalence of hospitalisation was 10.0% (95% CI 9.1, 11), mean readmissions was 1.55 ± 1.191, and mean length of stay was 6.43 ± 10.46 days. Hospitalisation was positively associated with male gender, not living with a partner, not having ingested alcoholic beverages in the last month, and multimorbidity. For hospital readmission, only multimorbidity ≥ 3 chronic conditions showed a statistically significant association. Regarding the length of stay, the risk was positive for males and negative for living in rural areas. Five disease groups connected to hospitalisation, readmission and length of stay were identified. To conclude, sociodemographic variables, such as gender, age group, and living in urban areas, and multimorbidity increased the risk of hospitalisation, mean number of readmissions, and mean length of stay. Background: Multimorbidity is defined as the presence of multiple chronic conditions in the same individual. Multimorbidity is more prevalent in older adults and can lead to several adverse health outcomes., Methods: We systematically reviewed evidence from observational studies to verify the association between multimorbidity and hospitalization in older adults. Furthermore, we also aimed to identify whether it changes according to gender, advanced age, institutionalization, and wealth of the country of residence. We searched the PubMed, Embase and Scopus databases from December 2020 to April 2021. The analysed outcomes were as follows: hospitalization, length of stay and hospital readmission., Results: Of the 6,948 studies identified in the databases, 33 were included in this review. From the meta-analysis results, it was found that multimorbidity, regardless of the country's wealth, was linked to hospitalization in older adults (OR = 2.52, CI 95% = 1.87-3.38). Both definitions of multimorbidity, ≥2 (OR = 2.35, 95% CI = 1.34-4.12) and ≥3 morbidities (OR = 2.52, 95% CI = 1.87-3.38), were associated with hospitalization. Regardless of gender, multimorbidity was associated with hospitalization (OR = 1.98, 95% CI = 1.67-2.34) and with readmission (OR = 1.07, 95% CI = 1.04-1.09). However, it was not possible to verify the association between multimorbidity and length of stay., Conclusions: Multimorbidity was linked to a higher hospitalization risk, and this risk was not affected by the country's wealth and patient's gender. The scope of this study was to measure the prevalence of negative self-perceived health and depressive symptoms in elderly adults according to the presence of urinary incontinence, after a follow-up of nine years. This is a prospective population-based cohort study entitled Bagé Cohort Study of Aging, from Rio Grande do Sul. A total of 1,593 elderly adults were interviewed in the baseline study (2008) and 735 between September 2016 and August 2017. The "urinary incontinence (UI)" exposure was assessed in the baseline study and the outcomes "negative self-perceived health" and "depressive symptoms" in 2016/17. The odds ratio and 95% confidence interval were calculated by Logistic Regression and adjusted for demographic, social, behavioral and health conditions. The prevalence of UI was 20.7% in 2008 and 24.5% in 2016/17; the incidence was 19.8%, being 23.8% among women and 14.6% among men (p = 0.009). Elderly adults with UI at the baseline study had a 4.0 (CI95%: 1.8-8.8) and a 3.4 (CI95%: 1.8-6.2) greater chance to develop negative self-perception of health and depressive symptoms, respectively, after nine years of follow-up, compared to those without UI. The results show a greater probability of mental problems among elderly adults with UI.

Background: Due to the coronavirus disease 2019 (COVID-19) pandemic, access to healthcare services may have become difficult, which may have led to an increase in chronic diseases and multimorbidity., Objectives: To assess the incidence of multimorbidity and its associated factors among adults living in the state of Rio Grande do Sul, Brazil., Design and Setting: Cohort study conducted in Rio Grande do Sul, Brazil., Methods: We included data from the two waves of the Prospective Study About Mental and Physical Health (PAMPA). Data were collected via online questionnaires between June and July 2020 (wave 1) and between December 2020 and January 2021 (wave 2). Multimorbidity was defined as the presence of two or more diagnosed medical conditions., Results: In total, 516 individuals were included, among whom 27.1% (95% confidence interval, CI: 23.5-31.1) developed multimorbidity from wave 1 to 2. In adjusted regression models, female sex (hazard ratio, HR: 1.97; 95% CI: 1.19-3.24), middle-aged adults (31-59 years) (HR: 1.78; 95% CI: 1.18-2.70) and older adults (60 or over) (HR: 2.41; 95% CI: 1.25-4.61) showed higher risk of multimorbidity. Back pain (19.4%), high cholesterol (13.3%) and depression (12.2%) were the medical conditions with the highest proportions reported by the participants during wave 2., Conclusion: The incidence of multimorbidity during a six-month period during the COVID-19 pandemic was 27.1% in the state of Rio Grande do Sul, Brazil.

Objective: To assess the association between household food insecurity (FI) and major depressive episodes (MDE) amid Covid-19 pandemic in Brazil., Design: Cross-sectional study carried out with data from four consecutive population-based studies., Setting: The study was conducted between May and June 2020, in Bagé, a Brazilian southern city. Household FI was measured using the short-form version of the Brazilian Food Insecurity Scale. Utilising the Patient Health Questionnaire-9, we used two different approaches to define MDE: the cut-off point of ≥ 9 and the diagnostic criteria proposed by the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV-TR). Association between FI and MDE was analysed using crude and adjusted Poisson regression models., Participants: 1550 adults (≥ 20 years old)., Results: The prevalence of household FI was 29·4 % (95 % CI 25·0, 34·4). MDE prevalence varied from 4·4 % (95 % CI 3·1, 6·0), when we used the DSM-IV-TR criteria to define this condition, to 9·6 % (95 % CI 7·3, 12·5) of the sample, when we used the cut-off point of ≥ 9 as definition. The prevalence of MDE was more than two times higher in those individuals living with FI, independent of the criteria adopted to define the outcome. Adjustment for potential confounders did not change the association's magnitude., Conclusions: Household FI has been positively associated with MDE amid Covid-19 pandemic, independent of socio-demographic characteristics of participants. Actions are needed to warrant basic living conditions to avoid FI and hunger and its consequences for the Brazilian population, especially those consequences linked to mental health disorders.

Background: This study aims to investigate inequalities in leisure-time physical activity (PA) practice amid the COVID-19 pandemic in a southern Brazilian city., Methods: Four repeated population-based surveys were carried out. PA was collected using a questionnaire proposed by the authors and an adapted version of the leisure-time section of the International Physical Activity Questionnaire. The sociodemographic variables were sex, age, skin color, and education level. The participants also answered questions regarding social distancing measures compliance. Descriptive analyses based on proportions and their respective P values for categorical variables were presented. The chi-square test for heterogeneity and linear trend was used., Results: The sample is composed of 1556 individuals (66.1% female). Overall, between rounds 1 and 2, PA prevalence declined, followed by a gradual increase thereafter. PA practice during the pandemic was higher among men, individuals with higher education level, and individuals with white skin color in all rounds. In rounds 2, 3, and 4, PA was lower among individuals who were practicing more social distancing., Conclusion: To tackle the PA inequalities, policymakers and stakeholders need to confront disparities, defending greater availability of public policies that are attentive to inequalities, especially regarding gender, skin color, and educational level, to promote PA as a human right.

The co-occurrence of two or more diseases is called multimorbidity, and the occurrence of two or more risk factors is called simultaneity of risk factors. Multimorbidity and simultaneity of risk factors are not widely understood in adolescence and early adulthood. This paper aims to describe how multimorbidity and simultaneity of risk factors are distributed throughout adolescence and early adulthood, considering demographic and socioeconomic characteristics, among the 1993 Pelotas Birth Cohort members. This study was carried out using data from the 11, 15, 18, and 22y of the 1993 Pelotas Birth Cohort, Brazil (n = 5249). A self-reported questionnaire assessed allergies, asthma/bronchitis/wheezing, diabetes, vision problems, hypertension, common mental disorders, other mental disorders, physical inactivity, sedentary behaviour, insufficient sleep time, smoking, alcohol abuse behaviour, and illicit drugs. Glucose, cholesterol, blood pressure, weight, and height were objectively collected. The prevalence of multimorbidity was 26.3%, 31.3%, 37.9%, and 44.4% at 11, 15, 18, and 22y, respectively. Around 70% of all ages presented simultaneity of risk factors. Women presented a higher prevalence of simultaneity of risk factors, and the multimorbidity gradually increased from 11 to 22y. The presence of both multimorbidity and simultaneity of risk factors was 19.7% at 11 and 35.4% at 22y. Less than 2% have no morbidity and no risk factors at each age assessed. Objective: The aims of this study were: 1) to estimate the prevalence of multimorbidity in 2013 and 2019 in adults aged 20-59 years; 2) to assess inequalities in the prevalence of multimorbidity in 2013 and 2019 according to educational level., Methods: Data from two cross-sectional surveys from the Brazilian National Health Survey in 2013 and 2019 were used. Multimorbidity was assessed from 14 lifetime self-reported morbidities (except back problems) and defined using the cutoff point of ≥2 diseases. The prevalence of multimorbidity and individual morbidities were described according to gender, age, skin color, and education. For education, crude, and relative inequalities in prevalence of multimorbidity were calculated using the Slope Index of Inequality and the Concentration Index, respectively., Results: The prevalence of multimorbidity increased from 18.7% (95%CI 18.0-19.3) in 2013 to 22.3% (95%CI 21.7-22.9) in 2019, being higher among women and adults between 30-59 years in both periods. Asthma/bronchitis, depression, and back problems were the conditions that increased the most in the study period. Absolute and relative inequalities by education status were observed in the study period, with worse multimorbidity profiles among the less educated., Conclusion: The prevalence of multimorbidity increased between 2013 and 2019. Inequalities in the prevalence of multimorbidity were observed according to educational level.

Objective: To estimate the prevalence of behavioral health risk factors and investigate clusters of simultaneous occurrences of these factors among students at a public university in Pelotas, state of Rio Grande do Sul, Brazil., Methods: This was a cross-sectional study among university students in the second semester of 2017. Physical inactivity, excessive sedentary behavior, inadequate sleep duration and smoking were the factors studied. Simultaneity of risk factors was evaluated using cluster analysis, through the ratio between observed and expected prevalence, and those that did not include the unit were considered clusters., Results: Among 1,716 students, the prevalence of inadequate sleep duration, physical inactivity, excessive sedentary behavior and smoking were 45.2% (95%CI 42.9;47.6), 44.4% (95%CI 42.7;47.2), 39.8% (95%CI 37.7;42.2) and 10.6% (95%CI 9.6;12.5) respectively. More than 80% of the students presented at least one risk factor. The clusters identified were related to inadequate sleep duration (O/E=1.15) and physical inactivity associated with sedentary behavior (O/E=1.24)., Conclusion: High prevalence of the four risk factors studied and only two clusters were observed.

Objective: Emergency services are essential to the organization of the health care system. Nevertheless, they face different operational difficulties, including overcrowded services, largely explained by their inappropriate use and the repeated visits from users. Although a known situation, information on the theme is scarce in Brazil, particularly regarding longitudinal user monitoring. Thus, this project aims to evaluate the predictive performance of different machine learning algorithms to estimate the inappropriate and repeated use of emergency services and mortality., Methods: To that end, a study will be conducted in the municipality of Pelotas, Rio Grande do Sul, with around five thousand users of the municipal emergency department., Results: If the study is successful, we will provide an algorithm that could be used in clinical practice to assist health professionals in decision-making within hospitals. Different knowledge dissemination strategies will be used to increase the capacity of the study to produce innovations for the organization of the health system and services., Conclusion: A high performance predictive model may be able to help decisionmaking in the emergency services, improving quality of care.

Objective: To analyze health service utilization and associated factors among university students., Methods: This was a cross-sectional study conducted with a sample of 2,708 students at the Federal University of Pelotas between November 2017 and July 2018., Results: Of the 1,865 individuals included in the study, 55.8% used health services in the last 12 months, 39.3% used specialized services, and 22.6% used health centers; 45.9% used services funded by the Brazilian National Health System (SUS); 49.1% used health services for the purposes of prevention. There was greater use among females (prevalence ratio, PR=1.34 - 95%CI 1.23;1.46) and students with health needs (PR=1.51 - 95%CI 1.40;1.63). Greater association was found with SUS-funded services: primary health care center (83.0% [95%CI 77.3;87.5]), accident and emergency (81.3% [95%CI 73.2;87.5]) and urgent care center (83.6% [95%CI 76.2;89.0])., Conclusion: The analysis indicated that women and people with health needs use health services more and that the SUS is the main service provider.

Objective: To assess health care for people with diabetes and hypertension, comparing the teams according to their participation in both cycles I and II of the Program for Improving Primary Health Care Access and Quality (PMAQ), and to verify its association with the characteristics of service users and municipalities., Methods: This was a cross-sectional study using PMAQ data from 2014. The following variables were used: team organization, request for tests and health care reported by service users., Results: Thirty-five percent of the teams presented adequate organization and 88% requested all tests. Among the users, 31% had their feet examined and 18% received adequate health care. Municipalities in the Southeast region, with more than 300,000 inhabitants and the highest human development index, presented the best indicators. The teams that took part in both cycles I and II showed greater prevalence of organization and request for tests., Conclusion: Health care for people with diabetes and hypertension in primary health care in Brazil needs improvement.

Background & Aims: Melatonin appears as a supplement capable of helping with diabetes. However, there is no evidence from meta-analyses that showed significant results in insulin resistance and glycated hemoglobin. This study aimed to review the literature on randomized clinical trials that evaluated melatonin supplementation effects, compared to placebo, on diabetes parameters in humans., Methods: We conducted a systematic review and meta-analysis in the following databases: Pubmed, LILACS, Scielo, Scopus, Web of Science, Cochrane, and Embase. We included randomized clinical trials investigating melatonin supplementation's effects, compared to placebo, on fasting blood glucose, insulin resistance, and glycated hemoglobin. Non-randomized clinical trials, observation studies, and animal models were excluded. The Cochrane scale assessed the quality of the studies. We conducted a meta-analysis on fasting blood glucose, insulin resistance, and glycated hemoglobin., Results: Sixteen studies were included, of which 56% showed benefits from supplementation with melatonin in diabetes parameters compared with placebo. Our meta-analysis showed significant results for fasting blood glucose [mean difference: -4.65; 95% CI: -8.06, -1.23; p = < 0.01; I 2  = 58%], glycated hemoglobin [mean difference: -0.38; 95% CI: -0.67, -0.10; p = 0.30; I 2  = 18%], and insulin resistance [mean difference: -0.58; 95% CI: -1.00, -0.15; p = 0.17; I 2  = 35%]., Conclusions: Our results showed that melatonin supplementation was useful for reducing diabetes parameters when compared to placebo., Competing Interests: Conflict of interest The authors declare no conflict of interest., (Copyright © 2021 Elsevier Ltd and European Society for Clinical Nutrition and Metabolism. Background: An increase in body mass index (BMI) is strongly associated with the occurrence of multimorbidity, and overweight and obesity are contributing factors for the increase in morbidities. Thus, the present study aimed to evaluate the occurrence of multimorbidity and associated factors in Brazilian adults with and without overweight or obesity., Methods: This was a cross-sectional population-based study with data from the National Health Survey (2013) including individuals aged 18 years or older. Multimorbidity was defined as having ≥2 diseases from the list of 15 morbidities on the self-reported questionnaire (self-reported medical diagnosis in life). BMI was categorized as: ≤24.9 kg/m 2 (low weight and eutrophy), 25.0-29.9 kg/m 2 (overweight), and ≥30.0 kg/m 2 (obesity). Sex, age, and schooling were the covariates. Poisson regression was used for crude and adjusted analyses for the variables representing access to health services estimating the prevalence ratio (PR) and 95% confidence interval (CI)., Results: The total sample consisted of 59,402 individuals. The prevalence of multimorbidity was 25% overall and was higher among overweight (25.8%) and obese (32.5%) individuals. Obese women 60 years or older had a higher occurrence of multimorbidity (80%). In the adjusted analysis, a lower prevalence of multimorbidity was observed among those with higher educational levels in all BMI classifications: low weight/eutrophy, PR=0.66 (95% CI, 0.58-0.75); overweight, PR=0.62 (95% CI, 0.56-0.70); and obesity, PR=0.75 (95% CI, 0.67-0.85)., Conclusion: A higher prevalence of multimorbidity was found among obese women who were 60 years of age or older. Schooling was an associated factor regardless of BMI.

Introduction: The development of multiple coexisting chronic diseases (multimorbidity) is increasing globally, along with the percentage of older adults affected by it. Multimorbidity is associated with the concomitant use of multiple medications, a greater possibility of adverse effects, and increased risk of hospitalisation. Therefore, this systematic review study protocol aims to analyse the impact of multimorbidity on the occurrence of hospitalisation in older adults and assess whether this impact changes according to factors such as sex, age, institutionalisation and socioeconomic status. This study will also review the average length of hospital stay and the occurrence of hospital readmission., Methods and Analysis: A systematic review of the literature will be carried out using the PubMed, Embase and Scopus databases. The inclusion criteria will incorporate cross-sectional, cohort and case-control studies that analysed the association between multimorbidity (defined as the presence of ≥2 and/or ≥3 chronic conditions and complex multimorbidity) and hospitalisation (yes/no, days of hospitalisation and number of readmissions) in older adults (aged ≥60 years or >65 years). Effect measures will be quantified, including ORs, prevalence ratios, HRs and relative risk, along with their associated 95% CI. The overall aim of this study is to widen knowledge and to raise reflections about the association between multimorbidity and hospitalisation in older adults. Ultimately, its findings may contribute to improvements in public health policies resulting in cost reductions across healthcare systems., Ethics and Dissemination: Ethical approval is not required. The objective was to analyze trends and inequalities in the prevalence of food insecurity during the COVID-19 pandemic according to sociodemographic factors and social distancing measures. We analyzed data from four serial epidemiological surveys on COVID-19 in May and June 2020, with adults and elderly living in Bagé, Rio Grande do Sul State, Brazil. Food insecurity was assessed with the short version of the Brazilian Food Insecurity Scale (EBIA), with the recall period adapted to the beginning of the social distancing period in the city. Sociodemographic characteristics and the adoption of social distancing measures were analyzed, and their associations with food insecurity were assessed with chi-square test. The temporal trend in food insecurity according to these characteristics was assessed via linear regression. Inequalities in food insecurity were assessed with the angular inequality index and concentration index. Of the 1,550 individuals studied, 29.4% (95%CI: 25.0; 34.4) presented food insecurity. Analysis of inequality showed higher concentration of food insecurity among the younger and less educated and those living with five or more residents in the same household. Over the course of the four surveys, prevalence of food insecurity decreased most sharply among the younger, those living in households with up to two residents, and those with two or more workers. There was a strong association between food insecurity and sociodemographic factors, which may indicate the pandemic´s potential economic impact on households' food situation.

The scope of this study was to estimate the prevalence of spinal disorders among the elderly and analyze potential associated factors. It involved a cross-sectional study of a sample of 1,593 elderly individuals aged 60 years or more residing in the urban area of Bagé, State of Rio Grande do Sul. In addition to the "spinal disorders" outcome, demographic, socioeconomic and behavioral variables, health perception, functional activities and the use of health services were investigated. Data collection was conducted by means of face-to-face interviews. Poisson regression analysis with robust variance was used to verify the factors associated with the outcome analyzed. Of the 1,593 participants, 37.4% reported spinal disorders. The factors related to the outcome were: low level of schooling, poor self-assessment of health, consultation in the last three months and the presence of hypertension, rheumatism and fractures. Interventions for musculoskeletal health can contribute to reduce the damages caused by spinal disorders among the elderly, such as loss of functional capacity, increased demands for health care and reduced quality of life.

This article aims to evaluate the coverage and inequalities in prenatal assistance. Data from the Brazilian National Health Survey cross-sectional study in 2013. Prenatal assistance assessed through indicators: counseling, guidance received, procedures, and exams performed during the pregnancy. An asset index constructed, the Slope Index of Inequality (SII) and Concentration Index (CIX) were used to measure. About 90% received counseling on healthy eating, not smoking and drinking, 80% not to use dye/hair straightening, and all the advice. Approximately 70% received guidance on delivery and signs of risk and 83.4% for breastfeeding and 60% all the guidelines. About 80% had the measure of pressure and weight checked, the measure of the abdomen and the auscultation of the heart of the baby, only 36.7% had their breasts examined e 33.4% all procedures performed. More than 90% held for HIV and urine and 77.9% for syphilis, 81.4% of women have carried out all the exams and 21.7% all the indicators. No significant absolute differences (SII) found. CIX evidenced in counseling for healthy eating and guidance for breastfeeding. There were no marked inequalities in prenatal assistance.

The scope of this paper was to identify patterns of food consumption and associated factors among children under two years of age in Brazil. It involved a cross-sectional, population-based study using data from the National Health Survey conducted in 2013. Food consumption was measured by dichotomous questions, listing the food ingested solely on the day prior to the interview. The patterns were identified through principal component analysis. Poisson regression was used to obtain a crude and adjusted prevalence ratio. The sample design effect was considered in the analyses that were stratified according to age. Among the 5,052 children, three patterns were identified, being defined as: a "healthy food consumption pattern," a "milk consumption pattern" and an "unhealthy food consumption pattern." There was an association between the patterns in each age group, mainly related to skin color, gender, schooling of the head of the family, area of residence and region. A quarter of the children from 12 to 23 months consumed soft drinks, more than a third drank artificial juices and almost half ate candies the day before the interview. Three dietary patterns were identified, besides observing early food ingestion among children under six months of age.

Objective: To assess the prevalence of multimorbidity among Brazilian adults and its association with socioeconomic indicators., Methods: Cross-sectional study that used data from the Pesquisa Nacional Sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos no Brasil (PNAUM - Brazilian National Survey on Access, Use and Promotion of Rational Use of Medicines), carried out between 2013 and 2014. The definition of multimorbidity was the coexistence, in a single individual, of two or more chronic diseases, measured through a list of 14 morbidities (self-reported medical diagnosis throughout life). Economic status and educational level were the socioeconomic indicators used, being the inequalities assessed through the Slope Index of Inequality (SII) and the Concentration Index, stratified by gender., Results: The study comprehended 23,329 adults (52.8% of which were women), with an average age of 37.9 years. Hypertension and high cholesterol levels were the most prevalent conditions. The prevalence of multimorbidity was of 10.9% (95%CI 10.1-11.7) representing nearly 11 million individuals in Brazil, of which 14.5% (95%CI 13.5-15.4) were women and 6.8% (95%CI 5.9-7.8) were men. The occurrence of multimorbidity was similar according to the socioeconomic indicators. In the inequality analysis, we observed absolute and relative differences in men with a higher purchasing power (SII = 3.7; 95%CI 0.3-7.0) and higher educational level (CIX = 7.1; 95%CI 0.9-14.7), respectively., Conclusions: The frequency of comorbidities in Brazilian adults is high, especially in absolute terms. We only observed socioeconomic inequalities in multimorbidities among men.

The study's objectives were to describe the prevalence of gestational weight gain and weight retention at 3 and 12 months postpartum and to identify possible socioeconomic inequalities in adequate gestational weight gain. A longitudinal study was performed with data from the 2015 Pelotas, Rio Grande do Sul State, Brazil, birth cohort. Gestational weight gain was classified according to the US Institute of Medicine guidelines. Weight retention at 3 and 12 months was calculated according to the mother's weight at each period, subtracting pregestational weight. To identify possible inequalities, the study used Slope Index (SII) and Concentration Index (CIX). The sample included mothers with information on the study's outcome (n = 4,102). Prevalence of adequate gestational weight gain was 33.5% (95% confidence interval - 95%CI: 32.1; 35.0). Slight inequalities were observed in the prevalence of adequate weight gain in mothers with less schooling [CIX = 1.88 (95%CI: -0.76; 4.52); SII = 4.27 (95%CI: -0.87; 9.41)] and in mothers belonging to the poorest income quintile- 1st quintile- [CIX = 1.04 (95%CI: -1.60; 3.67); SII = 2.93 (95%CI: -2.06; 7.92)], but these differences were not statistically significant. Mean postpartum weight gain was 2.3kg (standard deviation - SD = 6.4) and 1.4kg (SD = 8.8) at 3 and 12 months, respectively. One-third of the women showed adequate weight gain. The observed inequalities in prevalence of adequate gestational weight gain in women with less schooling and in the poorest income quintile were not statistically significant.

This study aimed to measure the occurrence of multimorbidity and to estimate the number of individuals in the Brazilian population 50 years or older at risk for severe COVID-19. This was a cross-sectional nationwide study based on data from the Brazilian Longitudinal Study of Aging (ELSI-Brazil), conducted in 2015-2016, with 9,412 individuals 50 years or older. Multimorbidity was defined as ≥ 2 chronic conditions based on a list of 15 diseases considered risk conditions for severe COVID-19. The analyses included calculation of prevalence and estimation of the absolute number of persons in the population at risk. Self-rated health status, frailty, and basic activities of daily living were used as markers of health status. Sex, age, region of the country, and schooling were used as covariables. Some 80% of the sample had at least one of the target conditions, which represents some 34 million individuals. Multimorbidity was reported by 52% of the study population, with higher proportions in the Central, Southeast, and South of Brazil. Cardiovascular diseases and obesity were the most frequent chronic conditions. An estimated 2.4 million Brazilians are at serious health risk. The results revealed inequalities according to schooling. The number of persons 50 years or older who presented risk conditions for severe COVID-19 is high both in absolute and relative terms. The estimate is important for planning strategies to monitor persons with chronic conditions and for preventive strategies to deal with the novel coronavirus.

To measure the occurrence of protective behaviors for COVID-19 and sociodemographic factors according to the occurrence of multimorbidity in the Brazilian population aged 50 or over was the objective of this study. We used data from telephone surveys among participants of ELSI-Brazil (Brazilian Longitudinal Study of Aging), conducted between May and June 2020. The use of non-pharmacological prevention measures for COVID-19, reasons for leaving home according to the presence of multimorbidity and sociodemographic variables were evaluated. among 6,149 individuals. Multimorbidity was more frequent in females, married, aged 50-59 years and residents of the urban area. Most of the population left home between once and twice in the last week, increasing according to the number of morbidities (22.3% no morbidities and 38% with multimorbidity). Leaving home every day was less common among individuals with multimorbidity (10.3%) and 9.3% left home in the last week to access health care. Hand hygiene (> 98%) and always wearing a mask when leaving home (> 96%) were almost universal habits. Greater adherence to social isolation was observed among women with multimorbidity when compared to men (PR = 1.49, 95%CI: 1.23-1.79). This adherence increased proportionally with age and inversely with the level of education. The protective behavior in people with multimorbidity seems to be greater in relation to the others, although issues related to social isolation and health care deserve to be highlighted. These findings can be useful in customizing strategies for coping with the current pandemic.

This study aimed to describe leisure-time physical activity (LPA) during the COVID-19 pandemic in a municipality of Rio Grande do Sul state, southern Brazil, according to gender, level of education, and adherence to social distancing. A population-based and cross-sectional descriptive study was carried out in Bagé (RS), Brazil. LPA during the pandemic, place of activity, and Physical Education professional's supervision, were described. The sample included 377 adults, and 24.4% reported LPA during the pandemic. Marked inequalities were observed. LPA prevalence among men was 20 percentage points (pp) higher than women and 40 pp higher among those with higher schooling than those with lower schooling. Among those reporting LPA, 53.5% practiced at home, and 64.8% did not report Physical Education professional supervision. No differences were observed between LPA and level of social distancing. Besides the recurrent discourse that people should include physical activity in the pandemic context, in the light of the marked inequalities observed, this study addressed sociocultural aspects and emphasized that LPA promotion initiatives require humanized approaches that consider the unequal living conditions of Brazilians.

Populational ageing is a current phenomenon and calls for the reconfiguration of health services and expansion of access for the elderly. This is a cross-sectional study with 6,624 elderly Brazilians over 60 that set out to evaluate access to healthcare by measuring lack of access and by describing the trajectory until recourse to hospitalization, emergency care and medical visits. The variables were analyzed according to the nature of funding to access the service (SUS, private insurance and cash-payment). The prevalence of lack of access was 2.5% (CI95% 1.6;4.0) for hospitalizations, 2.1% (CI95% 1.4; 3.1) for emergency rooms, and 0.6% (CI95% 0.3;0.9) for medical visits. SUS accounted for most of the care provided. Positive aspects of SUS were the higher number of medical visits in the city of residence and less money spent on transport. The private system stands out for the low frequency of long waiting times and higher frequency of referrals to return visits after hospitalization. The findings highlight the importance of SUS in Brazil in promoting equality and universality despite the existing deficiencies. In both public and private systems, greater articulation among the healthcare levels is required for integral healthcare to elderly individuals.

To quantify and compare 9-year all-cause mortality risk attributable to modifiable risk factors among older English and Brazilian adults. We used data for participants aged 60 years and older from the English Longitudinal Study of Ageing (ELSA) and the Bagé Cohort Study of Ageing (SIGa-Bagé). The five modifiable risk factors assessed at baseline were smoking, hypertension, diabetes, obesity and physical inactivity. Deaths were identified through linkage to mortality registers. For each risk factor, estimated all-cause mortality hazard ratios (HR) and population attributable fractions (PAF) were adjusted by age, sex, all other risk factors and socioeconomic position (wealth) using Cox proportional hazards modelling. We also quantified the risk factor adjusted wealth gradients in mortality, by age and sex. Among the participants, 659 (ELSA) and 638 (SIGa-Bagé) died during the 9-year follow-up. Mortality rates were higher in SIGa-Bagé. HRs and PAFs showed more similarities than differences, with physical inactivity (PAF 16.5% ELSA; 16.7% SIGa-Bagé) and current smoking (PAF 4.9% for both cohorts) having the strongest association. A clear graded relationship existed between the number of risk factors and subsequent mortality. Wealth gradients in mortality were apparent in both cohorts after full adjustment, especially among men aged 60-74 in ELSA. A different pattern was found among older women, especially in SIGa-Bagé. These findings call attention for the challenge to health systems to prevent and modify the major risk factors related to non-communicable diseases, especially physical inactivity and smoking. Furthermore, wealth inequalities in mortality persist among older adults.

Objective: to evaluate the implementation of schistosomiasis mansoni control actions under the Program to Combat Neglected Diseases in three municipalities in Pernambuco state, Brazil., Methods: implementation analysis was done in 2014, considering the following components - management, epidemiological surveillance, patient care, laboratorial support and health education -; direct observation and interviews were carried out with managers and technical personnel at the state, regional, and municipal levels., Results: partial implementation was found in municipalities A and B 69.7%; 62.2%, while there was full implementation in municipality C 79.5%; contextual weaknesses were found in communication between management levels, insufficient technical-management autonomy of decentralized levels, and professional staff job instability; potentialities identified were - continuing education, political articulation, knowledge about the program, and performance evaluation., Conclusion: contextual categories related to development and implementation stood out for their positive influence on the degree of implementation in the municipalities; we recommend intervention in the weaknesses found, in order to ensure program sustainability and institutionalization.

This article sought to measure lack of access and use of oral health services by elderly residents of Pelotas, Rio Grande do Sul State, Brazil. This is a cross-sectional population study carried out in 2014, in the city's urban zone, including individuals aged 60 years or more. Sociodemographic and self-reported needs variables were associated with outcomes. We used Poisson regression for the crude and adjusted analyses. We interviewed 1,451 elderly individuals using a structured questionnaire. The prevalence of lack of access in the previous year was of 1.8% (95%CI: 0.7-3.0). Elderly individuals who never had a consultation were 3.1% (95%CI: 2.2-4.0) and the prevalence of use of oral health services in the previous years was 38.3%(95%CI 36.0-41.0). Use in the previous year was positively associated with the following variables: younger age (PR = 1.16), having a partner (PR = 1.28), high educational level (PR = 1.31), mouth or teeth problems (PR = 1.93), need for dental prosthesis (PR = 1.36) and being edentulous (PR = 3.11). Lack of access in the previous year was low. Oral health service use was higher than that observed in other states. Findings seem to reflect the expansion of oral health services in the city, especially in the public network, and are useful for health policy planning.

Introduction: Health services utilization is an indirect measure of the rights and equity of a health system. A 2015 survey conducted in the Manaus metropolitan region showed that in the previous year, over 70% of adults visited the doctor and 1 in 3 had visited a dentist. Socioeconomic factors and inequality played a central role in the usage of healthcare services and health situation in this population. Since then, political and economic crisis are evolving in Brazil. This project aims to estimate the prevalence of use of health services and the health status of the adults residing in Manaus in 2019., Methods and Analysis: This is a population-based survey of adults (≥18 years old) residing in Manaus. This survey will be conducted in the first half of 2019 with 2300 participants who will be interviewed at home, selected from a probabilistic sampling in 3 stages (census tracts, household, and dweller), and stratified by sex and age quotas based on official estimates. The participants will be interviewed using previously validated tools and questions employed in Brazilian official surveys, which will cover use of health services and supplies, health status, and lifestyle. Primary outcome will be any healthcare usage in the last 15 days. Associations between health services usage and socioeconomic data and health outcomes will be assessed using a Poisson regression with a complex sampling design correction. Results will be reported according to the strengthening the reporting of observational studies in epidemiology statement., Ethics and Dissemination: This project was approved by the Ethics Committee of the Federal University of Amazonas, Manaus, Amazonas, Brazil. All participants will sign an informed consent before the interview. The results will be disseminated in peer-reviewed manuscripts, reports, conference presentations, and through the media.

Objective: to analyze variation in craving levels according to individual and behavioral characteristics of crack cocaine users at two public treatment services in Pelotas, Rio Grande do Sul, Brazil., Methods: this was a cross sectional study with a sample of 133 participants; the Cocaine Craving Questionnaire scale was used to verify craving levels., Results: 86% of interviewees had moderate and severe craving levels; regarding individual factors, a higher level of severe craving was found in women (45%), those with brown skin color/"mestizos" (60%), low schooling (46%) and minor psychiatric disorders (59%); regarding behavioral factors, the level of severe craving was higher in users who divorced the last year (44%), those who had problems with the Law (61%), those who practiced violence acts (57%), those using more than four psychoactive substances (67%), and those with heavy crack consumption (57%)., Conclusion: almost all crack users were found to have high craving levels; these were proportionally higher in some individual and behavioral variables.

Lower socioeconomic level is positively related to multimorbidity and it is possible that the clustering of health conditions carries the same association. The aim of this study was to identify prevalence of multimorbidity and clusters of health conditions among elderly, as well the underlying socioeconomic inequalities. This was a cross-sectional population-based study carried out with 60-year-old individuals. Multimorbidity was defined as the presence of 2+, 3+, 4+ or 5+ health conditions in the same individual. Schooling levels and the National Economic Index were used to investigate inequalities in the prevalence of multimorbidities among elderly. Slope and concentration indexes of inequality were used to evaluate absolute and relative differences. A factorial analysis was performed to identify disease clusters. In every ten older adults, about nine, eight, seven and six presented, respectvely, 2+, 3+, 4+ and 5+ health conditions. Three clusters of health conditions were found, involving musculoskeletal/mental/functional disorders, cardiometabolic, and respiratory factors. Higher inequalities were found the higher amount of health conditions (5+), when considering economic level, and for 3+, 4+ and 5+, when considering educational level. These findings show high multimorbidity prevalence among elderly, highlighting the persistence of health inequalities in Southern Brazil. Strategies by the health services need to focus on elderly at lower socioeconomic levels.

Background: Brazil is an endemic country for schistosomiasis in the Latin American and Caribbean countries. Pernambuco is a higher-endemic Brazilian state among the 19 states reporting the disease in the country; schistosomiasis affects 102 (55%) of its 185 municipalities. Our objective was to evaluate the effectiveness of the treatment cycles of the SANAR Program (Plan to Reduce and Eliminate Neglected Diseases) in Pernambuco State in Northeast Brazil., Methods: A cross-sectional population-based study was conducted in 2014 via a household survey in 117 hyperendemic locations in the state of Pernambuco. We compared the schistosomiasis prevalence rates in hyperendemic locations, aggregated by geographical region, before and after the intervention. The dependent variable was a positive stool test result by the Kato-Katz method, and the main exposure variable was the number of treatment cycles (one/two). The covariables were the regions of the state and socioenvironmental, socioeconomic, demographic and behavioral characteristics., Results: In all, 12,969 individuals were interviewed, 8932 of whom had stool tests. Of these, 4969 (55.6%) underwent two cycles of collective treatment. Changes in the environmental conditions since 2011 were minimal. Comparison before (2011) and after (2014) treatment showed an average schistosomiasis prevalence of 18.6%, decreasing to 4.1% and 2.0% in locations with one and two treatment cycles, respectively. In 2014, the highest schistosomiasis prevalence was found in the forest area (2.8%), while the lowest was found in the northern region (1.2%) of the state. The adjusted analysis showed a lower occurrence of schistosomiasis in individuals living in areas with two treatment cycles than in individuals from areas with just one cycle (PR 0.65, 95% CI: 0.47-0.89)., Conclusions: The political decision made in Pernambuco to implement the SANAR Program in 2011 greatly impacted the burden of schistosomiasis. This program was effective in reducing the occurrence of schistosomiasis in hyperendemic areas in Pernambuco, with a stronger response in areas with two cycles of collective treatment.

Objective: To evaluate the occurrence and factors associated with multimorbidity among Brazilians aged 50 years and over., Methods: This is a cross-sectional study in a nation-based cohort of the non-institutionalized population in Brazil. Data were collected between 2015 and 2016. Multimorbidity was assessed from a list of 19 morbidities, which were categorized into ≥ 2 and ≥ 3 diseases. The analysis included the calculation of frequencies and the most frequent 10 pairs and triplets of combinations of diseases. The crude and adjusted analyses evaluated the demographic, socioeconomic, behavioral, and contextual variables (area of residence, geopolitical region, and coverage of the Family Health Strategy) using Poisson regression., Results: From the total of 9,412 individuals, 67.8% (95%CI 65.6-69.9) and 47.1% (95%CI 44.8-49.4) showed ≥ 2 and ≥ 3 diseases, respectively. In the adjusted analysis, women, older persons, and those who did not consume alcohol had increased multimorbidity. There were no associations with race, area of residence, geopolitical region, and coverage of the Family Health Strategy. The 10 pairs (frequencies observed between 11.6% and 23.2%) and the 10 triplets (frequencies observed between 4.9% and 9.5%) of the most frequent diseases mostly included back problems (15 times) and systemic arterial hypertension (11 times). All combinations were statistically higher than expected by chance., Conclusions: The occurrence of multimorbidity was high even among younger individuals (50 to 59 years). Approximately two in three (≥ 2 diseases) and one in two (≥ 3 diseases) individuals aged 50 years and over presented multimorbidity, which represents 26 and 18 million persons in Brazil, respectively. We observed high frequencies of combinations of morbidities.
