Ectopic thyroid is well-known pathology for several decades. Many locations have been described. Nowadays, only three cases of skull base location have been described in literature. In our case, we reported a single case of a 39-year-old male presenting with a supra-sellar and suprachiasmatic location. He presented no clinical symptoms, no endocrine alteration on biology and no skull base invasion which make this case unique compared to actual literature. Major Depressive Disorder is a chronic, recurring, and potentially fatal disease, affecting up to 20% of the global population. Since the monoamine hypothesis was proposed more than 60 years ago, only a few relevant advances have been made, with very little disease course changing from a pharmacological perspective. Moreover, since the negative efficacy of novel molecules is frequently reported in studies, many pharmaceutical companies have put new studies on hold. Fortunately, relevant clinical studies are currently being performed extensively, developing immense interest among universities, research centers, and other public and private institutions. Depression is no longer considered a simple disease but a multifactorial one. New research fields are emerging, occurring a paradigm shift, such as the multi-target approach beyond monoamines. Objective: Adolescents and young adults (AYA) with sickle cell disease (SCD) experience high rates of acute care utilization and increased morbidity. At this high-risk time, they also face the need to transition from pediatric to adult services, which, if poorly coordinated, adds to heightened morbidity and acute care utilization. The study objective was to characterize the feasibility, acceptability, and short-term efficacy of a protocolized transition navigator (TN) intervention in AYA with SCD., Methods: We developed a protocolized TN intervention that used ecological assessment and motivational interviewing to assess transition readiness, identify goals, and remove barriers to transition, and to provide disease and pain management education and skills to AYAs with SCD., Results: Ninety-three percent (56/60) of enrolled individuals completed the intervention. Participation in the TN program was associated with significant improvement in mean transition readiness scores (3.58-4.15, P < .0001), disease knowledge scale (8.91-10.13, P < .0001), Adolescent Medication Barriers Scale (40.05-35.39, P = .003) and confidence in both disease (22.5-23.96, P = .048) and pain management (25.07-26.61, P = .003) for youth with SCD., Conclusion: The TN intervention was acceptable to youth with SCD, feasible to implement at an urban academic medical center, and addressed barriers to transition identified by the youth. Alzheimer's disease (AD) is a chronic, progressive, and fatal neurodegenerative disorder affecting cognition, behavior, and function, being one of the most common causes of mental deterioration in elderly people. Once thought as being just developed because of β amyloid depositions or neurofibrillary Tau tangles, during the last decades, numerous AD-related targets have been established, the multifactorial nature of AD became evident. In this context, the one drug-one target paradigm has resulted in being inefficient in facing AD and other disorders with complex etiology, opening the field for the emergence of the multitarget approach. In this review, we highlight the recent advances within this area, emphasizing in hybridization tools of well-known chemical scaffolds endowed with pharmacological properties concerning AD, such as curcumin-, resveratrol-, chromone- and indole-. We focus mainly on well established and incipient AD therapeutic targets, AChE, BuChE, MAOs, β-amyloid deposition, 5-HT4 and Serotonin transporter, with the aim to shed light about new insights in the AD multitarget therapy., (Copyright© Bentham Science Publishers; For any queries, please email at epub@benthamscience.net.)

During the last decade, the one drug-one target strategy has resulted to be inefficient in facing diseases with complex ethiology like Alzheimer's disease and many others. In this context, the multitarget paradigm has emerged as a promising strategy. Based on this consideration, we aim to develop novel molecules as promiscuous ligands acting in two or more targets at the same time. For such purpose, a new series of indolylpropyl-piperazinyl oxoethyl-benzamido piperazines were synthesized and evaluated as multitarget-directed drugs for the serotonin transporter (SERT) and acetylcholinesterase (AChE). The ability to decrease β-amyloid levels as well as cell toxicity of all compounds were also measured. In vitro results showed that at least four compounds displayed promising activity against SERT and AChE. Compounds 18 and 19 (IC 50  = 3.4 and 3.6 μM respectively) exhibited AChE inhibition profile in the same order of magnitude as donepezil (DPZ, IC 50  = 2.17 μM), also displaying nanomolar affinity in SERT. Moreover, compounds 17 and 24 displayed high SERT affinities (IC 50  = 9.2 and 1.9 nM respectively) similar to the antidepressant citalopram, and significant micromolar AChE activity at the same time. All the bioactive compounds showed a low toxicity profile in the range of concentrations studied. Molecular docking allowed us to rationalize the binding mode of the synthesized compounds in both targets. In addition, we also show that compounds 11 and 25 exhibit significant β-amyloid lowering activity in a cell-based assay, 11 (50% inhibition, 10 μM) and 25 (35% inhibition, 10 μM). This data article compares two daily datasets of temperature and relative humidity in two traditional Basque architectural models (B1, B2) located in Lea river valley (Basque Country, Northern Spain). The datasets are drawn from two different approaches to data collection, computer-based simulation and an in-situ monitoring. For this purpose, machine-learning models of the two traditionally constructed buildings were developed and simulated according to annual periods under their SWEC reference climate. These data were compared to the datasets derived from the indoor monitoring campaign, in which hygrothermal variables in two thermal zones of each building (B1&#95;Z1, B1&#95;Z2; B2&#95;Z1, B2&#95;Z2) were measured between September 2018 and September 2019. Introduction: The International Classification of Functioning, Disability and Health (ICF) provides a standardised language and conceptual framework for health and health-related conditions, aiming to unify communication between different disciplines., Objective: To describe the process of selecting (through national expert consensus) ICF categories representative of functioning problems in cancer patients at hospital discharge., Method: We conducted a descriptive qualitative and quantitative study that used the expert consensus method to design a clinical instrument. The study included 21 experts, consisting of 2 patients and 19 health professionals from different disciplines. A total of 208 ICF categories obtained in a previous stage through a systematic review and linking process were evaluated in 3 rounds. Participants evaluated each code and classified it as: highly relevant, relevant, slightly relevant, and not relevant at all. Only those codes assessed as «highly relevant» by more than 50% of the experts were included (cut off). In addition, another 143 ICF categories were evaluated in the second round., Results: In all, 24 ICF categories were obtained: 10 corresponded to the body function component, 11 to activities and participation, and 3 to environmental factors., Conclusions: Consensus using the common language of the ICF resulted in the selection of 24 representative categories of the main health problems related to functioning at hospital discharge in adults treated for cancer., (Copyright © 2019 Sociedad Española de Rehabilitación y Medicina Física. Rituximab is a plausible alternative first-line treatment of ANCA-associated vasculitis. Adverse effects related to its infusion are common and usually have a benign course. However, there have been reports of refractory cardiogenic shock simulating septic shock. We report an 81-year-old male with the diagnosis of ANCA associated vasculitis. Rituximab 500 mg was administered intravenously for a relapse. The infusion proceeded without incident. However, 24 hours after its administration the patient began with fever, chills, coughing and strong malaise. The patient was transferred to the critical patient unit where a septic shock was suspected and resuscitative measures were started. However, the fast response to moderate doses of vasoactive drugs and complementary tests did not support an infectious etiology for the shock. Antimicrobials were discontinued and systemic corticosteroids were maintained, achieving remission of the symptoms. Shock as an unusual adverse reaction to Rituximab was suspected.

Background: Smoking is one of the major Public Health problems worldwide., Aim: To study the frequency of tobacco smoking among undergraduate students of a Chilean university., Material and Methods: An opinion survey was sent by e-mail to all undergraduate students of a university, registering gender, age, study years, study area, smoking behavior, motivation (reason for smoking), intention to quit and tobacco law perception., Results: 1,008 (57% females) out of 11,679 surveys were answered back. Prevalence of active smoking among respondents was 36%, without association with gender, age or years of study. However, students from scientific areas had a lower prevalence. Seventy seven percent of smokers manifested the intention to quit the habit or have started quitting already. Ninety six percent were acquainted with the tobacco law and by 73% agreed with it., Conclusions: Smoking is highly prevalent among university students. It is necessary to develop strategies for smoking cessation within universities that may prevent or reduce tobacco smoking among students.

Water pollution caused by organic matter is a major global problem which requires continuous evaluation. Multivariate statistical analysis was applied to assess spatial and temporal changes caused by natural and anthropogenic phenomena along Potrero de los Funes River. Cluster analysis (CA), principal component analysis (PCA) and analysis of variance (ANOVA) were applied to a data set collected throughout a period of 3 years (2010-2012), which monitored 22 physical, chemical and biological parameters. Content of dissolved oxygen in water and biochemical oxygen demand in a watercourse are indicators of pollution caused by organic matter. For this reason, the Streeter-Phelps model was used to evaluate the water self-purification capacity. Hierarchical cluster analysis grouped the sampling sites based on the similarity of water quality characteristics. PCA resulted in two latent factors explaining 75.2 and 17.6 % of the total variance in water quality data sets. Multidimensional ANOVA suggested that organic pollution is mainly due to domestic wastewater run-offs and anthropogenic influence as a consequence of increasing urbanization and tourist influx over the last years. Besides, Streeter-Phelps parameters showed a low reaeration capacity before dam with low concentration of dissolved oxygen. Furthermore, self-purification capacity loss was correlated with the decrease of the Benthic Index. This measurement suggested that biological samplings complement the physical-chemical analysis of water quality.

Purpose: The aim of this paper was to develop a new recreational water quality index (RWQI) as a tool to ensure the health of swimmers and to take practical decisions., Methods: RWQI was elaborated with epidemiological data, and we carried out an exhaustive study of the different guidelines for recreational waters proposed by different organisations around the world. Different parameters were chosen, considering, as a priority, the swimmer's contact and the possibility of ingestion of water during the recreational activity. Furthermore, rating curves were established for pH, chemical oxygen demand, nitrate, phosphate, detergents, enterococci, total coliforms, faecal coliforms and Escherichia coli., Results and Conclusions: The index was applied to the data set on water quality of the Potrero de los Funes River (San Luis, Argentina), generated during 2 years (2009-2010). Following the RWQI values classification, most of the Potrero de los Funes water samples fell in the good quality range during the study period.

The present paper evaluates the applicability of a new FIA system for COD determination. The new system, flow injection microwave solid phase extraction by means of activated carbon (FI-MW-SPE), consists of a digestion circuit, placed in a home MW oven, coupled to an ICP-OES spectrophotometer. Doehlert experimental design was used to speed up the optimization of different experimental variables studied for assisted digestion methods. The method provided a high throughput of about 18 samples h(-1). To assess the accuracy of analytical methods linear regression, elliptic joint confidence region (EJCR) was used. A large linear range of 2.78-850 mg O(2) L(-1) with an excellent detection limit of 0.94 mg O(2) L(-1) was obtained. The interference by high chloride concentration was studied, and values below 3000 mg Cl(-) ions L(-1), allowed the estimation of COD load without any masking agents. COD values for various types of wastewater samples were correlated with those obtained by standard manual methods. Moreover, interferences due to matrix nature are absent; since matrix is washed out of the column before Cr (III) is eluted. Water quality indices provide a simple and understandable tool for managers on the quality and possible uses for irrigation water, however an individual quality factor alone is not enough to evaluate the irrigation water quality because it could be restrictive and sometime it could give an unfavorable qualification. The aim of this paper was propose a quality profile of irrigation water using the preexisting water quality indices to be applied to arid and semi-arid regions. As a case studied, the water of the Del Molle River (Nogolí, San Luis, Argentina) was researched. Samples were collected during the period October 2005-May 2006. Conductivity, pH, total hardness, sulphate, nitrate, nitrite, alkalinity, chloride, sodium, potassium, TDS, DO and phosphate were analyzed. The irrigation water quality, according to Riverside Norm, belongs to C(2)-S(1) class, according to Wilcox Norm as excellent to good, according to Scott quality factor it is good and according to SAR < 10 and according to RCS it is recommendable. From the obtained data, it can be concluded that the water quality profile was good, so it is useful for normal irrigation agriculture.

The purpose of this study was to evaluate the impact of acid mine drainage on the chemistry and the macrobenthos of the Carolina stream (San Luis - Argentina). Samples were obtained in the years 1997-1998 at two sites: site C(1), located 200 m upstream of the drainage, and site C(2), located 800 m downstream. The system buffer capacity was evaluated in the non - contaminated site by means of the buffer index calculation. The physico - chemical changes observed as a consequence of the contribution of acid mine drainage (AMD) were: a decreasing of pH and an increase in the ionic concentration, especially sulfate and Fe coming from the oxidation produced by chemiolithotrophic bacteria. The values obtained indicated a low buffer capacity and a high intrinsic vulnerability of the system to resist the impact originated by the AMD, producing a remarkable decreasing of pH of the receiving stream. These changes caused modifications in the original benthic community that was replaced by organisms more tolerant to the acid stress. A reduction in the abundance and in the taxonomic richness of the benthic macroinvertebrates was observed when compared with the reference station. An increase in the proportion of Chironomidae and of Acari and a decrease in the proportion of the remaining taxa were observed. The most sensitive groups were Ephemeroptera, Trichoptera and Mollusca. The community was mostly affected by the following variables: pH, conductivity, sulfate and dissolved total Fe.

Previously unsuspected pituitary tumors (incidentalomas) were analyzed in autopsies (4.8-27%) and magnetic resonance imaging (MRI) (10-37%), most of them being micro-pituitary incidentalomas (PI). However, patients with PIs sometimes had macroadenomas which may relate to previously unsuspected neurological and/or endocrine abnormalities. This study aims to establish the incidence of macro- vs. micro-PIs, the need for medical and/or surgical treatment and the neurological and endocrine dysfunction in a retrospective evaluation of patients with PIs studied over six years (1994-2000). Thirty-eight of 46 patients with PIs (22 males), aged 16-77, were followed for a mean of 3.2 years. Initial hormonal testing, ophthalmologic evaluation and MRI were repeated during follow-up. Twenty-nine (63%) of 46 patients had macro-PIs and 17 (17%) micro-PIs. Twenty-three males (75%) had macro-PIs, 10 (34.5%) with visual field defects. Consultations leading to PI diagnosis were chronic headache (28%), cranial trauma (15.3%), sinusitis (13%) and stroke (13%). Partial deficiencies of the anterior pituitary function were confirmed in 19 PIs (41.3%), with secondary hypogonadism prevailing (30%). Seven PIs (15%) were prolactinomas treated with dopamine agonists. Seventeen PIs (37%) underwent surgery. Immunohistochemical analysis showed gonadotrophinomas (30%), plurihormonal non-secreting adenomas (40%), and pituitary adenomas not reacting to any of the anterior pituitary hormone antibodies (30%). One operated macro-PI was a craniopharyngioma. Our data show a high percentage of PIs are macro-incidentalomas against expectations from necropsy and imaging studies. Most macro-PIs are found in males and are clinically non-functioning adenomas, 37% requiring surgery and hormonal substitution.

The clinical isolate, Escherichia coli 1941, exhibits high resistance to chloramphenicol and tetracycline (minimum inhibitory concentrations of 512 micrograms/ml). Neither resistance is linked to the large conjugative plasmid present in the strain. The intracellular accumulation of radiolabeled chloramphenicol increased about 9-fold after the addition of the energy uncoupler carbonyl cyanide m-chlorophenol-hydrazone to an E. coli 1941 culture, indicating the presence of an active efflux mechanism. Sequence analysis and expression study suggested that the multiple-antibiotic resistance marRAB locus and the AcrAB drug-efflux pump were not involved in this active efflux of chloramphenicol.

Objective: To develop a standardized surveillance system for monitoring hemodialysis vascular-access infections in order to compare infection rates between outpatient sites and to assess the effectiveness of infection control interventions., Design: Prospective descriptive analysis of incidence infection rates., Setting: An outpatient hemodialysis center with facilities in Idaho and Oregon., Patients: All outpatients receiving chronic outpatient hemodialysis., Results: There were 38,096 hemodialysis sessions (31,603 via permanent fistulae or grafts, 5,060 via permanent tunneled central catheters, and 1,433 via temporary catheters) during an 18-month study period in 1997 to 1998. We identified 176 total infections, for a rate of 4.62/1,000 dialysis sessions (ds). Of the 176, 80 involved permanent fistulae or grafts (2.53/1,000 ds), 69 involved permanent tunneled central catheter infections (13.64/1,000 ds), and 27 involved temporary catheter infections (18.84/1,000 ds). There were 35 blood-stream infections (0.92/1,000 ds) and 10 episodes of clinical sepsis (0.26 /1,000 ds). One hundred thirty-one vascular-site infections without bacteremia were identified (3.44/1,000 ds), including 65 permanent fistulae or graft infections (2.06/1,000 ds), 42 permanent tunneled central catheter infections (8.3/1,000 ds), and 24 temporary catheter infections (16.75/1,000 ds)., Conclusions: Infection rates were highest among temporary catheters and lowest among permanent native arteriovenous fistulae or synthetic grafts. This represents the first report of extensive incidence data on hemodialysis vascular access infections and represents a standardized surveillance and data-collection system that could be implemented in hemodialysis facilities to allow for reliable data comparison and benchmarking.

The aim of this study was to determine the distribution in France of the Enterobacter aerogenes prevalent clone isolated in the hospitals of the Marseille area (A. Davin-Regli, D. Monnet, P. Saux, C. Bosi, R. Charrel, A. Barthelemy, and C. Bollet, J. Clin. Microbiol. 34:1474-1480, 1996). A total of 123 E. aerogenes isolates were collected from 23 hospital laboratories and analyzed by random amplification of polymorphic DNA and enterobacterial repetitive intergenic consensus-PCR to determine their epidemiological relatedness. Molecular typing revealed that 21 of the 23 laboratories had isolated this prevalent clone harboring the plasmid encoding for extended-spectrum beta-lactamase of the TEM-24 type. Most isolates were susceptible only to imipenem and gentamicin. Their dissemination seems to be clonal and was probably the result of the general use of broad-spectrum cephalosporins and quinolones. Four isolates showed an alteration of their outer membrane proteins, causing decrease of susceptibility to third-generation cephalosporins and imipenem and leading to the critical situation of having no alternative therapeutic. The large dissemination of the E. aerogenes prevalent clone probably results from its good adaptation to the antibiotics administered in France and the hospital environment, particularly in intensive care units.

Enterobacter aerogenes is among the five most frequently isolated nosocomial pathogens in France, and this bacterium also shows increasing multidrug resistance. In this study, various E. aerogenes strains isolated from hospital units were characterized for their outer-membrane proteins, antibiotic susceptibilities (inhibition diameters and MICs) and resistance mechanisms associated with modification of envelope permeability (porin alteration and active efflux). Diminished outer-membrane permeability due to porin alterations was found in conjunction with the expression of an enzymic barrier in resistant isolates. Interestingly, changes in the functional expression of porins appeared to play a special role in susceptibility to cefepime. An active efflux to quinolones was also identified. Simultaneous changes in envelope permeability, i.e. a porin deficiency (in) and an efflux mechanism (out), were clearly evident in two clinical strains.

We evaluated the prevalence of impermeability as a mechanism associated with resistance against beta-lactam antibiotics in members of the family Enterobacteriaceae. During a 1-year period, 80 strains were selected from 3,110 routinely isolated strains according to their noticeable cross-resistance pattern to cephalosporins. They were tested for (i) outer membrane nonspecific porins involved in the entry of small hydrophilic molecules; (ii) the MICs of cefepime, cefotaxime, imipenem, and moxalactam; and (iii) beta-lactamase production. Immunological investigations using specific probes showed that 23 of 80 strains presented an alteration of the porin content, most of them expressing an additional resistance mechanism. The prevalence of this porin-deficient phenotype is especially high in Enterobacter aerogenes and concerns 6.4% of the clinical isolates.

With the recent resolution of the crystal structures of several bacterial porins, it is worthwhile to define the generality of their organization throughout the Enterobacteriaceae. The distribution of specific epitopes was analysed among various Gram-negative bacterial porins using anti-peptide antibodies specific to exposed, transmembrane spanning, or pore-forming regions of Escherichia coli porins. Anti-peptide antibodies which recognized the exposed epitopes indicated a great variability among the bacterial porins analysed. Interestingly, an antigenic site located in the internal loop L3 constricting the pore diameter was present in the majority of the bacterial porins tested. Two epitopes located in domains involved in subunit interaction were also highly conserved. The presence of these common peptides suggested a conservation of specific regions involved in the functional organization of the enterobacterial porins.

From the culture broth of a fungus, two metabolites have been isolated: bis(2-ethylhexyl)phthalate (DEHP) precedently isolated from Streptomyces sp. and 2-(4-hydroxyphenyl)-2-oxoacetaldehyde oxime (PHBA) here reported as a natural compound in the (E)-s-cis configuration. The producing organism was identified as a strain of Penicillium olsonii. Culture growth and chemical identification are discussed in the present work.

In order to evaluate the relationship between pollen exposure and prevalence of allergic respiratory symptoms, we performed a cross-sectional epidemiological study in 2 areas with contrasted exposure to cypress and grass pollens. The study population consisted of 5427 subjects 18 to 65-years-old representative of the general population. All answered an abridged version of the 1978 ATS questionnaire and a sample had a skin test evaluation including grass and cypress pollens. Prevalence of hay fever symptoms was equal to 31.5% and 14.1%, respectively, in exposed and less exposed community (P < 0.001). Prevalence of hay fever symptoms together with a positive skin test to pollen was also significantly higher in the exposed (13.6%) vs less exposed community (5.5%, P < 0.001). In contrast, overall prevalence of asthma was equal to 4.5% and 3.4%, respectively. Prevalence of asthma with positive skin tests was also not statistically significant, equal to 2.5% and 1.9%, respectively. Thus, high exposure to pollen is a risk factor for developing hay fever but not asthma.

A titrimetric method for determination of sulfide and of sulphur in steels with chloranilic acid by biamperometric and automatic potentiometric end-point detection is described. The construction of the sensor for potentiometric indication is also described. The results obtained agree with those of the iodine-thiosulfate method and with the certified values for the steels.

The antimicrobial activities of seven Epicoccum purpurascens strains isolated either from evergreen oak leaves (Quercus ilex) collected over a period of one year, or from the atmosphere were compared in vitro. Two strains sporulated and conspicuously inhibited the growth of Staphylococcus aureus and Trichophyton mentagrophytes. Thin-layer chromatographic studies showed the existence of some compounds, such as flavipin, which were common to all the strains. Epicorazine B was present in the extracts of only the two most active strains.

The airway and systemic arterial smooth muscle responsiveness to cholinergic agents of two strains of rats, Rat Albino (RA) and Brown Norway (BN), was compared in vivo and in vitro. In vivo, we measured the doses of carbachol that induced a 100% increase in lung resistance (PD100 RL), a 50% decrease in dynamic lung compliance (PD50 Cdyn), and the value of systolic blood pressure at the carbachol dose of 10 micrograms (Pa 10 micrograms). In vitro airway smooth muscle and systemic arterial smooth muscle responsiveness was assessed by measuring the maximal response to acetylcholine, the slope of the linear portion of the dose-response curve, and the negative logarithm of the molar concentration of acetylcholine producing 50% of the maximal response (pD2). PD100 and PD50 were about four times greater in BN rats than in RA rats. In contrast, Pa 10 micrograms was 1.5 lower in the BN rats. These differences persisted after bivagotomy. Tracheal pD2 was 25% greater in the RA than in the BN strain. The mean dose-response curve of parenchymal strips of RA rats was situated upward and to the left of the BN curve, but the reverse was observed for aortic smooth muscle dose-response curves. Thus 1) airway smooth muscle responsiveness to cholinergic agents is greater in RA strain than in BN, but the reverse is true for systemic arterial smooth muscle responsiveness; and 2) these differences are not due to factors extrinsic to the smooth muscle, since they occurred in vitro and may depend on different densities of muscarinic receptors.

The airborne fungal flora was studied in different departments of an hospital by using a slit sampler (Casella, London) where the measured volume of air is impacted to the surface of culture medium contained in Petri dishes. Colonies were counted after 3 days incubation at 25 degrees C and 37 degrees C. Many species of hyphomycetes were identified. The main fungi isolated were Cladosporium sp (especially C. cladosporioides), Penicillium sp., Alternaria alternata, Aspergillus sp. (A. flavus, A. niger, A. fumigatus). The results show that the variability of the spore number is related to the ventilation. The number of spores found indoors seemed proportional to those present outdoors in naturally ventilated rooms. The results are quite different in the airconditioned wards. Also, there might be a correlation between the counts and the activity and occupation of wards.

In certain cases, allergic respiratory phenomena appear to be connected with a particular dwelling place. This observations, if it is not explained by a specific allergen (eg. an animal), raises the possible contribution of domestic moulds. The author shows the results of a study on domestic moulds in 65 houses in the Bouches-du-Rhône by culture on Petri dishes. The species most often detected were Cladosporium, Penicillium, Aspergillus, Alternaria. In some houses identical fungi were found to those in the atmosphere, in others greater numbers were found inside than outside. The study of fungal spores is of great interest; it gives an idea of their numerical importance which can be considerable; in addition besides those fungi which are present in the routine battery of tests, it may show other species that should perhaps be considered in the diagnostic aetiology.

Two classes of entering freshman medical students at the University of Utah indicated career preferences prior to the start of preclinical classwork. A group of interested students applied to a special family practice curriculum track. From this group, 20 were randomly selected each year to participate in the track program. The number was limited because of the need for a small student/faculty ratio. Students participating in the program were found to select residencies in family practice at a significantly higher rate than their classmates who expressed an interest but were not selected to participate. Students who stated a preference for family practice as freshmen were also found to select residencies in family practice at a significantly higher rate than freshman not preferring it. Nevertheless, most senior students matched with residencies different from their freshman preferences.

Weekly atmospheric sampling in the Pocatello, Idaho, area and skin testing with a standard inland smut mixture of the smuts of common cereal grains and grasses have shown that smut is a prominent antigen and, used rationally in a mixture of related antigens, is useful in all seasons in the treatment of allergies. The history, agricultural aspects and medical significance of smuts are discussed.

An indirect titrimetric method for determination of phosphate is based on precipitation with excess of bismuth, and titration of the surplus with chloranilic acid, the end-point being determined biamperometrically.

A plane-tree pollen allergen was obtained by ion exchange and gel filtration chromatography. It is a glycoprotein with a molecular weight of 22,000. By isoelectrofocusing, two isomers with a high cross-allergenicity were obtained. The results suggest that this fraction is not the only allergen but certainly the most active.

"Paragerm" is commonly used in hospitals where its bactericide action is well known. Preliminary tests have been carried out in the laboratory in order to study the antifungal power of this product. Variation of the different concentrations in the culture medium, variation of the time of contact between "Paragerm" and the fungus and both the mechanical and chemical effects of the product have shown that, among the species selected, certain were more sensitive and others more resistant to this solution and that, in our experimentation, "Paragerm" had a fungistatic effect.

When allergen P was denatured by 8M urea, the modified molecule still reacted with IgE specific for the native allergen but not with hemagglutinating antibodies. Heating at 100 degrees C abolished the reaction in both cases. The results suggested differences between allergenic and antigenic capacities which may be based on structural differences of the antigenic determinants.

A comparative account of Alternaria and Cladosporium spore concentrations in four cities in the European Economic Community (EEC), is given for the years 1973 and 1974. Sijilar volumetric air sampling methods were employed in each of the cities, and concentrations of 3000 Cladosporium spores and 100 Alternaria spores/m3 were taken as the threshold concentrations for clinical significance. With Cladosporium, although highest concentrations were observed in Brussels, the differences were less marked at the other stations. Highest Alternaria concentrations were reocrded in Brussels, followed by London, Leiden and Munich. During the 2 years studied, the average number of days with spore concentrations greater than the threshold value for Cladosporium were 59 in Brussels, 36 in Leiden, 38 in London and 46 in Munich and for Alternaria, 48 in Brussels, 13 in Leiden, 33 in London and 5 in Munich.
