Commentary Using endoscopic skills creatively to treat complex pathologic conditions, especially in patients with challenging anatomy, is the core of therapeutic endoscopy. In this case, the authors describe a unique EUS-guided cholecystoduodenostomy in a patient with history of metastatic alveolar rhabdomyosarcoma and subsequent left orbital exenteration. Significant trismus from prior radiation exposure prevented the patient from opening his mouth more than 5 mm. Therefore, the authors creatively performed the procedure by accessing the GI tract through the left orbit. The innovation demonstrated in this case is what occurs when thoughtful endoscopic plans meet forward-thinking endoscopists. Taking existing endoscopic procedures and adapting them to treat patients with complex anatomy and challenging pathologic conditions is what pushes the field of endoscopy further and inspires creative endoscopic treatments.
