Aim: The aim of the present in vitro study was to investigate the effects of different sintering procedures on the fit, color parameters, and fracture load of monolithic fixed partial prostheses (FPPs)., Materials and Methods: A metal master model was scanned and FPPs were designed. Groups were created by fabricating FPPs using four different sintering procedures (n = 10): Prettau-Standard (PST); Prettau-Slow (PSL); Ice-Speed (ISP); Ice-Standard (IST). PST-PSL (Group P; N = 20) and ISP-IST (Group I; N = 20) were colored with different coloring liquids. The marginal and internal fit were measured using the silicone replica method. CIELAB values of the samples were measured using a spectrophotometer. Then, for each sample, the die was obtained from polymethyl methacrylate. The specimens were cemented into dies and tested in a universal testing machine for fracture load. One-way ANOVA was performed to assess the effect of the sintering procedure on the marginal and internal fit; fracture load; and ∆E00, ∆L', ∆C', and ∆H' values of the FPPs., Results: The PSL and PST groups showed significantly smaller internal and marginal fit values compared with the ISP group. Additionally, the internal fit values of the IST group were significantly higher than those of Group P. Sintering time reduction led to a decrease in ∆E00 values. Fracture load values were not statistically significantly affected by the different sintering procedures for both brands., Conclusion: Different sintering procedures did not have a clinically significant effect on fit and fracture load. Different sintering procedures were found to have an impact on the color change of monolithic zirconia restorations.

The purpose of this study was to investigate the fracture behavior of endodontically treated (ET) deciduous molar when directly restored with different restorative materials in Class II (MO) cavities in comparison with permanent teeth. MO cavities were prepared with 2.4-2.5 mm and 1.9-2.0 mm in buccolingual width, and mesiodistal width of each cavity walls, respectively, followed by direct restoration with different materials: resin-modified glass ionomer cement (RMGIC), composite resin (CR), and composite resin containing 25% short glass-fiber (SFRC). All specimens were subjected to mechanical loading tests at a speed of 1 mm/min and evaluated fracture resistance and fracture modes. A one-way ANOVA followed by a Tukey multiple comparisons analysis was used. Deciduous-SFRC (3,310.5±396.2 N) were significantly higher fracture resistance than permanent-RMGIC (1,633.8±346.8 N) (p<0.001), and permanent-CR (1,400.0±381.3 N) (p<0.001). For the direct restoration of MO cavity after endodontic treatment, SFRC demonstrated its promising performance in load-bearing capacity and failure mode, especially in ET deciduous molars.

Background/Objectives : Limited research has been performed to assess the strength of resin-bonded 3D-printed restorations. Based on that, this study investigates the impact of different manufacturing methods on the fracture load of indirect composite restorations (ICRs) following an aging process. Methods : Three manufacturing techniques-conventional (CRC), milled (MRC), and printed (PRC)-were evaluated using 60 specimens, each with a diameter of 10 mm and a thickness of 1.0 mm. Sandblasting with Al 2 O 3 particles was employed to optimize the bonding process, significantly influencing surface roughness parameters (Ra, Rz, RSm). All specimens were bonded to the dentin analog using composite resin cement and subjected to either 10,000 thermocycles (TC) or storage (ST) at 37 °C in distilled water. Fracture load assessments were performed using a universal testing machine. A finite element analysis was conducted to assess stress distribution. Results : Two-way ANOVA results indicated that the manufacturing method significantly affected mean fracture load values ( p < 0.001), with PRC showing the highest mean fracture load (4185 ± 914 N), followed by MRC (2495 ± 941 N) and CRC (599 ± 292 N). The aging protocol did not have a significant impact on fracture load. Conclusions : This study revealed that 3D-printed resin composite exhibited comparable strength to milled resin composite when adhesively cemented, suggesting it is a promising option for indirect composite restorations based on its mechanical performance. However, further research is needed to evaluate its bond strength and optimal surface treatment methods to prevent early debonding.

The mechanical properties and thickness of adult temporal and occipital bones were examined in modern Japanese forensic samples. Cranial bones were obtained from 293 Japanese corpses (179 men and 114 women). During autopsy, left temporal (LT), right temporal (RT), and occipital (O) bone samples were extracted from each skull. Sample thickness (ST) was measured using multidetector computed tomographic imaging. The fracture load (FL) of each sample was measured by a bending test, in which the flexural strength (FS) was calculated. The FL and ST values for O were significantly greater compared with those of the LT and RT bones. The temporal bones were thinner compared with other parts of the skull and at greater risk for fracture. There is a need to take precautions to prevent temporal bone fractures. There were no significant differences in any of the values between LT and RT, indicating bilateral symmetry of the temporal bones. There were significant negative correlations between age and the FL and FS values for all sites in both sexes, except for O in the male samples, suggesting that older individuals are at increased risk for fractures. No significant correlations were observed between age and ST values in any of the samples. Aim: This study aimed to investigate the influence of titanium base (ti-base) abutment macro- and micro-geometry on the mechanical stability of polymer-infiltrated ceramic network (PICN) screw-retained implant-supported single crowns (iSCs)., Materials and Methods: Twelve specimens per group were used, comprising six different implant/ti-base abutment combinations restored with PICN iSCs: Nb-T (gingival height [GH]: 1.5 mm, prosthetic height [PH]: 4.3 mm), CC (GH: 0.8 mm, PH: 4.3 mm), CC-P (GH: 0.8 mm, PH: 7 mm), Nb-V (GH: 1.5 mm, PH: 6 mm), St (GH: 1.5 mm, PH: 5.5 mm), and Th (GH: 0.5 mm, PH: 9 mm). The specimens underwent thermo-mechanical aging, and those that survived were subsequently subjected to static loading until failure. The data were analyzed using a one-way ANOVA test followed by Tukey post hoc test (α = .05)., Results: All specimens survived thermo-mechanical aging without complications, namely, visible cracks, debonding, or screw loosening. Th group demonstrated the highest strength values among all the groups, with significant differences compared to Nb-T (p < .05), CC (p < .001), and St (p < .001). Additionally, CC-P group exhibited significantly superior fracture strength results compared to CC (p < .05) and St (p < .05)., Conclusion: The choice of ti-base, particularly prosthetic height, had a significant influence on fracture resistance of PICN iSCs. Nevertheless, the height or geometrical features of the ti-base did not exhibit a significant influence on the mechanical behavior of the iSC/ti-base assembly under thermomechanical loading, as all specimens withstood the aging without complication or failure., (© 2023 John Wiley & Sons A/S. Fracture resistance is an important parameter used to predict the performance of indirect dental restorations. The purpose of this in vitro study was to assess the fracture load of posterior milled nanoceramic crowns, in comparison with the lithium disilicate crowns, after fatigue loading, for two different restoration occlusal thicknesses. Forty test metal dies were fabricated by duplicating a master metal model consisting of an anatomic abutment preparation of the maxillary first premolar for a single crown. The dies were divided into two groups of 20 each for the fabrication of nanoceramic (Lava Ultimate) and lithium disilicate (IPS e.max CAD) single crowns. Each material group was further divided into two sub-groups of 10 dies each, based on crown occlusal thickness, of 0.5-mm and 0.75-mm ( n = 10). Dental Type V stone dies poured from polyvinyl siloxane impressions of the test metal dies were laboratory scanned in order to design and mill 40 ceramic crowns. The crowns were cemented on to the test metal dies with a self-adhesive resin luting cement. All crowns were thermocycled (2500 cycles) and mechanically loaded (250,000 cycles) in a chewing simulator followed by static loading until failure, and the values noted. The data were statistically analyzed by 2-way ANOVA and Tukey HSD post-hoc multiple comparison tests (α = 0.05). The mean fracture loads ranged from 1022 to 1322 N for nanoceramic crowns and from 1145 to 1441 N for the lithium disilicate crowns. Two-way ANOVA revealed insignificant differences between the nanoceramic and lithium disilicate crowns ( p > 0.05) in terms of fracture load. Significant differences were noted in the fracture resistance of crowns based on occlusal thickness (303 N; p = 0.013) regardless of the material used. Multiple comparisons by Tukey HSD post-hoc test showed insignificant differences between the four material-occlusal thickness groups ( p > 0.05). The nanoceramic crowns were found to be comparable with lithium disilicate crowns in terms of fracture load. The mean fracture loads of all of the tested crowns were within the normal physiological masticatory load limits. Based on the fracture-resistance results, nanoceramic crowns seem to be suitable for clinical use for the tested occlusal thicknesses.

This study investigated the fracture load of implant-supported zirconia crowns (IZCs), in which indirect composite resin or feldspathic porcelain was layered onto zirconia frameworks with mechanical retentive devices. Three different zirconia frameworks were assessed: attaching mechanical retentive devices on glaze and opaque porcelain materials (GL and OP groups, respectively), and no attaching mechanical retentive devices (ND group). The frameworks were layered using feldspathic porcelain (FP veneer) and indirect composite resin (IC veneer). Fracture load of the specimens was measured. In FP veneer, the GL group recorded the highest fracture load. In the IC veneer, the GL and OP groups had significantly higher fracture load than the ND group. The fracture resistance of IZCs can be enhanced by applying glaze material before attaching mechanical retentive devices for porcelain layering. The mechanical retentive devices effectively yielded mechanical interlocking between the zirconia frameworks and the IC veneer in GL and OP groups.

Objectives: To simulate the replacement of a premolar with an implant-supported cantilever fixed dental prosthesis (ICFDP) and how the fracture load is affected by implant type, positioning within the zirconia blank, and aging protocol., Materials and Methods: Seventy-two ICFDPs were designed either within the enamel- or dentin layer of a 4Y-PSZ blank for bone-level and tissue-level titanium-zirconium implants. Fracture load was obtained on the cantilever at baseline (no aging) or after aging in a chewing simulator with the load applied within the implant axis (axial aging) or on the cantilever (12 groups with n = 6). A three-way ANOVA was applied (α = .05)., Results: A three-way ANOVA revealed a significant effect on fracture load values of implant type (p = .006) and aging (p < .001) but not for the position within the zirconia blank (p = .847). Fracture load values significantly increased from baseline bone level (608 ± 118 N) and tissue level (880 ± 293 N) when the implants were aged axially, with higher values for tissue level (1065 ± 182 N) than bone level (797 ± 113 N) (p < .001). However, when the force was applied to the cantilever, fracture load values decreased significantly for tissue-level (493 ± 70 N), while values for bone-level implants remained stable (690 ± 135 N)., Conclusions: For ICFDPs, the use of bone-level implants is reasonable as catastrophic failures are likely to be restricted to the restoration, whereas with tissue-level implants, the transmucosal portion of the implant is susceptible to deformation, making repair more difficult., (© 2024 The Authors. Objectives: To evaluate the fracture strength and linear elongation at break of three-units fixed partial dentures (FPDs) fabricated with traditional and new materials for fixed prosthodontics before and after ageing., Methods: Sixty models of three-units FPDs were fabricated and cemented onto a Co-Cr model simulating the replacement of a maxillary second premolar. The samples were randomly divided into 3 groups: metal-ceramic (MCR), graphene-doped polymethylmethacrylate (PMMA-GR) and polymethylmethacrylate (PMMA). Half of the samples were directly subjected to fracture test, while the remaining half underwent an ageing process and then a fracture loading test using an electrodynamic testing machine. Fracture load and elongation at break values were taken and statistically analysed., Results: Significant differences were detected between the different materials (p<0.05). All groups showed a reduction of the fracture load and elongation at break values after ageing, but not statistically significant, except for PMMA group (p = 2.012e-19) (p = 3.8e-11)., Conclusions: MCR and PMMA-GR three-units FPDs showed higher fracture strength and lower elongation at break compared to PMMA. Background: The screw-retrievable cement-retained (SRCR) design combines the benefits of both screwand cement-retained implant-supported restorations. This concept has sparked interest in implant dentistry. However, there is a lack of research on fracture behaviors and clinical performance of such restorations., Objectives: The aim of the present article was to review the current literature on the fracture loads and fracture modes of SRCR implant restorations - in vitro studies, and also studies demonstrating the clinical performance of such design., Material and Methods: A literature search was conducted from January 2000 to June 2022, using 6 databases to identify studies on fracture load and clinical performance that fulfilled the eligibility criteria. Thirty-eight studies met the inclusion criteria (22 in vitro and16 in vivo). The in vivo studies comprised case reports/series/letters (9), clinical techniques (2), retrospective/prospective studies (3), and randomized controlled trials (RCTs) (2)., Results: The reviewed articles reported the effects of the SRCR design on the fracture risk if screw access channels were filled or unfilled, with regard to their diameter, and the preparation before or after glazing. The effect of the type of material used in the construction on the fracture modes SRCR restorations was also reported. The long-term clinical data was mainly retrospective and referred to metal-ceramic constructions. Limited long-term clinical data was available for all-ceramic materials and high-performance polymers (HPPs)., Conclusions: Screw-retrievable cement-retained implant restorations appear to have potential in the monolithic design. If the SRCR construction is metal-ceramic or made of a veneered material, special design and abutment selection should be considered. High-performance polymers may be recommended as a substitute for posterior implant restoration.

Background : Classic endocrowns made of dental ceramics are considered a promising alternative to traditional post-endodontic restorations. The use of circular ferrules in endocrowns is a topic of controversial discussion. Therefore, the present study aims to evaluate the effect of ferrule design and cementation mode on the fatigue resistance of zirconia endocrowns. Methods : Eighty human molars were divided into four groups ( n = 20): NFC (no-ferrule, conventional cementation), NFA (no-ferrule, adhesive luting), FC (ferrule, conventional cementation) and FA (ferrule, adhesive luting). Both the classic and the modified endocrown preparation with a two-millimeter ferrule design were carried out. Endocrowns were fabricated from zirconia using the CEREC system. After thermocycling, specimens were loaded according to the step-stress test up to 1500 N. Results : Failure rate was low; 88.8% of total specimens passed the step-stress test. Fractures were distributed between all groups; no significant differences in fatigue resistance were detected for preparation design and cementation mode. Conclusions : Endocrowns appear to be a promising concept for endodontically treated molars. Ferrule and also cementation mode have only a minor influence on fatigue resistance of zirconia endocrowns. However, at very high forces, the marginal area of the ferrule represents a weak point.

Objectives: The aim of this study was to compare failure load and initial damage in monolithic, partially veneered, and completely veneered (translucent) zirconia cantilevered fixed partial dentures (CFPDs), as well as completely veneered metal-ceramic CFPDs under different support and loading configurations., Materials and Methods: Eight test groups with anatomically congruent CFPDs (n = 8/group) were fabricated, differing in CFPD material/support structure/loading direction (load applied via steel ball (Ø 6 mm) 3 mm from the distal end of the pontic for axial loading with a 2-point contact on the inner cusp ridges of the buccal and oral cusps and 1.3 mm below the oral cusp tip for 30° oblique loading): (1) monolithic zirconia/CoCr abutment teeth/axial, (2) monolithic zirconia/CoCr abutment teeth/oblique, (3) partially veneered zirconia/CoCr abutment teeth/axial, (4) partially veneered zirconia/CoCr abutment teeth/oblique, (5) completely veneered zirconia/CoCr abutment teeth/axial, (6) completely veneered CoCr/CoCr abutment teeth/axial (control group), (7) partially veneered zirconia/implants/axial, and (8) partially veneered zirconia/natural teeth/axial. Restorations were artificially aged before failure testing. Statistical analysis was conducted using one-way ANOVA and Tukey post hoc tests., Results: Mean failure loads ranged from 392 N (group 8) to 1181 N (group 1). Axially loaded monolithic zirconia CFPDs (group 1) and controls (group 6) showed significantly higher failure loads. Oblique loading significantly reduced failure loads for monolithic zirconia CFPDs (group 2). Initial damage was observed in all groups except monolithic zirconia groups, and fractography revealed design flaws (sharp edges at the occlusal boundary of the veneering window) in partially veneered zirconia CFPDs., Conclusions: Monolithic zirconia CFPDs might be a viable alternative to completely veneered CoCr CFPDs in terms of fracture load. However, oblique loading of monolithic zirconia CFPDs should be avoided in clinical scenarios. Design improvements are required for partially veneered zirconia CFPDs to enhance their load-bearing capacity., Clinical Relevance: Monolithic zirconia may represent a viable all-ceramic alternative to the established metal-ceramic option for CFPD fabrication. However, in daily clinical practice, careful occlusal adjustment and regular monitoring should ensure that oblique loading of the cantilever is avoided., (© 2024. The Author(s).)

The rehabilitation of free-end situations is a frequent indication in prosthetic dentistry. Cantilever fixed dental prostheses (cFDPs) made of 1st and 2nd generation zirconia are one treatment option. Due to a unique gradient technology, combinations of different zirconium dioxide generations are thus feasible in one restoration. However, data about these materials are rare. The purpose of this study was therefore to investigate the fracture resistance and fracture modes of tooth-supported cFDPs fabricated from different zirconia materials (gradient technology) and different framework thicknesses. A total of 40 cFDPs were fabricated using the CAD/CAM approach and belonged to five test groups. The different groups differed in the yttria content, the proportion of the tetragonal/cubic phases, or in wall thickness (0.7 mm or 1 mm). After completion, the cFDPs were subjected to thermal cycling and chewing simulation (1.2 × 10 6 load cycles, 108 N load). Afterwards, cFDPs were statically loaded until fracture in a universal testing machine. A non-parametric ANOVA was compiled to determine the possible effects of group membership on fracture resistance. In addition, post-hoc Tukey tests were used for bivariate comparisons. The mean fracture loads under axial load application ranged from 288 to 577 N. ANOVA detected a significant impact of the used material on the fracture resistances ( p < 0.001). Therefore, the use of cFDPs fabricated by gradient technology zirconia may not be unreservedly recommended for clinical use, whereas cFPDs made from 3Y-TZP exhibit fracture resistance above possible masticatory loads in the posterior region.

The aim of this study was to evaluate load-bearing capacity and wear performance of experimental short fiber-reinforced composite (SFRC) and conventional lithium-disilicate CAD/CAM fabricated fixed partial dentures (FPDs). Two groups (n = 12/group) of three-unit CAD/CAM fabricated posterior FPDs were made. The first group used experimental SFRC blocks, and the second group fabricated from lithium-disilicate (IPS e.max CAD). All FPDs were luted on a zirconia testing jig with dual-curing resin cement. Half of FPDs per group were quasi-statically loaded until fracture. The other half experienced cyclic fatigue aging (100.000 cycles, Fmax = 500 N) before loading quasi-statically until fracture. Fracture mode was examined using SEM. Wear test was performed using 15,000 loading cycles. Both material type and aging had a significant effect on the load-bearing capacity of FPDs. Experimental SFRC CAD without fatigue aging had significantly the highest load-bearing capacity (2096 ± 149N). Cyclic fatigue aging decreased the load-bearing capacity of the SFRC group (1709 ± 188N) but increased it for the lithium-disilicate group (1546 ± 155N). Wear depth values of SFRC CAD (29.3μm) were significantly lower compared to lithium-disilicate (54.2μm). Experimental SFRC CAD demonstrated the highest load-bearing capacity before and after cyclic fatigue aging, and superior wear behavior compared to the control material., (© 2023 The Authors. Aim: To evaluate the fracture load and type of failure of two different monolithic restorative materials bonded to standardized titanium bases and fabricated by two different procedures regarding the bonding interface., Materials and Methods: All screw-retained implant crown specimens (n = 40), subjected to fatigue by thermomechanical loading, differed in the restorative material (lithium disilicate [LDS] or polymer-infiltrated ceramic network [PICN], referred to as 'hybrid ceramic' [HYC]) and the interface type between the restorative material and the titanium base abutment (prefabricated ex-factory or produced during a CAM-milling procedure). This resulted in the following groups (n = 10/group): 1) LDS-M: lithium disilicate crown with a CAM-milled interface; 2) LDS-P: lithium disilicate crown with a prefabricated interface; 3) HYC-M: PICN crown with a CAM-milled interface; and 4) HYC-P: PICN crown with a prefabricated interface. Aged specimens underwent static fracture load testing. The load (N) at which the initial crack occurred was denoted as Finitial, and the maximal load (N) at which the restorations fractured as Fmax. All specimens were examined under a stereomicroscope to determine the failure mode., Results: The median Finitial values were 180 N for LDS-M, 343 N for LDS-P, 340 N for HYC-M, and 190 N for HYC-P. The median Fmax values were 1822 N for LDS-M, 2039 N for LDS-P, 1454 N for HYC-M, and 1581 N for HYC-P. The intergroup differences were significant for Finitial (KW: P = 0.0042) and for Fmax (KW: P = 0.0010). The failure types also showed differences between the restorative groups., Conclusions: The choice of restorative material had a stronger influence on the fracture load than the abutment interface workflow. Lithium disilicate showed the highest load for initial crack appearance (Finitial) and for complete fracture of the restoration (Fmax).

Purpose: To assess the effect of CAD-CAM technique (monolithic, CAD-on, or CAD-on cemented) and thermomechanical fatigue on the marginal discrepancy and fracture load of ceramic crowns., Material and Methods: A total of 90 brass master dies were fabricated to investigate marginal adaptation and fracture load. A mandibular first molar crown's median measurements were loaded into CAD software and divided into 2 crown design groups: Monolithic (M)(IPS e.max zirCAD)(n = 30) or CAD-on core (IPS e.max zirCAD) and lithium disilicate veneer (IPS e.max CAD) (n = 60). The crowns and cores were milled, seated on their respective dies, and marginal discrepancy values were measured by using microcomputed tomography. After veneers were milled, the cores in the veneer groups were divided into 2 groups; veneers bonded with fusion glass-ceramic (IPS e.max CAD Crystal) in CAD-on group (CO) and CAD-on cemented group (CO-C) where veneers were cemented (RelyX U200) onto cores (CO-C). The marginal discrepancy measurements were remade and the crowns were subjected to thermomechanical fatigue (TMF) by using a chewing simulator and thermocycling (5-55°C, 1,200,000 cycles). Marginal discrepancy measurements were repeated and the crowns were subjected to fracture load test by using a universal test device. Data were analyzed statistically by analysis of variance (ANOVA) and Tukey's honestly significant difference test (α = 0.05)., Results: All crown groups had similar marginal discrepancy before veneering. Veneering and cementation on die increased the marginal discrepancy of crowns in cemented CAD-on group. Thermomechanical fatigue increased the marginal discrepancy of both CAD-on groups. Monolithic crown group had the lowest marginal discrepancy after thermomechanical fatigue (p < 0.001), and the highest fracture load (p < 0.001) CONCLUSIONS: Fabrication technique affected the marginal fit and fracture load of CAD-CAM crowns after thermomechanical fatigue. All crowns survived the thermomechanical fatigue test without dislodgement or fracture. Monolithic crowns had the best fit and highest fracture load after fatigue testing. Purpose: The purpose of this study was to determine the effect of the connector configuration on the fracture load in conventional and translucent zirconia of three-unit fixed dental prostheses (FDPs)., Materials and Methods: Six different three-unit FDPs were prepared (n = 6) from three types of zirconia (3Y-TZP (Katana ML ® ), 4Y-TZP (Katana STML ® ), and 5Y-TZP (Katana UTML ® )) in combination with two connector configurations (4 × 2.25, 3 × 3 mm). The Co-Cr master models were scanned, and the FDPs were designed and fabricated using CAD-CAM. The FDPs were cemented on the metal model and then loaded with a UTM at a crosshead speed of 1 mm/min until failure. Two-way ANOVA and Tukey's test were used for statistical analysis (α = .05)., Results: Fracture loads of 3Y-TZP (2740.6 ± 469.2 and 2718.7 ± 339.0 N for size 4 × 2.25 mm and 3 × 3 mm, respectively) were significantly higher than those of 4Y-TZP (1868.3 ± 281.6 and 1663.6 ± 372.7 N, respectively) and 5Y-TZP (1588.0 ± 255.0 and 1559.1 ± 110.0 N, respectively) ( P < .05). No significant difference was found between fracture loads of 4Y-TZP and 5Y-TZP ( P > .05). The connector configuration within 9 mm 2 was found to have no effect on the fracture loads on all three types of zirconia ( P > .05)., Conclusion: Fracture loads of three-unit FDPs were affected by the type of zirconia. The fracture loads of conventional zirconia were higher than those of translucent zirconia. Purpose: To compare the biomechanical behavior of vertebrae with vertebral compression fractures (VCF) treated by a novel system with pedicular anchorage (dowelplasty) versus balloon kyphoplasty., Methods: Four cadaveric spines (T12-L5) were harvested, cleaned from soft tissues, and separated into vertebrae. Axial compressive loads were applied to each vertebra until a VCF was generated. Half of the vertebrae (n = 11) were instrumented using the "dowelplasty" system, consisting of a hollow titanium dowel anchored into the pedicle, through which a cannulated titanium nail is inserted and locked and through which cement is injected. The other half (n = 11) were instrumented using balloon kyphoplasty. Axial compressive loads were re-applied to each vertebra until fracture. Fracture load and fracture energy were calculated from load-displacement data for the pre- and post-treatment states., Results: Compared to balloon kyphoplasty, dowelplasty granted greater net change in fracture load (373N; 95%CI,-331-1076N) and fracture energy (755Nmm; 95%CI,-563-2072Nmm). A sensitivity analysis was performed without L4 and L5 vertebrae from the dowelplasty group, since the length of the cannulated nails was too short for these vertebrae: compared to balloon kyphoplasty, dowelplasty granted an even greater net change in fracture load (680N; 95%CI,-96-1457N) and fracture energy (1274Nmm; 95%CI,-233-2781Nmm)., Conclusion: Treating VCFs with dowelplasty grants increased fracture load and fracture energy compared to the pre-treatment state. Furthermore, dowelplasty grants greater improvement in fracture load and fracture energy compared to balloon kyphoplasty, which suggests that dowelplasty may be a good alternative for the treatment of VCF., Level of Evidence: level IV., (© 2023. Polyetheretherketone (PEEK) is an emerging thermoplastic polymer with good mechanical properties and an elastic modulus similar to that of alveolar bone. PEEK dental prostheses for computer-aided design/computer-aided manufacturing (CAD/CAM) systems on the market often have additives of titanium dioxide (TiO 2 ) to strengthen their mechanical properties. However, the effects of combining aging, simulating a long-term intraoral environment, and TiO 2 content on the fracture characteristics of PEEK dental prostheses have rarely been investigated. In this study, two types of commercially available PEEK blocks, containing 20% and 30% TiO 2 , were used to fabricate dental crowns by CAD/CAM systems and were aged for 5 and 10 h based on the ISO 13356 specifications. The compressive fracture load values of PEEK dental crowns were measured using a universal test machine. The morphology and crystallinity of the fracture surface were analyzed by scanning electron microscopy and an X-ray diffractometer, respectively. Statistical analysis was performed using the paired t -test (α = 0.05). Results showed no significant difference in the fracture load value of the test PEEK crowns with 20% and 30% TiO 2 after 5 or 10 h of aging treatment; all test PEEK crowns have satisfactory fracture properties for clinical applications. Fracture surface analysis revealed that all test crowns fractured from the lingual side of the occlusal surface, with the fracture extending along the lingual sulcus to the lingual edge, showing a feather shape at the middle part of the fracture extension path and a coral shape at the end of the fracture. Crystalline analysis showed that PEEK crowns, regardless of aging time and TiO 2 content, remained predominantly PEEK matrix and rutile phase TiO 2 . We would conclude that adding 20% or 30% TiO 2 to PEEK crowns may have been sufficient to improve the fracture properties of PEEK crowns after 5 or 10 h of aging. Aging times below 10 h may still be safe for reducing the fracture properties of TiO 2 -containing PEEK crowns.

Objective: Sternal fractures can have life-threatening complications. To understand chest injury mechanisms, sufficient data regarding the mechanical properties and structure of the sternum are required. The aim of this study was to examine the mechanical properties and size of the sternum in a Japanese forensic sample., Materials and Methods: Sterna were obtained from 120 Japanese dead bodies of known age and sex. The sample thickness (ST) and the sample width (SW) were measured using a computed tomographic image. Three-point-bending tests were conducted using a three-point-bending apparatus to assess the fracture load (FL) of the sample. Then, the flexural strength (FS) was calculated and the natural logarithm of FL (ln FL) and FS (ln FS) were also calculated., Results: The values of ST, ln FL, and ln FS for male samples were significantly greater than those for female samples. Both ln FL and ln FS had significant negative correlations with age regardless of sex; the Pearson product-moment correlation coefficients were larger for female samples than for male samples. Although age was significantly negatively correlated with SW in female samples, there was no significant correlation between age and SW in male samples. No significant correlations were found between age and ST regardless of sex., Conclusion: This is the first study to present quantitative data on the biomechanical properties of the sternum. This study analyzed the fracture load before and after a chewing simulation of zirconia crowns that were trepanned and repaired using composite resin. Overall, 3 groups with 15 5Y-PSZ crowns in each group were tested. For group A, the fracture load of the unmodified crowns was evaluated. For group B, the crowns were trepanned and repaired using composite resin, also followed by a fracture test. For group C, crowns were prepared like in group B but received thermomechanical cycling before the final fracture tests. Furthermore, scanning electron microscopy (SEM) and X-ray microscopy (XRM) analysis were performed for group C. The mean fracture loads and standard deviation were 2260 N ± 410 N (group A), 1720 N ± 380 N (group B), and 1540 N ± 280 N (group C). Tukey-Kramer multiple comparisons showed a significant difference between groups A and B ( p < 0.01) and groups A and C ( p < 0.01). After ageing, surface fissures were detected via SEM, but no cracks that reached from the occlusal to the inner side of the crown were detected via XRM. Within the limitations of this study, it can be stated that trepanned and composite-repaired 5Y-PSZ crowns show lower fracture loads than 5Y-PSZ crowns without trepanation.

Background: This in-vitro study was conducted to assess the effect of aging on the fracture load of molar crowns fabricated with monolithic CAD/CAM materials., Methods: The crown restorations were produced from Cerasmart, Vita Enamic, and IPS e.max CAD blocks. Aging was applied to the 10 samples each of monolithic CAD/CAM materials (n = 10). Dual-axis chewing simulator (50 N, 1.1 Hz, lateral movement: 1 mm, mouth opening: 2 mm, 1,200,000 cycles) and thermocycling (± 5-55 °C, 6000 cycles) were applied as an aging procedure. 10 samples each of monolithic CAD/CAM materials without aging (n = 10) were considered the control group. 6 tested groups were obtained. Then, all samples were evaluated in a universal testing machine to determine the fracture loading values'., Results: There was not a statistically significant difference between the fracture load values before and after aging for all samples of Cerasmart, Vita Enamic, and IPS e.max CAD (p > 0.005). In a comparison of the monolithic materials together, a statistically significant difference was found between the fracture load values of IPS e.max CAD and Vita Enamic crowns before aging (p = 0.02). Also, Vita Enamic crowns (1978,71 ± 364,05 N) were found different from the IPS e.max CAD (p = 0.005) and Cerasmart crowns (p = 0.041) after aging., Conclusions: Dynamic aging with 1.200.000 cycles was found to have no effect to fracture loading on milled Cerasmart, Vita Enamic, and IPS e.max CAD monolithic crowns., (© 2022. Objectives: The aim of this study is to investigate the influence of the material and corresponding sintering protocol, layer thickness, and aging on the two-body wear (2BW) and fracture load (FL) of 4Y-TZP crowns., Materials and Methods: Multi-layer 4Y-TZP crowns in three thicknesses (0.5 mm/1.0 mm/1.5 mm) were sintered by high-speed (Zolid RS) or conventional (Zolid Gen-X) sintering. 2BW of ceramic and enamel antagonist after aging (1,200,000 mechanical-, 6000 thermal-cycles) was determined by 3D-scanning before and after aging and subsequent matching to determine volume and height loss (6 subgroups, n = 16/subgroup). FL was examined initially and after aging (12 subgroups, n = 16/subgroup). Fractographic analyses were performed using light-microscope imaging. Global univariate analysis of variance, one-way ANOVA, linear regression, Spearman's correlation, Kolgomorov-Smirnov, Mann-Whitney U, and t test were computed (alpha = 0.05). Weibull moduli were determined. Fracture types were analyzed using Ciba Geigy table., Results: Material/sintering protocol did not influence 2BW (crowns: p = 0.908, antagonists: p = 0.059). High-speed sintered Zolid RS presented similar (p = 0.325-0.633) or reduced (p < 0.001-0.047) FL as Zolid Gen-X. Both 4Y-TZPs showed an increased FL with an increasing thickness (0.5(797.3-1429 N) < 1.0(2087-2634 N) < 1.5(2683-3715 N)mm; p < 0.001). For most groups, aging negatively impacted FL (p < 0.001-0.002). Five 0.5 mm specimens fractured, four showed cracks during and after aging., Conclusions: High-speed sintered crowns with a minimum thickness of 1.0 mm showed sufficient mechanical properties to withstand masticatory forces, even after a simulated aging period of 5 years., Clinical Relevance: Despite the manufacturer indicating a thickness of 0.5 mm to be suitable for single crowns, a minimum thickness of 1.0 mm should be used to ensure long-term satisfactory results., (© 2022. Objective: Zirconia implants are assumed to satisfy the esthetic requirements that titanium implants cannot meet, however, there are not enough studies in the literature about narrow-diameter and angled zirconia implants that can be preferred especially in the anterior region. This in vitro study aimed to evaluate the fatigue resistance and fracture strength of narrow-diameter zirconia implants with angled abutments., Materials and Methods: Forty-eight one-piece experimental zirconia implants and monolithic zirconia crowns were produced from 3-YTZP blanks. The implant diameters (3.0 or 3.7 mm) and the restoration types were determined according to three intraoral regions (upper central incisors, lower central incisors, and upper canine), and abutments were designed as straight or 15° angled. The samples were subjected to chewing simulation corresponding to 5 years of clinical performance and then the static loading test. The survival rates of groups were measured by the Kaplan-Meier log-rank test, and the fracture load values were estimated by using Kruskal-Wallis and Mann-Whitney U tests (p < 0.05)., Results: All 3.0 mm diameter implants failed the fatigue test. The fatigue resistance of the implants with angled abutments was significantly lower than the straight abutments, and different crown designs were found to affect survival rates significantly. The fracture strengths of the surviving groups were above the maximum physiological chewing forces, and the differences were not significant., Conclusions: The implant diameter, abutment angle, and restoration type have a significant effect on the fatigue behavior of zirconia implants., Clinical Significance: The fracture strengths of the one-piece zirconia implants with a 3.7 mm diameter and a 15° abutment angle are above the chewing forces for the anterior intraoral region., (© 2022 Wiley Periodicals LLC.)

(1) Background: Due to advantages such as avoidance of chipping, pulp-friendly tooth preparation and cost reduction, zirconia is increasingly being used monolithically without veneering. Nevertheless, to enable good aesthetics, various multilayer systems have been developed. The aim of this study was to investigate the impact of different zirconia multilayer strategies and yttria levels on fracture load, fracture pattern, stress distribution and surface roughness. (2) Methods: Monolithic three-unit anterior FDPs were made from three different color-gradient zirconia multilayer materials with different yttria levels corresponding to varying strength and degrees of translucency grades (Katana HTML, STML, UTML, Kuraray) and one strength-gradient zirconia multilayer material (Katana YML, Kuraray) and artificially aged in a chewing simulator (1.2 × 106 load cycles, 50 N, 2 × 3000 thermocycles, 5−55 °C). Analyses of fracture load, fracture pattern, fracture surfaces, stress distribution and roughness were performed after the fracture load test. Shapiro−Wilk, Kruskal−Wallis, Mann−Whitney U-tests and one-way ANOVA were used (p < 0.05). (3) Results: Fracture loads of the high strength color-gradient material HTML and the strength-gradient material YML were comparable after 5 years of aging (p = 0.645). Increasing yttria levels resulted in a decrease in fracture resistance of 42−57% (p < 0.05). Surface roughness of different zirconia generations is comparable after polishing and aging. (4) Conclusions: Color-gradient multilayer zirconia materials and new strength-gradient zirconia materials with similar yttria levels in the basal layers show comparable mechanical properties and are suitable for anterior FDPs.

Introduction: On rare occasions, fractures of the tibial plateau may occur after uni-compartmental knee arthroplasty (UKA) and account for 2% of total UKA failures. The purpose of this narrative review is to identify and discuss potential risk factors that might lead to prevention of this invalidating complication., Materials and Methods: Electronic database of Pubmed, Scopus, Cochrane and Google Scholar were searched. A total of 457 articles related to the topic were found. Of those, 86 references were included in this narrative review., Results: UKA implantation acts as a stress riser in the medial compartment. To avoid fractures, surgeons need to balance load and bone stock. Post-operative lower limb alignment, implant positioning, level of resection and sizing of the tibial tray have a strong influence on load distribution of the tibial bone. Pain on weight-bearing signals bone-load imbalance and acts as an indicator of bone remodeling and should be a trigger for unloading. The first three months after surgery are critical because of transient post-operative osteoporosis and local biomechanical changes. Acquired osteoporosis is a growing concern in the arthroplasty population. Split fractures require internal fixation, while subsidence fractures differ in their management depending of the amount of bone impaction. Loose implants require revision knee arthroplasty., Conclusion: Peri-prosthetic fracture is a rare, but troublesome event, which can lead to implant failure and revision surgery. Better knowledge of the multifactorial risk factors in association with a thorough surgical technique is key for prevention., (© 2021. ZrO 2 nanoparticles (ZNPs) have excellent physical properties. This study investigated the fracture load of implant-supported, fixed cantilevered prosthesis materials, reinforced with ZNPs and various polymerization techniques, compared with conventional and CAD/CAM materials. Sixty specimens were made from two CAD/CAM; milled (MIL) (Ceramill TEMP); and 3D-printed (NextDent Denture 3D+). Conventional heat-polymerized acrylic resin was used to fabricate the other specimens, which were grouped according to their polymerization technique: conventionally (HP) and autoclave-polymerized (AP); conventionally cured and reinforced with 5 wt% ZNPs (HPZNP); and autoclave reinforced with 5 wt% ZNPs (APZNP). The specimens were thermocycled (5000 cycles/30 s dwell time). Each specimen was subjected to static vertical loading (1 mm/min) using a universal Instron testing machine until fracture. Scanning electron microscopy was used for fracture surface analyses. The ANOVA showed significant fracture load differences between all the tested groups ( p = 0.001). The Tukey post hoc tests indicated a significant difference in fracture load between all tested groups ( p ˂ 0.001) except HP vs. HPZNP and AP vs. MIL. APZNP had the lowest mean fracture load value (380.7 ± 52.8 N), while MIL had the highest (926.6 ± 82.8 N). The CAD/CAM materials exhibited the highest fracture load values, indicating that they could be used in long-term interim prostheses. Autoclave polymerization improved fracture load performance, whereas ZrO 2 nanoparticles decreased the fracture load performance of cantilevered prostheses.

Objectives: This investigation evaluated the impact of multiple firings during the veneering process of anterior all-ceramic restorations on the fracture load (FL) of zirconia crowns veneered with different ceramics (ZRT, ZRO, HFZ, STR)., Methods: All-ceramic crowns with anatomically reduced zirconia frameworks were prepared using four different veneering ceramics (N = 192/n = 48 per veneering ceramic). The crowns were fired 2 and 10 times. Half of them were thermocycled (5000 cycles, 5 °C/55 °C, 20s). FL using Voss shear test was measured. Data was analyzed using ANOVA with partial eta squared and post-hoc Scheffé-test as well as t-test and Weibull analysis., Results: Regarding FL, greatest differences were among the number of firings (p<0.001, η p 2  = 0.369). FL was not influenced by the interaction of both number of firings and aging level (p = 0.231) and the interaction of number of firings and veneering ceramic (p = 0.222). Differences were found comparing FL values of ZRT and STR (p<0.001) and of HFZ and STR (p<0.001). No differences occurred among the veneering ceramics after 2 (p = 0.430) and after 10 firings (p = 0.057) in respect to initial FL. For all initially tested veneering ceramics, FL showed higher values after 10 firings (p≤0.001-0.014). When aged, ZRO, HFZ, and STR also provided higher FL values after 10 firings (p≤0.001). For ZRT, FL values differed between initial and aged testing after 2 firings (p = 0.001) with aged specimens providing higher FL values. After 10 firings, none of the veneering ceramics showed differences regarding FL values between initial and aged testing. Weibull modulus was higher for ZRT specimens after 10 firings when tested initially (m = 10.1) and for aged ZRT specimens after 2 firings (m = 11.1). Failures occurred either with chipping of the veneer or total fracture., Significance: Veneered zirconia restorations tested presented higher FL after 10 firings compared to 2 firings. Purpose: To evaluate the fracture resistance behaviors of titanium-zirconium, one-piece zirconia, and two-piece zirconia implants restored by zirconia crowns and different combinations of abutment materials (zirconia and titanium) and retention modes (cement-retained and screw-retained zirconia crowns)., Material and Methods: Three research groups (n=12) were evaluated according to combinations of abutment material, retention mode, and implant type. In the control group (TTC), titanium-zirconium implants (∅ 4.1 mm RN, 12 mm, Roxolid; Straumann USA, Andover, MA) and prefabricated titanium abutments (RN synOcta Cementable Abutment, H 5.5 mm; Straumann USA) were used to support cement-retained zirconia crowns. In the second group (ZZC), one-piece zirconia implants (PURE Ceramic Implant Monotype, ∅ 4.1 mm RD, 12 mm, AH 5.5 mm; Straumann USA) were used to support cement-retained zirconia crowns. In the third group (ZTS), two-piece zirconia implants (PURE Ceramic Implant, ∅ 4.1 mm RD, 12 mm) and prefabricated titanium abutments (CI RD PUREbase Abutment, H 5.5 mm) were used to support screw-retained zirconia crowns. All zirconia crowns were manufactured in the same anatomic contour with a 5-axis dental mill and blended 3 and 5 mol% yttria-stabilized zirconia (LayZir A2). Implants were inserted into specimen holders made of epoxy resin-glass fiber composite. All specimens were then subject to artificial aging in an incubator at 37°C for 90 days. Fracture resistance of specimen assemblies was tested under static compression load using the universal testing machine based on ISO14801 specification. The peak fracture loads were recorded. All specimens were examined at the end of the test microscopically at 5× and 10× magnification to detect any catastrophic failures. Comparisons between groups for differences in peak fracture load were made using Wilcoxon Rank Sum tests and Weibull and Kaplan-Meier survival analyses (α = 0.05)., Results: The TTC group (942 ± 241 N) showed significantly higher peak fracture loads than the ZZC (645 ± 165 N) and ZTS (650 ± 124 N) groups (p < 0.001), while there was no significant difference between ZZC and ZTS groups (p = 0.940). The survival probability based on the Weibull and Kaplan-Meier models demonstrated different failure molds between titanium-zirconium and zirconia implants, in which the TTC group remained in the plastic strain zone for a longer period before fracture when compared to ZZC and ZTS groups. Catastrophic failures, with implant fractures at the embedding level or slightly below, were only observed in the ZZC and ZTS groups., Conclusions: Cement-retained zirconia crowns supported by titanium-zirconium implants and prefabricated titanium abutments showed superior peak fracture loads and better survival probability behavior. One-piece zirconia implants with cement-retained zirconia crowns and two-piece zirconia implants with screw-retained zirconia crowns on prefabricated titanium abutment showed similar peak fracture loads and survival probability behavior. Titanium-zirconium and zirconia implants could withstand average intraoral mastication loads in the incisor region. Objective: To compare four different types of monolithic zirconia crowns in terms of survival rate and fracture resistance after thermocycling and/or thermo-mechanical loading in a chewing simulator., Methods: Partially stabilized zirconia (PSZ) crowns with fiber-reinforced resin die assemblies (n = 80) were fabricated using: multi-yttria-layered 5Y-PSZ/3Y-PSZ, multi-yttria-layered 5Y-PSZ/4Y-PSZ, monolithic 4Y-PSZ, and monolithic 3Y-PSZ as control (n = 20). Half of the samples in each group were subjected to thermo-mechanical loading under 110 N, 1.4 Hz, 1.2 million cycles with simultaneous thermocycling (10,000 cycles, 5-55°C). The other half were subjected to thermocycling alone. The samples were loaded to failure to measure their fracture resistance. The data were analyzed using by two-way ANOVA and Tukey's HSD post-hoc test (α = 0.05)., Results: All specimens survived the aging protocols. The yttria content significantly affected the fracture resistance of the crowns (p < 0.0001). The mean fracture resistance, from highest to lowest: 3Y-PSZ, 4Y-PSZ, followed by the two multi-yttria-layered systems. The mean difference between the two multi-yttria-layered systems were not statistically significant (p = 0.98). The mechanical loading protocol did not affect the mean fracture resistance within each group (p = 0.18)., Conclusions: Within each group, there was no difference in fracture resistance after thermocycling alone and thermo-mechanical loading. However, increasing the yttria concentration at the occlusal third of the crown decreased its fracture resistance., Clinical Significance: The term "monolithic zirconia" alone without specifying the actual yttria content is misleading. This term represents different materials with different mechanical properties. The yttria content has an inverse relationship with the fracture resistance of zirconia crowns. Purpose: Additive manufacturing (AM) technologies are capable of fabricating complex geometries with different porosities. However, the effect of such porosities on mechanical properties of stereolithography (SLA) AM zirconia with different porosities is unclear. The purpose of this in vitro study was to investigate the mechanical properties namely flexural strength, and flexural modulus of AM zirconia with different porosities., Materials and Methods: A bar (25 × 4 × 3 mm) for flexural strength test (ISO standard 6872/2015) was designed by CAD software program and standard tessellation language (STL) file was obtained. The STL file was used to fabricate a total of 80 bars in four groups. Three experimental groups each containing 20 samples were manufactured using an SLA ceramic printer (CeraMaker 900; 3DCeram Co) and zirconia material (3DMix ZrO 2 paste; 3DCeram Co) with different sintering post processing to achieve different porosities including 0%-porosity (AMZ0), 20%-porosity (AMZ20), and 40%-porosity (AMZ40). The same STL file was used for subtractive manufacturing or milling of 20 zirconia bars as control group (CNCZ) with the same dimensions using a commercial zirconia. Three-point bending tests were performed for all groups following ISO standard 6872/2015 specification using a universal testing machine. Outcomes measured included load at fracture, mean flexural strength, and flexural modulus and they were compared across the experimental groups using a one-way ANOVA. Post hoc pair wise comparison between each pair of the groups were performed using Tukey test., Results: There was a significant difference between the four groups, in terms of fracture load, flexural strength and flexural modulus using one-way ANOVA. AM zirconia with 0% porosity (AMZ0) showed the highest value for fracture load (1132.7 ± 220.6 N), flexural strength (755.1 ± 147.1 MPa) and flexural modulus (41,273 ± 2193 MPa) and AM zirconia with 40% porosity (AMZ40) showed the lowest fracture load (72.13 ± 13.42 N), flexural strength (48.09 ± 8.95 MPa) and flexural modulus (7177 ± 506 MPa). Tukey's pairwise comparisons detected a significant difference between all the possible pairs for all variables except flexural modulus between AMZ0 and CNCZ. The Weibull moduli presented the lowest value for AMZ20 (4.4) followed by AMZ40 (6.1), AMZ0 (6.1), and the highest value was for CNCZ (8.1)., Conclusion: AM zirconia with 0% porosity showed significantly higher flexural strength and flexural modulus when compared to milled and AM zirconia with 20% and 40% porosities., (© 2021 by the American College of Prosthodontists.)

To evaluate the fracture resistance of computer-aided design and computer-assisted manufacturing restorations as the abutment of removable partial dentures, experimental blocks, with the rest seat made of feldspar, hybrid resin composite, lithium disilicate glass ceramic, or zirconia, were subjected to loading by a metallic occlusal rest. The rest contacted the rest seat with an accurate fit and two mismatch contact conditions: bottom and sidewall contact. Zirconia exhibited the highest fracture load, and the fracture load of the accurate fit was significantly higher than that of the sidewall contact (p<0.05) and insignificantly higher than that of the bottom contact (p>0.05). A finite element analysis of the sidewall contact revealed a higher tensile stress concentration at the bottom of the rest seat than the other contact conditions. The mismatch between the rest and the restoration reduced fracture resistance, while zirconia as the abutment withstood the average occlusal force of the posterior region.

Background: The objective was to determine the optimal connector size and position within zirconia disks for implant-supported cantilever fixed dental prostheses (ICFDP)., Methods: Two-unit ICFDPs ( n = 60) were designed for the premolar region with connector sizes of either 9 or 12 mm 2 and positioned in the enamel or dentin layer of two different types of zirconia disks. The restorations were milled and cemented onto zirconia implants. After simulated chewing for 1.2 Mio cycles, the fracture load was measured and fractures were analyzed., Results: No fractures of ICFDPs or along the implants were detected after simulated aging. The mean fracture load values were significantly higher for a connector size of 9 mm 2 (951 N) compared with 12 mm 2 (638 N). For the zirconia material with a higher biaxial flexural strength, the fracture load values were increased from 751 to 838 N, but more implant fractures occurred. The position within the zirconia disk did not influence the fracture load., Conclusions: A connector size of 9 mm 2 and a zirconia material with a lower strength should be considered when designing ICFDPS on zirconia implants to reduce the risk of fractures along the intraosseous implant portion.

Aims: Periprosthetic hip fractures (PPFs) after total hip arthroplasty are difficult to treat. Therefore, it is important to identify modifiable risk factors such as stem selection to reduce the occurrence of PPFs. This study aimed to clarify differences in fracture torque, surface strain, and fracture type analysis between three different types of cemented stems., Methods: We conducted biomechanical testing of bone analogues using six cemented stems of three different types: collarless polished tapered (CPT) stem, Versys Advocate (Versys) stem, and Charnley-Marcel-Kerboull (CMK) stem. Experienced surgeons implanted each of these types of stems into six bone analogues, and the analogues were compressed and internally rotated until failure. Torque to fracture and fracture type were recorded. We also measured surface strain distribution using triaxial rosettes., Results: There was a significant difference in fracture torque between the three stem types (p = 0.036). Particularly, the median fracture torque for the CPT stem was significantly lower than that for the CMK stem (CPT vs CMK: 164.5 Nm vs 200.5 Nm; p = 0.046). The strain values for the CPT stem were higher than those for the other two stems at the most proximal site. The fracture pattern of the CPT and Versys stems was Vancouver type B, whereas that of the CMK stem was type C., Conclusion: Our study suggested that the cobalt-chromium alloy material, polished surface finish, acute-square proximal form, and the absence of a collar may be associated with lower fracture torque, which may be related to PPF. Objectives: To determine the fracture load before and after artificial aging of implant-supported 4-unit cantilever fixed dental prostheses (FDP) with frameworks made of two differently filled polyetheretherketone (PEEK) compounds and veneered using three different techniques., Methods: A total of 120 duplicate 4-unit FDP frameworks were produced (n = 60 milled from PEEK, 20% TiO 2 filled and n = 60 pressed from PEEK, 30% TiO 2 filled) and veneered using three different techniques: (i) digital veneerings, (ii) conventional resin composite veneerings and (iii) prefabricated veneers (n = 20 per subgroup). The FDPs were adhesively bonded to titanium abutments and the fracture load was measured in a universal testing machine (1 mm/min) before and after artificial aging in a mastication simulator (1,200,000 cycles, 50 N, TC: 5/55 °C, 6000 cycles). The fracture patterns were analyzed using digital microscopy. Data were analyzed using the Kolmogorov-Smirnov test, two-way ANOVA, post hoc Scheffé, Chi 2 -test (p < 0.05), and Weibull modulus m, as well as fracture patterns using the Ciba-Geigy table., Results: Veneering technique and filler content significantly affected the fracture load (p < 0.001). Prefabricated veneers showed higher fracture load (p ≤ 0.001) whereas digital and conventional veneerings were similar (p = 0.451). PEEK with 30% filler content presented higher fracture load (p < 0.001) compared with PEEK with 20%. Aging showed no effect on fracture load (p = 0.176). Regarding fracture types, no significant differences were found among the groups (p = 0.055)., Conclusions: Filler content of PEEK compound as well as veneering technique influenced fracture load while aging had no effect on fracture load. Objective: The aim of this study was to compare the fracture behavior of cantilever fixed dental prostheses (FDPs) fabricated from 3Y-TZP, 4Y-TZP, or 5Y-TZP zirconia., Method and Materials: Posterior cantilever FDPs (three groups, each n = 16) were digitally designed and milled. After sintering, eight specimens/group were stored in demineralized water for 72 hours; eight specimens/group were subjected to accelerated aging using chewing simulation with thermal cycling (1,200,000 × 50 N; 2 × 3,000 × 5°C/55°C; H2O, 2 minutes/cycle) and finally stored in demineralized water for 24 hours. The cantilever FDPs were then statically loaded to failure to determine fracture load. Phase transformation towards the monoclinic phase was controlled with x-ray diffractometry., Results: One failure was identified after accelerated aging in the 5Y-TZP group. Fracture load decreased with increasing yttria content and decreasing bending strength. Accelerated aging using chewing simulation with thermal cycling caused a significant decline in fracture load. All FDPs failed in the area of the prepared tooth and in proximity to the cantilever, where the largest bending moment occurred. The yttria content had no influence on the type of fracture. With increasing yttria content, the relation between deformation and fracture load decreased only slightly. On the surface of FDPs that had been subjected to chewing simulation, no phase transformation towards the monoclinic phase (eg, low temperature degradation) was detected., Conclusion: Within the limitations of a laboratory study, the data indicate that 3Y-, 4Y-, and 5Y-TZP may be employed as material for the fabrication of permanent posterior cantilever FDPs. Purpose: To evaluate the effect of different placing strategies performed in the connector area on fracture resistance and fracture behaviour of monolithic multi-layered translucent zirconia fixed dental prostheses (FDPs)., Materials and Methods: Thirty 3-unit monolithic FDPs were produced and divided into three groups (n = 10) based on the different strategies for placing the connector area of FDPs in multi-layered zirconia blank with varying contents of yttria ranging from 4 to 5 mol%. The groups were as follows: FDPs with connectors placed in dentin layer with 4 mol% yttria content, FDPs with connectors placed in gradient layer, and FDPs with connectors placed in translucent layer with 5 mol% yttria content. A final group (n = 10) of conventional monolithic zirconia with a monolayer of yttria content (4 mol%) has been used as a control group. The specimens were artificially aged using thermocycling and pre-loading procedures and subsequently loaded to fracture using a universal testing machine. Fracture loads and fracture behaviour were analyzed using one-way ANOVA and Fisher's exact tests and statistically evaluated (p ≤ 0.05)., Results: There were no significant differences in fracture loads among the groups based on the placing strategies of the connector area of the FDPs in the multi-layered translucent zirconia blank (p > 0.05). There was no significant difference in fracture loads between monolithic multi-layered translucent zirconia and conventional monolithic translucent zirconia materials (p > 0.05). Fracture behaviour of FDPs with connector area placed in translucent layer differed significantly compared to FDPs with connector area placed in dentin layer and FDPs in control group (p = 0.004)., Conclusion: The placing strategies of the connector used in the computer aided design and manufacturing procedures do not considerably affect fracture resistance of monolithic FDPs made of multi-layered translucent zirconia. Background: The periprosthetic femoral fracture is one of the most severe complications after total hip arthroplasty and is associated with an increased mortality. The underlying causes and the patient- and implant-specific risk factors of periprosthetic femoral fractures remain insufficiently understood. The aim of this study was to gain a more profound understanding of the underlying fracture mechanisms and to provide experimental datasets for validation of computational models., Methods: Six cadaveric femurs were implanted with straight hip stems (Zweymueller design) and loaded until fracture reproducing the clinically relevant load cases stumbling and sideways fall. Displacements and the strain distribution on the surface of the femurs and implants, as well as the fracture load and implant subsidence were measured., Findings: For the load case stumbling the mean fracture load was 6743 N and two different mechanisms leading to fracture could be identified: high subsidence with low femoral bending and small subsidence with high femoral bending. For the load case sideways fall the mean fracture load was 1757 N and both tested femurs fractured due to a rotation of the hip stem around its own axis. The detailed datasets provided by this study can be used in future computational models., Interpretation: We demonstrated that the underlying fracture mechanisms of periprosthetic femoral fractures can be fundamentally different in the load case stumbling. Background: A myriad of materials and procedures have been recommended for restoring the root-filled teeth with significant loss of tooth structure and the most common method being the use of "posts.", Material and Methods: A mesio-occluso-distal preparation was done on eighty intact maxillary first premolars, followed by access preparation and cleaning and shaping with rotary files. All samples were obturated and divided into four groups ( n = 20). In Group 1, 3-mm gutta-percha was removed and restored using composite resin; Group 2 - Group 1 + crowns; Group 3 - prefabricated fiber post + crowns; and Group 4 - prefabricated metal post + crowns. All the groups were subdivided into eight groups ( n = 10), fractured with 90° vertical and 45° oblique loading by a universal testing machine., Statistical Analysis: Fracture resistance was analyzed and compared by means of two-factorial two-way analysis of variance and Scheffe's post hoc test., Results: Under 90° loading, the fracture load of teeth restored with the Nayyar's composite resin core and crown was greatest. Under 45° loading, the fracture load of teeth restored with prefabricated metallic posts was significantly ( P < 0.05) less than that in other groups. Fit accuracy and fracture strength of milled monolithic zirconia (Zi) and zirconia-reinforced lithium silicate (ZLS) crowns are important parameters determining the success of these restorations. This study aimed to evaluate and compare the marginal and internal fit of monolithic Zi and ZLS crowns, along with the fracture load, with and without mechanical aging. Thirty-two stone dies acquired from a customized master metal molar die were scanned, and ceramic crowns (16 Zi Ceramill Zolid HT + and 16 ZLS Vita Suprinity) were designed and milled. Absolute marginal discrepancies (AMD), marginal gaps (MG), and internal gaps (IG) of the crowns, in relation to the master metal die, were evaluated using x-ray nanotomography ( n = 16). Next, thirty-two metal dies were fabricated based on the master metal die, and crowns (16 Zi; 16 ZLS) cemented and divided into four groups of eight each; eight Zi with mechanical aging (MA), eight Zi without mechanical aging (WMA), eight ZLS (MA), and eight ZLS (WMA). Two groups of crowns (Zi-MA; ZLS-MA) were subjected to 500,000 mechanical cycles (200 ± 50 N, 10 Hz) followed by axial compressive strength testing of all crowns, until failure, and the values were recorded. Independent sample t tests (α = 0.05) revealed no significant differences between Zi and ZLS crowns ( p > 0.05); for both internal and marginal gaps, however, there were significant differences in AMD ( p < 0.005). Independent samples Mann-Whitney U and Kruskal-Wallis tests revealed significant differences between the two materials, Zi and ZLS, regardless of fatigue loading, and for the individual material groups based on aging (α = 0.05). Multiple comparisons using Bonferroni post-hoc analysis showed significant differences between Zi and ZLS material groups, with or without aging. Within the limitations of this study, the ZLS crown fit was found to be on par with Zi, except for the AMD parameter. As regards fracture resistance, both materials survived the normal range of masticatory forces, but the Zi crowns demonstrated greater resistance to fracture. The monolithic Zi and ZLS crowns seem suitable for clinical application, based on the fit and fracture strength values obtained.

Background: The aim of this study was to develop a mathematically valid method of assessing fracture resistance of roots. The model developed used mesial roots of lower molars instrumented using stainless steel hand files (SS) and two rotary nickel-titanium (NiTi) systems., Methods: Eighty human lower molars were selected and randomly divided into four groups (n = 20). After instrumentation, the root canals were obturated using thermoplasticized gutta percha. The roots were covered with a simulated periodontal ligament and mounted vertically in autopolymerizing acrylic in PVC tubes. Using a universal testing machine, the force to fracture (N) was applied and the maximum load (FL) was recorded. Remaining dentine volume was calculated and the fracture resistance (FR) was recorded. The data were analyzed using SPSS version 22 with P < .05., Results: There were no significant differences among the instrumentation methods for FL but in FR the roots instrumented using rotary NiTi showed significantly lower values than control groups and SS files (P < 0.001)., Conclusions: Considering the effect of root length, volume of the root, and volume of the instrumented canal as well as the maximum failure load may be a more objective method of reporting fracture resistance of roots., (© 2021. Objectives: To investigate the impact of different cleaning methods on the fracture load and two-body wear of additively manufactured three-unit fixed dental prostheses (FDP) for long-term temporary use, compared to the respective outcomes of milled provisional PMMA FDPs., Materials and Methods: Shape congruent three-unit FDPs were 3D printed using three different resin-based materials [FPT, GCT, NMF] or milled [TEL] (N = 48, n = 16 per group). After printing, the FDPs were cleaned using: Isopropanol (ISO), Yellow Magic 7 (YEL), or centrifugal force (CEN). Chewing simulation was carried out with a vertical load of 50 N (480,000 × 5 °C/55 °C). Two-body wear and fracture load were measured. Data were analyzed using global univariate ANOVA with partial eta squared, Kruskal-Wallis H, Mann-Whitney U, and Spearman's rho test (p < 0.05)., Results: TEL showed less wear resistance than FPT (p = 0.001) for all cleaning methods tested. Concerning vertical material loss, NMF and GCT were in the same range of value (p = 0.419-0.997), except within FDPs cleaned in ISO (p = 0.021). FPT showed no impact of cleaning method on wear resistance (p = 0.219-0.692). TEL (p < 0.001) showed the highest and FPT (p < 0.001) the lowest fracture load. Regarding the cleaning methods, specimens treated with ISO showed lower fracture load than specimens cleaned with CEN (p = 0.044) or YEL (p = 0.036)., Conclusions: The material selection and the cleaning method can have an impact on two-body wear and fracture load results., Clinical Relevance: Printed restorations showed superior two-body wear resistance compared to milled FDPs but lower fracture load values. Purpose: To evaluate the effect of endodontic access hole preparation on fracture resistance of translucent zirconia (5Y) and conventional zirconia crowns (3Y) with varying occlusal thicknesses., Materials and Methods: Polymethylmethacrylate (PMMA) dies, representing a prepared tooth, were milled. Zirconia crowns with 1 mm thick axial walls and varying occlusal thicknesses were milled from 3Y (Cercon HT) or 5Y (Cercon XT) zirconia discs and sintered. 160 crowns were divided into 16 groups (n = 10 per group) based on the zirconia type (3Y, 5Y), occlusal thickness (0.5, 1.0, 1.5, 2.0 mm), and access hole preparation (with access hole, control). Crowns were cemented on the PMMA dies with resin-modified glass ionomer cement (Rely X Luting Plus) under constant weight (500 g) and thermocycled for 10,000 cycles. In half of the samples, following 5000 cycles of thermocycling, a uniform endodontic access hole was created using a diamond bur and restored immediately with resin composite (Filtek Supreme Ultra, 3M ESPE). The fracture resistance of the specimens was tested on an Instron 5566 universal testing machine with a stainless steel ball indenter (9.0 mm dia.) and the maximum load before failure was recorded as fracture load (N). Three-way ANOVA testing examined the effect of zirconia type, occlusal thickness, and access hole preparation on fracture loads of the crowns. Statistical tests were two-sided and significance level was set at 95% (α = 0.05)., Results: Fracture load was significantly affected by the type of zirconia, occlusal thickness, and access hole preparation (p < 0.001). Pairwise comparisons revealed that access hole preparation significantly reduced the fracture load of 3Y crowns with 0.5 or 1.0 mm of occlusal thickness and 5Y zirconia crowns with 0.5, 1.0, or 1.5 mm of occlusal thickness (p < 0.05). Increasing occlusal thickness reduced the effect of access hole preparation on fracture load., Conclusion: Type of zirconia, occlusal thickness, and access hole preparation had significant effects on the fracture load of zirconia crowns. Recently, translucent zirconia has become the most prevalent material used as a restorative material. This study aimed to compare the crown fracture load of the four most common different translucent zirconia brands available in the market at 1.5 mm thickness. Standardized tooth preparations for a full ceramic crown were designed digitally with software (AutoCAD) by placing a 1.0 mm chamfer margin and 1.5 mm occluso-cervical curvature for the crown sample manufacturing. Stylized crowns were chosen to control the thickness of the crown. The axial and occlusal thickness were standardized to 1.5 mm thickness except at the central pit, which was 1.3 mm thick. The STL file for the tooth dies was prepared using software (3Shape TRIOS ® Patient Monitoring, Copenhagen, Denmark). The tooth dies were printed with a resin material (NextDent Model 2.0, Vertex-Dental B.V., Soesterberg, The Netherlands) using a 3D printing software (3D Sprint ® Client Version 3.0.0.2494) from a 3D printer (NextDent™ 5100, Vertex-Dental B.V., Soesterberg, The Netherlands). The printing layer thickness was 50 µm. Then, a total of twenty-eight ( N = 28) stylized crowns were milled out of AmannGirrbach (Amann Girrbach GmbH, Pforzheim, Germany) ( n = 7), Cercon HT (Dentsply Sirona, Bensheim, Germany) ( n = 7), Cercon XT (Dentsply Sirona, Bensheim, Germany) ( n = 7), and Vita YZ XT (Zahnfabrik, Bäd Sackingen, Germany) ( n = 7). Following sintering the crowns, sandblasting was performed and they were bonded to the tooth dies with the resin cement (RelyX U-200, 3M ESPE, Seefeld, Germany) and permitted to self-cure under finger pressure for 6 min. The crowns were loaded on the occlusal surface in a universal testing machine (MTS Centurion) with a stainless-steel ball indenter (7 mm radius) with a loading rate of 1 mm/min to contact the stylized crowns on each of the four cusps until failure. A rubber sheet (1.5 mm thickness) was positioned between the crown and indenter, which helped with the load distribution. Statistical analysis was done using SPSS version 20 (IBM Company, Chicago, USA). The fracture loads were analyzed using Dunnett's T3 test, and the number of cracks was analyzed using the Mann-Whitney U test among the groups. The significant level was set at p value = 0.05. The mean fracture loads were 3086.54 ± 441.74 N, 4804.94 ± 70.12 N, 3317.76 ± 199.80 N, and 2921.87 ± 349.67 N for AmannGirrbac, Cercon HT, Cercon XT, and Vita YZ XT, respectively. The mean fracture loads for the surfaces with the greatest number of cracks (excluding the occlusal surfaces) were on the lingual surface for AmannGirrbach and Cercon HT, on the distal and mesial for Cercon XT, and on the buccal for Vita YZ XT. We found that the AmannGirrbach had the most overall cracks. Cercon XT had the greatest number of occlusal cracks and appeared to be the most shattered. Cercon HT had the least number of cracks. In conclusion, Cercon HT presented the best strength properties, the highest fracture load, and no visible cracks. AmannGirrbach presented the lowest strength properties.

Objectives: To evaluate, in vitro, the fracture load of IPS e.max CAD occlusal veneers at thicknesses of either 0.3 or 0.6 mm luted to enamel or dentin., Materials and Methods: Fifty human molars were randomly distributed into five groups (n = 10): G1 - healthy teeth (control); G2-0.3-mm-thick veneers luted to enamel; G3-0.6-mm-thick veneers luted to enamel; G4-0.3-mm-thick veneers luted to dentin; and G5-0.6-mm-thick veneers luted to dentin. After the luting procedures, the specimens were immersed in distilled water at 37°C for 24 hours and then subjected to mechanical loading (10 6  cycles at 200 N load). The specimens were subjected to a fracture load test in a universal testing machine. Two-way ANOVA and Tukey's test (α = 0.05) were used to analyze data., Results: Only the thickness factor was significant (P = .002). Values of fracture load followed by distinct letters represent significant differences (P < .05): G1 (3204 N ± 730) ab ; G2 (3144 N ± 729) ab ; G3 (2489 N ± 606) b ; G4 (3591 N ± 776) a ; and G5 (2770 N ± 598) ab ., Conclusion: IPS e.max ultrathin occlusal veneers luted to enamel or dentin obtained fracture load comparable to that of the healthy tooth., Clinical Significance: IPS e.max CAD ultrathin occlusal veneers at 0.3 or 0.6-mm-thick seem to provide good perspectives in relation to the clinical use., (© 2020 Wiley Periodicals LLC.)

The aim of this study was to investigate the load to fracture and fracture pattern of prosthetic frameworks for tooth-supported fixed partial dentures (FPDs) fabricated with different subtractive computer-aided design and computer-aided manufacturing (CAD-CAM) materials., Materials and Methods: Thirty standardized specimens with two abutments were fabricated to receive three-unit posterior FDP frameworks with an intermediate pontic. Specimens were randomly divided into three groups (n = 10 each) according to the material: group 1 (MM)-milled metal; group 2 (L)-zirconia; and group 3 (P)-Polyetheretherketone (PEEK). The specimens were thermo-cycled and subjected to a three-point bending test until fracture using a universal testing machine (cross-head speed: 1 mm/min). Axial compressive loads were applied at the central fossa of the pontics. Data analysis was made using one-way analysis of variance, Tamhane post hoc test, and Weibull statistics (α = 0.05)., Results: Significant differences were observed among the groups for the fracture load ( p < 0.0001). MM frameworks showed the highest fracture load values. The PEEK group registered higher fracture load values than zirconia samples. The Weibull statistics corroborated these results. The fracture pattern was different among the groups., Conclusions: Milled metal provided the highest fracture load values, followed by PEEK, and zirconia. However, all tested groups demonstrated clinically acceptable fracture load values higher than 1000 N. PEEK might be considered a promising alternative for posterior FPDs.

Objective: The objective was to in-vitro evaluate the mode of failure of abutments supporting fixed partial dentures (FPDs) via different retention techniques., Materials and Methods: Thirty-six implants with diameter and length of 4.5 and 13 mm, respectively, were used to prepare 18 samples of FPDs. Based on the variations on abutment's design, the FPDs were divided into three groups: Group A is cement-based retention; Group B is screw-based retention; and Group C is multiunit screw-based retention. Using a chewing simulator, cyclic loads of 1,250,000 load cycles with the load of 70N were applied on all samples to simulate 5 years of human functional chewing. The samples were loaded until failure using an electromechanical test machine. Sample-size estimation was done and fracture-load values were recorded as means and corresponding standard deviations; and group comparisons were done using one-way analysis of variance and Tukey's post hoc tests. A P value below 0.01 was nominated as an indicator of statistical significance., Results: In total, 36 samples (12 implant-abutment connections per group) were assessed. Abutment bending was observed in 6 (50%), 6 (50%) and 6 (50%) samples in groups A, B and C, respectively. De-attachment of the FPD form the abutment occurred in 5 (41.7%) and 5 (41.7%) samples in groups A and C, respectively. Screw fracture and fracture of FPD at the connector side occurred in 1 (8.3%) and 1 (8.3%) sample in groups A and C, correspondingly. Failure of the FPD was more often encountered in groups A (100%) and C (100%), compared to group B (50%)., Conclusion: All abutments underwent failures under cyclic loading with abutment bending being the most common failure mode. Objectives: To evaluate mechanical stability (survival and complication rates) and bending moments of different all-ceramic monolithic restorations bonded to titanium bases (hybrid abutment-crowns) or to customized titanium abutments compared to porcelain-fused-to-metal crowns (PFM) after thermo-mechanical aging., Material and Methods: Sixty conical connection implants (4.3 mm-diameter) were divided in five groups (n = 12): PFM using gold abutment (GAbut-PFM), lithium disilicate crown bonded to customized titanium abutment (TAbut+LDS), lithium disilicate abutment-crown bonded to titanium base (TiBase+LDS), zirconia abutment-crown bonded to titanium base (TiBase+ZR), polymer-infiltrated ceramic-network (PICN) abutment-crown bonded to titanium base (TiBase+PICN). Simultaneous thermocycling (5°-55°C) and chewing simulation (1,200,000-cycles, 49 N, 1.67 Hz) were applied. Catastrophic and non-catastrophic events were evaluated under light microscope, and survival and complication rates were calculated. Specimens that survived aging were loaded until failure and bending moments were calculated., Results: Survival rates after aging were 100% (TAbut+LDS, TiBase+LDS), 91.7% (GA-PFM), 66.7% (TiBase+ZR) and 58.3% (TiBase+PICN) and differed among the groups (p = .006). Non-catastrophic events as screw loosening (GA-PFM) and loss of retention or micro-/macro-movement (TiBase groups) were observed. Complication rates varied among the groups (p < .001). TiBase+PICN had lower bending moment than all the other groups (p < .001)., Conclusions: Hybrid abutment-crowns made of lithium disilicate can be an alternative to PFM-based restorations, although concerns regarding the bonded interface between the titanium base and abutment-crown can be raised. PICN and zirconia may not be recommended due to its inferior mechanical and bonding outcomes, respectively. Purpose: To assess the effect of yttria mol% concentration and material thickness on the biaxial fracture load (N) of zirconia with and without mastication simulation., Materials and Methods: Disk-shaped specimens (N = 120) of 3 mol% yttria-partially stabilized zirconia, 3Y-PSZ (Katana High Translucent, Kuraray Noritake), 4 mol% yttria-partially stabilized zirconia, 4Y-PSZ (Katana Super Translucent Multi Layered) and 5 mol% Yttria-partially stabilized zirconia, 5Y-PSZ (Katana Ultra Translucent Multi Layered) were prepared to thicknesses of 0.7 and 1.2 mm. For each thickness, the biaxial fracture load (N) was measured with and without mastication simulation with 1.2 million cycles at a 110-N load and simultaneous thermal cycling at 5°C to 55°C. The data were analyzed by three-way Analysis of Variance (α = 0.05) and Tukey-Kramer adjusted multiple comparison test., Results: Yttria mol% concentration and material thickness had a statistically significant effect on the mean biaxial fracture load (F = 388.16, p < 0.001 and F = 714.33, p < 0.001 respectively). The mean biaxial fracture load ranged from the highest to the lowest; 3Y-PSZ, 4Y-PSZ, and 5Y-PSZ (p = 0.012). The mean biaxial fracture load of the 1.2 mm thickness groups was significantly higher than 0.7 mm thickness at any given condition (p = 0.002). Not all specimens survived the mastication simulation protocol. Fifty percent of the 0.7-mm-thick 4Y-PSZ specimens, 70% of the 0.7-mm-thick 5Y-PSZ specimens and 20% of 1.2-mm-thick 5Y-PSZ specimens fractured during mastication simulation. Mastication simulation had no statistically significant effect on the biaxial fracture load (F = 1.24, p = 0.239) of the survived specimens., Conclusions: Lowering yttria mol% concentration and increasing material thickness significantly increases the fracture load of zirconia. At 0.7 mm thickness, only 3Y-PSZ survived masticatory simulation. A minimum material thickness of 1.2 mm is required for 4Y-PSZ or 5Y-PSZ., (© 2020 by the American College of Prosthodontists.)

Objectives: To investigate and compare the mechanical and optical properties of novel strength- and color-gradient multilayered zirconia and the impact of conventional and high-speed sintering., Materials and Methods: Following zirconia materials were analyzed and compared: the high-speed sintered Katana Zirconia Block STML (4Y-TZP, KZC), the conventionally sintered Katana Zirconia Disc STML (4Y-TZP, KZL) and IPS e. max ZirCAD Prime (5Y-TZP/3Y-TZP, EZL). As control group acted the crystallized lithium disilicate ceramic IPS e. max CAD (ELC). Monolithic single molar crowns were fabricated and half of them were aged in a chewing simulator with human enamel antagonists (1.2 × 10 6 cycles, 50 N, lateral movement of 0.7 mm, 5/55 °C). The fracture load was tested in a universal testing machine (N = 96/n = 12). The two-body wear was determined using 3D matching of pre- and post-scans (N = 48/n = 12). Translucency (N = 36/n = 10) was evaluated with UV/Vis spectrophotometer. Data was analyzed using Kolmogorov-Smirnov test, one-way ANOVA followed by post-hoc Scheffé test, unpaired t-test and Weibull analysis (p < 0.05)., Results: Zirconia groups showed higher fracture load than ELC (p ≤ 0.001). Initially, all zirconia materials ranged in the same values (p > 0.05). After chewing simulation, EZL showed higher fracture load than KZC (p < 0.001) and KZL (p = 0.043). Zirconia materials showed no material loss, whereas the ELC showed the volumetric wear of 0.334 ± 0.34 mm 3 and vertical wear of 0.155 ± 0.07 mm. Enamel antagonist wear ranged in the same values (p = 0.083-0.906). The translucency values within each zirconia material showed significant differences between the enamel and the dentin layers (p < 0.001)., Conclusions: The novel strength and color-gradient multilayered zirconia showed higher mechanical properties than lithium disilicate ceramic. The high-speed sintering of zirconia showed neither a negative impact on the fracture load nor on the two-body wear.
