Objective: The study was designed to investigate the right ventricular (RV) reaction to weaning from mechanical ventilation (MV) in patients with and without volume loading after coronary artery bypass grafting (CABG)., Design: Controlled study., Setting: Surgical intensive care unit in a university hospital., Patients: 18 patients were randomized in two groups, Control group (n = 9) and Volume group (n = 9), when the established criteria for weaning from the respirator were satisfied., Intervention: During MV, patients in the Volume group received in rapid (10-min) 6 ml/kg infusion of a 6% hydroxyethyl starch preparation., Measurements and Results: Hemodynamic parameters were measured using a combined right ventricular (RV) ejection fraction-oximetry pulmonary artery catheter at T0 (during MV: baseline), T1 (during MV: 10 min after volume loading or at the same time in the Control group), T2 [after 20 min of spontaneous ventilation (SV)]. In the Control group, RV volumes did not differ throughout the study, while cardiac index (CI) and RV stroke work index (RVSWI) increased from T1 to T2. In the Volume group, RV volumes increased from T0 to T1, further increasing from T1 to T2, whereas CI increased only from T0 to T1. In this group, RVSWI increased from T0 to T1 in 8/9 patients and from T1 to T2 in 6/9 patients., Conclusions: An increase in RV volumes with a concomitant increase in RVSWI was observed in high preload patients when going from MV to SV, suggesting a preserved RV function during weaning from MV in this group compared with control patients. The depression in RV contractility observed in some patients suggested that rapid volume expansion before weaning from MV in CABG patients must be done carefully.

A 66-year-old man was admitted for mitral valve replacement required by a mitral regurgitation resulting in a severe heart failure. He also suffered from chronic respiratory failure, related to a left concavity cyphoscoliosis. Postoperatively, hypoxemia occurred. His chest X-ray showed a left pleural effusion indicating a percutaneous pleural drainage. A local anaesthesia with a 21 G needle was performed prior to drain insertion, which was easily introduced into the pleural cavity. A few minutes later, the patient experienced an acute haemorrhagic shock. He was immediately transferred into the operating room, where the cardiac surgeon discovered and treated a coronary artery effraction. The vascular lesion was attributed to the needle used for local anaesthesia. The patient was discharged twelve days later without sequelae. Two factors were responsible for this accident: the patient's cyphoscoliosis and his left ventricle enlargement. This complication is uncommon. However, in case of a particular patient's anatomy, it is suggested to use a score devised to anticipate the risk of a difficult pleural drainage. This score should include general, thoracic, spinal, cardiac, hepatic and splenic morphology.

The authors have studied a population of 51 children in which 63 central venous catheters were used for the treatment of leukemias, solid tumors and lymphomas. Two groups were isolated: continuous hospitalization (group A) and intermittent hospitalization with periodical day care (group B). Mean duration of catheterization was 129 days in group A and 176 days in group B (whole population: 149 days), with a total of 9,724 days of catheterization. There was no significant differences between the two groups concerning infection, bleeding, thrombosis or migration. So the authors believe that permanent hospitalization is not mandatory for chemotherapy in children with severe malignant diseases. Periodical day care therapy allows a near normal life for children and a better cooperation from their parents.
