Shoulder dislocations remain the most frequent of joint dislocations, with anterior displacement of the humeral head being the direction of dislocation seen most often (97%). Recently, the Dutch clinical guideline on shoulder dislocations has been revised on the basis of predetermined bottlenecks in clinical practice. In this paper, the guideline is translated to clinical practice by means of two fictional cases, in which the novel recommendations are incorporated. The following topics were systematically assessed based on the best available scientific evidence: primary diagnostics, reduction techniques, painmedication/ sedation surrounding reduction and the need for physiotherapy, stabilization surgery and immobilization. Also, a best practice care pathway is advocated. Since scientific evidence is often inconclusive to provide undebatable therapeutic rules, the committee graded the available evidence and additionally used expert opinion to carefully draft recommendations. The paper concludes with an overview of all the recommendations stated in the updated multidisciplinary guideline.

To date, there is no widely accepted system for evaluating soft tissue early healing events in oral surgery. This study aimed to identify variables used to assess early oral soft tissue wound healing. The PubMed-MEDLINE and Cochrane CENTRAL databases were searched up to and including March 1, 2022. Papers were included if they presented scores of wound healing after performing a mucoperiosteal flap operation with the intention of primary closure. The search yielded 2,286 unique titles and abstracts, of which 31 met the eligibility criteria. In general, the available definitions/indices were found to be imprecise. Based on the literature reviewed, six variables were considered relevant for scoring early oral soft tissue wound healing: primary closure, necrosis, redness, swelling, bleeding, and suppuration.

A considerable part of the burden of disease in the Netherlands is caused by unhealthy behavior. Prevention is therefore an important part of recent national agreements, which must now be given practical shape. It is widely known that behavior change is difficult and requires intervening at many levels simultaneously. A relatively scarcely used instrument are financial incentives for lifestyle behavior, despite increasing knowledge about its effectiveness. In this paper, we present ten insights from science for practice about offering financial incentives for lifestyle support, with the aim to fuel the discussion.

In this paper we discuss the case of a 52-year-old man who consulted the emergency department because of confusion. Based on anamnesis, clinical presentation, various technical investigations and recovery after discontinuation of disulfiram, the diagnosis of disulfiram encephalopathy is made. This is a less common but serious complication of a frequently used therapy and underscores the importance of early recognition and careful but also controlled prescription of disulfiram. We describe the pathophysiology behind this complication and reflect on some important numbers.

Background: Studies have shown impairments in neurocognitive functions which persist more than 3 months after COVID-19 (long COVID). It remains unclear what these impairments entail, how long they persist and what proportion of the patients exhibit them., Aim: To define the specific neurocognitive profile and to determine the proportion of deficits in at least one cognitive domain in patients with long COVID., Method: We conducted a systematic search in PubMed according to PRISMA 2020 guidelines with the following inclusion criteria: peer reviewed publications in which patients were assessed more than 3 months following acute COVID-19 by means of a test battery for different domains of neurocognition., Results: We found a total of 1178 papers, of which 7 cohort studies and 1 case-control study were selected. The proportion of patients having deficits in at least one domain of neurocognition ranged from 23% to 100%. Most frequent impairments were found in attention and speed of information processing, anterograde memory, working memory and executive function. Quality of the included studies was moderate., Conclusion: Impairments in neurocognitive functions are highly prevalent among patients with long COVID and include various cognitive domains. We encourage further research to continue studying the complex interaction of COVID-19, neurocognitive impairments and neuropsychiatric syndromes.
