Abstract

Background: Emergency departments are a last resort for some socially vulnerable patients without an acute medical illness (colloquially known as 'socially admitted' patients), resulting in their occupation of hospital beds typically designated for patients requiring acute medical care. In this study, we aimed to explore the perceptions of health care providers regarding patients admitted as 'social admissions.' Methods: This qualitative study was informed by grounded theory and involved semistructured interviews at a Nova Scotia tertiary care centre. From October 2022 to July 2023, we interviewed eligible participants, including any health care clinician or administrator who worked directly with 'socially admitted' patients. Virtual or in-person individual interviews were audio-recorded and transcribed, then independently and iteratively coded. We mapped themes on the 5 domains of the Quintuple Aim conceptual framework. Results: We interviewed 20 nurses, physicians, administrators, and social workers. Most identified as female (n = 11) and White (n = 13), and were in their mid to late career (n = 13). We categorized 9 themes into 5 domains: patient experience (patient description, provision of care); care team well-being (moral distress, hierarchy of care); health equity (stigma and missed opportunities, prejudices); cost of care (wait-lists and scarcity of alternatives); and population health (factors leading to vulnerability, system changes). Participants described experiences caring for 'socially admitted' patients, perceptions and assumptions underlying 'social' presentations, system barriers to care delivery, and suggestions of potential solutions. Interpretation: Health care providers viewed 'socially admitted' patients as needing enhanced care but identified individual, institutional, and system challenges that impeded its realization. Examining perceptions of the people who care for 'socially admitted' patients offers insights to guide clinicians and policy-makers in caring for socially vulnerable patients., Emergency departments have become a destination of last resort for some patients who are made vulnerable by social circumstances, resulting in their occupying hospital beds typically designated for people with [...]

This play is a new interpretation of Urvashi and Arjuna from the epic Mahabharata. Immortal Urvashi of heaven was the life partner of mortal emperor Pururavas of earth, Arjuna's fortieth ancestor. The story is that Urvashi, assigned to be an eternal virgin who was denied motherhood, experiences the bliss of motherhood because of Arjuna. Indra, Arjuna's father, has brought him to heaven so that he might annihilate Indra's enemies. Urvashi, who has fallen in love with Arjuna, approaches him for love but is rejected by Arjuna. Urvashi curses Arjuna to become a eunuch. Arjuna, who has realized that Urvashi was Pururavas' wife, tells her in despair that she is no different from his biological mother Kunti and his stepmother Indrani, Indra's wife. Urvashi's dormant motherhood bursts forth as she hears this. At the end of the story, Urvashi embraces Arjuna with maternal love, lays him on her lap, and sings a lullaby. Arjuna and Urvashi are the protagonists of this play. The other characters include Urvashi's two friends and two nymphs who appear in the interlude. Dr. Sadanam Harikumar is more than just a kathakali performer. A recipient of the President's National Award to Teachers, the Kerala State Lalitha Kala Akademi Award for Sculpture, the Kerala Sangeetha Nataka Akademi Award for kathakali, and the Outstanding Young Indian Award by the Indian Junior Chamber and the Fellowship of the Department of Culture for Research, Harikumar is a top-grade artist of Indian Television in kathakali. He is also an A-grade classical singer on All India Radio. Harikumar has composed twenty-one kathakali plays, including the kathakali adaptation of Shakespeare's Julius Caesar (Vanderstoel 2011). He has composed, directed, and sung about twenty-five innovative musical works for Doordarshan (national TV channel of India). He has had several solo exhibitions of his paintings and sculptures. He served as kathakali Professor at VishwaBharati University Santinikethan for five years. He is currently the Secretary of the Gandhi Seva Sadanam Kathakali Akademi in Kerala. During the years of the Nazi regime, well over 1,000 European Jewish youths migrated to the United States in organized unaccompanied child migration schemes. These youths left an abundant, and largely untapped, trove of sources in which they constructed narratives of their lives and emotions to their parents in letters, to their social workers in their various interactions, and to themselves in their diaries. Though refugee youths undeniably felt a range of emotions, in this article I suggest that emotional expression tells us less about the emotional inner lives of youths than the attempt to exert and subvert control and power in a topsy-turvy world. By drawing attention to the language of emotions, their inherent power dynamics, and the potential gulf between emotions and experience, this article opens a conversation about our capacity to document children's agency and to study emotions to explain decision-making and experience.
