Abstract

The study examines if percutaneous coronary intervention.(PCI) provides an incremental benefit in quality of life over that offered by optimal medical therapy among patients with chronic coronary artery disease. Patients with stable angina in both treatment groups showed definite improvement in health status, whereas the PCI group showed small, but significant, incremental benefits that disappeared by 36 months.

Abstract

Recently, it has been demonstrated in multiple clinical research studies that non-Q-wave myocardial infarction shares many of the features of unstable angina pectoris and that both diseases initially are managed similarly. Important new antiplatelet drugs (glycoprotein IIb-IIIa inhibitors) and antithrombin agents (low-molecular-weight heparin) are currently recommended for patients with unstable angina pectoris/non-ST-segment elevation MI who are at high or intermediate risk on the basis of symptoms, electrocardiographic findings, and the presence or absence of serum markers (eg, troponin I, troponin T, and creatine kinase-MB). This review provides important information concerning the results of clinical studies of glycoprotein IIb-IIIa inhibitors (tirofiban hydrochloride and eptifibatide) when used with unfractionated heparin in patients with this syndrome or with low-molecular weight heparin (enoxaparin sodium) in similar patients. The Thrombolysis in Myocardial Infarction IIIB, Veterans Affairs Non-Q-Wave Infarction Studies in Hospital, and Fast Revascularization During Instability in Coronary Artery Disease II studies evaluating a conservative, ischemia-guided approach vs an early aggressive approach to such patients are presented, with a practical algorithm for treating such patients.

Abstract

The injection of radioactive thallium-201 can be used to observe the flow of blood through the coronary arteries. The injection of dipyridamole is often used in conjunction with this technique to detect impaired blood flow to the heart muscle past areas of stenosis, or narrowing of the coronary arteries (most often due to atherosclerosis). Narrow segments, which may perform acceptably during normal conditions, may prove inadequate when blood flow increases during stress, and this can be observed using dipyridamole. Although promising, the technique currently remains a subject of investigation, and is not yet routinely used in the evaluation of patients with heart disease. To further evaluate the safety and efficacy of the method, 10 patients recovering from uncomplicated myocardial infarctions (heart attacks) were examined. Three patients developed ischemia (reduced blood supply) and associated angina pectoris (chest pain) during the technique, but the events were not serious. Aminophylline was found to promptly reverse the effects of dipyridamole and return the patient's hemodynamic measurements to baseline. The results confirm that the dipyridamole-thallium imaging technique is effective and safe for patients with myocardial infarction when used in a controlled setting along with aminophylline reversal of the drug effects. However, clinicians should be reminded that there are no data to suggest that the technique is safe for patients with complicated myocardial infarction. In the presence of heart failure, angina, low blood pressure, or ventricular arrhythmias, the technique should be used only with extensive precautions and great care.
