Objetivos: En el presente trabajo se revisara nuestra serie de casos de adenomas corticotropos silentes y revisar la literatura acerca de esta patologia fascinante. Material y Metodo: Se revisaron las historias clinicas de 5 pacientes del Hospital Universitario Fundacion Santa Fe de Bogota en el periodo de enero de 1990 a enero de 2008, que se le realizo tratamiento para una masa sellar y se le realizo diagnostico patologico de Adenoma Corticotropos Silente. Discusion: Los adenomas de hipofisis corresponden entre el 17% de los tumores intracraneales. El 60% de estas lesiones se asocian a hipersecrecion hormonal. Los adenomas corticotropos silentes son tumores hipofisarios que histologicamente muestran diferenciacion corticotropica con algun grado de secrecion de ACTH pero sin la evidencia clinica de sindrome de Cushing. Conclusion: Aunque estos tumores sean clinicamente significativos y su histopatologia fascinante, hay poca literatura y estudios clinicos con enfoque especial sobre estas lesiones. Estos tumores son muy importantes tenerlos en cuenta en la patologia selar ya que su tratamiento requiere mayor dedicacion y seguimiento a estos pacientes. Material and Methods: we review the clinical history of 5 patients treated and diagnosed with Silent corticotrofic adenomas in University Hospital Fundacion Santa Fe de Bogota in the period of January 1990 till January 2008. Discussion: Pituitary adenomas count for 10 to 15% of intracranial tumors. In 60% of the cases there is a hormonal hypersecretory syndrome associated with these mass. Silent Corticotophic adenomas are pituitary tumors which histologically show a corticotrophic differentiation with secretion of ACTH but do not present with clinical evidence of Cushing disease. Although silent corticotophic adenomas are clinically significant and histologically fascinating there are only few clinical studies and medical literature with regard to these tumors. It is very important to take in regard this tumor in the pathology of the selar region since these patients demand a major dedication and a strict follow up. Craniocerebral gunshot wounds are often devastating. The in-hospital mortality for civilians with penetrating craniocerebral injury is 52-95% depending on the proportion of suicide victims in the series. The devastating nature of this lesion is accentuated by the fact that predominantly affects young and healthy patients. The most important predictive factor is the post- resuscitation Glasgow Coma Score (GCS). Early aggressive resuscitation, surgery and vigorous control of intracranial pressure offers the best chance of achieving a satisfactory outcome. 4. Analisis epidemiologico de 189 casos de hematoma epidural tratados quirurgicamente

- Abstract
Objective: To analyze the epidemiology, clinical and radiological presentation of patients with extradural hematoma (EDH) who underwent surgery in our service and their neurological status at the time of discharge. Method: we reviewed the charts of 189 patients who were admitted to our service with EDH and were treated surgically, during the period of August 1998 to January 2007. Results: In 49,2% the mechanism of trauma was falling; 49,7% of the cases had GCS between 13-15; 45,0% had skull fractures; 32,8% had associated intracranial injuries. Conclusion: we observed that 76,2% of surgically treated patients were discharged with minimum or no neurological deficits. 5. Ponticulus posticus incidence in brazilian Atlas vertebrae--a cadaveric study

- Abstract
The suboccipital segment of the vertebral artery (VA) passes around the lateral mass of atlas (C1). It is located in a groove on the superior surface of the arch of C1, the so-called vertebral artery sulcus (VAS). Occasionally, a small bony arch is found to connect the retroglenoid tubercle with the posterior arch of C1. This anatomical structure is known as ponticulus posticus (PP). The partial or total incidence of PP reported in the literature ranges from 5.14% to 51%. The clinical implication of PP includes the cervicogenic migraine, vertigo, and the Barre-Lieou syndrome. The scope of this paper is to determine the incidence of PP in isolated cadaveric vertebrae in the Brazilian population. The cervical vertebrae C1 from thirty not identified adult cadavers were examined at the Laboratory of Anatomy of the Federal University of Sergipe, Brazil. PP was present in 40% of evaluated C1 and it involved a complete bony arch in 42% of the cases. In 58% of the cases, PP was observed bilaterally. In the cases where the presence of PP was unilateral, this structure was situated exclusively on the right, eventhough a bigger incidence on the left side has been described for the majority of right-handed people. PP was a frequent finding in our study and its presence always must be suspected while setting the diagnosis of cervicogenic chronic headache and cervical pain without radiation to upper limbs. - Abstract
Introduction: Median nerve compression is the most common nerve entrapment syndrome. After carpal tunnel release, patients often complain about the scar cosmetic appearance. Currently the endoscopic devices can provide a faster and safer surgery with better cosmetic result. Methods: From november 2010 to march 2012 fifteen patients was operated by endoscopic techniques. Anesthesia with propofol and laringeal mask was realizated in 6 cases without cuff and local infiltration. Results: The mean of age was 37,4 years and the avarege of the time of the hospitalization was 5,9 hours and the pos operative pain was lower. Conclusion: Endoscopic Surgical treatment of Carpal tunnel syndrome provide a faster and safer surgery. - Abstract
The physical process of incision, traction and tissue cut utilized to proceed craniotomy stimulate nervous terminations and specific nociceptors, resulting in postoperative pain. Post operative cephalalgia is present in the majority of patients submitted to craniotomy procedures. The management of this clinical entity is yet to be standardized. Moreover, addressing outcome related to pain therapy is always a challenge due to the subjectiveness of pain. The goal of this study is to perform a review of the therapeutic options available in clinical practice in order to help clinicians and neurosurgeons when dealing with this common pain disorder. This is a narrative review of the literature from 1970 to December 2011 including reports, systematic reviews, all types of study and other literatures concerning acute pain management after craniotomy. The data was collected by doing a search of PubMed, EMBASE, Cochrane Reviews, and a manual search of all pertinent references in the literature. Sixty five researches were included and discussed on present review. Literature includes pharmacological treatment for post-craniotomy pain management as use of opioids like codeine, tramadol and morphine, non-steroidal anti-inflamatory drugs like cyclo-oxygenase 2 Inhibitors, gabapentin and scalp nerve block. Non-pharmacological strategies were identified in form of electromyography and criotherapy. The lack of an ideal protocol to treat patients suffering from post-craniotomy pain may represent an incomplete understanding of the physiopathology and neural mechanisms related to this syndrome. Future investigations focused on ideal protocols to control of pain after craniotomies are needed. - Abstract
La tercer ventriculostomía endoscópica (TVE) es actualmente una opción terapéutica ampliamente aceptada para el tratamiento de la hidrocefalia obstructiva. El reciente desarrollo tecnológico en este campo ha dado lugar a una renovación del interés en la TVE, considerado un procedimiento neuroquirúrgico seguro y bien establecida. A pesar de la relativa seguridad y eficacia de la TVE, innumerables informes han descrito complicaciones implicadas en el procedimiento. Como complicación postoperatoria de derivaciones ventriculares, el higroma subdural es un evento conocido, sin embargo, rara vez reportado en asociación con la TVE. A la luz del aumento en la utilización de técnicas endoscópicas, en especial la TVE, existe simultáneamente un incremento en la aparición de complicaciones relacionadas con el método. Esto nos motiva a realizar un estudio más detallado de sus mecanismos fisiopatológicos, y nos llevó a hacer una revisión de la aparición y de la fisiopatología del higroma subdural después de la TVE. The endoscopic thirdventriculostomy (ETV) is currently a widely accepted therapeutic option for the management of obstructive hydrocephalus. The recent technological development in the field has triggered a renewal in the interest in ETV, considered a safe and well-established neurosurgical procedure. Despite the relative safety and efficacy of the ETV, innumerable reports have described complications involved in the procedure. As a postoperative complication of ventricular derivations, the subdural hygroma is a commonly known event, however it is rarely reported in association with the ETV. In light of the increase in the utilization of endoscopic techniques, especially the ETV, there is simultaneously an increase in the occurrence of complications related to the method. This motivates us to perform a more detailed study of its physiopathological mechanisms, and brings us to make a review of the occurrence of the post-ETV subdural hygroma physiopathology. 9. Spontaneous thrombosis of aneurysm and posterior cerebral artery

- Abstract
Los aneurismas que se originan en la arteria cerebral posterior tienen particularidades que los distinguen de otros aneurismas intracraneales. También son raros, representando sólo el 0,7% y el 2,3% de todos los aneurismas intracraneales, con una tendencia mayor a ser grande o gigante. Trombosis parcial de los aneurismas gigantes es un fenómeno relativamente común, sin embargo trombosis total es un fenómeno poco frecuente. Este es un caso de un paciente que se presentó con déficits visuales asociadas con convulsiones. Sobre la investigación complementaria, se le diagnosticó un aneurisma gigante parcialmente trombosado en la arteria cerebral posterior. Ese paciente fue remitido a nuestro servicio para realizar el tratamiento endovascular. Se realizo una resonancia magnética que mostró trombosis total de la lesión con la oclusión del vaso portador. Para confirmar este hecho, una nueva angiografÃa cerebral se llevó a cabo lo que confirmó la oclusión espontánea del aneurisma y la arteria cerebral posterior. Esta situación, con la oclusión del aneurisma y el vaso original representa un acontecimiento raro, con sólo unos pocos casos descritos en la literatura. Aneurysms originating in the posterior cerebral artery have particularities which distinguish them from aneurysms in other intracranial circulation vessels. They are also rare, representing only 0.7% to 2.3% of all intracranial aneurysms, with an even greater tendency to be large or giant. Partial thrombosis of giant aneurysms is a relatively common phenomenon, however total thrombosis is an infrequent phenomenon. This is a case of a patient who presented with visual deficits associated with seizures. Upon complementary investigation he was diagnosed with a giant aneurysm, partially thrombosed, in the posterior cerebral artery. That patient was referred to our department to perform endovascular treatment. A magnetic resonance imaging was then performed and showed a total thrombosis of the lesion with occlusion of the carrier vessel. To confirm this fact, a new cerebral angiography was performed which confirmed the spontaneous occlusion of the aneurysm and the posterior cerebral artery. This situation, with occlusion of the aneurysm and its vessel of origin represents a rare event, with only a few cases reported in the literature. Renal carcinoma metastasis to meningioma is a rare phenomenon. Four cases have been described in the literature. We report a 74 year-old woman with progressive headache and right hemiparesia. Brain CT and MRI suggested a meningioma. The tumour was resected and the histological study confirmed intrameningioma metastatic renal clear cells. These findings permit us the diagnosis of the primary renal carcinoma and the posterior treatment of the primary tumour. Key words: Meningioma, renal carcinoma, metastasis, tumour to tumour. - Abstract
Introduction/Objective: The anterior temporal lobe has important subcortical structures, especially white fibers that lead visual information. Understanding this anatomical region, important for microsurgical practice, is based on fibers dissection techniques. They provide three-dimensional perspective for this region and add a successful surgical approach for the treatment of mesial temporal lesions. The purpose of this paper is to study the anatomy of the lateral wall of the lateral ventricle in order to determine a free area of the optical radiation. Methods: Ten cerebral hemispheres were dissected, prepared according to Klingler's techniques. Wooden spatulas with tips of various sizes were used. The optical radiation was delimited and measures were taken from this structure to the temporal pole, used as a reference point. Results: Approaches to the temporal horn larger than 27 mm beyond the temporal pole can cross Meyer's loop and determine injury to the optical radiation with consequent postoperatively deficits in visual fields. Conclusion: The determination of free area of optical radiation fibers is feasible. In this work we could infer that free area of optical radiation is located in the anterioinferior region of the temporal lobe at a distance of up to 2.7 centimeters from the temporal pole and allows access to the hippocampus and amygdala during epilepsy surgery. Larger resections than these measures can possibly determine injury to the optical radiation with consequent deficits in visual fields. Introduccion/Objetivos: El lobulo temporal anterior tiene importantes estructuras subcorticales, especialmente fibras blancas que llevan la informacion visual. La comprension de esta region anatomica, importantes para la practica microquirurgica, se basa en tecnicas de diseccion de fibras. Ellos proporcionan perspectiva tridimensional de esta region y anaden un enfoque quirurgico exitoso para el tratamiento de las lesiones temporales mesiales. El proposito de este trabajo es el estudio de la anatomia de la pared lateral del ventriculo lateral con el fin de determinar una zona libre de la radiacion optica. Metodos: Se diseccionaron diez hemisferios cerebrales, preparados de acuerdo con tecnicas de Klingler. Se utilizan espatulas de madera con puntas de diferentes tamanos. La radiacion optica fue delimitada y las medidas se tomaron a partir de esta estructura para el polo temporal, que se utiliza como punto de referencia. Resultados: Abordajes para el cuerno temporal superior a 27 mm mas alla del polo temporal pueden cruzar asa de Meyer y determinar un perjuicio a la radiacion optica con los consiguientes deficits en los campos visuales. Conclusion: La determinacion de la zona de libre de fibras de radiacion optica es factible. En este trabajo se podria inferir que el area libre de la radiacion optica se encuentra en la region anterioinferior del lobulo temporal a una distancia de hasta 2,7 centimetros desde el polo temporal y permite el acceso a el hipocampo y la amigdala durante la cirugia de la epilepsia. Resecciones mas grandes que estas medidas permiten aclarar de una lesion a la radiacion optica con los consiguientes deficits en los campos visuales. - Abstract
Moyamoya disease (MMD) is characterized by progressive stenosis and occlusion of the internal carotid arteries at the base of the skull. It was first observed in 1957 by Takeuchi and Shimizu in abnormal arteriograms of the brain. It accounts for 6% of strokes in children, and it is a major cerebrovascular disease in this age group in Japan. Most are sporadic cases of MMD, but there is also a familiar variant that occurs in approximately 8% of the times. The treatment of MMD depends on the patient's clinical presentation and the stage of the disease. Treatment options include observation and monitoring, medical treatment and surgical treatment, or even combinations of them. In general, there is no established recommendation regarding the management of the disease, once there is an absence of prospective randomized trials with reasonable follow-up. In this article we perform a review on the literature about the MMD, with emphasis on its surgical treatment. Key words: Moyamoya disease, cerebrovascular disorders, neurosurgical procedures. La enfermedad de Moyamoya (EMM) se caracteriza por la estenosis progresiva y la oclusion de las arterias carotidas internas en la base del craneo. Se observo por primera vez en 1957 por Takeuchi y Shimizu en arteriografias anormales del cerebro. Esto representa el 6% de los accidentes cerebrovasculares en los ninos, y es una enfermedad cerebrovascular importante en este grupo de edad en Japon. La mayoria son casos esporadicos de la EMM, pero hay tambien una variante familiar que se produce en aproximadamente el 8% de las veces. El tratamiento de la EMM depende de la presentacion clinica del paciente y la etapa de la enfermedad. Las opciones de tratamiento incluyen la observacion y el seguimiento, el tratamiento medico y el tratamiento quirurgico, o incluso combinaciones de ellos. En general, no existe una recomendacion establecida en relacion con la gestion de la enfermedad, una vez que hay una ausencia de estudios prospectivos aleatorizados con seguimiento razonable. En este articulo realizamos una revision de la literatura acerca de la EMM, con enfasis en su tratamiento quirurgico. El dolor ciático es una patologÃa inhabilitante que puede tener diversas etiologÃas, pero rara vez se describe como causada por un lipoma glúteo intermuscular. Los autores describen a un paciente con un dolor ciática derecha que empeora progresivamente durante 6 meses y con dificultad para caminar durante 3 meses. La resonancia magnética (RM) mostró un proceso expansivo compatible con lipoma entre el gluteo máximo, y el glúteo medio. El lipoma fue retirado por completo y sin lesión del nervio ciático. Después de la cirugÃa, el paciente está asintomático. Este es un caso inusual y representa un importante diagnóstico diferencial que debe ser considerado en los pacientes con ciática. Palabras clave: Ciática, Lipoma, Hernia Discal Lumbar. Sciatic pain is a disabling pathology that can have various etiologies, but is rarely described as caused by an intermuscular gluteal lipoma. The authors describe a patient with a progressively worsening right sciatic pain for 6 months and with walking difficulty for 3 months. Magnetic resonance imaging (MRI) showed an expansive process compatible with lipoma between the gluteus maximus and medius, which was completely removed without sciatic nerve injury. After surgery, the patient is asymptomatic. This is an unusual case and represents an important differential diagnosis that should be considered in sciatic pain patients. Los tumores cerebrales metastásicos son el tipo más frecuente de cáncer que afecta el sistema nervioso central (SNC). Muchos sitios pueden generar implantes para todo el cerebro, pero hay órganos que raramente generan metastasis para el SNC. Carcinoma peritoneal primario es un cáncer raro, y su patologÃa no es bien conocida, asà como sus vÃas metastásicas. Se presenta un caso de una paciente que se presentó con lesiones cerebrales difusas debido a un carcinoma peritoneal primario diagnosticado previamente. Brain metastatic tumors are the most frequent type of cancer affecting central nervous system. Many sites can generates implants for the entire brain, but there are organs that rarely generate metastasis. Primary peritoneal carcinoma is a rare cancer, and its pathology is not well known as well as its metastatic pathways. It is reported a case of a female patient who presented with diffuse brain lesions due to a primary peritoneal carcinoma previously diagnosed. We also perform a brief review about the theme. - Abstract
Objetivo: Evaluar los resultados en el manejo de pacientes con hemorragias intracerebrales lobares espontaneas, supratentoriales y sin extensión ventricular a través de la comparación entre el tratamiento quirúrgico y el tratamiento médico. Material y Método: Se realizó un estudio de casos y controles en el Hospital General "Roberto Rodríguez Fernández" de Morón, Ciego de Ávila, Cuba, en el período comprendido entre enero de 2009 y diciembre de 2013. La muestra quedó conformada por aquellos que reunieron los siguientes criterios de inclusión: 1) Edad de 18-60 años; 2) 48 horas de inicio de las manifestaciones clínicas; 3) volumen de la hemorragia entre 20 y 60 [cm.sup.3]; 4) Escala de coma de Glasgow al ingreso superior a 4 puntos. Se constituyeron dos grupos, en el Grupo I fueron incluidos los enfermos que recibieron tratamiento quirúrgico y médico. En el Grupo II se incluyeron los que recibieron tratamiento médico solamente. La inclusión en uno de los dos grupos quedó sujeta al criterio del médico que recibió al paciente en el departamento de emergencias, no existió aleatorización en el proceso. Resultados: Se incluyeron en el estudio un total de 147 pacientes 54 (36,73%) en el Grupo I y 93 (63,27%) en el Grupo II. En el Grupo I la mortalidad a los seis meses del egreso fue de 16 pacientes (29,64%), mientras que en el grupo II murieron 33 (35,48%). Conclusiones: Se obtuvo un discreto pero relevante predominio de los resultados satisfactorios en el grupo de pacientes operados y la mortalidad fue también menor en este grupo. The aim of this paper was to evaluate the surgical versus medical treatment in patients with supratentorial spontaneous intracerebral hemorrhage without ventricular extension. Methods: A case-control study was carried out in the "Roberto Rodríguez Fernández" general hospital in Moron, Ciego de Avila, Cuba, in the period between January 2009 and December 2013. The sample was conformed by those patients that gathered the following inclusion criteria: 1) Age of 18-60 years; 2) 48 hours of beginning of clinical manifestation; 3) Volume of the hemorrhage between 20-60 [cm.sup.3]; 4) GCS superior to 4 points. Conclusions: A discreet but outstanding of the satisfactory results was obtained in the group of operated patients and the mortality was also smaller in this group. The following papers is a technical note about a new endoscopic access to lateral ventricle through occipital horn to place ventricles catheters, 20 patient was operated using Deck System of endoscopy with lens of 30 grades, 2,7 mm of diameter and 30 cm of larger, sheets and working canals including. CT scan was performing 24 hours after surgery and optimal position was obtained in 95%. Median distance from occipital bon to frontal horn of the ventricles was 10,5 cm. El siguiente trabajo constituye una nota tecnica sobre un nuevo proceder endoscopico de colocacion de cateteres ventriculares permanente a traves del cuerno occipital. Se colocaron 20 cateteres permanentes utilizando el sistema DECK con un endoscopio Hopkins II de 30[degrees], diametro 2,7 mm y longitud 30 cm, con vainas y canales de trabajos. La posicion del cateter fue evaluada por tomografia de craneo realizada a las 24 horas de la cirugia, obteniendose una posicion a (optima) en el 95% de los casos, con una distancia promedio de 10,5 cm desde la tabla externa del hueso occipital al cuerno frontal. En ninguno de los casos disfunciono el sistema derivativo y un solo paciente presento como complicacion un hematoma del lecho quirurgico. Introducción: Para utilizar un meta-análisis de todos los casos reportados de la estimulación cerebral profunda (DBS) para la distonia para determinar cuáles son los factores significativos resultados influencia relacionada con el destino. La escala de movimiento Burke-Fahn-Marsden (BFM), la medida más informado, fue elegida como la principal medida de resultado para este análisis. Material y Métodos: Una búsqueda en MEDLINE identificaron 137 pacientes que se sometieron a DBS para la distonia en 24 estudios que tenían puntuaciones individuales BFM. Datos de los pacientes individuales, incluyendo la edad de inicio de la distonia, la edad de la cirugía, el género, la distribución de la distonia, la etiología de la distonia, la presencia de características asociadas, anormalidad de las imágenes preoperatorias, cirugias estereotáxica anteriores, el núcleo estimulado, el tipo de anestesia que se utiliza, el tiempo de respuesta a la estimulación, y el momento de la evaluación de resultados se introdujeron en una base de datos de SPSS para el análisis estadístico. Resultados: La media BFM cambio porcentual (mejora en la puntuación postoperatoria de la línea de base) fue 51,8% (rango--34% a 100%). Significativamente se lograron mejores resultados con la estimulación del globo pálido interno (GPI) que con la estimulación de la parte posterior del núcleo lateral ventral (VLP) del tálamo (p = 0,0001). La etiología de la distonia también tuvo un efecto significativo en los resultados. Mejorías estadísticamente significativas en los resultados se observaron para todas las categorías etiológicos, excepto encefalitis. Distonia debido a la lesión en el parto y la encefalitis tenían significativamente peores resultados de los pacientes que eran positivos DYT1 gen, gen DYT1 negativos o tenían asociada a pantotenato quinasa-degeneración neuro (PKAN), disquinesia tardía y distonías idiopáticas y postraumático. La mayor duración de síntomas relacionados con distonia una correlación negativa con el resultado quirúrgico. Modelo de regresión utilizando el sitio tres variables-estimulación, la etiología de la distonia, y la duración de la distonia síntomas-explicó el 51% de la varianza en los resultados. Conclusión: La estimulación cerebral profunda de la IPG proporciona una mejoría en las puntuaciones de BFM en una variedad de condiciones distónicos. Introduction: To use a meta-analysis on all reported cases of deep brain stimulation (DBS) for dystonia to reevaluate the good effect using the GPi as a target, which factors significant influence outcome related to the target. The Burke-Fahn-Marsden (BFM) movement scale, the most reported measure, was chosen as the primary outcome measure for this analysis. Material and Methods: Computerized MEDLINE searches on English literature search identified 137 patients who underwent BBS for dystonia in 24 studies that had individual BFM scores. The study was done with statistical analysis by intention to treat. Statistical analysis was made with a significant p- value of 0.05. For the comparison of pre- and postoperative scores, a test Wilcoxon signed was used. Results: The mean BFM percentage change (improvement in postoperative score from baseline) was 46.3%(range--34% to 100%). At last follow-up, disease severity and the degree of disability and pain on the BFM were significantly improved by 70.4%, and 67.8%, respectively (p < 0.05, Wilcoxon signed-rank test). Significantly better outcomes were achieved with stimulation of the globus pallidus internus (GPI) than with stimulation of the posterior portion of the ventral lateral (VLp) nucleus of the thalamus (p < 0.05). The etiology of the dystonia also had a significant effect on outcomes. Statistically significant improvements in outcomes were seen for all etiologic categories, except encephalitis. Dystonia due to birth injury and encephalitis had significantly worse outcomes of patients who were DYT1, or had pantothenate-kinase-associated neurodegeneration (PKAN), tardive dyskinesia, and idiopathic and posttraumatic dystonias. Longer duration of dystonia symptons correlated negatively with surgical outcome. Conclusion: Deep Brain stimulation of the GPi provides improvement in BFM scores in a variety of dystonic conditions. Introducción: El Glioblastoma (GB) o Astrocitoma grado IV (OMS), representan 15-20% de los tumores del SNC y aproximadamente 50% de los gliomas en adultos. Objetivo: Revelar el perfil epidemiológico del HSCMRP, correlacionar los hallazgos macroscópicos y microscópicos durante la cirugía de enero de 2011 a noviembre de 2015. Método: Estudio epimedeológico observacional, descriptivo, retrospectivo, 429 casos de tumores intracraneales a partir de los datos obtenidos de los archivos de La institución y los registros patológicos de los pacientes tratados quirúrgicamente. Resultados: Tumores encontrados 429 y 96 (22,37%) gB, edad media de 59 años, predominante séptima década 33%. Una relación entre mujeres y hombres fue de 1:1.12, respectivamente. Las quejas más frecuentes: dolor de cabeza (58%), confusión (41%), hemiparesia 37%. Comorbilidades frecuentes: hipertensión (64%), diabetes (22%) y fumadores (24%). La topografía más común fue la frente izquierdo. El tiempo medio de inicio de los síntomas a la cirugía fue de 39 días. Resección completa en 76% de los casos. La duración media de la recurrencia fue de 96 días, en 68% de los pacientes se observó una exuberancia de los vasos trombosados durante la cirugía Hallazgos patológicos: necrosis 98%, mitosis atípica 96%, proliferación microvascular 73% y polimorfismo nuclear 57%. Discusión: gB estado del arte. Conclusión: Nuestros resultados son similares con la literatura. Observación intraoperatoria de vasos trombosados y agresividad tumoral en pacientes con peor pronóstico y menor tiempo de recaída sugiere que es real, sin embargo. El pequeño número de casos, necesita más investigación, incluyendo otros hallazgos y resultados inmunohistoquímicos. The average time of onset of symptoms to surgery was 39 days. Complete resection in 76% of cases. The mean length of postoperative recurrence was 96 days, in 68% patients were noticed an exuberance of thrombosed vessels during surgery. Pathological findings: necrosis 98%, atypical mitosis 96%, microvascular proliferation 73% and nuclear polymorphism 57%. Discussion: GB state of art. Conclusion: Our results are very slightly with the literature. The association of intraoperative observation thrombosed vessels, and tumor aggressiveness in patients with worse prognosis and shorter time to relapse, suggests that it is real, however, the small number of cases, needs further investigation, including other findings and immunohistochemical results. - Abstract
Objective: Multiple subpial transections (MST) represent a technical option of surgical treatment for patients with epileptogenic foci located in eloquent cortical areas. They could be performed in addition to other surgical techniques or alone. We report the clinical results of 10 patients who received as single MST surgery with a minimal follow-up of 5 years. Methods: The authors studied all patients who underwent a surgical intervention between 2007 and 2019 for refractory epilepsy. Among them, 10 had radiating MST (rMST) as the only surgical treatment with a follow-up of at least 5 years. Results: At 5-year follow-up, 80% of our patients were Engel class I, 20% were Engel class II, none were Engel class III, and none were Engel class IV. At last follow-up, 6 patients (60%) were free of seizures, two (20%) had an over 75% decrease, and two (20%) did not improve after the procedure. None of the Engel I patients had seizure recurrence, and those belonging to an intermediate class improved during follow-up, in some cases in association with an anti-epileptic drug modification. Two (20%) had a minor transient complication, and two patients (20%) had a minor permanent complication. Conclusions: MST performed alone gives a favorable outcome in 80% of the patients at a minimum 5-year follow-up with few minor complications. This procedure appears to be effective even with a prolonged follow-up in refractory epilepsy with the epileptogenic foci located in eloquent areas. Key words: MST, refractory epilepsy surgery. Objetivo: Las transecciones subpiales múltiples (MST) representan una opción técnica de tratamiento quirúrgico para pacientes con focos epileptogénicos ubicados en áreas corticales elocuentes. Podrían realizarse además de otras técnicas quirúrgicas o solo. Presentamos los resultados clínicos de 20 pacientes que recibieron una cirugía MST única con un seguimiento mínimo de 5 años. Métodos: Los autores estudiaron a todos los pacientes que se sometieron a una intervención quirúrgica entre 2007 y 2019 para la epilepsia refractaria. Entre ellos, 20 tenían MST radiante (rMST) como el único tratamiento quirúrgico con un seguimiento de al menos 5 años. Resultados: A los 5 años de seguimiento, el 80% de nuestros pacientes eran Engel clase I, el 20% eran Engel clase II, ninguno era Engel clase III y ninguno era Engel clase IV. En el último seguimiento, 12 pacientes (60%) no sufrieron convulsiones, quatro (20%) tuvieron una disminución de más del 75% y quatro (20%) no mejoraron después del procedimiento. Ninguno de los pacientes con Engel I tuvo recurrencia de ataques, y los que pertenecían a una clase intermedia mejoraron durante el seguimiento, en algunos casos en asociación con una modificación farmacológica antiepiléptica. Cuatro (20%) tuvieron una complicación transitoria menor, y cuatro pacientes (20%) tuvieron una complicación permanente menor. Conclusiones: La MST realizada sola da un resultado favorable en el 80% de los pacientes (16) con un seguimiento mínimo de 5 años con pocas complicaciones menores. Este procedimiento parece ser efectivo incluso con un seguimiento prolongado en la epilepsia refractaria con los focos epileptogénicos ubicados en áreas elocuentes. Introduction: Although the vagal nerve stimulation has been described significant results in the management of medically intractable seizures, it still remain a question regarding its applicability in pediatric patients. Objective: To analyse and to discuss the risks, complications, results as well de prognosis of vagal nerve stimulation in pediatric patients. Methods: It was performed bibliographical consultation, using the databases MEDLINE, LILACS, SciELO, utilizing language as selection criteria, choosing preferably recent articles in Portuguese, Spanish or English. Results: The vagal nerve stimulation has been described associated to a low technical difficulty, short surgical time and enhance of control of seizures. Vagal stimulation has been demonstrated a significant effect in the reduction of seizures frequency and drop attacks' intensity and duration, as well as the improvement in quality of life in pediatric patients. Conclusion: In spite of the results described in childhood epilepsy, it is still initial surgical approach of epilepsy and needs more clinical studies to verify the impact of this procedure in these patients in the long term. Introducción: Aunque la estimulación del nervio vago ha sido descrita como resultados significativos en el tratamiento de las convulsiones médicamente intratables, sigue siendo una cuestión con respecto a su aplicabilidad en pacientes pediátricos. Objetivo: Analizar y discutir los riesgos, las complicaciones, los resultados y el pronóstico de la estimulación del nervio vago en pacientes pediátricos. Métodos: Se realizó consulta bibliográfica, utilizando las bases de datos MEDLINE, LILACS, SciELO, utilizando el idioma como criterio de selección, eligiendo preferiblemente artículos recientes en portugués, español o inglés. Resultados: La estimulación del nervio vagal se ha descrito como una dificultad técnica baja, un tiempo quirúrgico corto y un mejor control de las convulsiones. La estimulación vagal ha demostrado un efecto significativo en la reducción de la frecuencia de los ataques y en la intensidad y duración de los ataques de caída, así como en la mejora de la calidad de vida en pacientes pediátricos. Conclusión: A pesar de los resultados descritos en la epilepsia infantil, sigue siendo un abordaje quirúrgico inicial de la epilepsia y necesita más estudios clínicos para verificar el impacto de este procedimiento en estos pacientes a largo plazo. Cerebrovascular malformations are rare pathologies in general population (1%), which are diagnosed by neurological symptoms produced by alteration on the dynamics of blood flow, or as incidental findings during CT Scan or Magnetic resonance with contrast agent studies. These lesions have been classified in four types: Anomalies on venous development, capillary telangiectasia, arteriovenous malformations and cavernous angioma. Nevertheless, it has not been possible to conclude if there are different entities or a same disease with different stages, since in some cases there is possible to identify more than one kind of lesion in the same patient, which is known as mixed vascular malformation, making a challenging situation for the medical treatment. The objective of this paper is to expose two cases of patients with mixed vascular malformations, it is the association between a venous development anomaly and capillary telangiectasia, in a supratentorial and in posterior fossa location, as an incidental finding during a magnetic resonance with contrast agent. We share these cases given the infrequency of their report in literature, which surely will allow to complement the knowledge about the natural history and etiology of cerebrovascular malformations, understanding better the pathology and giving a better treatment to the patients. Las malformaciones cerebrovasculares son lesiones infrecuentes en la población general (1%), siendo diagnosticadas por síntomas neurológicos secundarios a la alteración de la dinámica del flujo sanguíneo o como hallazgos incidentales durante estudios por tomografía o resonancia magnética con medio de contraste. Se han clasificado comúnmente en cuatro tipos: Anomalías del desarrollo venoso, telangiectasias capilares, malformaciones arteriovenosas y angiomas cavernosos. Sin embargo aún no se ha podido llegar a concluir si se trata de entidades diferentes o a una misma entidad con diversas etapas evolutivas, ya que se presentan casos en donde es posible identificar más de una lesión en un mismo paciente, a lo que se conoce como malformaciones vasculares mixtas, presentando de esta manera un reto para el manejo médico cuando así sucede. El objetivo de este artículo es exponer dos casos de pacientes con malformaciones vasculares mixtas, se trata de la asociación infrecuente de una anomalía del desarrollo venoso y una telangiectasia capilar, de ubicación supratentorial y en fosa posterior, hallados de forma incidental durante un estudio de resonancia magnética con medio de contraste. Compartimos estos casos dado lo infrecuente de sus reportes en la literatura, lo que seguramente permitirá a futuro complementar el conocimiento con respecto a la historia natural y etiología de las malformaciones cerebrovasculares, repercutiendo esto en un mejor entendimiento de la patología y por consecuencia del manejo a nuestros pacientes. Introducción: La HSA constituye del 3 al 5% de todos los ataques cerebro vasculares en la población general. Es una enfermedad de alto impacto en la sociedad, por su elevada morbimortalidad. Se han realizado múltiples estudios retrospectivos, pero no hay ningún estudio prospectivo nacional actualizado de dicha patología en Uruguay. Aquí se publican datos preliminares de un trabajo anual con el objetivo de conocer las características demográficas, epidemiológicas, así como las características topográficas y morfo-estructurales de los aneurismas, y la evolución de los pacientes portadores de esta patología. Materiales y Métodos: Se esta realizando un trabajo multicéntrico de una cohorte longitudinal prospectivo de todos los pacientes que se presentan con HSA en el Uruguay en un período de un año. En esta presentación preliminar se incluyen para un primer análisis todos los pacientes entre el 1 de noviembre de 2019 y el 31 de enero de 2020. Se incluyeron todos los pacientes mayores de 18 años que consultaron y se les diagnosticó una HSA espontánea. Una vez hecho el diagnóstico de HSA, dependiendo de la gravedad del paciente se realizó un estudio vascular (angio-TC o arteriografía) para valorar la presencia o no de aneurisma, y la topografía del mismo para decidir el tratamiento. Se analizaron las siguientes variables: edad, sexo, procedencia geográfica, antecedentes, motivo de consulta, escala de coma de Glasgow (GCS), escala de World Federation of Neurological Surgeons (WFNS), escala Hunt y Hess, escala de Fisher, consultas previas al ingreso, tratamiento indicado, complicación vinculada a la HSA, complicación vinculada al tratamiento indicado, y escala de rankin modificado (mRS) al mes. Se creó una máscara de captura, y el análisis se procesó utilizando el software SPSS versión 22. La investigación fue aprobada por todos los Comité de Ética de los diferentes centros que participaron del estudio. Resultados: De un total de 36 pacientes, la edad media fue de 55,2 años, con mayor frecuencia en el sexo femenino. En el 83,3% de los pacientes el síntoma que predominó fue la cefalea y en la mitad de los casos estuvo asociada a vómitos. El trastorno de conciencia se presentó en 55% de los casos. Hubo 25% de error diagnóstico. El estudio diagnóstico de aneurisma más frecuente usado fue la angio TC en el 80% de los casos, y la arteriografía se realizó en el 41%. En el 63% de los casos se realizó cirugía abierta, y en el 14% se realizó tratamiento endovascular. Los aneurismas más frecuentes fueron el comunicante anterior, el silviano y el comunicante posterior. El 80% presentó complicaciones vinculadas a su HSA. A los 30 días del ictus 11 pacientes (30%) fallecieron (mRS 6), en la gran mayoría de los casos la causa fue neurológica. Debemos destacar que la mayoría de los pacientes que fallecieron ingresaron con un mal estado neurológico. Conclusiones: Este trabajo presenta las primeras cifras sobre las características epidemiológicas de la Hemorragia subaracnoidea en el Uruguay. It is a disease of high impact on society, because of its high morbidity and mortality. Multiple retrospective studies have been carried out, but there is no updated national prospective study of this pathology in Uruguay. Preliminary data from an annual study are published here with the aim of knowing the demographic, epidemiological characteristics, as well as the topographic and morpho-structural characteristics of aneurysms, and the evolution of patients with this pathology. Materials and Methods: A multicenter study of a prospective longitudinal cohort of all patients who presented with SAH in Uruguay in a period of one year is carried out, including in this first analysis all patients between November 1, 2019 and January 31, 2020. All patients older than 18 years who consulted were included. They were diagnosed with spontaneous SAH. Once the diagnosis of SAH was made, depending on the severity of the patient, a vascular study was performed (angio-CT or arteriography) to assess the presence or absence of aneurysm, and the topography of it to decide the treatment. The following variables were analyzed: age, sex, geographical origin, history, reason for consultation, Glasgow Coma Scale (GCS), World Federation of Neurological Surgeons (WFNS) scale, Hunt and Hess scale, Fisher scale, previous consultations at admission, indicated treatment, complication related to SAH, complication related to indicated treatment, and modified rankin scale (mRS) at month. A capture mask was created, and the analysis was processed using SPSS version 22 software. The research was approved by all the Ethics Committee of the different centers that participated in the study. Results: out of a total of 36 patients, the average age was 55,2 years, more frequently in females. In 83,3% of the patients the predominant symptom was headache and in half of the cases it was associated with vomiting. Loss of consciousness occurred in 55% of cases. There was a 25% diagnostic error. The diagnostic study of the most frequent aneurysm used was CT angiography in 80% of cases, and arteriography was performed in 41%. Open surgery was performed in 63% of cases, and endovascular treatment in 14 % of cases. The most frequent aneurysms were anterior communicant, sylvian and posterior communicating. 80% presented complications related to their SAH. 30 days after stroke, 11 patients (30%) died (mRS 6), in the great majority of cases the cause was neurological. We must emphasize that the majority of patients who died were admitted with a poor neurological status. Conclusions: This paper presents the first figures on the annual incidence and epidemiological characteristics of Subarachnoid Hemorrhage in Uruguay. Antecedentes: El tratamiento quirúrgico de los tumores en aquellas áreas del cerebro consideradas de difÃcil abordaje se está volviendo accesible gracias a la cirugÃa mÃnimamente invasiva. Mediante el uso de un sistema retractor tubular, se proporciona desplazamiento circular y distribución simétrica del cerebro, reduciendo las complicaciones postoperatorias de retracción prolongada, asà como el riesgo de edema e infarto. Esto se ha convertido en un método ideal para llegar a áreas como las regiones parenquimatosas profundas, subcorticales e intraventriculares. Métodos: De enero de 2015 a noviembre de 2018, se seleccionaron trece casos de pacientes diagnosticados con tumores intraparenquimatosos o intraventriculares. La resección quirúrgica se realizó con la ayuda del microscopio, neuroendoscopio y retractores tubulares. El estudio volumétrico y la profundidad del tumor se obtuvieron con la RM mediante el sistema de neuronavegación. La profundidad se midió desde el punto de entrada en la superficie cortical hasta el área más cercana al tumor. Resultados: Se logró resección total macroscópica en ocho pacientes, resección subtotal en tres, resección parcial en un caso y se realizó una biopsia. Un paciente desarrolló hidrocefalia y otro una fÃstula de lÃquido cefalorraquÃdeo (LCR) como complicación posquirúrgica. Conclusiones: La cirugÃa mÃnimamente invasiva para la escisión de tumores podrÃa permitirnos acceder directamente a lesiones cerebrales profundas y disminuir la lesión del tejido cerebral circundante, asà como prevenir adherencias de este en los retractores tubulares. Simultáneamente, la técnica microquirúrgica y endoscópica podrÃa acortar el tiempo quirúrgico. El propósito de esta técnica es reducir la morbilidad asociada a los procedimientos quirúrgicos convencionales y los dÃas de hospitalización en la Unidad de Cuidados Intensivos, mejorando asà la calidad de vida. Background: Surgical treatment of tumors in those areas of the brain considered difficult to reach is becoming accessible thanks to minimally invasive surgery. Through the use of a tubular retractor system, a circular displacement and a symmetrical distribution of the brain are provided, reducing the postoperative complications of prolonged retraction, as well as the risk of edema and infarction. This has become an ideal method to reach areas such as deep parenchymal, subcortical and intraventricular regions. Methods: From January 2015 to November 2018, we selected thirteen cases of patients diagnosed with intraparenchymal or intraventricular tumors. Surgical resection was performed with the assistance of a microscope, neuroendoscope, and brain tubular retractors. The volumetric study and the depth of the tumor were obtained on the MRI using the neuronavigation system. Depth was measured from the entry point on the cortical surface to the area closest to the tumor. Results: Gross total resection was achieved in eight patients, subtotal resection in three, partial removal in one case and one biopsy was performed. One patient developed hydrocephalus and one a cerebrospinal fluid (CSF) fistula as a post-surgical complication. Conclusions: Minimally invasive surgery for tumor excision could allow us to directly access deep brain lesions and decrease injury to the surrounding brain tissue, as well as prevent adhesions from it in tubular retractors. Simultaneously, the microsurgical and endoscopic technique could shorten the surgical time. The purpose of this technique is to reduce the morbidity associated with conventional surgical procedures and the days of hospitalization in the Intensive Care Unit, thus improving quality of life. - Abstract
Introducción: Los síndromes paraneoplásicos neurológicos son manifestaciones remotas del cáncer extraneurológico mediadas por el sistema inmune. A pesar de ser poco frecuentes, es muy importante tenerlos en cuenta a la hora del proceso diagnóstico de los síndromes neurológicos, debido a que una detección y tratamiento oportunos puede mejorar ostensiblemente la calidad de vida de los pacientes. Material y Métodos: Se realizó una revisión de los trabajos relacionados con el tema publicados en los últimos 10 años, en las plataformas Medline/Pubmed, Imbiomed y SciELO. Conclusiones: Cuatro pilares generales sustentan el diagnóstico de los síndromes neurológicos paraneoplásicos: sospecha clínica, exclusión de otras causas del síndrome neurológico, determinación de anticuerpos paraneoplásicos y búsqueda del cáncer responsable. Los tres principios para su tratamiento son: terapias de soporte, control del cáncer responsable y la inmunoterapia. Background: Neurological paraneoplastic syndromes are remote manifestations of extraneurological cancer mediated by the immune system. Despite being infrequent, it is very important to take them into account when diagnosing neurological syndromes, because timely detection and treatment can significantly improve the quality of life of patients. Material and Methods: A review of the papers related to the topic published in the last 10 years was carried out on the Medline / Pubmed, Imbiomed and SciELO platforms. Conclusions: Four general pillars support the diagnosis of paraneoplastic neurological syndromes: clinical suspicion, exclusion of other causes of the neurological syndrome, determination of paraneoplastic antibodies and search for the responsible cancer. The three principles for its treatment are: support therapies, responsible cancer control and immunotherapy. La neuromiotización en lesiones de nervio radial es una técnica quirúrgica utilizada por neurocirujanos, así como también por traumatólogos, donde, a través de transferencias tendinosas a nivel del antebrazo y en la muñeca, se busca reestablecer la función motora distal, perdida por la lesión neural. A saber: la dorsiflexión de la muñeca, la extensión de los dedos trifalángicos y la extensión del dedo pulgar. Aunque es una técnica quirúrgica que tiene una baja incidencia debido a los buenos resultados de la microcirugía y rehabilitación frente las lesiones de nervio radial, la neuromiotización actualmente está considerada como la "última carta a jugar" en este tipo de lesiones de nervio periférico. Para lograr el objetivo mencionado de la técnica, se necesita un largo proceso que involucra tres cirugías distintas, kinesioterapia, rehabilitación funcional y reeducación mental al considerar que, un tendón que originalmente realizaba una función motora, ahora realizará otra diferente. Sabiendo esto, en el presente trabajo se mencionarán temas como historia de la técnica, sus generalidades, indicaciones y contraindicaciones, la técnica quirúrgica propiamente tal, resultados locales en comparación con algunas series internacionales, terminado con algunos ejemplos destacables de pacientes que se les ha practicado esta cirugía. Neuromyotization in Radial Nerve Injuries: A surgical technique used by neurosurgeons as well as orthopedic surgeons, wherein tendon transfers at the forearm and wrist levels are performed to restore distal motor function lost due to neural injury. Specifically, wrist dorsiflexion, three-phalange finger extension, and thumb extension are targeted. Despite its infrequent application owing to the favorable outcomes of microsurgery and rehabilitation for radial nerve injuries, neuromyotization is currently regarded as the 'last resort' in such cases of peripheral nerve injury. Achieving the technique's intended goal necessitates a lengthy process involving three distinct surgeries, physiotherapy, functional rehabilitation, and mental reeducation, considering that a tendon originally serving one motor function will now serve another. Given this context, the present work will delve into topics such as the history of the technique, its general aspects, indications and contraindications, the surgical procedure itself, local outcomes compared to certain international series, and will conclude with notable patient case examples who have undergone this surgery.
