Abstract Background This study aimed to investigate a technique for treating pediatric popliteal cysts with a triple-channel knee arthroscopic approach, hypothesizing that this technique exhibits favorable clinical outcomes with reduced recurrence rates. Methods A retrospective analysis was conducted on the clinical data of 14 patients with symptomatic pediatric popliteal cysts admitted to the joint surgery center using an arthroscopic minimally invasive technique from December 2017 to December 2020. Arthroscopy was employed for adequate enlargement of the posterior medial flap opening between the cyst and the joint cavity and to completely resect the cyst wall. The popliteal cyst recurrence was assessed through Magnetic Resonance Imaging(MRI) at 3 and 12 months post-surgery, and the Rauschning and Lindgren scores were recorded to evaluate the clinical prognosis. Results A total of 14 children completed the follow-up, including 8 boys and 6 girls, with a mean age of 9.9 ± 1.1 years (5–12 years); 5 cases in the left knee and 9 cases in the right knee; the mean operative time was 19.2 ± 2.7 min (10–25 min); and the mean follow-up time was 15.1 ± 2.4 months (12–24 months). There was no intraoperative vascular or nerve injury and no incisional exudate infection. Postoperatively, posterior knee symptoms disappeared in the children, Rauschning and Lindgren scores significantly improved, and no recurrence signs were observed on MRI. Conclusions This study determined that the arthroscopic three-channel minimally invasive technique is effective and safe for treating popliteal cysts in children, making it worthy of clinical promotion.

Abstract Background This study aimed to compare the clinical effect of modified anterolateral and traditional acromioplasty in arthroscopic rotator cuff repair. Methods The clinical data of 92 patients with total rotator cuff tears admitted to the Department of Joint Surgery of Jinhua Central Hospital from January 2016 to December 2019 were retrospectively analyzed. Among them, 42 patients underwent traditional acromioplasty during arthroscopic rotator cuff repair, and 50 underwent modified anterolateral acromioplasty. Patients were evaluated for preoperative and postoperative shoulder function, pain and critical shoulder angle, and incidence of rotator cuff re-tear at 12 months postoperatively. Results The preoperative general data of patients in the classic and modified anterolateral acromioplasty groups did not differ significantly (P > 0.05) and were comparable. The UCLA, ASES, and Constant shoulder joint scores were significantly improved in both groups. The VAS score was significantly decreased at 12 months postoperative than preoperative, with a statistically significant difference (P ≤ 0.05). Shoulder function and pain scores did not differ significantly between the two groups at 12 months postoperatively (P > 0.05). The CSA did not differ significantly between preoperative and postoperative 12 months in the traditional acromioplasty group (P > 0.05). However, 12 months postoperative CSA in the modified anterolateral acromioplasty group was significantly smaller than the preoperative CSA, with a statistically significant difference (P ≤ 0.05). The rates of rotator cuff re-tears were 16.67% (7/42) and 4% (2/50) in the two groups at 12 months postoperatively, respectively, with statistically significant differences (P ≤ 0.05). Conclusions Traditional and modified anterolateral acromioplasty while treating total rotator cuff tears using arthroscopic rotator cuff repair significantly improves shoulder joint function. However, modified anterolateral acromioplasty significantly reduced the CSA value and decreased the incidence of rotator cuff re-tears.

Marine ecosystem governance is an important component of the goal of developing a strong maritime nation, as well as an urgent need to ensure national maritime security. The “Bay Chief System”(BCS) is a novel policy concept with Chinese roots that seeks to improve the marine ecological environment. In this paper, we take the coastal cities in China as an example and use the panel data of prefecture-level cities from 2010 to 2022 to analyse the impact of the Bay Chief System on the regional marine ecological environment by using a differences in-differences model. The baseline regression results reveal that the implementation of the Bay Chief System leads to a 0.122 percentage point improvement in marine ecosystems but it has yet to be linked to the River Chief System(RCS) to establish a systematic synergistic mechanism. Heterogeneity analysis reveals that the implementation of the Bay Chief System is more effective in coastal cities with smaller populations, a heavier proportion of secondary industries, and a geographic location closer to the South. The analysis of the impact mechanism indicates that the Bay Chief System mitigates the adverse effects of near-shore resource development on the marine ecological environment by regulating the intensity of fishery and coastal tourism development. Additionally, it optimizes water quality by substantially reducing pollutants like chemical oxygen demand (COD) and ammonia nitrogen (AN) in seawater, ultimately contributing to the protection of the marine ecological environment.

BackgroundUveitis, characterized by inflammation of the iris, ciliary body, and choroid, presents a significant global clinical challenge, contributing substantially to visual impairment. Risk factors include autoimmune diseases and immune cell dysfunctions, yet many remain unidentified. Immune cells, notably T cells, B cells, and monocytes, play pivotal roles in uveitis pathogenesis. While biologic agents show promise, comprehensive studies on immune cell types in ocular diseases are lacking. Genome-wide association studies (GWAS) and Mendelian randomization (MR) present promising avenues to elucidate genetic susceptibilities and causal relationships between immune cell traits and uveitis risk.MethodsTwo-sample MR analysis was used to evaluate the causal relationship between 731 immune cells and uveitis, and genome-wide significance analysis was performed for genetic variation in 731 immune cells traits (P < 5 × 10-8). Immune characteristics include median fluorescence intensity (MFI), relative cell counts (RC), absolute cell counts (AC), and morphological parameters (MP), which were determined by published GWAS, and public data from the IEU Open GWAS database. The main analysis method of MR is inverse variance weighting (IVW). Heterogeneity and horizontal pleiotropy were also assessed.Results5 immunophenotypes, including CD62L-DC %DC, IgD+ CD38dim %B cell, CD3 on CM CD4+T cell, CD3 on CD45RA-CD4 +T cell, and CD3 on CD39+ CD4+ Treg may increase the risk of uveitis. 5 immunophenotypes, including CD11b on CD33dim HLA DR-Myeloid cell, HLA DR on CD33dim HLA DR+ CD11b-myeloid cell, CD14-CD16 + %monocyte, HLA DR on CD14-CD16 + monocyte and PDL-1 on CD14-CD16 + monocyte was negatively associated with the risk of uveitis. Among them, HLA DR on CD14-CD16 + monocyte (OR=0.921, 95%CI =0.875-0.970, P=0.001) and HLA DR on CD33dim HLA DR+ CD11b- (OR=0.879, 95%CI = 0.833-0.927, P=0.00) were negatively associated with the risk of uveitis in bi-direction.ConclusionThese results indicate that 10 immune cells traits are significantly associated with the risk of developing uveitis and 2 of them were strongly associated with uveitis bi-directionally, after excluding the effects of confounding factors such as some immune diseases, which provided new ideas and therapeutic targets for the study of immune mechanism of uveitis.

This study aimed to evaluate the efficacy and safety of intravitreal dexamethasone implants in the treatment of ocular toxocariasis (OT). A retrospective analysis was performed on 6 cases in which laboratory tests diagnosed OT. All patients were administered with intravitreal dexamethasone implants with or without vitrectomy. The average follow-up time was 19.7 months. All operated eyes achieved anatomic success, and all patients’ visual acuity was improved. Five of these six had a visual acuity of 20/100, and three had final acuity of 20/40 or even better. Intravitreal dexamethasone implants can be used to treat different types of OT, which not only effectively control inflammation and improve the patient’s vision but also reduce the use of systemic glucocorticoids.

Amid global climate change and population growth, the prevalence of saline–alkali lands significantly hampers sustainable agricultural development. This study employs theories of asymmetric information and bounded rationality to construct an evolutionary game model, analyzing the interactions among small farmers, family farms, and seed industry enterprises in the context of saline–alkali land management. It investigates the strategic choices and dynamics of these stakeholders under the influence of economic incentives and risk perceptions, with a focus on how government policies can foster green development. Utilizing Delay Differential Equations (DDEs) for simulations, this study highlights the risk of “market failure” without government intervention and underscores the need for government participation to stabilize and improve the efficiency of the green development process. The findings reveal that factors such as initial willingness to participate, the economic viability of salt-tolerant crops, seed pricing, research and development costs, and the design of incentive policies are crucial for sustainable land use. Accordingly, the paper proposes specific policy measures to enhance green development, including strengthening information dissemination and technical training, increasing the economic attractiveness of salt-tolerant crops, alleviating research and development pressures on seed companies, and optimizing economic incentives. This study provides a theoretical and policy framework for the sustainable management of saline–alkali lands, offering insights into the behavioral choices of agricultural stakeholders and supporting government strategies for agricultural and environmental protection.

Rhodopsin is a light-sensitive G protein-coupled receptor that initiates the phototransduction cascade in rod photoreceptors. Mutations in the rhodopsin-encoding gene RHO are the leading cause of autosomal dominant retinitis pigmentosa (ADRP). To date, more than 200 mutations have been identified in RHO. The high allelic heterogeneity of RHO mutations suggests complicated pathogenic mechanisms. Here, we discuss representative RHO mutations as examples to briefly summarize the mechanisms underlying rhodopsin-related retinal dystrophy, which include but are not limited to endoplasmic reticulum stress and calcium ion dysregulation resulting from protein misfolding, mistrafficking, and malfunction. Based on recent advances in our understanding of disease mechanisms, various treatment methods, including adaptation, whole-eye electrical stimulation, and small molecular compounds, have been developed. Additionally, innovative therapeutic treatment strategies, such as antisense oligonucleotide therapy, gene therapy, optogenetic therapy, and stem cell therapy, have achieved promising outcomes in preclinical disease models of rhodopsin mutations. Successful translation of these treatment strategies may effectively ameliorate, prevent or rescue vision loss related to rhodopsin mutations.

Abstract Background Most of the studies assessing the corrective posterior total hip arthroplasty (THA) mainly focused on the mini-incision approach. Studies exploring the short external rotator sparing approach are rare. Therefore, this study aimed to compare the effectiveness of standard posterior approach and short external rotator sparing approach. Methods This prospective observational study included 126 patients who underwent THA in June 2017–June 2018. Patients were assigned to standard (standard posterior approach) and corrective (short external rotator sparing approach) groups based on the surgical method. Surgical data were recorded postoperatively. Postoperative hip joint recovery was assessed using the times to ambulation and independent stair use, and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score, Harris score, and Oxford hip score (OHS) at 2 and 8 postoperative weeks. The visual analog scale (VAS) was used for postoperative pain assessment. Results Postoperative changes of creatine kinase (CK), myoglobin, CRP, and prosthesis position were similar in both groups (P > 0.05). However, intraoperative blood loss (P < 0.001) and postoperative 6-h drainage volume (P = 0.03), hospital stay, blood transfusion rate, and times to ambulation and independent stair use were significantly reduced in the corrective group. Postoperatively, Oxford, and WOMAC scores significantly decreased in both groups. After surgery, the VAS score was more overtly decreased in the corrective group compared with the standard group. Conclusions This study concluded that the less invasive short external rotator sparing approach for THA caused less damage, reducing perioperative blood loss, shortening functional recovery time, maintaining prosthesis stability, and improving postoperative pain.

Niche theory is significant for understanding the function of community structure, interspecific relationships, and community dynamic succession. However, there are few studies on the soil animal niche in returning farmland to forest areas on the Loess Plateau, making it challenging to comprehend the utilization of soil animal resources, the stability of the local community, and the succession process in the areas. Therefore, this study collected soil animals in five typical vegetation types: Robinia pseudoacacia (R), Hippophae rhamnoides (H), Populus simonii (P), Pinus tabulaeformis (T), and Armeniaca sibirica x Hippophae rhamnoides (M), with abandoned grassland (G) used as a control group. Then, the number of soil animal taxa, individuals, diversity, and niche were sampled and examined in the study areas during the four seasons of spring, summer, autumn, and winter using the manual sorting method and the Tullgren method. The results revealed that 3872 soil animals from 3 Phyla, 8 Classes, 22 Orders, and 49 Families were captured in the study areas. The dominant groups of soil macrofauna were Diptera larvae, Julidae, and Formicidae, and the dominant groups of meso–micro soil fauna were Oribatida, Protospira, and Collembola juveniles. Soil animals have rich nutritional function groups, with the most saprophytic soil animal groups. The individual density and taxa number of soil animals in G were lower than other vegetation on the whole. H, M, and P had a higher Shannon–Winner index than the other vegetation. Seasonal changes had different effects on macro and meso–micro soil fauna. The diversity of soil macrofauna is higher in spring and summer, and that of meso–micro soil fauna is higher in autumn and winter. Oribatida, Diptera Larvae, and Formicidae had a large niche width in the main taxa of soil animals, with universal adaptability to the environment. Cicadellidae and Culicidae had narrow niche widths and were highly dependent on resources and the environment. There were 67 pairs of highly overlapped (Oik > 0.8) taxa of soil animals and 56 pairs of moderately overlapped (0.6 < Oik ≤ 0.8) taxa, accounting for 80.39% of the total number of taxa. Soil animals had high commonality in resource utilization, intense competition, and poor community stability. As a result, we can conclude that the soil animal community in the study areas was in the stage of succession.

Abstract Background The aim of this study was to determine the functional and radiological outcomes of arthroscopic treatment of anterior ankle impingement (AAI) in patients with chronic lateral ankle instability (CAI). Methods All patients with CAI between June 2012 and May 2015 were invited to participate in this investigation. All of them accepted open modified Broström repair of lateral ankle ligaments and were divided into two groups: AAI group (with anterior ankle impingement) and pure CAI group (without anterior ankle impingement). All of them were followed up using American Orthopaedic Foot and Ankle Society Score (AOFAS), Karlsson Ankle Functional Score and Tegner activity score. Ankle dorsiflexion was also examined. X-ray examination was applied to investigate anterior tibiotalar osteophytes. Results Finally, a total of 60 patients were followed up at a mean of 37 ± 10 months, including 22 patients in the AAI group and 38 patients in the pure CAI group. Preoperatively, the AAI group had significant lower AOFAS score (62.9 ± 11.7 vs 72.9 ± 11.1; p = 0.002) and Tegner activity score (1.5 ± 0.8 vs 2.1 ± 1.0; p = 0.04) respectively when compared with the pure CAI group. The ankle dorsiflexion of the AAI group (13 ± 2.1) was also significantly lower than that of the pure CAI group (26.2 ± 2.1) (p = 0.001). However, there was no significant difference in the AOFAS score or the Karlsson score or the Tegner score or the Ankle dorsiflexion between the two groups postoperatively. The postoperative X-ray images demonstrated complete osteophyte resection in all patients, and no recurrence of osteophyte. Conclusion The functional outcome scores and dorsiflexion had significantly improved postoperatively. Combined treatment of chronic ankle instability and anterior ankle impingement produced satisfactory surgical outcomes in patients with CAI accompanied by anterior ankle impingement symptom.
