Background: Idiopathic clubfoot is a common skeletal deformity in newborns that can cause functional limitations, pain, and reduced quality of life. The Ponseti method has become the gold standard for clubfoot treatment, replacing previous extensive surgical approaches. However, there is a lack of prospective long-term data comparing surgical and Ponseti treatments. Methods: This is a follow-up on a single-center, prospective, randomized clinical trial that started in 2001. The aim of this study was to compare the Ponseti method with surgical treatment. The current report presents the long-term results at adulthood of 12 clubfeet (9 subjects) in the Ponseti group and 9 clubfeet (5 subjects) in the surgical group. The evaluation included morphology, function, and quality of life, which were assessed through gait analysis, X-rays, and standardized questionnaires (FRS, ICFSG, AOFAS, SF-36). A Functional Rating System for clubfeet was defined as the primary outcome. Results: The Ponseti group had higher Functional Rating System scores, better ankle dorsiflexion mobility, and lower pain levels. Additionally, they reported better quality of life according to the Short Form 36 survey. However, neither group achieved radiographically normal foot morphology. Conclusions: The 18-year follow-up results indicate that the Ponseti method produces superior outcomes in terms of mobility, gait, function, and quality of life when compared to surgically treated feet, despite the presence of persistent morphological deviations.

One-third of Germany’s land surface area is covered by forest (around 11.4 million hectares), and thus, it characterizes the landscape. The forest is a habitat for a large number of animal and plant species, a source of raw materials, important for climate protection, and a well-being refuge for people, to name just a few of its many functions. During the annual forest condition surveys, the crown condition of German forests is assessed on the basis of field samples at fixed locations, as the crown condition of forest trees is considered an important indicator of their vitality. Since the start of the surveys in 1984, the mean crown defoliation of all tree species has increased, now averaging about 25% for all tree species. Additionally, it shows a strong rise in the rate of dieback. In 2019, the most significant changes were observed. Due to the drastic changes in recent years, efforts are being made to assess the situation of the forest using different remote sensing methods. There are now a number of freely available products provided to the public, and more will follow as a result of numerous projects in the context of earth-observation (EO)-based monitoring and mapping of the forests in Germany. In 2020, the situation regarding the use of remote sensing for the German forest was already investigated in more detail. However, these results no longer reflect the current situation. The changes of the last 3 years are the content of this publication. For this study, 84 citable research publications were thoroughly analyzed and compared with the situation in 2020. As a major result, we found a shift in the research focus towards disturbance monitoring and a tendency to cover larger areas, including national-scale studies. In addition to the review of the scientific literature, we also reviewed current research projects and related products. In congruence to the recent developments in terms of publications in scientific journals, these projects and products reflect the need for comprehensive, timely, large-area, and complementary EO-based information around forests expressed in multiple political programs. With this review, we provide an update of previous work and link it to current research activities. We conclude that there are still gaps between the information needs of forest managers who usually rely on information from field perspectives and the EO-based information products.

Background and purpose: Albeit pediatric flexible flat foot (FFF) is a common condition, only a minority of patients become symptomatic. Long-term outcomes of surgically treated pediatric patients with symptomatic FFF are largely unknown. In this systematic review, studies providing outcomes at a mean follow-up of at least 4 years after the procedure in these patients were analyzed. Material and methods: A PubMed search was undertaken involving original articles published up to July 2021 on outcome in children aged 6 to 14 with surgically treated FFF and mean (or minimum) follow-up of at least 4 years. Radiographic and clinical outcomes were analyzed. Results: Of initially 541 entries, 10 could be included in the systematic review (all level IV), involving 846 pediatric patients with 1,536 symptomatic FFF. Pooled mean radiological (n = 8) and clinical follow-up (n = 10) was 5.3 (range 0.5–15) and 7.0 (range 4.1–15) years, respectively. Surgical procedures included arthroereisis (n = 8), lateral column lengthening (n = 1), and Horseman procedure (n = 1). Overall relative frequency of implant-associated complications and wound-healing problems was 3.2% and 1.3%, as well as 2.8% and 1.6% following subtalar arthroereisis only. From preoperative to latest radiological assessment following subtalar arthroereisis (including 3 studies with radiological follow-up < 48 months), pooled median decrease in talonavicular coverage angle (TNCA; –9.2°), anteroposterior talocalcaneal angle (A-TCA; –6.5°), lateral talocalcaneal angle (L-TCA; –3.5°), talar declination angle (TDA; –14°), Moreau Costa Bertani angle (MCB; –13°), and talo-firstmetatarsal angle (L-T1MA; –10°) was observed, as was an increase in calcaneal pitch (4.5°). Interpretation: In symptomatic pediatric FFF patients, surgery is associated with a manageable complication profile, and results in satisfactory long-term clinical as well as radiological outcome. Yet scientific evidence is low, warranting larger scaled studies in the future.

On a daily basis, political decisions are made, often with their full extent of impact being unclear. Not seldom, the decisions and policy measures implemented result in direct or indirect unintended negative impacts, such as on the natural environment, which can vary in time, space, nature, and severity. To achieve a more sustainable world with equitable societies requires fundamental rethinking of our policymaking. It calls for informed decision making and a monitoring of political impact for which evidence-based knowledge is necessary. The most powerful tool to derive objective and systematic spatial information and, thus, add to transparent decisions is remote sensing (RS). This review analyses how spaceborne RS is used by the scientific community to provide evidence for the policymaking process. We reviewed 194 scientific publications from 2015 to 2020 and analysed them based on general insights (e.g., study area) and RS application-related information (e.g., RS data and products). Further, we classified the studies according to their degree of science–policy integration by determining their engagement with the political field and their potential contribution towards four stages of the policy cycle: problem identification/knowledge building, policy formulation, policy implementation, and policy monitoring and evaluation. Except for four studies, we found that studies had not directly involved or informed the policy field or policymaking process. Most studies contributed to the stage problem identification/knowledge building, followed by ex post policy impact assessment. To strengthen the use of RS for policy-relevant studies, the concept of the policy cycle is used to showcase opportunities of RS application for the policymaking process. Topics gaining importance and future requirements of RS at the science–policy interface are identified. If tackled, RS can be a powerful complement to provide policy-relevant evidence to shed light on the impact of political decisions and thus help promote sustainable development from the core.

Earth Observation satellite data allows for the monitoring of the surface of our planet at predefined intervals covering large areas. However, there is only one medium resolution sensor family in orbit that enables an observation time span of 40 and more years at a daily repeat interval. This is the AVHRR sensor family. If we want to investigate the long-term impacts of climate change on our environment, we can only do so based on data that remains available for several decades. If we then want to investigate processes with respect to climate change, we need very high temporal resolution enabling the generation of long-term time series and the derivation of related statistical parameters such as mean, variability, anomalies, and trends. The challenges to generating a well calibrated and harmonized 40-year-long time series based on AVHRR sensor data flown on 14 different platforms are enormous. However, only extremely thorough pre-processing and harmonization ensures that trends found in the data are real trends and not sensor-related (or other) artefacts. The generation of European-wide time series as a basis for the derivation of a multitude of parameters is therefore an extremely challenging task, the details of which are presented in this paper.

Background and purpose — Heavily displaced radial neck fractures in children are sometimes associated with poor outcome. A substantial number of these fractures require open reduction. We hypothesized that Judet type-IV fractures with a completely displaced radial head would result in a worse outcome than radial neck fractures with remaining bony contact. Patients and methods — We analyzed 19 children (median age 9.7 (4–13) years) who were treated for Judet type-IV radial neck fractures between 2001 and 2014. The outcome was assessed at the latest outpatient visit using the Linscheid-Wheeler score at a median time of 3.5 (1–8) years after injury. The patients were assigned either to group A (9 fractures with remaining bony contact between the radial head and the radial neck) or to group B (10 fractures without any bony contact). Results — The 2 groups were similar concerning age and sex. The rate of additional injuries was higher in group B (7/10 vs. 1/9 in group A; p = 0.009). The rate of open reduction was higher in group B (5/10 vs. 0/9 in group A; p = 0.01). Poor outcome was more common in group B (4/10 vs. 0/9 in group A; p = 0.03). In group B, the proportion of children with poor outcome (almost half) was the same irrespective of whether open or closed reduction had been done. Interpretation — The main causes of unfavorable results of radial neck fracture in children appear to be related to the energy of the injury and the amount of displacement—and not to whether open reduction was used.

Forests in Germany cover around 11.4 million hectares and, thus, a share of 32% of Germany’s surface area. Therefore, forests shape the character of the country’s cultural landscape. Germany’s forests fulfil a variety of functions for nature and society, and also play an important role in the context of climate levelling. Climate change, manifested via rising temperatures and current weather extremes, has a negative impact on the health and development of forests. Within the last five years, severe storms, extreme drought, and heat waves, and the subsequent mass reproduction of bark beetles have all seriously affected Germany’s forests. Facing the current dramatic extent of forest damage and the emerging long-term consequences, the effort to preserve forests in Germany, along with their diversity and productivity, is an indispensable task for the government. Several German ministries have and plan to initiate measures supporting forest health. Quantitative data is one means for sound decision-making to ensure the monitoring of the forest and to improve the monitoring of forest damage. In addition to existing forest monitoring systems, such as the federal forest inventory, the national crown condition survey, and the national forest soil inventory, systematic surveys of forest condition and vulnerability at the national scale can be expanded with the help of a satellite-based earth observation. In this review, we analysed and categorized all research studies published in the last 20 years that focus on the remote sensing of forests in Germany. For this study, 166 citation indexed research publications have been thoroughly analysed with respect to publication frequency, location of studies undertaken, spatial and temporal scale, coverage of the studies, satellite sensors employed, thematic foci of the studies, and overall outcomes, allowing us to identify major research and geoinformation product gaps.

Purpose: Presently, there is no publication combining clinical follow-up with magnetic resonance imaging (MRI) to determine possible post-traumatic alterations following paediatric intra-articular fractures of the medial malleolus. Therefore, the aims of this study were to retrospectively analyze a cohort of patients with Salter–Harris (SH) III and IV fractures of the medial malleolus and to evaluate their long-term outcome. MRI was used to assess possible changes of the articular surface that cannot be diagnosed on native radiographs. Patients and Methods: Fifty-four patients with SH III ( n = 38) or IV fractures ( n = 17) of the medial malleolus treated between 2001 and 2011 were invited for a follow-up examination. Clinical outcome was assessed with the Weber score and osteoarthritis with the Kellgren and Lawrence classification. MRI was rated by the Outerbridge classification. Correlations between the clinical and radiological outcomes were calculated. Results: Seventeen patients were recruited for long-term follow-up at a mean of 112 (range, 65–184) months. The Weber score was very good for 5 patients, good for 10 patients and poor for 2 patients; the Kellgren and Lawrence score revealed a favourable grade 0 in 15 patients and grade 1 in 2 patients. The MRI-based Outerbridge classification yielded grade 0 for 12 patients, grade 1 for 1 patient, grade 2 for 2 patients and grade 3 for 1 patient. The Outerbridge score significantly ( p < 0.05) correlated with the Weber score. Conclusion: This study shows excellent and good outcome of SH III and IV fractures of the medial malleolus. Worse clinical outcome correlated with post-traumatic changes of the articular cartilage seen on MRI.

The incidence of osteomyelitis constantly declines. While the disease most commonly affects the long bones, involvement of the patella is rarely seen. Due to this rarity and the variable clinical presentation, diagnosis is often delayed. The present case report describes a 10-year-old female patient with a delayed diagnosis of patella osteomyelitis. The diagnostic procedures and the treatment regimen are described. Additionally, a detailed literature review of the available publications reporting osteomyelitis of the patella in children is presented.
