Abstract Background The societal value of cats, dogs and horses is high, and the companion and sport animal health care sector is growing. Clinical research concerning cats, dogs and horses is crucial for the development of evidence-based medical care that benefits animals and their owners, and has implications for human and environmental health from a One Health perspective. Basic information on companion animal and equine research enables more directed measures to improve conditions for research within the area. The aim of the present study was to describe Nordic companion animal and equine clinical research from 2010 to 2019, including bibliometrics, human resources and funding. Results There were 2 042 published research publications originating from Nordic countries on cats (n = 282), dogs (n = 1 086), and horses (n = 781) from 2010 to 2019. The majority (83%) of the publications came from the four Nordic universities with veterinary programs. Seven percent of the publications were collaborations between two or more Nordic universities. Approximately 18% of the PhD theses (178 out of 970) from veterinary faculties or corresponding units concerned these species, most of them dogs (n = 86), followed by horses (n = 64), cats (n = 15) or a combination of these species (n = 13). The scientific areas cardiology, infectious diseases, reproduction, and surgery were prominent for all three species. A large proportion of grants were received from small- to medium-sized funding bodies, mainly funding running costs and only to a limited degree salaries. During 2010–2019, costs for veterinary and other services for cats and dogs steadily increased. The growth of the veterinary healthcare sector was not reflected in an increasing number of clinical research publications, for which no increase was seen after 2014. Conclusions Despite a high societal value of the species, veterinary clinical research on sports and companion animals has not increased, in contrast to the veterinary healthcare sector. Activities stimulating the research area, e.g. funding bodies enabling coverage of salaries, are needed. The development of Nordic veterinary clinical care may benefit from strengthened research cooperation between countries.

Abstract Background In COPD, the balance between matrix metalloproteinases (MMPs) and their natural inhibitors [tissue inhibitors of metalloproteinases (TIMPs)] is shifted towards excessive degradation, reflected in bronchoalveolar lavage (BAL) as increased MMP concentrations. Because of their critical role in lung homeostasis, MMP activity is tightly regulated, but to what extent this regulation occurs through epigenetic mechanisms remains unknown. Methods To explore the interplay between MMPs, TIMPs, and DNA methylation (DNAm) we (1) analysed MMP-9, -12, and TIMP-1 concentrations in BAL fluid, and profiled DNAm in BAL cells from 18 COPD and 30 control subjects, (2) estimated protein–COPD relationships using multivariable regression, (3) identified protein quantitative trait methylation loci (pQTMs) with COPD as a potential modifier in a separate interaction model, and (4) integrated significant interactions with a previous COPD GWAS meta-analysis. Results COPD was associated with higher levels of BAL MMP-12 (p = 0.016) but not with MMP-9 or TIMP-1. Further examination of MMP-12 identified association with DNAm at 34 loci (pQTMs), with TGFBR2 (p = 2.25 × 10–10) and THBS4 (p = 1.11 × 10–9) among the top ten pQTM genes. The interaction model identified 66 sites where the DNAm–MMP-12 association was significantly different in COPD compared to controls. Of these, one was colocalized with SNPs previously associated with COPD. Conclusions Our findings indicate that airway MMP-12 may partially be regulated by epigenetic mechanisms and that this regulation is disrupted in COPD. Furthermore, integration with COPD GWAS data suggests that this dysregulation is influenced by a combination of environmental factors, disease processes, and genetics, with the latter potentially playing a lesser role.

Abstract Background Depression is a common and serious problem in older adults, but few have access to psychological treatments. Internet-delivered Cognitive Behavioral Therapy (ICBT) has the potential to improve access and has been found to be effective in adults with depression. The aim of this study was to examine the effects of tailored ICBT for depression in older adults aged 65 years or older. We also investigated if cognitive flexibility could predict outcome. Methods Following online recruitment from the community, included participants were randomly allocated to either ten weeks of clinician guided ICBT (n = 50) or to an active control group in the form of non-directive support (n = 51). Primary depression outcome was the Geriatric Depression Scale (GDS-15). Several secondary outcomes were used, such as the Beck Depression Inventory (BDI-II) and the Patient Health Questionnaire (PHQ-9). Results Both treatment and active control groups significantly reduced their levels of depression, and the treatment group showed significantly greater improvement on the GDS-15 and BDI-II, but not on the PHQ-9. Between-group effect sizes as Cohen’s d were 0.78 (CI95% 0.36–1.20) on the GDS-15 and 0.53 (CI95% 0.11–0.94) on the BDI-II. Conclusions Tailored ICBT is superior to an active control for older adults with depression. Between-group effects were smaller than in previous RCTs, most likely because of the use of an active control condition. Cognitive flexibility did not predict outcome. We conclude that ICBT can be used for older adults with depression, and thus increase access to psychotherapy for this group. Abstract Information on the effects of changing agricultural management on crop and livestock performance is critical for developing evidence-based policies, investments, and programs. Evidence for Resilient Agriculture (ERA) v1.0.1 presents a dataset that harmonizes and aggregates 112,859 observations from 2,011 agricultural studies taken place in Africa between 1934 and 2018. The dataset includes information on the effect of 364 combinations of management practices and technologies on 87 environmental, social, and economic indicators of outcomes. Observations are geolocated and temporally tagged and thus can be linked to other datasets such as historical weather, soil properties, and road networks. ERA offers a new resource for understanding the impacts of changing farming practices under diverse environmental contexts, providing data to support strategic interventions aimed to enhance productivity, resilience, and sustainability of African agriculture.

Abstract Canine reproduction differs from that of many other domestic animals, and increased knowledge on biochemical changes during canine pregnancy is important for investigations of infertility or subfertility. The total glycosylation pattern, i.e., the glycome, of body fluids reflects cellular status in health and disease. The aim of the present pilot study was to investigate pregnancy-related changes of the serum N-glycome in bitches. A method based on Rapifluor HILIC-UPLC-FLR-MS was optimized and applied for analysis and quantification of N-glycans in canine serum. Serum samples from six pregnant and five non-pregnant bitches, collected at four well-defined time points, were included. The levels of sialylated and galactosylated complex glycans were significantly elevated in serum from pregnant bitches, consistent with previous reports on human pregnancy. The levels of fucosylated and agalactosylated glycans decreased significantly in pregnant dogs. In non-pregnant dogs, the glycosylation pattern did not change during the cycle. Pregnancy is an inflammatory state, but our findings during canine pregnancy are quite the opposite to changes that have previously been described for dogs with a known parasitic infection. Evaluation of the canine glycome may thus be valuable in studies of canine pregnancy, possibly differing inflammatory changes related to pregnancy to those caused by an infection.

Abstract Hair cortisol concentrations (HCC) are measured to assess long-term HPA-axis activity and may represent a valuable non-invasive tool to evaluate chronic stress in cats. This study investigated combing as a novel, low-stress method for HCC assessment, as well as possible associations between HCC and cat characteristics in 167 owned cats. Hair was sampled at veterinary clinics through clipping and/or combing the cat, or at home by the owner combing the cat. A questionnaire was sent to cat owners, including inquiries about the cat’s sex, health status, and exposure to stress. HCC was quantified using a commercial cortisol assay kit. Despite variations within and between sampling methods, Spearman’s correlation and Bland–Altman plots revealed a moderate correlation between clipped and combed samples (rs = 0.61, LOA -5.51 ± 22.54). In multiple linear regression, variations in HCC were observed based on sex, health status and cat group size. No associations were found between HCC and stress as assessed by owners. Despite study limitations and remaining uncertainty regarding factors influencing HCC, combing presents a convenient approach for evaluating long-term HPA-axis activity in clinical settings. The association between health and HCC suggests alterations in cortisol levels that are related to disease processes and stress-inducing events associated with the disease.

Abstract Background Occurrence of low blood taurine concentrations (B‐TauC) and predisposing factors to taurine deficiency in English Cocker Spaniels (ECS) are incompletely understood. Objectives Investigate the occurrence of low B‐TauC in a Swedish population of ECS and evaluate the association between B‐TauC and dog characteristics, clinical variables, and diet composition. Animals One‐hundred eighty privately owned ECS. Methods Dogs were prospectively recruited and underwent physical examination, blood analyses, and echocardiographic and ophthalmic examinations. Dogs with clinical signs of congestive heart failure (CHF) also underwent thoracic radiography. Taurine concentrations were analyzed in plasma (EDTA and heparin) and whole blood. Diets consumed by the dogs at the time of the examination were analyzed for dietary taurine‐ (D‐TauC), cysteine‐ (D‐CysC), and methionine concentrations (D‐MetC). Results Fifty‐three of 180 dogs (29%) had low B‐TauC, of which 13 (25%) dogs had clinical and radiographic signs of CHF, increased echocardiographic left ventricular (LV) dimensions and volumes, and impaired LV systolic function. IntroductionType 1 diabetes involves immune-mediated destruction of insulin-producing beta cells, with eosinophils potentially playing a significant role. Recent studies suggest that leukotriene inhibition might influence this process. This case report presents a novel observation of montelukast, a leukotriene receptor antagonist, reducing insulin requirements in a patient with Latent Autoimmune Diabetes in Adults (LADA). A 55-year-old male with LADA experienced substantial reductions in insulin dosage when treated with montelukast for respiratory symptoms. Initially diagnosed with LADA in 2018, the patient had been on insulin therapy. Montelukast therapy, initiated due to respiratory symptoms, led to a 60.5% reduction in insulin requirements which increased upon discontinuation. A subsequent montelukast course resulted in an 87.9% insulin reduction. Although the insulin-lowering effect diminished with continued montelukast use, the patient reported reduced postprandial hyperglycemia. Blood tests indicated stable glucose levels despite reduced insulin doses.ConclusionsThis case suggests that montelukast may reduce insulin needs in type 1 diabetes patients, potentially through its anti-inflammatory effects on eosinophils. These findings highlight the need for further research into montelukast’s role in type 1 diabetes management and its potential to preserve beta-cell function or reduce insulin dependence.

Abstract Positron emission tomography (PET) imaging of tau aggregation in Alzheimer’s disease (AD) is helping to map and quantify the in vivo progression of AD pathology. To date, no high-affinity tau-PET radiopharmaceutical has been optimized for imaging non-AD tauopathies. Here we show the properties of analogues of a first-in-class 4R-tau lead, [18F]OXD-2115, using ligand-based design. Over 150 analogues of OXD-2115 were synthesized and screened in post-mortem brain tissue for tau affinity against [3H]OXD-2115, and in silico models were used to predict brain uptake. [18F]OXD-2314 was identified as a selective, high-affinity non-AD tau PET radiotracer with favorable brain uptake, dosimetry, and radiometabolite profiles in rats and non-human primate and is being translated for first-in-human PET studies.

We investigated long-term changes using a harmonised 22-year data set of aerosol light absorption measurements, in conjunction with air mass history and aerosol source analysis. The measurements were performed at Zeppelin Observatory, Svalbard, from 2002 to 2023. We report a statistically significant decreasing long-term trend for the light absorption coefficient. However, the last 8 years of 2016–2023 showed a slight increase in the magnitude of the light absorption coefficient for the Arctic haze season. In addition, we observed an increasing trend in the single-scattering albedo from 2002 to 2023. Five distinct source regions, representing different transport pathways, were identified. The trends involving air masses from the five regions showed decreasing absorption coefficients, except for the air masses from Eurasia. We show that the changes in the occurrences of each transport pathway cannot explain the reductions in the absorption coefficient observed at the Zeppelin station. An increase in contributions of air masses from more marine regions, with lower absorption coefficients, is compensated for by an influence from high-emission regions. The proportion of air masses en route to Zeppelin, which have been influenced by active fires, has undergone a noticeable increase starting in 2015. However, this increase has not impacted the long-term trends in the concentration of light-absorbing aerosol. Along with aerosol optical properties, we also show an increasing trend in accumulated surface precipitation experienced by air masses en route to the Zeppelin Observatory. We argue that the increase in precipitation, as experienced by air masses arriving at the station, can explain a quarter of the long-term reduction in the light absorption coefficient. We emphasise that meteorological conditions en route to the Zeppelin Observatory are critical for understanding the observed trends.

Abstract Targeted protein degradation systems developed for eukaryotes employ cytoplasmic machineries to perform proteolysis. This has prevented mitochondria-specific analysis of proteins that localize to multiple locations, for example, the mitochondria and the nucleus. Here, we present an inducible mitochondria-specific protein degradation system in Saccharomyces cerevisiae based on the Mesoplasma florum Lon (mf-Lon) protease and its corresponding ssrA tag (called PDT). We show that mitochondrially targeted mf-Lon protease efficiently and selectively degrades a PDT-tagged reporter protein localized to the mitochondrial matrix. The degradation can be induced by depleting adenine from the medium, and tuned by altering the promoter strength of the MF-LON gene. We furthermore demonstrate that mf-Lon specifically degrades endogenous, PDT-tagged mitochondrial proteins. Finally, we show that mf-Lon-dependent PDT degradation can also be achieved in human mitochondria. In summary, this system provides an efficient tool to selectively analyze the mitochondrial function of dually localized proteins.

Abstract Efficient teamwork is crucial to provide optimal health care. This paper focuses on teamwork between Anaesthesiologists (ANES), Certified Registered Nurse Anaesthetists’ (CRNA) and Critical Care Registered Nurses (CCRN) working in challenging environments such as the intensive care unit (ICU) and the operating room (OR). Conflicts are common between physicians and nurses, negatively impacting teamwork. Social hierarchies based on professional status and power inequalities between nurses and physicians plays a vital role in influencing teamwork. Foucault was a famous thinker especially known for his reasoning regarding power/knowledge. A Foucauldian perspective was therefore incorporated into this paper and the overall aim was to explore CCRN/CRNA perception of knowledge/power in teamwork with ANES. Methods A mixed-method approach was applied in this study. Data was collected using a web-based questionnaire containing both closed-end and open-ended questions. A total of 289 CCRNs and CRNAs completed the questionnaire. Data analysis was then conducted through five stages as outlined by Onwuebugzie and Teddlie; analysing quantitative data in SPSS 27.0 and qualitative data with a directed content analysis, finally merging data together in ATLAS.ti v.23. Results The result reveals a dissonance between quantitative and qualitative data; quantitative data indicates a well-functioning interdisciplinary teamwork between CCRN/CRNA and ANES - qualitative data highlights that there are several barriers and inequalities between the two groups. Medicine was perceived as superior to nursing, which was reinforced by both social and organisational structures at the ICU and OR. Conclusion Unconscious rules underlying current power structures in the ICU and OR works in favour of the ANES and biomedical paradigm, supporting medical knowledge. To achieve a more equal power distribution between CCRN/CRNAs and ANES, the structural hierarchies between nursing and medicine needs to be addressed. A more equal power balance between the two disciplines can improve teamwork and thereby reduce patient mortality and improve patient outcomes.

Abstract Knowledge of factors associated with semen quality may help in investigations of the aetiology and pathophysiology. We investigated the correlation between biomarkers for testicular cell function (anti-müllerian hormone, AMH, Inhibin B, testosterone, free androgen-index (testosterone/sex-hormone binding globulin), insulin like peptide 3, INSL-3), alkaline phosphate (ALP), canine prostate-specific esterase (CPSE), and heterophilic antibodies with dog variables, semen quality, and fertility. Blood and semen were collected from 65 Bernese Mountain Dogs. We evaluated total sperm count, motility and morphological parameters. The semen quality ranged from poor to excellent, with an average total sperm count of 1.1 × 109 and 50% morphologically normal spermatozoa (MNS). Age and abnormal testicular consistency correlated with decreased motility and MNS. Higher ALP correlated with higher total sperm count. AMH could not be detected in seminal plasma. AMH in blood correlated with head defects and high AMH concentration correlated with a severe decline in several semen parameters. Testosterone was negatively and CPSE positively correlated with age. No correlations were found for INSL-3, inhibin B, or heterophilic antibodies. Our findings contribute to the understanding of factors associated with semen quality in dogs, particularly related to Sertoli cell function.

Decisions to admit or refuse admission to intensive care for acute exacerbations of COPD (AECOPD) can be difficult, due to an uncertainty about prognosis. Few studies have evaluated outcomes after intensive care for AECOPD in patients with chronic respiratory failure requiring long-term oxygen therapy (LTOT). In this nationwide observational cohort study, we investigated survival after first-time admission for AECOPD in all patients aged ≥40 years admitted to Swedish intensive care units between January 2008 and December 2015, comparing patients with and without LTOT. Among the 4,648 patients enrolled in the study, 450 were on LTOT prior to inclusion. Respiratory support data was available for 2,631 patients; 73% of these were treated with noninvasive ventilation (NIV) only, 17% were treated with immediate invasive ventilation, and 10% were intubated after failed attempt with NIV. In this paper we investigate how verum is realized in Xhosa and Zulu, two Southern Bantu languages belonging to the Nguni group. The data for our study were collected through interviews with native speakers who were prompted to produce sentences in discourse contexts that typically license utterances with verum. We found that the main grammatical strategy for the expression of verum in Xhosa and Zulu involves the removal of phrasal constituents from the focus domain (the VP). This leaves the verb as the sole remaining focus host, and allows auxiliary features of the verb, such as polarity, to be marked as focus. Consequently, we analyse verum in Xhosa and Zulu as polarity focus, which is expressed indirectly, via the backgrounding of potentially focusable phrasal material. We also examined the prosodic properties of verum utterances in Xhosa. Based on findings from previous studies on Nguni intonation, we expected to observe lengthening of the penultimate vowels of phrase-final verbs and utterance-final words in our data. However, contrary to expectation, we did not find evidence of penultimate vowel lengthening in Xhosa sentences with verum, a (preliminary) result which suggests that the expression of verum may have an effect on prosody in Nguni languages.

Abstract Background Subject-level real-world data (RWD) collected during daily healthcare practices are increasingly used in medical research to assess questions that cannot be addressed in the context of a randomized controlled trial (RCT). A novel application of RWD arises from the need to create external control arms (ECAs) for single-arm RCTs. In the analysis of ECAs against RCT data, there is an evident need to manage and analyze RCT data and RWD in the same technical environment. In the Nordic countries, legal requirements may require that the original subject-level data be anonymized, i.e., modified so that the risk to identify any individual is minimal. The aim of this study was to conduct initial exploration on how well pseudonymized and anonymized RWD perform in the creation of an ECA for an RCT. Methods This was a hybrid observational cohort study using clinical data from the control arm of the completed randomized phase II clinical trial (PACIFIC-AF) and RWD cohort from Finnish healthcare data sources. The initial pseudonymized RWD were anonymized within the (k, ε)-anonymity framework (a model for protecting individuals against identification). Propensity score matching and weighting methods were applied to the anonymized and pseudonymized RWD, to balance potential confounders against the RCT data. Descriptive statistics for the potential confounders and overall survival analyses were conducted prior to and after matching and weighting, using both the pseudonymized and anonymized RWD sets. Results Anonymization affected the baseline characteristics of potential confounders only marginally. The greatest difference was in the prevalence of chronic obstructive pulmonary disease (4.6% vs. 5.4% in the pseudonymized compared to the anonymized data, respectively). Moreover, the overall survival changed in anonymization by only 8% (95% CI 4–22%). Both the pseudonymized and anonymized RWD were able to produce matched ECAs for the RCT data. Anonymization after matching impacted overall survival analysis by 22% (95% CI -21–87%). Conclusions Anonymization may be a viable technique for cases where flexible data transfer and sharing are required. As anonymization necessarily affects some aspects of the original data, further research and careful consideration of anonymization strategies are needed.

Abstract Testosterone is thought to be incorporated in growing hair strands so that specific hair segments reflect average free hormone concentrations from the corresponding time period. However, the exact mechanisms of hormone integration in scalp hair have not yet been established and it is not known how testosterone is stored in the hair segments over time. The aim of this study was to investigate the stability of testosterone concentrations in hair as it grows and to determine if segmental hair analysis can be used as a retrospective testosterone diary. Thirty men and 40 women provided two hair samples and 16 saliva samples during a period of three months. Hair growth between the two samplings was measured. Hair samples were cut into 10 mm segments resulting in three segments from the first sampling and six segments from the second sampling. Hair samples were pulverised and extracted with methanol. Hair testosterone concentrations were analysed using an in-house radioimmunoassay. Salivary testosterone was analysed using a commercial enzyme-linked immunosorbent assay (Demeditec). Introduction Efficacy trials show that evidence-based injury prevention training reduces injuries in youth athletes but effectiveness, that is, outside the controlled setting, is lower and, consequently this training has had limited public health impact. Insufficient involvement of end-users at the individual and organisational levels is identified as a main barrier to successful implementation. The ‘Implementing injury Prevention training ROutines in TEams and Clubs in youth Team handball (I-PROTECT)’ uses an ecological participatory design incorporating the perspectives of multiple stakeholders throughout the project. Within the I-PROTECT research project, the specific aim of this study is to investigate the implementation of the end-user-targeted I-PROTECT programme.Methods and analysis This pragmatic two-armed cluster randomised controlled trial is conducted collaboratively with the Swedish Handball Federation that has overall responsibility for handball in Sweden. Randomly selected clubs in Sweden offering handball for both female and male youth players are invited to participate. 18 clubs are randomised (stratified by club size) to intervention (I-PROTECT plus tailored implementation support) or control (injury prevention programme currently available through the Swedish Handball Federation). The anticipated total number of potential participants (players, coaches, club administrators, parents/guardians) is ~3500. The I-PROTECT programme includes end-user-targeted information and physical and psychological injury prevention training available in a specifically developed interactive mobile application. Implementation strategies were selected from the Expert Recommendations for Implementing Change discrete implementation strategy compilation, based on feedback from end-users. Implementation outcomes will be investigated at the end of the handball season using the Reach, Effectiveness, Adoption, Implementation and Maintenance implementation evaluation framework. A study-specific questionnaire, app downloads and/or workshops will be used to collect data.Ethics and dissemination The Swedish Ethical Review Authority approved the study. Results will be disseminated in peer-reviewed scientific journals, as popular science articles, at international conferences and communicated via the Swedish Handball Federation.Trial registration number NCT05696119.

Recent advancements in amorphous materials have opened new avenues for exploring unusual magnetic phenomena at the sub-nanometer scale. We investigate the phenomenon of low-temperature “magnetic hardening” in heterogeneous amorphous Fe–Ni–B–Nb thin films, revealing a complex interplay between microstructure and magnetism. Magnetization hysteresis measurements at cryogenic temperatures show a significant increase in coercivity (HC) below 25 K, challenging the conventional Random Anisotropy Model (RAM) in predicting magnetic responses at cryogenic temperatures. Heterogeneous films demonstrate a distinct behavior in field-cooled and zero-field-cooled temperature-dependent magnetizations at low temperatures, characterized by strong irreversibility. This suggests spin-glass-like features at low temperatures, which are attributed to exchange frustration in disordered interfacial regions. These regions hinder direct exchange coupling between magnetic entities, leading to magnetic hardening. This study enhances the understanding of how microstructural intricacies impact magnetic dynamics in heterogeneous amorphous thin films at cryogenic temperatures.

The absorption of shortwave irradiance in snow depends on the physical properties of snow (e.g., snow grain size and shape, liquid water content, etc.) and light-absorbing particles (LAP). Originating from natural and anthropogenic sources, LAP has been reported to accelerate snowmelt significantly in different regions globally. Yet, our process-level understanding of LAP after deposition onto snow remains rather limited. Here we investigate the impacts of artificial deposition of different LAP onto snow surfaces in an outdoor environment of northern Finland. Following LAP dry deposition into a custom-made tent standing on top of the snowpack, the albedo was followed along with the properties of snow in snow pit measurements throughout the spring season. The results showed that the albedo decay at the end of the season for the different spots were linked to the initial amount and type of LAP that were deposited onto the snowpack. Measured snow temperature profiles from LAP doped snow versus natural reference snow illustrated that the LAP affected snow had higher temperatures in the subsurface snow layers. Collected snow samples analyzed for size distribution of soot particles revealed no apparent agglomeration of soot particles during thaw-freezing events taking place during the experiment. Despite the relatively large perturbation of the experimentally deposited LAP, their impact on the season length was only up to 3 days. Additional experiments are, nevertheless, needed to better constrain the effects of LAP on snow albedo, melt rate, and other associated processes.

We introduce the concept photophysical image analysis (PIA) and an associated pipeline for unsupervised probabilistic image thresholding for images recorded by electron-multiplying charge-coupled device (EMCCD) cameras. We base our approach on a closed-form analytic expression for the characteristic function (Fourier-transform of the probability mass function) for the image counts recorded in an EMCCD camera, which takes into account both stochasticity in the arrival of photons at the imaging camera and subsequent noise induced by the detection system of the camera. The only assumption in our method is that the background photon arrival to the imaging system is described by a stationary Poisson process (we make no assumption about the photon statistics for the signal). We estimate the background photon statistics parameter, λbg, from an image which contains both background and signal pixels by use of a novel truncated fit procedure with an automatically determined image count threshold. Prior to this, the camera noise model parameters are estimated using a calibration step. Utilizing the estimates for the camera parameters and λbg, we then introduce a probabilistic thresholding method, where, for the first time, the fraction of misclassified pixels can be determined a priori for a general image in an unsupervised way. We use synthetic images to validate our a priori estimates and to benchmark against the Otsu method, which is a popular unsupervised non-probabilistic image thresholding method (no a priori estimates for the error rates are provided). For completeness, we lastly present a simple heuristic general-purpose segmentation method based on the thresholding results, which we apply to segmentation of synthetic images and experimental images of fluorescent beads and lung cell nuclei. Our publicly available software opens up for fully automated, unsupervised, probabilistic photophysical image analysis.

AbstractBackground Phantoms accurately mimicking the electromagnetic and thermal properties of human tissues are essential for the development, characterization, and quality assurance (QA) of clinically used equipment for Hyperthermia Treatment (HT). Currently, a viable recipe for a fat equivalent phantom is not available, mainly due to challenges in the fabrication process and fast deterioration.Materials and methods We propose to employ a glycerol-in-oil emulsion stabilized with ethylcellulose to develop a fat-mimicking material. The dielectric, rheological, and thermal properties of the phantom have been assessed by state-of-the-art measurement techniques. The full-size phantom was then verified in compliance with QA guidelines for superficial HT, both numerically and experimentally, considering the properties variability.Results Dielectric and thermal properties were proven equivalent to fat tissue, with an acceptable variability, in the 8 MHz to 1 GHz range. The rheology measurements highlighted enhanced mechanical stability over a large temperature range. Both numerical and experimental evaluations proved the suitability of the phantom for QA procedures. The impact of the dielectric property variations on the temperature distribution has been numerically proven to be limited (around 5%), even if higher for capacitive devices (up to 20%).Conclusions The proposed fat-mimicking phantom is a good candidate for hyperthermia technology assessment processes, adequately representing both dielectric and thermal properties of the human fat tissue while maintaining structural stability even at elevated temperatures. However, further experimental investigations on capacitive heating devices are necessary to better assess the impact of the low electrical conductivity values on the thermal distribution.

The Arctic is a rapidly changing ecosystem, with complex ice–ocean–atmosphere feedbacks. An important process is new particle formation (NPF), from gas-phase precursors, which provides a climate forcing effect. NPF has been studied comprehensively at different sites in the Arctic, ranging from those in the High Arctic and those at Svalbard to those in the continental Arctic, but no harmonised analysis has been performed on all sites simultaneously, with no calculations of key NPF parameters available for some sites. Here, we analyse the formation and growth of new particles from six long-term ground-based stations in the Arctic (Alert, Villum, Tiksi, Zeppelin Mountain, Gruvebadet, and Utqiaġvik). Our analysis of particle formation and growth rates in addition to back-trajectory analysis shows a summertime maxima in the frequency of NPF and particle formation rate at all sites, although the mean frequency and particle formation rates themselves vary greatly between sites, with the highest at Svalbard and lowest in the High Arctic. The summertime growth rate, condensational sinks, and vapour source rates show a slight bias towards the southernmost sites, with vapour source rates varying by around an order of magnitude between the northernmost and southernmost sites. Air masses back-trajectories during NPF at these northernmost sites are associated with large areas of sea ice and snow, whereas events at Svalbard are associated with more sea ice and ocean regions. Events at the southernmost sites are associated with large areas of land and sea ice. These results emphasise how understanding the geographical variation in surface type across the Arctic is key to understanding secondary aerosol sources and providing a harmonised analysis of NPF across the Arctic.

Abstract Background Remission is documented in a substantial proportion of cats with diabetes. The effects of diabetes mellitus (DM) on the lives of cats and their owners should be considered when evaluating treatment success. Objectives To study outcome in cats with DM and the impact DM has on the life situation of cat and owner. Animals Domestic and pedigree cats with a diagnosis of DM (n = 477) insured by a Swedish insurance company during 2009 to 2013. Methods Retrospective cross‐sectional study. A questionnaire was sent to 1369 owners of cats diagnosed with DM. The questions concerned the cat, treatment, owner perceptions of the disease and treatment and disease outcome. Data were analyzed using multiple linear and logistic regression, with outcomes set as survival for more than 4 weeks after diagnosis, survival time, achieving remission, remission without relapse and quality of life (QoL) for the cat. Results The response rate was 35%, leaving 477 questionnaires for analysis. The remission rate among treated cats was 29% (118/405). Feeding a commercially available wet diet was associated with both remission (OR 3.16, 95% confidence interval 1.27‐8.12) and remission without relapse (OR 14.8, 95% confidence interval 2.25‐153.8). Remission was associated with a better QoL for the cat. Conclusions and Clinical Importance The association between feeding a commercially available wet diet and remission is important and strengthens the role of diet in treatment of DM in cats. Linking remission and a better QoL for the cat emphasizes remission as a goal in disease management.

BackgroundProximal humerus fractures (PHFs) are common fractures, especially in older female patients. These fractures are commonly treated surgically, but the consensus on the best treatment is still lacking.Methods and findingsThe primary aim of this multicenter, randomized 3-arm superiority, open-label trial was to assess the results of nonoperative treatment and operative treatment either with locking plate (LP) or hemiarthroplasty (HA) of 3- and 4-part PHF with the primary outcome of Disabilities of the Arm, Shoulder, and Hand (DASH) at 2-year follow-up. Between February 2011 and December 2019, 160 patients 60 years and older with 3- and 4-part PHFs were randomly assigned in 1:1:1 fashion in block size of 10 to undergo nonoperative treatment (control) or operative intervention with LP or HA. In total, 54 patients were assigned to the nonoperative group, 52 to the LP group, and 54 to the HA group. Five patients assigned to the LP group were reassigned to the HA group perioperatively due to high comminution, and all of these patients had 4-part fractures. In the intention-to-treat analysis, there were 42 patients in the nonoperative group, 44 in the LP group, and 37 in the HA group. The outcome assessors were blinded to the study group. The mean DASH score at 2-year follow-up was 30.4 (standard error (SE) 3.25), 31.4 (SE 3.11), and 26.6 (SE 3.23) points for the nonoperative, LP, and HA groups, respectively. At 2 years, the between-group differences were 1.07 points (95% CI [-9.5,11.7]; p = 0.97) between nonoperative and LP, 3.78 points (95% CI [-7.0,14.6]; p = 0.69) between nonoperative and HA, and 4.84 points (95% CI [-5.7,15.4]; p = 0.53) between LP and HA. No significant differences in primary or secondary outcomes were seen in stratified age groups (60 to 70 years and 71 years and over). At 2 years, we found 30 complications (3/52, 5.8% in nonoperative; 22/49, 45% in LP; and 5/49, 10% in HA group, p = 0.0004) and 16 severe pain-related adverse events. There was a revision rate of 22% in the LP group. The limitation of the trial was that the recruitment period was longer than expected due to a high number of exclusions after the assessment of eligibility and a larger exclusion rate than anticipated toward the end of the trial. Therefore, the trial was ended prematurely.ConclusionsIn this study, no benefit was observed between operative treatment with LP or HA and nonoperative treatment in displaced 3- and 4-part PHFs in patients aged 60 years and older. Further, we observed a high rate of complications related to operative treatments.Trial registrationClinicalTrials.gov NCT01246167.

Introduction Atrial fibrillation (AF) is a major risk factor for ischaemic stroke and transient ischaemic attack (TIA), and AF detection can be challenged by asymptomatic and paroxysmal presentation. Long-term ECG monitoring after ischaemic stroke or TIA is recommended by all major societies in cardiology and cerebrovascular medicine as a secondary prophylactic measure. However, data on stroke reduction are lacking, and the recommendations show significant diversity.Methods and analysis AF SPICE is a multicentre, national, investigator-initiated, randomised, parallel-group, register-based trial comparing extended ECG monitoring versus standard ECG monitoring in patients admitted with ischaemic stroke or TIA, with a composite endpoint of stroke, all-cause-mortality and intracerebral bleeding. Patients aged ≥70 years without previous AF will be randomised 1:1 to control (standard ECG monitoring) or intervention (extended ECG monitoring). In the control arm, patients will undergo 48±24 hours (ie, a range of 24–72 hours) of continuous ECG monitoring according to national recommendations. In the intervention arm, patients will undergo 14+14 days of continuous ECG monitoring 3 months apart using an ECG patch device, which will provide an easy-accessed, well-tolerated 14-day continuous ECG recording. All ECG patch recordings will be read in a core facility. In cases of AF detection, oral anticoagulation will be recommended if not contraindicated. A pilot phase has been concluded in 2022, which will transcend into the main trial during 2023–2026, including approximately 30 stroke units. The sample size was calculated to be 3262 patients. The primary outcome will be collected from register data during a 36-month follow-up.Ethics and dissemination Ethical approval has been provided by the Swedish Ethical Review Authority, reference 2021–02770. The trial will be conducted according to the ethical principles of the Declaration of Helsinki and national regulatory standards. Positive results from the study have the potential for rapid dissemination in clinical practice.Trial registration number NCT05134454.

Microscopic colitis (MC) is classified as collagenous colitis (CC) and lymphocytic colitis (LC). Genetic associations between CC and human leucocyte antigens (HLAs) have been found, with smoking being a predisposing external factor. Smoking has a great impact on metabolomics. The aim of this explorative study was to analyze global metabolomics in MC and to examine whether the metabolomic profile differed regarding the type and course of MC, the presence of IBS-like symptoms, treatment, and smoking habits. Of the 240 identified women with MC aged ≤73 years, 131 completed the study questionnaire; the Rome III questionnaire; and the Visual Analog Scale for Irritable Bowel Syndrome (VAS-IBS). Blood samples were analyzed by ultra-high-performance liquid chromatograph mass spectrometry (UHLC-MS/UHPLC-MSMS). The women, 63.1 (58.7–67.2) years old, were categorized based on CC (n = 76) and LC (n = 55); one episode or refractory MC; IBS-like symptoms or not; use of corticosteroids or not; and smoking habits. The only metabolomic differences found in the univariate model after adjustment for false discovery rate (FDR) were between smokers and non-smokers. Serotonin was markedly increased in smokers (p < 0.001). No clear patterns appeared when conducting a principal component analysis (PCA). No differences in the metabolomic profile were found depending on the type or clinical course of the disease, neither in the whole MC group nor in the subgroup analysis of CC.

The reasons for spatial and temporal variation in methane emission from mire ecosystems are not fully understood. Stable isotope signatures of the emitted methane can offer clues to the causes of these variations. We measured the methane emission (FCH4) and 13C signature (δ13C) of emitted methane by automated chambers at a hemiboreal mire for two growing seasons. In addition, we used ambient methane mixing ratios and δ13C to calculate a mire-scale 13C signature using a nocturnal boundary-layer accumulation approach. Microbial methanogenic and methanotrophic communities were determined by a captured metagenomics analysis. The chamber measurements showed large and systematic spatial variations in δ13C-CH4 of up to 15 ‰ but smaller and less systematic temporal variation. According to the spatial δ13C–FCH4 relations, methanotrophy was unlikely to be the dominating cause for the spatial variation. Instead, these were an indication of the substrate availability of methanogenesis being a major factor in explaining the spatial variation. Genetic analysis indicated that methanogenic communities at all sample locations were able to utilize both hydrogenotrophic and acetoclastic pathways and could thus adapt to changes in the available substrate. The temporal variation in FCH4 and δ13C over the growing seasons showed hysteresis-like behavior at high-emission locations, indicative of time-lagged responses to temperature and substrate availability. The upscaled chamber measurements and nocturnal boundary-layer accumulation measurements showed similar average δ13C values of −81.3 ‰ and −79.3 ‰, respectively, indicative of hydrogenotrophic methanogenesis at the mire. The close correspondence of the δ13C values obtained by the two methods lends confidence to the obtained mire-scale isotopic signature. This and other recently published data on δ13C values of CH4 emitted from northern mires are considerably lower than the values used in atmospheric inversion studies on methane sources, suggesting a need for revision of the model input.

Background Many seniors need to increase their physical activity (PA) and participation in fall prevention exercise. Therefore, digital systems have been developed to support fall-preventive PA. Most of them lack video coaching and PA monitoring, two functionalities that may be relevant for increasing PA. Objective To develop a prototype of a system to support seniors’ fall-preventive PA, which includes also video coaching and PA monitoring, and to evaluate its feasibility and user experience. Methods A system prototype was conceived by integrating applications for step-monitoring, behavioural change support, personal calendar, video-coaching and a cloud service for data management and co-ordination. Its feasibility and user experience were evaluated in three consecutive test periods combined with technical development. In total, 11 seniors tested the system at home for four weeks with video coaching from health care professionals. Results Initially, the system's feasibility was non-satisfactory due to insufficient stability and usability. However, most problems could be addressed and amended. In the third (last) test period, both seniors and coaches experienced the system prototype to be fun, flexible and awareness-raising. Interestingly, the video coaching which made the system unique compared to similar systems was highly appreciated. Nonetheless, even the users in the last test period highlighted issues due to insufficient usability, stability and flexibility. Further improvements in these areas are needed. Conclusions Video coaching in fall-preventive PA can be valuable for both seniors and health care professionals. High reliability, usability and flexibility of systems supporting seniors are essential.

Lignocellulosic biomass is the most abundantly available resource in nature. However, its potential as a replacement of oil in plastic production has not been fully exploited. To reduce the carbon footprint, the use of lignocellulose biomass to produce bio-based plastics is attracting increasing global interest. The aim of this review article is to systematically summarize the recent advancements of the development of lignocellulose materials that possess thermoplastic properties, meaning they can be processed/shaped by common plastic processing techniques. The approaches used for modification of lignocellulose biomass and the properties of the modified materials, as well as factors affecting the properties of these, are discussed. The regulatory aspects and policy directions of bio-based plastics, including thermoplastic lignocellulose, are also mentioned. Current challenges of producing thermoplastic lignocellulose and the way forward to solve this are also explored.

Abstract Background Skeletal muscle fiber type distribution has implications for human health, muscle function, and performance. This knowledge has been gathered using labor-intensive and costly methodology that limited these studies. Here, we present a method based on muscle tissue RNA sequencing data (totRNAseq) to estimate the distribution of skeletal muscle fiber types from frozen human samples, allowing for a larger number of individuals to be tested. Methods By using single-nuclei RNA sequencing (snRNAseq) data as a reference, cluster expression signatures were produced by averaging gene expression of cluster gene markers and then applying these to totRNAseq data and inferring muscle fiber nuclei type via linear matrix decomposition. This estimate was then compared with fiber type distribution measured by ATPase staining or myosin heavy chain protein isoform distribution of 62 muscle samples in two independent cohorts (n = 39 and 22). Results The correlation between the sequencing-based method and the other two were rATPas = 0.44 [0.13–0.67], [95% CI], and rmyosin = 0.83 [0.61–0.93], with p = 5.70 × 10–3 and 2.00 × 10–6, respectively. The deconvolution inference of fiber type composition was accurate even for very low totRNAseq sequencing depths, i.e., down to an average of ~ 10,000 paired-end reads. Conclusions This new method ( https://github.com/OlaHanssonLab/PredictFiberType ) consequently allows for measurement of fiber type distribution of a larger number of samples using totRNAseq in a cost and labor-efficient way. It is now feasible to study the association between fiber type distribution and e.g. health outcomes in large well-powered studies.

Abstract Frequent fruit consumption has been associated with lower office blood pressure. Less is known about associations between fruit consumption and home blood pressure. Our aim was to study the correlation between consumption of specific fruits and home blood pressure in a large randomly selected study population. The main outcome was systolic home blood pressure. Home blood pressure measurements were performed with calibrated oscillometric meters during seven consecutive days. Means for all available measurements were used. Validated food frequency questionnaires were used for estimating frequency of fruit consumption. The specified fruits were bananas, apples/pears and oranges/citrus fruit. Complete case analysis regarding fruit consumption, office‐ and home blood pressure measurements and other relevant variables was performed in 2283 study participants out of 2603 available. Multivariable linear regression analysis was performed. There were statistically significant associations between consumption of all fruit types and lower systolic home blood pressure unadjusted (p for trend; bananas, apples/pears and oranges/citrus fruit p < .001). The numerical differences between most and least frequent consumption of fruit were for bananas ‐2.7 mm Hg, apples/pears ‐3.9 mm Hg and for oranges/citrus fruit ‐3.4 mm Hg. When adjusted for covariates, both consumption of apples/pears and oranges/citrus fruit had an independent statistically significant association with lower blood pressure (p = .048 resp. p = .009). Future controlled interventional studies are needed to evaluate the effect of specific fruit on home blood pressure.

Regular device-scale DNA waves for high DNA concentrations and flow velocities have been shown to emerge in quadratic micropillar arrays with potentially strong relevance for a wide range of microfluidic applications. Hexagonal arrays constitute another geometry that is especially relevant for the microfluidic pulsed-field separation of DNA. Here, we report on the differences at the micro and macroscopic scales between the resulting wave patterns for these two regular array geometries and one disordered array geometry. In contrast to the large-scale regular waves visible in the quadratic array, in the hexagonal arrays, waves occur in a device-scale disordered zig-zag pattern with fluctuations on a much smaller scale. We connect the large-scale pattern to the microscopic flow and observe flow synchronization that switches between two directions for both the quadratic and hexagonal arrays. We show the importance of order using the disordered array, where steady-state stationary and highly fluctuating flow states persist in seemingly random locations across the array. We compare the flow dynamics of the arrays to that in a device with sparsely distributed pillars. Here, we observe similar vortex shedding, which is clearly observable in the quadratic and disordered arrays. However, the shedding of these vortices couples only in the flow direction and not laterally as in the dense, ordered arrays. We believe that our findings will contribute to the understanding of elastic flow dynamics in pillar arrays, helping us elucidate the fundamental principles of non-Newtonian fluid flow in complex environments as well as supporting applications in engineering involving e.g., transport, sorting, and mixing of complex fluids.

Abstract Background The Ponseti method is the gold standard for clubfoot treatment. However, relapse and residual gait deviations are common, and follow-up until 7 years of age is recommended. We evaluated the reliability of the foot drawing method, a new instrument for the follow-up of clubfoot. The method uses drawings of the foot in the neutral position and external rotation to measure foot length and outward rotation. Methods Nineteen children aged 2.5–7 years who were treated with the Ponseti method for congenital clubfoot were included. Two raters made the drawings twice (D1 and D2). Each rater measured foot length, foot rotation, and foot–tibial rotation independently (D1). Later, the raters repeated the measurements (D2). Interrater reliability was assessed using the D1 from each rater. Intrarater reliability was assessed using the measurements from each rater’s D1 and D2. Bland–Altman plots were used to visualize the limits of agreement (LoA). The mean, 95% confidence interval, and one standard deviation of the differences in all measurements were calculated. För lärare är texter och skriftpraktiker centrala inslag i den dagliga yrkesutövningen. Syftet med den här studien är att bättre förstå läraryrket och dess förutsättningar med fokus på textanvändning, och studien söker svar på hur några lärare i skolans senare år (7–9 och gymnasiet) beskriver, uppfattar och förhåller sig till läsande och skrivande inom yrket samt hur de beskriver skriftpraktiksanknutna utmaningar och resurser. Materialet för studien består av ljudinspelade fokusgruppsamtal. Studien finner att lärarnas samtal präglas av allmängiltiga resonemang om texter och textanvändning genom att lärarna fokuserar på aspekter som är relevanta för i princip alla skrivsituationer, till exempel tydlighet och textlängd. Vidare framträder bilden av att 7–9-lärarna har ett mer kollektivt orienterat förhållningssätt gentemot yrkets skriftpraktiker än vad gymnasielärarna har. Ifråga om skriftpraktiksanknutna utmaningar ser lärarna framför allt utmaningar i skrivandet av texter som de menar har indirekt undervisningsanknytning, och särskilt beskriver de utmaningar med att formulera sig på sätt så att mottagare inte blir upprörda. Av samtalen framgår också att de resurser som lärarna ser sig ha för att utveckla kompetens att hantera skriftpraktiksanknutna utmaningar utgörs av informella strukturer, vilket är anmärkningsvärt med tanke på att de aktuella skriftpraktikerna är centrala i deras yrkesutövning.

The Zeppelin Observatory (78.90∘ N, 11.88∘ E) is located on Zeppelin Mountain at 472 m a.s.l. on Spitsbergen, the largest island of the Svalbard archipelago. Established in 1989, the observatory is part of Ny-Ålesund Research Station and an important atmospheric measurement site, one of only a few in the high Arctic, and a part of several European and global monitoring programmes and research infrastructures, notably the European Monitoring and Evaluation Programme (EMEP); the Arctic Monitoring and Assessment Programme (AMAP); the Global Atmosphere Watch (GAW); the Aerosol, Clouds and Trace Gases Research Infrastructure (ACTRIS); the Advanced Global Atmospheric Gases Experiment (AGAGE) network; and the Integrated Carbon Observation System (ICOS). The observatory is jointly operated by the Norwegian Polar Institute (NPI), Stockholm University, and the Norwegian Institute for Air Research (NILU). Here we detail the establishment of the Zeppelin Observatory including historical measurements of atmospheric composition in the European Arctic leading to its construction. We present a history of the measurements at the observatory and review the current state of the European Arctic atmosphere, including results from trends in greenhouse gases, chlorofluorocarbons (CFCs) and hydrochlorofluorocarbons (HCFCs), other traces gases, persistent organic pollutants (POPs) and heavy metals, aerosols and Arctic haze, and atmospheric transport phenomena, and provide an outline of future research directions.

Sputter-deposited thin films (33–1160 nm) from EUROFER97 were obtained on different substrates (C, Si, W, MgO) in argon and a mix of argon and deuterium atmosphere. The composition, microstructure, and mechanical properties of the films were analyzed and compared to those of the bulk material. The films feature lower density (-10%), higher hardness (+79%), and smaller crystallites in comparison to the bulk. Despite such differences, the elemental atomic composition of the films and the bulk was very similar, as determined by ion beam analysis. Deposition in deuterium-containing atmosphere resulted in a low deuterium incorporation (0.28% of atomic content), indicating low retention of hydrogen-isotopes in the deposited material.

High acid-consumption and lipid oxidation are challenges when recovering functional proteins from herring co-products via pH-shift-processing. Here, lingonberry press-cake (LP), which is abundant in organic acids and phenolics, was added to alkali-solubilized herring-co-product-proteins (2.5–30 % LP per dry weight) aiming to aid protein precipitation, save hydrochloric acid (HCl) and provide oxidative stability. The results revealed 5–30 % LP addition reduced HCl-consumption by 13–61 % and 19–79 % when precipitating proteins at pH 5.5 and 6.5, respectively. Higher LP% decreased protein content and lightness of protein isolates but raised the lipid content. Precipitation at pH 6.5 used less acid, reduced total protein yield and raised moisture content and darkness of isolates. Contrary to controls, lipid oxidation-derived volatiles did not develop in protein isolates precipitated with 10 % and 30 % LP, neither during the process itself nor during 21 days on ice. Altogether, LP was identified as a promising all-natural processing-aid to use during herring protein isolation.

Carbon dioxide storage is one of the main long-term strategies for reducing carbon dioxide emissions in the atmosphere. A clear example is Norway’s Longship project. If these projects should succeed, the transport of huge volumes of carbon dioxide from the emissions source to the injection points may become a complex challenge. In this work, we propose the production of sodium-based organic carbonates that could be transported to storage sites and be reconverted to CO2. Solid carbonates can be transported in considerably lower volumes than gases or pressurized liquids. Sodium-based carbonates are insoluble in most of the organic solvents and will therefore precipitate in contrast to in aqueous solutions. Particularly, here we focus on sodium hydroxide-ethanol mixtures as solvents for precipitating sodium ethyl carbonate and sodium bicarbonate. Previous works on this approach used limited sodium hydroxide concentrations, which are insufficient to prove the effectiveness of the proposed process. In this paper, we studied higher sodium hydroxide concentrations in sodium hydroxide-ethanol mixtures than previously reported in the literature. To this end, we use the following strategy: (1) In-line monitoring of the formation of carbonates using an in-line FTIR; (2) In-line measurements of the weight increase, which correspond directly to the captured carbon dioxide and reveal the absorption capacity; (3) Characterization of the solids with X-ray diffraction and scanning electron microscope. Our FTIR results confirmed that both sodium ethyl carbonate and sodium bicarbonate were formed, which agrees with X-ray diffraction and scanning electron microscope. With this reactor design, the absorption capacities reached approximately 80–93% of the theoretical values (4.8–13.3 g/L respectively). We hypothesize that full conversion is hampered because the gas might take preferential paths due to gel formation during the experiments.

Abstract Background Intramedullary nailing is the method of choice for diaphyseal fractures of the femur and tibia and is also commonly used to treat trochanteric hip fractures. Perioperative imaging is essential for visualising adequate reduction, achieving an optimal entry point (EP) and performing distal locking (DL) of intramedullary nails. This methodological study aims to compare biplanar and uniplanar imaging in some steps of intramedullary nailing. Methods We used a biplanar preassembled imaging device (Biplanar™ 600s, Swemac Imaging) and a uniplanar imaging device (Ziehm Solo FD, Ziehm Imaging) to measure procedural and radiation times for antegrade and retrograde femoral and antegrade tibial nailing in fully soft flexible tissue encased legs with radiopaque sawbones (SKU:1515–7-11, Sawbones Europe, Malmö, Sweden). Four orthopaedic surgeons with different levels of experience performed all procedures in all three phantoms with both image techniques in random order, producing in total 12 EPs and nailings with DL with each imaging device. Time to EP, radiation times, time to DL for both devices and the number of swings of the uniplanar device for the two procedures were measured. Comparisons between the biplanar and uniplanar systems with a paired-samples t-test were conducted. Results Using the biplanar device, time to optimal EP was shorter for retrograde femoral (26 s (SD15) vs 35 s (SD13), p = 0.01) and for antegrade tibial nailing (23 s (SD13) vs 49 s (SD24), p = 0.001). No statistically significant differences in time to EP, radiation time or time to DL were found for antegrade femoral nailing. A median of two swings of the uniplanar device was needed to obtain optimal EP for all procedures. Conclusions Biplanar imaging slightly but statistically significantly reduced time to EP for retrograde femoral and antegrade tibial nailing in this methodological study comparing biplanar and uniplanar imaging techniques. Biplanar imaging can reduce time and radiation exposure when defining the EP around the knee in intramedullary nailing procedures, but the clinical relevance of these time savings remain to be defined. For antegrade femoral nailing we found no clear benefit with biplanar imaging in the investigated steps of nailing.

This study aimed to increase knowledge of how special needs education (SNE) in Finnish comprehensive schools was arranged at a distance during the COVID-19 school closures in the spring of 2020 and to clarify what types of challenges special needs teachers (SNTs) faced when teaching and supporting students with special educational needs (SEN). Data were collected through semistructured interviews with 12 SNTs and the analysis was based on a thematic analytical approach. The results revealed that SNTs’ experiences during the COVID-19 distance education period varied greatly. Still, it is possible to distinguish some general features across the dataset. Teaching and support arrangements for students included the use of educational technology and distance learning materials, implementation of SNE via exceptional arrangements and flexible solutions and providing SNE through differentiation, interaction, and technology. In addition, the results revealed, the SNTs faced a multitude of challenges during the COVID-19 distance education period; this included changes in their daily work life, increased stress, challenges in remote communication and collaboration and structural and organisational issues affecting SNE.

Summary: Background: Cancer immunity is based on the interaction of a multitude of cells in the spatial context of the tumour tissue. Clinically relevant immune signatures are therefore anticipated to fundamentally improve the accuracy in predicting disease progression. Methods: Through a multiplex in situ analysis we evaluated 15 immune cell classes in 1481 tumour samples. Single-cell and bulk RNAseq data sets were used for functional analysis and validation of prognostic and predictive associations. Findings: By combining the prognostic information of anti-tumoural CD8+ lymphocytes and tumour supportive CD68+CD163+ macrophages in colorectal cancer we generated a signature of immune activation (SIA). The prognostic impact of SIA was independent of conventional parameters and comparable with the state-of-art immune score. The SIA was also associated with patient survival in oesophageal adenocarcinoma, bladder cancer, lung adenocarcinoma and melanoma, but not in endometrial, ovarian and squamous cell lung carcinoma. We identified CD68+CD163+ macrophages as the major producers of complement C1q, which could serve as a surrogate marker of this macrophage subset. Consequently, the RNA-based version of SIA (ratio of CD8A to C1QA) was predictive for survival in independent RNAseq data sets from these six cancer types. Finally, the CD8A/C1QA mRNA ratio was also predictive for the response to checkpoint inhibitor therapy. Interpretation: Our findings extend current concepts to procure prognostic information from the tumour immune microenvironment and provide an immune activation signature with high clinical potential in common human cancer types. This article provides a first analysis of the expression of phasal polarity in Swahili. Phasal polarity (henceforth PhP) refers to linguistic concepts which express the phase of a given situation in relation to a prior and/or subsequent phase, as well as expressing whether a certain situation holds or not. These concepts, represented here in English as a meta-language with already, no longer, still and not yet, are interrelated in interesting ways and form a semantic sub-system. In contrast to many other Eastern Bantu languages, we show that the dedicated expressions for PhP concepts in Swahili are mainly adverbs, with limited use of verbal affixes. It also stands out in the area by not having any gaps in the expressibility of PhP concepts, and by making use of internal negation. In order to target present-day spoken Swahili, the results are based on speaker interviews, through the use of carefully introduced contexts. The main strategy for expressing already was through the verbal affix sha-. There is also an adverb tayari ‘ready’ to express this concept, which occurs not infrequently in our results. We show that there are differences in their distribution, and hypothesize that this could be related to an ongoing change in the use of (me)sha- in relation to perfective me-. For all other PhP concepts, adverbs are used as the main strategy. There was variability in speaker responses in the use of constructions which we have considered contextual paraphrases rather than dedicated PhP expressions. The current work is inspired by a recent increase in interest in PhP systems in languages of the African continent, previously relatively unexplored (Kramer 2021). The analysis is based on the parameters for cross-linguistic comparison as presented by Kramer (2017)

PurposeTestosterone analysis in hair allows for retrospective evaluation of endogenous testosterone concentrations, but studies devoted to investigating confounders in hair testosterone analysis have hitherto been scarce. The current study examined the stability of testosterone concentrations between two hair samples collected three months apart and investigated two potential confounding factors: natural hair colour and cosmetic hair treatments.MethodsTestosterone was analysed with an in-house radioimmunoassay with a limit of detection adequate for the purpose.ResultsThe testosterone concentrations from the two samplings, at baseline and three months later, had an intra-individual correlation of moderate strength (rho = 0.378, pConclusionThe current results suggest that cosmetic hair treatments need to be considered in hair testosterone analyses and demonstrate the utility of a radioimmunoassay to reliably measure testosterone concentrations in small hair samples in women.

Attaining sufficient flow uniformity in catalytic aftertreatment systems is a major challenge for the automotive industry. Computational fluid dynamics (CFD) simulations offer means of analyzing and quantifying this flow uniformity in silico. In this work, predictions from numerical simulations of flow uniformity obtained using a conventional steady-state Reynolds-Averaged Navier-Stokes (RANS) approach are contrasted against comprehensive Detached Eddy Simulations (DES) where the large-scale turbulence is resolved in space and time. It is shown that the DES approach provides access to data on flow uniformity fluctuations that could be significant for the catalyst light-off behavior. However, the computational cost of the DES is approximately three orders of magnitude larger than that of the corresponding RANS simulation.

IntroductionCollagenous colitis (CC) is an inflammatory bowel disease, which usually responds to budesonide treatment. Our aim was to study the immunological background of the disease.MethodsAnalyses of peripheral and mucosal MAIT (mucosa associated invariant T cells) and NK (natural killer) cells were performed with flow cytometry. Numbers of mucosal cells were calculated using immunohistochemistry. We studied the same patients with active untreated CC (au-CC) and again while in remission on budesonide treatment. Budesonide refractory patients and healthy controls were also included. The memory marker CD45R0 and activation marker CD154 and CD69 were used to further study the cells. Finally B cells, CD4+ and CD8+ T cells were also analysed.ResultsThe percentages of circulating CD56dimCD16+ NK cells as well as MAIT cells (CD3+TCRVa7.2+CD161+) were decreased in au-CC compared to healthy controls. This difference was not seen in the mucosa; where we instead found increased numbers of mucosal CD4+ T cells and CD8+ T cells in au-CC. Mucosal immune cell numbers were not affected by budesonide treatment. In refractory CC we found increased mucosal numbers of MAIT cells, CD4+ and CD8+ T cells compared to au-CC.DiscussionPatients with active collagenous colitis have lower percentages of circulating MAIT and NK cells. However, there was no change of these cells in the colonic mucosa. Most mucosal cell populations were increased in budesonide refractory as compared to au-CC patients, particularly the number of MAIT cells. This may indicate that T cell targeting therapy could be an alternative in budesonide refractory CC.

ABSTRACT: Providing support to children in their younger years is prominent in Finnish early childhood education and care (ECEC), as most children need some form of support for learning and development during this stage. Itinerant early childhood special education teachers (ECSETs) are important resources in providing support to children with special educational needs (SEN). Previous research in Finland addresses areas where itinerant ECSETs predominantly work in contexts where Finnish is the medium of instruction. Therefore, it is of interest to examine itinerant ECSETs' views of elements affecting their work with supporting children with SEN in Swedish-medium ECEC settings. This research is explorative to its character and data was collected through a questionnaire sent to all itinerant ECSETs working in Swedish-speaking regions of Finland. Descriptive statistics were used to depict the work conditions for ECSETs'. The results show that ECSETs own professional ambition and children’s support needs affect the work the most. Furthermore, inequality in ECSETs working conditions have direct consequences for practice. This study concludes with a discussion of how ECSETs' working conditions influence the support that children receive and areas that should be addressed to ensure equal and efficient learning for all children.

Background and aim: Poor dental health status has been linked to increased risk of cardiovascular events in type 2 diabetes (T2D). Less is known about self-perceived dental health and cardiovascular risk. Our aim with this study was to investigate this association. Methods: Recruitment of T2D patients took place between 2005 and 2008 in Swedish primary care. Teeth satisfaction was assessed by questionnaire at baseline. The major adverse cardiovascular events (MACE) in this study were hospitalization due to myocardial infarction, stroke or cardiovascular death. Cox regression models were used. Results: Out of 761 participants 601 had complete data. Ninety-two MACEs occurred (median follow-up time: 11.6 years). Those satisfied with their teeth (n = 458) had 61 events (1.2 events per 100 person-years), while those dissatisfied with teeth (n = 143) had 31 events (2.2 events per 100 person-years). Dissatisfaction with teeth was associated with an increased risk of MACE independent of age, sex and levels of CRP (HR 1.85, 95% CI 1.20 – 2.86). Conclusions: In patients with T2D, dissatisfaction with teeth was associated with increased risk of MACE and may be considered as a marker of risk.

IntroductionAtropine is an essential part of the treatment protocol for equine uveitis. Topical atropine administration has been associated with decreased intestinal motility and abdominal pain in horses. Experimental studies have indicated that frequent dosing is associated with a higher risk than dosing every 6 h. Unfortunately, no quantitative pharmacodynamic data for inhibition of the equine gut are published.Materials and methodsEight standardbred horses were assigned to receive either atropine or saline (control) to be infused over 30 min in a two-treatment cross-over design. Atropine concentrations in plasma were measured using ultra-high-performance liquid chromatography–tandem mass spectrometry. Intestinal motility was measured using borborygmi frequency and electrointestinography (EIG). Experimental data were analyzed using a non-linear mixed effects model. The model was then used to simulate different dosing regimens.ResultsAtropine significantly decreased borborygmi response and EIG response. Six horses developed clinical signs of abdominal pain. The pharmacokinetic typical values were 0.31, 1.38, 0.69, and 1.95 L/kg·h for the volumes of the central, the highly perfused, the scarcely perfused compartments, and the total body clearance, respectively. The pharmacodynamic typical values were 0.31 μg/L and 0.6 and 207 nV27 cpm for the plasma concentration at 50% of the maximum response and the maximum response and the baseline of cecal EIG response, respectively. Six different dosing regimens of topical atropine sulfate to the eye (0.4 and 1 mg every hour, every 3 h, and every 6 h) were simulated.ConclusionThe IV PK/PD data coupled with simulations predict that administration of 1 mg of topical atropine sulfate administered to the eye every hour or every 3 h will lead to atropine accumulation in plasma and decreased intestinal myoelectric activity. Administration every 6 h predicted a safe dosing regimen in full-sized horses. Clinical studies would be valuable to confirm the conclusions. For smaller equids and horses put at risk for colic due to othercauses, droplet bottles that deliver 40 μl of 1% atropine sulfate per drop or less may be used to lower the risk further.
