ABSTRACT Background: The anorectal anomalies consist in a complex group of birth defects. Laparoscopic-assisted anorectoplasty improved visualization of the rectal fistula and the ability to place the pull-through segment within the elevator muscle complex with minimal dissection. There is no consensus on how the fistula should be managed. Aim: To evaluate the laparoscopic-assisted anorectoplasty and the treatment of the rectal urinary fistula by a bipolar sealing device. Method: It was performed according to the original description by Georgeson1. Was used 10 mm infraumbilical access portal for 30º optics. The pneumoperitoneum was established with pressure 8-10 cm H2O. Two additional trocars of 5 mm were placed on the right and left of the umbilicus. The dissection started on peritoneal reflection using Ligasure(r). With the reduction in the diameter of the distal rectum was identified the fistula to the urinary tract. The location of the new anus was defined by the location of the external anal sphincter muscle complex, using electro muscle stimulator externally. Finally, it was made an anastomosis between the rectum and the new location of the anus. A Foley urethral probe was left for seven days. Results: Seven males were operated, six with rectoprostatic and one with rectovesical fistula. The follow-up period ranged from one to four years. The last two patients operated underwent bipolar sealing of the fistula between the rectum and urethra without sutures or surgical ligation. No evidence of urethral leaks was identified. Conclusion: There are benefits of the laparoscopic-assisted anorectoplasty for the treatment of anorectal anomaly. The use of a bipolar energy source that seals the rectal urinary fistula has provided a significant decrease in the operating time and made the procedure be more elegant.

PURPOSE: To evaluate the prevalence of testicular microlithiasis among pediatric patients with inguinoscrotal affections. METHODS: Between January 2005 and January 2010, we evaluated, prospectively 1504 children ranging from 1 to 15 years with inguinoscrotal affections with a high-frequency ultrasound system, which employs a 10-MHz transducer. RESULTS: Testicular microlithiasis was identified in 20 testes of eleven children (0.71% of 1504 patients evaluated), through an ultrasound scan. Testicular microlithiasis was found in 5 children with cryptorchidism (3.93% of 127 patients), 4 children with retractile testes (14.8% of 27 patients), 1 child with a hypotrophic testis (100% of 1 patient), and 1 child with inguinal hernia (0.07% of 1349 patients). The children with testicular microlithiasis were submitted to annual physical examinations and ultrasound evaluations. CONCLUSIONS: Testicular microlithiasis was a rare condition and occurred in 0.7% of the subjects studied. The association with cryptorchidism, retractile and hypotrophic testis was significant.OBJETIVO: Avaliar a prevalência de microlitíase testicular entre pacientes pediátricos com afecções inguinoescrotais. MÉTODOS: Estudo prospectivo entre janeiro de 2005 a janeiro de 2010, utilizando ultrasonografia escrotal em 1504 crianças (de 1 a 15 anos) com afecções inguinoescrotais. RESULTADOS: Microlitíase testicular foi identificada em 20 testículos de 11 crianças (0,71% dos 1504 pacientes). 5 crianças com criptorquidia (3,93% de 127 pacientes), 4 com testículo retrátil (14,8% de 27 pacientes), 1 com hipotrofia testicular e 1 com hérnia inguinal (0,07% de 1349 crianças). As crianças foram avaliadas anualmente com exame físico e ultrassonografia inguinoescrotal. CONCLUSÕES: A microlitíase testicular é uma entidade rara, ocorrendo em 0,7% dos pacientes pediátricos com afecções inguinoescrotais. A associação com a criptorquidia, testículo retrátil e a hipotrofia testicular foi significativa.

PURPOSE: To assess the effects of Roux-en-Y jejunal limb length on gastric emptying and enterogastric reflux. METHODS: Seventy male Wistar rats were submitted to antrectomy with Roux-en-Y reconstruction and then were divided into two groups of 35 animals. Group A, short limb (7.5 cm) and Group B, standard limb (15 cm). Group A and B were subdivided into five subgroups each in order to study enterogastric reflux at 30 and 60 minutes and to evaluate gastric emptying at 5, 10 and 15 minutes. In order to measure gastric emptying and enterogastric reflux, radiotracers 99m Tc-Phytate and 99m Tc-DISIDA were respectively used. RESULTS: For gastric emptying, the radiotracer concentration was lower in Group A than in Group B after five minutes. The enterogastric reflux was present, but there were no significant differences between enterogastric reflux indexes concerning both A and B Groups. CONCLUSION: A standard Roux limb, besides being unable to protect the stomach from the enterogastric reflux, may become a functional barrier for gastric emptying.OBJETIVO: Determinar os efeitos do comprimento da alça jejunal em Y de Roux sobre o esvaziamento gástrico e o refluxo enterogástrico. MÉTODOS: Setenta e cinco ratos machos foram submetidos à antrectomia com reconstrução em Y de Roux e divididos em dois grupos de 35 animais. Grupo A, alça curta (7,5cm) e Grupo B (15cm), alça de comprimento padrão. Os grupos A e B foram subdivididos em cinco subgrupos cada para o estudo do refluxo enterogástrico aos 30 e 60 minutos e para o estudo do esvaziamento gástrico aos 5, 10 e 15 minutos. 99m Tc-Fitato and 99m Tc-DISIDA foram utilizados para os estudos do esvaziamento gástrico e do refluxo enterogástrico, respectivamente. RESULTADOS: No estudo do esvaziamento gástrico, a concentração do radiotraçador foi menor no grupo A do que no Grupo B aos cinco minutos. Foi encontrado o refluxo enterogástrico, nos grupos A e B, sem diferenças entre eles. CONCLUSÃO: A alça em Y de Roux de comprimento padrão foi ineficaz em proteger o estômago do refluxo enterogástrico, e pode tornar-se uma barreira funcional ao esvaziamento gástrico.

PURPOSE: The aim of this study was to investigate alterations compatible with hepatic ischemia-reperfusion after bilioduodenal shunt (BD) in rats with obstructive jaundice (OB) . METHODS: Thirty six animals were divided into 6 experimental groups: CO1 and CO2 - control groups, sham-operated (SO) and evaluated 6 and 24 hours after, respectively; OB1 and OB2, - obstructive jaundice groups, sham-operated 15 days after bile duct ligature and evaluated 6 and 24 hours after SO, respectively; DBD1and DBD2 - obstructive jaundice groups evaluated ,respectively, 6 and 24 hours after BD performed 15 days after bile duct ligature. The parameters evaluated were serum total bilirubin, aminotransferase activity (AST, ALT), TNFalpha, liver mitochondrial functions and parenchymatous injury. RESULTS: Bilirubin decreased while aminotransferase activity increased 6 hours after BD (pOBJETIVO: O objetivo do estudo foi investigar alterações compatíveis com o fenômeno de isquemia-reperfusão hepática em ratos com icterícia obstrutiva (OB) após derivação bilioduodenal (BD). MÉTODOS: Trinta e seis animais foram divididos em seis grupos experimentais: CO1 e CO2 - grupos controle avaliados com 6 e 24 horas após operação simulada (SO), respectivamente; OB1 e OB2 - grupos com obstrução biliar, submetidos a SO 15 dias após ligadura do ducto biliar, e avaliados em 6 e 24 horas após a SO, respectivamente; DBD1 e DBD2 - grupos com 15 dias de obstrução biliar, avaliados em 6 e 24 horas, respectivamente, após BD. Os parâmetros avaliados foram bilirrubinas, aminotranferases (ALT e AST), funções mitocondriais hepáticas, dosagem de TNFalfa e lesão do parênquima hepático. Introdução: A Histiocitose de células de Langerhans (HCL) corresponde a um grupo heterogêneo de desordens caracterizadas pela proliferação monoclonal de células dendríticas. Com incidência de 5 a 10 casos/milhão/ano, apresenta etiologia desconhecida e um amplo espectro clínico, manifestando-se principalmente por meio de lesões ósseas e cutâneas. Predomina no sexo masculino e em menores de 15 anos. Relato de caso: Relato de caso de uma criança do sexo feminino, 4 anos de idade, que apresentava uma grande massa em mediastino anterior, aderida aos vasos da base do coração. A biópsia da lesão sugeriu HCL, o que foi confirmado pelo estudo imuno-histoquímico, positivo para os marcadores CD1a e S100. Na investigação complementar, também foi observado acometimento cutâneo pela doença. Conclusão: O acometimento linfonodal pela HCL ocorre em apenas 5-10% dos casos. Ao afetar os linfonodos mediastinais, pode resultar em tumores de tamanhos variados. Nesses casos, o diagnóstico constitui um desafio, haja vista o grande número de patologias que se manifestam por massas mediastinais. Dessa forma, enfatiza-se a importância de incluir a HCL no diagnóstico diferencial das massas mediastinais em crianças.

Visceral leishmaniasis (VL), also known as Kala-azar, is a systemic infection caused by a protozoan (Leishmania) and, in its classic form, is a serious illness associated with malnutrition, anemia, hepatosplenomegaly, infectious processes and coagulopathies. The effect of splenectomy in patients with visceral leishmaniasis is not well defined; however, it is known that the spleen is the largest reservoir of infected cells belonging to the reticulo endothelial system. Therefore, the surgical procedure is an option for the debulking of parasites, providing a cure for refractory VL and minimizing the complications of hypersplenism.
