Bullet embolism of pulmonary artery is a vascular traumatic entity that is a very rare but with serious complications. The bullet can lead to vascular embolism by entering directly into the intravascular area or by migration. In pulmonary artery bullet embolism, complications with high mortality such as thrombosis, sepsis, endocarditis, cerebral stroke may be encountered. Observation and follow-up are sufficient in asymptomatic venous bullet embolism cases. In this case report, we aimed to share our experience with a patient who developed a bullet embolism of pulmonary artery as a result of gunshot injury from the right iliac region.

INTRODUCTION[|]The relationship between the surgeon's experience/volume of performed operations and postoperative results of thyroid surgery is a pressing issue that has been widely discussed in recent publications. This study aimed to compare the complication rates in thyroidectomy operations performed before and after specialization and evaluate the effect of specialization on the outcomes of thyroid surgeries.[¤]METHODS[|]The study included patients who had undergone thyroidectomy with or withoutneck dissection due to benign or malign thyroid diseases in a single tertiary reference hospital between April 2013 and March 2017. The patients were divided into two groups: those who were operated on before specialization (BS) and after specialization (AS). Age, gender, operation type, postoperative hypocalcemia, incidental parathyroidectomy, recurrent laryngeal nerve (RLN) injury, and postoperative bleeding or hematoma were compared between the groups.[¤]RESULTS[|]Of the thyroid patients, 776 were operated on (367 (47%) and 409 (53%) of the BS and AS groups, respectively). No significant difference was found between the two groups regarding the postoperative Ca2+ level, while the parathormone was significantly lower in the BS group (p=0.2 and p=0.02, respectively). Aim:This prospective clinical trial study aimed to compare the effects of 0.2% glyceryl trinitrate (GTN) and 0.5% topical nifedipine (ND) on acute anal fissure treatment and recurrence prevention.Method:The study included 100 patients who were diagnosed with acute anal fissure and randomly divided into two groups: the 0.2% GTN and 0.5% topical ND groups. On the first visit, age, sex, constipation, pain duration after defaecation and visual analogue scale (VAS) score were recorded. After 21 days of treatment, symptomatic relief levels, healing, VAS score and drug side effects were recorded.Results:The female/male ratios for the GTN and ND groups were 50%/50% and 54%/46%, respectively. The mean ages were 29.3 and 30.7 years for the GTN and ND groups, respectively. After treatment, the excellent symptomatic relief level in the ND group (56%) was higher than in the GTN group (22%; p=0.005). Enterobius vermicularis is a common intestinal nematode of humans that can be considered relatively harmless. A polypoid lesion mimicking malignancy was detected in the rectum of a 66-year-old female patient who had been operated for sigmoid colon adenocarcinoma in the past. Histopathological examination of the lesion revealed no malignancy but there was adult E. vermicularis nematodes and eggs. In this case report, we aimed to present an enterobiasis infestation that produces non-necrotizing granuloma tissue in the rectum.

Aim:Despite surgical and radiological innovations, gallbladder cancer (GC) is usually diagnosed incidentally by the pathological examination of the cholecystectomy specimens, and it is defined as incidental GC (IGC).Methods:Medical files of patients, who underwent cholecystectomy, were analyzed retrospectively. We investigated the relationship between clinicopathological features and survival in patients with IGC.Results:We performed cholecystectomy surgery in 6225 patients in 20 years. Only 21 patients with IGC were included in this study. The distribution of the tumor stages was as follows: in situ cancer (n=1), T1 (n=2), T2 (n=7), and T3 (n=11). Tumor subtypes were identified as adenocarcinoma (n=16), neuroendocrine tumor (n=2), mucinous carcinoma (n=2), and adenosquamous carcinoma (n=1). Advanced ‘‘T stage’’, conversion of laparoscopic cholecystectomy to open cholecystectomy (OCC), positive surgical margins, positive lymphovascular invasion and increased levels of pre-operative alkaline phosphatase (ALP) were found to be associated with poor survival.Conclusion:In case of OCC or high preoperative ALP activity in cholelithiasis, IGC should be kept in mind. Pathology report on IGC should give information on all histopathological prognostic features in order to avoid loss of time associated with re-examination of specimens due to absence of sufficient information in the initial pathology report and enable the surgical team to perform re-operation for T1b tumors or more advanced IGC in a timely fashion.

Aim: Anal Bowen’s disease is a rare in-situ squamous cell carcinoma of the anus. Symptoms such as pruritus, burning, and pain in the perianal region are the first complaints of patients at admission. The disease should be differentiated from many dermatological diseases such as psoriasis, Paget’s disease, eczema, seborrheic dermatitis, and lichen planus. Factors such as human papilloma virus, arsenic, radiation exposure, and immunosuppression have been idefined in the etiology. In this study, we retrospectively reviewed five patients who were diagnosed with and treated for anal Bowen’s disease in our clinic. Method: We retrospectively analyzed parameters such as age, gender, symptoms, lesion size, surgical treatment technique, comorbidity, and predisposing factors of five patients who were diagnosed with anal Bowen’s disease and treated surgically in our clinic between 2000 and 2017. Results: The mean age of the cases was 64 years. Three were female and two were male. The most common symptom was anal itching. One of the patients refused treatment, while two underwent excision + primary suture. One patient underwent excision + Limberg flap, while excision + V-Y advancement flap were performed in the other case. Conclusion: Anal Bowen’s disease is a rare in-situ squamous cell carcinoma that is diagnosed by lesion biopsy. Wide surgical excision is a safe and definitive treatment. Primary repair or skin flaps can be used for reconstruction.
