As mangrove plants’ medicinal potentials are less explored, this study sought to qualitatively and quantitatively verify the bioactive components of Shirakiopsis indica fruits methanolic extract (SIF-ME) at the side of its analgesic, anti-inflammatory and anti-oxidant effects followed by in-silico studies.Methods: The in-vivo assessments of analgesic activity involved the hot plate test, acetic acid-induced writhing test, and formalin-induced licking test. The anti-inflammatory efficacy was assessed through the human RBC membrane stabilization assay (HRBC), protein denaturation assay, and xylene-induced ear edema methods. Antioxidant potential was implemented by the DPPH scavenging method.Results: The SIF-ME consistently displayed significant anti-nociceptive activity in a dose-dependent pattern (p < 0.05). The maximum analgesic activity was found in the highest dose (200 mg/kg; p < 0.001) in a hot plate, acetic acid-induced writhing test 43.47%, and in formalin-induced licking test in both early phase (43.3%; p < 0.01) and late phase (61.84%; p < 0.001%). The extract provided optimal protection against hemolysis (83.41% decrease) at 1000 μg/mL and significantly inhibited protein denaturation (67.34– 26.05%) at doses of 1000– 62.5 μg/mL. At 200 mg/kg, the extract showed dose-dependent and substantial inhibition (54.07%; p < 0.01) of xylene-induced ear edema. The in-vitro DPPH (IC50 = 469.5 μg/mL) results showed remarkable scavenging activity and concentration-dependent reducing power. The extract demonstrates no acute oral toxicity, as indicated by an LD50 value exceeding 1000 mg/kg body weight. Gas Chromatography-Mass Spectrometry/Mass Spectrometry (GC-MS/MS) analysis was performed which yielded sixty bioactive compounds. In-silico and molecular docking studies revealed favorable pharmacological properties, including good binding affinities and ADME/T profiles.Conclusion: These results support the medicinal use of the plant, which makes it a potential source of analgesic, anti-inflammatory, and antioxidant candidates.Keywords: natural products, medicinal plants, shirakiopsis indica, acute oral toxicity, analgesic, anti-inflammatory, antioxidant, GC-MS/MS, molecular docking

In this study, CVD risk was calculated among individuals with newly diagnosed MetS using the Framingham Risk Score (FRS) and Globorisk Score. The FRS and Globorisk score are particularly relevant in predicting CVD risk as these scores include key MetS-related risk factors like blood pressure, cholesterol levels, and age.Patients and Methods: A community-based cross-sectional study was conducted at various sites in Karachi, Pakistan, from February 2022 to August 2022. Newly diagnosed cases of MetS with no physical disability, known illness, and not taking any regular medication were recruited. MetS was defined based on the definition of International Diabetes Federation. The major outcome was 10-year risk for CVD using the FRS and Globorisk Score.Results: Of 304 patients, 59.2% were classified as low risk according to FRS, while 20.4% were classified as moderate and high risk each. Using the Globorisk score, 44.6% of 224 patients were classified as low risk, 34.4% as moderate risk, and 21.0% as high risk. A moderate positive correlation was observed between the two CVD risk scores (r = 0.651, 95% CI 0.58– 0.71). Both risk scores have reported age, gender, and current smokers as significant risk factors in predicting CVD in 10-years (P < 0.05).Conclusion: The outcome of both CVD risk scores predicted moderate-to-high risk of CVD in 10-years in almost half of the newly diagnosed patients with MetS. A convenient sample of patients was selected, and the study sample size was calculated using the item response ratio. Factorial, known group, and construct validities were determined. Internal consistency and reliability were also determined.Results: Responses were provided by 500 patients. The average medication adherence score was 30 (median 31). The normed fit index (NFI) was 0.950, the comparative fit index (CFI) was 0.963, the incremental fit index (IFI) was 0.963, and the root-mean-square error of approximation (RMSEA) was 0.071. The results from these fit indices indicated a good model. Factorial, known group and construct validities were all established. A significant association was found between adherence score and age (P = 0.03) since a larger proportion of older patients were found to have high adherence compared to patients in other age groups. The reliability (α) of the questionnaire was 0.834.Conclusion: The Arabic version of GMAS was validated in Sudanese patients with T2DM making it a suitable scale to be used in this population.Keywords: medication adherence, patient compliance, medication persistence, chronic illness, Sudan

A total of 315 participants completed the self-administered and validated questionnaire during the period from August 2018 to March 2019.Results: There was poor perception and knowledge of pharmacovigilance and ADRs reporting among pharmacists as well as intern pharmacists. However, pharmacists had better knowledge score compared to interns (P=0.043). Most of the respondents believed that ADRs reporting is important. The majority of both interns and pharmacists stated that they did not receive adequate education about pharmacovigilance during their undergraduate or internship program.Conclusion: There is a gap in knowledge and perception about pharmacovigilance among practicing pharmacists and new pharmacy graduates. 5. A rare case of sarcomatoid carcinoma of the pancreas associated with pancreatolithiasis

Abstract: Pancreatolithiasis is a risk factor for developing pancreatic cancer. We report here a rare case of sarcomatoid carcinoma of the pancreas in a 55-year old diabetic male associated with pancreatolithiasis. CT scan of abdomen revealed a large operable mass occupying the distal body and tail of the pancreas. Per-operative survey revealed a small metastatic nodule in the surface of hepatic segment IVa. Histopathology of the distal pancreatic lesion revealed sarcomatoid carcinoma. Hepatic nodule was a metastatic adenocarcinoma. Distal pancreatectomy and splenectomy was done en-mass, along with non-anatomical resection of the hepatic metastatic nodule. It has gelling properties when it interacts with counterions such as sulfates or polyphosphates and when it crosslinks with glutaraldehyde. This characteristic facilitates its usefulness in the coating or entrapment of biochemicals, drugs, antigenic molecules as a vaccine candidate, and microorganisms. Therefore, chitosan together with the advance of nanotechnology can be effectively applied as a carrier system for vaccine delivery. In fact, chitosan microspheres have been studied as a promising carrier system for mucosal vaccination, especially via the oral and nasal route to induce enhanced immune responses. Moreover, the thiolated form of chitosan is of considerable interest due to its improved mucoadhesivity, permeability, stability, and controlled/extended release profile. This review describes the various methods used to design and synthesize chitosan microspheres and recent updates on their potential applications for oral and nasal delivery of vaccines. 7. Metal additive manufacturing of damage-controlled elements for structural protection of steel members

Abstract: This paper develops hybrid steel members by integrating additively manufactured, ultra-lightweight, damage-controlled elements (DCEs) into hot-rolled structural steel members. This approach relies on segmenting a structural member into distinct sections; one or two segments are enlarged to be capacity protected; however, another end or middle DCE segment is optimized to emulate the conventional member’s strength and stiffness. A small-scale DCE was topologically optimized and then additively manufactured using a powder bed fusion technique through a direct metal laser sintering process of 17-4PH stainless steel and then was experimentally tested to study the buckling behavior under compression. The experimental testing of the optimized DCE shows a compressive strength of 81,000 times the specimen’s weight with stable post-peak buckling behavior. Numerical simulation confirms experimental results, showing a good correlation in fracture energy. A parametric study on four DCE specimens, scaled up by three, four, five, and six times, was performed and compared to hollow structural sections (HSS) of A500 Gr. C in tensile and compression strengths. The numerical simulation shows a linear relation between the weight ratio and HSS length. Additionally, numerical simulation of conventional member, DCE (scaled by three), and three hybrid members revealed that failure occurred in DCE as intended.

Abstract: This research discusses one of the most important crimes against humanity, where national and international statistics and reports revealed that the bad living conditions or bad troubled situations have forced thousands of women, children and men in their countries to be susceptible to the trafficking and exploitation. The study has been divided into two main sections. The first section presents concept of the trafficking in persons in international charters and conventions, reasons, seriousness and elements of the crime in general. The second section studies the Saudi lawmaker's addressing the crime in terms of definition, forms, prescribed penalty and reasons for augmentation of penalty. The study has concluded to a set of results and recommendations, the most important of which is that the Saudi lawmaker adopted position of the international conventions in defining crime and determining its forms. Moreover, the Kingdom has also made great strides in combating this type of crime at both international and local levels. The study recommended the need to impose harsh penalties on employers, who practice patterns of forced slavery to their workers, as well as the need to intensify efforts in order to take adequate measures to prevent smuggling the illegal labor.

9. Exploring sustainable urban mobility in Africa-and-MENA universities towards intersectional future research

Abstract: As university campuses in Africa and the MENA region represent key traffic hubs in their cities, this paper develops a Systematic Literature Review (SLR) to conceptualize a “framework of action” for intersectional sustainable urban mobility. The SLR includes the screening of 258 articles with filtering strategies resulting in 43 articles, following the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) and Bibliometrics analysis, Systematics, Meta-analysis, and Storytelling techniques (BSMS). Six thematic areas are identified, travel behavior, policy and planning, environment and energy, gendered mobility, health and pandemics, and access for persons with disabilities, in which the most recurrent theme is travel behavior, and research method is survey questionnaires. The study explores the intersectional possibilities of future research-themes with an overview of issues, indicators and determinants. Themes are categorized geographically, methodologically and chronologically from 2007 to 2023. The proposed “framework of action” promotes intersectional research encouraging universities to lead sustainable mobility initiatives from a microscale approach, impacting city-wide sustainability efforts. This framework helps Africa-and-MENA universities to act as focal points for decision-making towards sustainable urban mobility in the city, benefitting urban planners, municipalities and inter-governmental collaboration by providing targeted interventions within-to-and-around campuses.

10. Development of a new design methodology for slab track systems

Abstract: Abstract Owing to the rapid increase in the demands of train speed and axle loads, the slab track has been introduced to replace the ballast in the classical ballasted track with reinforced concrete slab or asphalt-bearing layer to improve the track stability, strength, and durability. This paper aims to develop a new methodology for estimating the rail deformations for the most common slab track systems (BÖGL, Shinkansen, and RHEDA 2000. This methodology yielded the first design aid for slab track systems based on design equations and graphs for high-speed systems. Using a regression analysis of more than 300 finite element models which are validated by experimental tests, the relationship between the rail deflection, modulus of elasticity for subgrade and replacement, and the replacement thickness was determined for the most common slab tracks under the American (AREMA) and European (EN) loads. According to EN, it was found that the minimum modulus of elasticity for subgrade to fulfill the rail deflection criterion without a replacement soil ranges from 128 to 143 MPa for the most common slab track systems; meanwhile, for AREMA, it ranges from 59 to 70 MPa. Furthermore, for these slab track systems, one simple design chart was introduced to aid engineers with the design of the slab track replacement layer according to each design code.

Abstract: Abstract Slab track is a recent technology used to cope up with the train high axle loads and speed, it has replaced the ballast material in classical ballasted track with either reinforced concrete slab or asphalt layer in order to increase both stability and durability of the railway lines. This paper aims to propose a new slab track design model which can be used to design/analyze any slab track systems under vertical loads using AREMA and EN specifications for high-speed systems (300 kmph). This model has been validated through experimental work held in Heriot-Watt University then applied to the most common slab track systems (BÖGL, Shinkansen, and RHEDA 2000) in the world. The standard section of RHEDA 2000 slab track has shown the best structural performance and efficiency compared with BÖGL and Shinkansen standard sections regarding the rail deflection, stresses of rails, and stress of replacement soil layer and subgrade soil. This paper has concluded the rail deflection is the most critical factor for the slab track design regarding EN specifications while the subgrade stresses is the vital criterion concerning AREMA specifications. Furthermore, EN-Specifications are found to be more conservative than AREMA specifications for the design or analysis of all the slab track types.

12. Thermal and Structural Performances of Screen Grid Insulated Concrete Forms (SGICFs) Using Experimental Testing

Abstract: The demand for sustainable building materials and systems with the emphasis on energy efficiency is on the rise. Insulating Concrete Forms (ICFs) are an example of such structural systems. Screen Grid Insulated Concrete Forms (SGICFs) are an innovative system that combines structural strength and thermal performance. ICF walls are commonly used in Western countries to provide high-level insulation and internal weather control. Accordingly, the current research conducts a comparative thermal analysis for a market-supplied ICF wall, a SGICF proposed design, and three typical brick walls used regionally in the Middle East. The heat transfer through the five walls is simulated by COMSOL Multiphysics and validated experimentally by utilizing a guarded hot box facility under the regulations of the ASTM C1363 standard. The market-supplied ICF walls showed better thermal insulation properties than the proposed SGICF walls, because of their higher thermal mass of concrete than in the SGICF walls. However, both walls had a remarkably higher insulation performance than the other three typical brick walls available in the market. The results reveal that the market-supplied ICF walls are overdesigned for use in the Middle East region, and SGICFs, with their comparative thermal transmittance, are a very good competitor in the Middle East market.

Abstract: The interest in hydrogen is rapidly expanding because of rising greenhouse gas emissions and the depletion of fossil resources. The current work focuses on employing affordable Al alloys for hydrogen production and storage to identify the most efficient alloy that performs best in each situation. In the first part of this work, hydrogen was generated from water electrolysis. The Al alloys that are being examined as electrodes in a water electrolyzer are 1050-T0, 5052-T0, 6061-T0, 6061-T6, 7075-T0, 7075-T6, and 7075-T7. The flow rate of hydrogen produced, energy consumption, and electrolyzer efficiency were measured at a constant voltage of 9 volts to identify the Al alloy that produces a greater hydrogen flow rate at higher process efficiency. The influence of the electrode surface area and water electrolysis temperature were also studied. The second part of this study examines these Al alloys’ resistance to hydrogen embrittlement for applications involving compressed hydrogen gas storage, whether they are utilized as the primary vessel in Type 1 pressure vessels or as liners in Type 2 or Type 3 pressure vessels. Al alloys underwent electrochemical charging by hydrogen and Charpy impact testing, after which a scanning electron microscope (SEM) was used to investigate the fracture surfaces of both uncharged and H-charged specimens. The structural constituents of the studied alloys were examined using X-ray diffraction analysis and were correlated to the alloys’ performance. Sensitivity analysis revealed that the water electrolysis temperature, electrode surface area, and electrode material type ranked from the highest to lowest in terms of their influence on improving the efficiency of the hydrogen production process. The 6061-T0 Al alloy demonstrated the best performance in both hydrogen production and storage applications at a reasonable material cost.

14. Compact high-efficiency energy harvesting positive and negative DC supplies voltage for battery-less CMOS receiver

Abstract: Abstract In this paper, novel compact high-efficiency multi-band rectifiers that supply positive and negative output voltages are demonstrated for energy harvesting applications. The proposed voltage doubler circuits are used as real DC voltage supplies of radio frequency mm-wave CMOS receivers. Operating multi-band rectifiers have a complicated structure that required more resonance networks to force the rectifier to work in multi-band. Novel series and parallel resonance networks are implemented to force the rectifier to operate in dual-band at frequencies of 850 and 1400 MHz. The proposed resonance network eliminates the Schottky diode impedance variation as the input power or frequency changes and supports the impedance matching and minimizes the insertion loss. A novel high-quality sine-shape micro-strip inductor that obtains a quality factor above 65 over the frequency band from 200 to 1400 MHz and inductance equal to 14 ± 2 nH is designed to improve efficiency and enhance performance at low power levels. The first suggested RF voltage doubler rectifier with series resonance feedback between the input and cathode of the diode and parallel resonance operates at two frequency bands of 850 and 1400 MHz and obtains a peak conversion efficiency of 59%, a saturated output DC voltage is 2.5 V, and the conversion efficiency is 40% at RF-input-power of − 10 dBm. This voltage doubler achieves the required DC supply parameter (1.1 V and 450 uA) for biasing the mm-wave receiver at an RF input power of 0 dBm. Otherwise, the second suggested negative voltage rectifier has a maximum simulated conversion efficiency of 65%, saturated negative DC-voltage is − 3.5 V, and the conversion efficiency is 45% at an RF input power of − 10 dBm. The negative voltage rectifier obtains DC supply parameters (− 0.5 V and no current condition used for a gate bias) at − 10 dBm input power.

15. Wall panel structure design optimization of a hexagonal satellite

Abstract: Considering that it satisfies high strength and stiffness at a low weight, the grid structure is the ideal option for meeting the requirements for developing the wall panel structure for the satellite. The most attractive grid structures for the satellite wall panel industry are isogrid and honeycomb structures. The first part of this work involves studying the mechanical and dynamic performance of five designs for the satellite wall panel made of 7075-T0 Al-alloy. These designs include two isogrid structures with different rib widths, two honeycomb structures with different cell wall thicknesses, and a solid structure for comparison. The performance of these designs was evaluated through compression, bending, and vibration testing using both finite element analysis (FEA) with the Ansys workbench and experimental testing. The FEA results are consistent with the experimental ones. The results show that the isogrid structure with a lower rib thickness of 2 mm is the best candidate for manufacturing the satellite wall panel, as this design reveals the best mechanical and dynamic performance. The second part of this work involves studying the influence of the length of the sides of the best isogrid structure in the range of 12 mm–24 mm on its mechanical and dynamic performance to achieve the lowest possible mass while maintaining the structure’s integrity. Then, a modified design of skinned wall panels was introduced and dynamically tested using FEA. Finally, a CAD model of a hexagonal satellite prototype using the best-attained design of the wall panel, i.e., the isogrid structure with a 2 mm rib width and 24 mm-long sides, was built and dynamically tested to ensure its safe design against vibration. Then, the satellite prototype was manufactured, assembled, and successfully assessed.

Abstract: Background: This study assessed the potential cost-effectiveness of high (80–100%) vs low (21–35%) fraction of inspired oxygen (FiO2) at preventing surgical site infections (SSIs) after abdominal surgery in Nigeria, India, and South Africa. Methods: Decision-analytic models were constructed using best available evidence sourced from unbundled data of an ongoing pilot trial assessing the effectiveness of high FiO2, published literature, and a cost survey in Nigeria, India, and South Africa. Effectiveness was measured as percentage of SSIs at 30 days after surgery, a healthcare perspective was adopted, and costs were reported in US dollars ($). Results: High FiO2 may be cost-effective (cheaper and effective). In Nigeria, the average cost for high FiO2 was $216 compared with $222 for low FiO2 leading to a −$6 (95% confidence interval [CI]: −$13 to −$1) difference in costs. In India, the average cost for high FiO2 was $184 compared with $195 for low FiO2 leading to a −$11 (95% CI: −$15 to −$6) difference in costs. In South Africa, the average cost for high FiO2 was $1164 compared with $1257 for low FiO2 leading to a −$93 (95% CI: −$132 to −$65) difference in costs. The high FiO2 arm had few SSIs, 7.33% compared with 8.38% for low FiO2, leading to a −1.05 (95% CI: −1.14 to −0.90) percentage point reduction in SSIs. Conclusion: High FiO2 could be cost-effective at preventing SSIs in the three countries but further data from large clinical trials are required to confirm this.

Abstract: This paper presents a dual-band split-ring structure that provides end-fire radiation characteristics for multi-input multi-output (MIMO) mm-wave 5G applications. The proposed single antenna element consists of a main split-ring resonator (SRR) directly fed using a high impedance microstrip transmission line. A lamda/4 transformer is employed to match the high impedance transmission line with a 50-ohm line. Two symmetric parasitic SRRs are used as directing elements to give end-fire radiation properties for the introduced antenna at the first resonant frequency. The parasitic SRRs are also utilized to produce the second resonant frequency by establishing a capacitive link with the main SRR. The single element antenna radiates at 24.5 and 28.5 GHz. A cross-shaped MIMO structure composed of four elements of the proposed antenna is introduced to improve the isolation of the MIMO system due to its orthogonal polarization characteristics. The proposed MIMO demonstrates acceptable values of gain and radiation efficiency, reaching 9.8 dB and 93%, respectively, at the first resonant frequency, and 8.7 dB and 95%, respectively, at the second band. MIMO performance is also studied, and the proposed MIMO system provides a very low envelope correlation coefficient of lower than $10^{-4}$ over the two operating frequency bands. A diversity gain is also calculated, it is around 10 dB over the two operating bands. The proposed high-isolation dual-band cross-shaped MIMO array is a promising candidate for millimeter-wave 5G applications.

Abstract: This paper presents a reconfigurable high efficiency, high output voltage, and multiband class-F radio frequency (RF) voltage doubler and two stages voltage doubler rectifiers for energy harvesting applications using RO4003C substrate. The proposed voltage-doubler circuits are suitable for multi-band LTE, IoT, WSN, UHF ISM 900 MHz, GSM 900, energy harvesting, and multi-standard applications. The voltage doubler circuits are implemented using the Schottky diode (HSMS2852). Also, the voltage-doubler circuits are optimized and refabricated to recompense the unwanted parasitic and achieve the required performance. The voltage doubler and two stages voltage doubler rectifiers have a measured efficiency exceeding 40% in the frequency band from 650 MHz to 900 MHz at −10dBm input power and a load resistance of 10kohm, 22kohm, and an output voltage of 1 V at −10dBm input power. The proposed class-F RF voltage doubler achieves maximum efficiency of 72%, saturated output voltage equals 3.5 V, and the efficiency equals 51% at −10dBm input power. On the other hand, the proposed class-F two stages RF voltage doubler has a peak efficiency of 54%, saturated output voltage equals 6.5 V at 4dBm input power, and the efficiency equal 38% at −10dBm input power. Finally, the PCB areas of the proposed voltage-doubler and two stages voltage-doubler equal 2.8cm2and 3.6 cm2, respectively.

Abstract: RESUMO Objetivo O objetivo deste trabalho é avaliar o perfil epidemiológico das conjuntivites atendidas no Pronto Socorro do Hospital Banco de Olhos de Porto Alegre (HBO) com destaque à conjuntivite infecciosa e compará-lo aos atendimentos após o início da pandemia pelo Coronavírus, observando como o isolamento social e as questões de higiene amplamente enfatizadas e estimuladas desde o início a pandemia influenciaram o número de conjuntivites bacterianas e virais atendidas neste serviço de referência. Métodos Estudo transversal e retrospectivo através da análise dos prontuários eletrônicos dos pacientes atendidos no serviço de emergência do HBO no período entre março e junho de 2019 e 2020. Resultados Observou-se uma redução significativa no número de atendimento no departamento de emergência após o início da pandemia. De março a junho de 2019, 21.678 atendimentos foram realizados. No mesmo período de 2020, este número foi de 14.194, uma queda de 34,5%. O índice de conjuntivite como principal causa da visita oftalmológica diminuiu significativamente, sendo a conjuntivite infecciosa aquela que teve a maior queda. Em 2019, a mesma foi responsável por 30,2% dos atendimentos e, em 2020, por 15,5%. Não foi observado mudanças em relação ao sexo mais atendido, no entanto, observou-se um aumento significante na idade do paciente. Conclusão O estudo demonstrou uma importante redução no número de atendimentos na emergência em relacionados à conjuntivite infecciosa após o início da pandemia pelo Coronavírus. Acredita-se que este fato esteja diretamente relacionado ao aumento dos cuidados de higiene, isolamento social e a restrições impostas pela pandemia.

20. Predictors of recycling behavior: a survey-based study in the city of Sharjah, United Arab Emirates

Abstract: Purpose – Sharjah is the United Arab Emirates' (UAE) third largest city and third highest waste producer (9.9% of ~26 million in 2012). A total of about ~$1bn has been invested into programs pursuing zero-landfill waste by 100% recycling. Besides infrastructure, assessing knowledge, attitudes and practices (KAP) are needed for effective waste management. The purpose of this study is to examine recycling KAP in Sharjah. Design/methodology/approach – A cross-sectional study, using pilot-tested questionnaires, was conducted among public venues during March and April 2016. A total of 400 residents (18–55 years) were selected via convenience sampling. SPSS-22 was used for analysis. Findings – Mean age [28 years (±9.4)] with 56.5% females. Knowledge level [51.8% (±18.1)]; students reported the highest (53.7%, p = 0.007, 95% CI [51.4–56.1]). 22.9% knew recycling benefits, 31.2% knew how to correctly prepare items. Attitude level 92.6% (±16.5); 51.1% and 46.8% would recycle more with a financial reward or penalty, respectively. 70.4% reported practicing recycling; 16.0% always recycle. Logistic regression model indicated knowledge (OR = 1.013, 95% CI [1.001–1.025]) and attitude (OR = 1.014, 95% CI [1.001–1.028]) as the only significant predictors of recycling practice. Originality/value – This is the first study measuring recycling KAP in the UAE. Results could be starting points for improved local waste management and subsequently reduced public health concerns.

21. Identifying Façade Orientations with Closely Similar Thermal Performance for Unifying Façade Design Features in Hot Arid Climate

Abstract: Large-scale residential projects require architects to balance energy efficiency, thermal comfort, and cost-effectiveness in designing building units distributed in different orientations. Applying a single design to all orientations may not result in comfort conditions and energy efficiency; meanwhile, creating a unique design solution for each orientation can be time-consuming and costly to implement. Despite numerous studies recommending design alternatives for particular orientations, the ranges where orientation changes do not demand a change in the façade design still need to be determined. This research aims to identify ranges for which changes in building orientation do not necessitate changes in the façade design. It involves conducting parametric energy and thermal comfort simulations in a test room and testing the findings using real-life datasets from a large-scale residential project. Glass type, window-to-wall ratio (WWR), and utilization of shading devices were among the design parameters investigated. Results show that for all-year-round occupancy projects, a common solution can be applied for orientation changes between 0° to 10° from the north, −10° from the south, and ±5° from east and west for WWR of less than 50% in hot arid climates. These ranges can increase for small WWRs and when reflective glazing or shading devices are applied. This helps streamline design and construction processes, lower costs, and save energy in large-scale residential projects.

22. The Integrated Use of Heavy-Metal Pollution Indices and the Assessment of Metallic Health Risks in the Phreatic Groundwater Aquifer—The Case of the Oued Souf Valley in Algeria

Abstract: In this research, contamination levels and the spatial pattern identification, as well as human and environmental health risk assessments of the heavy metals in the phreatic groundwater aquifer of the Oued Souf Valley were investigated for the first time. The applied methodology comprised a combination of heavy-metal pollution indices, inverse distance weighting, and human health risk assessment through water ingestion on samples collected from (14) monitoring wells. The contamination trend in the phreatic aquifer showed Al > B > Sr > Mn > Fe > Pb > Ni > Cr > Ba > Cu > Zn. Similarly, the enrichment trend was Al > B > Sr > Mn > Ni > Pb > Cr > Ba > Cu > Zn. Ecologically, most of the analyzed metals reflected a low potential ecological risk, except for two wells, S13 and S14, which represented a considerable and high ecological risk in terms of Pb. According to the applied grouping method, the samples in the first group indicated a lower risk of contamination in terms of heavy metals due to their lower concentration compared to the second group. This makes the area containing the second group’s samples more vulnerable in terms of heavy metals, which could affect urban, preurban, and even agricultural areas. All of the samples (100%) indicated the possibility of potential health risks in the case of children. While six samples showed that the non-cancer toxicity risk is considered low, the rest of the samples had high Hazard Index (HI) values, indicating the possibility of health risks occurring in the case of adults. The constructed vertical drainage system is acting as a supporter and accelerator of the pollution levels in the shallow groundwater aquifer. This is due to its contribution to the penetration of different pollutants into this aquifer system, depending on the residence time of the water, which appears to be long within the drainage system.

Abstract: Recent studies have highlighted the necessity to thoroughly evaluate medicinal plants due to their therapeutic potential. The current study delves into the phytochemical profile, antioxidant capacity, and hepatoprotective effect of Andrographis paniculata. The investigation specifically targets its effectiveness in mitigating liver dysfunction induced by carbon tetrachloride (CCl4) in Wistar albino rats, aiming to uncover its promising role as a natural remedy for liver-related ailments. A. paniculata leaf extract was screened for phytoconstituents and antioxidant and hepatoprotective effects in Wistar albino rats against CCl4-induced liver dysfunction. Phytochemical analysis revealed the presence of flavonoids, alkaloids, and phenolic compounds in all extracts. The phenolic concentration ranged from 10.23 to 19.52 mg gallic acid per gram of the sample, while the highest flavonoid concentration was found in the ethanol fraction (8.27 mg rutin equivalents per gram). The antioxidant activity varied from 10.23 to 62.23. GC-MS analysis identified several phytochemicals including octadecanoic acid, stigmasterol, phenanthrenecarboxylic acid, and others. Effects of the ethanol extract of A. paniculata were evaluated in four groups of animals. Biochemical estimations of serum glutamine oxaloacetate transaminase, serum glutamine pyruvate transaminase, and serum bilirubin were significantly higher (p < 0.05) in the CCl4-treated group. Treatment with 300 mg/kg b.w. of the ethanol extract of A. paniculata significantly (p < 0.05) decreased these serum enzymes. Lipid peroxidation levels in carbon tetrachloride-treated animals showed a substantial (p < 0.05) rise when compared to untreated animals, while the lipid peroxidation levels were considerably (p < 0.05) reduced after treatment with ethanol extract at 300 mg/kg b.w. Liver biochemical catalase activities were significantly reduced in the carbon tetrachloride-treated animals. The results of this study conclusively demonstrate that A. paniculata extracts are a rich source of phytochemicals and possess significant antioxidant, free radical scavenging, and hepatoprotective properties.

Abstract: In an epoch dominated by escalating concerns over climate change and looming energy crises, the imperative to design highly efficient catalysts that can facilitate the sequestration and transformation of carbon dioxide (CO2) into beneficial chemicals is paramount. This research presents the successful synthesis of nanofiber catalysts, incorporating monometallic nickel (Ni) and cobalt (Co) and their bimetallic blend, NiCo, via a facile electrospinning technique, with precise control over the Ni/Co molar ratios. Application of an array of advanced analytical methods, including SEM, TGA–DSC, FTIR-ATR, XRD, Raman, XRF, and ICP-MS, validated the effective integration and homogeneous distribution of active Ni/Co catalysts within the nanofibers. The catalytic performance of these mono- and bimetallic Ni/Co nanofiber catalysts was systematically examined under ambient pressure conditions for CO2 hydrogenation reactions. The bimetallic NiCo nanofiber catalysts, specifically with a Ni/Co molar ratio of 1:2, and thermally treated at 1050 °C, demonstrated a high CO selectivity (98.5%) and a marked increase in CO2 conversion rate—up to 16.7 times that of monometallic Ni nanofiber catalyst and 10.8 times that of the monometallic Co nanofiber catalyst. This significant enhancement in catalytic performance is attributed to the improved accessibility of active sites, minimized particle size, and the strong Ni–Co–C interactions within these nanofiber structures. These nanofiber catalysts offer a unique model system that illuminates the fundamental aspects of supported catalysis and accentuates its crucial role in addressing pressing environmental challenges.

Abstract: Summary: Background: Early death after cancer surgery is higher in low-income and middle-income countries (LMICs) compared with in high-income countries, yet the impact of facility characteristics on early postoperative outcomes is unknown. The aim of this study was to examine the association between hospital infrastructure, resource availability, and processes on early outcomes after cancer surgery worldwide. Methods: A multimethods analysis was performed as part of the GlobalSurg 3 study—a multicentre, international, prospective cohort study of patients who had surgery for breast, colorectal, or gastric cancer. The primary outcomes were 30-day mortality and 30-day major complication rates. Potentially beneficial hospital facilities were identified by variable selection to select those associated with 30-day mortality. Adjusted outcomes were determined using generalised estimating equations to account for patient characteristics and country-income group, with population stratification by hospital. Findings: Between April 1, 2018, and April 23, 2019, facility-level data were collected for 9685 patients across 238 hospitals in 66 countries (91 hospitals in 20 high-income countries; 57 hospitals in 19 upper-middle-income countries; and 90 hospitals in 27 low-income to lower-middle-income countries). The availability of five hospital facilities was inversely associated with mortality: ultrasound, CT scanner, critical care unit, opioid analgesia, and oncologist. 26. Prevalence, median time, and associated factors with the likelihood of initial antidepressant change: a cross-sectional study in Qatar

Abstract: Abstract Background Major Depressive Disorder (MDD) requires therapeutic interventions during the initial month after being diagnosed for better disease outcomes. International guidelines recommend a duration of 4–12 weeks for an initial antidepressant (IAD) trial at an optimized dose to get a response. If depressive symptoms persist after this duration, guidelines recommend switching, augmenting, or combining strategies as the next step. Premature discontinuation of IAD due to ineffectiveness can cause unfavorable consequences. We aimed to determine the prevalence and the patterns of strategies applied after an IAD was changed because of a suboptimal response as a primary outcome. Secondary outcomes included the median survival time on IAD before any change; and the predictors that were associated with IAD change. Methods This was a retrospective study conducted in Mental Health Services in Qatar. A dataset between January 1, 2018, and December 31, 2019, was extracted from the electronic health records. Inclusion and exclusion criteria were defined and applied. The sample size was calculated to be at least 379 patients. Descriptive statistics were reported as frequencies and percentages, in addition, to mean and standard deviation. The median time of IAD to any change strategy was calculated using survival analysis. Associated predictors were examined using several cox regression models. Results A total of 487 patients met the inclusion criteria of the study, 431 (88%) of them had an occurrence of IAD change to any strategy before end of the study. Almost half of the sample (212 (49%); 95% CI [44–53%]) had their IAD changed less than or equal to 30 days. The median time to IAD change was 43 days with 95% CI [33.2–52.7]. The factors statistically associated with higher hazard of IAD change were: younger age, un-optimization of the IAD dose before any change, and comorbid anxiety. Conclusions Because almost half of the patients in this study changed their IAD as early as within the first month, efforts to avoid treatment failure are needed to ensure patient-treatment targets are met. Our findings offered some clues to help clinicians identify the high-risk predictors of short survival and subsequent failure of IAD.

27. Using a quality improvement project to enhance the standard vaccination rate for long-term patients in mental health services in Qatar

Abstract: Background: Countries launch vaccination programs to ensure vaccination coverage as part of the global health security. Special populations including patients with severe mental illness are under average vaccination rate. We aimed to improve the percentage of long-term patients in Qatar's mental health services who received their routine vaccination by the end of 2020 from a baseline rate of 10% to 90% from all vaccination types needed. Methods: This was an interrupted time series quality improvement project using two structured Plan-Do-Study-Act cycles to test the success of the outcome and process measures to reach the desired aim. Run charts were utilized to display the monthly vaccination rates as an outcome measure and the rate of vaccination refusals by the patients as a process measure. A multidisciplinary team was assembled. Root cause analysis was performed. Prioritizations for certain types of vaccines to be given were done. Results: We identified 50 eligible patients. Throughout 12 months, we reached a final vaccination rate of 92%. Number need to treat was used to express the effect size and was calculated as 1.2. Refusals to vaccinations by patients dropped from 41% at the start of the project to only 4% by the end. Conclusion: High vaccination rates over a 12-month period can serve as an indication for the success of the intervention. Sustainability of the results can be achieved by multiple strategies. These results may be useful to hospitals considering vaccine implementation or those currently struggling with implementation barriers.

28. Preoperative topical testosterone for penile hypospadias repair `a comparative study`

Abstract: Abstract Background Although the outcome of hypospadias repair has much improved, repair of a small-sized phallus is still challenging. Whether preoperative hormonal therapy is beneficial remains controversial. In this report, we prospectively conducted a clinical trial comparing two groups of patients regarding the effect of preoperative topical testosterone application on the outcome of primary hypospadias repair. Results The study included 40 cases of primary hypospadias with a small glans. Their age ranged from 6 months to 3 years. Patients were divided into two equal groups. Group A were treated by local application of testosterone topical gel 1% for 30–40 days and stopped one month before operation, while cases in group B did not receive preoperative hormonal treatment. Genital skin biopsies were taken for histopathological examination. For group A, penile measurements were documented before and after hormonal therapy. There was a significant increase in all penile parameters after hormonal stimulation in group A. Interestingly, the ventral penile length proximal to the meatus showed more growth in response to hormonal stimulation compared to ventral penile length distal to meatus. We did not find significant difference between both groups regarding the rate of postoperative complications. Also, the cosmesis and parent satisfaction were ‘more or less’ similar. On the histopathological level, we could not detect a difference between both groups regarding micro-vessel density using CD31, which is a vascular marker indicating neovascularization supposed to play a fundamental role in the healing process. Conclusion The use of preoperative topical testosterone significantly increases the size of the hypospadiac phallus. However, this increase in size appears to be disproportionate on the ventral penile surface and has not been reflected on improving the surgical outcome. Level of evidence This is a case control study (level III evidence).

Abstract: Around the first COVID-19 epidemic in Jordan, we aimed to explore awareness of COVID-19 (knowledge and attitudes) disease, treatment options, and preventive measures among patients who were admitted to Jordan University Hospital (JUH) and planned to undergo elective surgery over a period of 3 months in 2020. This prospective cross-sectional article uses a questionnaire based survey which was provided to and was answered by patients themselves, then collected data was migrated to computer software and analyzed. The sample resulted in 292 patients. More than 95% were aware of the disease and and given age, educational levels and gender, we found no significant differences in knowledge and awareness of COVID-19 and protective and preventive measures. More than 60% of those surveyed were aware of COVID-19 serious complications and risk factors for developing them. Around 28% of patients who were admitted had significant anxiety from having COVID-19. The media appears to largely influence and spread some misconceptions about COVID-19 transmission and claimed therapies.

Abstract: Abstract Background & Objective: A two-year experiment was carried out in Karaj to investigate the effect of potassium sulfate and ammonium sulfate fertilizers on agronomic traits, seed yield, oil content and yield of camelina under drought stress conditions. Materials & Methods: The experimental design was split plot-factorial in an RCBD with three replications. Irrigation regimes as full irrigation, irrigation interruptions from silicle formation and flowering stages in the main plots and combine of potassium sulfate (0, 25, 50, and 75 kg.ha-1) and ammonium sulfate (0, 25, 50, and 75 kg.ha-1) fertilizers in the sub plots. Results: The number of silicle/plant, silicle length, number of seed/silicle, 1000-seed weight, biological yield, seed yield, harvest index, and oil content and yield were affected by a three-way interaction between irrigation regime × potassium sulfate × ammonium sulfate. When compared with full irrigation regime, the seed yield, oil content, and oil yield were decreased by 46, 8.5, and 51.2% in the irrigation interruption from the silicle formation stage, and by 51, 7.18, and 78.7% in the irrigation interruption from the flowering stage, respectively. The highest seed yield, oil content, and oil yield among the fertilizer treatments belonged to the application of 75 kg.ha-1 potassium sulfate+75 kg.ha-1 ammonium sulfate fertilizer treatment with an average of 1950 kg.ha-1, 31.8%, and 643 kg.ha-1, respectively. Conclusion: In general, camelina should be cultivated in the full irrigation conditions and treated with the potassium sulfate (75 kg.ha-1) + ammonium sulfate (75 kg.ha-1) fertilizers for achieving the maximum seed and oil yields.

Abstract: Space syntax involves a set of techniques for analyzing the spatial configurations of various spaces at building and urban scales. Religious spaces, such as prayer halls, are examples of buildings where observers experience space mostly from a single point of view. Furthermore, traditional space syntax is mainly used in the analysis of visibility and space cognition in terms of isovists and graph-based measures. The other aspects of space cognition, such as day lighting, artificial lighting, and glare, are carried in the isolation of the space syntax analysis. This paper proposes the scaling of the space syntax field for the inclusion of other parameters, such as daylighting, and integration of the associated performative measures to space syntax analysis of the mosque typology to aid in the studying of overall space cognition based on comfort and environmental parameters. We present a case study on a typical mosque layout using multi-objective optimization. The analysis presented in the paper has implications for the architectural designs of spaces with respect to glare management and daylight potential. Moreover, it is unique and builds on our previous work for the exploration of comfort, visibility, and proximity thresholds for stationary observers. Abstract: A dual-band mm-wave compact antenna is proposed in this paper. A quarter-wave half-slot resonator is used instead of a half-wave slot to achieve miniaturization of the antenna size. The antenna consists of two stacked half-slot resonators to radiate at two frequencies, f1 = 24 GHz and f2 = 28.5 GHz, with the same microstrip line feed. By using this stacked structure, a dual-band is obtained and the two resonant frequencies become independently matched with a minor effect on each other. Measurements show the proposed antenna has an impedance bandwidths of 6.3% and 15% at f1 and f2, respectively; furthermore, the antenna has good radiation characteristics. The antenna gains in addition to the radiation efficiencies at f1 and f2, are 3.5dBi, 92%, 4dBi, and 95% respectively. The equivalent circuit model for the proposed antenna is introduced to show the electrical behavior of the antenna. Finally, the antenna design is inserted in multiple-input multiple-output (MIMO) system. The proposed antenna is analyzed and optimized using ANSYS HFSS EM simulator and its equivalent circuit is performed by Agilent Advanced Design System (ADS). The simulated as well as measured results show good agreement. The designated antenna resembles good candidate for 5G wireless communication systems.

Abstract: Introduction Surgical site infection (SSI) is one of the most common healthcare-associated infections (HAIs). However, there is a lack of data available about SSI in children worldwide, especially from low-income and middle-income countries. This study aimed to estimate the incidence of SSI in children and associations between SSI and morbidity across human development settings.Methods A multicentre, international, prospective, validated cohort study of children aged under 16 years undergoing clean-contaminated, contaminated or dirty gastrointestinal surgery. Any hospital in the world providing paediatric surgery was eligible to contribute data between January and July 2016. The primary outcome was the incidence of SSI by 30 days. Relationships between explanatory variables and SSI were examined using multilevel logistic regression. Countries were stratified into high development, middle development and low development groups using the United Nations Human Development Index (HDI).Results Of 1159 children across 181 hospitals in 51 countries, 523 (45·1%) children were from high HDI, 397 (34·2%) from middle HDI and 239 (20·6%) from low HDI countries. The 30-day SSI rate was 6.3% (33/523) in high HDI, 12·8% (51/397) in middle HDI and 24·7% (59/239) in low HDI countries. SSI was associated with higher incidence of 30-day mortality, intervention, organ-space infection and other HAIs, with the highest rates seen in low HDI countries. Median length of stay in patients who had an SSI was longer (7.0 days), compared with 3.0 days in patients who did not have an SSI. Use of laparoscopy was associated with significantly lower SSI rates, even after accounting for HDI.Conclusion The odds of SSI in children is nearly four times greater in low HDI compared with high HDI countries. Policies to reduce SSI should be prioritised as part of the wider global agenda.

Abstract: Background: Acquisition of basic clinical skills by undergraduate medical students is becoming of greater concern. Clinical skills laboratories may provide a comfortable environment for training and may allow students to gain adequate performance level. The aim of this study is to evaluate students' performance and confidence levels before and after training of selected procedural skills; also to explore students' expectation towards skills laboratory training. Methods: Two questionnaires were conducted before and after training sessions in the clinical skills laboratory, school of medicine, University of Jordan, Amman, Jordan. The skills selected for this study: suture practice, venous access, arterial access, intradermal and intramuscular injection, central venous cannulation, male and female urinary catheterization, nasogastric tube placement and rectal examination. Although fifty-seven 6th year medical students filled the first questionnaire at the beginning before training, only 29 students could attend all training sessions, and fill the second questionnaire. 35. Drought Resistance Loci in Recombinant Lines of Iranian Oryza sativa L. in Germination Stage

Abstract: In order to locate control genes related to Oryza sativa L. traits at the germination stage under normal conditions and at drought stress levels (−4.5 and −9.0 bar), we evaluated 120 F8 generation offspring from the cross between two cultivars Neda × Ahlemitarum in a factorial experiment in a completely randomized block design with three replications in 2013 in the botanical laboratory of Gonbad Kavous University. A linkage map was prepared using 90 Simple Sequence Repeats (SSR) markers and 28 Inter Simple Sequence Repeats (ISSR), and 6 iPBS and 9 IRAP markers (265 polymorphic alleles). The results of the analysis of variance showed that all of the evaluated traits had a significant difference at the probability level of 1%. Hence, it can be noted that the desired genetic diversity can be found between genotypes. The results of the stepwise regression analysis for the germination percentage as a dependent variable and other traits as independent variables in the studied treatments showed that under normal conditions, there was variable coleoptile length, but under drought stress of −4.5 and −9.0 bar, the variable plumule dry weight entered the model. In this study, the markers included in RM1-RM490 and ISSR2-3-RM133 of chromosomes 1 and 6 of Oryza sativa were identified as the main regulators of traits associated with Oryza sativa drought resistance. In particular, they present the quantitative trait loci (QTL) that control the first stages of germination of Oryza sativa in water stress conditions.

36. Effect of D-Panthenol on Corneal Epithelial Healing after Surface Laser Ablation

Abstract: Purpose. To study the effect of D-panthenol (provitamin B5) on corneal epithelial healing, in cases of surface laser ablation. Patients and Methods: 45 eyes, of 45 patients undergoing laser surface ablation, received D-panthenol 2% in one eye and artificial tear drops of similar composition not containing D-panthenol in the other eye, postoperatively, for 2 months. Patients were examined daily for 3 days after the procedure. They were then examined weekly for 1 month. An additional examination was made after 2 months. Visual acuity (Log MAR) was assessed at every visit. Rate of healing (% of covered area) and subjective sensation of discomfort (scale 0–5) were assessed in the 1st 3 visits. Residual manifest cylinder (D) (as a parameter of corneal irregularity) and corneal clarity (epithelial and stromal haze) were assessed from week 1 to month 2. Results. During the first 3 days, both groups showed statistically nonsignificant (P>0.05) results. From week 1 to month 2, eyes receiving D-panthenol showed better vision and less residual cylinder (P0.05) difference. Still, eyes receiving D-panthenol showed better values at the majority of the parameters tested. Conclusion. D-Panthenol effect on corneal epithelial regeneration is of minimal clinical relevance. A different dosage and a larger sample of patients might reveal a statistical relevance. This trial is registered with https://doi.org/10.1186/ISRCTN81441126.

37. Quality of Vision after Deep Anterior Lamellar Keratoplasty (Fluid Dissection) Compared to Penetrating Keratoplasty for the Treatment of Keratoconus

Abstract: Purpose. To compare the visual quality of patients with keratoconus who underwent penetrating keratoplasty (PKP) or deep anterior lamellar keratoplasty (DALK) with fluid dissection. Design. Cross-sectional, observational study. Methods. Twelve eyes that underwent PKP (PKP group) were compared to 24 eyes that underwent DALK (DALK group) after complete removal of sutures and stability of refraction. Visual, refractive, corneal topographic, corneal aberrometry, and ocular aberrometry parameters were compared for both groups. The χ2 and Mann–Whitney U tests were used for comparisons as appropriate. P0.05, all comparisons). All aberrations, point spread functions (PSF), and the modulation transfer function (MTF) were not statistically different between groups (P>0.05). Conclusion. For our small study, the postoperative PKP and DALK with fluid dissection patient groups had vision/optical quality parameters that were not statistically different. This may indicate that DALK with fluid dissection can replace PKP for keratoconus without compromising vision quality.

38. Polymer Concrete for Bridge Deck Closure Joints in Accelerated Bridge Construction

Abstract: Prefabricated concrete bridge deck panels are utilized in Accelerated Bridge Construction (ABC) to simplify bridge deck construction. Concrete with good bond and shear strength as well as excellent flowability is required to fill bridge deck closure joints. This paper discusses the use of polymer concrete (PC) for bridge deck closure joints in ABC. PC produced using poly methyl methacrylate and standard aggregate was tested. Test results of PC are compared to Ultra-High Performance Concrete (UHPC). Development length, lap splice length and shear strength of unreinforced PC were tested. It is shown that PC has a development length of 3.6 to 4.1 times the reinforcing bar diameter that is close to one-half the development length of 6 to 8 times the bar diameter required with UHPC. PC also showed a shorter splice length compared with that reported for UHPC. Finally, unreinforced PC showed shear strength that is twice that of UHPC. It is evident that using PC in bridge deck closure joints in ABC can improve constructability and provide cost-savings and eliminate reinforcing bar congestion.

39. Durable Bridge Columns using Stay-In-Place UHPC Shells for Accelerated Bridge Construction

Abstract: Ultra-high performance concrete (UHPC) is a durable material that allows the construction of innovative structural elements and conforms with accelerated bridge construction (ABC) goals. The main idea of this research is to utilize UHPC to prefabricate a shell that acts as a stay-in-place form for bridge columns. The prefabricated shell eliminates the conventional formwork while reducing the on-site construction time and acting as a durable protective layer for the normal concrete inside the shell against environmental attacks. In addition, the UHPC shell provides additional confinement to the column concrete, which improves the column’s structural performance. During construction and after completing the column reinforcement work onsite, based on the conventional construction methods, the prefabricated UHPC shell is placed around the column reinforcement, followed by casting a portion of UHPC for a column-to-footing connection, which improves the capacity of the connection and shifts the plastic hinge zone above the connection. Once the UHPC portion hardens, normal concrete is placed inside the shell, forming a permanent concrete-filled UHPC shell. The construction process is finalized by placing and connecting a prefabricated cap beam to the column through the same developed connection as that in this research. This technical note presents the development of two test specimens using an UHPC shell in lieu of a conventional formwork with the advantage of improving the column performance and durability.

40. Association of age at menarche with metabolic syndrome and its components in rural Bangladeshi women

Abstract: Abstract Background Early age at menarche is associated with increased risk of metabolic syndrome in both China and the West. However, little is known about the impact of age at menarche and metabolic syndrome in South Asian women, including those from low-income country, where age at menarche is also falling. The aim of the present study was to investigate whether age at menarche is inversely associated with metabolic syndrome in Bangladeshi women, who are mostly poor and have limited access to and or poor health care facilities. Methods This community-based cross-sectional study was performed using 1423 women aged between 15–75 years from rural Bangladesh in 2009 and 2010. Metabolic syndrome was defined according to standard NCEP-ATP III criteria. Logistic regression was used to estimate the association between age at menarche and metabolic syndrome, with adjustment of potential confounding variables, including age, education, marital status, tobacco users, use of contraceptives and number of pregnancies. Results Early onset of menarche (13 years) was found to be associated with a higher prevalence of metabolic syndrome (odds ratio=1.55; 95 % confidence interval =1.05-2.30). Age at onset of menarche was also inversely associated with prevalence of high triglycerides (P for trend P for trend = 0.01), but positively associated with prevalence of high fasting blood glucose (P for trend =0.02). However, no significant association was found between age at menarche, high blood pressure and elevated waist circumference. Conclusion Early onset of menarche might promote or trigger development of metabolic syndrome. Thus, knowledge of the history of age at onset of menarche may be critical in identifying women at risk of developing metabolic syndrome and those likely to benefit the most from early interventions.

41. Spatial differences in prey preference by tigers across the Bangladesh Sundarbans reveal a need for customised strategies to protect prey populations

Abstract: The Sundarbans is the only mangrove habitat in the world to support tigers Panthera tigris, whose persistence there is believed to be dependent on a very limited number of prey species. Conservation managers therefore need to understand how tigers utilise available prey species on a spatial scale in order to formulate a prey-based protection strategy for this global-priority tiger landscape. A total of 512 scat samples were collected during a survey of 1984 km2 of forest across 4 sample blocks in the 6017 km2 of the Bangladesh Sundarbans. Analysis of scat composition and prey remains reliably identified 5 major prey species, of which spotted deer Axis axis and wild pig Sus scrofa contributed a cumulative biomass of 89% to tiger diet. Tiger preference for prey species was highly skewed towards spotted deer and wild pig, but the relative contribution of these 2 species differed significantly across the 4 study areas, which spanned the Sundarbans, demonstrating important spatial patterns of tiger prey preference across the Sundarbans landscape. Given the comparatively limited number of prey species available to support the dwindling tiger population, different strategies are needed in different parts of the Sundarbans to support tiger populations and to protect spotted deer and wild pig populations from unabated poaching.
