Abstract The aim of this study was to evaluate whether polymorphisms in SOD2 and SOD3 genes modulate the oral health-related quality of life (OHRQoL) of Para athletes with dental caries experience. The cross-sectional study included 264 Para athletes (143 in athletics, 61 in weightlifting and 60 in swimming). A trained and calibrated team recorded the decayed, missing and filled teeth index (DMFT). The Brazilian version of the Oral Health Impact Profile (OHIP-14) was used to measure OHRQoL. Genomic DNA was extracted from the athletes’ saliva, and genetic polymorphisms in the SOD2 (rs5746136 and rs10370) and SOD3 (rs2855262 and rs13306703) genes were analyzed by real-time polymerase chain reaction. Univariate and multivariate analyses were performed. A multivariate General Linear Model analysis, adjusted for sex, revealed that the SOD3 gene polymorphism (rs2855262) had a significant effect on the psychological disability domain [codominant (p = 0.045) and recessive (p=0.038) models]. The SOD2 gene polymorphism (rs5746136) had a significant effect on the total OHIP-14 score [dominant model (p = 0.038)] and the psychological discomfort [dominant model (p = 0.034)] and physical disability [codominant model (p=0.037)] domains. Presence of the SOD2 rs10370 polymorphism led to statistical differences in the total score [codominant (p = 0.026) and dominant (p = 0.023) models] and the handicap domain scores [codominant (p = 0.027) and dominant (p = 0.032) models]. Polymorphisms of the SOD2 and SOD3 genes may be important biomarkers of OHRQoL in Para athletes with dental caries experience.

Abstract Background Although a trend towards dental caries reduction has been observed, further investigations on its pattern and development are still needed. Objective To investigate the influence of clinical, social and environmental variables on dental caries prevalence in adolescents from a Brazilian municipality supplied with fluoridated water in 2018. Method This study was conducted with a non-probabilistic sample of 277 adolescents aged 12 years of public schools in Nova Friburgo, state of Rio de Janeiro, Brazil. Clinical examinations were carried out by calibrated examiners to assess caries, periodontal disease, and fluorosis according to criteria of the World Health Organization (WHO). Semi-structured questionnaires were applied to record socio-environmental information. Results Individuals living in regions supplied with fluoridated water below the minimum required levels (OR=3.60, p=0.0006), with low income (OR=1.90, p=0.0444), presence of gingivitis (OR=3.36; p=0.0016), and whose reason for visiting the dentist was dental treatment (OR=2.41; p=0.0203) were more likely to have dental caries. Objective: To evaluate the association of oral health indicators and social variables on the frequency of visit to the dentist by persons with intellectual disability (ID). Material and Methods: The study comprised a sample consisting of 149 participants with ID, aged from 11 to 29 years, from non-governmental institutions. Semi-structured interviews were held to collect sociodemographic and psychosocial information with their parents/guardians, followed by oral health evaluations in accordance with World Health Organization criteria. Crude analyses and multiple analysis were conducted to test whether oral health indicators and social data were predictors of the visit to dental care services. Results: In the multiple logistic regression model, individuals with lower DMF-t (OR=3.13; 95% CI=1.40-6.97) and those with less crowded housing (OR=2.33; 95% CI=1.06-5.12) presented less frequency of visits to oral health services. Conclusion: DMFT and crowded housing are associated to the frequency of persons with intellectual disability to dental care as well as this outcome measure affects the oral health of persons with ID. Therefore, identifying limiting factors to dental care of persons with intellectual disability is needed so that this group can receive adequate attention.

Abstract The aim of this study was to evaluate the influence of the Brazilian School Health Program (SHP) on dental clinical disorders such as caries, gingivitis, and dental trauma as well as on the oral-health-related quality of life (OHRQoL) of adolescents in Nova Friburgo, RJ, Brazil. The study consisted of a non-probabilistic (convenience) sample comprising 319 12-year-old adolescents, both participants and non-participants of the SHP, for at least two years. Socio-demographic and OHRQoL questionnaires (CPQ11-14, the Child Perceptions Questionnaire) were applied as well as clinical examinations for caries, periodontal disease, and dental trauma following the World Health Organization (WHO) criteria by calibrated researchers. Mann-Whitney, chi-square tests, and multiple logistic regression models were used for the data analysis. Statistically significant differences were observed between the groups covered and not covered by the SHP for gingivitis (p = 0.0373) and OHRQoL on the social welfare subscale (p = 0.0265) and total scale (p = 0.0449) of CPQ11-14. Multivariate analysis showed that female adolescents were 1.74 times more likely to present a lower OHRQoL (OR = 1.74, 95%IC = 1.10–2.77, p = 0.0183) than males. In addition, non-SHP subjects were 1.56 times more likely to have poor OHRQoL (OR = 1.56, 95%IC = 0.94–2.59, p = 0.0873) than program participants. In conclusion, SHP positively influenced the OHRQoL of adolescents. However, follow-up studies are needed to verify the continued effectiveness of this Brazilian SHP in improving the oral health levels and quality of life of adolescents.

Objective: to evaluate the fluoridation scenario in a city of the State of Rio de Janeiro, since its implantation to the present day. Material and Methods: water samples were collected from 2011 to 2017, in 21 locations of the city, analyzed by the electrometric method and classified based on the recommended national fluoride concentrations. Results: from a total of 2,297 samples collected, only 688 (29.95%) were within the recommended range (0.65-0.94 ppmF), with concentrations ranging from 0.00 to 1.92 ppmF and with an average concentration of 0.41 ppmF. During the period of evaluation, it was observed that the locations that were closest to the central region of the city were constantly fluoridated, and most of the samples were in accordance with the parameters recommended by CECOL-USP. On the other hand, in the peripheral neighborhoods, the samples were considered underfluoridated, which might result in inefficacy in the prevention of caries disease. Conclusion: the city showed a clear deficiency in maintaining of fluoride levels at adequate concentrations, which reinforces the relevance of controlling of this measure as part of a continuous surveillance action in oral health care.

Aim: To identify the prevalence of dental caries in inland Brazilian adolescents, and to analyze the influence of socio-demographic and clinical variables, and access to dental service on caries experience. Methods: This study had a non-probabilistic sample comprising 504 adolescents aged 12 years, attending public schools in the city of Nova Friburgo, mountain region of Rio de Janeiro, Brazil. The World Health Organization (WHO, 1997) criterion was used by previously calibrated examiners to report dental caries. Variables were obtained by means of a semistructured questionnaire applied to the adolescents’ parents. Results: The D3 MFT mean was 1.90 and the Significant Caries (SIC) Index was 4.54, mainly represented by the carious component. Multiple logistic regression analysis revealed that parents with over 8 years of schooling (OR=0.579), absence of pain (OR=0.396) and not visiting the dentist (OR=0.270) might suggest protective factors against the disease; the possible risk factors were male gender (OR=1.982) and pain, extraction and others were reasons for consultation (OR=2.435). Conclusions: Although the prevalence of caries was slightly below the national mean, polarization of the disease was clearly observed. Education and no access to the dentist led to protection against the disease. These results may contribute to planning of oral health actions directed towards this target population for the control of caries.

Introdução/Objetivo: A cavidade bucal possui um microbiota que, quando em simbiose com o hospedeiro, atua como uma proteção natural contra a colonização de microrganismos patogênicos. Entretanto, esta pode ser alterada por fatores externos, como higiene bucal, fluxo salivar e antibioticoterapia. No momento da admissão na UTI, alguns indivíduos podem já estar colonizados por microrganismos que não são comuns à cavidade oral, como Staphylococcus aureus, Pseudomonas aeruginosa, Klebsiella pneumoniae e Acinetobacter Baumannii, por vezes, Multidroga-Resistente (MDR), o que pode aumentar a morbidade e mortalidade de indivíduos sob terapia intensiva. O objetivo do presente estudo foi pesquisar a presença bactérias multidrogas-resistentes relacionadas com infecções respiratórias, como S. aureus, P. aeruginosa, K. pneumoniae e A. Baumannii, no biofilme oral em indivíduos, no momento da admissão na UTI e determinar o perfil de suscetibilidade aos antimicrobianos. Métodos: Amostras de biofilme oral de 88 participantes foram coletadas com auxílio de swab no dia de sua internação na UTI de um hospital de Nova Friburgo-RJ. Bactérias foram isoladas e identificadas por espectrometria de massas por MALDI-TOF, seguido do teste de sensibilidade aos antimicrobianos. Resultados: Foram coletadas amostras de 48 (57,8%) pacientes do sexo masculino e 35 do sexo feminino (42,2%) com Média (M) de idade = 64.486 (+15,60); índice de cárie CPOD, M = 25,725 (DP=9,24); número de dentes M = 10,925 (DP=12,37). Foram isoladas 15 (44,11%), cepas Multidrogas Resistentes (MDR) de um total de 34 cepas, distribuídas da seguinte forma: K. pneumoniae (2 cepas MDR); A. baumannii (6 cepas MDR); S. aureus (7 cepas MDR). Conclusões: Foi observada a presença bactérias MDR no biofilme oral de pacientes anteriormente à internação no UTI. Não foram identificadas correlações entre o perfil de susceptibilidade e condições bucais (biofilme bucal e infecções prévias) e gerais (idade, sexo, presença de infecção, comorbidades, uso de antibiótico, motivo da internação).

Objective: to access and identify barriers to oral health care of people with intellectual disabilities (ID), from the caregivers’ perspective. Material and Methods: the random sample was composed of 55 caregivers of people with ID and an interview script with a qualitative approach was applied to deepen the understanding of this group about oral health services accessibility. The qualitative analysis was performed through the Discourse of the Collective Subject Method. Results: we observed some barriers to access and care to oral health to patients with disabilities, such as: lack of structure and organization of the service, deficient training of professionals for attendance, among others. In addition, the health care within a practice little paid in and weakly based on health promotion were other factors that compromised the access of people with ID. Conclusion: the access evaluation is extremely important, because it allows the reordering of actions and services according to the users’ needs. However, despite the advances, the patient with ID still has been facing barriers, basically to the accessibility to treatment performed by professionals, to reception and low management.
