Abstract Sore throat represents one of the main causes of antibiotic overprescription in children. Its management is still a matter of debate, with countries considering streptococcal pharyngotonsillitis a benign and self-limiting condition and others advocating for its antibiotic treatment to prevent suppurative complications and acute rheumatic fever. Italian paediatricians frequently prescribe antibiotics on a clinical basis regardless of microbiological results. Moreover, broad-spectrum antibiotics are inappropriately prescribed for this condition. In this regard, an intersociety consensus conference was issued to promote the judicious use of antibiotic therapy in paediatric outpatient settings. A systematic review of the literature was performed, and updated recommendations were developed according to the GRADE methodology. Antibiotic treatment with amoxicillin (50 mg/kg/day) for 10 days is recommended in all children with proven streptococcal pharyngitis. Benzathine-penicillin could be prescribed in children with impaired intestinal absorption or inability to tolerate enteral intake and in those at high risk of suppurative complications with low compliance to oral therapy. In children with suspected amoxicillin allergy, third-generation cefalosporins for five days are recommended in low-risk patients, and macrolides are recommended in high-risk ones. Candidates for tonsillectomy due to recurrent pharyngitis could be treated with amoxicillin-clavulanic acid, clindamycin, or combined therapy with amoxicillin plus rifampicin for four days, in an attempt to avoid surgery.

Abstract Background Paracetamol and ibuprofen are the most commonly used drugs for pain treatment in children and their combination has shown improved analgesic effect compared to treatment with either drug alone. Current literature lacks specific guidelines regarding the settings in which this combination should be adopted. Methods The survey, conducted with Delphi methodology, involved 75 hospital and outpatient pediatricians with clinical experience in the management of pain in children. Pediatricians involved were asked to validate or not the results of the previous NominalGroup Tecnique (NGT) consensus and thus specify the optimal clinical settings in which the paracetamol/ibuprofen fixed-dose combination could be adopted. Results The results confirm the importance of the fixed-dose paracetamol and ibuprofen combination for the control of mild-to-moderate acute pain in children. Particularly, this association seems to be appropriate in case of headache, earache, odontalgia and musculoskeletal pain, and in specific settings such as post-operative and post-procedural pain. The broadening of the panel brought to slight variations in clinical management practices between hospital and outpatient specialists. Nonetheless, overall consensus supports the notion that the fixed dose combination is more efficacious than monotherapies and it is well tolerated. Moreover, experts unanimously agree on the usefulness of the combination for caregivers, leading to improved adherence and effectiveness. Conclusions Both the NGT consensus and the broader Delphi consensus confirm the usefulness of the paracetamol-ibuprofen fixed-dose combination in pediatric pain. This is attributed to its superior effectiveness compared to monotherapies, a good tolerability profile, and improved compliance and ease of use. Some pain settings related to chronic, inflammatory and rheumatological pathologies remain to be investigated to evaluate the use of this combination.

Abstract Indoor air quality is negatively affected by the emission of different combustion sources releasing airborne particles and related particle-bound toxic compounds (e.g., heavy metals and polycyclic aromatic hydrocarbons). To date, very few studies focused on the chemical characterization of the airborne particles emitted by indoor sources were carried out; moreover, no data on their size-resolved chemical compositions are available. In the present study, an experimental analysis aimed at determining the size-segregated content of heavy metals and polycyclic aromatic hydrocarbons in airborne particles (including sub-micrometric ones) emitted by widely-used indoor combustion sources (i.e., incenses, candles, mosquito-coils, and cooking activities) was carried out. To this purpose, airborne particles were collected through an electric low-pressure impactor and were post-analyzed by means of chromatography–mass spectrometry and atomic emission spectrometry techniques. Results of the analyses showed that the chemical composition of the emitted particles is not invariant to the particle size, indeed, an important contribution of sub-micrometric particle range to the total mass of chemical compounds emitted by the sources was noticed. These findings also demonstrated that significant underestimations of particle-bound compounds depositing in the lungs could occur if size-dependent compositions are not adopted.

A multispecialty trauma team must provide care for pelvic gunshot wounds (PGW) due to the high risk of associated morbidity and mortality, the high density of organs that might be wounded within the pelvis, and the potential consequences of these complicated injuries. We present a case of a 59-year-old woman hemodynamically stable with firearm injury to the left buttock. CT examination showed free air in the peritoneal cavity and in the retroperitoneum and a focal contrast extravasation within the uterine fundus. The patient underwent urgent laparotomy that revealed triple bowel perforation (sigmoid colon, medium rectum, ileum) and a laceration of the posterior and anterior uterine wall at level of the cervix with no signs of active bleeding. The bullet was lodged above the peritoneal reflection, in the right pelvis, and it was removed, and handed over to the judicial authority. The perforated bowel segments were resected with Hartmann's procedure and ileal anastomosis. The uterine laceration was repaired. Although all the viscera and the structures along the trajectory can be harmed, pelvic gunshot wounds have the potential to inflict serious injury. Nongravid uterine traumas are a unique occurrence, and proper care requires an understanding of lesion grading. Finding the gynecological lesion in female patients is essential to receiving the best care and protecting the reproductive system.

There is not a welfare protocol for horses reared for meat production in semi-extensive systems. The aims of the study were to develop a specific welfare protocol suitable to be applied at pasture and on stable; and to evaluate whether the welfare items were influenced by the farming system (pasture vs stable). 52 non-animal-based measures (N-ABMs) and 14 animal-based measures (ABMs), classified into 6 thematic areas (training, feeding, facilities, ABMs, biosecurity, health management) were selected by a focus group. The protocol was applied on a total of 429 Catria horses located across 26 pastures during the warm seasons and on 7 stables during the cold seasons. Differences obtained within each horse-unit were calculated by using the diversity index (VARNC) and the distance from the ideal (dfi) index. Abstract Pediatric asthma management is a compelling challenge for every pediatrician. Different aspects require attention and definition. The present Intersocietal Survey aimed to collect real-world experiences from a sample of Italian pediatricians. A web platform was used to collect anonymous answers to the survey questions. Four hundred four pediatricians participated in this initiative promoted by the Italian Society of Pediatric Allergy and Immunology (SIAIP), the Society of Preventive and Social Pediatrics (SIPPS), and the Federation of Italian Pediatricians (FIMP). The results showed an extensive participation of primary care pediatricians (72%). There was a large consensus about diagnostic criteria and medication choice. However, treatment duration and device choice were various. Adherence to guidelines on general aspects of practical clinical management was high. In conclusion, the present Intersocietal Survey confirmed that pediatric asthma management is rather satisfactory, even if further improvement should concern a more widespread use of ICS for acute asthma/wheezing attacks, a better definition of the duration of ICS and bronchodilator use, and hospital-primary care integration.

Study region: In this study, we compared the spatiotemporal evolution of rainfall trends in E-OBS and ERA5 to those detected using historical rainfall series recorded by ground-based networks in Southern Italy. In particular, the study is applied to the Campania, Basilicata, Apulia, Calabria and Sicily regions (84,000 km2 in total) on seasonal and annual scales. Study focus: Meteorological gridded datasets at large spatial scales are widely used in many hydroclimatic applications as they provide long and spatially homogeneous records. Regional trend analyses based on these data need to be treated with caution due to some potential limitations at relevant hydrological scales, such as the coarse spatial resolution and the spatio-temporal inhomogeneity of the underlying data. Gradual trends and abrupt change points were studied on rainfall data from 1979 to 2019. New hydrological insights for the region: Both gridded datasets capture the major trends in observed rainfall, with a predominance of positive values driven by changes in September-November. Overall, ERA5 returns flatter results compared to E-OBS, with the former comparing well with observations in Sicily and Apulia, while the latter is performing well in Campania and partially in Calabria and Basilicata. Most statistically significant trends are associated with discontinuities in the early 2000s, and this is well captured by both ground and gridded datasets. The general behavior in inter-annual variability trends in Southern Italy is captured by both datasets, with ERA5 also detecting regional patterns.

Abstract The preservation of cultural heritage, including ancient photographic materials, is of paramount importance in the field of conservation science. In this context, the microbial diversity of ‘Skull and Crossbones’, a 1983 photograph by Robert Mapplethorpe printed on silver gelatine, was assessed. We employed both culture-dependent and culture-independent methods to characterize microbial communities inhabiting this artwork. Vibrational Raman micro spectroscopy and FT-IR spectroscopy were utilized to assess the chemical degradation condition and characterize the chemical components of the silver gelatin print. The combination of molecular sequencing methods (Sanger and HTS approach) and non-invasive vibrational spectroscopy yielded valuable insights into the microbial communities thriving on photographic material and the chemical degradation of the print. Isolated fungal strains were added to the Fungal Collection at the University of Naples Federico II, and their deteriorative potential was investigated by adding substrates, commonly used in canvas photographs to the culture media. These results establish a link between microbial communities colonizing ancient photographic materials, paper decomposition, and the enzymatic patterns of the retrieved microorganisms. This information is invaluable for understanding and addressing biodeterioration progression on valuable works of art, such as historical photographs, which remain understudied.

Per- and polyfluoroalkyl substances (PFASs) are carbon–fluorine compounds with widespread industrial and domestic use, posing potential toxicological risks to humans and ecosystems. Several analytical methods have been developed to assess the occurrence of PFASs in the environment, but a standardized method, applicable to all matrices, is still lacking. This paper reviews the extraction and chromatographic detection methods for PFAS assessment in environmental samples, considering parameters such as the LOD, LOQ, and recoveries. Solid phase extraction (SPE) is commonly used, showing high recovery rates for water, soil, and sediment samples using HBL and WAX polymeric sorbents (85–100% and 93–111.5%, respectively). LC-MS has demonstrated low LODs and LOQs in seawater (0.01–0.08 ng L−1; 0.03–0.24 ng L−1), marine sediment (0.002–0.018 ng g−1; 0.004–0.054 ng g−1), and dust (0.08–0.68 pg g−1; 0.26–2.25 pg g−1), indicating its sensitivity when detecting trace PFAS levels. Evaluating PFASs is crucial for the development of future removal strategies and risk assessments. Potential solutions including the use of PFAS substitutes and innovative adsorption techniques for their adsorption could present promise in reducing their environmental presence.

Trombosis pada sindrom nefrotik dapat timbul dari kebocoran protein dengan berat molekul yang tinggi sehingga menyebabkan hiperkoagulabilitas, peningkatan sintesis faktor yang mendorong trombosis, aktivasi dan agregasi trombosit, aktivasi sistem koagulasi, atau penurunan antikoagulan endogen. Pasien sudah diterapi dengan heparin dan terapi untuk sindrom nefrotik. Pasien dipulangkan dengan perbaikan klinis dan kontrol rutin ke poliklinik.

ABSTRACTThe first hydrogeological mapping of Ischia Island at the 1:10,000 scale is presented and discussed. The ‘Map of the geohydrologic units of Ischia Island’ and the accompanying hydrostratigraphic sequence at a basin scale are based on the most recent geological maps and data from the CAR.G Project, and on new volcanological and hydrogeological surveys and studies. Data sources include the database stored by the INGV, Sezione di Napoli, Osservatorio Vesuviano and field investigations, including a survey of the springs and thermo-mineral springs which has been neglected up to now. In total 130 volcanostratigraphic units and 18 geohydrologic units were recognized; the distribution of fumaroles sites was enhanced, and 60 springs/thermal springs were identified/rediscovered. The proposed hydrogeological map provides an overview of the volcano-tectonic evolution of Ischia Island and upgrades the hydrogeological model, becoming a catalyst for the effort to acquire better data and to manage natural both resources and risks.

Summary: Background: To assess the equivalence of tranexamic acid (TRAN) versus synthetic oxytocin (OXY) in reducing post-partum blood loss, in full-term patients (37–42 weeks), at low risk of post-partum hemorrhage, with vaginal childbirth. Methods: Phase III, randomized (1:1), open-label, longitudinal, multi-center, prospective clinical trial (Prot. n 63209, ClinicalTrials.gov Identifier: NCT02775773). From January 7, 2020, to June 30, 2023, a total of 256 women were enrolled at two general urban community hospitals in Italy, serving a multi-ethnic patient population with National Health Insurance. The primary outcome was to explore a potential equivalence between the two treatments (OXY and TRAN) in preventing total blood loss. Therefore, we randomized 231 women into two groups: Group A (OXY), 127 women who were administered 10UI intramuscularly within 5 min from childbirth; Group B (TRAN), 104 women to whom 1-g slow intravenous infusion was administered within 5 min from childbirth. Findings: At the time of delivery, mean blood loss for OXY group versus TRAN group was 269.12 mL versus 263.88 mL, respectively, with equivalence between the two groups. Similarly, there was equivalence in total blood loss between the OXY and the TRAN group (397.66 mL versus 405.64 mL, respectively. No statistical differences between Hb levels at admission and discharge in the two groups were reported. No difference was found in terms of additional uterotonic and surgical therapies between the two groups of patients. Neither group showed thrombotic complications at check-up performed after 7 days or after a questionnaire regarding adverse effects, subjected after 40 days. Interpretation: The study shows the equivalence of tranexamic acid versus synthetic oxytocin in post-partum blood loss prophylaxis in term patients at low risk of PPH with vaginal childbirth. The safety profiles of OXY and TRAN were similar. Funding: None.

Abstract Excitons are realizations of a correlated many-particle wave function, specifically consisting of electrons and holes in an entangled state. Excitons occur widely in semiconductors and are dominant excitations in semiconducting organic and low-dimensional quantum materials. To efficiently harness the strong optical response and high tuneability of excitons in optoelectronics and in energy-transformation processes, access to the full wavefunction of the entangled state is critical, but has so far not been feasible. Here, we show how time-resolved photoemission momentum microscopy can be used to gain access to the entangled wavefunction and to unravel the exciton’s multiorbital electron and hole contributions. For the prototypical organic semiconductor buckminsterfullerene (C60), we exemplify the capabilities of exciton tomography and achieve unprecedented access to key properties of the entangled exciton state including localization, charge-transfer character, and ultrafast exciton formation and relaxation dynamics.

Environmental pollution and its impact on human health has become a topic of great concern. In recent years, the scientific community has significantly increased its attention towards the protection of human health and an increasing number of analytical determinations are being carried out on food and environmental matrices to guarantee their quality. Within these determinations, the monitoring of air quality, both in indoor and outdoor environments, is of particular scientific interest. In particular, the presence of micrometric particles, atmospheric particulate matter (PM) and ultrafine particulate matter (UFP) has become a marker of air quality in recent years. The study of these substances is particularly important since the diameter of the particles is inversely proportional to their ability to penetrate the respiratory system. In places of greatest attendance and areas with high vehicular traffic, units are installed for continuously monitoring the air quality. This paper aims to bring a snapshot of the concentrations of these particles in Molise, a small region in Italy. The results obtained present rather limited PM10, PM4, PM2.5, PM1 and UFP ranges, especially as regards Campobasso, the regional capital.

Introduction: Surgical and non-surgical facial procedures have gained immense popularity thanks to their aesthetic benefits. Understanding the vascularization of the face and assessing potential risks is crucial to ensure patient safety and optimal outcomes. This study aims to evaluate the utility of thermal scanning and Doppler ultrasonography in evaluating the vascular risk for facial procedures. Material and Methods: A cohort of 60 patients were included in this prospective study. Preoperative thermal scanning and Doppler ultrasonography were performed in order to visualize the facial vasculature and identify potential areas of risk. Results: For 30 patients, we performed preprocedural Doppler ultrasound imaging and thermal scanning at the injection site. Only 6.66% of them suffered vascular complications, whilst 23.33% among those who didn’t undergo ultrasound and thermoscanning suffered vascular injuries. The combined use of these tools proved to be valuable in assessing the vascular risk for facial procedures. The findings emphasize the importance of preoperative assessment to identify patients at risk for vascular complications. Conclusions: Thermal scanning and Doppler ultrasonography offer a comprehensive approach to assessing the vascular risk for facial procedures. By ensuring the identification of abnormal vascular patterns and altered blood flow dynamics, these techniques enhance patient safety and procedure planning.

Cardiovascular disease is a leading cause of death. The current approach to the prevention of arterial thrombosis in cardiovascular disease is dependent on the use of therapies which inhibit the activation of platelets. Predictably these are associated with an increased risk of haemorrhage which causes significant morbidity. The thrombotic potential of an activated platelet is modifiable; being determined before thrombopoiesis. Increased megakaryocyte ploidy is associated with larger and more active platelets carrying an increased risk of thrombosis. The reduction in the ploidy of megakaryocytes is therefore a novel area of therapeutic interest for reducing thrombosis. We propose a new therapeutic approach for the prevention and treatment of thrombosis by targeting the reduction in ploidy of megakaryocytes. We examine the role of a receptor mediated event causing megakaryocytes to increase ploidy, the potential for targeting the molecular mechanisms underpinning megakaryocyte endomitosis and the existence of two separate regulatory pathways to maintain haemostasis by altering the thrombotic potential of platelets as targets for novel therapeutic approaches producing haemostatically competent platelets which are not prothrombotic.

The "Trojan horse effect" of microplastics for organic and inorganic contaminants is an interesting topic. So far, the scientific community has focused on microplastics strictly as contaminants, but their role as vectors is still undefined. Adsorption of pollutants follows the Freundlich model by physisorption mechanism. Furthermore, ages and types of microplastics influence the adsorption of pollutants onto microplastics. Moreover, natural particles, like algae particles, present in the environment can interfere in the adsorption mechanisms. Due to their chemical composition of mainly O and N, it has been suggested that natural particles have a stronger adsorption affinity for some pollutants. Furthermore, microplastic's role as vector of pollutants into organisms is controversial. In fact, it has been suggested that the release is species-specific. In T. japonicus, accumulation of Hg loaded onto microplastics increased by 2.5 times, whereas a reduction of bioavailability of Hg was observed in R. lens.

Abstract Gas leakage from deep geologic storage formations to the Earth’s surface is one of the main hazards in geological carbon sequestration and storage. Permeable sediment covers together with natural pathways, such as faults and/or fracture systems, are the main factors controlling surface leakages. Therefore, the characterization of natural systems, where large amounts of natural gases are released, can be helpful for understanding the effects of potential gas leaks from carbon dioxide storage systems. In this framework, we propose a combined use of high-resolution geoelectrical investigations (i.e. resistivity tomography and self-potential surveys) for reconstructing shallow buried fracture networks in the caprock and detecting preferential gas migration pathways before it enters the atmosphere. Such methodologies appear to be among the most suitable for the research purposes because of the strong dependence of the electrical properties of water-bearing permeable rock, or unconsolidated materials, on many factors relevant to CO2 storage (i.e. porosity, fracturing, water saturation, etc.). The effectiveness of the suggested geoelectrical approach is tested in an area of natural gas degassing (mainly CH4) located in the active fault zone of the Bolle della Malvizza (Southern Apennines, Italy), which could represent a natural analogue of gas storage sites due to the significant thicknesses (hundreds of meters) of impermeable rock (caprock) that is generally required to prevent carbon dioxide stored at depth from rising to the surface. The obtained 3D geophysical model, validated by the good correlation with geochemical data acquired in the study area and the available geological information, provided a structural and physical characterization of the investigated subsurface volume. Moreover, the time variations of the observed geophysical parameters allowed the identification of possible migration pathways of fluids to the surface.

Mild-to-moderate ulcerative colitis (UC) management is centred on 5-aminosalicylic acid (5-ASA) derivatives. Whether supplementing 5-ASA with nutraceuticals can provide real advantages in UC-relevant outcomes is unclear. This retrospective multicentre study compared clinical remission, response rates, and faecal calprotectin levels in a two-arm design, including patients treated with 5-ASA alone and those with additional H. erinaceus-based multi-compound supplementation. In the 5-ASA alone group, clinical response rates were 41% at three months (T1) and 60.2% at six months (T2), while corresponding clinical remission rates were 16.9% and 36.1%. In the nutraceutical supplementation group, clinical response rates were 49.6% (T1) and 70.4% (T2), with clinical remission rates of 30.4% (T1) and 50.9% (T2). No significant differences in clinical response rates between the groups at T1 (p = 0.231) and T2 (p = 0.143) emerged. Clinical remission rates differed significantly at both time points (p = 0.029 and p = 0.042, respectively). Faecal calprotectin levels decreased significantly in both groups during the retrospective follow-up (p < 0.05), and this was more pronounced in nutraceutical supplementation patients at both T1 (p = 0.005) and T2 (p = 0.01). No adverse events were reported. This multi-component nutraceutical supplementation offers real-world potential in controlling disease activity in patients with mild-to-moderate UC.

The exposure of humans to polycyclic aromatic hydrocarbons (PAHs) through fruits is a scarcely investigated topic. The atmospheric deposition of PAHs could contribute to such an issue. The present paper would like to propose an easy, fast, and routinary analytical method to extract and quantify PAHs in apples, pears, and grapes. Dispersive liquid–liquid microextraction allowed us to recover PAHs ranging between 68.0 ± 1.2 and 96.2 ± 0.8% from fruit. Gas chromatography equipped with flame ionization detector analysis showed satisfactory analytical parameters, with details like R2 > 0.9912 in a concentration range of 0.5–500 µg mL−1, with a variability ranging within 0.7–2.3%. Rural fruit samples were found to be more contaminated by PAHs compared to urban samples, likely due to the use of non-green fuels in rural areas considered in this study. Further in-depth research on this topic is strongly recommended due to the relevance of fruits in the Mediterranean diet.

The rapid spread and accumulation of microplastics (MPs) in environmental ecosystems result from extensive plastic usage. MPs have been found in both indoor and outdoor air. Outdoor MP levels vary widely across global cities, with reported ranges from 36 to 118 MPs m−2 day−1. However, differing measurement units complicate comparisons. Indoor MPs are particularly concerning due to the significant amount of time people spend indoors. For instance, MP concentrations in workplaces like reception areas and nail salons were found to be 309 ± 214 and 46 ± 55 MPs m−3, respectively. Technological limitations hinder the identification of MPs, with methods like µ-ATR-FTIR, µ-FTIR, and µ-Raman identifying MPs of different sizes. MPs smaller than 0.3 µm pose a health risk as they can be internalized in lung cells, while MPs larger than 10 µm are too large to enter alveolar macrophages. This review highlights the current understanding of airborne MPs, focusing on their sources, transport, and deposition mechanisms. It aims to provide a foundation for further studies to deeply assess the presence, abundance, and occurrence of MPs in aerosols, a subject that remains underexplored.

Over the years, monitoring the quality of the environment, food, and human health has emerged as a fundamental objective of scientific investigation. Various analytical techniques have been devised and continually refined to ensure a high standard of living. Among these methods, dispersive liquid–liquid microextraction (DLLME) stands out as a widely used approach to conduct analytical evaluations and pre-concentrations of chemical compounds. This review provides an account of the evolution of DLLME over the past five years (2018–2023). The Scopus search engine was used to search for scientific articles. The selection of relevant articles was carried out according to the criteria described below. Emphasis was placed on the evolution that DLLME has undergone over the years by integrating it with other pre-concentration methods in order to amplify the concentration factor and improve analytical precision. Factors such as solvent dispersion and extraction, dispersion formation, and the recovery of the solvent containing the extracted analytes were examined. Additionally, derivatization systems were evaluated to refine both qualitative and quantitative analysis. These efforts aim to introduce methods that are safe for operators, environmentally friendly, and economical. This review evaluates the advantages and disadvantages displayed by the evolution of this technique in the phases before, during, and after its application.

The potato (Solanum tuberosum) is one of the most widespread and cultivated Solanaceae in the world. To overcome the increasing problems of intensive cultivation and climate change, finding new strategies to guarantee the needs of today and future generations has become mandatory. The use of biostimulants based on Trichoderma spp. can be an excellent alternative to reduce the use of pesticides, as well as to mitigate the effects of biotic and abiotic stresses. In this study we evaluated the effects of a new bioformulation containing two Trichoderma strains on potato growth and yield. Trichoderma strains were characterised morphologically and molecularly. Application of the new bioformulate was able to promote potato plant growth and caused a significant increase in plant fresh (+107%) and dry weight (+74%), and potato tuber fresh weight (+37%) and number (+41%), and it also improved potato yield (+36%). These findings suggest that the bioformulation is a viable alternative to reduce pesticide use and mitigate biotic and abiotic stress in potato cultivation.

The exposure of humans to microplastics through food is a topic of great interest. Foods of marine origin, such as fish and salt, have been the most extensively studied in this regard. Conversely, foods considered less likely to be contaminated (such as dairy products) have been investigated to a lesser extent. This is the first study addressing the occurrence of microplastics in cheese. In this paper, we report the first analytical approach for cheese preparation, before a chemical analysis of microplastics in cheese was performed. Therefore, the most suitable digestion methods were investigated. Alkaline digestion (i.e., KOH 5 M, 50 °C, 48 h) achieved a digestion efficiency of 97.5 ± 0.8%. To assess the feasibility of the preparative method proposed, a recovery rate of spiked polystyrene microbeads (~10 µm) of 98.5 ± 0.4% was determined. Further, the effects of the digestion agent on the microbeads were also investigated. To confirm whether the preparative method allows for the confirmation of the plastic-nature of microparticles, a qualification of spiked microplastics (polystyrene, 150 µm, and polyethylene terephthalate, 300 µm) was performed using pyrolysis–gas chromatography–mass spectrometry.

Alternative splicing (AS) of RNA molecules is a key contributor to transcriptome diversity. In humans, 90%–95% of multi-exon genes produce alternatively spliced RNA transcripts. Therefore, every single gene has the opportunity of producing multiple splice variants, including long non-coding RNA (lncRNA) genes that undergo RNA maturation steps such as conventional and alternative splicing. Emerging evidence suggests significant roles for these lncRNA splice variants in many aspects of cell biology. Differential changes in expression of specific lncRNA splice variants have also been associated with many diseases including cancer. This review covers the current knowledge on this emerging topic of investigation. We provide exclusive insights on the AS landscape of lncRNAs and also describe at the molecular level the functional relevance of lncRNA splice variants, i.e., RNA-based differential functions, production of micropeptides, and generation of circular RNAs. Finally, we discuss exciting perspectives for this emerging field and outline the work required to further develop research endeavors in this field.

Abstract Background Acute pain is a common symptom in children of all ages, and is associated with a variety of conditions. Despite the availability of guidelines, pain often remains underestimated and undertreated. Paracetamol and ibuprofen are the most commonly used drugs for analgesia in Pediatrics. Multimodal pain management by using a combination of paracetamol and ibuprofen results in greater analgesia. Methods An investigation using the Nominal Group Technique was carried out between May and August 2022. Two open (non-anonymous) questionnaires were consecutively sent to a Board of ten clinicians to understand their opinions on the use of the oral paracetamol and ibuprofen association. Answers were examined in a final meeting where conclusions were drawn. Results The board achieved a final consensus on a better analgesic power of paracetamol and ibuprofen in fixed-dose combination as compared to monotherapy, without compromising safety. Strong consensus was reached on the opinion that the fixed-dose combination of paracetamol and ibuprofen may be a useful option in case of inefficacy of one or other drug as monotherapy, especially in case of headaches, odontalgia, earache, and musculoskeletal pain. The use of the fixed combination may be also considered suitable for postoperative pain management. Conclusions The use of the fixed-dose combination may represent advantage in terms of efficacy and safety, allowing a better control of the dose of both paracetamol and ibuprofen as monotherapy, thus minimizing the risk of incorrect dosage. However, the limited evidence available highlights the need for future well designed studies to better define the advantages of this formulation in the various therapeutic areas.

Phthalate esters (PAEs) are a group of chemicals used to improve the flexibility and durability of plastics. The chemical properties and the resistance to high temperatures promote their degradation and release into the environment. Food and beverages can be contaminated by PAEs through the migration from packaging material because they are not covalently bound to plastic and also via different kinds of environmental sources or during processing. For instance, alcoholic drinks in plastic containers are a particular risk, since the ethanol contained provides a good solubility for PAEs. According to its role as an endocrine disruptor compound and its adverse effects on the liver, kidney, and reproductive and respiratory systems, the International Agency on Research Cancer (IARC) classified di-(2-ethylhexyl) phthalate (DEHP) as a possible human carcinogen. For this reason, to control human exposure to PAEs, many countries prohibited their use in food as non-food substances. For example, in Europe, the Commission Regulation (EU) 2018/2005 restricts the use of DEHP, dibutyl phthalate (DBP), benzyl butyl phthalate (BBP), and diisobutyl phthalate (DiBP) to a concentration equal to or below 0.1 by weight in plasticizers in articles used by consumers or in indoor areas. There are reports from the US Food and Drug Administration (FDA) that some beverages (and food as well), particularly fruit juices, contain high levels of phthalates. In some cases, the deliberate adulteration of soft drinks with phthalate esters has been reported. This paper would like to show the difficulties of performing PAE analysis in beverage matrices, in particular alcoholic beverages, as well as the main solutions provided for quality control in the industrial branches.

Per- and polyfluorinated alkyl substances (PFASs) are a group of anthropogenic chemicals used in a range of industrial processes and consumer products. Recently, their ubiquitous presence in the environment as well as their toxicological effects in humans have gained relevant attention. Although the occurrence of PFASs is widely investigated in scientific community, the standardization of analytical method for all matrices still remains an important issue. In this review, we discussed extraction and detection methods in depth to evaluate the best procedures of PFAS identification in terms of analytical parameters (e.g., limits of detection (LODs), limits of quantification (LOQs), recoveries). Extraction approaches based on liquid–liquid extraction (LLE), alkaline digestion, and solid phase extraction (SPE), followed by liquid chromatography–mass spectrometry (LC-MS) and gas chromatography–mass spectrometry (GC-MS) analysis are the main analytical methods applied in the literature. The results showed detectable recoveries of PFOA and PFOS in meat, milk, vegetables, eggs products (90.6–101.2% and of 89.2–98.4%), and fish (96–108%). Furthermore, the low LOD and LOQ values obtained for meat (0.00592–0.01907 ng g−1; 0.050 ng g−1), milk (0.003–0.009 ng g−1; 0.010–0.027 ng g−1), fruit (0.002–0.009 ng g−1; 0.006–0.024 ng g−1), and fish (0.00369–0.017.33 ng g−1; 0.05 ng g−1) also confirmed the effectiveness of the recent quick, easy, cheap, effective, rugged, and safe method (QuEChERS) for simple, speedy, and sensitive ultra-trace PFAS analysis.

Background: The presence of a wound can be anywhere from non-problematic to life-threatening on a severity spectrum, with bacterial infection and resistance playing a major role in the development of chronicity, delaying wound healing. Wound colonization with multiple organisms and the limited number of effective antibiotics place a heavy burden on the healthcare system, with patients going through multiple surgeries during a prolonged hospitalization time. By analyzing the resistance patterns of pluri-bacterial populations and the approach used in managing complex cases, we aim to improve the protocols applied in caring for chronic wounds in our practice and share our experiences and observations. Methods: We designed a retrospective study on 212 diabetic and non-diabetic patients, aiming to evaluate the course of chronic wound treatment in our practice. We focused on the impact that MDR bacteria and diabetes have on surgical outcomes and their role in the healing process. Results: Patients who received empiric antibiotic therapy before being admitted eventually presented with multiple MDR bacteria compared to those who did not receive antibiotics (p = 0.014). The presence of at least one MDR bacteria in the wound bed was associated with ulcers reaching bone (p = 0.02) and was positively correlated with the number of surgeries performed (p < 0.001). Diabetes played a significant role in surgery-related complications (p = 0.02) and hospitalization time (p < 0.001). Conclusions: Proper management of chronic wounds requires a comprehensive, multidisciplinary approach and a thorough understanding of antibiotic usage. To address this need, we have developed and implemented a chronic wound treatment protocol in our clinic, with the goal of discharging patients once their ulcers have been treated and closed. A key summary of the protocol presented is to reduce the incidence of MDR bacteria and improve the patient’s quality of life.

Metformin is a highly effective medication for managing type 2 diabetes mellitus. Recent studies have shown that it has significant therapeutic benefits in various organ systems, particularly the liver. Although the effects of metformin on metabolic dysfunction-associated steatotic liver disease and metabolic dysfunction-associated steatohepatitis are still being debated, it has positive effects on cirrhosis and anti-tumoral properties, which can help prevent the development of hepatocellular carcinoma. Furthermore, it has been proven to improve insulin resistance and dyslipidaemia, commonly associated with liver diseases. While more studies are needed to fully determine the safety and effectiveness of metformin use in liver diseases, the results are highly promising. Indeed, metformin has a terrific potential for extending its full therapeutic properties beyond its traditional use in managing diabetes.

The recent introduction of immunotherapy and targeted therapy has substantially enriched the therapeutic landscape of metastatic melanoma. However, cerebral metastases remain unrelenting entities with atypical metabolic and genetic profiles compared to extracranial metastases, requiring combined approaches with local ablative treatment to alleviate symptoms, prevent recurrence and restore patients’ biological and psychological resources for fighting malignancy. This paper aims to provide the latest scientific evidence about the rationale and timing of treatment, emphasizing the complementary roles of surgery, radiotherapy, and systemic therapy in eradicating brain metastases, with a special focus on the distinct response of intracranial and extracranial disease, which are regarded as separate molecular entities. To illustrate the complexity of designing individualized therapeutic schemes, we report a case of delayed BRAF-mutant diagnosis, an aggressive forearm melanoma, in a presumed psychiatric patient whose symptoms were caused by cerebral melanoma metastases. The decision to administer molecularly targeted therapy was dictated by the urgency of diminishing the tumor burden for symptom control, due to potentially life-threatening complications caused by the flourishing of extracranial disease in locations rarely reported in living patients, further proving the necessity of multidisciplinary management.

A WebGis tool called GoProsit has been developed to support winegrowers in planning a new sustainable vineyard and in the identification of high-quality terroir in Tuscany, Central Italy, by providing various information on soils, climate, hydrological risks, and fertilization. GoProsit, hosted by the web platform GEAPP, is a free, user-friendly, and interactive Geographic Decision Support System (GDSS). Soil data behind the WebGis tool has a 1 ha resolution, achieved by processing the legacy vector-type soil database of the Tuscany Region with the DSMART (Disaggregation and Harmonization of Soil Map Units Through Resampled Classification Trees as supervised classification) algorithm, which disaggregated the map to 297,023 vineyard grid cells. Each grid cell holds climatic and pedologic information, along with physical and chemical features for each horizon of the most probable soil. GoProsit also provides soil maps in image format obtained by georeferencing about 50 historical soil maps (1969–2012). Finally, GoProsit runs and returns the outputs of six models: (a) carbon footprint, (b) potential erosion and maximum vine row length compatible with tolerable erosion, (c) potential water stress, (d) risk of runoff/waterlogging, (e) identification of suitable rootstocks, and (f) nutritional needs before planting. Statistics of the main model results for the investigated area are reported. This promising tool will soon be usable for the whole Italian territory; however, its potential makes it suitable for use in any wine-growing district.

Analysis of a series of thermal mappings obtained by UAS flights on quiescent volcanoes requires some special techniques to be performed. The main challenge is represented by the difficulty of separating hot and cold pixels in areas where their temperatures are quite similar. This task is indeed much simpler, for example, for lava flows where the temperature differences between the hot lava and the cold soil is rather big. This paper shows various software developed in order to perform this extraction and calculate the trends over time of both the average temperature and the heat flux from the soil. This prototypal implementation used thermal flights performed over a time span of a few years on an area in the Campi Flegrei caldera in southern Italy. Standard image manipulation techniques were used to segmentate and clusterise each thermal mapping in order to reduce the thermal anomalies to some sets of simpler features characterised by their fundamental parameters. The temporal trends of some physical parameters (temperature, heat flux, etc.) were extracted from these sets, and we found interesting results necessary for correlations and for ongoing research with other parameters.

In order for neural progenitors (NPs) to generate distinct populations of neurons at the right time and place during CNS development, they must switch from undergoing purely proliferative, self-renewing divisions to neurogenic, asymmetric divisions in a tightly regulated manner. In the developing Drosophila optic lobe, neuroepithelial (NE) cells of the outer proliferation center (OPC) are progressively transformed into neurogenic NPs called neuroblasts (NBs) in a medial to lateral proneural wave. The cells undergoing this transition express Lethal of Scute (L'sc), a proneural transcription factor (TF) of the Acheate Scute Complex (AS-C). Here we show that there is also a peak of expression of Asense (Ase), another AS-C TF, in the cells neighboring those with transient L'sc expression. These peak of Ase cells help to identify a new transitional stage as they have lost NE markers and L'sc, they receive a strong Notch signal and barely exhibit NB markers. This expression of Ase is necessary and sufficient to promote the NE to NB transition in a more robust and rapid manner than that of l'sc gain of function or Notch loss of function. Thus, to our knowledge, these data provide the first direct evidence of a proneural role for Ase in CNS neurogenesis. Strikingly, we found that strong Delta-Notch signaling at the lateral border of the NE triggers l'sc expression, which in turn induces ase expression in the adjacent cells through the activation of Delta-Notch signaling. These results reveal two novel non-conventional actions of Notch signaling in driving the expression of proneural factors, in contrast to the repression that Notch signaling exerts on them during classical lateral inhibition. Finally, Suppressor of Hairless (Su(H)), which seems to be upregulated late in the transitioning cells and in NBs, represses l'sc and ase, ensuring their expression is transient. Thus, our data identify a key proneural role of Ase that is integrated with the sequential activities of Delta-Notch signaling, L'sc, and Su(H), driving the progressive transformation of NE cells into NBs.

Introduction: Hydrogen sulfide (H2S) is emerging as an important potential therapeutic option for respiratory inflammatory diseases. In this study, we investigated the effectiveness of a novel corticosteroid derivative, that is chemically linked to an H2S donor, in managing asthma features.Methods: The effects of prednisone (PS), H2S donor (4-hydroxybenzamide; TBZ), and their combination (PS-TBZ) have been evaluated in vitro and in vivo. The in vitro experiments were conducted using lipopolysaccharidestimulated J774 macrophages, while the in vivo experiments utilizing an experimental asthma model.Results: In the in vitro study we found that PS-TBZ exhibited an increased effect compared to the individual parent compounds in modulating the production of inflammatory mediators. TBZ also significantly reduced bronchial contractility and enhanced bronchial relaxation. In the in vivo experiments, where we administered PS, TBZ, or PS-TBZ to ovalbumin-sensitized BALB/c mice, we confirmed that PS-TBZ had a significantly better action in controlling airway hyperreactivity as compared to TBZ or PS alone. Moreover, PS-TBZ was more effective in restoring salbutamol-induced relaxation. The immunohistochemistry analysis demonstrated a significant reduction in the production of α-SMA and procollagen III, indicating the efficacy of PS-TBZ in controlling airway remodeling. Moreover, PS-TBZ also promoted epithelial repair, recovery of the bronchial and parenchyma structure and inhibited mucin production.Discussion: In conclusion, PS-TBZ offers an important opportunity to optimize the beneficial impact of corticosteroids on asthma features.

Abstract Bronchiolitis is an acute respiratory illness that is the leading cause of hospitalization in young children. This document aims to update the consensus document published in 2014 to provide guidance on the current best practices for managing bronchiolitis in infants. The document addresses care in both hospitals and primary care. The diagnosis of bronchiolitis is based on the clinical history and physical examination. The mainstays of management are largely supportive, consisting of fluid management and respiratory support. Evidence suggests no benefit with the use of salbutamol, glucocorticosteroids and antibiotics with potential risk of harm. Because of the lack of effective treatment, the reduction of morbidity must rely on preventive measures. De-implementation of non-evidence-based interventions is a major goal, and educational interventions for clinicians should be carried out to promote high-value care of infants with bronchiolitis. Well-prepared implementation strategies to standardize care and improve the quality of care are needed to promote adherence to guidelines and discourage non-evidence-based attitudes. In parallel, parents' education will help reduce patient pressure and contribute to inappropriate prescriptions. Infants with pre-existing risk factors (i.e., prematurity, bronchopulmonary dysplasia, congenital heart diseases, immunodeficiency, neuromuscular diseases, cystic fibrosis, Down syndrome) present a significant risk of severe bronchiolitis and should be carefully assessed. This revised document, based on international and national scientific evidence, reinforces the current recommendations and integrates the recent advances for optimal care and prevention of acute bronchiolitis.

Background & Aims: The intestinal epithelium intrinsically renews itself ex vivo via the proliferation of Lgr5+ intestinal stem cells, which is sustained by the establishment of an epithelial stem cell niche. Differentiated Paneth cells are the main source of epithelial-derived niche factor supplies and produce Wnt3 as an essential factor in supporting Lgr5+ stem cell activity in the absence of redundant mesenchymal Wnts. Maturation of Paneth cells depends on canonical Wnt signaling, but few transcriptional regulators have been identified to this end. The role of HNF4α in intestinal epithelial cell differentiation is considered redundant with its paralog HNF4γ. However, it is unclear whether HNF4α alone controls intrinsic intestinal epithelial cell growth and fate in the absence of a mesenchymal niche. Methods: We used transcriptomic analyses to dissect the role of HNF4α in the maintenance of jejunal epithelial culture when cultured ex vivo as enteroids in the presence or absence of compensatory mesenchymal cells. Results: HNF4α plays a crucial role in supporting the growth and survival of jejunal enteroids. Transcriptomic analyses revealed an autonomous function of HNF4α in Wnt3 transcriptional regulation and Paneth cell differentiation. We showed that Wnt3a supplementation or co-culture with intestinal subepithelial mesenchymal cells reversed cell death and transcriptional changes caused by the deletion of Hnf4a in jejunal enteroids. Conclusions: Our results support the intrinsic epithelial role of HNF4α in regulating Paneth cell homeostasis and intestinal epithelium renewal in the absence of compensatory Wnt signaling.

BackgroundPrevious studies have shown sex differences in stroke care. Female patients have both lower thrombolytic treatment rates with OR reported as low as 0.57 and worse outcomes. With updated standards of care and improved access to care through telestroke, there is potential to reduce or alleviate these disparities.MethodsAcute stroke consultations seen by TeleSpecialists, LLC physicians in the emergency department in 203 facilities (23 states) from January 1, 2021 to April 30, 2021 were extracted from the Telecare by TeleSpecialists™ database. The encounters were reviewed for demographics, stroke time metrics, thrombolytics candidate, premorbid modified Rankin Score, NIHSS score, stroke risk factors, antithrombotic use, admitting diagnosis of suspected stroke, and reason not treated with thrombolytic. The treatment rates, door to needle (DTN) times, stroke metric times, and variables of treatment were compared for females and males.ResultsThere were 18,783 (10,073 female and 8,710 male) total patients included. Of the total, 6.9% of females received thrombolytics compared to 7.9% of males (OR 0.86, 95% CI 0.75–0.97, p = 0.006). Median DTN times were shorter for males than females (38 vs. 41 min, p

Notwithstanding the increased interest in wild edible plants, little is known on how some domestic thermal processes can affect their content. The aim of this study was to investigate the amounts of minerals, B1 and B2 vitamins, tocols, and carotenoids in raw, boiled, and steamed wild edible plants, namely, Sonchus asper (L.) Hill s.l., Sonchus oleraceus L., Cichorium intybus L., and Beta vulgaris L. var cicla. All vegetables were confirmed as high sources of lutein (from 6 to 9 mg/100 g) and β-carotene (from 2 to 5 mg/100 g). Quite high amounts of violaxanthin and neoxanthin were found. Alfa-tocopherol and γ-tocopherol were the main tocols, with same contents in raw and processed vegetables (about 2.5 mg/100 g). The most abundant macro element and trace element were, respectively, potassium and iron. B1 and B2 vitamins were found in low amounts in almost all plants, with the exception of thiamine in Beta vulgaris (about 1.6 mg/100 g). Boiling led to a significant loss of minerals (up to 60%) and B-group vitamins (up to 100%), while, among carotenoids, it only affected violaxanthin levels (up to 90%). Steamed vegetables showed only a slight reduction, about 20%, in β-carotene and lutein, with a marked decrease in violaxanthin and neoxanthin. One hundred grams of all fresh and cooked plants can be claimed as a source of vitamin A and E.

Abstract Background We retrospectively evaluated safety and performance of magnetic seed localisation of nonpalpable breast lesions. Methods We reviewed records of patients with nonpalpable breast lesions preoperative localised by placing magnetic Magseed® marker between February 2019 and December 2020. During surgery, Sentimag® magnetic probe was used to localise the marker and guide surgery. Safety, lesion identification and excision with tumour with free margins and re-excision rate were assessed. Results A total of 77 Magseed® devices were placed into the breasts of 73 patients, 44 under ultrasound and 33 under stereotactic guidance (4 bilateral). All devices were retrieved as were the target lesions. Magnetic marker placement was successful in all cases without any adverse event. Intraoperative identification and excision of the localised lesion were successful in 77 of 77 of cases (100%). In three cases (all of them calcifications with the seed placed under stereotactic guidance), the seed did not reach the exact target position of the biopsy clip; thus, larger excision was needed, with localisation failure attributed to incorrect clip insertion (n = 1) or to clip dislocation (n = 2). Migration of the marker was negligible in all patients. Complete excision after the initial procedure with at least 1-mm disease-free margins was obtained in 74 out of 77 (96.1%) lesions. The re-excision rate was 3 out of 77 (4%). Conclusions Magnetic marker localisation for nonpalpable breast lesions was safe, reliable, and effective in terms of lesion identification, excision with tumour-free margins and re-excision rate.

Introduction. The Campania population is characterized by a high incidence and prevalence of heart failure (HF). The aim of this study is to describe the clinical epidemiology, comorbidities, and treatments in HF patients admitted to Internal Medicine Wards (IMW) in Campania. Materials and Methods. It is a retrospective, regional, multicentric, observational study including patients admitted to 15 IMW in Campania, with an HF diagnosis, over a period of three consecutive months. Results. We identified 427 patients, stratified by ejection fraction (EF) category (127 EF≤40%; 216 EF 41-49%; 84 EF≥50%). In comparison with HFpEF subjects, patients with HFrEF were younger (74 years vs. 9 years), more commonly male (67% vs. 32%), and more likely to have an ischaemic aetiology (45% vs. 25%). The most used drugs at the time of hospitalization and after were Diuretics (80.3/93.5%), BBs (69.6/92.6%), and Statins (52.1/63.7%), with statistically significant pre-/post- differences (P≤0.05). Conclusions. EF is more likely to be non-preserved in younger males and in patients with CAD etiology. Hospitalization influences in a statistically significant way the change or adjustment of therapy for almost all drugs.

In this study, a simple, fast, and effective methodology has been developed for the detection and quantification of seven phthalates potentially released in hot drinks from disposable containers used in vending machines. The authors determined the optimal conditions to be applied during the various steps of extraction of seven phthalates (DMP, DEP, DBP, DiBP, DEHP, DNOP, and DDP) from hot beverages using a model solution. The extraction and preconcentration technique used was ultrasound–vortex-assisted dispersive liquid–liquid microextraction (UVA-DLLME) followed by gas chromatographic analysis obtaining recoveries from 66.7% to 101.2% with precision and reproducibility −1 and 15.4 ng mL−1 and limits of quantification (LOQs) from 1.6 ng mL−1 to 35.8 ng mL−1, both of them lower than those found by FID, were obtained.

Purpose:The purpose of this study was to discriminate between benign and malignant breast lesions through several classifiers using, as predictors, radiomic metrics extracted from CEM and DCE-MRI images. In order to optimize the analysis, balancing and feature selection procedures were performed. Methods: Fifty-four patients with 79 histo-pathologically proven breast lesions (48 malignant lesions and 31 benign lesions) underwent both CEM and DCE-MRI. The lesions were retrospectively analyzed with radiomic and artificial intelligence approaches. Forty-eight textural metrics were extracted, and univariate and multivariate analyses were performed: non-parametric statistical test, receiver operating characteristic (ROC) and machine learning classifiers. Results: Considering the single metrics extracted from CEM, the best predictors were KURTOSIS (area under ROC curve (AUC) = 0.71) and SKEWNESS (AUC = 0.71) calculated on late MLO view. Considering the features calculated from DCE-MRI, the best predictors were RANGE (AUC = 0.72), ENERGY (AUC = 0.72), ENTROPY (AUC = 0.70) and GLN (gray-level nonuniformity) of the gray-level run-length matrix (AUC = 0.72). Considering the analysis with classifiers and an unbalanced dataset, no significant results were obtained. After the balancing and feature selection procedures, higher values of accuracy, specificity and AUC were reached. The best performance was obtained considering 18 robust features among all metrics derived from CEM and DCE-MRI, using a linear discriminant analysis (accuracy of 0.84 and AUC = 0.88). Conclusions: Classifiers, adjusted with adaptive synthetic sampling and feature selection, allowed for increased diagnostic performance of CEM and DCE-MRI in the differentiation between benign and malignant lesions.

Whispering-gallery mode microresonators have gained wide popularity as experimental platforms for different applications, ranging from biosensing to nonlinear optics. Typically, the resonant modes of dielectric microresonators are stimulated via evanescent wave coupling, facilitated using tapered optical fibers or coupling prisms. However, this method poses serious shortcomings due to fabrication and access-related limitations, which could be elegantly overcome by implementing a free-space coupling approach; although additional alignment procedures are needed in this case. To address this issue, we have developed a new algorithm to excite the microresonator automatically. Here, we show the working mechanism and the preliminary results of our experimental method applied to a home-made silica microsphere, using a visible laser beam with a spatial light modulator and a software control.

In recent years, human populations’ exposure to microplastics via foods is becoming a topic of concern. Although microplastics have been defined as “emerging contaminants”, their occurrence in the environment and food is quite dated. This systematic review aims to investigate the discrepancies which are characterizing the research in the microplastics field in foods, with particular regard to sample preparations, microplastics’ concentrations and their effect on humans. For the selection of papers, the PRISMA methodology was followed. Discrepancies in the methodological approaches emerged and in the expression of the results as well, underlying the urgency in the harmonization of the methodological approaches. Uncertainties are still present regarding the adverse effects of microplastics on the human body. The scientific evidence obtained thus far is, in fact, not sufficient to demonstrate a concrete negative effect. This review has clearly underlined the need to standardise laboratory approaches to obtain useful results for better food safety management.

Mental health problems in early childhood are common, but there is a lack of psychiatric research on this age group. DC:0-5 is a multiaxial classification system for mental disorders in early childhood, providing a framework for standardizing clinical practice and research. However, research on the validity of DC:0-5 is scarce. The Developmental Psychiatry Diagnostic Challenges Study (DePsy) is a multi-site, prospective clinical study including six German early childhood mental health (ECMH) clinics. The main objective of the study is to contribute to the validation of Axis I and Axis II of DC:0-5. A second aim of the study is to describe the population of the participating clinics regarding diagnoses, family context, and treatment outcomes. Additionally, the impact of environmental risk factors, including parental Adverse Childhood Experiences (ACEs) and media use, on child psychopathology and caregiver–child relationships will be examined. Over two years, patients aged 0.0–5.9 years old will be enrolled in the study. Assessments include ICD-10 and DC:0-5 diagnoses, developmental tests, video-based observations of caregiver—child interactions, and questionnaires on child psychopathology, media use, parental stress, and treatment satisfaction. Study results will promote the standardization of assessment and treatment in ECMH clinics aiming to improve the development of patients and their families.

An economy based on renewable energy sources is the hallmark of responsible companies. Climate policy and energy crisis commitments have led to a search for alternative ways to produce energy. Bioenergy is considered the most consistent renewable energy source due to its economic and environmental benefits. Biogas and biomethane are promising forms of renewable energy derived from widely available evergreen raw materials. Agricultural, animal, industrial and food wastes are excellent substrates used to produce clean and sustainable energy in a circular economy context. Their conversion into biogas and biomethane through the anaerobic digestion (AD) process is an efficient solution to the treatment of waste of different origins. The production and use of biomethane favor important environmental advantages, such as the reduction in greenhouse gas emissions compared with those deriving from the use of conventional fossil fuels. This review would like to highlight modern trends and approaches to evaluate processes and strategies to control biogas and biomethane production. In particular, the use of livestock waste for the digestion process and the reuse of the by-product as fertilizer, as well as the potential development of biogas and biomethane as prospects for the improvement and optimization of renewable energy sources, are discussed.

Abstract Background Since operative vaginal delivery may be risky for women and might cause neonatal complications, the aim of this study is to assess appropriateness of the procedure. This is a prospective, longitudinal, multicenter, observational study and it was conducted in three Italian Obstetric Units (Pisa, Massa Carrara and Prato). All term pregnant women, either nulliparous and multiparous, with singleton pregnancy and a cephalic fetus, with spontaneous or induced labour, requiring vacuum-assisted delivery were enrolled. Indications to operative vaginal delivery were grouped as alterations of fetal cardiotocography (CTG) patterns, delay/arrest of second stage of labour or elective shortening of second stage of labour. A board consisting of five among authors evaluated appropriateness of the procedure. Results Overall, 466 women undergoing operative vaginal deliveries were included. Cardiotocography, classified as ACOG category 2 or 3 was the indication for vacuum assisted delivery in 253 patients (54.29%). Among these, 66 women (26.1%) had an operative vaginal delivery which was then considered to be inappropriate, while in 114 cases (45.1%) CTG traces resulted to be unreadable. Conclusion Decision making process, which leads clinicians to go for operative vaginal delivery, is often influenced by shortness of time and complexity of the situation. Therefore, clinicians tend to intervene performing vacuum delivery without adopting critical analysis and without adequately considering the clinical situation. Tweetable abstract Operative vaginal delivery might be a risky procedure and should be performed only when clinically indicated and after adequate critical analysis.
