BackgroundProfessional society guidelines are emerging for cardiovascular care in cancer patients. However, it is not yet clear how effectively the cancer survivor population is screened and treated for cardiomyopathy in contemporary clinical practice. As electronic health records (EHRs) are now widely used in clinical practice, we tested the hypothesis that an EHR-based cardio-oncology registry can address these questions. ObjectiveThe aim of this study was to develop an EHR-based pragmatic cardio-oncology registry and, as proof of principle, to investigate care gaps in the cardiovascular care of cancer patients. MethodsWe generated a programmatically deidentified, real-time EHR-based cardio-oncology registry from all patients in our institutional Cancer Population Registry (N=8275, 2011-2017). We investigated: (1) left ventricular ejection fraction (LVEF) assessment before and after treatment with potentially cardiotoxic agents; and (2) guideline-directed medical therapy (GDMT) for left ventricular dysfunction (LVD), defined as LVEF

Rickets, one of the leading causes of bony deformities and short stature, can be calciopenic (inciting event is defective intestinal calcium absorption) or phosphopenic (inciting event is phosphaturia). Early diagnosis and timely treatment of rickets are crucial for correction of the limb deformities. Guidelines exist for nutritional rickets, but the diagnosis and management of the relatively uncommon forms of rickets are complex. This consensus aims to formulate a simplified diagnostic approach for rickets, especially in resource-limited settings. The consensus statement has been formulated by a 29-member committee from the Endocrine Society of Bengal. The process included forming a working group, conducting a literature review, identifying controversies, drafting, and discussion at a consensus meeting. Participants rated their agreement with the clinical practice points, and a 70% consensus was required. Input integration and further review led to the final consensus statements. Children with suspected rickets should initially be examined for distinctive skeletal deformities. The diagnosis of rickets should be confirmed with characteristic radiographic abnormalities. It is advisable to order tests for serum calcium, inorganic phosphorus (Pi), liver function, 25-hydroxyvitamin D (25OHD), parathyroid hormone, creatinine, and potassium in all patients with rickets. In cases of refractory rickets, it is also recommended that assessments be conducted for spot urine calcium, Pi, creatinine, and, blood gas analysis. In children with rickets and metabolic acidosis, tests for glycosuria, uricosuria, aminoaciduria, low molecular weight proteinuria, and albuminuria should be conducted. In children with resistant calciopenic rickets and sufficient serum 25OHD levels, serum 1,25(OH)2D concentration should be tested. 1,25(OH)2 D and fibroblast growth factor 23 estimation is useful for certain forms of phosphopenic rickets.

Background: Maintaining optimal hearing health and preventing ear infections is crucial across all life stages, spanning from before birth and infancy to middle age and beyond. The primary aim of the research was to evaluate and compare the effectiveness of the telemedicine-enabled otoscope (TEO) in diagnosing ear diseases with that of the conventional otoscope. Materials and Methods: The databases PubMed, Cochrane, and Embase were thoroughly searched to find original studies on TEO at the community as well as hospital levels along with meta-analysis for comparison with standard diagnostic methods using traditional otoscopes. Results: The telemedicine-equipped otoscope displays a sensitivity of 82% (confidence interval 0.78–0.95) and a specificity of 95% (confidence interval 0.93–0.96). When employed by physicians, the combined sensitivity and specificity become 84% (confidence interval 0.79–0.88) and 91% (confidence interval 0.87–0.94), respectively. Community Health Workers (CHWs) using telemedicine-enabled otoscopes exhibit a collective sensitivity of 80% (confidence interval 0.72–0.87) and a collective specificity of 97% (confidence interval 0.95–0.98). Snapping multiple photographs and removal of cerumen where necessary in increasing the quality of the photographs for diagnosis. Conclusions: The TEO holds the promise of substantially enhancing the accessibility of audiology services, encompassing evaluation, public outreach, and fundamental care. Its implementation has the potential to fortify primary health care environments, contributing to the prevention of ear diseases and assisting in mitigating the shortage of skilled ear care professionals.

Abstract Background Hepatocellular carcinoma (HCC), a leading cause of cancer-related deaths globally, poses significant challenges in early detection. Improved diagnostic accuracy can drastically influence patient outcomes, emphasizing the need for innovative, non-invasive biomarkers. Methods: This study utilized a cohort of 402 participants, including healthy controls, chronic hepatitis patients, and HCC patients from Bangladesh, to evaluate DNA methylation signatures in peripheral blood mononuclear cells (PBMC). We performed targeted next-generation sequencing on selected genes previously identified to assess their methylation dynamics. The development of M8 and M4 scores was based on these dynamics, using Receiver Operating Characteristic (ROC) analysis to determine their effectiveness in detecting early-stage HCC alongside existing markers such as epiLiver and alpha-fetoprotein (AFP). Results: Integration of M8 and M4 scores with epiLiver and AFP significantly enhances diagnostic sensitivity for early-stage HCC. The M4+epiLiver score achieves a sensitivity of 79.4% in Stage A HCC, while combining M4 with AFP increases sensitivity to 88.2–95.7% across all stages, indicating a superior diagnostic performance compared to each marker used alone. Conclusions: Our study confirms that combining gene methylation profiles with established diagnostic markers substantially improves the sensitivity of detecting early-stage HCC. This integrated diagnostic approach holds promise for advancing non-invasive cancer diagnostics, potentially leading to earlier treatment interventions and improved survival rates for high-risk patients.

Abstract Dinanath grass (Pennisetum pedicellatum Trin.) is an extensively grown forage grass known for its significant drought resilience. In order to comprehensively grasp the adaptive mechanism of Dinanath grass in response to water deficient conditions, transcriptomic and metabolomics were applied in the leaves of Dinanath grass exposed to two distinct drought intensities (48-hour and 96-hour). Transcriptomic analysis of Dinanath grass leaves revealed that a total of 218 and 704 genes were differentially expressed under 48- and 96-hour drought conditions, respectively. The genes that were expressed differently (DEGs) and the metabolites that accumulated in response to 48-hour drought stress mainly showed enrichment in the biosynthesis of secondary metabolites, particularly phenolics and flavonoids. Conversely, under 96-hour drought conditions, the enriched pathways predominantly involved lipid metabolism, specifically sterol lipids. In particular, phenylpropanoid pathway and brassinosteroid signaling played a crucial role in drought response to 48- and 96-hour water deficit conditions, respectively. This variation in drought response indicates that the adaptation mechanism in Dinanath grass is highly dependent on the intensity of drought stress. In addition, different genes associated with phenylpropanoid and fatty acid biosynthesis, as well as signal transduction pathways namely phenylalanine ammonia-lyase, putrescine hydroxycinnamoyl transferase, abscisic acid 8’-hydroxylase 2, syntaxin-61, lipoxygenase 5, calcium-dependent protein kinase and phospholipase D alpha one, positively regulated with drought tolerance. Combined transcriptomic and metabolomic analyses highlights the outstanding involvement of regulatory pathways related to secondary cell wall thickening and lignin biosynthesis in imparting drought tolerance to Dinanath grass leaves. These findings collectively contribute to an enhanced understanding of candidate genes and key metabolites relevant to drought response in Dinanath grass. Furthermore, they establish a groundwork for the creation of a transcriptome database aimed at developing abiotic stress-tolerant grasses and major crop varieties through both transgenic and genome editing approaches.

In the present study, nano-ligninwas synthesized from rice straw using Lactobacillus rhamnosus for the cost-effective removal of harmful dye from wastewater. The UV absorption maxima of the synthesized nano-lignin were around 280–290 nm. Several FTIR bands between 1075 cm−1 to 3500 cm−1 confirmed the successful confirmation of nano-lignin synthesis from rice straw. STEM and DLS studies confirmed the spherical morphology of nano-lignin with an average particle size of 286.3 nm. The nano-lignin showed a negative zeta potential value of −16.9 ± 5.94 mv. Afterward, nano-lignin was used to remove basic methylene blue (MB), phenol red (PR),and brilliant green (BG) dye from aqueous solutions. The adsorption of these dyes increased with increased incubation time. The adsorption concentration of the MB, PR, and BG dyes ranged from 3.5 mg/g to a final of 24 mg/g, 2.5 mg/g to 42.5 mg/g, and 1.5 gm/g to 5.5 mg/g, respectively.

Autoimmune hemolytic anaemia (AIHA) is an uncommon cause of antibody-induced hemolytic anemia in children. It is divided into three categories: warm AIHA, cold antibody AIHA and paroxysmal cold hemoglobinuria. The diagnostic work-up typically begins with a peripheral smear and a direct antiglobulin test. Further diagnostic approaches and pathogenesis of all three entities are discussed. Clinical trials are lacking for AIHA in children. First-line therapy for warm AIHA is corticosteroids and for cold antibody AIHA is rituximab. Data on other therapeutic agents is reviewed. Supportive care is an important aspect, particularly in cold AIHA and paroxysmal cold hemoglobinuria. Issues related to blood transfusion due to antibodies including the least incompatible blood are discussed. Tables and figures provide an overview of pathology, diagnosis and diagnostic algorithm.

In recent scenarios, induction motors are used in many different fields due to their small size, advanced technology, and capacity to handle large amounts of power. One of the annoying downsides of conventional AC traction drive systems is the engine's speed and the uncertainty of the ripple content. The goal of the proposed work is to use various control techniques to analyze and enhance the performance of two five-phase induction motors that are supplied in parallel via an inverter. The control techniques are scalar control, six-step voltage type source inverter (VSI), space vector modulation (SVM) based VSI inverter, field-oriented control, model reference adaptive system-based field-oriented control (FOC), direct torque control (DTC), direct torque control -SVM, model reference adaptive system based direct torque control-SVM for traction application. The modified space vector modulation-based induction motor (IM) sensorless drive is realized together with a comparison of several control strategies. It makes use of direct torque control approaches with a model reference adaptive system (MRAS). An IM is designed, and performances are analyzed in high-speed and low-speed regions. Two bogies sets were considered in the closed-loop scheme, tested under parameter variations and in low-speed areas. The simulation result examined the most significant control methods for an IM fed by an inverter for electric traction applications. The performance of the torque in terms of reduced torque ripple in the IM drive is analyzed. The proposed direct torque control technique will reduce torque harmonic ripple by 100 % under the steady-state condition. Under a change in a parameter, the torque harmonic ripple is decreased by 99 %. For the IM drive, the hardware implementation of the advanced control approaches is examined.

ABSTRACT2´-O-methylation (Nm) is one of the most abundant modifications found in both mRNAs and noncoding RNAs. It contributes to many biological processes, such as the normal functioning of tRNA, the protection of mRNA against degradation by the decapping and exoribonuclease (DXO) protein, and the biogenesis and specificity of rRNA. Recent advancements in single-molecule sequencing techniques for long read RNA sequencing data offered by Oxford Nanopore technologies have enabled the direct detection of RNA modifications from sequencing data. In this study, we propose a bio-computational framework, Nm-Nano, for predicting the presence of Nm sites in direct RNA sequencing data generated from two human cell lines. The Nm-Nano framework integrates two supervised machine learning (ML) models for predicting Nm sites: Extreme Gradient Boosting (XGBoost) and Random Forest (RF) with K-mer embedding. Evaluation on benchmark datasets from direct RNA sequecing of HeLa and HEK293 cell lines, demonstrates high accuracy (99% with XGBoost and 92% with RF) in identifying Nm sites. Deploying Nm-Nano on HeLa and HEK293 cell lines reveals genes that are frequently modified with Nm. In HeLa cell lines, 125 genes are identified as frequently Nm-modified, showing enrichment in 30 ontologies related to immune response and cellular processes. In HEK293 cell lines, 61 genes are identified as frequently Nm-modified, with enrichment in processes like glycolysis and protein localization. These findings underscore the diverse regulatory roles of Nm modifications in metabolic pathways, protein degradation, and cellular processes. AbstractNovel magnetic and metallic nanoparticles garner much attention of researchers due to their biological, chemical and catalytic properties in many chemical reactions. In this study, we have successfully prepared a core–shell Fe3O4@SiO2@PDA nanocomposite wrapped with Ag using a simple synthesis method, characterised and tested on small cell lung cancer and antibacterial strains. Incorporating Ag in Fe3O4@SiO2@PDA provides promising advantages in biomedical applications. The magnetic Fe3O4 nanoparticles were coated with SiO2 to obtain negatively charged surface which is then coated with polydopamine (PDA). Then silver nanoparticles were assembled on Fe3O4@SiO2@PDA surface, which results in the formation core–shell nanocomposite. The synthesised nanocomposite were characterized using SEM-EDAX, dynamic light scattering, XRD, FT-IR and TEM. In this work, we report the anticancer activity of silver nanoparticles against H1299 lung cancer cell line using MTT assay. The cytotoxicity data revealed that the IC50 of Fe3O4@SiO2@PDA@Ag against H1299 lung cancer nanocomposites cells was 21.52 µg/mL. Furthermore, the biological data of nanocomposites against Gram-negative ‘Pseudomonas aeruginosa’ and Gram-positive ‘Staphylococcus aureus’ were carried out. The range of minimum inhibitory concentration was found to be 115 µg/mL where gentamicin was used as a standard drug. The synthesized AgNPs proves its supremacy as an efficient biomedical agent and AgNPs may act as potential beneficial molecule in lung cancer chemoprevention and antibacterial strains.

Background: The development of periodontal diseases has multifactorial causes including genetic factors. Limited investigations have been conducted to explore the association between ABO blood groups and the development and progression of periodontal diseases. Aim: To evaluate and assess the association of ABO Phenotype and Rhesus factor with oral hygiene status, severity of chronic periodontitis and blood parameters like hemoglobin level and Platelet count in localized and generalized chronic periodontitis. Material and methods: Study was carried out on 100 patients, out of which 80 patients of Generalized Chronic Periodontitis and 20 patients of Localized Chronic Periodontitis. Patients were categorized into Mild, Moderate and Severe Periodontitis. Result: A highly significant association was found between severity of periodontitis and blood groups with blood group B and O were found to be at a greater risk to develop moderate to severe form of chronic periodontitis. Also subjects with blood group B and O showed worst oral hygiene among all the blood groups. Also patients suffering from chronic periodontitis showed a general trend towards lower limit of both hemoglobin level and platelet. Conclusion: Genetic factors such as ABO blood group antigens may act as a risk influencer that plays a role in progression and severity of the chronic periodontitis, with blood group B and O being worst affected. Another observation was that a long standing case of chronic periodontitis can lead to anemia thus having systemic implications.

Abstract Viral diseases arising in farmed fish are an ongoing challenge to the aquaculture industry, causing severe mortality and economic losses. Recently, there has been a spike in the incidence of a viral disease caused by Tilapia Parvovirus (TiPV) inflicts irreparable damage, and large-scale fish kills in the farmed tilapia species. We investigated a case of disease outbreak and severe mortality in cage and wetland farms of tilapia in West Bengal and Odisha, India. The symptomatic fish showed clinical signs, including hemorrhage, discoloration, ulcer, and redness in the body surfaces. Further analysis revealed that Tilapia Parvovirus was associated (validated by PCR, phylogenetic analysis, and cell line assay) with the infection and mortality of tilapia. The virus was detected in gill, heart, spleen, liver, and kidney samples collected from apparently healthy (asymptomatic) and symptomatic tilapia samples from cage and wetland farms. At the same time, negative results were found in the brain and skin tissue samples. The histological analysis revealed that TiPV induces severe damage invariably in almost all studied tissue, including the liver, kidney, spleen, gill, heart, and brain of tilapia samples. The viral quantification analysis showed that the viral genome was higher in the liver, spleen, and heart than in the tilapia samples’ gill, kidney, or brain tissue. Furthermore, the study indicated that TiPV infection has a significant effect on the health of tilapia. The tilapia exhibited an immune reactivity toward TiPV infection (upregulation of chemokine receptors, CRs and interleukin 1β, IL-1β), the majority of the studied immune genes (interleukin 8, IL-8; Toll-like receptors 7, TLR7; tumour necrosis factor α, TNF-α; major histocompatibility complex II, MHC II and nuclear factor kappa B, NF-kB) were significantly downregulated in the kidney, spleen and liver tissue samples of symptomatic tilapia. Further, the in vivo challenge assay confirms that the isolated TiPV is a novel parvovirus pathogen that causes massive mortality in tilapia. The lessons learned from the first cellular and molecular description associated with TiPV epidemiology from wetland and cage farms of tilapia could be critical to developing the current state of the tilapia farming industry. Additionally, a holistic approach is needed to develop management measures to control the virulence and risk factors of TiPV.

Abstract This study investigated the isolation and bioactivity of Actinomycetia from the soil of Northeast India, a region rich in microbial diversity. A total of 187 presumptive Actinomycetia isolates were obtained and 53 were found to exhibit antimicrobial properties. Phylogenetic analysis based on 16 S rRNA gene sequencing revealed that the isolates were predominantly from the genus Streptomyces. Among these, the strain Streptomyces sp. NP14 (ANP14ARS) demonstrated specific and significant antimicrobial activity against gram-positive bacterial pathogens. The strain was further assayed against a panel of clinically important bacterial pathogens including Methicillin-resistant Staphylococcus aureus (MRSA). Its antimicrobial activity was confirmed using disc diffusion and membrane disruption assay, and its minimum inhibitory concentration was determined to be ≥ 3.12 ± 0.5 µg/ml against MRSA. Chemical analyses using FTIR and GC-MS identified key bioactive compounds, including Pyrrolo[1,2-a]pyrazine-1,4-dione, hexahydro-3-(2-methylpropyl)- and Phenol 3,5-bis(1,1-dimethylethyl), known for their antimicrobial properties. Toxicity studies against animal liver cell lines indicated that the ethyl acetate extract was non-toxic at concentrations below 125 µg/mL, compared to Vancomycin, which was found to be hepatotoxic at similar concentrations. These findings highlight the potential of ANP14ARS as a source of bioactive natural products that is specific towards priority pathogens such as MRSA.

Abstract Phase-separating peptides (PSPs) self-assembling into coacervate microdroplets (CMs) are a promising class of intracellular delivery vehicles that can release macromolecular modalities deployed in a wide range of therapeutic treatments. However, the molecular grammar governing intracellular uptake and release kinetics of CMs remains elusive. Here, we systematically manipulate the sequence of PSPs to unravel the relationships between their molecular structure, the physical properties of the resulting CMs, and their delivery efficacy. We show that a few amino acid alterations are sufficient to modulate the viscoelastic properties of CMs towards either a gel-like or a liquid-like state as well as their binding interaction with cellular membranes, collectively enabling to tune the kinetics of intracellular cargo release. We also demonstrate that the optimized PSPs CMs display excellent transfection efficiency in hard-to-transfect cells such as primary fibroblasts and immune cells. Our findings provide molecular guidelines to precisely program the material properties of PSP CMs and achieve tunable cellular uptake and release kinetics depending on the cargo modality, with broad implications for therapeutic applications such as protein, gene, and immune cell therapies.

Abstract The purpose of this study is to analyze the impact of dimension of dynamic capabilities such as Entrepreneurial orientation (EO), technological innovation (TIN), and digital transformation (DT) and dimension of Stakeholder environment, society, and governance (ESG) practices on environmental performance (EP) in the handicraft industry. This study also investigates the mediating impact of dynamic capabilities such as Technological innovation (TIN) and Entrepreneurial orientation between ESG and EP and has applied the Sobel test. This study also checks the moderating impact of Digital transformation (DT) on EP through TIN. The Sobel test is most suitable for mediation analysis, whether partial or complete mediation. For this study, exploratory research methods , convenience sampling, and tools used a 5-point Likert scale. Therefore, the study was well suited to this type of experimental research, since it used the PLS–SEM path coefficient algorithm and PCA analysis software to examine several correlations simultaneously. A total of 150 respondents participated in this survey who were artisan’s new entrepreneurs and firm owners in the handicraft industry. Because of their outsized influence on emissions worldwide and consumption of resources, businesses worldwide have a responsibility to ensure environmental stability while also working to improve their practices to improve the craft industry as part of the small industry. These findings prove that by adopting ESG principles and Entrepreneurial orientation, the impact of technological innovation supports the environment's performance. The development of eco-friendly goods and procedures, the enhancement of craft productivity, and the evolution of sustainable development strategies are all greatly aided by digital transformation, technological innovation handicraft business environmental sustainability in the face of the escalating difficulties brought on by climate change. For verification and exploration of entrepreneurial orientation, technological innovation, Environmental performance, and environmental social and governance (ESG), this study is supported by the Dynamic capability theory (DCT), stakeholder theory, and especially ESG criteria. The research shows that firms' environmental performance can be significantly improved by emphasizing ESG factors and embracing technological advancements and entrepreneurial orientation.

Potato production in Indonesia has grown very rapidly, making Indonesia the largest potato producer in Southeast Asia. However, there are challenges for farmers in growing potatoes. Such as treating potatoes for various diseases. 2 diseases will occur in potato plants if not treated quickly, namely early blight disease caused by the fungus Alternaria solani and late blight disease caused by the microorganism Phytophthora infestans. The project "Potato Plant Leaf Disease Detector (MR-PoLod)" aims to design an android application that can classify leaves on potato plants into 3 classifications, namely healthy, early, and late blight disease. This application uses the CNN (Convolutional Neural Network) Machine Learning Algorithm because currently, CNN is recognized as the most efficient and effective model in pattern and image recognition tasks. This application uses the Python programming language which is rich in library and framework availability so that it can meet the needs of machine learning and image classification tasks. The total data used for training data, data validation and data testing is 3165 images. With each division of the data process on the training data of 70%, validation of 15% testing of 15% to test the effectiveness of the model that has been created. The performance of MR-PoLod for each class, obtained a precision value, recall, and f1-score of 0.99. Likewise, the accuracy value achieved by the model is 0.99 or 99%. Thus, the expected application can facilitate farmers in classifying diseases on potato plant leaves.

Lactic acid has gained considerable attention globally due to its multi-purpose application. Commercial lactic acid production uses the fungal species Rhizopus oryzae, which produces other organic acids. A crucial point of effective fungal organic acid production is matching the fungal strains' requirements, where the carbon source plays a major role. The highest production rate is achieved when glucose is used as a carbon source. Alternatively, we can apply carbon-rich agricultural residues as carbon sources. Using agricultural waste for lactic acid production provides a sustainable and cost-effective feedstock but also helps to reduce greenhouse gas emissions by diverting waste from landfills and decreasing reliance on fossil fuels. Moreover, polylactic acid (PLA) produced from lactic acid monomers can occur in numerous agricultural applications. We should delve deeper into sustainable methods of using carbon residues to recycle waste, foster the circular economy, and advance sustainable agriculture. Therefore, there is a need for further research on the commercial use of agricultural and food industry wastes for lactic acid production.

Comparing polymer matrix composites with conventional composites, such as continuous fiber-reinforced composites, shows a considerable increase in strength and fracture toughness. In this study, compression molding was used to create hybrid (carbon and silk fabric-reinforced) and carbon fabric-reinforced epoxy matrix composites using a hand layup process. This article discusses the evaluated flexural strength (FS) and plane strain Mode-I fracture toughness of composite materials. The impact of carbon fabric reinforcement on the fracture toughness of these composites was assessed using the obtained results. It was found that the addition of silk fabric reinforcement decreased the hybrid composite’s plane strain Mode-I fracture toughness. Fracture toughness and FS are higher in carbon fabric-reinforced composite than in hybrid composite.

Abstract Black holes and wormholes in the gravitational path integral can be used to calculate the statistics of heavy operators. An explicit example in higher dimensions is provided by thin shells of matter. We study these solutions in 3D gravity, and reproduce the behavior of black holes and wormholes from the dual CFT using the large-c conformal bootstrap. The CFT operator that creates a thin shell black hole is a line defect, so we begin by using the bootstrap to study the statistics of line defects, both at finite c and in the holographic large-c limit. The crossing equation leads to a universal formula for the average high-energy matrix elements of the line defect in any compact, unitary 2d CFT with c > 1. The asymptotics are controlled by a line defect in Liouville CFT at the same value of the central charge. At large c, three distinct quantities are related: the statistics of line defects in holographic CFTs, the individual matrix elements of a line defect in Liouville CFT, and the on-shell action of black holes and wormholes in 3D gravity. The three calculations match for black holes, and if the statistics of the line defects are assumed to be approximately Gaussian, then a class of wormholes is also reproduced by the dual CFT.

Abstract We describe a one-parameter family of Euclidean wormhole solutions with the topology of a compact hyperbolic space times an interval in Einstein gravity minimally coupled to a massless scalar field in AdS d+1 commonly referred to as Einstein-dilaton gravity. These solutions are locally described by the same metric and dilaton profile as the single-boundary Janus domain wall solutions in the same theory which are usually studied in the context of holographic RG flows. The wormholes compute the averaged product of partition functions of CFTs on either boundary deformed by different marginal couplings to the scalar operator dual to the dilaton. We observe that the renormalised volumes of these wormholes increase monotonically with the difference in the marginal couplings on the boundary thereby showing that the pair of CFTs on the boundaries get increasingly decorrelated as the difference in the marginal couplings increases. We use the partition functions of the three-dimensional wormhole solutions to determine the variance of the OPE data of local operators between the marginally deformed 2d CFTs and quantify how the variance decays with the difference in marginal couplings. In addition, a family of wormholes sourced by a thin shell of dust determine how the variance of the matrix elements of the dual line defect decays with the difference in marginal couplings. Applying the GKPW dictionary to wormholes, we compute averages of integrated dilaton correlators treating the wormhole amplitude as a functional of the dilaton sources. We observe that the crossed two-point correlators with a dilaton insertion on either boundary decay monotonically with the difference in marginal couplings consistent with the observation that the CFTs increasingly decorrelate as the difference in marginal couplings grows.

Abstract Background One of the most promising approaches for early and more precise disease prediction and diagnosis is through the inclusion of proteomics data augmented with clinical data. Clinical proteomics data is often characterized by its high dimensionality and extremely limited sample size, posing a significant challenge when employing machine learning techniques for extracting only the most relevant information. Although there is a wide array of statistical techniques and numerous analysis pipelines employed in proteomics data analysis, it is unclear which of these methods produce the most efficient, reproducible, and clinically meaningful results. Abstract The integration of biomolecules into supramolecular nanostructures forms the basis of the natural world. Naturally occurring liquid-liquid phase separation resulting in biomolecular condensates has inspired the formation of biomolecule-based smart materials with multi-dimensional applications. A non-covalent bio-condensation between biomass DNA and guanosine monophosphate (GMP) has been described, mimicking chromatin folding and creating a unique “all-nucleic” DNA-GMP condensates. These condensates initiate the formation of G-quadruplex-based superstructures, assembling into super-helical fibres driven by synergistic hydrogen bonding and stacking, which have been thoroughly investigated. This simple, one-step method for the bio-condensation of biomass DNA leads to an “all-nucleic” hydrogel with higher-order self-assembly and excellent mechanical properties. While most of the reported DNA based biomaterials, including hydrogels, require precisely sequenced and molecularly architectured DNA building blocks, we have developed a simple, universal, and facile bio-condensation method that utilizes biomass DNA acquired from any bio-resource to fabricate DNA hydrogels. The hydrogel efficiently encapsulates and sustains the release of both hydrophilic and hydrophobic drugs, demonstrating its competency as a drug carrier. We believe this energy-efficient and low-cost method represents a new technique for using biomass DNA as building blocks for the next generation of soft materials.

Abstract The mechanisms through which systemic inflammation exerts its effect on the central nervous system (CNS) are still not completely understood. Exosomes are small (30 to 100 nm) membrane-bound extracellular vesicles released by most of the mammalian cells. Exosomes play a vital role in cell-to-cell communication. This includes regulation of inflammatory responses by shuttling mRNAs, miRNAs, and cytokines, both locally and systemically to the neighboring as well as distant cells to further modulate the transcriptional and/or translational states and affect the functional phenotype of those cells that have taken up these exosomes. The role of circulating blood exosomes leading to neuroinflammation during systemic inflammatory conditions was hereby characterized. Serum-derived exosomes from LPS-challenged mice (SDEL) were freshly isolated from the sera of the mice that were earlier treated with LPS and used to study the effects on neuroinflammation. Exosomes isolated from the sera of the mice injected with saline were used as a control. In-vitro studies showed that the SDEL upregulate pro-inflammatory cytokine gene expression in the murine cell lines of microglia (BV-2), astrocytes (C8-D1A), and cerebral microvascular endothelial cells (bEnd.3). To further study their effects in-vivo, SDEL were intravenously injected into normal adult mice. Elevated mRNA expression of pro-inflammatory cytokines was observed in the brains of SDEL recipient mice. Proteomic analysis of the SDEL confirmed the increased expression of inflammatory cytokines in them. Together, these results demonstrate and strengthen the novel role of peripheral circulating exosomes in causing neuroinflammation during systemic inflammatory conditions.

Abstract Biomaterials can act as pro- or anti-inflammatory agents. However, effects of biomaterials crystallinity on immune responses are poorly understood. We demonstrate that the adjuvant-like behaviour of covalent organic framework (COF) biomaterial is dependent on its crystallinity. COF crystallinity is inversely correlated with the activation of mouse and human dendritic cells (DC), but with antigen presentation by mouse DCs only. Amorphous COFs upregulates NFkB, TNF, and RIG-I signalling pathways, as well as the chemotaxis-associated gene Unc5c, when compared to crystalline COFs. Meanwhile, Unc5c inhibition disrupts the correlation between crystallinity and DC activation. Furthermore, COFs with the lowest crystallinity admixed with chicken ovalbumin (OVA) antigen prevent OVA-expressing B16F10 tumour growth in 60% of mice, with this protection associated with the induction of antigen-specific, pro-inflammatory T cell. The lowest crystalline COFs admixed with TRP2 antigen can also prevent non-immunogenic YUMM1.1 tumour growth in 50% of mice. These findings demonstrate that the crystallinity of biomaterials is an important aspect to consider when designing immunotherapy for pro- or anti-inflammatory applications.

Abstract Northern forests are an important carbon sink, but our understanding of the driving factors is limited due to discrepancies between dynamic global vegetation models (DGVMs) and atmospheric inversions. We show that DGVMs simulate a 50% lower sink (1.1 ± 0.5 PgC yr−1 over 2001–2021) across North America, Europe, Russia, and China compared to atmospheric inversions (2.2 ± 0.6 PgC yr−1). We explain why DGVMs underestimate the carbon sink by considering how they represent disturbance processes, specifically the overestimation of fire emissions, and the lack of robust forest demography resulting in lower forest regrowth rates than observed. We reconcile net sink estimates by using alternative disturbance-related fluxes. We estimate carbon uptake through forest regrowth by combining satellite-derived forest age and biomass maps. We calculate a regrowth flux of 1.1 ± 0.1 PgC yr−1, and combine this with satellite-derived estimates of fire emissions (0.4 ± 0.1 PgC yr−1), land-use change emissions from bookkeeping models (0.9 ± 0.2 PgC yr−1), and the DGVM-estimated sink from CO2 fertilisation, nitrogen deposition, and climate change (2.2 ± 0.9 PgC yr−1). The resulting ‘bottom-up’ net flux of 2.1 ± 0.9 PgC yr−1 agrees with atmospheric inversions. The reconciliation holds at regional scales, increasing confidence in our results.

Abstract Background Pregnant and parenting youth in Canada face multiple challenges in fulfilling their parenting responsibilities while meeting their own unique needs. The COVID-19 pandemic and its associated public health restrictions exacerbated their challenges by reducing access to support networks, limiting opportunities to promote growth and well-being, and destabilizing their financial and economic standing. The impacts of the pandemic on the mental, physical, and emotional health of individuals are well-documented in the literature. However, there is limited research on how the pandemic influenced the lives, health, and well-being of pregnant and parenting youth and their children in Canada. Therefore, we aimed to examine the wider impacts (social and structural determinants that directly or indirectly impact health and well-being) of the pandemic and its implications for this population. Methods We used a constructivist grounded theory approach to explore the experiences of pregnant and parenting youth during the pandemic and how certain social determinants of health intersected to shape health outcomes and influence their ability to access services. We conducted qualitative interviews using a semi-structured interview guide over Zoom. Using grounded theory principles, we inductively analyzed the data and categorized our findings into themes. Results We recruited a total of 50 participants from different regions in Canada. The findings from the interviews were categorized into the following themes: (1) “They bear the burden alone”: Struggles of pregnant and parenting youth, (2) “How will I live the next day?”: Concerns and consequences arising from the pandemic, and (3) “Try to keep your mask on”: Navigating healthcare services while maintaining public health guidelines. Our findings highlight the various challenges encountered by the youth during the pandemic, particularly those associated with financial and economic constraints, lack of accessible health and social services, and increased stress from parenting responsibilities. Conclusion Pregnant and parenting youth in Canada encountered various barriers while accessing health and social services, maintaining their well-being, and connecting with reliable sources of support during the pandemic. Our study findings emphasize the urgent need for adaptable and comprehensive support systems explicitly catering to the needs of pregnant and parenting youth during crises, such as the pandemic.

Introduction The Kayakalp guidelines for public healthcare facilities under the Swachh Bharat Abhiyan (Clean India Mission) focus on improving sanitation, cleanliness, and infection control at public hospitals in India. This study was conducted in a 960-bed tertiary-level teaching hospital in eastern India. Housekeeping has been a challenge in public institutions, with factors like overcrowding and resource constraints. Tobacco and betel nut chewing, spitting, poor sanitation practices, and open urination are major challenges in ensuring sanitation at the hospital. The research objective was to study the implementation of the Kayakalp guidelines for quality improvement in housekeeping services at the institution. Methods A pre- and post-interventional study was conducted using the Plan-Do-Check-Act (PDCA) quality tool. Plan phases included the gap assessment using the Kayakalp checklist with numerical scoring. Necessary interventions were done under three headings: structure, processes, and outcomes in the “Do” phase. The “Check” phase included monitoring of the activities followed by the “Act” phase, which included a review of the action plan. External experts nominated by the government conducted the final assessments and recommended it as one of the cleanest hospitals. Results A 360-degree improvement was observed in hospital services, with assessment score improvement from 73.68% to 95.0%. The institution received the first prize in 2020 and 2021 and runners-up Kayakalp National Award under category “B” (Institute of National Importance) Hospitals in 2019. Conclusion The implementation of Kayakalp guidelines of the Government of India proved to be efficient in the improvement of housekeeping and infection control practices in the institution.

Abstract A growing number of humans have suffered severe chronic illnesses, which has caused a boost in the requirement for diagnostic and medical treatment procedures that are both accurate and fast. Improved patient conditions and enhanced Decision-Making Systems (DMS) for healthcare professionals are the primary objectives of the Clinical Decision Support System (CDSS) recommended in this research article. The main drawback of traditional Machine Learning (ML) techniques is their failure to predict reliably. To solve this problem, the proposed model creates an Ensemble Extreme Learning Machine (EN-ELM) algorithm that combines predictors trained on several different data sets. This lowers the chance of overfitting. The suggested CDSS uses many different data processing methods, including Adaptive Synthetic (ADASYN) and isolation Forest (iForest), which fix problems like outliers and class imbalance. This approach significantly enhances the framework’s classification performance. Also, the CDSS is compatible with an EC model, which enables real-time computation while minimizing the requirement for integrated systems. The recommended CDSS applies iForest and ADASYN to execute large-scale trials validating high standards of accuracy across numerous datasets. Researchers concluded that a suitable ELM classification threshold of 85% is the most effective, which substantially boosts the accuracy of the predictive model. When applied to various medical datasets, such as Hepatocellular Carcinoma (HCC), Cervical Cancer, Chronic Kidney Disease (CKD), Heart Disease, and Arrhythmia, the EN-ELM achieved accuracy rates of 99.36%, 98.15%, 97.85%, 97.06%, and 96.72%, respectively. By measuring this progress, the CDSS could dramatically improve the accuracy of chronic illness diagnosis and treatment, which similarly affects clinicians.

Abstract Background The PKD2, encoding polycystin-2 (PC2) protein, is second major genetic determinant of autosomal dominant polycystic kidney disease (ADPKD) after PKD1. However, the structural and functional consequences of genetic variants in PKD2 remain poorly understood. Given the complexity and heterogeneous nature of ADPKD, understanding its pathogenesis at cellular and molecular levels is vital for deciphering genotype–phenotype correlations and disease severity, thus informing patient-centered treatments. We analyzed missense variants of PKD2 to assess their impact on RNA structure using computational tools and explored associated protein structure dynamics through MD simulation. Results Our findings reveal distinct structural alterations and dynamic behaviors associated with specific missense variants. The variants such as c.1789C > A (p.L597M), c.1109G > A (p.S370N), c.1849C > A (p.L617I), and c.646 T > C (p.Y216H) induced major changes not only in RNA structure and accessibility profile but also in protein structure dynamics. In contrast, variants such as c.915C > A (p.N305K), c.1354A > G (p.I452V), and c.568G > A (p.A190T) resulted in minor alterations in RNA structure but exhibited noticeable effects on certain parameters of protein structure dynamics. Conclusion This study suggests the multifaceted impact of these missense variants on both RNA and protein levels. It lays the groundwork in identifying high-impact variants in terms of pathogenicity and prioritizing these for further implications in understanding disease heterogeneity and eventually contributing to the development of targeted therapeutic interventions for ADPKD. Abstract Although significant progress has been achieved in elucidating the genetic architecture of systemic lupus erythematosus (SLE), identifying genes underlying the pathogenesis has been challenging. The NZM2410-derived lupus susceptibility Sle3 locus is associated with T cell hyperactivity and activated myeloid cells. However, candidate genes associated with these phenotypes have not been identified. Here, we narrow the Sle3 locus to a smaller genomic segment (Sle3k) and show that mice carrying Sle3k and Sle1 loci developed lupus nephritis. We identify Klf13 as the primary candidate gene that is associated with genome-wide transcription changes resulting in higher levels of proinflammatory cytokines, enhanced T cell activation, and hyperresponsiveness of myeloid cells. Correspondingly, Klf13 –/– mice display repression of genes involved in mediating immune activation, including key proinflammatory cytokines/chemokines in T cells and dysregulation in cytokine signaling pathways in myeloid cells in response to toll receptor ligands. Klf13 upregulation is associated with increased production of RANTES, a key chemokine in lupus nephritis, in activated T cells and the kidneys of lupus-prone mice. In sum, our findings reveal Klf13 as a key gene in the Sle3 interval in mediating lupus pathogenesis that may have implications in the rational design of new therapies for SLE.

Background: Lumbosacral transitional vertebrae (LSTV) or lumbarization of sacral vertebrae and sacralization of lumbar vertebrae are congenital variations. These are associated with alterations in anatomy and biomechanics of spinal and paraspinal structures, which lead to low back pain (LBP). Aims and Objectives: This radiological study was designed to identify the prevalence of LSTV in patients presenting with LBP and to classify the anatomical variations in LSTV according to the Castellvi classification. Materials and Methods: This hospital-based, cross-sectional, observational cohort study was conducted in the Department of Radiodiagnosis, in different medical colleges and hospitals of West Bengal. The study was done on 1500 patients aged 20–75 years with a history of LBP. Antero-posterior and lateral lumbosacral radiographs were done to identify LSTV. Results: The present study in West Bengal shows that 24.1% of subjects had LSTV among LBP patients, Castellvi Type IIA (33.4%) being the most common anatomical variant, followed by Type IB (19.6%). Sacralization (22.3%) was found more common than lumbarization (1.8%). Conclusion: LSTV comprises almost one-fourth of cases of low back ache which can be easily diagnosed by simple radiograph. It has important implications for surgery in the lumbosacral region.

Abstract The objective of this investigation was to improve the diagnosis of breast cancer by combining two significant datasets: the Wisconsin Breast Cancer Database and the DDSM Curated Breast Imaging Subset (CBIS-DDSM). The Wisconsin Breast Cancer Database provides a detailed examination of the characteristics of cell nuclei, including radius, texture, and concavity, for 569 patients, of which 212 had malignant tumors. In addition, the CBIS-DDSM dataset—a revised variant of the Digital Database for Screening Mammography (DDSM)—offers a standardized collection of 2,620 scanned film mammography studies, including cases that are normal, benign, or malignant and that include verified pathology data. To identify complex patterns and trait diagnoses of breast cancer, this investigation used a hybrid deep learning methodology that combines Convolutional Neural Networks (CNNs) with the stochastic gradients method. The Wisconsin Breast Cancer Database is used for CNN training, while the CBIS-DDSM dataset is used for fine-tuning to maximize adaptability across a variety of mammography investigations. Data integration, feature extraction, model development, and thorough performance evaluation are the main objectives. The diagnostic effectiveness of the algorithm was evaluated by the area under the Receiver Operating Characteristic Curve (AUC-ROC), sensitivity, specificity, and accuracy. The generalizability of the model will be validated by independent validation on additional datasets. This research provides an accurate, comprehensible, and therapeutically applicable breast cancer detection method that will advance the field. These predicted results might greatly increase early diagnosis, which could promote improvements in breast cancer research and eventually lead to improved patient outcomes.

Abstract This study focuses into strategizing the usage of self-reacting bobbin tool friction stir welding (SRBT-FSW) to obtain consistent microstructural homogeneity along the thickness of AA6061 aluminium alloy (AA) thick plates during welding. The SRBT-FSW technology, distinguished by its dual-shoulder design, represents a significant step forward in FSW by eliminating the requirement for a backing anvil and promoting balanced heat distribution. This study seeks to address the issues of maintaining uniform microstructural characteristics throughout the weld zone, which is crucial for the mechanical performance and durability of welded joints in structural applications. The experimental study entails the systematic welding of AA6061 plates of 6 mm thickness using a self-reacting bobbin tool under a fixed processing condition. Electron backscatter diffraction (EBSD) was used to characterize the grain structure and phase distribution over the weld. Mechanical parameters like as tensile strength and hardness were determined to establish correlations between microstructure and mechanical performance. The results demonstrated that SRBT-FSW significantly enhances microstructural homogeneity across the weld zone, leading to improved mechanical properties. In the Bottom Zone (BZ), a refined grain structure with an average grain size (AGS) of 3.53 µm and a random or weak texture was observed, contributing to enhanced hardness and mechanical performance, with an ultimate tensile strength (UTS) of 220 MPa. In contrast, the Top Zone (TZ) exhibited a coarser AGS of 4.33 µm with a pronounced {111} crystallographic texture, resulting in a slightly lower UTS of 205 MPa. The Middle Zone (MZ), influenced by the greater heat input from both the TZ and BZ, showed an intermediate AGS of 3.99 µm, predominantly oriented along the {101} plane, and achieved a UTS of 194 MPa, with a slight reduction in ductility. This study highlights the potential of self-reacting bobbin tool friction stir welding as a reliable method for making high-quality, homogeneous welds in thick aluminium plates and paving way for their wider application in the aerospace, automotive, and shipbuilding industries, where homogeneous microstructural qualities are of significant importance.

There is a growing interest in understanding and establishing forest nurseries in swamp ecosystem areas of Bangladesh. This study aims to evaluate the socio-economic and ecological contribution of swamp small-scale tree nurseries. The result revealed that government and non-government organizations established swamp nurseries in Bangladesh. Swamp nursery producer generates an annual average net profit of USD 4745. Small-scale swamp nursery owners benefited in multiple forms: subsistence, house construction, savings purposes, and, most importantly, expansion of their businesses. However, the ecological aspect of seedling production was not satisfactory because of the lack of facilities such as skilled labor supply, access to high-quality germplasm, technical skills, nursery facilities, etc. In the nurseries observed, only two dominant swamp tree species, locally known as Hijol (Barringtonia acutangular) and Koroch (Pongamia pinnata) seedlings were found. In most cases, those seedlings were morphologically unhealthy, and the root structure was deformed, which was identified as a major problem of reforestation programs in wetland areas. Therefore, to make the reforestation program successful and create livelihoods for the rural poor in the wetland areas, there is an urgent need to develop quality seedlings-based swamp nurseries, which are only possible through proper nursery management techniques.

Diabetic nephropathy (DN) is a serious diabetes-related complication leading to kidney damage. Cinnamomum tamala (CT), traditionally used in managing diabetes and kidney disorders, has shown potential in treating DN, although its active compounds and mechanisms are not fully understood. This study aims to identify CT’s bioactive compounds and explore their therapeutic mechanisms in DN. Active compounds in CT were identified using the Indian Medicinal Plants, Phytochemicals and Therapeutics database, and their potential targets were predicted with PharmMapper. DN-related targets were sourced from GeneCards, and therapeutic targets were identified by intersecting the compound–target and disease–target data. Bioinformatics analyses, including the Kyoto Encyclopedia of Genes and Genomes and Gene Ontology enrichment studies, were performed on these targets. A protein–protein interaction network was constructed using STRING and Cytoscape. Molecular docking and dynamics simulations validated the most promising compound–target interactions. Six active compounds in CT were identified, along with 347 potential therapeutic targets, of which 70 were DN-relevant. Key targets like MMP9, EGFR, and AKT1 were highlighted, and the PPAR and PI3K-AKT signaling pathways were identified as the primary mechanisms through which CT may treat DN. CT shows promise in treating DN by modulating key pathways related to cellular development, inflammation, and metabolism.

Background: The design of the current research work was to formulate mouth-dissolving films (MDFs) of aceclofenac (ACF) to improve patient compliance and convenience for older and younger patients, ultimately leading to improved therapeutic outcomes. Method: Evaluations were conducted on film formers such as HPMC and MC and film modifiers such as PEG and starch acting as solubilizing agents. Results: The physicomechanical qualities, in vitro disintegration time, and in vitro dissolving characteristics of the produced MDFs were assessed. Good mechanical qualities, including as tensile strength, folding durability, and percentage elongation, were demonstrated by every created MDF. FTIR, SEM, and X-RD analyses were used to assess MDFs. In contrast to other formulations, MDFs containing F8 provided superior dissolving properties. Conclusion: When pitted against other mixtures, the MDFs with sodium alginate (5%), methylcellulose (5%), and hydroxypropyl methylcellulose (HPMC)(5%) showed superior dissolving capabilities. In contrast to other mixtures, the F8 mixture, including HPMC, sodium alginate, and methylcellulose, demonstrated a complete and accelerated dissolution within 50 seconds. The mechanism behind this release is diffusion, as indicated by the release kinetics data.

This research is motivated by the registration of candidacy through individual channels which are required to use access through the Candidacy Information System (SILON). However, there are still many obstacles in the system starting from the location of implementation such as in Samosir Regency with network conditions that have not been evenly stable and problems with the system. This research aims to explain how the use of the nomination information system at the stage of registration of candidates through individual channels in Samosir Regency and analyze what are the obstacles in using the system. This research method is descriptive qualitative which presents the facts found comprehensively with data collection techniques through interviews, documentation, and observation. The results showed that the Samosir Regency KPU has implemented a nomination information system in the 2020 Regional Head election for individual candidates in the Samosir Regency by the technical instructions and stages specified. However, the use and workings of the system have not been optimal to support the efficiency and principles of the KPU with frequent system changes and environmental conditions that have not been supportive. In every process of use, it is necessary to evaluate policies in the nomination information system.

Abstract The effective lifetime of the B s 0 $$ {\textrm{B}}_{\textrm{s}}^0 $$ meson in the decay B s 0 → J / ψK S 0 $$ {\textrm{B}}_{\textrm{s}}^0\to \textrm{J}/{\uppsi \textrm{K}}_{\textrm{S}}^0 $$ is measured using data collected during 2016–2018 with the CMS detector in s $$ \sqrt{s} $$ = 13 TeV proton-proton collisions at the LHC, corresponding to an integrated luminosity of 140 fb −1. The effective lifetime is determined by performing a two-dimensional unbinned maximum likelihood fit to the B s 0 $$ {\textrm{B}}_{\textrm{s}}^0 $$ meson invariant mass and proper decay time distributions. The resulting value of 1.59 ± 0.07(stat) ± 0.03(syst) ps is the most precise measurement to date and is in good agreement with the expected value.

Abstract Diboson production in association with jets is studied in the fully leptonic final states, pp → (Z/γ *)(Z/γ *) + jets → 2ℓ2ℓ′ + jets, (ℓ, ℓ′ = e or μ) in proton-proton collisions at a center-of-mass energy of 13 TeV. The data sample corresponds to an integrated luminosity of 138 fb −1 collected with the CMS detector at the LHC. Differential distributions and normalized differential cross sections are measured as a function of jet multiplicity, transverse momentum p T, pseudorapidity η, invariant mass and ∆η of the highest-p T and second-highest-p T jets, and as a function of invariant mass of the four-lepton system for events with various jet multiplicities. These differential cross sections are compared with theoretical predictions that mostly agree with the experimental data. However, in a few regions we observe discrepancies between the predicted and measured values. Further improvement of the predictions is required to describe the ZZ+jets production in the whole phase space.

Pigmented epithelioid melanocytoma (PEM) is a rare and diagnostically challenging skin neoplasm. Differentiating this lesion from melanoma is challenging due to overlapping clinical, histopathological, and immunohistochemical features. Some PEMs have a benign course, while others may be associated with more aggressive behaviour and have potential for local recurrence or distant metastasis. So, careful evaluation and follow-up are crucial for individuals with a diagnosis of PEM. This case study focuses on a 42-year-old woman with a gradually enlarging, dark, pigmented gluteal lesion. Histopathological analysis following complete excision showed nodular proliferation of spindled and epithelioid melanocytes in the dermis, extending into subcutaneous fat. Cellular atypia or mitotic figures were absent, confirming the diagnosis as PEM. This case underscores the intricate balance between monitoring and intervention in complex dermatological cases, shedding light on diagnostic challenges and ultimately striving for improved patient care.

Introduction Forensic odontology plays a pivotal role in identifying individuals in legal contexts. This study analyses the efficacy of lip prints, tongue prints and mandibular canine width in determining gender, aiming to contribute to the evolving field of forensic dentistry. Methods A cross-sectional study was conducted involving 100 participants, analysing lip prints based on Tsuchihashi’s classification, recording tongue shapes and features through the use of alginate impressions and measuring mandibular canine width using a vernier calliper. The findings were reported in accordance with the Strengthening the Reporting of Observational Studies in Epidemiology checklist. Results In lip prints, Tsuchihashi’s Type I lip print was predominantly found in females, whereas Tsuchihashi’s Type III prevailed in males. Tongue shapes revealed a predominance of pentagonal shapes in males and ovoid shapes in females. The lingual apex was blunt in males and sharp in females. Mandibular canine width was higher in males than females on both the left and right sides. Conclusion The findings of this study support the use of lip prints, tongue prints and canine width in determining gender as ancillary and supporting methods.

Introduction Upper ureteric stricture is always a challenging case to treat for any urologist. Due to chronic inflammation and multiple interventions, it becomes a complex entity to treat. Buccal Mucosal Graft (BMG) Ureteroplasty is a reconstructive surgery used to treat upper ureteric stricture but the results and experience with this modality is less explored so far. We present here our study of 16 cases of BMG ureteroplasty and its outcomes done by the laparoscopic and robotic approaches. Patients and Methods We analysed 16 cases of BMG ureteroplasty, which were performed both laparoscopically and robotically. All these cases were long ureteric strictures, not amenable to excision or endoscopic intervention. We performed using an onlay BMG without complete mobilisation of the ureter. The omentum or nearby fat was used as a bed for onlay BMG. Results All 16 patients underwent onlay ureteroplasty. The reconstructed ureter was wrapped with omentum in nine of the cases, while in seven patients, nearby fat was used. The median stricture length was 5.28 cm, and the median operative time was 143.5 min. The mean operative time was 143.5 min. 15 of 16 (93.75%) cases were successfully clinically and radiologically on follow-up. Conclusion Long-segment upper ureteric strictures are a difficult entity to operate on. BMG ureteroplasty is a safe and effective way of managing such strictures. Robot-assisted ureteroplasty provides the benefits of improved ergonomics, easy manoeuvrability and precision surgery to the patients. Our experience with both laparoscopic and robotic ureteroplasty would encourage urologists all over to use BMG ureteroplasty as an effective long-term procedure for ureteral reconstruction.

Background Alveolar ridge is dynamic in nature. Preservation of residual alveolar ridge is always desirable. Bone grafts and platelet concentrates can be used for socket preservation. Maintenance of bone volume is essential and various ridge augmentation techniques are used for desirable functional and esthetic outcomes. Aim Evaluation and comparison of the efficacy of autogenous dentin block versus allograft block in preserving alveolar ridge dimension with 6-month follow-up. Materials and Methods Twenty subjects indicated for tooth extraction were randomly divided into three groups. In Group I, autogenous dentin graft with platelet-rich fibrin (PRF) membrane was used as socket filling material, Group II: allogenic bone graft with PRF membrane, or Group III: control group (socket left ungrafted). Clinical and radiographic examinations were done using periodontal probe and cone-beam computed tomography. Results On intragroup comparison of socket size, the mean for Group I was found to be 33.270 mm2 at baseline and 25.358 mm after 12 months for Group II was found to be 53.250 mm2 at baseline and socket fill was 41.875 mm2 at 12 months. Group III was found to be 43.483 mm2 at baseline, and socket fill was 15.063 mm2 at 12 months. On intergroup comparison, the values were found to be statistically nonsignificant both at baseline (12 months postoperatively. The crestal bone loss was 0.975 for Group I, 1.075 for Group II, and 2.925 mm for Group III. Crestal bone loss was seen highest in Group III and lowest for Group I. The mean percentage of socket fill was found to be 80.01% for Group I, 76.13% for Group II, and 34.03% for Group I. Conclusion A significant amount of radiographic socket fill was appreciated over 12 months in all the three groups. The percentage of socket fill in Group I was 80.01% and in Group II was 76.13% and the difference between them was statistically nonsignificant. Statistically significant socket fill was seen for both Group 1 and Group 2.

Abstract Ectopic thyroid tissue along the line of descent of thyroid from foramen caecum along thyroglossal duct to the normal anatomic location in neck has been reported. Ectopic thyroid tissue in adrenal gland (ETTAG) is rarely encountered and very few cases have been reported in literature. The most common differential diagnosis to be considered when thyroid follicles are noted in adrenal gland are metastasis from a thyroid malignancy or a teratoma or an ectopic thyroid tissue in adrenal gland. We present a case of an adrenal incidentaloma in a young pregnant female which was diagnosed to be ectopic thyroid tissue in adrenal gland. The review of literature of similar cases of this rare embryological aberrance is discussed. To our knowledge, only 16 such cases have been documented in literature and our case is the first one from Indian subcontinent and the largest documented with respect to size.

Abstract Background Mesenchymal stem cells (MSCs) have emerged as promising candidates for immune modulation in various diseases that are associated with dysregulated immune responses like Graft-versus-Host-Disease (GVHD). MSCs are pleiotropic and the fate of MSCs following administration is a major determinant of their therapeutic efficacy. Methods Human MSCs were derived from bone marrow (BM) and Wharton’s Jelly (WJ) and preconditioned through exposure to hypoxia and induction of apoptosis, either sequentially or simultaneously. The immune programming potential of preconditioned MSCs was evaluated by assessing their effects on T cell proliferation, induction of Tregs, programming of effector T-cell towards Th2 phenotype, macrophage polarization in the direct co-culture of MSCs and aGVHD patients-derived PBMNCs. Additionally, efferocytosis of MSCs and relative change in the expression of immunomodulatory soluble factors were examined. Results Our study demonstrated that hypoxia preconditioned apoptotic MSCs (BM-MSCs, WJ-MSCs) bear more immune programming ability in a cellular model of acute Graft-versus-Host-Disease (aGVHD). Our findings revealed that WJ-MSCsHYP+APO were superior to BM-MSCsHYP+APO for immune regulation. These induced the differentiation of CD4+T-cell into Tregs, enhanced Th2 effector responses, and simultaneously mitigated Th1- and Th17 responses. Additionally, this approach led to the polarization of M1 macrophages toward an M2 phenotype. Conclusion Our study highlights the potential of WJ-MSCs conditioned with hypoxia and apoptosis concurrently, as a promising therapeutic strategy for aGVHD. It underscores the importance of considering MSC apoptosis in optimizing MSCs-based cellular therapy protocols for enhanced therapeutic efficacy in aGvHD. Graphical Abstract

Abstract Background Woolly apple aphid (Eriosoma lanigerum Hausmann) (Homoptera: Aphididae) infests the aerial and edaphic portions of apple trees. It is responsible for the serious qualitative and quantitative reduction in apple fruits. This pest is mainly managed by chemical insecticides, so this present study was focused on the isolation of native entomopathogenic fungi (EPF), studying their growth characteristics and virulence against woolly apple aphids. Results Clonostachys rogersoniana, as an EPF from an insect cadaver, was isolated and characterized, using morphological, microscopic, and molecular techniques. To the best of our knowledge, this is the first report of the occurrence of this fungus from India and it is named “C. rogersoniana isolate PUSACR01”. The radial growth and conidial yield of this isolate were excellent on PDAY among ten different culture media, whereas the relative growth rate and conidial germination were good on PDA and CA. The isolated fungus showed 85 and 86.67% mortality rates in adult females of the woolly apple aphid on day 11 at a concentration of 1.0 × 107 and 1.0 × 108 conidia ml−1, respectively. The LC50 values determined by probit regression analysis for days 7, 9, and 11 were 1.12 × 107, 2.52 × 106 , and 4.74 × 105 conidia ml−1 , respectively, while LT50 for concentrations of 1.0 × 105, 1.0 × 106, 1.0 × 107, and 1.0 × 108 conidia ml−1 were 18.73, 9.39, 6.71, and 5.03 days, respectively. Conclusion Based on the present study, C. rogersoniana would be considered a good biological control agent against the woolly apple aphid in apple orchards.

Abstract Background Anaemia continues to be a pressing public health concern in India. Despite the high burden of anaemia among lactating adolescents in the country, research addressing this concern among this population remains scarce. Hence, this study attempted to examine the prevalence and determinants of anaemia among lactating adolescents in India. Methods We used data from three latest rounds of the National Family Health Survey (NFHS) carried out in 2005-06, 2015-16, and 2019-21 and analysed a pooled sample of 10,689 lactating adolescents aged 15–19 years from these three rounds. Bivariate statistics was used to analyse the prevalence of anaemia according to the background characteristics of lactating adolescents. Further, binary multivariate logistic regression was carried out to examine the factors associated with anaemia in the study population. Results Three in every five lactating adolescents from the pooled sample were found to have anaemia (66.98% in NFHS-3, 61.08% in NFHS-4 and 66.27% in NFHS-5). Lactating adolescents with a higher education had a 30% lower likelihood [adjusted odds ratio (AOR): 0.70, 95% CI: 0.49–1.01, p = 0.050] of being anaemic compared to those with no education. The likelihood of being anaemic among the poorest quintile was 41% (AOR: 1.41, 1.12-1.77; p = 0.003) higher than those from the richest quintile. Lactating adolescents from the Scheduled Tribe category had higher odds (AOR: 1.29, 95% CI: 1.01–1.46, p = 0.001) of anaemia than those categorised as ‘Others’. Those living in rural areas were 22% more likely to suffer from anaemia (AOR: 1.22, 95% CI: 1.00-1.26, p = 0.050) compared to their urban counterparts. Additionally, underweight lactating adolescents were 15% more likely (AOR: 1.15, 95% CI: 1.05–1.26, p = 0.002) to be anaemic than those with normal BMI. Odds of being anaemic were higher among the ones receiving supplementary nutrition during pregnancy (AOR: 1.18, 95% CI: 1.08–1.29, p < 0.001) in contrast to those not receiving. Conclusion The consistently high prevalence of anemia among lactating adolescents, especially among those with lower education level, rural residence, underweight status, and belonging to ST category and poorest quintile, underscore the urgent need for targeted public health interventions to address and reduce anemia in this population.

Background and Objectives: Functional dissociative seizures (FDS) are very common in neurology clinics, they straddle both neurology and psychiatry. In India, scarce resources and guidelines are available for effectively conveying their diagnosis and management, which leads to a substantial gap in care. We aimed to provide an instructional module for FDS, tailored to clinicians, patients, and caregivers in Hindi, to enhance communication of diagnosis and facilitate suitable management strategies. Methods: The instructional module development process was based on clinical settings and expert opinions. A standard translation process was performed to ensure the linguistic and cultural equivalence. Validation was carried out by a panel of 15 validators, comprising 13 neurologists and two psychiatrists, to identify key psychoeducation components and their significance. They were provided a questionnaire consisting of five questions that were related to completeness, understandability, legibility, clarification, and usefulness of educational material for clinicians, patients, and caregivers. Items were rated using a 5-point Likert scale. Results: There was agreement on four items (Q1, Q3, Q4, and Q5), whereas approximately 85% of the validators agreed on the remaining one item (Q2). The instructional module demonstrated high content validity as indicated by both item level-content validity index (0.98) and scale-level content validity index (0.97), alongside content validity ratio and modified kappa statistics. Conclusions: The developed Hindi instructional module for FDS serves as a valuable tool to enhance the communication of FDS diagnosis and dispel misconceptions. This offers a comprehensive resource for clinicians, patients, and caregivers in India, potentially bridging gap in FDS care.
