Background: Urinary tract infection (UTI) stands out as the third-most common infection following gastrointestinal and respiratory tract infections. Over the past decade, the biomarker procalcitonin (PCT) has gained prominence to facilitate the detection of bacterial infections and reduce excessive antibiotic exposure. Objective: The objective of this study was to mitigate the overuse of antibiotics, by promoting the noninitiation or early discontinuation of empirical antibiotics, which would significantly help minimize the proliferation of multidrug-resistant bacteria. Methodology: A prospective observational study was carried out at the tertiary care center in the Department of General Medicine of Kalinga Institute of Medical Sciences, Bhubaneswar, involving 200 patients with symptoms of lower UTI such as increased frequency, urgency, burning micturition, retention, and suprapubic tenderness with or without positive urinalysis. Detailed demographic profiles along with symptoms at the time of admission were recorded in a pretested structured format. To determine a positive diagnosis of UTI, signs and symptoms of UTI with or without urinary cultures were tested. The PCT level was estimated using enhanced chemiluminescence technique. Other routine tests such as complete blood count, renal function test, liver function test, urine routine microscopy, culture, chest X-ray, and ultrasonography abdomen pelvis were done and recorded. All patients, who had an initial serum PCT level of < 0.5 ng/mL, were kept under observation with only conservative and symptomatic treatments. Patients were further reviewed for improvement in symptoms and repeat urine microscopy. All patients, who had an initial serum PCT level of > 0.5 ng/mL, were initiated with antibiotics as per the culture and sensitivity reports. Patients were followed up for improvement in symptoms with reports of repeated urinalysis. Results: Our study reported the fact that 9.5% of the patients with initial serum PCT ≥ 0.5 ng/mL showed no improvement in symptoms despite starting antibiotics while significantly higher number of symptomatic patients (60%) with initial serum PCT < 0.5 ng/ml showed improvement in symptoms with conservative treatment without antibiotics. Conclusion: A lower PCT level rules out bacterial invasion and thus can be used as a novel marker in antibiotic stewardship.

EX Lupi, a low-mass young stellar object, went into an accretion-driven outburst in 2022 March. The outburst caused a sudden phase change of ∼112° ± 5° in periodically oscillating multiband lightcurves. Our high-resolution spectra obtained with the High Resolution Spectrograph (HRS) on board the Southern African Large Telescope also revealed a consistent phase change in the periodically varying radial velocities (RVs), along with an increase in the RV amplitude of various emission lines. The phase change and increase in RV amplitude morphologically translates to a change in the azimuthal and latitudinal location of the accretion hotspot over the stellar surface, which indicates a reconfiguration of the accretion funnel geometry. Our three-dimensional magnetohydrodynamic simulations reproduce the phase change for EX Lupi. To explain the observations, we explored the possibility of forward shifting of the dipolar accretion funnel as well as the possibility of the emergence of a new accretion funnel. During the outburst, we also found evidence of the hotspot’s morphology extending azimuthally asymmetrically with a leading hot edge and cold tail along the stellar rotation. Further, our high-cadence photometry showed that the accretion flow has clumps. We also detected possible clumpy accretion events in the HRS spectra that showed episodically highly blueshifted wings in the Ca ii IR triplet and Balmer H lines.

Background: Current evidence on vertical transmission of severe acute respiratory syndrome Coronavirus 2 (SARS-CoV-2) and neonatal outcome among exposed newborns is emerging and posing a challenge for preventive interventions. Perinatal transmission to the neonates especially during breastfeeding and rooming in is also relatively unknown. Methods: This prospective observational study was conducted in Kalinga Institute of Medical Science (KIMS), Odisha state from 1st May to 20th October 2020. A total of 165 neonates born to SARS-CoV-2 infected mothers were enrolled. Real time polymerase chain reaction (RT PCR) testing was done in first 32 neonates in initial 24 h of life. Results: The clinical characteristics of 162 mothers & 165 neonates were analyzed. Mode of delivery was by caesarian section in most (n = 103, 60%) cases. Three (3/32, 9.4%) inborn and 6 outborn neonates were SARS-CoV-2 positive. Thirty-eight (23%) babies needed neonatal intensive care. Clinical characteristics of neonates were meconium-stained amniotic fluid (MSAF [23.63%]), prematurity (16.9%), respiratory distress (10.5%), moderate to severe hypoxic ischemic encephalopathy (3.6%), sepsis (7%) and hyperbilirubinemia (8.7%). Out of 138 stable babies kept on mother side and initiated breast feeding, none of them developed any signs and symptoms attributable to SARS-CoV-2. Five (3%) neonates died in COVID hospital of which one baby was SARS-CoV-2 positive. Conclusion: There was an increased rate of incidences of hypoxic ischemic encephalopathy, meconium stained liquor and cesarean section delivery in COVID hospital. We found a possible vertical transmission in 9.4% cases. None of the neonates developed sign and symptoms of SARS-CoV-2 infection during rooming in and breast feeding.

We present here the results of 8 yr of our near-simultaneous optical–near-infrared spectro-photometric monitoring of the bona fide FU Ori (FUor)-type candidate V2493 Cygnus (V2493 Cyg) starting from 2013 September to 2021 June. During our optical monitoring period (between 2015 October 16 and 2019 December 30), the V2493 Cyg is slowly dimming with an average dimming rate of ∼26.6 ± 5.6 mmag yr ^−1 in the V band. Our optical photometric colors show a significant reddening of the source post the second outburst pointing toward a gradual expansion of the emitting region post the second outburst. The mid-infrared colors, on the contrary, exhibit a blueing trend, which can be attributed to the brightening of the disk due to the outburst. Our spectroscopic monitoring shows a dramatic variation of the H α line as it transitioned from absorption feature to the emission feature and back. Such transition can possibly be explained by the variation in the wind structure in combination with accretion. Combining our time evolution spectra of the Ca ii infrared triplet lines with the previously published spectra of V2493 Cyg, we find that the accretion region has stabilized compared to the early days of the outburst. The evolution of the O i λ 7773 Å line also points toward the stabilization of the circumstellar disk post the second outburst.

We present a comprehensive analysis of the Galactic mid-infrared bubble [HKS2019] E70 (E70) by adopting a multiwavelength approach to understand the physical environment and star formation scenario around it. We identified a small (radius ∼1.7 pc) stellar cluster inside the E70 bubble, and its distance is estimated as 3.26 ± 0.45 kpc. This cluster is embedded in the molecular cloud and hosts massive stars, as well as young stellar objects (YSOs), suggesting active star formation in the region. The spectral type of the brightest star (M1) of the E70 cluster is estimated as O9V, and a circular ring/shell of gas and dust is found around it. The diffuse radio emission inside this ring/shell, the excess pressure exerted by the massive star M1 at the YSO’s core, and the distribution of photodissociation regions, a class I YSO, and two ultracompact H ii regions on the rim of this ring/shell clearly suggest positive feedback of the massive star M1 in the region. We also found a low-density shell-like structure in the ^12 CO( J = 1–0) molecular emission along the perimeter of the E70 bubble. The velocity structure of the ^12 CO emission suggests that the feedback from the massive star appears to have expelled the molecular material, and the subsequent swept-up material is what appears as the E70 bubble.

Autonomous driving and its real-world implementation have been among the most actively studied topics in the past few years. In recent years, this growth has been accelerated by the development of advanced deep learning-based data processing technologies. Moreover, large automakers manufacture vehicles that can achieve partially or fully autonomous driving for driving on real roads. However, self-driving cars are limited to some areas with multi-lane roads, such as highways, and self-driving cars that drive in urban areas or residential complexes are still in the development stage. Among autonomous vehicles for various purposes, this paper focused on the development of autonomous vehicles for garbage collection in residential areas. Since we set the target environment of the vehicle as a residential complex, there is a difference from the target environment of a general autonomous vehicle. Therefore, in this paper, we defined ODD, including vehicle length, speed, and driving conditions for the development vehicle to drive in a residential area. In addition, to recognize the vehicle’s surroundings and respond to various situations, it is equipped with various sensors and additional devices that can notify the outside of the vehicle’s state or operate it in an emergency. In addition, an autonomous driving system capable of object recognition, lane recognition, route planning, vehicle manipulation, and abnormal situation detection was configured to suit the vehicle hardware and driving environment configured in this way. Finally, by performing autonomous driving in the actual experimental section with the developed vehicle, it was confirmed that the function of autonomous driving in the residential area works appropriately. Moreover, we confirmed that this vehicle would support garbage collection works through the experiment of work efficiency.
