Background and purpose: The occurrence of medical errors in healthcare centers is highly significant due to the sensitive nature of providing care and saving patients' lives. Failure to report such errors can result in both financial and moral harm to patients and have adverse effects on the healthcare system. Medical errors have significant clinical and economic consequences and can influence mortality rates. Despite the wide range of medical errors in healthcare delivery, the errors committed by medical staff—due to the close care relationship between them and the patient—are among the most complex issues in the management of treatment systems. Therefore, the purpose of this study was to explore the causes of non-reporting of medical errors in hospitals and methods of encouraging the reporting of such errors. Materials and methods: This qualitative study was conducted using semi-structured, in-depth individual interviews with a conventional content analysis approach. The goal was to identify the factors contributing to the non-reporting of medical errors in 1402 (Iranian calendar year). The study population included doctors, specialists, and providers actively involved in patient safety in the hospital. In other words, individuals with knowledge and valuable experience in this area were interviewed. Data collection was conducted using a semi-structured questionnaire, designed by reviewing the literature and with the assistance of expert professors for the interviews. Sampling was performed purposefully until data saturation was reached. Data saturation was achieved after interviewing 22 experts. After the interviews, items and sub-items were identified through content analysis. Results: Key factors contributing to the failure to report medical errors were identified in two themes, six categories, and 45 items. The extracted themes included organizational and management factors, as well as legal issues. Organizational and management factors encompassed organizational culture, communication, reporting, and monitoring, while legal factors involved the consequences of reporting and control mechanisms. In total, 38 solutions were proposed to encourage the reporting of medical errors. The experts' suggestions for strategies to promote error reporting were identified under two themes, six categories, and 38 items. The themes included organizational and management factors and legal issues. Organizational and management factors covered organizational culture, the reporting process, monitoring, and communication, while legal factors involved the consequences of reporting and control. Conclusion: Errors in medical procedures may occur due to the inherent nature and sensitivity of these procedures. However, it is essential that medical staff consider themselves obligated to minimize errors and reduce harm to patients. Studying the causes of non-reporting of medical errors and understanding the underlying reasons are critical for improving patient safety and the quality of healthcare services. This knowledge can assist policymakers and healthcare practitioners in making informed decisions about error reporting and in delivering higher-quality services to the public. Additionally, it is important to train medical teams on the various types of errors and how to appropriately address them.

Herein, a non-boiling two-phase flow containing air and water through a downward flow in a vertical tube with helical corrugations has been investigated. In this simulation, various flow rates for air and water are considered, and three corrugation pitches 1, 1.5, and 2 cm are included. It can be seen in the results that the pressure drop values decrease with an increase in volume fraction. It should be noted that the reduction of pressure drop values with the reduction of volume fraction (VF) is based on the reduction of the water flow rate, which is visible. By comparing the pressure drop values for each corrugation pitch, it can be seen that as the pitch decreases, the pressure drop values increase significantly. The results for Nusselt number show that Nusselt number decreased with an increase in the volume fraction. By reducing the water flow rate, the intensity of the main flow is reduced the intensity of turbulence is also reduced and the heat transfer coefficient is reduced. Ultimately, the cost-benefit ratio has been utilized to show real results for each studied case.

Abstract Introducing suitable alternatives to the existing fossil fuels can provide insightful aspects of the expected benefits. In the energy sector, the transportation system is one of the key contributors to fossil fuel consumption and greenhouse gas productions. To elaborate on the performance of each introduced alternative, two perspectives are considered; short-term and long-term evaluations. In the short outlook, the PROMETHEE method is used for evaluation. Six scenarios are introduced based on the technical, economic, social, and policy criteria and each scenario benefits from different weights. In this scheme, the impacts of evolutions in the battery applications in the vehicles are investigated. Based on the short-term study, CNG and gasoline are considered the best options for the fuels of vehicles in Iran by taking into consideration the current situation. By viewing the technical and economic criteria, it was concluded that the Li-Ion battery provides better performance in comparison with gasoline in the long run. By 2040, the number of EVs will reach 10% of the overall vehicle production. It is obtained that the benefits of the electrical vehicles’ presence as the alternative to the internal combustion vehicles can provide growing interest in this outlook from 2.02 × 106 US$ in 2025 to 17.55 × 108 US$ in 2040.

Abstract Background To the best of our knowledge, no population-based studies have provided insights into the prevalence of hysterectomy and its risk factors among northern Iranian females. Thus, the present study aimed to investigate the prevalence and sociodemographic determinants of hysterectomy in a large cohort of northern Iranian females. Methods This cross-sectional study included data from the enrollment phase of the Tabari cohort study (TCS). The TCS consists of 10,255 adults (4,149 males and 6,106 females) aged 35–70 years who reside in Sari, Mazandaran, Iran, of which 6103 females were included in the study. Multiple logistic regression analysis was used to search for hysterectomy determinants. Results Our results revealed that the prevalence of hysterectomy among northern Iranian females was 9.7% (595/6103). Additionally, 50–59 (OR: 4.63, 95% CI: 3.57–6.01) and 60–70 (OR: 5.83, 95% CI: 4.28–7.95) age groups, higher socioeconomic levels (OR: 1.66, 95% CI: 1.13–2.42), a history of tubectomy (OR: 1.27, 95% CI: 1.05–1.53), and more gravida (OR: 5.35, 95% CI: 1.62–17.63) were found to increase the odds of hysterectomy, whereas living in mountainous areas (OR: 0.57, 95% CI: 0.43–2.75) and having a job (OR: 0.62, 95% CI: 0.45–0.86) were found to decrease the odds of hysterectomy. Conclusion Older age groups, living in urban areas, higher socioeconomic levels, not having a job, a history of tubectomy, and more gravida were found to increase the odds of hysterectomy.

Background and purpose: The process by which family physicians refer patients to specialized levels of the healthcare system is presented. In other words, the referral system can assure us that the clients of health care services are optimally allocated at the specialized level of services. In addition to providing proper time and providing proper health care for health benefits. The referral process is a specialized outpatient consulting process that determines the course and communication of the individual in three levels of the health system to provide the integrity and continuity of services this study aimed to determine the quality of referral and feedback forms in the medical family physician of Mazandaran province in 2019. Materials and methods: This descriptive cross-sectional study was conducted to examine the form of insurance referral of social security insurance by physicians of urban families and feedback from experts. After receiving the code of ethics and obtaining permission from the relevant organization the researcher paid the social security organization 1397 issued by the physicians in the province. The statistical population consisted of all forms of reference and the feedback from experts to the social security insurance organization to get the right According the report of the department of medical record department of the social security insurance organization of Mazandaran province in1397 has been handed over to that organization in a total of 151075 reference sheets completed to obtain the right of the specialists. the collected data were entered into Excel software for statistical analysis was performed in descriptive statistics using central dices (mean, median, mode) and dispersion index(variance, standard deviation, range of changes, coefficients of variation ) for quantitative variables, and index, percentage, and prevalence for qualitative data. The data were analyzed by using spss-25 software. Results: The study of 3200 cases of referral and referral forms of family physicians and specialists showed that among the Physicians 2321(72.5) were male and 879(27.5) were female. The patient's records in 586(%18.3) were incomplete. Clinical findings Were found in 98(2.7%) cases and paraclinical findings were incomplete in 22 cases (0.7%) and complete in 171 cases (5.3%). in 884 cases (27.6%) the papers were illegible the cause of referral was written in 560(17.5%). the reason for further referral in 187 cases (5.8%) was written by experts. In addition, 1454 cases (45.4%) of the return sheets completed by specialists were illegible. Conclusion: The results obtained in this study show that the status of the quality of completion of the referral form by the family doctors and also the status of providing feedback by the specialist doctors are not suitable. to improve this situation the implementation of face-to-face and non-face-to-face training programs for both groups and using electronic referral forms can be put on the agenda.

Abstract Background Cleft in the mitral valve leaflet is a primary cause of congenital mitral regurgitation, stemming from developmental anomalies in the mitral valve and frequently associated with other congenital heart defects. Concurrent presence of cleft in mitral valve leaflet with atrial septal defect and ventricular septal defect is relatively rare. Echocardiography, especially transesophageal echocardiography, is essential in diagnosing cleft mitral valve leaflet and related congenital heart defects, providing critical, detailed imagery for accurate assessment. This study presents a young female patient whose anterior mitral cleft, along with atrial septal defect and ventricular septal defect, was revealed through three-dimensional transesophageal echocardiography. Case presentation A 25-year-old Iranian female, experiencing progressive dyspnea and diminished physical capacity over 3 months, was referred to our hospital. Initial examination and transthoracic echocardiography indicated severe mitral regurgitation. Further evaluation with transesophageal echocardiography corroborated these findings and identified a cleft in the anterior mitral valve leaflet, coupled with mild left ventricular enlargement and significant left atrial enlargement. The complexity of the patient’s condition was heightened by the diagnosis of cleft mitral valve leaflet in conjunction with atrial septal defect and ventricular septal defect, showing the complex nature of congenital defects. Conclusion This case emphasizes the critical role of transthoracic echocardiography in diagnosing cleft of mitral valve leaflet and associated cardiac anomalies, showcasing its superiority over transthoracic echocardiography for detailed visualization of cardiac structures. The identification of multiple congenital defects highlights the necessity for a comprehensive diagnostic approach to manage and treat patients with complex congenital heart diseases effectively. Future research should aim to refine diagnostic methodologies to enhance patient outcomes for cleft of mitral valve leaflets and related congenital conditions.

Background: According to the cholinergic hypothesis for Alzheimer’s disease, potentiation of cholinergic neurotransmission is one of the best strategies for combating dementia. Methods: A new series of benzamide derivatives bearing 1,3,4-thiadiazole nucleus were synthesized and subsequently, their anticholinesterase activity was evaluated. Molecular docking was carried out to explore the likely binding mode and interactions. Results: Fortunately, some of the tested compounds exhibited more activity than donepezil as a reference drug (IC50 = 0.6 ± 0.05 µM). Some of the evaluated derivatives displayed potency in the nanomolar range. Compound 7e with fluorine atom on the meta position of the phenyl ring was the most active compound in this series (IC50 = 1.82 ± 0.6 nM). Conclusion: The 1,3,4-thiadiazole derivatives that were synthesized and tested in the current manuscript demonstrated remarkable anticholinesterase activity. Therefore, these compounds could be suggested as potential anti-Alzheimer agents.

Background: Major β-Thalassemia is the most prevalent singlegene disorder in Iran. Over 2 million beta-thalassemia carriersand more than 25,000 patients have been identified nationwide. This study aimed to evaluate the Major β-Thalassemia (MBT) screening surveillance system in the Jiroft district, southeast Iran.Methods: This descriptive study was conducted using the census method and was based on the information recorded in the files of all major thalassemia births (born between 2011 and 2021) in the health centers of Jiroft University of Medical Sciences. A formula was used to calculate the program’s incidence, expected incidence, and success rate. Stata version 14 and Excel software were utilized to analyze the data, and the qualitative data are presented as numerical values and percentages.Results: During the study periods, 91 patients with betathalassemia Major were born in the population covered by Jiroft University of Medical Sciences. By performing prenatal diagnosis tests in the first weeks of pregnancy and obtaining parental consent for legal abortion, the birth of 422 sick children was prevented. The 11-year incidence of beta-thalassemia major was 5.32 per 10,000, and the expected incidence (if no prevention program was implemented) was 30 per 10,000 live births. The mean success rate of the Major β-Thalassemia surveillance system during the study period was 82.26%, which reached 95.45% in 2021.Conclusion: The i mplementation of efficient, e ffective h ealth policies influenced by the native culture of the region has resulted in the thalassemia prevention program in Jiroft City being completely effective. The implementation success rate of this program has exceeded 95%. Modeling and adapting these preventive policies can assist in implementing thalassemia prevention programs in other parts of the country and developing countries.

This research evaluates two types of vertical axis turbines from both an aerodynamic and economic perspective. The turbines used in this study have straight (H-shaped) and angled (V-shaped) blades, with equivalent dimensional characteristics that are suitable for the climatic conditions and average wind speed distribution in Zahedan city. The aerodynamic evaluation of the turbines was conducted using the semi-analytical DMST method. The results indicate that the V-shaped turbine generates significantly less power than the H-shaped turbine due to the reduction of the effective area of the turbine and the torque produced by its rotor. However, during startup, the V-shaped turbine exhibits about seven times less negative power than the H-shaped turbine, which improves the overall startup process. Economically, the cost of energy production per kWh for a V-shaped turbine is approximately 20% lower than that of an H-shaped one. This makes the V-shaped turbine a more suitable option for urban and small-scale applications.

Abstract Purpose Global warming has led to an increase in the number and intensity of extreme heat events, posing a significant threat to the health and safety of workers, especially those working outdoors, as they often have limited access to cooling strategies. The present systematic literature review (a) summarizes the current knowledge on the impacts of climate change on outdoor workers, (b) provides historical background on this issue, (c) explores factors that reduce and increase thermal stress resilience, (d) discusses the heat mitigation strategies, and (e) provides an overview of existing policy and legal frameworks on occupational heat exposure among outdoor workers. Materials and methods In this systematic review, we searched scientific databases including Scopus (N = 855), Web of Science (N = 828), and PubMed (N = 202). Additionally, we identified relevant studies on climate change and heat-stress control measures through Google Scholar (N = 116) using specific search terms. In total, we monitored 2001 articles pertaining to worker populations (men = 2921; women = 627) in various outdoor climate conditions across 14 countries. After full-text assessment, 55 studies were selected for inclusion, and finally, 29 eligible papers were included for data extraction. Results Failure to implement effective control strategies for outdoor workers will result in decreased resilience to thermal stress. The findings underscore a lack of awareness regarding certain adaptation strategies and interventions aimed at preventing and enhancing resilience to the impact of climate change on heat stress prevalence among workers in outdoor tropical and subtropical environments. However, attractive alternative solutions from the aspects of economic and ecological sustainability in the overall assessment of heat stress resilience can be referred to acclimatization, shading, optimized clothing properties and planned breaks. Conclusion The integration of climate change adaptation strategies into occupational health programs can enhance occupational heat resilience among outdoor workers. Conducting cost-benefit evaluations of health and safety measures for thermal stress adaptation strategies among outdoor workers is crucial for professionals and policymakers in low- and middle-income tropical and subtropical countries. In this respect, complementary measures targeting hydration, work-rest regimes, ventilated garments, self-pacing, and mechanization can be adopted to protect outdoor workers. Risk management strategies, adaptive measures, heat risk awareness, practical interventions, training programs, and protective policies should be implemented in hot-dry and hot-humid climates to boost the tolerance and resilience of outdoor workers.

Abstract Background This study was conceived to assess the postoperative stability of condylar position following fixation with miniplates and lag screws after bilateral sagittal split osteotomy (BSSO). Methods This retrospective study included a cohort of 20 patients undergoing BSSO using the Obwegeser-Dal Pont modification. The bony segments were stabilized using either miniplates with two 2.0-mm monocortical screws per segment or three 2.0-mm bicortical lag screws along the mandible’s superior border. Pre- and postoperative (7-day interval) spiral computed tomography scans were conducted to assess skeletal changes across both groups. Data analysis employed Wilcoxon signed-rank and Wilcoxon rank-sum tests (α = 0.05). Results No statistically significant difference was observed between the pre-and postoperative condylar position parameters (P>0.05). However, the lag screw group showed a marginal significant increase in the left condyle’s angulation (preoperative: 24.83 ± 6.37 vs. postoperative: 32.5 ± 4.93; P = 0.04). Changes in condylar height, length, and width were not statistically significant before and after BSSO in either groups (P>0.05). Nor was any statistically significant difference found between the miniplates and lag screws groups regarding condylar position parameters (P>0.05). Conclusion The results indicated that both lag screw and miniplate fixation methods can be effectively employed in BSSO procedures without impacting condylar position parameters. Thus, either fixation method can be chosen depending on factors such as the surgeon’s preference and clinical outcomes.

Abstract Background Policymakers are tasked with both driving the rapid expansion of renewable energy technologies and, additionally channelling the limited national potential of biomass into areas where it can provide the greatest benefit to the energy system. But do current policy instruments promote the use of biomass in these areas? As biomass is limited, its use must be sustainable without leading to further biodiversity loss or depleting forest or soil resources. In this study, short-term energy scenarios are generated using the BenOpt model, which take into account both current and alternative policy instruments under limited biomass utilisation. The results are compared with long-term, cost-optimal energy scenarios for the use of biomass. Results The analysis reveals that the instrument of a GHG quota does not promote the use of biofuels in hard-to-electrify areas of the transport sector, where they should be cost-optimally allocated according to long-term energy scenarios. Biofuels are promoted for use in passenger road transport and not in the shipping or aviation sector. In contrast, alternative policy scenarios indicate that the sole instrument of a high CO2 price is more conducive to direct electrification and could displace more fossil fuels by 2030 than the GHG quota alone. This instrument also promotes the optimal use of biogas plants in the power sector in accordance with long-term cost-optimal developments. Conclusions The instrument of a GHG quota might lead to counterproductive developments in passenger road transport, but it also helps to ramp up the biofuel capacities required in shipping and aviation in the long term. However, it does not provide the necessary incentives for the ramp-up of battery electric vehicles, which would be the cost optimal solution in passenger road transport according to the long-term scenarios. Even though alternative policy scenarios show that the sole instrument of a high CO2-price is more conducive to direct electrification, a high CO2 price alone is not enough (e.g. in the heat sector) to promote the efficient use of biomass instead of simply covering the base load demand.

Background: The role of mathematics in the present digital world is very important. Due to its nature, mathematics is mostly disliked by students, especially those with learning disorders. Using e-learning in mobile environments and new teaching methods, the complementary sessions needed for students with learning disorders can be enriched. This group of students is most of the time forgotten. This study investigated the impact of adaptive game-based instruction on the mathematical performance of students with mathematics learning disorders. Aims: The present study was conducted to investigate the impact of adaptive game-based instruction on the mathematical performance of students with learning difficulty in a mobile learning environment. Methods: The current research design was the quasi-experimental type of pre-test-post-test and control group. The statistical population of the research included the students with a mathematics learning disorder in Malard, Tehran, in the second grade of primary school, in the academic year of 2022-2023 from which 10 samples were available that randomly were divided into two groups: experimental (5 people) and control (5 people). The experimental group attended 8 educational sessions in the Shad mobile environment in June 2023 using Shad facilities including messenger and games which are proposed by the experts of the Ministry of Education. For the pre-test and post-test, the questions from the Kangaroo Mathematics Competition 2021 were used and the data were analyzed using SPSS26 and univariate analysis of variance. Results: The research findings show that there is a meaningful difference between pre-test and post-test (F= 25.878, p= 0.001). There is also a meaningful difference between the control group and the experimental group in the post-test (F= 6.392, p= 0.039). According to the obtained results, it was concluded that adaptive game-based instruction has a positive impact on the mathematical performance of students with learning disorders in a mobile learning environment. Conclusion: The results showed that using game-based adaptive learning in a mobile learning environment for students with mathematics learning disorder can help these students improve their mathematical performance at their preferred pace and content with educational support. As Shad environment facilities are available for all students in Iran, schools and learning disorder centers can use game-based adaptive instruction in Shad environment to help the students improve their performance through ongoing learning.

Abstract As climate issues gradually attract public attention worldwide, the operation of crude oil pipelines has been attached with new emission‐reduction targets. Though previous studies concerning life cycle assessment (LCA) of oil and gas pipelines have estimated the carbon footprint to some extent, there is a lack of research on life cycle carbon emissions that take the process of crude oil pipelines transportation. the main objective of this study is to build a comprehensive LCA model of carbon footprint for crude oil pipeline systems. A gate‐to‐gate analysis is provided for the LCA model, which means that the entire pipeline's life cycle from the extraction to disposal is considered. The LCA approach used both midpoint and endpoint impact assessment methods to evaluate the environmental impacts of crude oil pipeline transportation in Iran. The study analyzed the major environmental impact categories, such as greenhouse gas emissions, energy consumption, water usage, and solid waste generation. The results showed that the crude oil pipelines transportation has significant environmental impacts in Iran. The three highest impacts on the midpoint scale are marine ecotoxicity by 61.54 kg 1,4‐DCB, terrestrial ecotoxicity by 52.26 kg 1,4‐DCB and fossil resource scarcity by 40.22 kg oil eq. The endpoint impacts of three stations were also compared. The Chamran, Abadan and Zanganeh stations have 95.44%, 94.22%, and 6% impacts on human health, respectively. Also, Abadan station has the largest amount of damage to ecosystems and resources with 96.65% and 80.02%, respectively. The findings of this study could be used by policymakers, oil companies, and other stakeholders to make informed decisions and implement strategies to reduce the environmental impact of oil transportation in Iran.

Abstract In this study, a novel nanobiocomposite consisting of agar (Ag), tragacanth gum (TG), silk fibroin (SF), and MOF-5 was synthesized and extensively investigated by various analytical techniques and basic biological assays for potential biomedical applications. The performed Trypan blue dye exclusion assay indicated that the proliferation percentage of HEK293T cells was 71.19%, while the proliferation of cancer cells (K-562 and MCF-7) was significantly lower, at 10.74% and 3.33%. Furthermore, the Ag-TG hydrogel/SF/MOF-5 nanobiocomposite exhibited significant antimicrobial activity against both E. coli and S. aureus strains, with growth inhibition rates of 76.08% and 69.19% respectively. Additionally, the hemolytic index of fabricated nanobiocomposite was found approximately 19%. These findings suggest that the nanobiocomposite exhibits significant potential for application in cancer therapy and wound healing.

Abstract Background Multiple studies have provided evidence of suboptimal or poor immune responses to SARS-CoV-2 vaccines in recipients of hematopoietic stem cell transplantation (HSCT) and chimeric antigen receptor-T (CAR-T) cell therapy compared to healthy individuals. Given the dynamic nature of SARS-CoV2, characterized by the emergence of many viral variations throughout the general population, there is ongoing discussion regarding the optimal quantity and frequency of additional doses required to sustain protection against SARS-CoV2 especially in this susceptible population. This systematic review and meta-analysis investigated the immune responses of HSCT and CAR-T cell therapy recipients to additional doses of the SARS-CoV-2 vaccines. Methods Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, the study involved a comprehensive search across PubMed, Scopus, Web of Science Core Collection, Embase, and Cochrane Biorxiv and medRxiv, focusing on the serological responses to the third and fourth vaccine doses in HSCT and CAR-T cell patients. Results This study included 32 papers, with 31 qualifying for the meta-analysis. Results showed that after the third dose, the seroconversion rate in HSCT and CAR-T cell therapy recipients who didn’t respond to the second dose was 46.10 and 17.26%, respectively. Following the fourth dose, HSCT patients had a seroconversion rate of 27.23%. Moreover, post-third-dose seropositivity rates were 87.14% for HSCT and 32.96% for CAR-T cell therapy recipients. Additionally, the seropositive response to the fourth dose in the HSCT group was 90.04%. Conclusion While a significant portion of HSCT recipients developed antibodies after additional vaccinations, only a minority of CAR-T cell therapy patients showed a similar response. This suggests that alternative vaccination strategies are needed to protect these vulnerable groups effectively. Moreover, few studies have reported cellular responses to additional SARS-CoV-2 vaccinations in these patients. Further studies evaluating cellular responses are required to determine a more precise assessment of immunogenicity strength against SARS-CoV-2 after additional doses.

The design and synthesis of biocompatible nanostructures for biomedical applications are considered vital challenges. Herein, a nanobiocomposite based on acacia hydrogel, natural silk fibroin protein, and synthetic protein fibers of polyvinyl alcohol was fabricated and magnetized with iron oxide nanoparticles (Fe3O4 MNPs). The structural properties of the hybrid nanobiocomposite were investigated by essential analyses such as Fourier Transform Infrared Spectrometer (FTIR), Field emission scanning electron microscopy (FE-SEM), and X-ray powder diffraction)XRD(analyses, Thermogravimetric and Differential thermogravimetric analysis (TGA-DTG), Vibrating-sample magnetometry (VSM), and Energy Dispersive X-Ray Analysis (EDX). The biological activities and functional properties of the prepared magnetic nanobiocomposite were studied. Results proved that this nanobiocomposite is non-toxic to the healthy HEK293T cell line. In addition, the synthesized nanobiocomposite showed an approximately 22 % reduction in cell viability of BT549 cells after 72 h. All results confirmed the anti-cancer properties of nanobiocomposite against breast cancer cell lines. Therefore, the prepared nanobiocomposite is an excellent material that can use for in-vivo application. Finally, the hyperthermia application was evaluated for this nanobiocomposite. The SAR was measured 93.08 (W/g) at 100 kHz.

In the context of rural resource constraints, how different dimensions of social capital and entrepreneurial passion affect entrepreneurial alertness is an important issue worth studying. The purpose of this study was to predict the entrepreneurial alertness of rural cooperative members based on social capital based on the mediator role of entrepreneurial passion. The research population was composed of 3,851 members of rural cooperatives in Kermanshah province, Iran. A standard questionnaire was used for data collection. Formal validity, convergent validity, and divergent validity were checked for the questionnaire, and its reliability was confirmed by a pilot test and the calculation of Cronbach’s alpha coefficient (.82–.90). The SPSS 22 and SmartPLS 1 software suites were used to analyze the data. The survey methodology was used to achieve the results and performance. The findings showed that social capital ( p = .428) and entrepreneurial passion ( p = .383) had a positive effect on entrepreneurial alertness. Also, the mediator role of entrepreneurial passion in the relationship between social capital and entrepreneurial alertness was confirmed. This study contributes to an understanding of the entrepreneurial alertness in rural cooperatives by asking members making up social capital how they perceive the main factors that can better enable sustainable ventures to thrive and play a role in sustainable transitions.

Abstract Background With the ban on the use of antibiotics in poultry nutrition, the opinion of nutritionists turned to their alternatives. Garlic and mushroom are the two important phytobiotic compounds in poultry nutrition. Objectives This experiment was done to investigate the effect of garlic powder (GP) and mushroom powder (MP) on the growth performance, meat quality, serum lipid profile, and intestinal morphology of broilers. Methods Five hundred and seventy‐six one‐day‐old male Ross 308 broiler chicks were assigned to eight treatments with six replications based on a completely randomized design in a factorial arrangement of 4*2 with four levels of GP (0.00, 0.50, 1.00, 1.50%) and two levels of MP (0.00, 1.00%). Results No significant effects of GP and MP on the performance were observed. With increasing levels of GP in the diets, the lightness and redness of breast meat decreased and increased, respectively (p < 0.05). The effect of increasing the amount of GP on the reduction of total cholesterol level was similar in the absence or presence of MP. With increasing levels of GP in the diets, the villus height (VH) and VH to crypt depth ratio (VH: CD) increased. The use of MP in the diets significantly increased VH and VH: CD (p < 0.05). Conclusion The addition of GP and MP to the broilers’ diets did not have any negative effect on the performance. These pharmaceutic herbs improved intestinal morphology. In addition, increasing the level of GP amended the meat color and reduced the level of serum cholesterol.

Key Clinical Message Cardiac papillary fibroelastomas (CPF) are rare, benign tumors with thromboembolic potential. We present a 40‐year‐old male with a right atrial CPF, referred with acute chest pain. Advanced imaging and surgical excision with tricuspid valve repair were crucial, emphasizing the need for early detection and intervention in symptomatic and asymptomatic cases.

Key Clinical Message We report a case of a 21‐year‐old male with stage IIIB sigmoid colon adenocarcinoma who experienced atypical chest pain post‐adjuvant chemotherapy with Capecitabine (5‐FU prodrug). Evaluation revealed an unexpectedly detected interventricular septum hemangioma. Due to the vasospasm effect of chemotherapy presenting with semi‐ischemia, conservative management was chosen for atypical presentation.

Human error constitutes one of the primary causes of accidents, particularly in the context of loading and unloading operations involving road trucks, especially those carrying petroleum products. The process of identifying and evaluating human errors within these operations involves several key steps. Initially, all sub-tasks associated with loading and unloading are meticulously identified and analyzed utilizing Hierarchical Task Analysis (HTA), achieved through direct observation, document examination, and interviews. Subsequently, potential human error modes within each task are delineated using the Systematic Human Error Reduction and Prediction Approach (SHERPA). Finally, essential data for determining the criticality, probability, and severity of each error are gathered through expert elicitation and the application of Fuzzy Inference Systems (FIS).Through the analysis of SHERPA worksheets, a total of 37 errors during loading operations and 14 errors during unloading operations of petroleum products were identified. Among these errors, the predominant category during loading operations was action errors, comprising 31 instances, while communication errors were the least frequent, occurring only twice. Similarly, action errors were most prevalent during unloading operations, constituting 13 instances. These errors were further categorized and ranked based on their risk levels, resulting in 27 levels for loading operations and 12 levels for unloading operations.The consistent occurrence of action errors underscores the need for implementing control measures to mitigate their frequency and severity. Such strategies may include periodic training sessions to reinforce proper work procedures and the development of monitoring checklists, among other interventions.

In this experimental work, the 2-phase air-water non-boiling ascending fluid flow in a vertical tube with helical corrugations has been investigated. The results showed that the head loss values decreased with an increase of the volume fraction. Also, by comparing the head loss values for each corrugation pitch, it can be observed that as the corrugation pitch goes down, the head loss values significantly increase. As a result, the intensity of vapors increases perpendicular to the main flow of water, which leads to an increase in the intensity of disturbance in the flow, and then the head loss increases. The Nusselt number goes down when the volume fraction experiences an increment. Looking at the figures related to Nusselt number, it is easy to see that the curves are drawn for a constant air flow rate. Consequently, an increase in volume fraction (VF) is equal to a decrease in the water flow rate. By reducing the water flow rate, the intensity of the main flow is reduced the intensity of turbulence is also reduced and the heat transfer coefficient is reduced. As a result, the amount of heat transfer has increased due to air injection. It should be noted that pipes with the largest corrugation pitch had the best Cost-benefit ratio (C.B.R.) factor values (which means the lowest value). This means that air injection in larger corrugation pitch tubes was more beneficial than in smaller corrugation pitch tubes.

Abstract This study involves the development of a new nanocomposite material for use in biological applications. The nanocomposite was based on tragacanth hydrogel (TG), which was formed through cross-linking of Ca2+ ions with TG polymer chains. The utilization of TG hydrogel and silk fibroin as natural compounds has enhanced the biocompatibility, biodegradability, adhesion, and cell growth properties of the nanobiocomposite. This advancement makes the nanobiocomposite suitable for various biological applications, including drug delivery, wound healing, and tissue engineering. Additionally, Fe3O4 magnetic nanoparticles were synthesized in situ within the nanocomposite to enhance its hyperthermia efficiency. The presence of hydrophilic groups in all components of the nanobiocomposite allowed for good dispersion in water, which is an important factor in increasing the effectiveness of hyperthermia cancer therapy. Hemolysis and 3-(4,5 dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide assays were conducted to evaluate the safety and efficacy of the nanobiocomposite for in-vivo applications. Results showed that even at high concentrations, the nanobiocomposite had minimal hemolytic effects. Finally, the hyperthermia application of the hybrid scaffold was evaluated, with a maximum SAR value of 41.2 W/g measured in the first interval.

Background and Objective: One of the most important challenges that is considered in process industries to improve performance and increase productivity is the optimization of the incident command system (ICS). It is a standard approach to control and coordinate the management of incidents on the scene. The present study aims to evaluate and prioritize the factors affecting the effectiveness of the ICS using analytic hierarchy process (AHP) and decision-making trial and evaluation laboratory (DEMATEL) methods in process industries in Iran. Materials and Methods: In this research, firstly, a questionnaire was compiled and designed using the AHP method in order to weigh the criteria and prioritize them, and a questionnaire was designed using the DEMATEL method in order to determine the effectiveness and mutual effect of the criteria and sub-criteria. Thereafter, the questionnaires were completed by 15 incident commanders, process industry safety officials, and crisis management activists working in the oil and petrochemical industry. Results: Based on the results, AHP analysis ranked 'command' highest with a standard weight of 0.256. DEMATEL results demonstrated 'command' as the most influential (standard weight of 10.59), effective (11.9), and interactive (21.96) indicator. 'Standardization' (1.35) and 'professionalism' (1.19) emerged as causal factors, while 'command' (-0.50), 'planning/organizational structure' (-1), 'facilities and resources' (-0.18), and 'disability index' were responsive indicators. Conclusion: According to the results of both AHP and DEMATEL methods, the command index assumes critical importance and effectiveness in the success of the incident command system in process industries in Iran.

Background and purpose: Type 2 diabetes is a health problem and is an emergency that has the fastest growth worldwide. Diabetes is one of the priorities of all public health systems this study aimed to determine the quality of diabetes control in urban family physician centers of Sari city in 1382-1398. Materials and methods: This study was a cross-sectional and cross-sectional study. The required sample size was 275 patients who were selected randomly (a total of 300 patients) and 20 clusters (center), each consisting of 15 diabetic patients, were controlled. The population of the study included all patients with type 2 diabetes referred to health care centers of sari. The cases included age, sex, level of FBS, 2 hpp, HbA1c, blood pressure, triglyceride, cholesterol, HDL, LDL, and body mass index as well as eye examination and amputation were also recorded all the information contained the seal of the family physician. In this study, after data collection, the degree of conformity was compared with ADA guidelines. According to the American diabetes association (ADA), the goal of the treatment of this disease is reaching FBS: 90-130, HbA1c below 7%, BS2hpp

Objectives: Understanding patterns of quality of life in people who inject drugs (PWID) can help healthcare providers plan and manage their health problems in a more focused manner. Therefore, the current study aimed to identify patterns of health-related quality of life in PWID in southeast Iran. Methods: This cross-sectional study was conducted in southeast Iran on men who had injected drugs at least once during the last year. We used convenience sampling in 2 drop-in centers and venue-based sampling at 85 venues. Demographic characteristics, high-risk behaviors, and health-related quality of life were evaluated using the 5-level EQ-5D version (EQ-5D-5L) questionnaire. Latent profile analysis was used to identify patterns of quality of life. Results: This study enrolled 398 PWID, who had a mean age of 34.1±11.4 years. About 47.9% reported a prison history in the last 10 years, 59.2% had injected drugs in the last month, and 31.6% had a history of sharing syringes. About 46.3% reported having 2 or more sexual partners in the last 6 months, and 14.7% had a history of sex with men. Out of the 5 EQ-5D-5L profiles, the fifth profile had the most people (36.6%). Most variations in quality of life were related to pain and discomfort. Conclusions: We showed that the pain and discomfort dimension of EQ-5D-5L had more variation in PWID. This finding will be useful for allocating appropriate interventions and resources to promote health-related quality of life in this population.

Background & Objective: Students constantly go through experiences that can be a source of physical, emotional, or psychological stress. Dysfunctional attitudes are the root of numerous mental health problems. On the other hand, perceived social support is recognized as one of the most effective solutions in the treatment of stress and its related disorders. The present study aimed to assess the relationship of social support and dysfunctional attitude with perceived academic stress in the students of Ferdows School of Medical Sciences. Materials & Methods: This descriptive-correlational study was conducted on 121 students of Ferdows School of Allied Medicine and Public Health who were selected via stratified sampling method in the academic year 2019-2020. The data collection tools were demographic characteristics form and three standard questionnaires of social support appraisals (SS-A) scale, Vaux A, Student Life Stress Inventory (SLSI), and Dysfunctional Attitude Scale(DAS-26). Data were analyzed in SPSS software (version 22) using descriptive (mean and standard deviation) and inferential statistics (Kolmogorov-Smirnov test to determine whether sample data is normally distributed, Pearson and Spearman correlation test to check the relationship between variables, and independent t-test and analysis of variance to compare means between two groups. A p-value of 0.05 was considered statistically significant. Results: The mean scores of academic stress and perceived social support of students were reported as 2.61±0.49 and 2.42±0.47, respectively, which were lower than the average. Moreover, the mean score of dysfunctional attitude (110.24±78.5) was higher than the standard. Pearson's correlation test pointed to a significant relationship between social support and academic stress (r=0.36; P< 0.001), dysfunctional attitude and academic stress (P=0.006; r = -0.24) and dysfunctional attitude and social support (P=0.005; R=-0.25). Conclusion: The results of the present study pointed out that the variables were closely related to each other, highlighting the importance of these issues in student life. In other words, in order to adjust to academic stress, which is the cause of failure and disruption in academic performance, attention should be paid to various factors at the level of the university, society, and most importantly, family where a person is raised. In this regard, culture building is necessary at the university, society, and family level to shape people's attitudes positively and develop creative thinking based on hope and a burning desire for construction.

Background and purpose: Access to primary healthcare services, considering equity and equality in the health system, is a right of the global community. To provide primary health services, the rural family physician program was implemented at the beginning of 2005 to achieve universal health coverage in rural areas and small cities (with a population of less than 20,000). The specific goals of this program included the implementation of the referral system, increasing the responsiveness of the health market, increasing people’s access to health services, reducing unnecessary costs in the health market, and increasing service coverage. After approximately 7 years of implementation of this program, the urban family physician program started in Mazandaran and Fars province as a pilot project. The urban family physician program started in the Mazandaran province in 2012 as the first level of providing health services. In line with the better implementation of this program and considering the wide range of expected programs and activities, it is essential to entrust the appropriate activities with the time of the service providers. This study was conducted to assess the expected duration of activities assigned to the family physician. Materials and methods: This cross-sectional study was conducted on 15 urban family physician centers (6 government and 9 private centers) in Golugah, Behshahr, Neka, Ghaemshahr, Noor, and Chalus cities of Mazandaran province. Out of 15 cities with more than 20,000 people participating in this program in Mazandaran province, the six cities mentioned above were randomly selected, and 15 urban family doctors were randomly selected from the existing list. Finally, the target groups were included in the study for convenience sampling. To collect the data, checklists compiled based on care packages corresponding to 8 groups of service recipients (elderly (11 services), middle-aged (7 services), youth (13 services), teenagers (24 services), under 5 years old (6 services), mana (15 services), mothers (3 services), and non-target visit) were used. For each service recipient group, at least three individuals were examined. Central and dispersion indices were used to describe the data. Results: The results of measuring the time of providing service to 297 people (including 90 under 5 years old, 30 people receiving Mana services, 30 people from each of the groups of mothers, teenagers, young people, middle-aged, elderly, and 27 people from non-target visit) showed that the median and mean expected time for providing daily service by an urban family physician to the covered population is 2 h and 52 min and 3 h and 23 min, respectively. The estimated time was different in the groups receiving the service, the highest time of which was related to the visit of the non-target service group with median and mean of 84.17 and 87.20 min, respectively, and the lowest time was related to Mana services with a median and mean of 3.40 and 4.34 min, respectively. Conclusion: Urban family physicians have enough time to provide daily services, and therefore it seems that the health system has not assigned them more than their daily time capacity.

AbstractUnlike Fc receptors for switched immunoglobulin (Ig) isotypes, Fc receptor for IgM (FcµR) is selectively expressed by lymphocytes. The ablation of the FcµR gene in mice impairs B cell tolerance as evidenced by concomitant production of autoantibodies of IgM and IgG isotypes. In this essay, we reiterate the autoimmune phenotypes observed in mutant mice, ie IgM homeostasis, dysregulated humoral immune responses including autoantibodies, and Mott cell formation. We also propose the potential phenotypes in individuals with FCMR deficiency and the model for FcµR-mediated regulation of self-reactive B cells.

Introduction: Following Mass Casualty Incidents (MCIs), the sudden surge in demand for essential services disrupts the balance between available and required resources. This study aimed to systematically identify and categorize existing systems employed for dispatching professional or lay rescuers during emergencies. Methods: Adhering to the PRISMA 2020 Checklist, the research scrutinized international databases (PubMed, Scopus, and Web of Science) using formulated search strategies. Additionally, a manual search was conducted on Google Scholar and prominent journals employing specific keywords. Original articles introducing systems for dispatching rescuers to incident sites were included. Results: Thirty-one of the 23051 initially identified documents were included for data extraction and quality assessment. The comprehensive analysis revealed twenty-two dispatch systems worldwide, contributing to life-saving efforts in emergencies. Additionally, an evaluation of the articles' quality using the Mixed Methods Appraisal Tool (MMAT) with five scores, indicated that more than two-thirds of the identified articles scored four or higher. Summarizing the data extracted from these systems, four distinct categories of recall system characteristics were identified: general, dispatcher, responder, and other features. Conclusion: Technology has the potential to revolutionize the delivery of healthcare services. This study highlights four key elements necessary for the development of dispatch systems that can effectively mobilize healthcare providers to the incident scene. These elements include general characteristics, dispatcher roles, responder requirements, and additional features, which equip researchers with the knowledge for designing effective systems to recall healthcare providers during MCI.

ObjectivesThe percentage of shunt fraction significantly impacts the management of patients with congenital shunts, influencing strategic choices such as surgical or interventional procedures. This study compared the estimated shunt fraction (the ratio of pulmonary-to-systemic flow, Qp/Qs) for quantifying the left-to-right shunt in children with ventricular septal defect (VSD) using heart catheterization, four-dimensional (4D) flow, and two-dimensional (2D) flow magnetic resonance imaging (MRI). The goal was to establish a non-invasive and reliable measurement ratio between pulmonary and systemic blood flow in these patients.MethodsBetween July 2022 and June 2023, patients scheduled to undergo invasive right heart catheterization were included in this study. MRI was performed one hour before the catheterization procedure. The correlation of shunt fraction was assessed between all methods after calculating the Qp/Qs ratio from 2D and 4D flow MRI and catheterization.ResultsA total of 24 patients (aged 3–15 years, eight females) were ultimately included in the study. The Qp/Qs ratios obtained from 4D flow had a robust correlation (correlation coefficient r = 0.962) compared to those obtained during catheterization. Cardiac catheterization recorded the mean shunt fraction at 1.499 ± 0.396, while 4D flow measured it at 1.403 ± 0.344, with no significant difference between the two techniques. Moreover, there was a reasonable correlation (r = 0.894) between 2D flow measurements of Qp/Qs and the results obtained from catheterization, with a mean shunt fraction of 1.326 ± 0.283.Conclusion4D flow MRI has the potential to be a non-invasive method for accurately measuring the left-to-right shunt in children with VSD.

This research study focuses on a new magnetic nanobiocomposite designed and synthesized by the formation of cross-linked alginate (Alg) hydrogel using CaCl2 cross-linker agent, its modification with flaxseed mucilage hydrogel and silk fibroin (SF) biopolymer extracted, and as well, the in-situ synthesis of Fe3O4 magnetic nanoparticles (Fe3O4 MNPs) in the presence of this biopolymeric substrate. FT-IR, EDX, FE-SEM, XRD, VSM, and TG technical analyses clarified the physical and chemical features of magnetic cross-linked Alg/flaxseed mucilage hydrogel/SF nanobiocomposite. Following the structural characterization of this magnetic natural-based composition, the in-vitro cytotoxicity and hemolysis assay studies with different concentrations resulted in prominent biological outcomes. The survival rates of normal HEK293T cells (95.74%, 94.18%) and breast cancer BT549 cells (79.23%, 77.76%) after 48 and 72 h verified the biocompatibility and anticancer properties of this new nanobiocomposite. Also, less than 5% of the hemolytic effect disclosed its hemocompatibility. Furthermore, a high specific absorption rate value (72.42 W/g) is generated by 1 mg/mL of this magnetic nanobiocomposite. Overall, it can be deduced that magnetic responsive cross-linked Alg/flaxseed mucilage hydrogel/SF nanobiocomposite could functionally perform in magnetic hyperthermia treatment.

ABSTRACT: A total of 576-day-old Ross 308 broilers chicks (male) were used to evaluate the effect of various levels of pistachio green hull aqueous extract (PHE) and Eimeria challenge on the growth performance, intestinal health and antioxidant capacity. During infection period (25–42 d), treatments included: 1) control + unchallenged (negative control, NC), 2) 200 ppm PHE + unchallenged, 3) 300 ppm PHE + unchallenged, 4) 400 ppm PHE + unchallenged, 5) control + challenged (positive control, PC), 6) 200 ppm PHE + challenged, 7) 300 ppm PHE + challenged and 8) 400 ppm PHE + challenged (with 6 replications for each treatment). The outcomes revealed that in the challenged birds, average body weight gain (ABW), daily weight gain (DWG), and feed conversion ratio (FCR) linearly improved with increasing the PHE levels (P < 0.05). Infected broilers had lower daily feed intake (DFI) compared to unchallenged birds (P < 0.05). Villus height (VH), villus height to crypt depth (VH: CD) ratio and villus surface area (VSA) reduced linearly (P < 0.05), while muscle layer thickness (MT) increased linearly in challenged birds (P < 0.05). The consumption of the PHE significantly reduced the excreta oocytes and duodenum and jejunum lesion scores in Eimeria-challenged broilers (P < 0.05). By increasing the PHE levels, total antioxidant capacity (TAC) and superoxide dismutase (SOD) levels increased (P < 0.05), while the Eimeria challenge reduced TAC, SOD, and glutathione peroxidase (GPx) levels (P

The current study aimed to design and synthesize a new magnetic nanobiocomposite and assess its potential for biological applications and hyperthermia. For this purpose, in the first step, the Pectin (PC) and Tragacanth gum (TG) polymer was synthesized using CaCl2 as a cross-linking agent (PC-TG hydrogel). In the second step, natural Silk fibroin (SF) protein and graphitic carbon nitride (CN) were added to the hydrogel to upgrade the nanobiocomposite's strength and due to CN's pharmacology applications, respectively. Finally, for an enhanced hyperthermia application, PC-TG hydrogel/SF/CN was in situ magnetized with Fe3O4 magnetic nanoparticles (MNPs), and PC-TG hydrogel/SF/CN/Fe3O4 nanobiocomposite was synthesized. By using a vibrating-sample magnetometer (VSM), Fourier-transformed infrared (FTIR), thermogravimetric analysis (TGA), energy dispersive X-ray (EDX), X-ray diffraction (XRD), and field-emission scanning electron microscope (FESEM), the structural features and properties of the PC-TG hydrogel/SF/CN/Fe3O4 nanobiocomposite were determined. This magnetic nanobiocomposite's saturation magnetization value was 14.84 emu g-1. The hemolytic assay of this new nanobiocomposite demonstrated that the hemolysis percentage was 1.07 %, around 99.0 % of cells were able to survive, and the MTT assay was used to assess the anticancer activity against breast cancer cell lines (BT549). Additionally, 62.51 (W g-1) in 200.0 kHz was found to be the greatest specific absorption rate (SAR). These findings suggest that the recently created magnetic nanobiocomposite might function admirably in hyperthermia treatment when exposed to an alternating magnetic field.

Hepatitis E virus (HEV) is a major cause of acute viral hepatitis worldwide. Up to now, no approved treatment nor a globally licensed vaccine is available. Several recombinant HEV vaccines have been developed to protect against HEV infection in humans, including the commercially available Hecolin vaccine, which are mainly based on HEV genotype 1. However, the efficacy of these vaccines against other HEV genotypes, especially genotype 3 is unknown. In this study, we evaluated the protective efficacy of Hecolin® and a novel genotype 3-based vaccine p239(gt3) against HEV-3 in a pig infection model. Pigs were divided into three groups: one group was vaccinated with Hecolin®, the second group was vaccinated with p239(gt3), and the control group received no vaccine. All pigs were subsequently challenged with HEV genotype 3 to assess the effectiveness of the vaccines. Although all immunised animals developed a high titer of neutralizing antibodies, the results showed that both vaccine applications could not provide complete protection against HEV (gt3) infection: Two out of four animals of the Hecolin® group displayed even virus shedding, and viral RNA could be detected in bile and/or liver of three out of four animals in both vaccination groups. Only one out of four animals in each group was fully protected. Neither Hecolin® nor the novel p239(gt3) vaccine provided sufficient protection against genotype 3 infection. While Hecolin® only partial protected pigs from HEV shedding, the novel p239(gt3) vaccine was at least able to prevent infected pigs from virus shedding. The results highlight the need for further development of HEV vaccines that exhibit broad protection against multiple HEV genotypes and the use of appropriate animal infection models.

The INVELOX system is an innovative approach that offers improved energy absorption efficiency from wind flow and reduced costs by utilizing smaller wind turbines. This research focuses on investigating the steady-state performance of one, two, or three wind turbines arranged within the venturi section of the system. A comprehensive modeling approach using an improved Blade Element Momentum (BEM) theory is proposed and implemented as a MATLAB code. The code incorporates Prandtl's tip and hub loss factors, as well as turbulent wake corrections. The accuracy of the code is validated against experimental and numerical data. The results demonstrate that in a three-rotor tandem configuration in the INVELOX system, the power extracted from the second and third turbines is 0.54 and 0.24 times the power of the first turbine, respectively. Furthermore, for a two-turbine arrangement in the venturi section, the total power extracted from the system is 53.9% higher than that of a single turbine layout. In the case of a three-turbine configuration, the total power increases up to 1.78 times compared to a single turbine. The proposed model is suitable for geometric optimization and parameter studies. The system's performance is evaluated in terms of tip speed ratio, and the effects of different correction models are analyzed, including the local changes in forces and moments.

Abstract Over the past decade, CdS QDs have become versatile semiconductors. Surface modification of CdS QDs has become an interesting case study, as it can eliminate surface defects and improve their photochemical properties. In this study, we report a new strategy of using carbon quantum dots containing a large number of thiol groups (CQDs-SH) as a passivating agent for the stabilization of CdS quantum dots (QDs). Various characterization techniques have clearly revealed that the CdS QDs have been successfully passivated by CQDs-SH. The photocatalytic performance of CQDs-SH/CdS QDs was investigated for the degradation of the insecticide imidacloprid from an aqueous solution. Parameters affecting the photodegradation process, including the light source, photocatalyst amount, initial concentration of the pollutant, radiation time, pH, oxidizing agent, and temperature, were investigated. Furthermore, the HPLC technique was applied to quantitatively analyze imidacloprid and its degradation products. The results of the HPLC analysis revealed that under simulated visible light at pH 9, imidacloprid scarcely existed after 90 min of irradiation (90.13% degradation). The LC–MS method was also used to detect the degradation products and investigate the mechanism of photodegradation of the pesticide. The results showed that the CQDs-SH/CdS QDs composite was a promising photocatalyst for the degradation of imidacloprid in wastewater.

Abstract This study presents a novel class of pseudohomogeneous catalysts (PHC) based on carbon quantum dots functionalized with terpyridine ligands (CQDs-Tpy) to immobilize and stabilize palladium nanoparticles (Pd NPs). Extensive characterization techniques clearly confirmed the successful stabilization of Pd NPs on CQDs-Tpy. The effectiveness of the catalyst was demonstrated in the selective aerobic oxidation of primary and secondary of benzylic alcohols to aldehydes in the absence of additives and phase transfer catalyst (PTC). Remarkably, the reactions predominantly yielded aldehydes without further oxidation to carboxylic acids. By employing low catalyst loadings (0.13 mol%), high conversions (up to 89%) and excellent selectivity (> 99%) of the aldehyde derivatives were achieved. Moreover, the CQDs-Tpy/Pd NPs catalyst displayed suitable catalytic activity and recyclability, offering potential economic advantages. This promising approach opens up new opportunities in the field of catalysis for designing subnanometric metal-based PHCs.

Background and purpose: Isatin derivatives have excited attention due to their biological attractions, especially, anticancer properties. Isatin analogs such as semaxanib and sunitinib were exposed to tyrosine kinase inhibitory properties. N-substituted isatins were reported to show cytotoxic activity. On the other, the extension of impressive and cost-effective agents against leishmaniasis is necessary in third-world countries. The capability of isatin derivatives to create novel anticancer and anti-leishmanial compounds has been identified in medicinal chemistry research. The current study aimed to synthesize N-alkyl-isatin-3-imino aromatic amine compounds and evaluate their biological effects. Experimental approach: Synthesis started with the formation of 2-chloro-N-phenylacetamide derivatives by the reaction of aniline derivatives with chloroacetyl chloride. N-alkylation of isatin was performed in the presence of K2CO3 in N, N-dimethylformamide. Final products were prepared via the condensation of N-alkyl isatin derivatives with aromatic amines. Cell viability was checked out by using the MTT assay against cancer cells. Final compounds were screened for anti-leishmanial activity. The molecules were docked in the active sites of the epidermal growth factor receptor tyrosine kinase to define the possible interactions. Findings/Results: Compounds 5c and 4d with IC50 value of 50 μΜ showed cytotoxic activity on the MCF-7 cell line. Compound 5b presented anti-leishmanial activity against promastigote form after 48 h (IC50:59 μΜ) and 72 h (IC50: 41 μΜ) incubations. The highest docking score was -7.33 kcal/mol for compound 4d. Conclusions and implications: The nature of substitution in the N1 region of isatin seems to be able to influence the cytotoxic activity. Based on the obtained results of docking and cytotoxic tests, compound 4d seems to be a good compound for further investigations.

Vaccination is a highly effective medical intervention in the COVID-19 pandemic. Systemic Capillary Leakage Syndrome (SCLS) is a severe and rare condition that can occur following COVID-19 vaccination, leading to recurrent shock episodes in patients. This report highlights a case where a patient experienced leakage syndrome after receiving the AstraZeneca vaccine. The 31-year-old female patient experienced shortness of breath, blood pressure drops, edema, and limb coldness 48 hours after receiving the COVID-19 vaccine. The clinical assessments identified low blood pressure, rapid heart rate, and reduced breath sounds at the bases of the lungs. Bilateral pleural effusion was observed in a chest CT scan, and pericardial effusion of moderate severity was seen in echocardiography. Laboratory tests showed leukocytosis, hyponatremia, hypoalbuminemia, hypocalcemia, and transudative pleural fluid. The remaining observations fell within standard parameters. Measures such as fluid restriction, correction of electrolyte imbalances, prescription of corticosteroids, and antibiotic therapy were applied. These interventions resulted in an improvement in the patient's overall condition, leading to the resolution of her symptoms. Since the symptoms of leakage syndrome are non-specific, and its diagnosis is based on excluding other causes, and considering that no paraneoplastic evidence was found in the studied patient, the diagnosis of leakage syndrome was attributed to the individual. For individuals exhibiting hypovolemic shock and edema linked to pleural effusion or ascites, and upon excluding conditions such as nephrotic syndrome, hypercoagulability syndromes, sepsis, and connective tissue vascular diseases, attention should be directed towards the possibility of leakage syndrome. Supportive care is the mainstay of treatment in these patients, and fluid restriction is a key component of management. Awareness of the symptoms and signs of this syndrome, as well as factors that may trigger its recurrence, is essential for physicians to make appropriate decisions for timely diagnosis and treatment in similar cases

Background: The effects of ginseng on fatigue have been proven in patients with multiple sclerosis (MS), which have several similar manifestations to neuromyelitis optica spectrum disorder (NMOSD) patients. This study was designed to evaluate the effects of ginseng on fatigue in NMOSD patients. Methods: In this double-blinded randomized controlled clinical trial, 64 patients were recruited and were allocated into two study groups (ginseng or placebo) via block randomization. The participants received either 250-mg ginseng or placebo twice daily for a 3-month period. Also, the measurement of outcome was performed using the valid and reliable Persian version of fatigue severity scale (FSS) questionnaire, which was filled by patients once after enrollment in the study and once at the end of the study post-intervention. Results: In total, 58 patients finished the study with no major side effects. There were no significant differences in demographic, clinical, as well as FSS between two study groups (p>0.05). Ginseng supplementation significantly reduced fatigue (40.21±13.51 vs. 28.97±14.18; p˂0.01), while patients in the placebo group showed significantly higher fatigue score after 3 months post-intervention (35.03±13.51 vs. 38.79±12.27; P: 0.02). The extent of changes in the fatigue score in the ginseng group was significantly greater than in the placebo group (p ˂0.01). Conclusion: This study revealed positive effects of ginseng on reducing fatigue in NMOSD patients with no major side effects. In this regard, further studies are warranted to evaluate and clarify the effects of ginseng on fatigue in NMOSD.

Background and Objective: Anesthesiologists are exposed to significant risks due to advancements in the techniques of anesthesia and surgery, the emergence of modern equipment and drugs, as well as increased responsibilities and expectations. Therefore, the present study aimed to provide a model for assessing the risks and hazards existing in this profession. Materials and Methods: This is a descriptive study. Initially, various sources, including interviews with three experts and reviews of articles, were used to extract occupational risks in the field of anesthesiology. Subsequently, the study criteria, including consequence, probability, and exposure, were determined, and the weighting of these criteria was calculated using the Analytic Hierarchy Process (AHP) method. Finally, the prioritization of different risks was determined using the combined compromise solution (CoCoSo) method. Results: Based on the weights of criteria using the AHP method, consequence with a weight of 0.43 was the most important criterion, while exposure with a weight of 0.27 was the least important criterion in this study. Furthermore, among the investigated risks, inhaling anesthetic gases while wearing a mask and filling a vaporizer (R5) was selected as the most dangerous risk, followed by skin contact with scrubs or blood, patient's secretions, and infected tissues (R1), as well as needle sticking during venipuncture and patient's spinal anesthesia (R12), which were selected as the second and third most dangerous risks in anesthesiology, respectively. Conclusion: In this study, the consequence was identified as the most important criterion, and the risks of inhaling anesthetic gases during mask ventilation and filling the vaporizer were determined to be the most significant occupational risks in anesthesiology. Furthermore, it was established that the proposed method can be used for risk assessment and prioritization of occupational hazards in anesthesiology and other professions.

Background and Objective: Bus drivers are exposed to harmful agents such as noise and vibration, which may increase the level of job stress among them. This study aimed to evaluate job stress among urban bus drivers concerning daily noise and vibration exposure. Materials and Methods: This cross-sectional study was conducted on 103 urban bus drivers in Hamadan in 2020. The drivers' exposure to noise and vibration was measured using Svan 104 and Svan 106, respectively. Their job stress was measured through the Phillippe L. Rice questionnaire. Data analysis was performed using SPSS (version 24). Results: The mean exposure of drivers to noise was 79.50 ± 3.51 dB. Their exposure levels to whole-body vibrations (WBV) and hand-arm vibrations (HAV) were 0.62 ± 0.16 and 0.44 ± 0.06 m/s2, respectively. Moreover, most drivers (87.4%) had severe job stress. A significant positive relationship was observed between noise and job stress (P=0.021), whereas there was no significant relationship between WBV, HAV, and job stress. Conclusion: Regarding the high prevalence of severe job stress among bus drivers and the positive correlation between noise and job stress, it is highly recommended that noise exposure be subsided through periodic services and maintenance, as well as using new buses instead of the old ones.

Background and aim: CRISPR-Cas9, enable precise DNA manipulation via RNA-guided breaks. KHDC3L and PEG3 genes are vital; CRISPR-Cas9 studies their roles in reproduction, development, and gene regulation. Focusing on KHDC3L's impact on PEG3 promoter in HCT116 cells, insights into gene functions and disease mechanisms emerge, informing potential therapies. Method: This study aims to design sgRNAs for the KHDC3L gene using CRISPR tools involved ranking, off-target evaluation, and cloning. HCT116 cells were cultured, synchronized, and transfected with sgRNAs using lipofectamine. Successful transfections were confirmed by fluorescence microscopy. Clonal expansion followed, with DNA extracted and genotyped using PCR and Sanger sequencing. Bisulfite conversion analyzed DNA methylation, employing restriction enzymes for CpG site analysis. Statistical significance (p≤0.05) was assessed using SPSS software. Results: The neighboring regions exhibited significant genomic changes. The designed sgRNAs were cloned into the PX458 plasmid, directing Cas9 to create double-strand breaks (DSBs) in KHDC3L exon 3. Transfected cells showed around 65% efficiency. Gap-PCR confirmed knock-out in 3 out of 17 clones. COBRA analysis revealed allele-specific CpG island methylation in PEG3, indicating the impact of KHDC3L knock-out on PEG3 promoter methylation and expression. Conclusion: The study demonstrates increased PEG3 promoter methylation upon KHDC3L deletion, indicating its role in modulation. Knockout correlates with reduced cell proliferation and colony formation, suggesting KHDC3L's role in promoting cell growth. The gene's relevance in PEG3 regulation and potential therapeutic implications are underscored, though further mechanistic insights are warranted.

Airborne wind energy system (AWES) is a novel approach in wind energy harvesting. It has several advantages against conventional horizontal axis wind turbine (HAWT), like using less material and thus lower manufacturing cost, higher efficiency, stable electricity output and higher capacity for energy harvesting. It is obviously embedding a complex control system which makes the appropriate flight trajectory for the vehicle. These systems need to be carefully designed so using virtual flight simulators in design process is crucial. The main components of a typical AWES are: tether, flyer, and rotors. The flyer is designed to have a tether-constrained flight across the wind in a circular path. Consequently, the mounted rotors on the flyer’s wings will capture energy and this mechanical/electrical energy would be sent back to the ground via the same tether. It is notable that the flight path and the special design of the flyer, would make it capable to have a sustained motion in the circular loop with no energy consumption. A tethered drone equipped with several rotors is an example of such devices, already has been built and tested. In previous literature, the flight simulators usually contain some simple aerodynamic models for predicting the forces and moments generated by the rotors. It is derived by constant aerodynamic coefficients. In the current study, it has been developed a flight simulator for a typical AWES having onboard rotors. To make this flight simulator more accurate and to improve its fidelity in different environmental conditions, proper estimation of the external forces, particularly the aerodynamic forces and moments, seems to be necessary. Therefore, toward developing a high-fidelity simulator, Lagrangian dynamics and a new algorithm for estimation of the rotor aerodynamics, has been utilized. This new method is shown to have more accurate approximations of the system performance and also better description of the vehicle trajectory. By this framework, one could design optimized blades of the rotors and also the rotors arrangement. Implementing the new simulator, a single drone, as the flyer in AWES, having 3m wing span, would experience 40 percent improvement in the average power extracted which is near 2 KW.

Abstract In this research work, a magnetic nanobiocomposite is designed and presented based on the extraction of flaxseed mucilage hydrogel, silk fibroin (SF), and Fe3O4 magnetic nanoparticles (Fe3O4 MNPs). The physiochemical features of magnetic flaxseed mucilage hydrogel/SF nanobiocomposite are evaluated by FT-IR, EDX, FE-SEM, TEM, XRD, VSM, and TG technical analyses. In addition to chemical characterization, given its natural-based composition, the in-vitro cytotoxicity and hemolysis assays are studied and the results are considerable. Following the use of highest concentration of magnetic flaxseed mucilage hydrogel/SF nanobiocomposite (1.75 mg/mL) and the cell viability percentage of two different cell lines including normal HEK293T cells (95.73%, 96.19%) and breast cancer BT549 cells (87.32%, 86.9%) in 2 and 3 days, it can be inferred that this magnetic nanobiocomposite is biocompatible with HEK293T cells and can inhibit the growth of BT549 cell lines. Besides, observing less than 5% of hemolytic effect can confirm its hemocompatibility. Furthermore, the high specific absorption rate value (107.8 W/g) at 200 kHz is generated by a determined concentration of this nanobiocomposite (1 mg/mL). According to these biological assays, this magnetic responsive cytocompatible composite can be contemplated as a high-potent substrate for further biomedical applications like magnetic hyperthermia treatment and tissue engineering.

The induced phase separation method was used to fabricate polyvinyl chloride (PVC) flat sheets for membrane distillation (MD) of RO brine feed by using dimethylformamide (DMF) and water as solvent and nonsolvent, respectively. Polyvinylpyrrolidone (PVP) and zinc oxide (ZnO) nanoparticles were utilized to improve the membrane structure and modify pore surfaces. The Taguchi experimental design approach was employed to investigate the impacts of concentrations of PVP and ZnO nanoparticles on the membrane's structural characteristics and performance. SEM, XRD, and FT-IR were used to characterize the surface and cross-sectional morphology, as well as the presence of crystalline phases and cross-linked organic groups, respectively. The water contact angle was measured to determine the wettability of the surface membrane and the impact of ZnO nanoparticles on its hydrophobicity. The membrane synthesis and MD process parameters were optimized for a Persian Gulf feed brine to obtain a maximum contact angle of 148°, under 80 °C and 12 L.min-1 circulating feed water, and resulted in high salt rejection (96.4%) and proper permeability water flux (4.2 L.m-2h-1).

Abstract With the SARS-CoV-2 pandemic, the impact of recent coronavirus, especially in children, cannot be ignored. In this study, we evaluated the SARS-CoV-2 infection rates and associated features in children less than 18 years of age in “Fars” and “Kohgiluyeh and Boyer Ahmad”, provinces, Iran. 5943 children who were suspected cases to SARS-CoV-2 infection were enrolled in this study. Demographic and clinical data of SARS-CoV-2 patients were collected from 16 February 2020 to 20 June 2021. Underlying conditions were considered in this study as well. Among 5943 patients suspected COVID 19 cases, 13.51% were confirmed by real-time PCR assay. The female/male ratio was 1:1.3 with a mean age of 5.71 years. 11.2% of confirmed patients were transferred and admitted in Pediatric ICU. COVID 19 was significantly higher in children with malignancy and diabetes rather than those with other underlying diseases. Children of all ages were susceptible to COVID 19, and there is no significant difference between both sexes. Most of the COVID 19 cases were in 10–18 years old group. Among a number of children with different underlying diseases, children with malignancy had the highest rate of SARS-CoV-2 infection, followed by those with diabetes.

Multiple global crises are adding new elements to the discussion on securing and shaping a resilient, renewable energy system in Germany. Biomass from residues, forest wood and energy crops are major renewable energy contributors in Germany today. The role of energy crops within that system is controversially discussed for a long time and a phase-out from cultivated biomass is again in the focus. But what kind of trade-offs would be connected with such a phase-out and what would be the optimal allocation priorities of the remaining biomass potential? Through a detailed representation of biomass potentials, prices and conversion technologies in an energy system optimisation model, it could be shown in a scenario analysis that a phase-out increases the demand for energy imports by 1400 PJ per year, which is associated with on-costs of €14–25 billion annually in the long term. Finally, the results show that the decision on whether to grow energy crops in the future is directly influencing the future transformation strategy for high-temperature industrial heat applications. Solid biomass is identified as the future cost-optimal solution to fully transform this sector. However, a phase-out of energy crops changes the cost-optimal allocation priorities completely.
