New experimental results on magnetic compression and transport of low-temperature arc plasma are presented. In the experiments, the transport efficiency of the plasma stream in a multiple-mirror configuration was compared with the same in a uniform solenoidal field. The magnetic compression ratio of the plasma stream varied from 5 to 60 at the initial diameter of 5 cm. As the result, the plasma with (1 – 4)×1020 m-3 density at the axis was obtained at ∼3 m distance from the plasma source. Theory predicted that a multiple-mirror magnetic field should not significantly decelerate and weaken collisional plasma. The experiment confirmed this prediction. In the presented work, we simulated the baseline scenario of GOL-NB filling by cold start plasma.

3. Computed tomography-based mathematical modeling of ischemic stroke outcomes based on the focus characteristics

Objective: to identify the prognostic aspects of the ischemic stroke (IS) focus characteristics according to the data of computed tomography of the brain.Patients and methods. We examined 80 patients with hemispheric IS up to 1-day old (50 patients for constructing mathematical models of disease outcomes and 30 patients for subsequent testing of the obtained models) aged 30-84 years.Results and discussion. The analysis of the association between mortality probability and brain midline shift size shown that a brain midline shift of 4.5-5 mm did not increase mortality probability much, which indicates the synergistic stability of this system. System destabilization began after an increase of the brain midline shift for more than 5-5.5 mm. After a mild change in the initial indicator (6-8 mm), mortality probability increased from 25% to 90% and higher. When the brain midline shift was more than 8.5 mm, the system, from the synergistic viewpoint, became stable again but with an unfavorable prognosis. This analysis helps to identify the critical decision-making point when analyzing the IS focus neuroimaging characteristics. Thus, the point for the focus volume is 145 cm3, and for the brain midline shift - 5.0 mm.Conclusion. The results of our study about the prognostic value of the IS focus characteristics according to CT data may have additional value for decision-making in the management of patients with a poor prognosis.

4. The efficacy and safety of ethyl methyl hydroxypyridine succinate used as part of sequential therapy in patients with chronic cerebral ischemia

Objective: to investigate the efficacy and safety of Mexidol® FORTE 250 in patients with chronic cerebral ischemia (CCI) in the presence of hypertension and atherosclerosis.Patients and methods. The investigation enrolled 20 patients aged 45 to 75 years with CCI in the presence of hypertension and atherosclerosis, who received intravenous Mexidol® administered dropwise at a dose of 500 mg once a day for 14 days, followed by oral Mexidol® FORTE 250 mg thrice a day for 60 days (a study group). A control group consisted of 14 patients with CCI in the presence of hypertension concurrent with atherosclerosis, who were prescribed combination therapy for CCI without using these drugs. The patients were examined before and at 14 and 60 days of treatment. The investigators studied subjective complaints, neurological symptoms, and the indicators of the Tinetti Performance-Oriented Mobility Assessment in Elderly Patients; the Montreal Cognitive Assessment (MoCA); the Hamilton Anxiety Rating Scale (HARS); asthenia rating scales (Multidimensional Fatigue Inventory, MFI-20); and the Clinical Global Impression (CGI) scale over time.Results and discussion. Therapy with Mexidol® in patients with CCI in the presence of hypertension and atherosclerosis was found to be accompanied by positive changes in the asthenia rating scale MFI-20, cognitive functions assessed by MoCA, as well as Tinetti movement coordination. No significant differences in these indicators were noted in patients of the control group. Combination treatment for CCI with Mexidol®and Mexidol® FORTE 250 as a sequential therapy was twice more effective than that without using these drugs, as shown by the scales as a whole and it was up to 10 times greater for individual scale parameters. Conclusion. The study of Mexidol® FORTE 250 as part of the sequential therapy, which was used according to the above regimen, indicates its clinical efficacy and safety in patients with CCI.

АКТУАЛЬНОСТЬ: Развитие органных дисфункций и полиорганной недостаточности (ПОН) у пациентов с тяжелым ишемическим инсультом (ИИ) является фактором ухудшения клинического течения и исхода. ЦЕЛЬ ИССЛЕДОВАНИЯ: Изучить прогностические аспекты клинического течения и исходов тяжелого ИИ с развитием ПОН. МАТЕРИАЛЫ И МЕТОДЫ: В исследование включено 110 пациентов с тяжелым ИИ (National Institutes of Health Stroke Scale ≥ 16 баллов). С целью оценки тяжести ПОН на 1-е и 5-е сутки госпитализации применяли стандартные шкалы SAPS II, SOFA. Для определения предикторной значимости показателей выполнен сравнительный регрессионный анализ с применением метода наименьших квадратов. РЕЗУЛЬТАТЫ: У пациентов с летальным исходом при сравнении с выжившими наблюдали более высокую тяжесть ПОН по шкале SAPS II как на 1-е (31 (25–39) и 24 (21–29) соответственно, p

Electronic Integrated Disease Surveillance System (EIDSS) has been used to strengthen and support monitoring and prevention of dangerous diseases within One Health concept by integrating veterinary and human surveillance, passive and active approaches, case-based records including disease-specific clinical data based on standardised case definitions and aggregated data, laboratory data including sample tracking linked to each case and event with test results and epidemiological investigations. Information was collected and shared in secure way by different means: through the distributed nodes which are continuously synchronised amongst each other, through the web service, through the handheld devices. Electronic Integrated Disease Surveillance System provided near real time information flow that has been then disseminated to the appropriate organisations in a timely manner. It has been used for comprehensive analysis and visualisation capabilities including real time mapping of case events as these unfold enhancing decision making. Electronic Integrated Disease Surveillance System facilitated countries to comply with the IHR 2005 requirements through a data transfer module reporting diseases electronically to the World Health Organisation (WHO) data center as well as establish authorised data exchange with other electronic system using Open Architecture approach. Pathogen Asset Control System (PACS) has been used for accounting, management and control of biological agent stocks. Information on samples and strains of any kind throughout their entire lifecycle has been tracked in a comprehensive and flexible solution PACS. Both systems have been used in a combination and individually. Electronic Integrated Disease Surveillance System and PACS are currently deployed in the Republics of Kazakhstan, Georgia and Azerbaijan as a part of the Cooperative Biological Engagement Program (CBEP) sponsored by the US Defense Threat Reduction Agency (DTRA).
