The prone position is often used for patients with adult respiratory distress syndrome and specific surgical postures. When performing venous cannulation in this position, it is important to have a structured review to introduce the available major veins and ultrasound-guided procedure. In this review, we discuss the techniques of ultrasound-guided cannulation and provide insights into various aspects, including the anatomical locations of veins, vein sizes, placement techniques, surrounding structures at risk, and reported experiences with catheter placements. Eight major veins can be accessed in the prone position: the internal jugular vein, external jugular vein, brachiocephalic vein, basilic vein, mid-thigh femoral vein, popliteal vein, posterior tibial vein, and small saphenous vein. To minimize the risk of venous thromboembolism, the ratio of catheter diameter to vessel diameter should be less than 0.67. The review also presents the minimal requirement of venous diameter for different catheters in a tabulated form. For larger veins, real-time ultrasound guidance with the long-axis view/in-plane technique is suggested, while for smaller vessels, the short-axis view/out-of-plane technique is recommended. The review includes sonographic illustrations of the two techniques and surrounding arteries and nerves for the eight major veins. Background: Paediatric cancer and critically ill patients frequently require central venous catheters for prolonged intravenous therapy. The aim of this study is to compare the difficulty of catheter insertion and the morbidity related to this procedure in these two populations and to investigate risk factors for complications Methods: This prospective observational study was conducted at the Hedi Chaker University Hospital in Sfax, Tunisia, from July 2021 to July 2022. We included all patients aged three months to 14 years who required an infraclavicular subclavian vein catheterization. Patients were divided into two groups: Group 1 included children with malignancies; and Group 2 included critically ill paediatric patients. Then, we compared the demographic data, the difficulty of the catheterization procedure, and catheter-related complications. We also investigated risk factors for complications using a logistic regression model. The significance level was P<0.05 Results: We included 65 infants and children requiring central venous access, 28 of whom suffered from malignancies. The demographic parameters were comparable. However, the time for the procedure and the number of attempts were higher in the malignancy group with P<0.001. Central venous catheter complications were present 46.4% of the time in Group 1 compared to 21.6% in Group 2 (P=0.032). Objective: Intravenous therapy is one of the most commonly used routes for drug administration. The aim of this study was to determine the frequency of peripheral intravenous infiltration and risk factors in children. Methods: This cross-sectional and cohort study was conducted in a university hospital in pediatric patients. An information form and the Infiltration Rating Scale were used. Data were obtained from patients, their parents, and patient records. Infiltration data was obtained by researchers through direct observation. Significance was assumed as p<0.05 at 95% confidence interval. Results: A total of 176 pediatric patients made up the study cohort. Most of the peripheral venous catheters were inserted by nurses and into upper extremities. The incidence of infiltration was 57.9%, and most of the infiltrates were first-degree. The development of infiltration was significantly associated with the medical specialty being attended (p=0.001) and the number of catheter insertion attempts (p=0.013). Conclusion: The specific medical specialty and the number of catheter insertion attempts were shown to play a significant role in the development of infiltration. [ABSTRACT FROM AUTHOR]

The use of catheters and bladder catheters in hospitals can increase the risk of bacterial infections. This study aimed to identify the bacterial strains involved in catheter-related infections (CRI) in southern Benin hospitals. The study included 407 samples, including 95 catheter tip samples and 312 urine samples collected from bladder catheters from patients on the first day and 48 h after admission. The catheter tip samples were analyzed using traditional bacterial isolation and identification methods, while the urine samples were analyzed using VITEK-2. Antibiotic sensitivity was tested using the Kirby Bauer method, and virulence and resistance genes were detected through standard PCR. The results showed a predominance of Escherichia coli (53.5%), Klebsiella pneumoniae (23.3%), and Enterobacter aerogenes (7.0%) among Gram-negative bacilli, and coagulase-negative Staphylococcus as the most identified cocci. Bacterial susceptibility to antibiotics showed variable levels of resistance, with blaTEM being detected in 42.9% of identified bacterial species, followed by blaSHV (26.2%) and blaCTX-M-15 (16.7%). The blaNDM gene was only found in three identified bacterial strains, while vanA and vanB genes were detected in 3.2% of strains with a prevalence of 55% for the mecA gene. A prevalence of 18.8% for fimH was noted for the virulence genes. In conclusion, this study highlights the importance of following proper hygiene and aseptic practices during catheterization to effectively prevent CRIs. Variations in the veins of upper extremity are common. Knowing these variations is important for interventions to be carried out in this region. In this case report, a rare anastomosis between basilic vein and brachial vein is presented. During a routine cadaver dissection, a short and thick branch of the basilic vein was noticed to provide anastomosis between the basilic vein and the brachial vein in the right upper extremity of a 54-year-old male cadaver. The diameter of the basilic vein was quite narrow at the proximal part above the anastomosis. In addition, the axillary vein was noticed to be formed as a continuation of brachial vein, not basilic vein. The variation presented in this case report may cause serious complications, such as the rupture of the vessel in venous interventions related with the basilic vein. [ABSTRACT FROM AUTHOR]

OBJECTIVE: One of the leading venous access methods in chemotherapy is the use of a venous port catheter (VPC). An open surgical or ultrasound-guided technique can be performed. In our study, the VPC placement via both of these techniques was compared. METHODS: A total of 180 consecutive patients who underwent the VPC placement procedure either via the open or ultrasound- guided methods in two centers between January 2014 and January 2016 were included in the study. Patients' data were reviewed retrospectively. Groups were compared in terms of intervention-related complication rates, a total procedure time, and the requirement of control imaging with ionizing radiation. RESULTS: The mean total procedure time was significantly shorter (19.5±4.6 min, 46.7±19.6 min, p<0.001) in the ultrasound- guided group than the open method. The rate of catheter malposition was significantly less in the ultrasound-guided group than in the open group (p<0.001). The need for per-operative imaging with ionizing radiation and the need of reversion in the preferred technique were not observed in the ultrasound-guided group, whereas in the open group, they were observed in 90 (100%) and 6 (6.7%) patients, respectively (p<0.001, p=0.01). CONCLUSION: Intraoperative ultrasound guidance for the VPC placement shortens the processing time and eliminates the need for routine imaging methods that require the use of ionizing radiation. In accordance with the current guidelines recommendations, intraoperative ultrasonography should be preferred as much as possible during the VPC placement. However, the need for the surgical teams in centers to maintain the necessary educational processes for both techniques should not be overlooked. [ABSTRACT FROM AUTHOR]

Objective: Intimal hyperplasia is a well-known consequence of arterial injury and arterialization in vein grafts. However, the subacute and chronic vein wall changes which occur after catheterization have not been well studied. In this animal study, intimal hyperplasia in the vein wall after catheterization was examined. Methods: A silicon catheter was placed in the anterior caval vein of 54 rats. After in situ fixation at scheduled intervals (1 day to 6 months), the pathologic changes in the vein wall were studied on semi-serial histology sections by means of light microscopy. Results: Three forms of intimal hyperplasia could be observed: plaque-like, papillary-like and incorporation of the mural part of the sleeve into the underlying vein wall. Although the appearance of each was different, their composition was identical. All were mainly composed of α-actin-positive cells and collagen localized above the internal elastin layer, and covered by endothelium if facing the lumen. The plaque-like and papillary-like forms were mainly localized in the anterior vena cava, while sleeve incorporation mainly occurred in the jugular vein. Plaque-like and papillary-like intimal hyperplasia could be seen together on the same slide, but these two forms were never seen together with sleeve incorporation. Conclusion: Intimal hyperplasia occurs after venous catheterization and is probably caused by chronic injury to the vein wall due to knocking and rubbing movements of the catheter against the wall.Copyright © 2000 S. All available reports indicate that the echoproducing quality of agents used for contrast echocardiography is lost during their passage through the pulmonary or systemic capillary bed. A technique has been developed to transmit echoes to the left heart across the pulmonary vascular bed with the use of venous catheterization. In 43 patients with acquired or congenital heart disease, a balloon-tipped catheter was wedged in a pulmonary arterial branch, the balloon was inflated, then rapidly deflated after injection of an echoproducing substance, while M-Mode or two-dimensional echocardiograms were recorded. Echoes consistently appeared in the left heart following injection of indocyanine green (5–10 mg), saline (5 ml) or CO (0.5–1 ml). In 25 patients with a documented left-to-right shunt, echoes also appeared in the right heart chamber receiving the shunt. There were no false negatives nor false positives. No complications or side effects were observed. The presumed mechanism of transmission is the achievement of a high concentration of echoes and/or bypassing of pulmonary capillaries through physiological intrapidmonary shunts.
