The sport of boxing has existed since ancient times, with fights sometimes ending in death. In the modern world, with our greater medical knowledge, the risks of brain and other life-changing injuries have called into question whether boxing has a place in contemporary society. In fact, professional boxing has been banned in some European countries. There are also questions about whether it is appropriate to encourage spectators to watch two individuals attempting to punch each other into oblivion and about the effect this can have on those watching. The fact that, in recent years, we have seen the development of women's boxing both in the Olympics and at the professional level raises further questions. The particular emphasis of this paper is to note the number of committed Christians involved in boxing from grassroots to the elite level and to consider whether involvement in such a violent sport can be reconciled with the teaching of Jesus Christ. The main source of data is published biographies and autobiographies of Christian boxers and some interviews. There is very little published material at an academic level on the topic that we are considering. The conclusion is that despite the sincerity of the Christian boxers involved in the sport, it is difficult to justify boxing as a sport in relation to Christian teaching. [ABSTRACT FROM AUTHOR]

Knowledge exchange is a broad and consequential undertaking, analysed by diverse scholars, and rapidly growing as a field of academic practice. Its remit is to strengthen ties between research generators and users to support better material outcomes for society. This review paper considers how this increasingly codified academic field might engage with the research-practice concerns identified in the Indigenous and decolonial literature. We do so by bringing the two literature sets together for analysis, noting they are not mutually exclusive. We reveal how addressing discrimination towards Indigenous peoples from within the knowledge exchange field requires a fundamental reconsideration of the biases that run through the field's structures and processes. We prioritise two connected framing assumptions for shifting—jurisdictional and epistemological. The first shift requires a repositioning of Indigenous peoples as political–legal entities with societies, territories, laws and customs. The second shift requires engagement with Indigenous expert knowledge seriously on its own terms, including through greater understanding about expert knowledge creation with nature. These shifts require taking reflexivity much further than grasped possible or appropriate by most of the knowledge exchange literature. To assist, we offer heuristic devices, including illustrative examples, summary figures, and different questions from which to start the practice of knowledge exchange. Our focus is environmental research practice in western Anglophone settler-colonial and imperial contexts, with which we are most familiar, and where there is substantial knowledge exchange literature. Background: Myoepithelioma is a rare benign neoplasm, most commonly derived from salivary glands, but there are limited cases of extra salivary gland involvement too. There is little knowledge on typical investigative findings and, instead, diagnosis relies on immunohistochemistry analysis. To our knowledge, this paper reports the 13th case of sinonasal myoepithelioma in the English literature. Case report: This paper presents a 25-year-old man who complained of chronic nasal obstruction. A sinonasal mass was noted on examination that appeared benign on imaging. Biopsy revealed a grade 2 chondrosarcoma that was endoscopically resected; however, excisional margins were positive. On histopathological review at the multidisciplinary team meeting, the lesion was more in keeping with chondromyxoid fibroma, but immunohistochemistry analysis confirmed a myoepithelioma lesion. In light of this revised diagnosis, quorate opinion was for follow up with active monitoring. Conclusion: Sinonasal tumours require a thorough history, examination and investigation before a treatment plan can be formulated. If there is diagnostic uncertainty, it is important to keep a wide differential list and seek a second specialist opinion where possible. [ABSTRACT FROM AUTHOR]

We report a patient with congenital hypothyroidism due to athyreosis complicated by a heterozygous thyroid hormone receptor beta (THRβ) gene mutation (R320L), resulting in a severe resistance to thyroid hormone beta phenotype. The proband inherited the mutant allele from his father, presenting a very mild phenotype. While the precise reason for this discrepancy remains unknown, we postulate the possibility of de novo mutation and mosaicism in the father. Correlating thyrotropin (TSH) with free thyroxine (fT4) allowed us to predict the amount of fT4 required to normalize the proband's TSH, which supported the treatment with high dose of levothyroxine. New Zealand's iconic, flightless and endangered species of kiwi (Apterygidae) are at risk of extinction on the mainland due to predation by introduced mammals. In order to provide effective conservation management a robust understanding of genetic variation in the group is needed. Recent genomic analyses of kiwi suggest that several cryptic and as yet undescribed lineages occur in the South Island, most notably within a taxon that has until now been called Apteryx australis australis, the South Island Brown Kiwi or Tokoeka. In order for these lineages to be formally described and treated as separate conservation units it is first necessary to determine from which lineage of Tokoeka the holotype was collected. To determine this, we generated a near complete mitochondrial genome and nuclear SNP dataset for the holotype and compared these with living kiwi populations. Our results definitively assign the holotype to the Stewart Island population of brown kiwi which until now was recognized as a distinct subspecies Apteryx australis lawyri (Rothschild, 1893). This leads us to synonymize Apteryx lawryi Rothschild, 1893 within Apteryx australis Shaw, 1813. As the holotype was collected by sealers in the early 19th century, the result also provides a novel insight into the activities of early sealers in New Zealand. New names for taxa will need to be erected for any mainland South Island taxa described, because there are no scientific names available for taxonomy. [ABSTRACT FROM AUTHOR]

Background: Advances in immunohistochemistry have spearheaded major developments in our understanding and classification of sinonasal tumours. In the last decade, several new distinct histopathological entities of sinonasal cancer have been characterised. Objectives: This review aims to provide a clinical update of the major emerging subtypes for the ENT surgeon and an overview of the management strategies available for this heterogeneous group of pathologies. Evaluating impact of animal agriculture on air quality has been the focus of recent research. Ammonia (NH3) volatilization occurs when undigested protein in feces and urea in urine is broken down by bacteria and enzymes. Information regarding NH3 emission from equine facilities is limited, and effects of CP intake on NH3 emissions have not been investigated. Nine mature geldings were used in a 3 × 3 replicated Latin square design study to determine effects of dietary CP on potential NH3 losses from feces and urine. We hypothesized feeding horses above the CP requirement would result in an increase in NH3 emissions from urine and feces and different bedding materials would affect NH3 emissions from urine. Diets were formulated using different ratios of bahiagrass (Paspalum notatum) and Tifton-85 bermudagrass (Cynodon dactylon) hays, and a commercial vitamin mineral supplement to provide 3 different CP concentrations and labeled in relation to each other: LOW-CP, MED-CP, and HIGH-CP (10.6%, 11.5%, and 12%, respectively). Each study period consisted of an 11-d diet adaptation phase, followed by a 3-d total collection of urine and feces. To determine total nitrogen (TN) and urea-N concentrations, samples were pooled by period (n = 9). For in vitro determination of NH3 concentrations, urine and fecal samples were pooled within period by diet (n = 3) and mixed with either wheat straw or wood shavings. Ammonia emission of these samples was measured using a vessel system with an airflow rate (2.5 L/min) at 20°C over a 7-d period. Concentration of NH3 in each vessel was measured using a photoacoustic multigas analyzer. Temperature, airflow rate, and NH3 concentration in each vessel were used to calculate NH3 emission rate (ER). Data were analyzed using a mixed model ANOVA with repeated measures. Urinary TN and urea-N excretion increased as CP intake increased (P < 0.0001). Vessel urinary NH3 concentrations were not different across diets (P = 0.1225), ranging from 55.48 ppm (LOW-CP) to 101.14 ppm (HIGH-CP); however, they differed between bedding types (P < 0.0001), with straw higher than shavings (97 vs. 73.5 ppm, respectively). Cumulative urinary NH3 ER tended to be different across diets (P = 0.0550) ranging from 5.87 g/m2 to 9.97 g/m2 and bedding types (P = 0.0129), with straw being higher than shavings (11.1 vs. 6.9 g/m2, respectively). The MW 7.1 Darfield (Canterbury) earthquake, 4 September 2010, generated widespread hydrological effects in New Zealand ranging from instantaneous changes of piezometric levels, to more sustained postseismic changes in spring flow, river discharge and groundwater levels, and increased turbidity and declined yields of water abstracted from wells. Four years later, piezometric levels remained elevated in deeper (>40 m) aquifers along the north-western (upper) side of the Canterbury Plains near the Greendale Fault, with changes in mean piezometric level reaching +13 m. Linear reservoir modeling (eigen modeling) suggests that sustained high groundwater was not the result of changes in abstraction or land surface recharge. Step-drawdown tests at six wells within 15 km of Greendale Fault were carried out prior to the earthquake and were retested following fault rupture. Eden-Hazel analysis of discharge/drawdown relationships discriminates potential sources of head losses, and how these changed (or otherwise) as a result of the earthquake. Objective application of Eden-Hazel analysis provided confidence levels for the interpretation, including when step tests provide reliable/unreliable estimates of transmissivity change. Increases in both aquifer losses and well losses were observed in four wells, reflecting both a change in sediment transmissivity and decrease in well efficiency. At two locations, the data were unable to provide results that can be interpreted with confidence. As the majority of local groundwater flow occurs through high-permeability open framework gravel lenses, we suggest that reduction in the permeability of these gravels, due to fine-sediment incursion, is the cause of the reduction in transmissivity and increase in well losses. This study examined the electromyographic (EMG) responses from the vastus medialis (VM) for electrodes placed over and away from the innervation zone (IZ) during a maximal voluntary isometric contraction (MVIC) and sustained, submaximal isometric muscle action. A linear electrode array was placed on the VM to identify the IZ and muscle fiber pennation angle during an MVIC and sustained isometric muscle action at 50% MVIC. EMG amplitude and frequency parameters were determined from 7 bipolar channels of the electrode array, including over the IZ, as well as 10 mm, 20 mm and 30 mm proximal and distal to the IZ. There were no differences between the channels for the patterns of responses for EMG amplitude or mean power frequency during the sustained, submaximal isometric muscle action; however, there were differences between channels during the MVIC. The results of the present study supported the need to standardize the placement of electrodes on the VM for the assessment of EMG amplitude and mean power frequency. Based on the current findings, it is recommended that electrode placements be distal to the IZ and aligned with the muscle fiber pennation angle during MVICs, as well as sustained, submaximal isometric muscle actions. RNA-binding proteins (RBPs) bind to and post-transcriptionally regulate the stability of mRNAs. La-related protein 1 (LARP1) is a conserved RBP that interacts with poly-A-binding protein and is known to regulate 5′-terminal oligopyrimidine tract (TOP) mRNA translation. Here, we show that LARP1 is complexed to 3000 mRNAs enriched for cancer pathways. A prominent member of the LARP1 interactome is mTOR whose mRNA transcript is stabilized by LARP1. At a functional level, we show that LARP1 promotes cell migration, invasion, anchorage-independent growth and in vivo tumorigenesis. Furthermore, we show that LARP1 expression is elevated in epithelial cancers such as cervical and non-small cell lung cancers, where its expression correlates with disease progression and adverse prognosis, respectively. We therefore conclude that, through the post-transcriptional regulation of genes such as mTOR within cancer pathways, LARP1 contributes to cancer progression. [ABSTRACT FROM AUTHOR]

The GOSLON/5-year-old scoring systems have been used in various national and international studies. This study aims to identify the range of Modified Huddart and Bodenham (MHB) scores, which correlate with each GOSLON and 5-year-old category and thereby create a new scoring system that allows comparison with historical data. Two hundred and eighty-three unilateral cleft lip and palate study models from England and Scotland that had all been previously scored using the 5-year-old and GOSLON indices by calibrated examiners were scored using MHB on two separate occasions a month apart by two examiners. Reliability analysis using intraclass correlation and Bland Altman plots were performed. Ordinal regression was used to define the categories of MHB that correspond to the 5-year-old and GOSLON categories. The results revealed a high level of repeatability for both the 5 and the 10 year old models. The MHB scale was grouped into the five categories of the GOSLON and 5-year-old indices with more precision for the 5 year than the 10 year scores. In conclusion, there was high intra-examiner and inter-examiner agreement using the MHB scoring system. It also correlates well with the 5-year-old and GOSLON indices and has been shown to be a much more sensitive scoring system. [ABSTRACT FROM PUBLISHER]

Aims/hypothesis: Although skeletal muscle insulin resistance has been associated with activation of c-Jun N-terminal kinase (JNK), whether increased JNK activity causes insulin resistance in this organ is not clear. In this study we examined the metabolic consequences of isolated JNK phosphorylation in muscle tissue. Methods: Plasmids containing genes encoding a wild-type JNK1 (WT- JNK) or a JNK1/JNKK2 fusion protein (rendering JNK constitutively active; CA- Jnk) were electroporated into one tibialis anterior (TA) muscle of C57Bl/6 mice, with the contralateral TA injected with an empty vector (CON) to serve as a within-animal control. Results: Overproduction of WT-JNK resulted in a modest (∼25%) increase in phosphorylation (Thr/Tyr) of JNK, but no differences were observed in Ser phosphorylation of insulin receptor substrate 1 (IRS-1) or total IRS-1 protein, nor in insulin-stimulated glucose clearance into the TA muscle when comparing WT- JNK with CON. By contrast, overexpression of CA- Jnk, which markedly increased the phosphorylation of CA-JNK, also increased serine phosphorylation of IRS-1, markedly decreased total IRS-1 protein, and decreased insulin-stimulated phosphorylation of the insulin receptor (Tyr) and phosphorylation of Akt at (Ser and Thr) compared with CON. Moreover, overexpression of CA- Jnk decreased insulin-stimulated glucose clearance into the TA muscle compared with CON and these effects were observed without changes in intramuscular lipid species. Conclusions/interpretation: Constitutive activation of JNK in skeletal muscle impairs insulin signalling at the level of IRS-1 and Akt, a process which results in the disruption of normal glucose clearance into the muscle. The purpose of this study was to investigate fiber type-related differences in the patterns of responses for mechanomyographic-electromyographic (MMG-EMG) cross spectrum mean power frequency (MPF) in resistance-trained and aerobically-trained subjects during a fatiguing muscle action. Five resistance-trained and five aerobically-trained men performed a 45-s isometric muscle action of the dominant leg extensors at 50% MVC while MMG and EMG signals were recorded simultaneously from the vastus lateralis muscle. In addition, a biopsy was taken to determine the myosin heavy chain (MHC) isoform content of the vastus lateralis. The resistance-trained and aerobically-trained subjects demonstrated similar patterns of responses for MMG-EMG cross spectrum MPF during the sustained muscle action. The vastus lateralis of the resistance-trained subjects demonstrated primarily Type II MHC isoform expression, indicative of fast-twitch muscle fibers, while that of the aerobically-trained subjects was composed mostly of Type I MHC isoform expression, indicative of slow-twitch fibers. Thus, the differences in fiber type characteristics were not manifested in the patterns of responses for MMG-EMG cross spectrum MPF. [ABSTRACT FROM AUTHOR]

Background: Computed tomography (CT) fluoroscopy nowadays is the most preferred method of guidance to perform percutaneous lung biopsy of pulmonary masses. Conventional fluoroscopy is an increasingly forgotten technique that still can be used to perform lung biopsies, with many advantages. Purpose: To compare the accuracy, safety, and effective dose (ED) of conventional fluoroscopy-guided needle lung biopsy (FNLB) with CT-guided needle lung biopsy procedures (CTNLB) reported in the literature. Material and Methods: 100 consecutive patients who underwent FNLB were reviewed retrospectively. Using the final histological diagnoses and the clinical and radiological course of the disease as references, the accuracy and sensitivity of FNLB were calculated. The complication rates of FNLB were assessed. Using computer software (XDOSE), the ED was calculated. The accuracy, complication rates, and the ED of FNLB were compared with CTNLB reported in the literature. Results: The overall accuracy rate and sensitivity of FNLB were both 87%, which are comparable to the range of accuracies reported in the literature for CTNLB (74-97%). The complication rates of FNLB were also comparable to the complication rates reported for CTNLB. The commonest complication was pneumothorax, at a rate of 25%. The ED of FNLB was small, significantly lower than reported in the literature for CT-guided procedures. The mean ED of FNLB was 0.029 mSv, which is approximately equivalent to one chest X-ray. Conclusion: Conventional fluoroscopy is an accurate, safe, and low-dose alternative modality to CT to obtain an image-guided histological diagnosis of pulmonary lesions. [ABSTRACT FROM AUTHOR]

Molecular clocks are widely used to date phylogenetic events, yet evidence supporting the rate constancy of molecular clocks through time and across taxonomic lineages is weak. Here, we present 90 candidate avian clock calibrations obtained from fossils and biogeographical events. Cross-validation techniques were used to identify and discard 16 inconsistent calibration points. Molecular evolution occurred in an approximately clock-like manner through time for the remaining 74 calibrations of the mitochondrial gene, cytochrome b. A molecular rate of approximately 2.1% (± 0.1%, 95% confidence interval) was maintained over a 12-million-year interval and across most of 12 taxonomic orders. Minor but significant variance in rates occurred across lineages but was not explained by differences in generation time, body size or latitudinal distribution as previously suggested. The central governor model has recently been proposed as a general model to explain the phenomenon of fatigue. It proposes that the subconscious brain regulates power output (pacing strategy) by modulating motor unit recruitment to preserve whole body homoeostasis and prevent catastrophic physiological failure such as rigor. In this model, the word fatigue is redefined from a term that describes an exercise decline in the ability to produce force and power to one of sensation or emotion. The underpinnings of the central governor model are the refutation of what is described variously as peripheral fatigue, limitations models, and the cardiovascular/anaerobic/catastrophe model. This argument centres on the inability of lactic acid models of fatigue to adequately explain fatigue. In this review, it is argued that a variety of peripheral factors other than lactic acid are known to compromise muscle force and power and that these effects may protect against ‘catastrophe’. Further, it is shown that a variety of studies indicate that fatigue induced decreases in performance cannot be adequately explained by the central governor model. Instead, it is suggested that the concept of task dependency, in which the mechanisms of fatigue vary depending on the specific exercise stressor, is a more comprehensive and defensible model of fatigue. This model includes aspects of both central and peripheral contributions to fatigue, and the relative importance of each probably varies with the type of exercise. [ABSTRACT FROM AUTHOR]

BACKGROUND: The presence of lichenoid or granulomatous inflammation in an oral mucosal biopsy usually suggests a distinct range of diagnostic possibilities. However, the presence of both patterns of inflammation in the same biopsy is uncommon. METHODS: A clinico-pathological study of six patients. RESULTS: All the patients in this study presented with similar mucosal lesions of the upper lip. Microscopically the lesions were characterized by the presence of lichenoid inflammation with concomitant granulomatous inflammation. The lesions were persistent and refractory to treatment with steroid medications, but remained localized and did not appear to herald the onset of systemic inflammatory or neoplastic disease. CONCLUSION: We propose the designation 'lichenoid and granulomatous stomatitis' for the cases described in this study. The purpose of this study was to examine the acute effects of static stretching on peak torque (PT), the joint angle at PT, mean power output (MP), electromyographic (EMG) amplitude, and mechanomyographic (MMG) amplitude of the vastus lateralis (VL) and rectus femoris (RF) muscles during maximal, voluntary concentric isokinetic leg extensions at 60 and 240 degrees x s(-1) of the stretched and unstretched limbs. Twenty-one volunteers [mean age (SD) 21.5 (1.3) years] performed maximal, voluntary concentric isokinetic leg extensions for the dominant and non-dominant limbs at 60 and 240 degrees x s(-1). Surface EMG (muVrms) and MMG (mVrms) signals were recorded from the VL and RF muscles during the isokinetic tests. PT (Nm), the joint angle at PT, and MP (W) were calculated by a dynamometer. Following the initial isokinetic tests, the dominant leg extensors were stretched using four static stretching exercises. After the stretching, the isokinetic tests were repeated. PT decreased (P< or =0.05) from pre- to post-stretching for the stretched limb at 60 and 240 degrees x s(-1) and for the unstretched limb at 60 degrees x s(-1). EMG amplitude of the VL and RF also decreased (P< or =0.05) from pre- to post-stretching for the stretched and unstretched limbs. There were no stretching-induced changes (P>0.05) for the joint angle at PT, MP, or MMG amplitude. These findings indicated stretching-induced decreases in force production and muscle activation. The decreases in PT and EMG amplitude for the unstretched limb suggested that the stretching-induced decreases may be due to a central nervous system inhibitory mechanism. [ABSTRACT FROM AUTHOR]

Objective:This report describes the clinical presentation and management of a rare case of herpes simplex virus infection in the larynx of a patient treated with methotrexate.Case report:We report a case of a clinically suspicious laryngeal lesion in an 82-year-old woman who started methotrexate treatment for rheumatoid arthritis. Shortly afterwards she developed dysphonia, which worsened over four months. On microlaryngoscopy, there was bilateral erythema and ulceration of the vocal folds. No other mucocutaneous lesions or systemic features were present. Biopsies revealed herpes simplex virus infection of the vocal folds; there was complete resolution with oral aciclovir. A brief literature review for this rare entity is presented and the diagnostic challenges arising from under-recognition of atypical presentations are discussed.Conclusion:To our knowledge, this is the first report of a rare complication of herpes simplex virus infection in the context of methotrexate-induced immunosuppression. Aims To study immunohistochemically a group of 28 primary lung cancers which demonstrated histological features reminiscent of those characteristic of regression in malignant melanoma, to determine the phenotype of their immune cell infiltrates. Methods and results Using a standard s-ABC immunoperoxidase method then quantification by the Leica Q500 MC image analyser, the cellular infiltrate in these tumours, when compared to 67 control cases, showed excess CD3 + T-lymphocytes (P < 0.007), with an increase in CD4:CD8 ratio and increased CD68 + macrophages (P = 0.00001). CD57 + natural killer cells and S100 + Langerhans cells were also increased, but not quite significantly (P = 0.048 and P = 0.072, respectively). Conclusions This immunophenotype resembles that shown in regressing skin malignancies and suggests a similar process occurring in lung cancer. This study has examined muscle strength and cross-sectional area in a group of 35 healthy untrained male subjects and 8 subjects who had been engaged in a strenuous weight-training programme. The maximum voluntary knee extension force which could be produced by the untrained subjects was 742 +/- 100 N (mean +/- SD). The trained subjects could produce a significantly (p less than 0.001) greater force (992 +/- 162 N). Cross-sectional area of the knee-extensor muscle group was 81.6 +/- 11.8 cm2 in the untrained subjects and 104.1 +/- 12.3 cm2 in the trained subjects (p less than 0.001). In the untrained subjects, a significant correlation existed between strength and muscle cross-sectional area (r = 0.56, p less than 0.001). In the same group of subjects, there was a significant inverse relationship between muscle cross-sectional area and the ratio of strength to cross-sectional area (r = 0.55, p less than 0.001). The mean ratio of strength to cross-sectional area was 9.20 +/- 1.29 for the untrained group whereas for the trained group this ratio was 9.53 +/- 1.01. It is suggested that the inverse relationship between strength per unit cross-sectional area and cross-sectional area results in part from an increased angle of pennation in the larger muscles.
