This study attempts to explore the social determinants of patients with diabetes and hypertension. This was a cross-sectional study conducted in an urban slum on a random sample of 251 patients. The participants were interviewed at their home, where their anthropometry was performed. The social determinants significantly associated with both diabetes and hypertension were being literate, currently married, having an active occupation, belonging to Hindu religion, being in higher socio-economic group, having financial crunch, having social conflicts at home and inability to perform daily work, except, having professional stress and regular consumption of junk food, which were significantly associated with hypertension. The risk factors significantly associated with both diabetes and hypertension were obesity by body mass index, abdominal obesity by waist circumference as well as by waist-hip ratio, sedentary lifestyle and lack of physical activity. Among the diabetics, 15.7% smoked tobacco, 19.6% consumed oral tobacco and 9.8% were alcoholic; while among the hypertensive, 13.5% smoked tobacco, 25% consumed oral tobacco and 5% were alcoholic. Social determinants of health have significant ramifications on diabetes and hypertension. While assessing the patients with hypertension and diabetes in their clinics, clinicians are advised to address the social determinants of health as well. [ABSTRACT FROM AUTHOR]

Background: Cost studies are paramount for demonstrating how resources have been spent and identifying opportunities for more efficient use of resources. The aim of this study was to determine the unit cost of maternal and child health (MCH) programme provided at Primary Health Centers (PHCs) and to examine the variation in unit cost in different PHCs. Methodology: The present study was carried out in three PHCs of Ahmedabad district namely Sanathal, Nandej, and Uperdal, between 1 April, 2006 and 31 March, 2007. For estimating the cost of a health program, information on all the physical and human resources that were basic inputs to the PHC services were collected and grouped into two categories, non-recurrent (capital resources vehicles, buildings, etc.) and recurrent resources (salaries, drugs, vaccines, contraceptives, maintenance, etc.). To generate the required data, two types of schedules were developed, daily time schedule and PHC/SC (Subcenter) information schedule. Results: Unit cost for each contact of MCH beneficiaries was Rs. 54.87 at Sanathal PHC, Rs. 87.63 at Nandej PHC and Rs. 70.01 at Uperdal PHC. Conclusion: Even though maternal and child health services are free, utilization of these services at the health centres were low, particularly for delivery, leading to high unit costs. Background & Objectives: India, with 1.22 billion populations, is the second most populous country in the world. India contributes 17.31% population in the world. Demographers expect India's population to surpass the population of China by 2030. Recent data of NFHS III revealed that 13% of married women have unmet need for family planning in India so this study was planned to estimate the prevalence of unmet need for Family Planning among married women of reproductive age residing at UHTC area Government Medical College Bhavnagar and to find out the determinants of unmet need for Family Planning in the study population. Methods: This was a cross sectional descriptive study conducted in UHTC catchment area of Government Medical College; Bhavnagar from August 2011 to October 2011. The calculated sample size was 140 (P=9.76, L=5%). A total 150 married women of reproductive age group (15-49 years) were interviewed for the same with their informed consent. The data were entered and analysed in software epi.info.3.5.1. Results: The prevalence of unmet need for Family Planning in UHTC catchment area was found to 18.7%. The unmet need was higher for temporary methods of Family Planning (10.0%), in the women age group of 20-29 years (71.5%), Illiterate (42.8%), Muslim women(42.8%), poor knowledge of contraception(82.1%), no advice on Family Planning (57.1%) and no male participation (75.0%). These entire variable were found to be associated with high unmet need for Family Planning (P<0.05). Interpretation & Conclusion: The present study concluded that the percentage of unmet need years, low education and also in Muslim religious women. To decrease unmet need family planning measures should be specifically directed toward this group of women. Background: Cardiovascular diseases are becoming the major cause of morbidity and mortality in most of the developing countries including India. Various behavioral risk factors are the major risk factors for coronary artery disease. Present study was carried out to study the role of various behavioral risk factors in the occurrence of coronary artery disease. Materials and Methods: Present study was a hospital-based paired-matched case-control study, carried out at civil hospital, Ahmedabad. A total of 135 newly-diagnosed cases of coronary artery disease and 135 controls were studied after taking an informed written consent. Data was analyzed by using Epi-info version 3.5.1 computer package, Chi -square test, Z test and Odds ratio was calculated. Results: Among the total 135 cases, 70.4% were male and 29.6% were female, most of the cases (40%) belong to the age group of 51-60 years. Smokers, smokeless tobacco users, and alcoholic were significantly higher among the cases as compared to controls. Significant association was also observed between current smokers, smokeless tobacco users, and coronary artery disease. Dose-response relationship was observed between the frequency of smoking per day and coronary artery disease (OR = 3.72). Significant association was also observed between obesity, physical inactivity, and coronary artery disease. Salt and oil intake per day were significantly higher among the cases as compared to controls. Conclusion: Modifiable and preventable behavioral risk factors are major etiology behind the occurrence of coronary artery disease, which highlights the need of appropriate control strategies and measures. Background: Since 1976, seven outbreaks of hepatitis E occurred in Ahmedabad. Clusters of jaundice cases were reported on June 19, 2008, by a civic center, Girdharnagar ward, Ahmedabad. Objectives: The objectives were as follows: (1) to identify the etiological agent, source of outbreak, and mode of transmission; (2) to propose a control measure based on the outbreak investigation. Materials and Methods: We defined a case as an acute illness with (a) a discrete onset of symptoms and (b) jaundice or elevated serum aminotransferase levels, from March to September 2008 in the households of the Girdharnagar ward. We collected data through a door-to-door survey and hospital records. We described the outbreak in terms of time, place, and person. We collected laboratory investigation reports of case patients admitted to the civil hospital. To test our hypothesis we conducted a retrospective cohort study to find out the relative risk for hepatitis. We conducted environment investigation to find out the source of contamination of water supply. Results: A total 233 case patients of hepatitis were identified with the attack rate of 10.9/1000 population. Cases were reported in all the age groups with a higher attack rate in the age group of 20-29 years (18.5/1000). Out of 17 case patients, 16 were positive for the hepatitis E IgM antibody. The attack rate was two times more among those who were exposed to the leaking pipeline than the non-exposed (RR=2.3, 95% CI 1.76, 2.98). Environmental investigation also confirmed the sewage contamination of drinking water in the distribution system. Conclusion: The outbreak was due to hepatitis E virus. We recommended a temporary alternative water supply, repair of the leakages, and water quality surveillance. [ABSTRACT FROM AUTHOR]

Objectives: To determine the unit cost of curative care provided at Primary Health Centers (PHCs) and to examine the variation in unit cost in different PHCs. Results: The unit cost of curative care was lowest (Rs. 29.43) for the Sanathal PHC and highest (Rs. 88.26) for the Uperdal PHC, followed by the Nandej PHC with Rs. 40.88, implying severe underutilization of curative care at the Uperdal PHC. Conclusions: Location of health facilities is a problem at many places. The article determines the incidence of in-stent thrombosis (IST) after coronary stent implantation in patients with cocaine abuse. The medical records of patients at an institution was reviewed and a number of them was identified to have suffered IST. A number of patients presented with ST segment elevation myocardial infarction and with non-ST-segment elevation myocardial infarction.
